CLINICAL COMPARISION OF THE TREATMENT OF CERVICAL PROLAPSE OF INTERVERTEBRAL DISC BY USING CAGE OR TITANIUM PLATE

Objective To compare the clinical effects of treating cervical prolapse of intervertebral disc by using Solis cage, titanium cage and autogenous iliac crest graft (AICG) combined with titanium plate. Methods 64 cases of cervical prolapse of intervertebral disc were analyzed retrospectively. All the patients were followed up for about one year. Group A is composed of 20 patients (30 intervertebral spaces) treated with microdiscectomy and Solis cage fusion; group B is composed of 21 patients(22 intervertebral spaces) treated with titanium cage and group C is involved of 23 patients(28 intervertebral spaces) which were dealt with AICG combined with titanium plate. The differences in the total X ray exposure time, time working on iliac bone, incidence of complications, fusion rate, incidence of JOA score recovery rate more than 50% and rehabilitation time of the three groups are analyzed. Results All indexes from group A and B were more satisfactory than those from group C (P<0.05). The statistic analysis results of the incidence of complications of cervical part, rehabilitation time in group A are 5.0±1.8, 5.1±1.2; and that is 14.3±2.6, 7.5 ±1.6 and 26.1±6.2, 8.6±2.3 in group B and group C respectively. There is significant difference between group A and group B on the incidence of complications of cervical part and rehabilitation time (P<0.05). Results of the incidence of JOA score recovery rate >50% in group A(95.8±3.2) was more satisfactory than that in group B(93.6±2.9), while there was no statistical difference between group A and group B (P>0.05). Conclusion The Solis cage is better in treating the cervical prolapse of intervertebral disc than other ways. Also, it makes the operation easier, rehabilitation time shorter, and it causes fewer complications. 展开更多

Two types of titanium/steel composite plates with the same thickness were manufactured by parallel explosive welding and double vertical explosive welding and rolling, respectively. The comparative analysis of microstructure showed that the interface of double vertical explosive welding plate (B plate) tended to be straight while the interface of parallel explosive welding plate (A plate) was wavy bonding. Defects near the interface of B plate were extruded, and the thickness of the diffusion layer of B plate was thicker under the effects of preheating temperature and press-working. Comparative tests of mechanical properties indicated that the tensile shear strength of B plate was lower while its micro-hardness was higher. Specimens of these two types of plates were neither separated nor cracked after bending up to 180° in the three-point bending test. From the microstructural observation of tensile fracture characteristics, A plate had strong toughness fracture while B plate had mainly ductile fracture with cleavage fracture as the supplement. Macroscopically, the tensile strength of the latter was 7.9% less than that of the former. However, both satisfied the Chinese standard of tensile strength. 展开更多

Some of the major manufacturing processes and corresponding mechanical properties of titanium clad steel plates were analyzed, and the consequences of research, manufacturing, and application of titanium clad steel plates in both markets of China and overseas were also summarized. As an economical and environmentally friendly technology, the roll bonding process is expected to become the next-generation mainstream process for the manufacturing of titanium clad steel plate. Some of the crucial and most important technical problems of this particular process, including vacuum sealing technology, surface treatment process technology, application of a transition layer, and rolling process, were discussed along with the advantageous mechanical properties and life-cycle economy of these plates processed by this technology. Finally, the market needs, application trends, and requirements of titanium clad steel plate were also considered from industries of petrochemical, shipbuilding, marine, and electric power. 展开更多

In the present study, titanium plate was treated by a novel method of laser quenching, simultaneously combined with liquidnitrogen cryogenics(LQLNC). The microstructure and properties of the titanium plate after treatment were investigated by X-ray diffraction, scanning electron microscopy, energy dispersive spectrometry, Vickers hardness testing, and friction wear testing.The results show that the treated titanium plate is covered by a nitrided layer with a homogeneous thickness of about 60 μm,while the nitrided layer consists of Ti N and α-Ti phases. Compared to general laser quenching, the LQLNC treatment increases the hardness and wear resistance of the surface-modified layer of the titanium plate. As a result of grain refinement in the nitrided layer, the cracking induced by the rapid solidification of the conventional laser-quenching process has also been effectively solved. 展开更多

In this study,the interface characteristics of a direct hot rolling titanium-clad steel plate were analyzed, and the mechanism of interface cracking was explored. The detrimental effect from the formation of TiFe ,TiC,and a Si-enriched layer on the bonding strength was clarified, and an industrial-scaled titanium-clad steel plate with shear strength over 200 MPa was produced with a carefully set schedule accordingly. It was found that hot rolling titanium-clad steel plates had a flat interface without obvious cracks. In the rolling process,both Ti and Fe atoms interdiflhsed,but Fe difthsed much faster than Ti. The Fe-diffused area consisted of three regions. After a high temperature heat treatment, the diffusion depth of Fe and Ti elements increased significantly and evident Si segregation and TiFe layers were identified. Thermal cracking initiated in the Si segregation layer and then propagated along the TiFe layer and Fe-diffused layer on the titanium side. 展开更多

In this paper, the single-pass hot compression experiment of titanium clad steel plate was carried out by Gleeble-3500 thermal mechanics simulation test machine,and the effect of deformation temperature(T), strain rate(ε),thickness ratio(k), and friction coefficient(l) on flow pattern of the metal and stress in the deformation zone was analyzed.The results show that the metal flow behavior and the stress during compressive deformation depend strongly on the deformation temperature. At 800 and 850 °C, the bimetal can flow uniformly, while at 900 °C, the TA2 flows faster than Q235 B, and the phenomenon of TA2 wrapping Q235 B is observed. The metal flow of the bimetal material will coordinate each other through the bonding interface. It is noted that the stress increases with the increase of the ε and l and decreases when the metal flows along the contact area. 展开更多

The low cycle fatigue behavior of zirconium−titanium−steel composite plate under symmetrical and asymmetric stress control was studied.The effects of mean stress and stress amplitude on cyclic deformation,ratcheting effect and damage mechanism were discussed in detail.The results show that under symmetric stress control,the forward ratcheting deformation is observed.Under asymmetric stress control,the ratcheting strain increases rapidly with mean stress and stress amplitude increasing.Under high stress amplitude,the influence of mean stress is more significant.In addition,by studying the variation of strain energy density,it is found that the stress amplitude mainly promotes the fatigue damage,while the mean stress leads to the ratcheting damage.In addition,fractographic observation shows that the crack initiates in the brittle metal compound at the interface,and the steel has higher resistance to crack propagation.Finally,the accuracy of life prediction model considering ratcheting effect is discussed in detail,and a high-precision life prediction model directly based on mean stress and stress amplitude is proposed. 展开更多

Objective:To observe the clinical effect of anterior decompression and reconstruction using titanium mesh with locking plates in the treatment of cervical spondylotic myelopathy. Methods:One hundred and twenty patients with cervical spondylotic myelopathy were treated by anterior decompression and reconstruction using titanium mesh with locking plates. There were 66 men and 54 women ranges in age from 37 to 72 Years(mean age, 58.3 years). The mean Japanese orthopedic surgery association(JOA) scale was 9.6 points before operation. Patients were followed up clinically and radiographically. Results:Having stood surgery well, the operation time ranged between 60-100 min and bleeding during operation ranged between 20-200 ml. There were no case of postoperative infection, recurrent laryngeal nerve palsy, or esophageal or tracheal laceration or rupture. The average follow-up period was 14.3 months(range, 12 to 24 months) in 96 who were followed up. At the last follow-up visit the mean JOA scale had improved to 14.4 points, reflecting an improvement of 4.8 points. The results were considered to be excellent in 87 patients, good in 25, fair in 6, and poor in 2. No hardware-related complications or adjacent segment degenerative changes were encountered during the follow-up periods. Stable bone union was observed in all cases and the average time required for fusion was 5.7 months. Conclusion:Titanium mesh filled with autologous bone graft can avoid the complications associated with harvesting bone from the iliac crest donor site. When combined with cervical anterior locking plate, it can obtain satisfatory clinical results for the treatment of cervical spondylotic myelopathy. 展开更多

Chest wall reconstruction after en-bloc tumor resection is very important to preserve functional mobility and to enhance the cosmetic effect. Because they are flexible and pliable, titanium plates are proposed to reconstruct the chest wall, even though such chest wall reconstruction has been performed in only a few cases worldwide. We present a case of a 49-year-old man with a chondrosarcoma arising from the left 1st rib, invading the manubrium, clavicle, 2nd and 3rd ribs, and the anterior segment of the left upper lobe. After wide resection, the chest wall was reconstructed using titanium rib plates and Marlex mesh- the Bovine pericardium sandwich type. The patient tolerated the pain well, and fourteen months after surgery, the chest wall was well preserved function mobility and improved pulmonary function test. 展开更多

Though a bone/metal interface in osseointegration was also formed on the surface of uncoated 317L plate screw after implantation, a smal1 space was seen between the bone and surface of the screw, indicating that the tissue compatibility of titanium-coated 317L plate screw may be better than that of the non-coated screw. 展开更多

钛钢复合板在爆炸成形过程中会产生局部未熔合等缺陷,目前采用电弧焊修复时精度、质量与自动化程度相对较低,主要难点集中在钢表面铜过渡层的高质量制备方面.为此提出采用半导体激光同轴送粉的方法精确成形铜过渡层,并在铜层表面TIG熔... 钛钢复合板在爆炸成形过程中会产生局部未熔合等缺陷,目前采用电弧焊修复时精度、质量与自动化程度相对较低,主要难点集中在钢表面铜过渡层的高质量制备方面.为此提出采用半导体激光同轴送粉的方法精确成形铜过渡层,并在铜层表面TIG熔覆钛层实现复合板的局部修复,采用半导体激光熔覆试验系统,分析了激光功率、送粉速率、扫描速度对铜层熔覆成形参数及微观组织的影响,并在铜层表面继续TIG熔覆钛层,工艺优化后得到了成形良好的Ti-Cu-Fe接头并对接头进行了微观组织测试与性能评价.结果表明,采用半导体激光可以实现铜层熔覆厚度的精确控制,厚度范围为0.236~0.462 mm;从Cu-Fe接头能谱仪(energy dispersive spectrometer,EDS)可以发现,通过铜层厚度的控制,上表面区域的Fe元素含量明显减小,有利于减少脆硬的Fe-Ti金属间化合物生成;TIG堆焊钛层后,接头主要以CuTi2金属间化合物为主,接头平均抗剪强度为194MPa,接头断裂方式为脆性断裂.创新点:(1)通过半导体激光熔覆可以在钢基材表面实现铜熔覆层成形的精确控制(2)激光熔覆的Cu层厚度能满足TIG堆焊钛层对熔深的要求,接头以CuTi2金属间化合物为主,性能良好. 展开更多

复杂髋臼双柱骨折4种不同内固定方式的有限元分析

背景:髋臼双柱骨折类型复杂且常见,目前治疗髋臼双柱骨折多采用传统重建接骨板,其与骨面的不匹配性会加大手术难度。个性化接骨板能够实现接骨板与骨面贴合,但个性化接骨板与传统重建接骨板固定髋臼双柱骨折的生物力学对比研究较少,且... 背景:髋臼双柱骨折类型复杂且常见,目前治疗髋臼双柱骨折多采用传统重建接骨板,其与骨面的不匹配性会加大手术难度。个性化接骨板能够实现接骨板与骨面贴合,但个性化接骨板与传统重建接骨板固定髋臼双柱骨折的生物力学对比研究较少,且模拟的体位姿态较为单一。目的:采用三维有限元方法分析复杂髋臼双柱骨折不同内固定方式在各种体位下的生物力学特性,为临床应用提供必要的生物力学依据。方法:建立累及方形区的复杂髋臼骨折中最具代表性的双柱骨折模型,在此基础上建立异形个性化钛板固定(A组)、前后柱双钛板固定(B组)、前柱重建钛板+后柱拉力螺钉固定(C组)和前柱重建钛板+髋臼下螺钉固定(D组)4种不同内固定方式固定复杂髋臼双柱骨折的三维有限元模型,分别模拟静坐、站立、患侧前伸、患侧外展以及患侧单腿站立体位下对4种复杂髋臼双柱骨折模型进行有限元分析,比较4种内固定方式的生物力学性能差异。结果与结论:①在各种体位下,骨折线上节点位移以及平均位移均表现为A组<B组<C组<D组,在患侧单腿站立体位下的位移值相较于其他体位最大,且A组内固定系统最大位移在4组模型中最小;各节点位移在站立和患侧单腿站立体位时,4组之间无统计学差异(P>0.05);在静坐、患侧前伸和患侧外展体位下,A组与B组、B组与C组、C组与D组无统计学差异(P>0.05),A组与C组、A组与D组、B组与D组之间差异有显著性意义(P<0.05);②A组各个骨折块所受最大应力相较于其他组更小,且骨折块所受应力分布更加均匀;内固定物应力主要集中于接骨板在骨折断端附近区域,其中A、B组内固定方式应力分布较为均匀,C、D组内固定方式应力集中较为明显,且A组相较于其他3组内固定物最大应力值减小了4.86%-54.61%;③在患侧前伸和患侧外展体位下,A、B组均具有较大的应力遮挡率,两组最大差值为5.67%;在患侧单腿站立体位下,A组内固定应力遮挡率最高,而D组应力遮挡率最小;④提示异形个性化钛板固定(A组)相较于其他3组传统重建接骨板固定具有更好的生物力学稳定性。 展开更多

本文以TA1纯钛为研究对象,采用电子背散射衍射(EBSD)技术及拉伸试验机对不同变形温度下轧制的退火态TA1纯钛板材的微观组织、织构和力学性能进行表征和测试,分析了TA1纯钛板材力学性能的影响因素,揭示织构对力学性能影响的机制。结果表... 本文以TA1纯钛为研究对象,采用电子背散射衍射(EBSD)技术及拉伸试验机对不同变形温度下轧制的退火态TA1纯钛板材的微观组织、织构和力学性能进行表征和测试,分析了TA1纯钛板材力学性能的影响因素,揭示织构对力学性能影响的机制。结果表明,退火态板材随变形温度的升高,抗拉强度和屈服强度升高,断后延伸率呈现降低,晶粒尺寸呈现增大的趋势。随变形温度变化,板材晶体发生转动,导致织构差异,在拉伸过程中,较低温度轧制的退火态板材形成的‖ND(法线方向)的锥面织构更容易激活{1010}<1120>柱面滑移系,导致其较容易变形,强度较低。随变形温度的升高,Hall-Petch关系中受织构影响的强度系数k值增大,即晶粒间变形传递的难度增大,进而导致强度的升高。具有随机的晶体取向可以开动更多类型的滑移系,晶粒间激活多种变形转移模式,从而削弱取向对晶界变形转移的影响。 展开更多

目的探讨带袢钛板与下胫腓螺钉内固定治疗踝关节骨折合并下胫腓联合不稳患者的临床效果。方法选择安徽理工大学第一附属医院2022年7月至2024年7月收治的70例踝关节骨折合并下胫腓联合不稳患者作为研究对象,依照治疗方法不同分为对照组(n... 目的探讨带袢钛板与下胫腓螺钉内固定治疗踝关节骨折合并下胫腓联合不稳患者的临床效果。方法选择安徽理工大学第一附属医院2022年7月至2024年7月收治的70例踝关节骨折合并下胫腓联合不稳患者作为研究对象,依照治疗方法不同分为对照组(n=35)与观察组(n=35),对照组采用下胫腓螺钉内固定治疗,观察组采用带袢钛板内固定治疗,观察比较两组患者的手术指标、关节功能评分、并发症发生率。结果观察组的手术指标更优、关节功能评分高于对照组(P<0.05);观察组的并发症发生率为5.71%,明显低于对照组的20.00%(P<0.05)。结论带袢钛板内固定治疗效果优于下胫腓螺钉,其有利于改善患者手术指标,避免患者关节功能障碍持续加剧,减少了并发症出现的可能性。 展开更多

钛钢复合板焊接修复时铜过渡层难以有效成形,提出采用蓝光激光提高铜金属的吸收率,并针对激光功率、送粉速率、扫描速度对铜粉熔化及铜层成形参数的作用规律展开研究,通过能谱仪(energy dispersive spectrometer,EDS)面扫分析了不同工... 钛钢复合板焊接修复时铜过渡层难以有效成形,提出采用蓝光激光提高铜金属的吸收率,并针对激光功率、送粉速率、扫描速度对铜粉熔化及铜层成形参数的作用规律展开研究,通过能谱仪(energy dispersive spectrometer,EDS)面扫分析了不同工艺参数组合对Fe-Cu界面中铁元素扩散行为的影响,在铜层表面继续同轴熔覆钛层时,分析了蓝光激光对铜层的熔化行为并研究了Fe-Cu-Ti接头的微观组织与性能,结果表明,采用蓝光激光可以实现铜粉末的高效熔化,随着激光功率的增加,铜层的熔宽、余高、熔深与稀释率均有所增加;随着送粉量的增加,铜层的熔宽和余高增加,熔深和稀释率明显减小;随着扫描速度的增加,铜层的成形参数和稀释率都呈减小趋势,通过Fe-Cu接头的EDS面扫发现,控制铜层的厚度,可使铁元素在上表面区域的含量显著减小,避免脆性Fe-Ti金属间化合物的形成,形成的接头以TiCu和Ti_(3)Cu_(4)金属间化合物为主,未形成脆性的Fe-Ti金属间化合物,铜钛混合区平均硬度为326 HV0.2. 展开更多

采用电弧焊接方法(TIG/MIG)进行钛/钢(TA1/Q345)和钛/铜/钢(TA1/T2/Q345)复合板的对接焊接,借助SEM,EBSD,TEM,显微硬度、纳米压痕和拉伸试验系统研究了对接焊缝中的显微结构和力学性能.结果表明,钛/钢对接接头中,Cu-V焊缝主要以铜基固... 采用电弧焊接方法(TIG/MIG)进行钛/钢(TA1/Q345)和钛/铜/钢(TA1/T2/Q345)复合板的对接焊接,借助SEM,EBSD,TEM,显微硬度、纳米压痕和拉伸试验系统研究了对接焊缝中的显微结构和力学性能.结果表明,钛/钢对接接头中,Cu-V焊缝主要以铜基固溶体和铁基固溶体为主,局部生成的Fe_(2)Ti相被韧性较好的铜基固溶体包围;Cu-V/ERTi-1焊缝界面处存在多种Cu-Ti和Fe-Ti金属间化合物;Cu-V焊缝与TA1/Q345界面处,存在Fe-Ti,CuTi_(2)和β-Ti化合物.钛/铜/钢对接接头中,Cu/ERTi-1焊缝界面处分布着多种Cu-Ti金属间化合物,分布范围较广.钛/钢对接焊缝中Fe_(2)Ti脆性相的硬度较高,为20.7GPa,但由于其尺寸相对较小,因此接头的显微硬度分布与钛/铜/钢对接焊缝类似,高硬度区域均在铜基焊缝与ERTi-1焊缝界面处,达到400HV0.3,两种对接接头中大量分布的Cu-Ti化合物的硬度处于8~11GPa.钛/钢异质接头的抗拉强度为440MPa,钛/铜/钢异质接头的抗拉强度为225MPa,断裂位置均在焊缝区域,并且铜基焊缝与ERTi-1焊缝界面处均是脆性断裂特征.钛/钢对接焊缝中不可避免会存在Fe-Ti脆性相,虽然采用钛/铜/钢三层复合板的形式可以避免Fe-Ti脆性相的生成,但是接头中分布较广的Cu-Ti化合物仍旧是接头的一个薄弱区域. 展开更多

跗骨窦入路结合微型钛板治疗跟骨骨折

目的探讨跗骨窦入路结合微型钛板治疗SandersⅡ、Ⅲ型跟骨骨折的疗效。方法采用跗骨窦入路结合微型钛板治疗28例SandersⅡ、Ⅲ型跟骨骨折患者(29足)。记录跟骨长度、高度及B9hler角、Gissane角、踝关节活动度,根据AOFAS踝-后足评分标准... 目的探讨跗骨窦入路结合微型钛板治疗SandersⅡ、Ⅲ型跟骨骨折的疗效。方法采用跗骨窦入路结合微型钛板治疗28例SandersⅡ、Ⅲ型跟骨骨折患者(29足)。记录跟骨长度、高度及B9hler角、Gissane角、踝关节活动度,根据AOFAS踝-后足评分标准评价疗效。结果患者均获得随访,时间12~24个月。随访期间无感染及皮肤坏死等并发症发生。骨折均愈合,时间3~6个月。跟骨长度、高度及B9hler角、Gissane角术后均较术前改善(P<0.01)。末次随访时,患者行走步态均正常,踝关节背伸20°~30°(25.3°±5.2°),跖屈40°~50°(44.6°±6.2°),内翻30°~40°(34.3°±5.8°),外翻10°~20°(16.2°±4.6°);根据AOFAS踝-后足评分标准评估患足功能:优14足,良11足,可4足,优良率25/29。结论跗骨窦入路结合微型钛板治疗SandersⅡ、Ⅲ型跟骨骨折,能够解剖复位距下关节面,且并发症少,患足功能恢复较好。 展开更多

可吸收生物接骨板在颌面部骨折治疗中的临床应用效果

目的探讨在颌面部骨折治疗中采取可吸收生物接骨板的有效性。方法选取2022年1月—2023年12月南通瑞慈医院收治的68例颌面部骨折患者,按照使用材料的不同将患者分为两组,各34例。研究组应用可吸收生物接骨板,对照组应用微型钛板。比较两... 目的探讨在颌面部骨折治疗中采取可吸收生物接骨板的有效性。方法选取2022年1月—2023年12月南通瑞慈医院收治的68例颌面部骨折患者,按照使用材料的不同将患者分为两组,各34例。研究组应用可吸收生物接骨板,对照组应用微型钛板。比较两组恢复效果、面部功能、临床指标、并发症发生情况。结果研究组恢复优良率为97.06%(33/34),高于对照组的79.41%(27/34),差异有统计学意义(χ^(2)=5.100,P<0.05)。治疗后,研究组AF-BF距、AXB角、开口功能、咀嚼功能、颌面部功能均优于对照组,差异均有统计学意义(P均<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论采取可吸收生物接骨板治疗颌面部骨折,能够提高恢复效果,促进面部功能恢复,优化临床指标,减少并发症。 展开更多
