OBJECTIVE:To evaluate the efficacy and safety of Zhumian Tang formula granules(助眠汤配方颗粒)combined with eszopiclone for treating poor sleep quality.METHODS:This multi-center,dynamic blockrandomized,parallel-group superiority clinical trial included 130 patients.The combined treatment group received Zhumian Tang formula granules combined with eszopiclone treatment,and the control group received eszopiclone treatment only.The group allocation ratio was 1∶1.The duration of treatment was 2 weeks.Participants were asked to complete questionnaires before treatment,after 1 week of the intervention,after 2 weeks of the intervention,and at the follow-up on week 3.The primary outcomes were the Pittsburgh Sleep Quality Index(PSQI)score and the total effective rate of treatment.The secondary outcome was the rate of adverse effects.RESULTS:Compared with the eszopiclone treatment group,the PSQI score of the combined treatment group was significantly lower after 2 weeks of the intervention(6.98 vs 8.26,P<0.05).However,there was no significant difference in the mean PSQI score after 1 week of the intervention(9.89 vs 9.15,P=0.124).After the follow-up on week 3,the PSQI score of the combined treatment group remained significantly lower than that of the eszopiclone treatment group(6.12 vs 8.31,P<0.001).The total effective rates of treatment of the combined group and the eszopiclone group were 36.92%vs 35.38%(Z=0.033,P=0.855)after 1 week of the intervention,83.08%vs 58.46%(Z=9.519,P<0.05)after 2 weeks of the intervention,and 83.08%vs 61.54%(Z=7.530,P<0.05)and after the follow-up on week 3,respectively.There was no significant difference in the overall rate of adverse reactions between the combined and eszopiclone treatment groups(21.53%vs 31.8%,P=0.318).CONCLUSION:The combination of Zhumian Tang formula granules with eszopiclone was found to be safe and more effective in improving sleep quality than eszopiclone alone.Traditional Chinese medicine can enhance the effectiveness of Western medicine in the treatment of insomnia. 展开更多

BACKGROUND Lung cancer(LC)combined with chronic obstructive pulmonary disease(COPD)is a common combination of comorbidities.Anti-inflammation and modulation of oxidative/antioxidative imbalance may prevent COPD-induced LC,and are also crucial to the treatment of LC combined with COPD.Modern studies have shown that Tao Hong Si Wu Tang(THSW)has vasodilatory,anti-inflammatory,anti-fatigue,anti-shock,immunoregulatory,lipid-reducing,micronutrient-supplementing,and anti-allergy effects.AIM To observe the effects of THSW on COPD and LC in mice.METHODS A total of 100 specific pathogen-free C57/BL6 mice were randomly divided into five groups:Blank control group(group A),model control group(group B),THSW group(group C),IL-6 group(group D),and THSW+IL-6 group(group E),with 20 mice in each group.A COPD mouse model was established using fumigation plus lipopolysaccharide intra-airway drip,and an LC model was replicated by in situ inoculation using the Lewis cell method.RESULTS The blank control group exhibited a clear alveolar structure.The model control and IL-6 groups had thickened alveolar walls,with smaller alveolar lumens,interstitial edema,and several inflammatory infiltrating cells.Histopathological changes in the lungs of the THSW and THSW+IL-6 groups were less than those of the model control group.The serum IL-1β,IL-6,and TNF-αlevels and IL-6R,JAK,p-JAK,STAT1/3,p-STAT1/3,FOXO,p-FOXO,and IL-7R expression levels in lung tissues of mice in the rest of the groups were significantly higher than those of the blank control group(P<0.01).Compared with the model control group,the IL-6 group demonstrated significantly higher levels for the abovementioned proteins in the serum and lung tissues(P<0.01),and the THSW group had significantly higher serum IL-1β,IL-6,and TNF-αlevels and IL-7R expression levels in lung tissues(P<0.01)but significantly decreased IL-6R,JAK,p-JAK,STAT1/3,p-STAT1/3,FOXO,p-FOXO,and IL-7R levels(P<0.01).CONCLUSION THSW reduces the serum IL-1β,IL-6,and TNF-αlevels in the mouse model with anti-inflammatory effects.Its antiinflammatory mechanism lies in inhibiting the overactivation of the JAK/STAT1/3 signaling pathway. 展开更多

OBJECTIVE: To investigate composition of gut microbial community in a rat model of functional dyspepsia(FD) and to explore the interventional effects of Simo Tang(四磨汤, SMT). METHODS: A rat model of FD was established through the tail-clamping stimulation method. The rat model of FD was assessed by the state of rats, their weight, sucrose preference rate, and intestinal propulsion rate. The DNA was extracted from stool samples after treatment with SMT. Amplified polymerase chain reaction(PCR) products of the 16S r DNA were sequenced using Novase Q6000 after construction of libraries. Composition of gut microbial community in the stool samples was determined and analyzed by cluster analysis, bioinformatic analysis, and analysis of α-diversity and β-diversity.RESULTS: The rat model of FD was successfully established using the tail-clamping stimulation method. The statistical results of cluster analysis of operational taxonomic units(OTUs) showed that the relative abundance of OTUs in the FD group was the lowest, while it was the highest in the normal(N) group. The composition of microbiome in the four groups was similar at phyla level. Compared with the FD group, the abundance of Firmicutes was downregulated, and the abundance of Proteobacteria and Bacteroidetes was upregulated in the Simo Tang(SMT) and high-dose Simo Tang(SMT.G) groups. The ratio of Bacteroidetes/Firmicutes was also elevated. According to the analysis of α-diversity and β-diversity, the abundance of flora in FD rats was significantly reduced. The treatment using SMT appeared beneficial to improve the diversity of flora. SMT could improve the intestinal flora in FD rats. The results showed that FD rats had intestinal flora imbalance, and species diversity increased. The results suggested that SMT could regulate the disorders of intestinal flora caused by FD. CONCLUDIONS: SMT could restore gut homeostasis and maintain gut flora diversity by modulating the gut microbiota and its associated metabolites in rats, thereby treating gastrointestinal diseases. 展开更多

Background:Mufangji tang(MFJT)is composed of Ramulus Cinnamomi,Radix Ginseng,Cocculus orbiculatus(Linn.)DC.,and Gypsum.In clinical settings,MFJT has been effectively employed in addressing a range of respiratory disorders,notably including pulmonary arterial hypertension(PAH).However,the mechanism of action of MFJT on PAH remains unknown.Methods:In this study,a monocrotaline-induced PAH rat model was established and treated with MFJT.The therapeutic effects of MFJT on PAH rat model were evaluated.Network pharmacology was conducted to screen the possible targets for MFJT on PAH,and the molecular docking between the main active components and the core targets was carried out.The key targets identified from network pharmacology were tested.Results:Results showed significant therapeutic effects of MFJT on PAH rat model.Analysis of network pharmacology revealed several potential targets related to apoptosis,inflammation,oxidative stress,and vascular remodeling.Molecular docking showed that the key components were well docked with the core targets.Further experimental validation results that MFJT treatment induced apoptosis(downregulated Bcl-2 levels and upregulated Bax levels in lung tissue),inhibited inflammatory response and oxdative stress(decreased the levels of IL-1β,TNF-α,inducible NOS,and malondialdehyde,and increased the levels of endothelial nitric oxide synthase,nitric oxide,glutathione and superoxide dismutase),reduced the proliferation of pulmonary arterial smooth muscle cells(downregulated ET-1 andβ-catenin levels and ERK1/2 phosphorylation,increased GSK3βlevels).Conclusion:Our study revealed MFJT treatment could alleviate PAH in rats via induction of apoptosis,inhibition of inflammation and oxidative stress,and the prevention of vascular remodeling. 展开更多

BACKGROUND Cold-dampness-type knee osteoarthritis is a common middle-aged and elderly disease,but its pathogenesis is not fully understood,and its clinical treatment has limitations.Glucosamine sulfate capsules are commonly used for treating arthritis,and San Bi Tang is a classic formula of traditional Chinese medicine(TCM)that has the effects of warming yang,dispelling dampness,relaxing muscles,and activating collaterals.This research hypothesized that the combination of modified San Bi Tang and glucosamine sulfate capsules could enhance the clinical efficacy of treating cold-dampness-type knee osteoarthritis through complementary effects.AIM To analyze the clinical efficacy of San Bi Tang combined with glucosamine sulfate capsules when treating cold-dampness-type knee osteoarthritis.METHODS A total of 110 patients with cold-dampness-type knee osteoarthritis were selected as research subjects and randomly divided into a control group and an experimental group of 55 cases each.The control group received only treatment with glucosamine sulfate capsules,while the experimental group received additional treatment with modified San Bi Tang for a duration of 5 wk.The patients’knee joint functions,liver and kidney function indicators,adverse reactions,and vital signs were evaluated and analyzed using SPSS 26.0 software.RESULTS Before treatment,the two groups’genders,ages,and scores were not significantly different,indicating comparability.Both groups’scores improved after treatment,which could indicate pain and knee joint function improvement,but the test group had better scores.The TCM-specific symptoms and the clinical efficacy of the treatment in the test group were higher.Before and after treatment,there were no abnormalities in the patients’liver and kidney function indicators.CONCLUSION The combination of modified San Bi Tang and glucosamine sulfate capsules is superior to treatment with sulfated glucosamine alone and has high safety. 展开更多

目的:探讨加味涤痰汤对非酒精性脂肪肝病(NAFLD)大鼠生物节律正负转录/翻译反馈回路(TTFL)相关基因的影响及其防治NAFLD的作用机制。方法:65只健康SPF雄性SD大鼠随机分为正常组(20只)、模型组(15只)、加味涤痰汤低剂量组(10只)、加味涤... 目的:探讨加味涤痰汤对非酒精性脂肪肝病(NAFLD)大鼠生物节律正负转录/翻译反馈回路(TTFL)相关基因的影响及其防治NAFLD的作用机制。方法:65只健康SPF雄性SD大鼠随机分为正常组(20只)、模型组(15只)、加味涤痰汤低剂量组(10只)、加味涤痰汤中剂量组(10只)、加味涤痰汤高剂量组(10只)。模型组和加味涤痰汤低、中、高剂量组采用高脂饲料饲喂12周,并于第9周开始,加味涤痰汤低剂量组给予加味涤痰汤煎液2.68 g·kg^(-1)·d^(-1)、加味涤痰汤中剂量组给予加味涤痰汤煎液5.36 g·kg^(-1)·d^(-1)、加味涤痰汤高剂量组给予加味涤痰汤煎液10.72 g·kg^(-1)·d^(-1),灌胃4周,正常组和模型组给予等体积生理盐水灌胃4周。全自动生化分析仪检测大鼠血清中甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)水平,TG、游离脂肪酸(NEFA)测定试剂盒检测肝脏TG、NEFA水平,苏木素-伊红(HE)染色法观察大鼠下丘脑、肝脏病理形态学表现,油红O染色法观察大鼠肝脏脂质沉积情况,免疫组化染色法检测下丘脑、肝脏芳香烃受体核转运蛋白的类似蛋白1(BMAL1/ARNTL)水平,实时荧光定量聚合酶链式反应(Real-time PCR)及蛋白免疫印迹法(Western blot)检测大鼠下丘脑、肝脏BMAL1、时钟基因(CLOCK)、周期蛋白2(PER2)和隐花色素1(Cry1)mRNA和蛋白水平。结果:与正常组比较,模型组大鼠血清中TG、TC、LDL-C、AST、ALT水平显著升高(P<0.01),HDL-C水平明显降低(P<0.05),肝脏中TG、NEFA水平显著升高(P<0.01),下丘脑神经元细胞固缩深染,脑区见大量空泡,肝脏见明显脂质沉积,下丘脑和肝脏中CLOCK、BMAL1 mRNA和蛋白表达显著升高(P<0.01),Cry1、PER2 mRNA和蛋白表达显著降低(P<0.01);给予加味涤痰汤治疗后,加味涤痰汤低、中、高剂量组大鼠血清中TG、TC、LDL-C、ALT、AST水平明显降低(P<0.05,P<0.01),HDL-C水平明显升高(P<0.05),肝脏中TG、NEFA水平明显降低(P<0.05,P<0.01),下丘脑神经元细胞核固缩深染有所改善,肝脏脂质沉积状况减轻,下丘脑、肝脏中CLOCK、BMAL1 mRNA和蛋白表达明显降低(P<0.05,P<0.01),Cry1、PER2 mRNA和蛋白表达明显升高(P<0.05,P<0.01)。结论:加味涤痰汤可减轻NAFLD大鼠肝脏脂质沉积,并影响NAFLD大鼠生物节律TTFL中相关基因CLOCK、BMAL1、Cry1、PER2表达。 展开更多

改良M-Tang法阶梯式缝合技术在Ⅱ区指深屈肌腱修复中的应用

目的:分析改良M-Tang法阶梯式缝合在Ⅱ区指深屈肌腱修复手术中的应用效果。方法:选取2021年1月-2023年12月笔者医院收治的178例(共220个伤指)Ⅱ区指深屈肌腱损伤患者,按随机数字表法将伤指分为对照组(n=110)与观察组(n=110)。对照组采... 目的:分析改良M-Tang法阶梯式缝合在Ⅱ区指深屈肌腱修复手术中的应用效果。方法:选取2021年1月-2023年12月笔者医院收治的178例(共220个伤指)Ⅱ区指深屈肌腱损伤患者,按随机数字表法将伤指分为对照组(n=110)与观察组(n=110)。对照组采用津下缝合技术,观察组采用改良M-Tang法阶梯式缝合技术。统计比较两组手术时间、住院时间、患者满意度及术后1个月的手指主动关节活动度和手功能障碍评分。结果:观察组手术时间短于对照组(P<0.05),两组住院时间及患者满意度比较差异无统计学意义(P>0.05)。观察组总主动关节活动度优良率明显高于对照组(P<0.05);观察组近侧及远侧指间关节活动度优于对照组(P<0.05),两组掌指关节活动度比较差异无统计学意义(P>0.05);观察组手功能障碍程度评分低于对照组(P<0.05)。结论:改良M-Tang法阶梯式缝合技术在Ⅱ区指深屈肌腱修复中效果良好,能够改善手指主动活动范围与手功能,值得推广应用。 展开更多

目的本研究旨在探索三仁汤通过Tim-3/Galectin-9信号途径对幽门螺旋杆菌(Helicobacter pylori,Hp)相关性胃炎(HAG)脾胃湿热证巨噬细胞极化的作用。方法72只Wistar大鼠随机分为空白组、模型组、四联疗法组(奥美拉唑2 mg/kg+阿莫西林100 m... 目的本研究旨在探索三仁汤通过Tim-3/Galectin-9信号途径对幽门螺旋杆菌(Helicobacter pylori,Hp)相关性胃炎(HAG)脾胃湿热证巨噬细胞极化的作用。方法72只Wistar大鼠随机分为空白组、模型组、四联疗法组(奥美拉唑2 mg/kg+阿莫西林100 mg/kg+克拉霉素50 mg/kg+胶体果胶铋胶囊35 mg/kg)、三仁汤低剂量组(3 g/kg)、三仁汤中剂量组(7.5 g/kg)、三仁汤高剂量组(15 g/kg),每组12只。采用复合因素(劳倦+饮食+苦寒药+环境+生物因子(Hp菌液))构建HAG脾胃湿热证大鼠模型,并按组别持续给药21 d。实验中观察大鼠的基本状况;快速尿素酶实验检测Hp定植;利用HE染色观察胃黏膜炎症情况;透射电镜观察胃黏膜超微结构;ELISA法测定血清IFN-γ、TNF-α、IL-4、IL-10水平;免疫组化检测胃黏膜CD68的表达;流式细胞术检测巨噬细胞F4/80^(+)CD86^(+)和F4/80^(+)CD206^(+)亚型;RT-qPCR检测胃黏膜组织MCP-1、CD80、CD86、IL-4、CD206、CD163 mRNA表达;Western blot检测Tim-3、Galectin-9蛋白在胃黏膜组织中的表达。结果模型组胃黏膜组织病理较空白组明显损伤,且Hp定植率、血清IFN-γ和TNF-α、胃黏膜组织CD68、Tim-3、Galectin-9蛋白表达及MCP-1、CD80、CD86、CD163 mRNA表达较空白组显著升高(P<0.01);三仁汤高剂量干预显著改善了模型大鼠胃黏膜组织损伤,显著降低了Hp定植率,并抑制了血清IFN-γ和TNF-α、胃黏膜组织CD68、Tim-3、Galectin-9蛋白表达、胃黏膜组织MCP-1、CD80、CD86、CD163mRNA表达(P<0.05)。结论三仁汤对大鼠HAG脾胃湿热证症状有改善作用,同时可抑制炎症反应并调节胃黏膜巨噬细胞的极化,其作用机制可能与调节Tim-3/Galectin-9途径有关。 展开更多

目的 探讨香砂益胃汤对慢性萎缩性胃炎(CAG)大鼠胃黏膜损伤的改善作用及机制。方法 将大鼠随机分为正常对照组,模型组,香砂益胃汤低、中、高剂量组(6、12、18 g/kg,以生药量计),香砂益胃汤高剂量+740 Y-P组[香砂益胃汤18 g/kg+转化生长... 目的 探讨香砂益胃汤对慢性萎缩性胃炎(CAG)大鼠胃黏膜损伤的改善作用及机制。方法 将大鼠随机分为正常对照组,模型组,香砂益胃汤低、中、高剂量组(6、12、18 g/kg,以生药量计),香砂益胃汤高剂量+740 Y-P组[香砂益胃汤18 g/kg+转化生长因子β(1 TGF-β_(1))/磷脂酰肌醇3激酶(PI3K)/蛋白激酶B(Akt)通路激活剂740 Y-P 10 mg/kg],每组18只。各组大鼠灌胃/注射相应药物,每天1次,连续4周。检测大鼠胃黏膜血流量、血清中胃激素[胃动素(MTL)、胃泌素(GAS)、胃蛋白酶(PP)]和炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、IL-6]水平;观察大鼠胃黏膜组织病理形态;检测大鼠胃黏膜细胞凋亡率;检测大鼠胃黏膜组织中TGF-β_(1)/PI3K/Akt信号通路相关蛋白和凋亡相关蛋白[B细胞淋巴瘤2(Bcl-2)、Bcl-2相关X蛋白(Bax)]的表达水平。结果 与正常对照组比较,模型组大鼠胃黏膜组织结构异常,胃黏膜上皮细胞脱落,炎症细胞浸润明显;胃黏膜血流量,血清中MTL、GAS、PP水平,胃黏膜组织中Bcl-2蛋白表达水平均显著降低(P<0.05);血清中TNF-α、IL-1β、IL-6水平,细胞凋亡率,胃黏膜组织中Bax、TGF-β_(1)蛋白表达水平和PI3K、Akt蛋白磷酸化水平均显著升高(P<0.05)。与模型组比较,香砂益胃汤各剂量组大鼠胃黏膜组织病理损伤减轻,炎症细胞浸润减少,上述定量指标均显著改善(P<0.05)。与香砂益胃汤高剂量组比较,香砂益胃汤高剂量+740 Y-P组大鼠胃黏膜病理损伤明显加重,上述定量指标均显著逆转(P<0.05)。结论 香砂益胃汤可减轻CAG大鼠胃黏膜损伤,其作用机制与抑制TGF-β_(1)/PI3K/Akt信号通路相关。 展开更多
