Ménétrier disease(MD)is a rare gastric disorder characterized by hypertrophy of the gastric mucosa,resulting in giant gastric folds,excessive mucus secretion,and significant protein loss.It is most common in...Ménétrier disease(MD)is a rare gastric disorder characterized by hypertrophy of the gastric mucosa,resulting in giant gastric folds,excessive mucus secretion,and significant protein loss.It is most common in middle-aged males,although cases have also been reported in children.This condition,also known as hyperplastic hypersecretory gastropathy,primarily affects the fundus and body of the stomach,leading to protein-losing gastropathy due to increased mucosal permeability.The exact pathogenesis of MD remains unclear;however,it has been linked to excessive transforming growth factor-alpha signaling via the epidermal growth factor receptor(EGFR),which promotes mucosal hyperplasia.Clinical manifestations include epigastric pain,nausea,vomiting,anorexia,weight loss,and peripheral edema due to protein-losing enteropathy.Diagnosis is based on clinical presentation,endoscopic findings,and histopathology revealing foveolar hyperplasia and glandular atrophy.Treatment options vary from symptomatic management with proton pump inhibitors and nutritional support to monoclonal antibodies targeting EGFR(e.g.,cetuximab)in severe cases.In refractory situations,gastrectomy may be required.This review highlights the current understanding,diagnostic approaches,and therapeutic strategies for MD.展开更多
BACKGROUND Adult-onset Ménétrier’s disease is strongly associated with Helicobacter pylori(H.pylori)infection and an elevated risk of carcinogenesis.Cases of early-stage gastric cancer developed in H.pylori...BACKGROUND Adult-onset Ménétrier’s disease is strongly associated with Helicobacter pylori(H.pylori)infection and an elevated risk of carcinogenesis.Cases of early-stage gastric cancer developed in H.pylori-negative Ménétrier’s disease are extremely rare.We report a case of early gastric cancer in H.pylori-negative Ménétrier’s disease that was curatively resected with endoscopic submucosal dissection(ESD).CASE SUMMARY A 60-year-old woman was referred to our hospital after her medical examination detected anemia.Contrast-enhanced upper gastrointestinal(UGI)radiography revealed translucency of the nodule-aggregating surface with giant rugae.Blood tests showed hypoproteinemia and were negative for serum H.pylori immunoglobulin G antibodies.The 99mTc-DTPA-human serum albumin scintigraphy showed protein loss from the stomach.UGI endoscopy showed a 40-mm protruding erythematous lesion on giant rugae of the greater curvature of lower gastric body,suggesting early-stage gastric cancer due to Ménétrier’s disease.En bloc resection with ESD was performed for diagnosis and treatment.Histology of ESD showed well-differentiated tubular adenocarcinoma.The cancer was confined to the mucosa,and complete curative resection was achieved.Foveolar hyperplasia and atrophy of the gastric glands were observed in non-tumor areas,histologically corresponding to Ménétrier’s disease.Three years after ESD,gastric cancer had not recurred,and Ménétrier’s disease remained in remission with spontaneous regression of giant gastric rugae.CONCLUSION Complete curative resection was achieved through ESD in a patient with earlystage gastric cancer and H.pylori-negative Ménétrier’s disease.展开更多
Objective:Research on the biological mechanisms by which the liver controls dispersion and regulates emotion usually focuses on psychological stress.This study investigated the biological mechanisms underlying the liv...Objective:Research on the biological mechanisms by which the liver controls dispersion and regulates emotion usually focuses on psychological stress.This study investigated the biological mechanisms underlying the liver's regulation of emotion using the Trier Social Stress Test (TSST).Differences in subjective stress,subjective emotion,and physiological responses were examined between healthy women (Group A) and depressed women with liver qi stagnation syndrome (Group B).Method:Subjects were 72 women who met the inclusion criteria after physical examination.The EPQ-RSC,BAI,and BDI-21 were used to assess personality and emotion.Subjects completed a 5-point scale to measure subjective stress.Heart rate and heart rate variability were measured.To measure changes in activity of the locus coeruleus-norepinephrine (LCNE)/autonomic nervous system and the HPA axis,saliva samples were collected at the end of the rest and preparation periods,and 5 time pionts after the TSST task.The PAD Emotion Scale was completed at 4 time points before and after the TSST task.Saliva cortisol was tested.These indexes were compared between the two groups.Results:There were 33 subjects in Group A and 39 subjects in Group B.Subjects in Group B had lower levels of stress,pleasure,and dominance,but higher arousal (after the task:P <.05).They also had higher saliva cortisol levels,slower heart rate,higher BVP,and more stable RMSSD and SDNN.Conclusion:In relation to mood,the liver's regulation of emotion mainly affects negative emotional experience and the sense of control Regarding the biological mechanisms underlying the liver's regulation of emotion,the activity of the LC-NE/autonomic nervous system in women is more obvious in response to psychological stress when the dispersion of the liver is normal.The activity of the HPA axis in women is more obvious in response to psychological stress when the dispersion of the liver is abnormal.展开更多

However he actually reproduces Christianity's perspective that women are sinful by their dispositions. According to the classical creation story, Eve falls into temptation and then, convinces Adam to commit the same crime. All Abrahamic religions but especially Christianity identifies women with the body and bodily desires, rather than the mind which comes from nonmaterial and divine substance. Accordingly, women can easily be deceived by Satan since they contain a lesser amount of mental substance compared to men. In Antichrist, Trier reiterates the story of Adam and Eve in a different context. The woman writes her dissertation on women who are burnt with the accusation of being a witch. But she realizes that her body is sinful and deserving of punishment, too. She finds her salvation when she cuts her clitoris. In Nymphomaniac, the main character gets her clitoris whipped. While she resists traditional and bourgeois morality, she defines herself as morally evil because of her unmanageable sexual desires. Thus, reputed as critical of all social structures and morality, Trier in fact simply rehashes the classical sexist and misogynist perspective.展开更多
