培土生金中药联合西医常规疗法治疗支气管哮喘缓解期肺脾两虚证的Meta分析及GRADE质量评价

目的:系统评价培土生金中药联合西医常规疗法治疗支气管哮喘缓解期肺脾两虚证的疗效以及证据质量,为临床决策及疾病指南制订提供循证依据。方法:检索中国知网(CNKI)、万方数据库(Wangfang)、Pubmed、Web of Science等8大中英文数据库,由... 目的:系统评价培土生金中药联合西医常规疗法治疗支气管哮喘缓解期肺脾两虚证的疗效以及证据质量,为临床决策及疾病指南制订提供循证依据。方法:检索中国知网(CNKI)、万方数据库(Wangfang)、Pubmed、Web of Science等8大中英文数据库,由2名研究者独立进行文献筛选、信息提取、偏倚风险评估及方法学质量评价,并对纳入研究的总有效率、中医证候积分、第一秒用力呼气容积(FEV1)、第一秒用力呼气容积/用力肺活量(FEV1/FVC)、哮喘控制测试(ACT)评分等结局指标进行Meta分析,最终依照GRADE评估证据质量。结果:共纳入14项研究,合计1263例患者。结果显示:(1)培土生金中药联合常规西医疗法可有效改善患者总有效率,降低患者中医证候积分,改善患者FEV1水平、FEV1/FVC及ACT评分。(2)总有效率证据质量较优,可信度高,其余指标证据质量较低,尚待更多高质量的研究支持以提升证据级别。结论:培土生金中药联合西医常规疗法治疗支气管哮喘缓解期肺脾两虚证优于单用西医常规治疗,且在总有效率上证据质量较高,具有一定的循证学意义。 展开更多

Functionally graded cellular structures(FGCSs)have a multitude of applications to a wide range of industries.Utilising the ever-progressing technology of additive manufacturing(AM),FGCSs can be applied to control material grading and achieve the desired mechanical properties.The current study explores the design and optimisation of FGCSs for AM,with a focus on improving the compression and impact performance of below knee(BK)prosthetic limbs made of thermoplastic polyurethane(TPU).A multiscale research methodology integrating topology optimization(TO),finite element analysis(FEA),and design of experiments(Do E)was adopted to optimise lattice structures in terms of stiffness and lightweight properties.Two-unit cell designs were considered in the study:Schwarz P gyroid and body-centered cubic(BCC).Response surface methodology(RSM)was implemented to analyse the effect of minimum and maximum cell wall thickness,cell size,and unit cell type on the mechanical performance of TPU FGCS structures.The results indicated that a Schwarz P FGCS structure with cell size,minimum and maximum cell wall thickness of 6,0.9 and 2.8 mm,respectively,could be optimal for a compromise between performance and weight.In this optimized case,stiffness and volume fraction values of 684 N/mm and 0.64 were obtained,respectively.The study also presents a proof-of-concept design for a BK prosthetic damper,highlighting the potential of FGCSs to enhance patient comfort,reduce manufacturing costs,and enable personalised designs through 3D scanning and AM.The obtained results could be a step forward towards the incorporation of AM technologies in prosthetics,offering a pathway to lightweight,cost-effective,and functionally tailored solutions. 展开更多

双肾增强CT在基层医院对肾透明细胞癌WHO/ISUP分级的诊断价值研究

目的:探讨增强CT影像学特征在透明细胞型肾细胞癌(ccRCC)WHO/ISUP分级中的价值,旨在为术前风险分层和个体化治疗提供依据。方法:回顾性分析2015年1月1日—2025年6月30日于宁海县第一医院经行双肾增强CT检查并经术后病理确诊为ccRCC的86... 目的:探讨增强CT影像学特征在透明细胞型肾细胞癌(ccRCC)WHO/ISUP分级中的价值,旨在为术前风险分层和个体化治疗提供依据。方法:回顾性分析2015年1月1日—2025年6月30日于宁海县第一医院经行双肾增强CT检查并经术后病理确诊为ccRCC的86例患者的临床资料,依据2016年WHO/国际泌尿病理学会(ISUP)分级,将I、II级纳入低级别组(n=74),III、IV级纳入高级别组(n=12)。对患者的双肾增强CT图像进行影像学评估,包括病灶长径、钙化、囊变、假包膜完整性、平扫CT值、皮质期及髓质期CT值、皮质期及髓质期强化率。通过受试者工作特征(ROC)曲线分析各影像学指标的诊断效能,计算AUC、截断值、灵敏度及特异度。结果:两组的年龄、性别、肿瘤部位、钙化及囊变占比差异无统计学意义(P>0.05)。高级别组的假包膜完整率低于低级别组(P=0.002);高级别组的髓质期CT值[(82.67±19.37)HU vs.(102.14±30.51)HU,]、皮质期强化率(1.71±0.88 vs.3.14±2.23)及髓质期强化率(1.38±0.74 vs.2.47±1.68)均低于低级别组(P<0.05)。皮质期强化率的AUC为0.751,髓质期强化率的AUC为0.785,髓质期的AUC为0.725。结论:增强CT影像学特征有望辅助术前鉴别ccRCC低级别与高级别肿瘤,强化率指标在诊断价值方面优于单纯CT值,能够为术前风险分层和个体化治疗提供参考。 展开更多

基于CT影像组学的可解释机器学习模型预测肾透明细胞癌WHO/ISUP分级 被引量:2

目的观察基于瘤内和瘤周CT图像构建的影像组学模型预测肾透明细胞癌(ccRCC)WHO/ISUP分级的价值,并通过Shapley(Shapley Additive exPlanations)算法对模型进行可视化分析。方法回顾性纳入218例经病理证实的WHO/ISUP分级ccRCC患者,分为... 目的观察基于瘤内和瘤周CT图像构建的影像组学模型预测肾透明细胞癌(ccRCC)WHO/ISUP分级的价值,并通过Shapley(Shapley Additive exPlanations)算法对模型进行可视化分析。方法回顾性纳入218例经病理证实的WHO/ISUP分级ccRCC患者,分为低等级组(n=155)和高等级组(n=63)。所有患者术前均接受增强CT扫描,按7∶3的比例随机划分为训练集(n=152)与测试集(n=66),采用3D Slicer软件手动勾画肿瘤感兴趣区(ROI-intra),并环形扩张2 mm、5 mm,获得瘤周感兴趣区(Peri-2 mm、Peri-5 mm),手动调整瘤周勾画区域并整合为瘤内和瘤周的感兴趣体积,提取并筛选影像组学特征后,分别建立瘤内、瘤周及瘤内+瘤周的随机森林模型。评估各个模型的曲线下面积(AUC),以挑选出测试集效能最佳的模型,并观察最佳模型的校准度和净收益,最后,采用Shapley算法对特征进行重要性排序和可视化。结果相比于瘤内单一影像组学模型(训练集AUC=0.821;测试集AUC=0.794),瘤内及瘤周影像组学模型在训练集和测试集中均展现出更高的预测性能,其中瘤内及瘤周2 mm的联合模型预测效能最佳,训练集和测试集中AUC分别为0.906和0.868,决策曲线显示联合模型获得较高的临床净获益。在模型的可解释性分析上,灰度共生矩阵和一阶统计量是WHO/ISUP分级预测模型中贡献度最高的两个特征。结论基于瘤内和瘤周的CT影像组学联合模型在术前可无创预测ccRCC的WHO/ISUP分级。Shapley算法可以为模型提供可解释性,保障了模型的实用性。 展开更多

We performed this study to investigate pathological upgrading from biopsy to prostatectomy and clinicopathological factors associated with grade group(GG)upgrading in patients with International Society of Urological Pathology(ISUP)GG 1 and 2 prostate cancer(PCa)in a Chinese cohort.We included patients diagnosed with PCa with ISUP GG 1 and 2 at biopsy,who underwent RP at our institution.Pre-and postoperative clinical variables were examined.Univariate and multivariate logistic regression analyses were conducted to identify independent factors associated with GG upgrading.Patients in GG upgraded group had higher total prostatespecific antigen(tPSA;median:14.43 ng ml^(−1) vs 10.52 ng ml^(−1),P=0.001)and PSA density(PSAD;median:0.45 ng ml^(−2) vs 0.27 ng ml^(−2),P<0.001)than those in GG nonupgraded group.Patients in upgraded group had a higher ratio for Prostate ImagingReporting and Data System(PI-RADS)score>3(86.4%vs 67.9%,P<0.001).Those with GG 1 in biopsy were more likely to experience GG upgrading after RP than those with GG 2(71 vs 54,P=0.016).Independent preoperative factors predicting GG upgrading were PI-RADS score>3(odds ratio[OR]:2.471,95%confidence interval[CI]:1.132–5.393;P=0.023),higher PSAD(P=0.001),and GG in biopsy(OR:0.241,95%CI:0.123–0.471;P<0.001).The histopathological analyses of RP specimens revealed that perineural invasion(PNI;OR:1.839,95%CI:1.027–3.490;P=0.041)was identified as an independent factor associated with GG upgrading.Our results revealed that GG in the biopsy,PSAD,PI-RADS score>3,and PNI were independent factors of GG upgrading.These factors should be considered for patients with ISUP grade≤2 PCa. 展开更多

ISUP1级前列腺癌根治术后病理升级预测:基于临床、影像学及穿刺活检数据的列线图模型构建

目的分析穿刺活检确诊为国际泌尿病理学会(ISUP)1级前列腺癌患者行根治术后病理升级的危险因素,并构建预测列线图模型。方法回顾性纳入2017年1月至2024年5月于海军军医大学第一附属医院穿刺活检诊断为ISUP 1级前列腺癌,且接受根治性前... 目的分析穿刺活检确诊为国际泌尿病理学会(ISUP)1级前列腺癌患者行根治术后病理升级的危险因素,并构建预测列线图模型。方法回顾性纳入2017年1月至2024年5月于海军军医大学第一附属医院穿刺活检诊断为ISUP 1级前列腺癌,且接受根治性前列腺切除术的患者256例。收集患者的临床、影像学及穿刺活检资料,采用单因素及多因素二元logistic回归筛选独立预测因素并构建列线图模型,通过ROC曲线、临床影响曲线和决策曲线分析评估模型性能,采用Hosmer-Lemeshow检验评估模型的稳定性。结果多因素二元logistic回归分析显示,穿刺活检阳性针数(OR=1.80)、前列腺影像报告数据系统(PI-RADS)评分(OR=1.88)及前列腺特异性抗原密度(PSAD)分层(OR=1.43)是术后病理升级的独立预测因子(均P<0.01)。基于这3个预测因子建立的列线图模型预测病理升级的AUC值为0.82(95%CI 0.77~0.87);决策曲线分析显示模型在阈值概率0.01~0.99范围内具有良好临床适用性;临床影响曲线分析表明,当模型阈值提升至0.40时可减少不必要治疗45例(假阳性率降低12%),净临床获益达0.46;Hosmer-Lemeshow检验显示模型拟合良好(P=0.45)。结论本研究构建的列线图模型能准确预测ISUP1级前列腺癌患者根治术后病理升级风险,为主动监测策略的个体化实施提供了量化依据。 展开更多

目的:探讨基于CT的细胞外容积分数(ECV)术前评价透明细胞肾细胞癌(ccRCC)WHO/ISUP病理分级的价值。方法:回顾性收集我院收治经手术后病理组织学证实的67例ccRCC患者,患者术前均行腹部CT平扫+三期增强扫描,根据WHO/ISUP分级,将Ⅰ、Ⅱ级... 目的:探讨基于CT的细胞外容积分数(ECV)术前评价透明细胞肾细胞癌(ccRCC)WHO/ISUP病理分级的价值。方法:回顾性收集我院收治经手术后病理组织学证实的67例ccRCC患者,患者术前均行腹部CT平扫+三期增强扫描,根据WHO/ISUP分级,将Ⅰ、Ⅱ级纳为低级别组(48例),Ⅲ、Ⅳ级纳为高级别组(19例)。采集两组患者的一般临床资料,测量计算定量参数,包括延迟期主动脉净增值(ΔHU_(Aorta))、延迟期肿瘤净增值(ΔHU_(tumor))及ECV,并进行对比分析,通过绘制受试者操作特征曲线(ROC)评价ECV对ccRCC病理分级的诊断效能。结果:低级别组ccRCC的ΔHU_(tumor)及ECV均高于高级别组。ROC曲线显示,ECV评价WHO/ISUP低级别ccRCC的效能(AUC:0.834)高于ΔHU_(tumor)(AUC:0.725)。结论:基于CT的ECV术前评价ccRCC的病理分级具有一定的价值。 展开更多

目的 系统评价甜梦胶囊(口服液)治疗睡眠障碍的有效性与安全性。方法 通过计算机检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Embase、EBSCO、C... 目的 系统评价甜梦胶囊(口服液)治疗睡眠障碍的有效性与安全性。方法 通过计算机检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Embase、EBSCO、Cochrane Library数据库,检索时间均为建库至2021年3月,收集采用甜梦胶囊(口服液)治疗睡眠障碍的随机对照试验(RCT)。使用Cochrane偏倚风险评估工具评估纳入研究的质量。采用RevMan5.4及Stata14.0进行Meta分析,并使用GRADE profiler评价证据质量。结果 共纳入17篇文献,涉及1 396例患者。Meta分析结果显示,试验组的治疗有效率显著高于对照组(P<0.00001)。治疗后,试验组的匹兹堡睡眠质量指数(PSQI)评分显著低于对照组(P<0.00001)。治疗后,试验组的多导睡眠图(PSG)睡眠效率显著高于对照组(P<0.00001)。试验组的不良反应发生率低于对照组(P=0.03)。根据GRADE标准,有效率、PSQI评分、PSG睡眠效率为低质量证据,不良反应发生率为中等质量证据。结论 现有证据表明,甜梦胶囊(口服液)能有效提高睡眠效率,减少不良反应的发生,但上述结论尚需更多更高质量研究进一步验证。 展开更多

目的:系统评价加载口服中成药干预冠状动脉慢血流现象(CSFP)的疗效与安全性。方法:通过检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、EMbase、the Cochrane Library、Wed of Science数据库自建库至2022年12月1日收录的... 目的:系统评价加载口服中成药干预冠状动脉慢血流现象(CSFP)的疗效与安全性。方法:通过检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、EMbase、the Cochrane Library、Wed of Science数据库自建库至2022年12月1日收录的加载口服中成药干预CSFP病人的随机对照试验。采用Cochrane偏倚风险评估工具及改良Jadad量表评价文献质量。采用RevMan 5.3及Stata 17.0对主要结局指标[校正的心肌梗死溶栓试验帧数(cTFC)、心肌梗死溶栓试验(TIMI)血流3级发生率、冠状动脉微血管阻力指数(IMR)]、次要结局指标[超敏C反应蛋白(hs-CRP)、西雅图心绞痛量表(SAQ)评分]及不良反应发生率进行Meta分析。应用GRADEpro GDT评价证据质量。结果:共纳入20项研究,涉及1417例病人。纳入研究的文献质量普遍较低。Meta分析结果显示,与对照组比较,试验组cTFC[MD=-8.57,95%C(I-12.60,-4.54),P<0.0001]、IMR[MD=-4.15,95%C(I-5.60,-2.69),P<0.00001]和hs-CRP[MD=-1.57,95%C(I-1.81,-1.33),P<0.00001]降低,TIMI血流3级发生率[RR=2.04,95%C(I 1.66,2.50),P<0.00001]和SAQ评分[MD=67.93,95%C(I 54.24,81.63),P<0.00001]增加。安全性方面,两组不良反应发生率比较,差异无统计学意义(P>0.05)。GRADE分级显示TIMI血流3级发生率为中级,cTFC、hs-CRP、SAQ评分、不良反应发生率为低级,IMR为极低级。结论:现有证据表明,与西医常规治疗相比,加载口服中成药可进一步改善CSFP病人的冠状动脉血流、冠状动脉微血管功能及生活质量,抑制炎症反应。 展开更多

目的:探究基于CT构建机器学习(ML)及深度学习(DL)模型术前预测肾透明细胞癌(ccRCC)WHO/国际泌尿病理协会(ISUP)分级的价值。方法:回顾性收集常州市第一人民医院2011年1月—2023年5月收治的210例经手术病理证实的ccRCC患者的临床资料,根... 目的:探究基于CT构建机器学习(ML)及深度学习(DL)模型术前预测肾透明细胞癌(ccRCC)WHO/国际泌尿病理协会(ISUP)分级的价值。方法:回顾性收集常州市第一人民医院2011年1月—2023年5月收治的210例经手术病理证实的ccRCC患者的临床资料,根据WHO/ISUP分级分为低级别组(Ⅰ级、Ⅱ级)、高级别组(Ⅲ级、Ⅳ级),并按7:3随机分为训练集及验证集。从患者的CT平扫、皮髓质期及实质期中提取特征,采用支持向量机(SVM)、K近邻(KNN)、轻量梯度提升决策树(LightGBM)、随机森林(RF)、随机梯度下降(SGD)及极限梯度增强树(XGBoost)6种分类器构建ML及DL模型。结果:受试者工作特征(ROC)曲线下面积(AUC)显示在验证集中基于(平扫+皮髓质期+实质期)组合期构建的ML模型范围0.853~1.000、DL模型0.896~0.986;基于组合期,在验证集中采用RF、XGBoost及LightGBM分类器构建的ML模型AUC值均为1.000,DL模型分别为0.896、0.964、0.964(P<0.05),Delong检验差异有统计学意义(P<0.05);该三种分类器中,RF分类器的F1分数显示平扫0.722、皮髓质期0.648、实质期0.620、组合期1.000。结论:基于组合期,采用RF分类器构建的ML模型可为ccRCC肿瘤侵袭性术前评估提供可靠、无创的影像学方式。 展开更多

目的:基于推荐分级的评估、制订和评价系统(grading of recommendations assessment,development and e valuation,GRADE)与整合证据链法(integrated evidence chain-based efficacy evaluation of traditional Chinese medicine,i EC-E... 目的:基于推荐分级的评估、制订和评价系统(grading of recommendations assessment,development and e valuation,GRADE)与整合证据链法(integrated evidence chain-based efficacy evaluation of traditional Chinese medicine,i EC-Eff),评价电针治疗痛风性关节炎(gouty arthritis,GA)的证据质量。方法:计算机检索PubMed、Web of Science、Cochrane Library、Embase、中国知网、中文科技期刊数据库、万方医学期刊数据库、中国生物医学文献数据库等数据库,检索时限为建库至2025年4月,纳入电针治疗GA的随机对照试验(randomized controlled trial,RCT)、实验研究及临床经验文献,采用Cochrane偏倚风险工具以及针刺临床试验干预措施报告标准(standards for reportlng interventions in clinical trials of acupuncture,STRICTA)评估RCT质量,采用Stata 18软件进行Meta分析,采用GRADE工具对结局指标进行证据质量评级,通过i EC-Eff整合多维度证据。结果:GRADE评价中,单一电针干预血尿酸、疼痛视觉模拟评分(visual analogue scale,VAS)等指标证据为C级(低质量证据),针药结合降低血尿酸、改善炎症指标C反应蛋白(C-reactive protein,CRP)和红细胞沉降率(erythrocyte sedimentation reaction,ESR)为B级(中等质量证据),细胞因子等指标证据为C级(低质量证据)。i EC-Eff评价中,临床经验证据为B级,实验研究证据为A级,临床试验证据为B级,综合评定为BAB(中级证据)。GRADE与i EC-Eff评价体系差异源于前者对研究设计严谨性要求高,后者契合中医药整体模式。电针联合西药在降尿酸、抗炎方面具有优势,但需高质量RCT验证。结论:电针治疗GA可以改善关节红肿热痛症状、降低血尿酸水平、减轻关节炎症反应(如ESR、CRP等指标)等,证据质量较高,具有较好的临床应用前景。GRADE与i EC-Eff在电针治疗GA的证据评价方面各有优势,未来需将二者结合起来优化中医药疗效评价体系,推动电针治疗GA的循证医学证据积累与国际化认可。 展开更多

目的 系统评价小儿肺热咳喘口服液治疗肺炎支原体肺炎的有效性及安全性.方法 计算机检索知网、万方、维普、中国生物医学文献数据库、Pubmed、Cochrane Libr ary、Embase国内外数据库自建库至2024年3月31日所发表的小儿肺热咳喘口服液... 目的 系统评价小儿肺热咳喘口服液治疗肺炎支原体肺炎的有效性及安全性.方法 计算机检索知网、万方、维普、中国生物医学文献数据库、Pubmed、Cochrane Libr ary、Embase国内外数据库自建库至2024年3月31日所发表的小儿肺热咳喘口服液治疗肺炎支原体肺炎的随机对照试验文献,并追溯相关灰色文献.2名研究者分别独自进行文献筛选与资料提取,并对纳入研究进行质量评价,Meta分析及证据质量评价分别采用Rev Man5.4软件、GRADE系统进行.结果 共纳入16项研究,共1503例患者,其中试验组752例,对照组751例.Meta分析结果表明,联用小儿肺热咳喘口服液治疗肺炎支原体肺炎的总有效率、热退时间、咳嗽消失时间、咳痰消失时间、肺部啰音消失时间、C反应蛋白水平、白细胞介素-6水平、肿瘤坏死因子α水平均优于西医常规治疗,且并未增加不良反应发生率.GRADE证据分级结果显示:总有效率、咳痰消失时间、不良反应发生率的证据级别为低级;热退时间、咳嗽及肺部啰音消失时间、炎性指标(C反应蛋白、白细胞介素-6、肿瘤坏死因子α)、肺功能(第一秒用力呼气容积、第一秒用力呼气容积与用力肺活量的比值、最大自主通气量)的证据级别为极低级.结论 小儿肺热咳喘口服液治疗儿童肺炎支原体肺炎疗效确切,安全性优于西医常规治疗. 展开更多

The good combination of mechanical and wear properties for cemented carbides is crucial.In this work,the wear behavior of functionally graded cemented carbide(FGCC)and non-graded cemented carbide(CC),with CoNiFeCr multi-principal-element alloy(MPEA)binder,has been investigated by performing sliding wear tests and composition characterization.The results showed that compared with CC,FGCC had higher hardness,stronger fracture toughness,better wear performance,and similar TRS.FGCCs exhibited lower wear rates(3.44×10^(−7)–6.95×10^(−6)mm^(3)/(N m))and coefficients of friction(COFs)(0.27–0.39)than CCs from RT to 600℃due to mitigation of multiple risks caused by binder removal,fragmentation and pull-out of WC grains,high-temperature oxidation and softening.In the low-temperature wear stage,the MPEA binder underwent dynamic recrystallization(DRX)and twinning deformation before removing from the surface.The binder removal caused dislocation pile-ups and stacking faults(SFs)to form under high stress,resulting in fragmentation and pull-out of WC grains.The low-temperature wear was dominated by abrasive wear and adhesive wear,with a low wear rate and a high and unstable COF.In the high-temperature wear stage,initial pitting oxidation of WC grains generated many subgrain boundaries,reducing heat transfer and exacerbating oxidation,resulting in an oxide layer enriched with WO3,Mx Oy,and MWO4.High-temperature wear was dominated by oxidation wear and high-temperature softening,with a high wear rate and a low and smooth COF.The results from the present study do not only provide theoretical guidance for an understanding of the antiwear mechanism of WC-CoNiFeCr,but also a new approach for the preparation of cemented carbides with high wear resistance. 展开更多

It is important to analyze the damage evolution process of surrounding rock under different water content for the stability of engineering rock mass.Based on digital speckle correlation(DSCM),acoustic emission(AE)and electromagnetic radiation(EMR),uniaxial hierarchical cyclic loading and unloading tests were carried out on sandstones with different fracture numbers under dry,natural and saturated water content,to explore the fracture propagation,failure precursor characteristics and damage response mechanism under the influence of water content effect.The results show that with the increase of water content,the peak stress and crack initiation stress decrease gradually,and the decreases are 15.28%-21.11%and 17.64%-23.04%,respectively.The peak strain and crack initiation strain increase gradually,and the increases are 19.85%-44.53%and 19.15%-41.94%,respectively.The precracked rock with different water content is mainly characterized by tensile failure at different loading stages.However,with the increase of water content,the proportion of shear cracks gradually increases,while acoustic emission events gradually decrease,the dissipative energy and energy storage limits of the rock under peak load gradually decrease,and the charge signal increases significantly,which is because the lubrication effect of water reduces the friction coefficient between crack surfaces. 展开更多

The graded density impactor(GDI)dynamic loading technique is crucial for acquiring the dynamic physical property parameters of materials used in weapons.The accuracy and timeliness of GDI structural design are key to achieving controllable stress-strain rate loading.In this study,we have,for the first time,combined one-dimensional fluid computational software with machine learning methods.We first elucidated the mechanisms by which GDI structures control stress and strain rates.Subsequently,we constructed a machine learning model to create a structure-property response surface.The results show that altering the loading velocity and interlayer thickness has a pronounced regulatory effect on stress and strain rates.In contrast,the impedance distribution index and target thickness have less significant effects on stress regulation,although there is a matching relationship between target thickness and interlayer thickness.Compared with traditional design methods,the machine learning approach offers a10^(4)—10^(5)times increase in efficiency and the potential to achieve a global optimum,holding promise for guiding the design of GDI. 展开更多

BACKGROUND Endoscopic submucosal dissection(ESD)has become a widely accepted,minimally invasive treatment for gastrointestinal submucosal tumors.It has been reported that humanistic nursing care with graded psychological interventions can effectively enhance patients’physical activity in patients,reduce postoperative complications,and improve their postoperative quality of life.AIM To investigate the effects of combining humanistic care with graded psychological support on nursing satisfaction and quality of life in patients undergoing ESD for gastrointestinal submucosal tumors.METHODS A retrospective analysis was conducted on the clinical data of 180 patients who underwent ESD surgery for gastrointestinal submucosal tumors at our hospital between March 2021 and February 2023.Patients were allocated into groups based on the nursing care they received:The control group,which received routine care(n=90),and the observation group,which was subjected to humanistic nursing care in combination with graded psychological support(n=90).Patient anxiety and depression were assessed using the self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Quality of life was evaluated using the shortform 36 health survey,and additional indications such as time to first food intake,surgery duration,length of hospital stay,nursing satisfaction,and adverse reactions were also recorded.Data was analyzed using SPSS22.0,with t-tests employed for continuous variables andχ2 tests for categorical data.RESULTS Patients in the observation group experienced significantly shorter times to first postoperative meal,surgery,and hospital stay compared to the control group.After the intervention,the SAS score of the observation group was 43.17±5.68,and the SDS score was 41.57±6.52,both significantly lower than those of the control group,with SAS score of 52.38±5.21 and SDS score of 51.23±8.25.In addition,the observation group scored significantly higher in daily living,physical function,psychological well-being,and social functioning(80.01±6.39,83.59±6.89,81.69±5.34,and 85.23±6.05,respectively).Moreover,the observation group also exhibited higher satisfaction and selfefficacy scores and a lower incidence of adverse reactions compared to the control group(P<0.05).CONCLUSION For patients undergoing ESD for gastrointestinal submucosal tumors,humanistic nursing care in combination with graded psychological nursing care significantly shorten the times to first postoperative meal,surgery,and hospital stay,effectively alleviates anxiety and depression,improves quality of life and nursing satisfaction,and mitigate the incidence of adverse reactions. 展开更多

Fe-Mo functionally graded materials(FGMs)with different composition-change rates from 100%304 stainless steel to 100%Mo along the composition gradient direction were prepared by electron beam-directed energy deposition(EB-DED)technique,including three samples with composition mutation of 100%,composition change rate of 10%and 30%.Results show that the composition-change rate significantly affects the microstructure and mechanical properties of the samples.In the sample with abrupt change of composition,the sharp shift in composition between 304 stainless steel and Mo leads to a great difference in the microstructure and hardness near the interface between the two materials.With the increase in the number of gradient layers,the composition changes continuously along the direction of deposition height,and the microstructure morphology shows a smooth transition from 304 stainless steel to Mo,which is gradually transformed from columnar crystal to dendritic crystal.Elements Fe,Mo,and other major elements transform linearly along the gradient direction,with sufficient interlayer diffusion between the deposited layers,leading to good metallurgical bonding.The smaller the change in composition gradient,the greater the microhardness value along the deposition direction.When the composition gradient is 10%,the gradient layer exhibits higher hardness(940 HV)and excellent resistance to surface abrasion,and the overall compressive properties of the samples are better,with the compressive fracture stress in the top region reaching 750.05±14 MPa. 展开更多
