Inflammation-mediated carcinogenesis develops in the context of chronic inflammation and is a significant cause of cancer within the digestive system.In the chronic inflammation microenvironment,the metabolic activity of tissue cells undergoes extensive changes,which interfere with the normal function of immune cells.Dysregulation of cell metabolism and immune function has been identified as a key factor contributing to inflammation-mediated carcinogenesis within the major digestive organs,such as the stomach,liver,and colorectum.This metabolic-immune imbalance also corresponds to traditional Chinese medicine(TCM)theories of“yin-yang disharmony”and“disharmony between Ying-nutrients and Wei-defense.”The metabolic-immune imbalance has also been regarded as the key factor supporting“treatment of different diseases with the same method”,in which the same approach is adopted in the treatment of different conditions.In the TCM treatment process,it is necessary to first identify TCM patterns and then apply the corresponding TCM to correct the dysregulated metabolic and immune function,thereby blocking the progression from inflammation to malignancy.Our study findings deepen the TCM understanding of metabolic-immune dysregulation and the relationship between metabolic-immune dysregulation,pattern identification,and treatment method.They also provide new insights for the treatment of inflammation-mediated carcinogenesis in major digestive organs and help us further explore the scientific connotation of the TCM strategy of“treating different diseases with the same method”. 展开更多

Objective:This study aims to explore the benefit analysis of the same disease in different departments of public hospitals under the DIP payment method.Methods:This study is a retrospective analysis that selected clinical data from patients who received treatment in the Department of Orthopedics and the Department of Acupuncture and Moxibustion at our hospital from January 1,2023,to December 31,2023.The study compared the costs of medications,examinations,treatments,laboratory tests,nursing and other expenses,and total treatment costs between the two departments.It analyzed the cost structure of the two departments and proposed further improvement suggestions.Results:The study results indicated that the total costs in the Department of Acupuncture and Moxibustion were significantly higher than those in the Department of Orthopedics.Among medication costs,the total medication costs in the Department of Orthopedics were higher than those in the Department of Acupuncture and Moxibustion,with costs for Western medicine,proprietary Chinese medicine,and herbal medicine all being higher(p<0.05).Regarding examination costs,consultation fees in the Department of Orthopedics were lower than those in the Department of Acupuncture and Moxibustion,while examination costs were higher(p<0.05).In terms of treatment costs,orthopedic treatment and surgical fees were higher than those in the Department of Acupuncture and Moxibustion(p<0.05).For laboratory test costs,orthopedic laboratory fees were significantly higher than those in the Department of Acupuncture and Moxibustion(p<0.05).Among nursing and other expenses,orthopedic blood transfusion,bed fees,and other expenses were higher than those in the Department of Acupuncture and Moxibustion,while nursing fees were lower(p<0.05).Conclusion:Treatment fees in the Department of Acupuncture and Moxibustion are the core and account for a relatively high proportion of the total costs.The benefits generated by the Department of Orthopedics are primarily derived from medication,examination,and laboratory fees,aligning with the characteristics of combining diagnosis,medication,and surgical intervention in orthopedic treatment.Consultation fees,nursing fees,and bed fees in the Department of Acupuncture and Moxibustion are higher than those in the Department of Orthopedics,indicating a longer treatment cycle in acupuncture,which warrants clinical attention. 展开更多

In 2022,Leukemia is the 13th most common diagnosis of cancer globally as per the source of the International Agency for Research on Cancer(IARC).Leukemia is still a threat and challenge for all regions because of 46.6%infection in Asia,and 22.1%and 14.7%infection rates in Europe and North America,respectively.To study the dynamics of Leukemia,the population of cells has been divided into three subpopulations of cells susceptible cells,infected cells,and immune cells.To investigate the memory effects and uncertainty in disease progression,leukemia modeling is developed using stochastic fractional delay differential equations(SFDDEs).The feasible properties of positivity,boundedness,and equilibria(i.e.,Leukemia Free Equilibrium(LFE)and Leukemia Present Equilibrium(LPE))of the model were studied rigorously.The local and global stabilities and sensitivity of the parameters around the equilibria under the assumption of reproduction numbers were investigated.To support the theoretical analysis of the model,the Grunwald Letnikov Nonstandard Finite Difference(GL-NSFD)method was used to simulate the results of each subpopulation with memory effect.Also,the positivity and boundedness of the proposed method were studied.Our results show how different methods can help control the cell population and give useful advice to decision-makers on ways to lower leukemia rates in communities. 展开更多

Comparative Observation of the Curative Effect of Different Treatment Methods in Patients with Pelvic Inflammation

Methods: through double-blind randomization, 78 cases of pelvic inflammatory disease were divided into two groups (group A = 39 cases) and (Group B = 39 cases) from January 2020 to March 2021. Group B was treated with conventional drugs, and group A was treated with drugs + microwave combination. After the relevant treatment, the clinical efficacy of the two groups of cases was compared. Compared with the levels of IL-2 and TNF-A, the microwave binding method in group A was more advantageous than that in group B. In addition, in terms of the levels of inflammatory factors, group A tended to be more stable than group B, with significant differences in p-value data. Conclusion: targeted treatment of pelvic inflammatory disease, through drugs and microwave method to carry out comprehensive treatment, can promote the organic optimization of inflammatory level, and constantly improve the clinical efficacy, should be vigorously promoted in clinical. 展开更多

Objective:To explore the common mechanism of Huanglian Jiedu Decoction in treating coronary heart disease and type 2 diabetes by network pharmacology.Methods:All chemical components and targets of the four drugs in Huanglian Jiedu Decoction were retrieved through TCMSP,and the genes were standardized through Uniprot database.Acquire disease targets related to coronary heart disease and diabetes in OMIM and GeneCards databases.The network diagram of"drug-component-target-disease"is constructed by using the software of cytopscape 3.7.2,the PPI network diagram of protein interaction is constructed by using STRING database,and the network diagram of"drug-disease"core target is constructed by using the software of cytopscape 3.7.2.DAVID's online database platform was used to analyze GO biological process and KEGG pathway enrichment of common targets of Huanglian Jiedu Decoction in treating coronary heart disease and type 2 diabetes.Results:103 active ingredients of Huanglian Jiedu Decoction were retrieved,including 140 acting targets,5342 coronary heart disease targets,114 diabetes targets,and 14 common intersection targets of drugs and diseases,involving AR,PPARG,TNF,IL6,CCL2,VEGFA,PON1,etc.The GO biological process analysis results in 98 biological processes,10 cell components and 10 molecular functions.Among them are positive regulation of gene expression,positive regulation of nitric oxide biosynthesis process,Extracellular space,cytokine activity,steroid hormone receptor activity and other biological processes;The enrichment analysis of KEGG pathway yielded 20 signal pathways(P≤0.05).It mainly involves Malaria,cancer in cancer,HIF-1 signaling pathway,TNF signaling pathway,NOD-like receptor signaling pathway,PI3K-Akt signaling pathway,etc.Conclusion:Huanglian Jiedu Decoction"treats different diseases at the same time"coronary heart disease and type 2 diabetes have the characteristics of multiple components,multiple targets and multiple pathways,which provide theoretical basis for Huanglian Jiedu Decoction to treat coronary heart disease and type 2 diabetes in clinic,but the key targets and pathways of Huanglian Jiedu Decoction to treat diseases still need further experimental verification. 展开更多

Triple-negative breast cancer(TNBC)is marked by its high degree of malignancy and challenging treatment outcomes.While Western medicine predominantly relies on chemotherapy,issues such as drug resistance and toxic side effects remain prevalent.Drawing on the traditional Chinese medicine(TCM)principle of“treating the same disease with different methods,”Bian devises individualized therapeutic strategies tailored to the unique syndrome patterns of TNBC patients,adhering to the core tenets of“treatment based on syndrome differentiation”and“simultaneous regulation of the liver and spleen.”Through an analysis of clinical cases involving TNBC patients,this paper summarizes Bian’s clinical experience,which emphasizes foundational approaches such as strengthening the body’s resistance and soothing the liver to regulate qi,combined with personalized therapies including clearing heat and draining dampness,pacifying the liver and subduing yang,and resolving phlegm to unblock collaterals.The study highlights a diagnostic model of“multiple prescriptions for one disease,tailored to individual conditions,”providing valuable insights for the personalized TCM treatment of TNBC. 展开更多

表面处理是提高混凝土抗氯离子侵入阻力的重要措施。深入研究表面处理混凝土的传输特性,对于表面处理在实际工程中的应用与推广尤为重要。采用试验与数值模拟相结合的方法,探究了表面处理含盐混凝土的氯离子扩散特性。针对普通混凝土、... 表面处理是提高混凝土抗氯离子侵入阻力的重要措施。深入研究表面处理混凝土的传输特性,对于表面处理在实际工程中的应用与推广尤为重要。采用试验与数值模拟相结合的方法,探究了表面处理含盐混凝土的氯离子扩散特性。针对普通混凝土、表面涂层混凝土和表面复合涂层混凝土,利用NEL法测定了表面处理层和基底混凝土的氯离子扩散系数,并简要分析了表面涂层类型和水胶比对氯离子扩散系数的影响。将3种混凝土试件分别在盐溶液中浸泡45,90,135 d后,测定其氯离子分布,进而分析表面涂层类型、水胶比和初始含盐量对氯离子质量分数的影响。在此基础上,提出了考虑表面处理层性能退化的混凝土氯离子质量分数计算的差分法。与实测数据对比验证结果表明:该有限差分法具有较高的计算精度,可以应用于表面处理混凝土结构的使用寿命估算。 展开更多

基于UPLC-MS与网络药理学探究参芪补气颗粒治疗气虚证“异病同治”的潜在药效物质

目的通过参芪补气颗粒成分分析和网络药理学及分子对接技术探究参芪补气颗粒“异病同治”气虚型慢性萎缩性胃炎、肾小球肾炎、阻塞性肺病、肝硬化和心力衰竭的物质基础和分子机制。方法以耐缺氧和抗疲劳作为改善气虚证的评价标准,进行常... 目的通过参芪补气颗粒成分分析和网络药理学及分子对接技术探究参芪补气颗粒“异病同治”气虚型慢性萎缩性胃炎、肾小球肾炎、阻塞性肺病、肝硬化和心力衰竭的物质基础和分子机制。方法以耐缺氧和抗疲劳作为改善气虚证的评价标准,进行常压/低张性缺氧和负重游泳实验,通过比较各组小鼠的耐缺氧和负重游泳时间,评价参芪补气颗粒对气虚证的药效作用。采用UPLC-Q/TOF-MS技术系统表征与鉴定其化学成分,构建复方“化学成分-异病同治靶点”网络,经拓扑参数分析鉴定关键活性成分与核心靶点;最终采用AutoDock Tools完成活性成分和靶标分子对接验证。结果药效实验表明,与正常对照组比较,给药组小鼠常压缺氧条件下的存活时间及负重游泳时间均延长,表明参芪补气颗粒具有明显的耐缺氧及抗疲劳作用。参芪补气颗粒鉴定出共有成分44个,包括皂苷、黄酮、核苷等其他类成分。网络药理学获得参芪补气颗粒“异病同治”靶点72个,筛选出芒柄花苷、毛蕊异黄酮、人参皂苷Rg_(1)、黄芪紫檀烷苷等23个重要成分和AR、IL2、VEGFA、EGFR等关键靶点,这些靶点参与对细胞增殖、凋亡等生物过程的调控,涉及高级糖基化终末产物-受体(AGE-RAGE)、磷脂酰肌醇3激酶/蛋白激酶B(PI3K-Akt)、肿瘤坏死因子(TNF)、缺氧诱导因子-1α(HIF-1α)信号通路等。结论本研究初步阐释了参芪补气颗粒通过多成分、多靶点和多通路联合作用发挥治疗5种气虚型慢性疾病“异病同治”的作用,为其深入开发和应用提供了一定的基础。 展开更多

Objective:Mendelian randomization(MR)has been widely utilized for causal inference between diseases,and its implementation within the domain of traditional Chinese medicine(TCM)is considered feasible.Although previous clinical and epidemiological studies have demonstrated a close relationship between insomnia and depression,the inherent genetic factors underlying these associations are unclear.The aim of this study was to evaluate the causal relationship between depression and insomnia via bidirectional 2-sample MR and increase the understanding of the TCM theory of treating different diseases with the same method,particularly in the context of comorbid depression and insomnia.Methods:Genetic data related to depression and insomnia were extracted from published genome-wide association studies(GWAS)data sets.Single-nucleotide polymorphisms(SNPs)associated with depression and insomnia were used as instrumental variables to construct an“SNP-exposure-outcome”model.Bidirectional 2-sample MR analysis was conducted via inverse-variance weighted(IVW),weighted median,MR Egger regression,simple mode,and weighted mode methods.Furthermore,heterogeneity tests,pleiotropy analyses,and sensitivity analyses were performed.Results:The MR results revealed a causal relationship between depression and an increased risk of developing insomnia(IVW,OR=1.400,95%CI:1.246–1.573,P<0.001),and a causal relationship between insomnia and an increased risk of developing depression(IVW,OR=1.204,95%CI:1.144–1.266,P<0.001).Conclusions:There is a bidirectional causal relationship between depression and insomnia.These findings provide new theoretical support for the TCM approach of treating different diseases with the same method in the prevention and treatment of depression and insomnia and provide a scientific basis for the modernization of TCM. 展开更多

目的利用血浆代谢组学方法研究缺血性心脑血管病气虚血瘀证大鼠模型的发病机制,并阐明补阳还五汤治疗两种模型的干预作用。方法通过睡眠剥夺联合冠状动脉左前降支结扎法构建冠心病气虚血瘀证(CHD-QDBS)模型,微球致多发性脑梗联合睡眠剥... 目的利用血浆代谢组学方法研究缺血性心脑血管病气虚血瘀证大鼠模型的发病机制,并阐明补阳还五汤治疗两种模型的干预作用。方法通过睡眠剥夺联合冠状动脉左前降支结扎法构建冠心病气虚血瘀证(CHD-QDBS)模型,微球致多发性脑梗联合睡眠剥夺法构建脑梗气虚血瘀证(CI-QDBS)模型。每种模型分为假手术组、模型组、补阳还五汤给药组和阿司匹林组。补阳还五汤按临床剂量灌胃(CHD-QDBS:13 g/kg;CI-QDBS:21 g/kg),持续14 d。通过抓力测定、凝血功能、超声心动图及脑组织病理学观察对模型进行评价,并结合代谢组学分析筛选差异代谢物,探索CHD-QDBS与CI-QDBS的差异代谢物及代谢通路,分析两种疾病在气虚血瘀证下的共有及差异代谢机制。结果与模型组比较,补阳还五汤明显改善模型大鼠抓力(P<0.01)、凝血功能(P<0.05),并调控心功能(P<0.05)与脑组织病理形态。代谢组学分析显示,CHD-QDBS模型引发血浆76个差异代谢物明显异常,涉及甘油磷脂代谢、亚油酸代谢及鞘脂代谢紊乱;CI-QDBS模型则导致39个差异代谢物显著异常,以氨基酸代谢失调为主。补阳还五汤干预后,CHD-QDBS中37个差异代谢物水平恢复,主要调节脂质代谢通路;CI-QDBS中11个差异代谢物改善,主要涉及氨基酸代谢通路。两模型共同差异代谢物包括谷氨酸、焦谷氨酸等7种,提示氧化应激、能量代谢障碍、炎症反应及血管功能损伤是两疾病气虚血瘀证的共性特征。CHD-QDBS以脂质代谢紊乱驱动炎症与血栓形成,CI-QDBS则以氨基酸代谢失调加剧脑缺氧。结论本研究揭示缺血性心脑血管病气虚血瘀证存在共性代谢紊乱及特异通路,体现“异病同治”的中医理论科学性,为中医证候的生物学内涵解析提供了证据。补阳还五汤通过多靶点调控代谢网络、恢复脂质稳态并减轻氧化应激与炎症。本研究可为解析缺血性心脑血管病气虚血瘀证生物学基础及开发病证结合精准治疗策略提供依据。 展开更多

当归芍药散源自《金匮要略》,其应用范围已从“妇人腹中诸疾痛”拓展至消化、神经、泌尿等多系统疾病。近年研究表明其多靶点、多成分的调控特性在泌尿系统疾病的治疗中展现出显著潜力。该研究基于“临床疗效-药理研究-信号通路”模式... 当归芍药散源自《金匮要略》,其应用范围已从“妇人腹中诸疾痛”拓展至消化、神经、泌尿等多系统疾病。近年研究表明其多靶点、多成分的调控特性在泌尿系统疾病的治疗中展现出显著潜力。该研究基于“临床疗效-药理研究-信号通路”模式对当归芍药散治疗泌尿系统疾病进行多角度研究。药理学研究表明当归芍药散活性成分芍药苷、川芎嗪等可调控核因子κB(NF-κB)、PI3K/Akt/mTOR等信号通路,发挥抗炎、抗氧化及抗纤维化的协同作用;此外当归芍药散调节水通道蛋白2(AQP2)、抑制TGF-β/Smad通路及重塑肠道菌群-代谢轴实现多靶点干预。临床研究包括糖尿病肾病、肾病综合征、慢性肾小球肾炎及泌尿系结石等疾病,证实该方可显著降低尿蛋白水平、改善肾功能、缓解水肿。研究突破传统妇科应用框架,进一步推动当归芍药散在临床应用和现代研究的深入发展。 展开更多

目的 探讨并分析经皮肺穿刺组织宏基因组二代测序技术对不明原因肺部感染患者病原学诊断的临床价值。方法 收集2020年12月至2022年12月期间,在我院接受CT引导下经皮肺穿刺活检并结合m NGS检测的100例肺部感染患者的临床资料。患者根据... 目的 探讨并分析经皮肺穿刺组织宏基因组二代测序技术对不明原因肺部感染患者病原学诊断的临床价值。方法 收集2020年12月至2022年12月期间,在我院接受CT引导下经皮肺穿刺活检并结合m NGS检测的100例肺部感染患者的临床资料。患者根据是否患有基础性疾病被分为两组,对比两组间m NGS与传统检测方法在病原体检出阳性率及采信率等方面的差异,并分析不同类型病原体的m NGS检测阳性率。结果 在纳入的100例病例中,m NGS的阳性检测率为100%,显著高于常规检测方法的73%(P<0.05)。两组患者中,m NGS检测病原体的阳性率均显著高于常规检测(P<0.05)。此外,m NGS的临床采信率同样高于常规检测(P<0.05),而有基础疾病组与无基础疾病组间采信率无显著差异(P>0.05)。m NGS在检测各种病原体方面的效率均超过常规方法(P<0.05),特别是对真菌、结核分枝杆菌和混合感染的检出效率显著提升(P<0.001)。结论 对于不明原因的肺部感染,采用组织m NGS检测显示出较高的阳性检出率和采信率,显著增强了对多种病原体的检测能力,特别是对真菌、结核分枝杆菌及混合感染病原体的检测,具有显著的临床应用潜力。 展开更多

桃红四物汤治疗骨折与骨质疏松的“异病同治”机制研究——基于网络药理学与分子对接技术

目的:探讨桃红四物汤“异病同治”骨折与骨质疏松的作用机制,并通过分子对接技术验证其关键成分与靶点的结合能力。方法:基于TCMSP和DrugBank数据库筛选桃红四物汤中6味药材的活性成分及靶点,联合OMIM、DisGeNET与GeneCards数据库获取... 目的:探讨桃红四物汤“异病同治”骨折与骨质疏松的作用机制,并通过分子对接技术验证其关键成分与靶点的结合能力。方法:基于TCMSP和DrugBank数据库筛选桃红四物汤中6味药材的活性成分及靶点,联合OMIM、DisGeNET与GeneCards数据库获取骨折与骨质疏松相关靶点,取交集,获得潜在作用靶点,构建蛋白质–蛋白质相互作用网络及“药物–成分–靶点”网络,并进行拓扑分析,筛选核心成分与关键靶点。采用DAVID数据库进行基因本体(GO)功能富集分析和京都基因与基因组百科全书(KEGG)通路富集分析,最后通过AutoDockVina与PyMOL完成分子对接与可视化。结果:共筛得48个活性成分、208个药物靶点,骨折与骨质疏松交集靶点67个。富集分析显示其主要调控丝裂原活化蛋白激酶14(Mitogen–Activated Protein Kinase,MAPK)、磷脂酰肌醇3激酶(Phosphatidylinositol 3 Kinase,PI3K)–蛋白激酶B(Akt)、缺氧诱导因子–1(Hypoxia Inducible Factor–1,HIF–1)、肿瘤坏死因子(Tumor Necrosis Factor,TNF)等通路,涉及细胞凋亡、免疫及成骨调控等过程。拓扑分析识别出丝氨酸/苏氨酸蛋白激酶1(Akt Serine/Threonine Kinase 1,AKT1)、肿瘤蛋白p53(Tumor Protein p53,TP53)、表皮生长因子受体(Epidermal Growth Factor Receptor,EGFR)、白细胞介素–6(Interleukin–6,IL–6)、TNF等核心靶点及槲皮素等5个关键成分,分子对接显示其结合能力良好。结论:桃红四物汤通过“多成分–多靶点–多通路”协同机制实现对骨折与骨质疏松的“异病同治”,为中医药现代化研究提供理论依据。 展开更多
