Patients and Method: R... Introduction: Invasive cancers of the vulva are uncommon;it is a pathology of the elderly. More and more young women are concerned. Objectives: To study and analyze vulvar cancer in our context. Patients and Method: Retrospective, descriptive and analytical study carried out in the Gynecology Department of the Owendo University Hospital over a period of 5 years from 2020 to 2024. All cancer cases of the period were concerned and we included vulvar cancers. We studied the epidemiological, clinical and therapeutic aspects. Results: We collected 5 files, the median age was 41.8 years [30 - 57 years], the average consultation time was 18 months [12 - 36 months]. Ulcerated, budding, and itchy lesions were present in 80% of cases. Invasive squamous cell carcinoma was the histological type present in all patients. Surgery was performed in 40% of cases. Concomitant radiochemotherapy (CRC) was offered in all patients. HIV was the comorbidity found in 100% of cases. Conclusion: Vulvar cancers are rare. They are often seen at advanced stages because of their location, the taboo nature they represent on the one hand and underdiagnosis on the other. 展开更多

Background: The current study was carried out to evaluate the effects of mycotoxin biodegradation agent(MBA, composed of Bacillus subtilis ANSB01 G and Devosia sp. ANSB714) on relieving zearalenone(ZEA) and deoxynivalenol(DON) toxicosis in immature gilts.Methods: A total of forty pre-pubertal female gilts(61.42 ± 1.18 kg) were randomly allocated to four diet treatments: CO(positive control); MO(negative control, ZEA 596.86 μg/kg feed and DON 796 μg/kg feed);COA(CO + 2 g MBA/kg feed); MOA(MO + 2 g MBA/kg feed). Each treatment contained 10 replicates with 1 gilt per replicate. Gilts were housed in an environmentally controlled room with the partially slatted floor.Results: During the entire experimental period of 28 d, average daily gain(ADG) and average daily feed intake(ADFI)of gilts in MO group was significantly reduced compared with those in CO group. The vulva size of gilts was significantly higher in MO group than CO group. In addition, significant increases in the plasma levels of Ig A,Ig G, IL-8, IL-10 and PRL were determined in MO group compared with that in CO group. ZEA and DON in the diet upregulated apoptotic caspase-3 in ovaries and uteri, along with down-regulated the anti-apoptotic protein Bcl-2 in ovaries. The supplementation of MBA into diets co-contaminated with ZEA and DON significantly increased ADG, decreased the vulva sizes, reduced the levels of Ig G, IL-8 and PRL in plasma, and regulated apoptosis in ovaries and uteri of gilts.Conclusions: The present results indicated that feeding diet contaminated with ZEA and DON simultaneously(596.86 μg/kg + 796 μg/kg) had detrimental effects on growth performance, plasma immune function and reproductive status of gilts. And MBA could reduce the negative impacts of these two toxins, believed as a promising feed additive for mitigating toxicosis of ZEA and DON at low levels in gilts. 展开更多

Background:One of the biggest challenges in the swine industry is to increase female reproductive efficiency.Recently,vulva score categories(VSC),assessed prior to puberty,has been proposed as an indicator trait of efficient reproductive performance in sows.The objective of this study was to validate the use of VSC as an indicator trait for reproductive performance,and to perform genetic and genomic analyses for VSC.Methods:The phenotypic relationship of VSC,using a three-point scale:small(VSC-S),medium(VSC-M),and large(VSC-L),on reproductive performance was evaluated on three farms.VSC was measured at 15 weeks of age,for farms 1 and 2,and at 14 weeks of age for farm 3 on 3981 Yorkshire gilts,in which 1083 had genotypes(~50 K SNPs).Genetic parameters for VSC with reproductive traits were estimated using ssGBLUP.A Genome-wide association study(GWAS)for VSC was performed using BayesB.Results:For the phenotypic analysis of VSC across datasets,differences in performance were identified there was a significant effect(P≤0.05)for the interaction between Farm and VSC for total number dead(TND),and a trend(P<0.10)for total number born(TNB).There were significant(P≤0.05)pre-defined contrasts of VSC-S versus VSC-M+L on TNB,number born alive(NBA),TND,number of stillborn(NSB),and number of mummies(MUM).Heritability estimates for VSC as a categorical trait(VSCc)and a quantitative trait(VSCq)were 0.40±0.02 and 0.83±0.02,respectively,for across farm,0.13±0.07 and 0.20±0.10,respectively,for Farm1,0.07±0.07 and 0.09±0.09,respectively,for Farm2,and 0.20±0.03 and 0.34±0.05,respectively,for Farm3.For across farms,favorable genetic correlations estimates were found for TNB(0.28±0.19)and NBA(0.26±0.17).Within farms,moderate genetic correlations between VSC with reproductive traits were found for TNB(0.61±0.47)and MUM(0.69±0.47)for farm 1,for number of services until first farrow(NS;0.69±0.38)and unique service with successful first farrow(SFS;−0.71±0.38)for farm 3.Multiple genomic regions associated with VSCc were identified.Of these,a QTL located on chromosome 3 at 33–34Mb accounted for about 7.1%of the genetic variance for VSCc and VSCq.This region harbors the gene PRM1 that has been associated with early embryonic development in pigs.Conclusions:The results support potential of VSC for improved reproductive efficiency on first-parity performance,but the results might depend on the interaction between environmental factors and VSC,as well as potentially additive genetics. 展开更多

OBJECTIVE: To clarify the role of midkine (MK) in vulvar carcinogenesis though examination of its expression in vulvar lesions including vulvar condyloma acuminata (VCA), vulvar intraepithelial neoplasia (VIN) and vulvar squamous cell carcinomas (VSCC), and to analyze the relationship between MK expression and human papilloma virus (HPV) infection. METHODS: Thirty VSCC, 15 VIN and 10 VCA patients were studied by streptavidin-biotin-immunoperoxidase method. MK expression was compared with clinicopathologic features of vulvar tumors. RESULTS: MK was expressed in 26 of 30 VSCC (87%), 3 of 5 VIN III and all VCA samples, whereas no MK expression was detected in the VIN I-II samples or in normal epithelium. Objective To determine the incidence of infection with HPV and the distribution of HPV genotypes on patients with Condyloma acuminatum. Methods Twenty-three different HPV types were detected by PCR and reverse dot blot (RDB) hybridization over all 6 508 samples of vulva and cervix uteri in patients with condyloma acuminatum. Including 18 types were high-risk (HR)-HPV (16, 18, 31, 33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66, 68, 73, 83 and MM4), and 5 types were low-risk (LR)-HPV (6, 11, 42, 43 and 44). Results Among 6 508 cases, there were 3 288 cases with HPV infection and the incidence rate was 50.52%. The positive HPV patients overlap all 23 genotypes detected. There were 2 038 cases infected with a single HPV type and 1 250 cases infected with multiple HPV types. The constituent ratios in positive cases were 61.98% and 38.02%, respectively. There were 1 453 cases only with LR-HPV types infection, 945 cases only with HR-HPV types and 890 cases both with LR-HPV and HR-HPV infection, and the constituent ratios were 44.19%, 28.74% and 27.07%. There were 4 843 times positive HPV infection, including HR-HPV 2 361 times and LR-HPV 2 482 times. The common HR-HPV genotypes were 16, 52, 58, 56, 18, 66 and 33, and the incidence ratios were 6.31%, 5.06%, 4.04%, 2.60%, 2.41%, 2.40% and 2.28%. And the common LR-HPV genotypes were 6, 11 and 43, and the incidence ratios were 16.98%,11.09% and 6.75%, respectively. Overlap 23 types, the most common geno- types were HPV6 and HPVll, the incidence rates were higher than others (P〈0.05). Conclusion HPV infection, especially with HR-HPV genotypes infect genital tract, which caused Condyloma acuminatum and cervical lesions, or cervical cancer. The detection of HPV genotypes was very important to prevent, diagnose early and therapy for cervical lesions or cervical cancer. 展开更多

Ovarian metastatic malignant melanoma is extremely rare. We describe a patient in whom vulvar melanoma (previously resected) metastasized to the ovary, making ovarian metastatic malignant melanoma. A 43-year-old Japanese woman was referred to us because of left ovarian tumor. She had undergone resection for malignant melanoma on the right labia minora with inguinal lymph node metastasis (pT1bN1aM0, stage IIIA, FIGO 2008). Eighteen months after this surgery, CT scans revealed left ovarian tumor and swelled pelvic lymph nodes, with a pelvic examination disclosing a left adnexal solid mass, with normal serum CA125 level (21.7 U/mL). Laparotomy revealed a left solid ovarian tumor measuring 4 cm, which was covered with a smooth grayish capsule. The right ovary, uterus, and pelvic cavity appeared normal. Upon sectioning during the surgery, the cross-sectional surface of the left ovary revealed a dark brown solid tumor. Following an intra-operative frozen-section diagnosis as metastatic melanoma, total hysterectomy with bilateral salpingo-oophorectomy and pelvic lymph dissection was performed. Histological examination confirmed the diagnosis as malignant melanoma metastasis to the left ovary and the obturator lymph node: the same laterality (left) as the primary site. The tumor was entirely composed of malignant melanoma cells with no evidence of teratoma. Combined chemotherapy with dabrafenib mesylate and trametinib was planned based on the positive BRAF mutations. This case highlights the importance that physicians should have high index of suspicion for the occurrence of ovarian melanoma metastasis after melanoma surgery. We also made extensive literature review on this issue, of which description may contribute to better understanding of this condition. 展开更多

Squamous cell carcinoma (SCC) is the most common carcinoma of the vulva. Distant metastasis of recurrent vulvar squamous cell carcinoma is rare and occurs late in the disease process. We report the first case of kidney metastasis from a vulvar squamous cell carcinoma in a 68-year-old Caucasian female. On initial presentation she was treated with radical vulvectomy, upper urethrectomy with bilateral inguinal and deep femoral lymph node dissection. She was staged as FIGO stage IVA and also received adjuvant chemo-radiation. She remained in remission for 24 months. Subsequently she was found to have a kidney tumor and underwent nephrectomy and was diagnosed with metastatic squamous cell carcinoma from the vulva to the kidney. In cases of recurrent vulvar carcinoma distant metastasis to the bones, breast, and brain is only rarely reported. Metastasis to kidneys from vulvar carcinoma is exceptionally rare with no reported cases in the literature. Renal metastasis should be considered in the differential diagnosis of kidney tumor in this group of women. 展开更多

Objective: To review our experience in diagnosis, pathology, treatment, and prognosis of extramammary Paget's disease of the vulva. Methods: Seven patients with vulvar Paget's disease who were treated at the Cancer Hospital cAMS were retrospectively reviewed for the period from 1960 to 2002. The symptoms, location of disease, pathology, mode of treatment and current status of the patients were evaluated. Results: A total of seven women with Paget抯 disease of the vulva were admitted to the Department of Gynecologic of Oncology. The mean age was 67.3 years (range 54~81 years). Vulvar lesion and pruritus were the common symptoms and it took average 3.4 years to confirm the diagnosis. All patients underwent surgery as initial treatment, vulvectomy was performed for 2 patients and radical vulvectomy plus groin node dissection for 5 patients. Two patients had invasive disease and four were associated with underlying adenocarcinoma. Five patients experienced recurrence, on average, 16.2 months after the surgery. Four of them underwent radiotherapy or chemotherapy and the other received no further treatment. The mean follow-up time was 41.1 months. Three patients, with invasive lesion or underlying adenocarcinoma were dead of the disease. Conclusion: There is a delay in the diagnosis of vulvar Paget's disease. The major treatment is surgery. Recurrence is relatively common. Patients with invasive disease or underlying carcinoma have a poor prognosis. 展开更多

Objective: To analyze the clinicopathological features and differential diagnosis of angiomyofibroblastoma (AMFB) of the vulva. Methods: Two cases of AMFB were examined by light microscopy and immunohistochemical study and to discuss the clinicopathological features and differential diagnosis of AMFB with the reference to the literature. Results: Tumors were all circumscribed, and 〈5 cm in diameter. Microscopically, the tumors were composed of spindle or polygonal cells that were cellularly or hypocellularly arranged with perivascular accentuation in a mucoid or fibrocollagenous background. The tumors contained numerous small- to medium-sized blood vessels, which were characteristically thin walled. Immunohistochemically, two cases of tumor cells were positive for vimentin, SMA, CD34(+) and FⅧ(+). Desmin and MSA were positive in one case; Cytokeratin, S-100, CD31 were negative in both. Conclusion: AMFB is a rare, benign soft tissue tumor that occurs in the genital tract of adult women. The origin remains unclear, but it is suggested that an origin from a perivascular pluripotent stem cell that is capable of myofibroblastoma differentiation. Angiomyofibroblastoma should be differentiated from other neoplasms of the vulva such as aggressive angiomyxoma, superficial angiomyxoma and cellular angiofibroma. 展开更多

Vulvovaginal myeloid sarcoma with massive pelvic floor infiltration: A case report and review of literature

BACKGROUND Myeloid sarcoma(MS),including isolated and leukaemic MS,is an extramedullary myeloid tumour.MS can involve any anatomical site,but MS of the female genital tract is rare,with the ovaries and uterine body and cervix being the most commonly seen sites.Involvement of the vagina and vulva is extremely rare.CASE SUMMARY We report a rare case of MS with involvement of the vulva and vagina and massive infiltration of the pelvic floor.A 26-year-old woman presented with a vulvar mass,irregular vaginal bleeding and night sweats.Magnetic resonance imaging demonstrated an ill-defined,irregular vulvovaginal mass with massive involvement of the paravaginal tissue,urethra,posterior wall of the bladder,and pelvic floor.The signal and enhancement of the huge mass was homogeneous without haemorrhage or necrosis.Positron emission tomography/computed tomography showed high fluorodeoxyglucose uptake by the mass.Peripheral blood count detected blast cells.Vulvovaginal mass and bone marrow biopsies were performed,and immunohistochemistry confirmed the diagnosis of acute myeloid leukaemia(M-2 type,FAB classification)and vulvovaginal MS.The patient was treated with induction chemotherapy followed by allogeneic haematopoietic stem cell transplantation,and achieved complete remission.A systemic review of the literature on vulvovaginal MS was conducted to explore this rare entity’s clinical and radiological features.CONCLUSION Vulvovaginal MS is extremely rare.Diagnosis of vulvovaginal MS can only be confirmed histopathologically.Even though its clinical and imaging presentations are nonspecific,MS should be considered in the differential diagnosis of a newly developed T2-hyperintense,homogeneously enhanced vulvovaginal mass,especially in a patient with suspected haematological malignancy. 展开更多

BACKGROUND Langerhans cell histiocytosis(LCH)is a histiocytic proliferative disease caused by clonal proliferation of Langerhans cells,which is currently defined as an inflam-matory myeloid tumor.It is rare in adults,with an incidence of 1–2 per million,and is highly heterogeneous in clinical presentation,with unpredictable disease progression and outcome.CASE SUMMARY A 52-year-old postmenopausal female patient presented to the gynecology department in July 2023 with bilateral vulvar masses.She was diagnosed with recurrent multisystem LCH.The patient had previously been diagnosed with a single-system and single-focal LCH in October 2021 due to a right maxillofacial mass,which resolved after surgical treatment.A chemotherapy regimen was developed after multidisciplinary consultation.Six cycles of chemotherapy resulted in partial remission,and maintenance chemotherapy is currently being administered.CONCLUSION Recurrent LCH involving the bilateral vulva has been poorly reported.Compre-hensive imaging and pathological evaluation is important for diagnosis.The model of joint multidisciplinary specialist diagnosis and treatment is worthy of clinical application. 展开更多

Only 20 cases of primary vulva LCH have been previously reported in the medical literature. In this report, we describe an additional case of LCH on the vulva. A 28-year-old Chinese woman presented with a two-year history of ulcerous lesions on the left vulva. No associated temperature, tiredness, or general malaise was observed. A diagnosis of LCH had been made by biopsy. Histological and immunohistochemical findings were characteristic of LCH. After two weeks of combined medical therapy, including interferon, prednisone, and methotrexate (MTX), the lesion started to cicatrize. Now, the patient was well throughout a 18-month follow up, showing no symptoms or signs of local recurrence or systemic spread. The occurrence of LCH on the vulva is very unusual. It is necessary to perform a biopsy on the lesion, rule out the possibility of multiorgan involvement. There are no standard treatment options for this rare disease. The most effective treatment options remain elusive. In our case, combined medical therapy was proved to be effective. 展开更多

BACKGROUND Langerhans cell histiocytosis(LCH)is a rare clonal proliferative disease of Langerhans cells with unknown pathogenesis.An increasing number of clinicians recognize that LCH has a wide clinical spectrum and a highly varied course.Adults rarely develop LCH.Here,we report a case of adult localized LCH.CASE SUMMARY A 32-year-old woman presented with plaques and ulcers on the vulva and crissum,accompanied by pain that persisted for more than one year.Physical examination revealed a red-infiltrating plaque with ulcerations and exudates in the vulva and crissum.Pathological examination revealed a diffuse infiltration of lymphocytes,eosinophilic granulocytes,and histiocytoid cells in the superficial dermis.Proliferative histiocytoid cells showed mild atypia,partly with kidneyshaped nuclei.Immunohistochemical examination showed that the histiocytoid cells were positive for S100 protein and CD1 and weakly positive for CD68(20%+),with a Ki-67 index of 30%.Laboratory tests did not reveal any other systemic damage.The patient was diagnosed with adult localized LCH and was prescribed oral prednisone(20 mg)once daily.The skin lesions gradually improved and are still being followed-up.CONCLUSION Adult localized LCH is rare and must be differentiated from other common conditions. 展开更多

Object: To determine if vulvar melanosis progressed to melanoma over a period of 20 years or more. Methods: In 2010 the hospital records from the Royal Brisbane Hospital Vulvar Clinic between 1976 and 1988 were reviewed and cross checked with the state wide Queensland Centre for Gynaecological Cancer (QCGC) data base to determine if any patient had been lost to follow up and subsequently developed a vulvar melanoma. Data collected were stored and analysed using the computer software Statistical Package for the Social Sciences (SPSS) 11.0. Results: None of the 12 patients developed vulval melanoma in the years up to 2010. Conclusion: In this small group, followed for more than 20 years, melanosis was not a precursor of melanoma. One patient, who attended the Vulvar Clinic but was not included in this melanosis study, was found to have co-existing melanosis well away from her melanoma in situ and malignant melanoma at presentation. It was not possible to determine if these findings represented a progression of the benign to malignant. Biopsy of abnormal hyper pigmented vulvar skin is recommended. Current knowledge suggests that vulvar melanosis is a benign condition but to be on the safe side follow up of all hyper pigmented vulval lesions to detect early malignant change is recommended. 展开更多

Background: Vulva haematoma is an uncommon complication of childbirth that can cause maternal death if not properly managed. We present a case of large vulva haematoma managed conservatively with good outcome in UNTH Enugu. Clinical Presentation: This was a case of an unbooked 24 years old primiparous woman referred from a private hospital to UNTH, Enugu on account of left-sided vulva swelling of four hours duration following a spontaneous vaginal delivery of a live male baby that weighed 3.6 kilogrammes. She was given episiotomy which was repaired. Vaginal examination showed a swelling involving left labia majora and minora and extending to the perineal region and vagina. It measured 12 cm × 10 cm, firm and mildly tender. She was resuscitated with intravenous fluid, transfused with two units of blood and commenced on antibiotic and analgesic. The vulva haematoma was monitored and it remained the same size for two days, and then progressively regressed. She was discharged home on the 9th day. Conclusion: Conservative management of large vulva haematoma involving good counseling, correction of anaemia, institution of antibiotics, analgesic and close monitoring leads to effective resolution, reduced cost, scarring, pain and dyspareunia. 展开更多

Correlation between Vulvar Symptoms Stratification and Vulvar Cancer Detection: Prospective Cohort Observational Study

Cancer vulva can be prevented. Awareness of cancer vulva is deficient among women and healthcare providers. In this study we looked for a correlation between the most significant vulval symptoms to be associated with cancer vulva in order to educate women and to provide guidance for the health care providers. </span><b><span style="font-family:Verdana;">Methods:</span></b><span style="font-family:Verdana;"> 569 women were enrolled in this observational cohort study. The patients were stratified according to their symptoms </span><span style="font-family:Verdana;">into 5 groups. However, fractures of the penis are secondary to a direct trauma to the penis with rupture of the albuginea causing hematoma, pain and deviation of the penis. Animal bites are rare. <strong>Purpose:</strong> To describe the management of traumatic emergencies of the external genital organs at the CHU Bouaké. <strong>Patients and Methods:</strong> Retrospective and descriptive study on the patients treated at Bouaké University Hospital for traumatic emergencies of the external genital organs during the period from January 01, 2012 to December 31, 2018. The parameters studied were epidemiological, clinical and therapeutic. <strong>Results:</strong> The study period registered 26 patients. The mean age was 34.5 years, Closed trauma to the scrotum was a frequent reason with 30.8%, the trauma sat on the scrotum in 53.8%, the penis in 42.3%, the vulva in 3.8% of cases. The circumstances of discovery of trauma to the penis were dominated by coitus missteps (45.5%), on the stock exchange and vulva by accidents on the public highway (60.8%). Scrotal swelling was the frequent clinical sign (26.9%). Treatment was dominated by surgery in 92.30%. Seven patients (29.16%) underwent exploratory scrototomy associated with hematoma evacuation, five patients (20.83%), a simple trimming, five patients (20.83%), an evacuation of the hematoma associated with a cavernous suture, one patient (4.16%), a trimming associated with a suture of the vulva, one patient (4.16%), a trimming associated with a right orchiectomy. The hospital stay was less than 5 days. The course was simple in 92.30% of the cases;two patients (7.7%) had complications such as testicular necrosis and atrophy which have been treated by orchidectomy. <strong>Conclusion:</strong> Traumatic emergencies of the external genitalia are rare but serious.
