BACKGROUND Hepatocellular carcinoma(HCC)in segments VII and VIII poses technical challenges for both liver resection and radiofrequency ablation(RFA).Robotic-assisted techniques may enhance safety and precision,but co...BACKGROUND Hepatocellular carcinoma(HCC)in segments VII and VIII poses technical challenges for both liver resection and radiofrequency ablation(RFA).Robotic-assisted techniques may enhance safety and precision,but comparative evidence remains limited.AIM To compare the clinical outcomes of robotic liver resection(R-LR)and robotic intraoperative RFA(RIO-RFA)for HCC located in liver segments VII and VIII.METHODS We retrospectively analyzed 93 HCC patients in segments VII/VIII with de novo(n=57)or first recurrent(n=36).HCC who underwent R-LR or RIO-RFA between 2015 and 2024.Propensity score matching was performed to reduce selection bias.Primary outcomes were overall survival(OS)and recurrence-free survival(RFS).Kaplan-Meier curves,log-rank tests,and Cox regression were used to identify prognostic factors for OS and RFS.RESULTS In the de novo group,OS and RFS did not differ significantly between R-LR and RIO-RFA before or after propensity score matching.In contrast,the recurrent group showed significantly improved OS and RFS with R-LR(P=0.005 and P=0.012,respectively).Subgroup analyses revealed that low-risk de novo patients with smaller tumors achieved superior OS after R-LR,whereas carefully selected low-risk recurrent patients undergoing RIO-RFA(smaller tumors,absence of complications)achieved outcomes comparable to R-LR.Platelet count,tumor size,and postoperative complications constituted key prognostic factors.CONCLUSION For HCC in challenging liver segments VII and VIII,R-LR and RIO-RFA achieve comparable outcomes in de novo cases,whereas R-LR confers superior survival in recurrent disease.R-LR should be prioritized for small de novo HCCs and for recurrent disease overall;RIO-RFA may serve as an effective alternative in carefully selected lowrisk recurrent patients.Tumor size,platelet count,and postoperative complications are key prognostic indicators to guide individualized treatment.展开更多
BACKGROUND Laparoscopic hepatectomy is a proven safe and technically feasible approach for liver tumor resection,but laparoscopic anatomical SVIII resection(LASVIIIR)remains rarely reported due to poor accessibility,d...BACKGROUND Laparoscopic hepatectomy is a proven safe and technically feasible approach for liver tumor resection,but laparoscopic anatomical SVIII resection(LASVIIIR)remains rarely reported due to poor accessibility,difficult exposure,and the deep-lying Glissonean pedicle.This study examined the safety,feasibility,and perio-perative outcomes of LASVIIIR via a middle hepatic fissure approach at our in-stitution.AIM To investigate the safety,feasibility,and perioperative outcomes of LASVIIIR via a middle hepatic fissure approach at our institution.METHODS From November 2017 to December 2022,all patients with a liver tumor who underwent LASVIIIR were enrolled.The perioperative outcomes and postope-rative complications were evaluated.RESULTS Thirty-four patients underwent LASVIIIR via a middle hepatic fissure approach from the side or cranio side and were included.The mean operation time was 164±54 minutes,and the intra-operative blood loss was 100 mL(range:20-1000 mL).The mean operative times were,respectively,152±50 minutes and 222±29 minutes(P=0.001)for the caudal side and cranial side approaches.In addition,the median blood loss volumes were 100 mL(range:20-300 mL)and 250 mL(range:20-1000 mL),respectively,for the caudal and cranial sides(P=0.064).Three patients treated using the cranial side approach experienced bile leakage,while 1 patient treated using the caudal side approach had subphrenic collection and underwent percutaneous drainage to successfully recover.There were no differences regarding postoperative hospital stays for the caudal and cranial side approaches[9(7-26)days vs 8(8-19)days](P=0.226).CONCLUSION LASVIIIR resection remains a challenging operation,but the middle hepatic fissure approach is a reasonable and easy-to-implement technique.展开更多

The new synthesized metal chelates are investigated by elemental analysis, IR, mass spectra, thermal analysis and biological activity. Square planer structure of the prepared complexes obtained from the result of analysis. The antibacterial and antifungal of sodium barbitone ligand and its conforming metal chelates were screened against bacterial species Gram positive (Staphylococcus aureus), Gram negative bacteria (Escherichia coli) and fungi Aspergillus flavus and Candida albicans fungi. Ampicillin and amphotericin were used as references for antibacterial and antifungal studies. At room temperature the carrier density, n, is 3.5-5 × 10^19 cm^-3 for all the s...Single-crystal samples of type-VIII BasGa16-xCuxSn30 (x = 0, 0.03, 0.06, 0.15) clathrates were prepared using the Sn-flux method. At room temperature the carrier density, n, is 3.5-5 × 10^19 cm^-3 for all the samples, the carrier mobility, μH, increases to more than twice that of BasGa16Sn30 for all the Cu doping samples, and consequently the electrical conductivity is enhanced distinctly from 1.90×10^4 S/m to 4.40 ×10^4 S/m, with the Cu composition increasing from x = 0 to x = 0.15. The Seebeck coefficient,α, decreases slightly with the increases in Cu composition. The values are about 0.72 W/mK at 300 K and are almost invariant with temperature up to 500 K for the samples with x = 0 and x = 0.03. The lattice thermal conductivity, μL, decreases from 0.59 W/InK for x = 0 to 0.50 W/mK for x = 0.03 at 300 K. The figure of merit for x = 0.03 reaches 1.35 at 540 K.展开更多

They are related to diseases like cardiovascular diseases, cerebral thromboembolism. ABO blood group system may influence coagulation factor VIII which may increase the future risk of thrombosis. <strong>Aim:</strong> To assess the relation of ABO blood group with coagulation factor VIII in healthy adults.<strong> Material and Methods: </strong>A prospective type of analytical cross-sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka from July 2019 to June 2020. After obtaining ethical clearance, a total of 190 healthy adults were selected from different areas of Dhaka city based on inclusion and exclusion criteria, with ages ranging from 18 - 45 years. The subjects were interviewed and detailed history regarding personal, family, medical and drug were taken. Prior to sample collection, informed written consent was taken from the participants. Individuals of blood group A were selected as group A, blood group B as group B, blood group AB as group AB and blood group O as group O. Coagulation factor VIII was measured in the Department of Hematology and BMT Unit, Dhaka Medical College Hospital, Dhaka. Blood grouping was done in the Department of Physiology, Dhaka Medical College, Dhaka. <strong>Statistical Analysis:</strong> For statistical analysis, ONE way ANOVA followed by Bonferroni test were considered using SPSS 25.0 version. <strong>Results: </strong>In this study, blood group B was most common (33.2%). Coagulation factor VIII was significantly higher (p < 0.001) in blood group A (105.76% ± 11.82%), B (112.00% ± 15.02%), AB (109.80% ± 11.93%) than blood group O (82.00% ± 12.86%). No significant difference was observed among A, B and AB blood groups regarding coagulation factor VIII. <strong>Conclusions:</strong> It can be concluded that blood group A, B, AB individuals may have more chance of thrombosis due to significantly higher coagulation factor VIII than blood group O individuals.展开更多

AIM:To observe the expression of vascula r endothelial growth factor(VEGF)and Factor VIII related antigen(FVIII-R Ag )in rats brain tissue in the bor-der zone of the hematoma,and explore the relationship between the e...AIM:To observe the expression of vascula r endothelial growth factor(VEGF)and Factor VIII related antigen(FVIII-R Ag )in rats brain tissue in the bor-der zone of the hematoma,and explore the relationship between the expres-sion of VEGF and the brain angionesis after intracerebral hmorrhage(ICH).METHODS:Rat of model of ICH was induced by inje ction of collagenase physiological saline into the right caudate nucleus.The immunohistoch emical methods were performed by using diff erent antisera of VEGF and FVIII-R Ag in serial sections of rats brain at 12h,1d,3d and 7d after ICH respec-tively.RESULTS:T he amount of VEGF positive cells was markedly in-creased,and reached the highest at 7th day in the border zone after ICG(P<0.01),but there were no significant diffe rence among the mean value of A of cells at different time points(P>0.05).Compared with the controls,both the FVIII-R Ag positive endothelial cells and the A of cells were signific antly different in experimental ICH goups after 1day.CONCLUSION:The up-regulated expression of VEGF might i nduce proliferation of endothelial cell and angiogenesis in the border zone a fter ICH in rats.展开更多
