Objective:To investigate the effectiveness of Quality Control Circle(QCC)activities in reducing the loss rate during storage of Traditional Chinese Medicine(TCM)decoction pieces in our hospital pharmacy department.Met...Objective:To investigate the effectiveness of Quality Control Circle(QCC)activities in reducing the loss rate during storage of Traditional Chinese Medicine(TCM)decoction pieces in our hospital pharmacy department.Methods:A QCC team was established to systematically analyze the causes of TCM decoction piece losses.Using quality improvement tools including Pareto charts,the 80/20 rule,and fishbone diagrams,we identified key areas for improvement and developed targeted intervention strategies.The study period spanned from January to December 2023,with pre-intervention data collected from January to December 2022.Loss rates were calculated and compared before and after QCC implementation.Results:According to the“80/20 rule”analysis,the primary improvement targets were identified as inadequate maintenance of decoction pieces and dispensing losses.Following QCC implementation,the loss rate of TCM decoction pieces decreased significantly from 1.27%(pre-intervention)to 0.65%(post-intervention),achieving an improvement rate of 47.40%.The total monetary loss decreased from 9,592.62 RMB to 4,546.97 RMB.Conclusion:QCC activities can effectively reduce the loss rate of TCM decoction pieces in hospital pharmacy departments,improve the quality of herbal medicines,ensure patient medication safety,and enhance pharmaceutical service quality.This systematic approach provides a valuable framework for continuous quality improvement in TCM pharmacy management.展开更多
BACKGROUND Diabetic gastroparesis(DGP)is a prevalent complication of diabetes that impairs people's quality of life and places a significant financial burden on them.The gastrointestinal symptoms of DGP patients c...BACKGROUND Diabetic gastroparesis(DGP)is a prevalent complication of diabetes that impairs people's quality of life and places a significant financial burden on them.The gastrointestinal symptoms of DGP patients can be improved by several Traditional Chinese Medicine(TCM)decoctions that have been shown to be effective in treating the disease.There are still many unanswered questions regarding the identification of appropriate therapeutic agents for the treatment of DGP in clinical practice.AIM To analyze the efficacy of several TCM decoctions in the treatment of DGP using Bayesian network meta-analysis for reference.METHODS PubMed,EMBASE,Cochrane Library,Web of Science,China National Knowledge Infrastructure,The China Biology Medicine DVD,Wanfang,and CQVIP were searched from inception to September 17,2022,to collect randomized controlled trials(RCTs)about TCM decoctions for DGP.Clinical effects and symptom scores were the primary outcomes.Additionally,we assessed motilin(MOT),somatostatin(SS),gastrin(GAS),gastric emptying rate,gastric emptying time,and adverse drug events as secondary outcomes.RESULTS A total of 67 eligible RCTs involving 4790 DGP patients and 7 TCM decoctions were included.The results of network meta-analysis(NMA)and surface under the cumulative ranking curve showed that with western medicine(WM)as a common control,the Banxia Xiexin Decoction(BXXD)+WM was most effective in clinical effects and enhancing early satiety scores;the Simo decoction(SMD)+WM was most effective in improving nausea and vomiting scores and anorexia scores,bloating scores;the Chaishao Liujunzi Decoction(CSLJD)was most effective in MOT,the Zhishi Xiaopi Decoction(ZSXPD)was most effective in SS and upgrading emptying rate;the Jianpi Xiaozhi Decoction was most effective in GAS;the CSLJD+WM was most effective in improving gastric emptying time.CONCLUSION These NMA results suggest that the BXXD+WM and SMD+WM may be one of the potential optimal treatments.Due to various limitations,further large-sample,double-blind,multi-center randomized RCTs are needed.展开更多

The quality management level of TCM decoction pieces has a direct impact on clinical efficacy, patient safety and the overall level of medical care in China's medical institutions. Therefore, for the health of patients and the quality of public medical care, it is necessary to strengthen the quality control of TCM decoction pieces. OBJECTIVE:To evaluate the efficacy and safety of Xuefuzhuyu decoction for hyperlipidemia.METHODS:Randomized clinical trials on hyperlipidemia treated by Xuefuzhuyu decoction,either alone or with Western Medicine,were ...OBJECTIVE:To evaluate the efficacy and safety of Xuefuzhuyu decoction for hyperlipidemia.METHODS:Randomized clinical trials on hyperlipidemia treated by Xuefuzhuyu decoction,either alone or with Western Medicine,were searched in electronic databases.Databases searched were:MEDLINE,Allied and Complementary Medicine Database,EMBASE,The Cochrane Library 2013(Issue4),China National Knowledge Infrastructure Database,Chinese Biomedical Literature Database,and Wanfang Database up to 2 May,2013.Study selection,data extraction,quality assessment,and data analysis were conducted according to the Cochranestandards.RESULTS:Six randomized clinical trials involving748 patients(373 patients in the treatment group,375 patients in the control group)were included in the analysis.The studies were of low methodological quality.Meta-analysis indicated that the effect of Xuefuzhuyu decoction on hyperlipidemia was better than that in the control group[n=748,OR=5.07,95%CI(3.40,7.58),P<0.01].Weighted mean differencesin total cholesterol,low-densitylipoprotein cholesterol,triglycerides,and high-density lipoprotein cholesterol were﹣0.79,﹣0.74,﹣0.44,0.16,respectively,and Meta-analysis revealed that the treatment group was better than the control group with 95%CI(﹣1.21,﹣0.36),(﹣0.94,﹣0.55),(﹣0.77,﹣0.11),(0.04,0.27),respectively(all P<0.05).Some adverse events in evaluated studies wererecorded.CONCLUSION:Xuefuzhuyu decoction may be effective for treating hyperlipidemia.The studies we analyzed were of low methodological quality,which indicates that the above findings should be considered cautiously.Therefore,more strictly designed large-scale randomized clinical trials are needed to evaluate the efficacy of Xuefuzhuyu decoction in hyperlipidemia.展开更多
OBJECTIVE:To observe the effects of different doses of Wenxiao Ⅱ Decoction on the expression of monocyte chemoattractant protein-1(MCP-1) and vascular cell adhesion molecule-1(VCAM-1) in an experimental model of athe...OBJECTIVE:To observe the effects of different doses of Wenxiao Ⅱ Decoction on the expression of monocyte chemoattractant protein-1(MCP-1) and vascular cell adhesion molecule-1(VCAM-1) in an experimental model of atherosclerosis in rabbits and to explore the mechanism by which it alleviates atherosclerosis.METHODS:Sixty 3-4 month-old New Zealand rabbits of both sexes were randomly divided into six groups:simvastain;model;blank;and high-dose,mid-dose,and low-dose Wenxiao Ⅱ Decoction groups.Except for those in the blank group,all rabbits were fed a high-cholesterol diet.Carotid atherosclerosis was established by balloon-induced injury to the endothelium of the carotid artery in conjunction with consumption of a high-cholesterol diet.After 8 weeks,all rabbits were killed to evaluate the expression of MCP-1 and VCAM-1 by immunohistochemical staining.RESULTS:Expressions of MCP-1 and VCAM-1 were significantly decreased in all groups except the blank group compared with the model group(P<0.05).When compared with the simvastain group only variation of MCP-1 expression in low-dose group was not appreciable,and the differences were indistinct(P<0.05).When comparing among Wenxiao Ⅱ Decoction groups,MCP-1 expression in the mid-and high-dose groups was significantly lower than that seen in the low-dose group(P<0.01),but there were no differences among three dosage groups with respect to VCAM-1 expression(P>0.05).CONCLUSION:These data suggested that high,mid,and low doses of Wenxiao Ⅱ Decoction can inhibit the expression of MCP-1 and VCAM-1,which may prevent the formation of or stabilize atherosclerotic plaques.There may be a direct relationship between the dosage of Wenxiao Ⅱ Decoction and its therapeutic efficacy.展开更多

In order to implement the Medium and Long-term Talent Development Plan for Medicine and Health (2011-2020) and the "Twelfth Five-year Plan" for the Development of Traditional Chinese Medicine, understand the authentic medicinal materials and drug processing, and propose quality control measures for Chinese herbal pieces, and apply them to clinical practice. In this article, using Da-cheng-qi Decoction(DCQD) as a model of formula, traditional decoction(TD), machinery decoction under high pressure(MDHP), and machinery decoction under normal pressure(MDNP) were compared. Methods For chemical components, HPLC fingerprints were established and evaluated using AHP combined with CRITIC weighing method; For animals' effects, the experiments of small intestinal propulsion were conducted; For clinical effects, a randomized clinical trial(RCT) was designed and performed. Results Although there were some differences between TD and MDNP in chemical ingredients, there was no significant difference in animal experiments and clinical trials(P 〉 0.05). Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR ana...Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR analysis.Methods The herbal components of BBT were extracted separately with different solvents,such as 95%ethanol,50%ethanol,and distilled water.The vasodilatory effect of the extracts was studied using the in vitro pre-contracted aortic ring model.Among these extracts,the water extracts of Pinellia ternata[PT(W)]and Citri reticulatae[CR(W)],50%ethanol extracts of Castrodia elata[GE(50)]and Clycyrrhiza uralensis[GU(50)],and 95%ethanol extracts of Atractylodes macrocephala[AM(E)],and Poria cocos[PC(E)]were selected for the orthogonal stimulus-response compatibility group study due to their high effectiveness in vasodilation.Results The best combination ratio(G2) was GE(50),GU(50),AM(E),PC(E),CR(W),and PT(W) at EC0,EC5,EC5,EC5,EC5,and EC5,with EC50 of(0.06 ± 0.01) mg/mL.Comparison between the vasodilatory effects of extracts derived from G2 and BBT were also studied.From the results,the 50%ethanol extract following G2 ratio was found to be most potent extract with the highest vasodilatory effect compared with other extracts derived from G2 or BBT,with EC50 of(0.05 ±0.005) mg/mL.Conclusion The optimum ratio and appropriate extraction method for the new BBT formulae are obtained.G2 is determined as the best formulae with the highest potency in producing the vasodilatory effect on isolated rat aortic rings.The results obtained suggest that 50%ethanol is found to be the best solvent to extract the vasoactive compounds of G2 in order to exhibit the highest efficacy of vasodilatory effect.展开更多

文摘Objective To decompose and reformulate Banxia Baizhu Tianma Decoction(BBT) in order to optimize its ratio for vasodilatory effect.The fingerprints of these six component herbs were also studied via Tri-step FTIR analysis.Methods The herbal components of BBT were extracted separately with different solvents,such as 95%ethanol,50%ethanol,and distilled water.The vasodilatory effect of the extracts was studied using the in vitro pre-contracted aortic ring model.Among these extracts,the water extracts of Pinellia ternata[PT(W)]and Citri reticulatae[CR(W)],50%ethanol extracts of Castrodia elata[GE(50)]and Clycyrrhiza uralensis[GU(50)],and 95%ethanol extracts of Atractylodes macrocephala[AM(E)],and Poria cocos[PC(E)]were selected for the orthogonal stimulus-response compatibility group study due to their high effectiveness in vasodilation.Results The best combination ratio(G2) was GE(50),GU(50),AM(E),PC(E),CR(W),and PT(W) at EC0,EC5,EC5,EC5,EC5,and EC5,with EC50 of(0.06 ± 0.01) mg/mL.Comparison between the vasodilatory effects of extracts derived from G2 and BBT were also studied.From the results,the 50%ethanol extract following G2 ratio was found to be most potent extract with the highest vasodilatory effect compared with other extracts derived from G2 or BBT,with EC50 of(0.05 ±0.005) mg/mL.Conclusion The optimum ratio and appropriate extraction method for the new BBT formulae are obtained.G2 is determined as the best formulae with the highest potency in producing the vasodilatory effect on isolated rat aortic rings.The results obtained suggest that 50%ethanol is found to be the best solvent to extract the vasoactive compounds of G2 in order to exhibit the highest efficacy of vasodilatory effect.
