Anisotropy of Trabecular Bone from Ultra-Distal Radius Digital X-Ray Imaging: Effects on Bone Mineral Density and Age

Background: When applied to trabecular bone X-ray images, the anisotropic properties of trabeculae located at ultra-distal radius were investigated by using the trabecular bone scores (TBS) calculated along directions parallel and perpendicular to the forearm. Methodology: Data from more than two hundred subjects were studied retrospectively. A DXA (GE Lunar Prodigy) scan of the forearm was performed on each subject to measure the bone mineral density (BMD) value at the location of ultra-distal radius, and an X-ray digital image of the same forearm was taken on the same day. The values of trabecular bone score along the direction perpendicular to the forearm, TBS<sub>x</sub>, and along the direction parallel to the forearm, TBS<sub>y</sub>, were calculated respectively. The statistics of TBS<sub>x</sub> and TBS<sub>y</sub> were calculated, and the anisotropy of the trabecular bone, which was defined as the ratio of TBS<sub>y</sub> to TBS<sub>x</sub> and changed with subjects’ BMD and age, was reported and analyzed. Results: The results show that the correlation coefficient between TBS<sub>x</sub> and TBS<sub>y</sub> was 0.72 (p BMD and age was reported. The results showed that decreased trabecular bone anisotropy was associated with deceased BMD and increased age in the subject group. Background: When applied to trabecular bone X-ray images, a method for analyzing trabecular bone texture based on the initial slope of variogram (ISV) was used to assess the trabecular bone health. Methodology: Data from more than two hundred subjects were retrospectively studied. For each subject, a DXA (GE Lunar Prodigy) scan of the forearm was performed, and bone mineral density (BMD) value was measured at the location of ultra-distal radius, X-ray digital image of the same forearm was taken on the same day, and ISV value over the same location of ultra-distal radius was calculated. Pearson’s correlation coefficients were calculated to examine the correlation between BMD and ISV of the trabecular bones located at the same ultra-distal radius. ISV values changed with subjects’ age were also reported. Results: The results show that ISV value was highly correlated with the DXA-measured BMD of the same trabecular bone located at the ultra-distal radius. The correlation coefficient between ISV and BMD with the 95% confident was 0.79 ± 0.09. They also demonstrated that the age-related changes in trabecular bone health and differentiated age patterns in males and females, respectively. The results showed that the decrease in BMD was accompanied by a decrease in the initial slope of variogram (ISV). Conclusions: This study suggests that ISV might be used to quantitatively evaluate trabecular health for osteoporosis and bone disease diagnosis. 展开更多

胫骨逆行髓内钉结合阻挡螺钉治疗超胫骨远端骨折的疗效分析

目的探讨胫骨逆行髓内钉(retrograde intramedullary tibial nails,RTN)结合阻挡螺钉治疗超胫骨远端骨折的临床疗效。方法回顾性分析2021年1月至2024年6月在第九〇九医院骨科治疗的17例超胫骨远端骨折病人的临床资料,均采用闭合复位、... 目的探讨胫骨逆行髓内钉(retrograde intramedullary tibial nails,RTN)结合阻挡螺钉治疗超胫骨远端骨折的临床疗效。方法回顾性分析2021年1月至2024年6月在第九〇九医院骨科治疗的17例超胫骨远端骨折病人的临床资料,均采用闭合复位、不扩髓的RTN结合阻挡螺钉固定。记录病人骨折愈合时间、完全负重活动时间和骨折复位、并发症情况。末次随访时采用美国足踝外科医师协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足功能评分评估功能恢复情况。结果所有病人术后均获随访,随访时间为(16.8±7.1)个月(12~25个月),骨折愈合时间为(4.5±1.2)个月(3~7个月),术后完全负重活动时间为(68.5±10.1)天(58~82天),末次随访时AOFAS评分为(83.1±9.3)分(72~93分)。在X线片上测量骨折在冠状面、矢状面平均成角均<5°,无阻挡螺钉及RTN变形、断裂等情况。结论通过缩小胫骨远端髓腔直径,阻挡螺钉可实现辅助RTN复位超胫骨远端骨折、提高固定稳定性的效果,具有良好的临床疗效。 展开更多

Total elbow arthroplasty (TEA) is one of the treatment options for these fractures. While elbow motion is largely regained by TEA, comminuted condyle fragments are often ignored. Although numerous approaches for repair of condylar fragments around TEA are described, any universal fixation strategy for these fractures has not been established. This report describes, for the first time, application of an ultra-high molecular weight polyethylene (UHMWPE) tape for the treatment of distal humerus fracture in 2 patients with rheumatic elbow arthropathy. The post-operative clinical courses were good. Radiographs showed bony union of the condylar fragments without loosening in two cases. Because of its flat configuration, softness, and flexibility, UHMWPE tape is a promising material for stabilizing fracture of the distal humerus associated with TEA. 展开更多

目的 :探讨单侧简易外固定器超踝外固定治疗胫腓骨远端骨折的价值。方法 :自 1997年 10月以来 ,我院根据AO长管状骨骨折的综合分类 ,应用此外固定器超踝外固定治疗胫腓骨远端骨折 46例 ,对治疗结果进行了分析。结果 :经 6~ 12个月的随... 目的 :探讨单侧简易外固定器超踝外固定治疗胫腓骨远端骨折的价值。方法 :自 1997年 10月以来 ,我院根据AO长管状骨骨折的综合分类 ,应用此外固定器超踝外固定治疗胫腓骨远端骨折 46例 ,对治疗结果进行了分析。结果 :经 6~ 12个月的随访 ,优良率达 96% ,伤肢功能良好。结论 :采用单侧简易外固定器既可维持患肢长度又有牵伸延长断端的性能 ,对胫腓骨远端骨折AO综合分类每型的第 2、3组严重骨折行超踝外固定治疗 ,符合“关节韧带牵伸术”协助骨折复位、维持固定的原理 ,大大减少了骨外露、骨髓炎、骨坏死、皮肤坏死等并发症的发生。 展开更多

目的探讨利用超高分辨率CT(U-HRCT)微动脉造影技术获得股骨远端骨内与骨膜动脉三维(3D)模型的可行性并观察动脉分布。方法自2022年1月至2025年4月,中国人民解放军联勤保障部队第九八八医院骨科研究组在北京大学基础医学院人体解剖与组... 目的探讨利用超高分辨率CT(U-HRCT)微动脉造影技术获得股骨远端骨内与骨膜动脉三维(3D)模型的可行性并观察动脉分布。方法自2022年1月至2025年4月,中国人民解放军联勤保障部队第九八八医院骨科研究组在北京大学基础医学院人体解剖与组织胚胎学系对4只新鲜冰冻下肢标本进行研究,将红色氧化铅粉末研磨并用400目筛网过滤后,将20 g粉末与松节油按照1 g∶1.5 ml、1 g∶1.0 ml、1 g∶0.5 ml比例制成3种浓度的铅基造影剂混悬液。将标本于室温下解冻后,先用松节油经过股动脉冲洗血管,再从低浓度到高浓度依次灌注造影剂。灌注标本经福尔马林固定2周后取材于股骨远端,注意保留骨膜,然后于股骨髁上截骨。标本使用U-HRCT分段扫描,扫描分辨率为50μm,再用Mimics Medical软件依次重建分段模型后,再拟合形成股骨远端骨内动脉及骨膜动脉的三维模型,在软件中可观察分析股骨远端骨膜和骨内动脉分布。结果本方法可成功构建股骨远端骨内与骨膜动脉的3D模型。股骨远端内侧面骨膜动脉丰富,恒定可见两级动脉弓,外侧面的骨膜动脉较内侧面少,但关节面下动脉弓显影有间断或发育不良。股骨内侧髁血供主要来自内侧面骨膜动脉发出的骨内动脉。而股骨外侧髁血供来自外侧面和髁间窝后侧进入的骨内动脉,两者形成较多吻合。软骨下骨区域的血供主要来自于两侧骨膜关节面下动脉弓发出的骨内动脉向软骨下骨发出的分支,但外侧髁部分区域由髁间窝后侧的骨内动脉供血。结论U-HRCT微动脉造影分段扫描,可对较大尺寸骨样本的骨膜和骨内动脉进行有效显影,从而重建骨组织和动脉的3D模型并进行观察分析。股骨远端骨膜与骨内动脉分布可以为股骨内侧髁骨瓣、骨软骨瓣、创伤预后评估等提供依据。 展开更多

腹股沟下近端旁路和远端旁路手术在CLTI患者中的应用及疗效的对比研究

目的探讨旁路手术在慢性肢体威胁性缺血(CLTI)患者中的应用效果,评估在腔内手术失败或不适合行腔内手术的复杂病变情况下行腹股沟下近端旁路和远端旁路手术的疗效和预后。方法本研究为回顾性队列研究。分析复旦大学附属中山医院2021年7... 目的探讨旁路手术在慢性肢体威胁性缺血(CLTI)患者中的应用效果,评估在腔内手术失败或不适合行腔内手术的复杂病变情况下行腹股沟下近端旁路和远端旁路手术的疗效和预后。方法本研究为回顾性队列研究。分析复旦大学附属中山医院2021年7月至2024年6月收治的55例(57条肢体)行腹股沟以下近端旁路、远端旁路及超远端旁路手术的患者临床资料,其中男性49例,女性6例;年龄(62.56±12.90)岁;Rutherford分级均为3级及以上。根据既往是否接受过血流重建手术,分为腔内手术失效肢体组(n=36)和原发病变组(n=21)。根据病变血管位置分为近端旁路手术组(n=21)和远端(含超远端)旁路手术组(n=36)。评估血管旁路手术的应用情况及其疗效,以及在原发病变或腔内手术失效病变中的应用表现。采用独立样本t检验和χ^(2)检验比较两组的一般资料及手术资料。采用Kaplan-Meier绘制生存曲线,组间比较采用Log-rank检验。结果本研究中,共57条肢体接受腹股沟以下旁路手术。包括近端旁路21条肢体(近端旁路组),远端旁路36条(包括远端旁路29条,超远端旁路7条;远端旁路组)。57条肢体中,48条(84.21%)使用自体静脉移植物,其中近端旁路、远端旁路及超远端旁路使用自体静脉移植物的肢体比例依次为76.19%(16/21)、86.21%(25/29)、100%(7/7)。相较于近端旁路手术[(230.34±94.73)min],远端旁路手术[(378.90±54.54)min,t=6.32,P<0.01]及超远端旁路手术[(384.14±49.01)min,t=4.05,P<0.01]的手术时间更长。原发病变组21条肢体中,17条(80.95%)肢体行远端及超远端旁路手术;腔内失效病变组36条肢体中,19条(52.78%)行远端及超远端旁路手术,两组比较差异具有统计学意义(χ^(2)=4.52,P=0.03)。随访时间为(35.28±30.79)周(范围:3~133周)。近端旁路组与远端(含超远端)旁路组术后2年一期通畅率(63.61%比85.26%,Log-rankχ^(2)=3.05,P=0.08)、二期通畅率(85.71%比88.44%,Log-rankχ^(2)=0.91,P=0.34)及无截肢生存率(90.48%比95.00%,Log-rankχ^(2)=0.79,P=0.37)比较,差异均无统计学意义。原发病变组和腔内失效病变组旁路术后2年期一期通畅率(81.43%比70.08%,Log-rankχ^(2)=0.09,P=0.76)、二期通畅率(87.91%比84.58%,Log-rankχ^(2)=0.01,P=0.92)及无截肢生存率(87.30%比96.67%,Log-rankχ^(2)=1.38,P=0.24)比较,差异均无统计学意义。结论旁路手术是救治CLTI安全有效的外科术式,特别对于腔内手术失效或不适用的情况下,旁路手术具有不可替代的保肢效果。其中,远端或超远端旁路手术可以获得与近端旁路手术一致的疗效,在有特定需要的患者中,应被积极地考虑作为重要的保肢手段。 展开更多
