BACKGROUND Rheumatic immune diseases are a group of chronic inflammatory diseases charac-terized by joint and systemic multi-organ involvement,including rheumatoid arthritis,systemic lupus erythematosus,and Sjogren’s...BACKGROUND Rheumatic immune diseases are a group of chronic inflammatory diseases charac-terized by joint and systemic multi-organ involvement,including rheumatoid arthritis,systemic lupus erythematosus,and Sjogren’s syndrome,among others.The pathogenesis of these diseases is related to the abnormal activation and regulatory imbalance of the immune system.The prevalence and morbidity of rheumatic immune diseases are high,imposing a significant burden on patients'quality of life and socio-economic costs.Currently,the treatment of rheumatic immune diseases mainly relies on Western medicine,such as non-steroidal anti-inflammatory drugs,glucocorticoids,disease-modifying antirheumatic drugs,and biologics.However,the therapeutic effects of Western medicine are not ideal,some patients poorly respond or are resistant to Western medicine,and long-term use often causes various adverse reactions.AIM To systematically evaluate the efficacy and safety of Tripterygium wilfordii gly-cosides tablets combined with Western medicine in the treatment of patients with rheumatic immune diseases.METHODS This study conducted a meta-analysis to systematically evaluate the efficacy and safety of Tripterygium wilfordii glycosides tablets combined with Western medicine for patients with rheumatic immune diseases.Chinese and English databases were searched for randomized controlled trials(RCTs)on the treatment of rheumatic immune diseases with Tripterygium wilfordii glycosides tablets combined with Western medicine.The quality of the included studies was assessed using the Cochrane risk of bias assessment tool.Meta-analysis was performed using RevMan 5.4 software.RESULTS The meta-analysis included 11 RCTs involving 1026 patients with rheumatic immune diseases.The combined treatment significantly reduced the risk of disease recurrence(relative risk=1.07,95%confidence interval:1.01-1.15,P<0.05)and showed no significant heterogeneity(I2=0%,P=0.53),indicating that Tripterygium wilfordii glycosides tablets combined with Western medicine is an effective method to reduce the possibility of postoperative recurrence in patients with rheumatic immune diseases.However,due to the limited number and quality of the studies included,these results should be interpreted with caution.CONCLUSION Tripterygium wilfordii glycosides tablets combined with Western medicine is an effective and safe treatment option for patients with rheumatic immune diseases and can be considered a clinical choice.However,more high-quality research is needed to validate this conclusion and provide more solid evidence for clinical practice.展开更多

A total of 45 rats were randomly ...The present study was designed to evaluate the inhibitory effects of Tripterygium wilfordii multiglycoside(GTW) against testosterone-induced benign prostatic hyperplasia(BPH) in rats. A total of 45 rats were randomly divided into five groups: Group I, vehicle control group(sham-operated and treated with vehicle); Group II, BPH group; Group III, BPH rats treated with finasteride at a dose of 5 mg·kg-1; and Groups IV and V, BPH rats treated with GTW at dose levels of 10 and 20 mg·kg-1, respectively. The drugs were administered orally once a day for 14 days. Prostate weight, prostatic index, and the testosterone and dihydrotestosterone(DHT) levels in serum and prostate, and the serum prostate specific antigen(PSA) levels were measured; prostate tissues were taken for histopathological examination; and serum biochemical analysis was also performed. The BPH rats displayed an increase in prostate weight, prostatic index with increased testosterone and DHT levels in both the serum and prostate, and increased serum PSA levels. GTW treatment at both doses resulted in significant reductions in prostate weight, prostatic index, testosterone and DHT levels in both the serum and prostate, and serum PSA levels, compared with BPH group. Histopathological examination also indicated that GTW treatment at both doses inhibited testosterone-induced prostatic hyperplasia. Serum biochemical analysis showed that the liver and renal functions were normal. In conclusion, GTW inhibited testosterone-induced prostatic hyperplasia in rats, without host toxicity, providing a basis for the development of GTW as a novel therapy for BPH.展开更多

Juvenile shoots from one-year-old elite plants were used as the source of explants. New axillary shoots were obtained after 30 days of culture on a MS medium supplemented with BAP (2.0 mg.L^-1) and NAA (0.1 mg.L^-1). The optimal multiplication medium was a modified MS medium supplemented with BAP (1.0 mg.L^-1) and NAA (0.1 mg.L^-1). This yielded a multiplication rate of 2.4 for each subculture. Slightly more than 92% of shoots rooted when cultured on a modified MS medium containing IBA (0.2 mg.L^-1) and activated charcoal (0.5 mg.L^-1). Activated charcoal promoted both a strong and a high rooting rate during the rooting phase. Plantlets were transferred to pots for a short acclimatization stage in a greenhouse where 95% of the plantlets survived. This highly reproducible procedure can be adopted for large-scale propagation of T. wilfordii.展开更多
OBJECTIVE:To investigate the effectiveness a cream onjoint pain and swelling in patients with rheumatoid arthritis(RA).The cream,topically used,in was prepared with Tripterygium wilfordii Hook F(TwHF),Mangxiao(Nalrii ...OBJECTIVE:To investigate the effectiveness a cream onjoint pain and swelling in patients with rheumatoid arthritis(RA).The cream,topically used,in was prepared with Tripterygium wilfordii Hook F(TwHF),Mangxiao(Nalrii Sulfas),Chuanxiong(Rhizoma Chuanxiong),stir-frying with liquid adjuvant Ruxiang(Olibanum),and stir-frying with liquid adjuvant Moyao(Myrrh).METHODS:Patients were 1∶1 randomized to addon TwHF cream twice a day or placebo for 4 weeks.The primary endpoint was achievement rate of20%improvement in American College of Rheuma-tology criteria(ACR20)at week 4.Secondary endpoints were ACR50,28-joint count Disease Activity Score(DAS28)improvement and safety profiles.Statistical analyses were performed using intention to treat analysis(ITT)set.RESULTS:A total of 70 active RA patients were enrolled.At week 4,the ACR20 was 34.3%(12/35)in TwHF cream group and 11.4%(4/35)in placebo group(P=0.015).Similarly,a higher ACR50 responder proportion was seen in TwHF cream group with 17.1%(6/35)comparing to it in placebo group with 2.9%(1/35)(P=0.046).The TwHF cream group also had more improvement than the placebo group on DAS28-ESR(1.1 vs 0.5,P=0.001),DAS28-CRP(1.4 vs 0.7,P=0.001),tender joint count(5.5 vs2.6,P=0.018),swollen joint count(3.5 vs 1.6,P=0.003)and Physician's global assessment(25.8 vs13.0,P=0.002),as well as C-reactive protein(11.2 vs 2.7,P=0.048).Except 2 skin allergy events in TwHF cream group,no other substantive adverse events were observed.CONCLUSION:On the short term,TwHF cream is likely to be an effective and safety complimentary treatment in patients with active RA.展开更多
Tripterygium wilfordii multiglycoside(GTW)is a commonly used compound for the treatment of rheumatoid arthritis(RA)and immune diseases in clinical practice.However,it can induce liver injury and the mechanism of hepat...Tripterygium wilfordii multiglycoside(GTW)is a commonly used compound for the treatment of rheumatoid arthritis(RA)and immune diseases in clinical practice.However,it can induce liver injury and the mechanism of hepatotoxicity is still not clear.This study was designed to investigate GTW-induced hepatotoxicity in zebrafish larvae and explore the mechanism involved.The 72 hpf(hours post fertilization)zebrafish larvae were administered with different concentrations of GTW for three days and their mortality,malformation rate,morphological changes in the liver,transaminase levels,and histopathological changes in the liver of zebrafish larvae were detected.The reverse transcription-polymerase chain reaction(RT-PCR)was used to examine the levels of microRNA-122(miR-122)and genes related to inflammation,apoptosis,cell proliferation and liver function.The results showed that GTW increased the mortality of zebrafish larvae,while significant malformations and liver damage occurred.The main manifestations were elevated levels of alanine aminotransferase(ALT)and aspartate aminotransferase(AST),significant liver atrophy,vacuoles in liver tissue,sparse cytoplasm,and unclear hepatocyte contours.RT-PCR results showed that the expression of miR-122 significantly decreased by GTW;the mRNA levels of inflammation-related genes il1β,il6,tnfα,il10,cox2 and ptges significantly increased;the mRNA level of tgfβsignificantly decreased;the mRNA levels of apoptosis-related genes,caspase-8 and caspase-9,significantly increased;the mRNA level of bcl2 significantly decreased;the mRNA levels of cell proliferation-related genes,top2αand uhrf1,significantly reduced;the mRNA levels of liver function-related genes,alr and cyp3c1,significantly increased;and the mRNA level of cyp3a65 significantly decreased.In zebrafish,GTW can cause increased inflammation,enhanced apoptosis,decreased cell proliferation,and abnormal expression of liver function-related genes,leading to abnormal liver structure and function and resulting in hepatotoxicity.展开更多

Acute kidney injury(AKI)is a common and serious health issue with a growing incidence and mortality rate.Tripterygium wilfordii(TW)is a traditional Chinese medicine that has been reported to cause kidney damage.Howeve...Acute kidney injury(AKI)is a common and serious health issue with a growing incidence and mortality rate.Tripterygium wilfordii(TW)is a traditional Chinese medicine that has been reported to cause kidney damage.However,the associated mechanism of TW-induced AKI remains unclear.Therefore,we aimed to uncover the associated mechanisms of AKI induced by TW using network pharmacology and bioinformatics.The candidate compounds of TW and the potential targets were screened using TCMSP and CTD database,and the AKI-related targets were identified from the Dis Ge NET database.The disease targets were intersected with the drug targets,and the Wayne diagram was drawn by Venny 2.1.0 software.We developed proteinprotein interactions(PPI)network and the“disease-compound-target-pathway”network through the Cytoscape software.By using the DAVID database,GO and KEGG enrichment analysis was carried out to reveal the potential signaling pathways of the compound-TW-induced AKI.Meanwhile,the Auto dock vina 1.1.2 was used for molecular docking to verify the active compound and key targets’binding ability.Critical compounds and key targets of TW-induced AKI were identified,including triptolide,kaempferol,β-sitosterol,nobiletin,stigmasterol,TNF,and so on.The GO analysis showed that potential genes’biological function was mainly involved in apoptosis,oxidative stress,and inflammation.Moreover,eight signaling pathways were found,including the HIF-1 signaling pathway,VEGF signaling pathway,apoptosis,and so on.The molecular docking results proved that the core compound’s affinity with the corresponding protein of the gene targets was good.This study preliminarily predicted the core toxic compounds,targets,and related pathways of TW-induced AKI,providing a theoretical basis for the follow-up clinical rational drug use and related research work of TW.展开更多

Objective: To investigate the curative efficacy of vitamin D3 in combination with tripterygium wilfordii polyglycoside in patients with rheumatoid arthritis (RA) and its influences on indices of bone restoration. Meth...Objective: To investigate the curative efficacy of vitamin D3 in combination with tripterygium wilfordii polyglycoside in patients with rheumatoid arthritis (RA) and its influences on indices of bone restoration. Methods: A total of 320 patients with RA were collected as observation objects to be randomly divided into the control group and the observation group with 160 cases in each group. The control group was given treatment of tripterygium wilfordii polyglycoside, while the observation group was given treatment of vitamin D3 in combination with tripterygium wilfordii polyglycoside. All patients received a course treatment with 3 months. The curative efficacy, improvement of joint symptoms, changes of laboratory tests, indices of bone restoration and adverse reactions were compared. Results: After 3 months, assessment of curative efficacy showed that the observation group had a total curative efficiency ratio of 90.6%, which was significantly higher than that of 81.2% in the control group. Among the indices which reflect the improvement of joint symptoms, the joint pain index, joint tenderness index and joint swollenness index were statistically reduced in the observation group than those in the control group. And after treatment, in comparison with the control group, indices of laboratory tests of rheumatoid factor (RF), antistreptolyisn O antibody (ASO), c-reactive protein (CRP) were statistically lower respectively in the observation group. As to indices of bone restoration after treatment, the observation group had statistically lower serum level of receptor activator of nf-kb ligand (RNKL) and higher level of osteoprotegerin (OPG) than those in the control group. During the treatment, incidences of adverse reactions in the control group and the observation group were statistically same. Conclusion: Vitamin D3 in combination with tripterygium wilfordii polyglycoside has well curative efficacy in patients with RA, which can significantly improve joint symptoms, indices of laboratory tests and bone restoration.展开更多

文摘Tripterygium wilfordii Hook F has significant anti-inflammatory and immunosuppressive properties and is widely used for treating autoimmune and inflammatory diseases such as rheumatoid arthritis, systemic lupus erythematosus, and kidney disease, especially in traditional Chinese medicine. The mechanisms underlying its effects may be diverse but they remain unclear, and its toxicity and side effects limit its wider clinical application. This review summarizes the clinical application of Tripterygium wilfordii Hook F in recent years, as well as the results of studies into its mechanisms and toxicity, to provide a reference for its future clinical application.

文摘The present study was designed to evaluate the inhibitory effects of Tripterygium wilfordii multiglycoside(GTW) against testosterone-induced benign prostatic hyperplasia(BPH) in rats. A total of 45 rats were randomly divided into five groups: Group I, vehicle control group(sham-operated and treated with vehicle); Group II, BPH group; Group III, BPH rats treated with finasteride at a dose of 5 mg·kg-1; and Groups IV and V, BPH rats treated with GTW at dose levels of 10 and 20 mg·kg-1, respectively. The drugs were administered orally once a day for 14 days. Prostate weight, prostatic index, and the testosterone and dihydrotestosterone(DHT) levels in serum and prostate, and the serum prostate specific antigen(PSA) levels were measured; prostate tissues were taken for histopathological examination; and serum biochemical analysis was also performed. The BPH rats displayed an increase in prostate weight, prostatic index with increased testosterone and DHT levels in both the serum and prostate, and increased serum PSA levels. GTW treatment at both doses resulted in significant reductions in prostate weight, prostatic index, testosterone and DHT levels in both the serum and prostate, and serum PSA levels, compared with BPH group. Histopathological examination also indicated that GTW treatment at both doses inhibited testosterone-induced prostatic hyperplasia. Serum biochemical analysis showed that the liver and renal functions were normal. In conclusion, GTW inhibited testosterone-induced prostatic hyperplasia in rats, without host toxicity, providing a basis for the development of GTW as a novel therapy for BPH.

文摘In this study a reliable protocol was developed for the establishment of commercial in vitro cultures of Tripterygium wilfordii Hook f.. Juvenile shoots from one-year-old elite plants were used as the source of explants. New axillary shoots were obtained after 30 days of culture on a MS medium supplemented with BAP (2.0 mg.L^-1) and NAA (0.1 mg.L^-1). The optimal multiplication medium was a modified MS medium supplemented with BAP (1.0 mg.L^-1) and NAA (0.1 mg.L^-1). This yielded a multiplication rate of 2.4 for each subculture. Slightly more than 92% of shoots rooted when cultured on a modified MS medium containing IBA (0.2 mg.L^-1) and activated charcoal (0.5 mg.L^-1). Activated charcoal promoted both a strong and a high rooting rate during the rooting phase. Plantlets were transferred to pots for a short acclimatization stage in a greenhouse where 95% of the plantlets survived. This highly reproducible procedure can be adopted for large-scale propagation of T. wilfordii.

文摘OBJECTIVE:To investigate the effectiveness a cream onjoint pain and swelling in patients with rheumatoid arthritis(RA).The cream,topically used,in was prepared with Tripterygium wilfordii Hook F(TwHF),Mangxiao(Nalrii Sulfas),Chuanxiong(Rhizoma Chuanxiong),stir-frying with liquid adjuvant Ruxiang(Olibanum),and stir-frying with liquid adjuvant Moyao(Myrrh).METHODS:Patients were 1∶1 randomized to addon TwHF cream twice a day or placebo for 4 weeks.The primary endpoint was achievement rate of20%improvement in American College of Rheuma-tology criteria(ACR20)at week 4.Secondary endpoints were ACR50,28-joint count Disease Activity Score(DAS28)improvement and safety profiles.Statistical analyses were performed using intention to treat analysis(ITT)set.RESULTS:A total of 70 active RA patients were enrolled.At week 4,the ACR20 was 34.3%(12/35)in TwHF cream group and 11.4%(4/35)in placebo group(P=0.015).Similarly,a higher ACR50 responder proportion was seen in TwHF cream group with 17.1%(6/35)comparing to it in placebo group with 2.9%(1/35)(P=0.046).The TwHF cream group also had more improvement than the placebo group on DAS28-ESR(1.1 vs 0.5,P=0.001),DAS28-CRP(1.4 vs 0.7,P=0.001),tender joint count(5.5 vs2.6,P=0.018),swollen joint count(3.5 vs 1.6,P=0.003)and Physician's global assessment(25.8 vs13.0,P=0.002),as well as C-reactive protein(11.2 vs 2.7,P=0.048).Except 2 skin allergy events in TwHF cream group,no other substantive adverse events were observed.CONCLUSION:On the short term,TwHF cream is likely to be an effective and safety complimentary treatment in patients with active RA.

文摘Tripterygium wilfordii multiglycoside(GTW)is a commonly used compound for the treatment of rheumatoid arthritis(RA)and immune diseases in clinical practice.However,it can induce liver injury and the mechanism of hepatotoxicity is still not clear.This study was designed to investigate GTW-induced hepatotoxicity in zebrafish larvae and explore the mechanism involved.The 72 hpf(hours post fertilization)zebrafish larvae were administered with different concentrations of GTW for three days and their mortality,malformation rate,morphological changes in the liver,transaminase levels,and histopathological changes in the liver of zebrafish larvae were detected.The reverse transcription-polymerase chain reaction(RT-PCR)was used to examine the levels of microRNA-122(miR-122)and genes related to inflammation,apoptosis,cell proliferation and liver function.The results showed that GTW increased the mortality of zebrafish larvae,while significant malformations and liver damage occurred.The main manifestations were elevated levels of alanine aminotransferase(ALT)and aspartate aminotransferase(AST),significant liver atrophy,vacuoles in liver tissue,sparse cytoplasm,and unclear hepatocyte contours.RT-PCR results showed that the expression of miR-122 significantly decreased by GTW;the mRNA levels of inflammation-related genes il1β,il6,tnfα,il10,cox2 and ptges significantly increased;the mRNA level of tgfβsignificantly decreased;the mRNA levels of apoptosis-related genes,caspase-8 and caspase-9,significantly increased;the mRNA level of bcl2 significantly decreased;the mRNA levels of cell proliferation-related genes,top2αand uhrf1,significantly reduced;the mRNA levels of liver function-related genes,alr and cyp3c1,significantly increased;and the mRNA level of cyp3a65 significantly decreased.In zebrafish,GTW can cause increased inflammation,enhanced apoptosis,decreased cell proliferation,and abnormal expression of liver function-related genes,leading to abnormal liver structure and function and resulting in hepatotoxicity.

文摘Triptolide is an important active component of Tripterygium wilfordii Hook F (TWHF) and possesses anti-inflammatory, immunosuppressive, male anti-fertility, and anticancer properties. A new method combining different techniques, including solid-liquid extraction, liquid-liquid partition, column chromatography and high-speed counter-current chromatography (HSCCC) but avoiding the use of chloroform, was developed for the isolation and purification of triptolide from the leaves of TWHF. 48 mg of triptolide at 96.5% purity was obtained from 1 kg of air-dried leaves of TWHF.

文摘Acute kidney injury(AKI)is a common and serious health issue with a growing incidence and mortality rate.Tripterygium wilfordii(TW)is a traditional Chinese medicine that has been reported to cause kidney damage.However,the associated mechanism of TW-induced AKI remains unclear.Therefore,we aimed to uncover the associated mechanisms of AKI induced by TW using network pharmacology and bioinformatics.The candidate compounds of TW and the potential targets were screened using TCMSP and CTD database,and the AKI-related targets were identified from the Dis Ge NET database.The disease targets were intersected with the drug targets,and the Wayne diagram was drawn by Venny 2.1.0 software.We developed proteinprotein interactions(PPI)network and the“disease-compound-target-pathway”network through the Cytoscape software.By using the DAVID database,GO and KEGG enrichment analysis was carried out to reveal the potential signaling pathways of the compound-TW-induced AKI.Meanwhile,the Auto dock vina 1.1.2 was used for molecular docking to verify the active compound and key targets’binding ability.Critical compounds and key targets of TW-induced AKI were identified,including triptolide,kaempferol,β-sitosterol,nobiletin,stigmasterol,TNF,and so on.The GO analysis showed that potential genes’biological function was mainly involved in apoptosis,oxidative stress,and inflammation.Moreover,eight signaling pathways were found,including the HIF-1 signaling pathway,VEGF signaling pathway,apoptosis,and so on.The molecular docking results proved that the core compound’s affinity with the corresponding protein of the gene targets was good.This study preliminarily predicted the core toxic compounds,targets,and related pathways of TW-induced AKI,providing a theoretical basis for the follow-up clinical rational drug use and related research work of TW.

文摘The stem and branch extract of Tripterygium wilfordii(Celastraceae)afforded seven new dihydroagarofuran sesquiterpene polyesters[tripterysines A‒G(1‒7)]and eight known ones(8‒15).The chemical structures of these new compounds were established based on combinational analysis of HR-ESI-MS and NMR techniques.The absolute configurations of tripterysines A‒C(1‒3)and E‒G(5‒7)were determined by X-ray crystallographic analysis and circular dichroism spectra.All the compounds were screened for their inhibitory effect on inflammation through determining their inhibitory effect on nitric oxide production in LPS-induced RAW 264.7 cells and the secretion of inflammatory cytokines TNF-αand IL-6 in LPS-induced BV2 macrophages.Compound 9 exhibited significant inhibitory activity on NO production with an IC50 value of 8.77μmol·L−1.Moreover,compound 7 showed the strongest inhibitory effect with the secretion of IL-6 at 27.36%.

Its mechanism, adverse reaction and mutagenesis effects, and dosage in clinical application in the recent years are now briefly reviewed in this paper.

文摘Objective: To investigate the curative efficacy of vitamin D3 in combination with tripterygium wilfordii polyglycoside in patients with rheumatoid arthritis (RA) and its influences on indices of bone restoration. Methods: A total of 320 patients with RA were collected as observation objects to be randomly divided into the control group and the observation group with 160 cases in each group. The control group was given treatment of tripterygium wilfordii polyglycoside, while the observation group was given treatment of vitamin D3 in combination with tripterygium wilfordii polyglycoside. All patients received a course treatment with 3 months. The curative efficacy, improvement of joint symptoms, changes of laboratory tests, indices of bone restoration and adverse reactions were compared. Results: After 3 months, assessment of curative efficacy showed that the observation group had a total curative efficiency ratio of 90.6%, which was significantly higher than that of 81.2% in the control group. Among the indices which reflect the improvement of joint symptoms, the joint pain index, joint tenderness index and joint swollenness index were statistically reduced in the observation group than those in the control group. And after treatment, in comparison with the control group, indices of laboratory tests of rheumatoid factor (RF), antistreptolyisn O antibody (ASO), c-reactive protein (CRP) were statistically lower respectively in the observation group. As to indices of bone restoration after treatment, the observation group had statistically lower serum level of receptor activator of nf-kb ligand (RNKL) and higher level of osteoprotegerin (OPG) than those in the control group. During the treatment, incidences of adverse reactions in the control group and the observation group were statistically same. Conclusion: Vitamin D3 in combination with tripterygium wilfordii polyglycoside has well curative efficacy in patients with RA, which can significantly improve joint symptoms, indices of laboratory tests and bone restoration.

It is the first example of abietane diterpenoid glycoside isolated from Tripterygium wilfordii Hook.f. Its structure was identified to be 11-O-b-D-glucopyranosyl- neotriptophenolide based on spectral methods.
