This review examines the distinct mechanisms of Tuina massage and topical herbal medicine in pediatric diarrhea management,focusing on their independent regulation of gastrointestinal metabolism and barrier function.T...This review examines the distinct mechanisms of Tuina massage and topical herbal medicine in pediatric diarrhea management,focusing on their independent regulation of gastrointestinal metabolism and barrier function.Tuina therapy primarily enhances gastrointestinal motility through the stimulation of targeted acupoints such as Tianshu(ST25)and Shenque(CV8),activating vagal efferent pathways to normalize motility hormones including motilin while suppressing pro-inflammatory cytokines such interleukin-6 and tumor necrosis factor-alpha.It further reinforces mucosal defense by inhibiting mast cell degranulation and augmenting microcirculation via nitric oxide-mediated vasodilation.Herbal medicine containing bioactive compounds from Atractylodes macrocephala and Poria cocos directly restores intestinal barrier integrity by upregulating tight junction proteins occludin and zonula occludens-1,inhibiting epithelial apoptosis,and stimulating MUC2 production.These formulations additionally reprogram enterocyte metabolism by reactivating mitochondrial tricarboxylic acid cycle flux and modulating short-chain fatty acid profiles,with the independent prebiotic effects enhancing commensal butyrate production.Tuina primarily modulates neuroimmune pathways and motility,and herbal medicine directly targets epithelial repair and metabolism.These therapeutic pathways may address core diarrhea pathophysiology,providing holistic complementary therapies.Standardized protocols and interdisciplinary research are urgently needed for clinical translation.展开更多
Background:Neuropathic pain(NP)has a long course and is difficult to treat,causing great physical suffering and psychological burden to patients.Unfortunately,the pathogenesis of NP is still poorly understood.The occu...Background:Neuropathic pain(NP)has a long course and is difficult to treat,causing great physical suffering and psychological burden to patients.Unfortunately,the pathogenesis of NP is still poorly understood.The occurrence and progression of NP are influenced by microRNA(miRNA).It has been reported that Tuina may effectively alleviate NP,however,the mechanisms related to miRNA-mediated Tuina for NP are still unexplored.Methods:To generate NP,a rat model of chronic constriction injury(CCI)was developed.Rats’pain thresholds are studied through pain behaviour tests.The effect of Tuina on the structure and morphology of neurones in the spinal dorsal horn(SDH)of rats with neuropathological pain was examined using HE staining.The levels of miRNA in the SDH of rats with a CCI model were studied using RNA sequencing(RNA-seq)and bioinformatics analysis.The top three miRNAs with the highest fold changes were chosen for qRT-PCR detection to confirm the validity of the RNA-seq results.Results:On the 4th day after CCI surgery,the paw withdrawal threshold(PWT)of the CCI+Tuina group was considerably higher than that of the CCI group(P<0.001).Furthermore,there was a significant increase in the paw withdrawal latency(PWL)of the CCI+Tuina group from day 4 to day 17 after the CCI surgery(P<0.001).Differentially expressed miRNAs in the SDH of three groups of rats were examined using RNA-seq technology.Seven miRNAs were found to intersect in total.The Kyoto Encyclopaedia of Genes and Genomes(KEGG)and Gene Ontology(GO)both anticipated the possible roles of the miRNAs.The RNA-seq results matched the changes in the levels of miR-383-3p(P<0.01),miR-183-3p(P<0.01),and miR-184(P<0.01).Conclusion:CCI-induced NP in rats leads to changes in miRNA expression in the SDH,and Tuina may alleviate NP by modulating the expression pattern of miRNAs and their underlying mechanisms.These findings may provide new targets for future NP treatment.展开更多
BACKGROUND Knee osteoarthritis(KOA)is a chronic condition characterized by joint pain and dysfunction,driven by aging and obesity.Research indicates that the gut microbiota significantly influences KOA,potentially aff...BACKGROUND Knee osteoarthritis(KOA)is a chronic condition characterized by joint pain and dysfunction,driven by aging and obesity.Research indicates that the gut microbiota significantly influences KOA,potentially affecting inflammation and disease progression through the gut-joint axis.Traditional treatments like nonsteroidal anti-inflammatory drugs offer symptom relief but have adverse effects.Emerging therapies like electroacupuncture(EA)and Tuina(TN)have shown promise in alleviating pain and improving joint function by targeting the gut microbiota.AIM To clarify the efficacy of EA with TN in treating KOA and its effect on gut microbiota regulation.METHODS Sixty patients with KOA were allocated to EA or EA+TN(ET)group(n=30 each).Seven acupoints were punctured.The ET group received TN after each EA session.Both groups completed 12 sessions.The visual analog scale(VAS)for assessing pain and the Western Ontario and McMaster Universities osteoarthritis index(WOMAC)for measuring pain intensity,joint stiffness,and functional capacity were employed to assess clinical outcomes.Pre-and post-treatment fecal specimens underwent 16S ribosomal RNA sequencing to analyze the gut microbiota.RESULTS The ET group showed higher rates of“effective”and“markedly effective”outcomes.The VAS score of the ET group remained significantly lower than that of the EA group(P<0.001)immediately after treatment and 1 week post-treatment.The total WOMAC score(P<0.001),pain(P=0.191),stiffness(P=0.015),and function scores(P<0.001)decreased significantly in the ET group post-treatment.The gut microbiota analysis revealed no significant changes in alpha diversity in either group.Beta-diversity analysis indicated distinct patterns in the ET group before and after treatment.Significant changes in microbial abundance were detected in both groups,highlighting variations in Firmicutes,Actinobacteria,Proteobacteria,and Bacteroidetes.CONCLUSION ET outperforms EA alone in improving KOA pain,stiffness,and function,potentially via gut microbiota modulation,intestinal barrier protection,and inflammation reduction.展开更多
[Objectives]To investigate the therapeutic effects of combined treatment on children diagnosed with pseudomyopia,based on the theoretical framework that the liver is associated with eye health in traditional Chinese m...[Objectives]To investigate the therapeutic effects of combined treatment on children diagnosed with pseudomyopia,based on the theoretical framework that the liver is associated with eye health in traditional Chinese medicine.[Methods]A multicenter randomized controlled trial was conducted involving 100 children,with the treatment group consisting of 50 participants receiving a combined treatment,while the control group,also comprising 50 participants,received conventional tuina.In the treatment group,the Yizhichan Pushing Manipulation was applied at a frequency of 120 times/min and a strength of 0.5-1.0 kg,targeting acupoints such as Ganyu and Guangming.The alterations in naked eye visual acuity,diopter measurements,and ciliary muscle regulation function were compared between the two groups of children both prior to and following treatment,and the clinical efficacy was assessed.[Results]The treatment group demonstrated a statistically significant enhancement in visual acuity(1.74±0.49 lines vs.1.02±0.52 lines),diopter improvement(0.91±0.29 D vs.0.46±0.25 D),and overall efficiency(96%vs.78%)when compared to the control group(all P<0.01).[Conclusions]This combined treatment exerts a therapeutic effect through a dual mechanism that involves the regulation of liver meridian qi and blood,as well as the enhancement of ciliary muscle function.This approach merits further dissemination and application in the prevention and management of myopia in children.展开更多
BACKGROUND Prediabetes is a high-risk precursor for diabetes development and is associated with increased psychological distress.Conventional pharmacological treatments for prediabetes have limitations,including adver...BACKGROUND Prediabetes is a high-risk precursor for diabetes development and is associated with increased psychological distress.Conventional pharmacological treatments for prediabetes have limitations,including adverse effects and poor patient compliance.Tuina’s abdominal vibration technique,a traditional non-pharmacological intervention,has shown promising results in glycemic regulation.However,its effects on psychological well-being remain largely unexplored.AIM To investigate efficacy of abdominal vibration at varying frequencies in Tuina for alleviating psychological and metabolic effects in prediabetes patients.METHODS A prospective cohort study(April 2025 to April 2026)at Dongfang Hospital of Beijing University of Chinese Medicine included 120 prediabetes patients.Participants were allocated to three groups based on vibration frequency:(1)Lowfrequency(400 times/minute,n=40);(2)Medium-frequency(500 times/minute,n=40);and(3)High-frequency(600 times/minute,n=40).All participants received 30-minute sessions three times weekly for 3 months,with follow-up at 6 months and 12 months.Primary outcomes included changes in depression(Beck Depression Inventory-Ⅱ),anxiety(State-Trait Anxiety Inventory),and stress(Perceived Stress Scale)levels.RESULTS After a 3-month intervention period,with follow-ups at 6 months and 12 months,patients in all three groups showed significant improvements in depression,anxiety,and stress scores compared to baseline(P<0.001).The high-frequency group demonstrated the most substantial psychological improvements(mean reduction in depression scores:9.2±2.3 points;anxiety:8.7±2.1 points;stress:10.4±2.5 points).These psychological improvements correlated significantly with reductions in insulin resistance(r=0.68,P<0.001).The high-frequency group also showed the most significant improvements in glycemic parameters,with mean reductions in fasting plasma glucose,2-hour postprandial glucose,and glycosylated hemoglobin concentrations of 0.92 mmol/L,1.87 mmol/L,and 0.51%,respectively.Correlation analysis revealed significant associations between improved psychological parameters and enhanced glycemic control.CONCLUSION Tuina’s abdominal vibration technique,especially at high frequency reduces depression,anxiety,and stress in prediabetes patients,correlating with enhanced glycemic control and insulin sensitivity,suggesting a bidirectional relationship between psychological and metabolic health.展开更多
Objective:To clarify the preferences and core demands of professional Master’s students in Tuina at Yunnan University of Chinese Medicine regarding“concentrated”versus“dispersed”rotation schedules in the Tuina de...Objective:To clarify the preferences and core demands of professional Master’s students in Tuina at Yunnan University of Chinese Medicine regarding“concentrated”versus“dispersed”rotation schedules in the Tuina department,as well as their preferred year for primary rotation,providing evidence for optimizing rotation programs and improving the quality of standardized training.Methods:Paper questionnaires,including two multiple-choice questions and one open-ended question,were distributed to 30 professional Tuina Master’s students,completed on site,and collected immediately.Descriptive statistics were used to analyze preference distributions.Results:A total of 30 questionnaires were distributed and 30 valid responses were collected,achieving a 100%response rate.Among them,23 students(76.7%)supported“concentrated training,”while seven students(23.3%)preferred“dispersed training.”Regarding rotation year preference,18 students(60.0%)selected the second year for concentrated Tuina rotations,eight students(26.7%)selected the third year,and four students(13.3%)selected the first year.The core demands supporting concentrated training were“skill continuity,deep involvement in diagnosis and treatment,and smooth mentorship transition,”whereas those supporting dispersed training were“integration of multi-department knowledge,coordination of research time,and avoidance of fatigue from a single department.”Conclusion:Most professional Tuina Master’s students prefer concentrated Tuina rotations in the second year.It is recommended that institutions adopt a“second-year core department concentrated+auxiliary department flexible dispersed”model and establish flexible coordination and feedback mechanisms to balance clinical skill development with individualized growth needs.展开更多
Objective:To investigate the effects of“Three Methods and Three Acupoints”(TMTP)Tuina therapy on spinal microcirculation in sciatic nerve injury(SNI).Methods:Thirty-six SpragueeDawley rats were randomly assigned to ...Objective:To investigate the effects of“Three Methods and Three Acupoints”(TMTP)Tuina therapy on spinal microcirculation in sciatic nerve injury(SNI).Methods:Thirty-six SpragueeDawley rats were randomly assigned to four groups:normal,sham operation,model,and TMTP Tuina.Successful model induction was confirmed by observable hind limb lameness.After 20 sessions,hind limb grip strength and motor nerve conduction velocity(MNCV)were measured at baseline and following the 10th and 20th intervention.CD31 and a-SMA in the ventral horn of SNI model rats were detected using immunofluorescence.Motor neurons in the ventral horn were detected by Nissl staining.PTEN levels in the ventral horn were measured by ELISA,and PI3K,Akt,BDNF,VEGF,and HIF-1a expression was determined by RT-PCR.Spinal cord microcirculation was evaluated by western blotting analysis of the levels of Akt,p-Akt,BDNF,and VEGF.Results:Hind limb grip strength and MNCV significantly improved in the TMTP Tuina group compared to the model group(both P<.001).Morphology of ventral horn motor neurons in the TMTP Tuina group improved compared to the model group,with increased expressions of a-SMA(P=.002)and CD31(P=.006).Western blot analysis indicated increased expression of VEGF(P=.005),p-Akt(P<.001),and BDNF(P=.008)in the ventral horn following Tuina treatment.RT-PCR analysis revealed increased expression of PI3K,Akt,BDNF,VEGF and HIF-1a(all P<.05).In contrast,expression of PTEN decreased compared to the model group(P<.001).Conclusion:TMTP Tuina therapy may restore motor function in rats,enhance ventral horn motor neuron morphology,and promote angiogenesis and vascular smooth muscle proliferation.The mechanism may involve the activation of the PI3K/Akt signaling pathway.展开更多

The clinical efficacy was observed after 6 treatments. Results The treatment of pediatric allergic rhinitis by combining Tuina and Chinese herbal medicines had an overall effectivness rate of 85.71%. Conclusion Combined treatment using pediatric Tuina and an herbal mixture of Yupingfeng powder (玉屏风散) and Guizhi decoction (桂枝汤) has positive therapeutic efficacy for treatment of pediatric allergic rhinitis, and should be considered as a treatment for this condition.展开更多

Methods With a randomized, controlled and clinical method, Eighty-four full-term primiparas were divided i...Objective To explore the efficacy of Tuina for postpartum lactation and work out an optimal protocol involved. Methods With a randomized, controlled and clinical method, Eighty-four full-term primiparas were divided into a Tuina group (58 cases) and a control group (26 cases). While the patients in the control group received rooming in conventional nursing, those in the Tuina group were additionally treated with Tuina, including local manipulations at the breasts combined with acupoint manipulations. The colostrum time, lactation volume and serum prolactin were observed to make the comparisons between the two groups. Results The scores of lactation quantity after the 1st, 2nd, 3rd of the treatment were 1.660±0.785, 2.530±1.030, 2.880±1.171 in the Tuina group and 1.270±0.533, 1.460±0.811, 1.500±0.583 in the control group respectively, where there were significant differences in each time stage between the two groups (all P0.001). The time of colostrum was (21.6±10.5) h in the Tuina group and (22.5±9.7) h in the control group, in which the difference was not statistically significant (P0.05). The levels of prolactin were (314.35±110.37) ng/mL and (321.56±109.61) ng/mL in the Tuina group, (385.78±85.19) ng/mL and (340.12±103.10) ng/mL in the control group before and after treatment respectively, suggested that there were no significant difference (both P0.05). Conclusion Postpartum Tuina at the breasts could increase the quantity of lactation and delay the decreasing of the levels of prolactin, which contributes primiparas to lactate more and sooner.展开更多

The rats were housed in cages with free access to water and food in a temperature-controlled room [(22 ± 1) ℃] and12-h/12-h light-dark cycle. Thirty-two rats were randomly divided into five groups: naive, sham, chronic compression of dorsal root ganglion(CCD), Tuina(7 d) and Tuina(21 d). CCD rat model was established via unilateral DRG compression by"L"liked steel bar. Chinese Tuina treatment was accepted once per day. Behavior monitoring of paw withdrawal threshold(PWT) and paw withdrawal latency(PWL) were tested. The expression of Piezo1 and Piezo2 in myelinated nerve fiber were analyzed by immunohistochemistry and Western-blotting.RESULTS: There was a high expression of Piezo2 and a low expression of Piezo1 in the naive and CCD groups. In contrast, the expression of Piezo2 was down regulated and Piezo1 was increased after a period of Tuina. There was significant difference(P ≤ 0.05) between the groups.CONCLUSION: Our findings suggest that Tuina therapy can increase the expression of Piezo2 and decrease the expression of Piezo1 in the test rats.The different changes in the expressions of Piezo1 and Piezo2 may play an important role in alleviating CCD-induced allodynia and hyperalgesia.展开更多
OBJECTIVE:To determine the clinical effect,treatment times,and rheoencephalogram changes in vertebral artery type cervical spondylosis patients treated with innovative Tuina manipulations.METHODS:One hundred and twent...OBJECTIVE:To determine the clinical effect,treatment times,and rheoencephalogram changes in vertebral artery type cervical spondylosis patients treated with innovative Tuina manipulations.METHODS:One hundred and twenty six cervical spondylosis patients(vertebral artery type) were randomly divided into test and control groups.Patients in the test group were treated with innovative Tuina manipulations,while those in the control group were treated with the routine Tuina manipulations according to the textbook of Chinese Massage for Acupuncture and Moxibustion majors.The clinical effects,treatment times,clinical symptoms,and cerebral blood flow were measured.RESULTS:The response to the treatment was 100% in the test group and 88.71% in the control group.Patients in the test group required(7 ± 4) treatments before recovery,while those in the control group required(15 ± 7) treatments before recovery(P<0.05).The clinical symptoms exhibited greater improvement in the test group compared to the control group(P<0.05).There were no differences in cerebral blood flow between the two groups.CONCLUSION:Both innovative Tuina manipulations and routine Tuina manipulations produced satisfactory therapeutic results in vertebral artery type cervical spondylosis patients.However,the innovative manipulation was more effective in improving the functional symptoms,although there were no changes in the cerebral blood flow.展开更多
OBJECTIVE:To investigate whether the Chinese massage system,Tuina,exerts analgesic effects in a rat model of chronic constriction injury(CCI)by remodeling the synaptic structure in the spinal cord dorsal horn(SCDH).ME...OBJECTIVE:To investigate whether the Chinese massage system,Tuina,exerts analgesic effects in a rat model of chronic constriction injury(CCI)by remodeling the synaptic structure in the spinal cord dorsal horn(SCDH).METHODS:Sixty-nine male Sprague–Dawley rats were randomly and evenly divided into the normal group,sham group,CCI group,CCI+Tuina group,CCI+MK-801[an N-methyl D-aspartate receptor subtype 2B(NR2B)antagonist]group,and CCI+MK-801+Tuina group.The neuropathic pain model was established using CCI with right sciatic nerve ligation.Tuina was administered 4 d after CCI surgery,using pressing manipulation for 10 min,once daily.Motor function was observed with the inclined plate test,and pain behaviors were observed by the Von Frey test and acetone spray test.At 19 d after surgery,the L3-L5 spinal cord segments were removed.Glutamate,interleukin 1β(IL-1β),and tumor necrosis factor-α(TNF-α)levels were detected by enzyme-linked immunosorbent assay.The protein expression levels of NR2B and postsynaptic density protein-95(PSD-95)were detected by Western blot,and the synaptic structure was observed by transmission electron microscopy(TEM).RESULTS:CCI reduced motor function and caused mechanical and cold allodynia in rats,increased glutamate concentration and TNF-αand IL-1βlevels,and increased expression of synapse-related proteins NR2B and PSD-95 in the SCDH.TEM revealed that the synaptic structure of SCDH neurons was altered.Most of these disease-induced changes were reversed by Tuina and intrathecal injection of MK-801(P<0.05 or<0.01).For the majority of experiments,no significant differences were found between the CCI+MK-801 and CCI+MK-801+Tuina groups.CONCLUSIONS:Chinese Tuina can alleviate pain by remodeling the synaptic structure,and NR2B and PSD-95 receptors in the SCDH may be among its targets.展开更多

BACKGROUND The most effective treatment for knee joint pain is total knee arthroplasty(TKA),but the risk of pain and swelling in patients after surgery is high.Ice application,ankle pump exercise and non-steroidal ant...BACKGROUND The most effective treatment for knee joint pain is total knee arthroplasty(TKA),but the risk of pain and swelling in patients after surgery is high.Ice application,ankle pump exercise and non-steroidal anti-inflammatory painkillers are the primary clinical treatments after surgery.However,long-term use of nonsteroidal anti-inflammatory pain relievers can easily cause gastrointestinal damage.Traditional Chinese medicine(TCM)ointments and tuina therapy integrate TCM and manipulation,which effectively promotes the penetration of TCM into the skin lesions,improves local blood circulation and inflammatory reaction and has good long-term effects on patients.AIM To evaluate the efficacy of TCM ointment combined with tuina therapy in the treatment of pain and swelling after TKA.METHODS The randomized controlled clinical trial enrolled 80 patients who underwent TKA via the same procedure.The patients were randomly divided among the treatment group(n=40)and the control group(n=40).The control group was given an analgesia pump in addition to oral painkillers as the postoperative intervention.The treatment group received TCM ointment with tuina therapy in addition to the analgesia pump and oral painkillers in the postoperative period.The following variables were recorded 3 d before surgery and 3 d,7 d and 14 d after surgery:Visual analogue scale(VAS)score;skin temperature;circumferences at 15 cm above and below the patella;maximum active knee flexion angle;and the knee injury and Osteoarthritis Outcome score(KOOS).RESULTS After treatment,VAS was significantly lower in the treatment group than the control group at 7 d(t=7.536,P<0.001)and 14 d(t=8.563,P<0.001).The skin temperature of participants in the treatment group was significantly lower than that in the control group at 7 d(t=2.968,P=0.004)and 14 d(t=4.423,P<0.001).The circumference values of the two positions in the treatment group were lower than those in the control group at 7 d[t=2.315,P=0.023(above);t=2.121,P=0.037(below)]and 14 d[t=2.374,P=0.020(above);t=2.095,P=0.039(below)].After 14 d of treatment,the maximum active knee flexion angle and KOOS of the two groups were significantly improved but were significantly higher in the treatment group(P<0.05 for both).CONCLUSION TCM ointment and tuina therapy have significant advantages over standard care in the treatment of pain and swelling after TKA.This additional treatment may improve knee function but additional studies are needed to confirm our observations.展开更多
OBJECTIVE:To observe the analgesic effects of Tuina on neuropathic pain(NPP)and the underlying mechanisms.METHODS:Forty-eight Sprague-Dawley(SD)rats were assigned by random into three treatment groups:sham,chronic con...OBJECTIVE:To observe the analgesic effects of Tuina on neuropathic pain(NPP)and the underlying mechanisms.METHODS:Forty-eight Sprague-Dawley(SD)rats were assigned by random into three treatment groups:sham,chronic constriction injury(CCI),and Tuina.Each group contained sixteen rats.CCI model was generated by ligating the right sciatic nerve.Behavioral changes of CCI were assessed by the paw withdrawal threshold(PWT)and paw withdrawal latency(PWL).In addition,biochemical techniques such as immunofluorescence staining,enzyme-linked immunosorbent assay(ELISA)and Western blotting were used to profile levels of microglia activation and inflammatory factors in the spinal dorsal horn(SDH)of rats.Tuina(clockwise pressing and rubbing)was performed at Chengshan(BL57)to observe the analgesic effects on CCI rats and the underlying mechanisms.RESULTS:Rats with CCI experienced significant reduction in the PWT and PWL of the right hind paw relative to CCI group at day 3.Tuina treatment rescued this situation significantly on days 10 and 14.Besides,Iba-1,microglia M1 receptor CD68,tumor necrosis factor-α(TNF-α),and interleukin-1β(IL-1β)were higher in the right SDH for CCI group compared to the sham group on day 14.As expected,Tuina partially downregulated the CCI-induced overexpressed Iba-1,CD68,TNF-α,and IL-1βin the SDH of CCI model.CONCLUSION:Tuina induces a time-dependent cumulative analgesic effect in CCI rats by inhibiting the activation of microglia and the secretion of IL-1βand TNF-αin SDH.展开更多

文摘This review examines the distinct mechanisms of Tuina massage and topical herbal medicine in pediatric diarrhea management,focusing on their independent regulation of gastrointestinal metabolism and barrier function.Tuina therapy primarily enhances gastrointestinal motility through the stimulation of targeted acupoints such as Tianshu(ST25)and Shenque(CV8),activating vagal efferent pathways to normalize motility hormones including motilin while suppressing pro-inflammatory cytokines such interleukin-6 and tumor necrosis factor-alpha.It further reinforces mucosal defense by inhibiting mast cell degranulation and augmenting microcirculation via nitric oxide-mediated vasodilation.Herbal medicine containing bioactive compounds from Atractylodes macrocephala and Poria cocos directly restores intestinal barrier integrity by upregulating tight junction proteins occludin and zonula occludens-1,inhibiting epithelial apoptosis,and stimulating MUC2 production.These formulations additionally reprogram enterocyte metabolism by reactivating mitochondrial tricarboxylic acid cycle flux and modulating short-chain fatty acid profiles,with the independent prebiotic effects enhancing commensal butyrate production.Tuina primarily modulates neuroimmune pathways and motility,and herbal medicine directly targets epithelial repair and metabolism.These therapeutic pathways may address core diarrhea pathophysiology,providing holistic complementary therapies.Standardized protocols and interdisciplinary research are urgently needed for clinical translation.

文摘Background:Neuropathic pain(NP)has a long course and is difficult to treat,causing great physical suffering and psychological burden to patients.Unfortunately,the pathogenesis of NP is still poorly understood.The occurrence and progression of NP are influenced by microRNA(miRNA).It has been reported that Tuina may effectively alleviate NP,however,the mechanisms related to miRNA-mediated Tuina for NP are still unexplored.Methods:To generate NP,a rat model of chronic constriction injury(CCI)was developed.Rats’pain thresholds are studied through pain behaviour tests.The effect of Tuina on the structure and morphology of neurones in the spinal dorsal horn(SDH)of rats with neuropathological pain was examined using HE staining.The levels of miRNA in the SDH of rats with a CCI model were studied using RNA sequencing(RNA-seq)and bioinformatics analysis.The top three miRNAs with the highest fold changes were chosen for qRT-PCR detection to confirm the validity of the RNA-seq results.Results:On the 4th day after CCI surgery,the paw withdrawal threshold(PWT)of the CCI+Tuina group was considerably higher than that of the CCI group(P<0.001).Furthermore,there was a significant increase in the paw withdrawal latency(PWL)of the CCI+Tuina group from day 4 to day 17 after the CCI surgery(P<0.001).Differentially expressed miRNAs in the SDH of three groups of rats were examined using RNA-seq technology.Seven miRNAs were found to intersect in total.The Kyoto Encyclopaedia of Genes and Genomes(KEGG)and Gene Ontology(GO)both anticipated the possible roles of the miRNAs.The RNA-seq results matched the changes in the levels of miR-383-3p(P<0.01),miR-183-3p(P<0.01),and miR-184(P<0.01).Conclusion:CCI-induced NP in rats leads to changes in miRNA expression in the SDH,and Tuina may alleviate NP by modulating the expression pattern of miRNAs and their underlying mechanisms.These findings may provide new targets for future NP treatment.

文摘BACKGROUND Knee osteoarthritis(KOA)is a chronic condition characterized by joint pain and dysfunction,driven by aging and obesity.Research indicates that the gut microbiota significantly influences KOA,potentially affecting inflammation and disease progression through the gut-joint axis.Traditional treatments like nonsteroidal anti-inflammatory drugs offer symptom relief but have adverse effects.Emerging therapies like electroacupuncture(EA)and Tuina(TN)have shown promise in alleviating pain and improving joint function by targeting the gut microbiota.AIM To clarify the efficacy of EA with TN in treating KOA and its effect on gut microbiota regulation.METHODS Sixty patients with KOA were allocated to EA or EA+TN(ET)group(n=30 each).Seven acupoints were punctured.The ET group received TN after each EA session.Both groups completed 12 sessions.The visual analog scale(VAS)for assessing pain and the Western Ontario and McMaster Universities osteoarthritis index(WOMAC)for measuring pain intensity,joint stiffness,and functional capacity were employed to assess clinical outcomes.Pre-and post-treatment fecal specimens underwent 16S ribosomal RNA sequencing to analyze the gut microbiota.RESULTS The ET group showed higher rates of“effective”and“markedly effective”outcomes.The VAS score of the ET group remained significantly lower than that of the EA group(P<0.001)immediately after treatment and 1 week post-treatment.The total WOMAC score(P<0.001),pain(P=0.191),stiffness(P=0.015),and function scores(P<0.001)decreased significantly in the ET group post-treatment.The gut microbiota analysis revealed no significant changes in alpha diversity in either group.Beta-diversity analysis indicated distinct patterns in the ET group before and after treatment.Significant changes in microbial abundance were detected in both groups,highlighting variations in Firmicutes,Actinobacteria,Proteobacteria,and Bacteroidetes.CONCLUSION ET outperforms EA alone in improving KOA pain,stiffness,and function,potentially via gut microbiota modulation,intestinal barrier protection,and inflammation reduction.

文摘[Objectives]To investigate the therapeutic effects of combined treatment on children diagnosed with pseudomyopia,based on the theoretical framework that the liver is associated with eye health in traditional Chinese medicine.[Methods]A multicenter randomized controlled trial was conducted involving 100 children,with the treatment group consisting of 50 participants receiving a combined treatment,while the control group,also comprising 50 participants,received conventional tuina.In the treatment group,the Yizhichan Pushing Manipulation was applied at a frequency of 120 times/min and a strength of 0.5-1.0 kg,targeting acupoints such as Ganyu and Guangming.The alterations in naked eye visual acuity,diopter measurements,and ciliary muscle regulation function were compared between the two groups of children both prior to and following treatment,and the clinical efficacy was assessed.[Results]The treatment group demonstrated a statistically significant enhancement in visual acuity(1.74±0.49 lines vs.1.02±0.52 lines),diopter improvement(0.91±0.29 D vs.0.46±0.25 D),and overall efficiency(96%vs.78%)when compared to the control group(all P<0.01).[Conclusions]This combined treatment exerts a therapeutic effect through a dual mechanism that involves the regulation of liver meridian qi and blood,as well as the enhancement of ciliary muscle function.This approach merits further dissemination and application in the prevention and management of myopia in children.

文摘BACKGROUND Prediabetes is a high-risk precursor for diabetes development and is associated with increased psychological distress.Conventional pharmacological treatments for prediabetes have limitations,including adverse effects and poor patient compliance.Tuina’s abdominal vibration technique,a traditional non-pharmacological intervention,has shown promising results in glycemic regulation.However,its effects on psychological well-being remain largely unexplored.AIM To investigate efficacy of abdominal vibration at varying frequencies in Tuina for alleviating psychological and metabolic effects in prediabetes patients.METHODS A prospective cohort study(April 2025 to April 2026)at Dongfang Hospital of Beijing University of Chinese Medicine included 120 prediabetes patients.Participants were allocated to three groups based on vibration frequency:(1)Lowfrequency(400 times/minute,n=40);(2)Medium-frequency(500 times/minute,n=40);and(3)High-frequency(600 times/minute,n=40).All participants received 30-minute sessions three times weekly for 3 months,with follow-up at 6 months and 12 months.Primary outcomes included changes in depression(Beck Depression Inventory-Ⅱ),anxiety(State-Trait Anxiety Inventory),and stress(Perceived Stress Scale)levels.RESULTS After a 3-month intervention period,with follow-ups at 6 months and 12 months,patients in all three groups showed significant improvements in depression,anxiety,and stress scores compared to baseline(P<0.001).The high-frequency group demonstrated the most substantial psychological improvements(mean reduction in depression scores:9.2±2.3 points;anxiety:8.7±2.1 points;stress:10.4±2.5 points).These psychological improvements correlated significantly with reductions in insulin resistance(r=0.68,P<0.001).The high-frequency group also showed the most significant improvements in glycemic parameters,with mean reductions in fasting plasma glucose,2-hour postprandial glucose,and glycosylated hemoglobin concentrations of 0.92 mmol/L,1.87 mmol/L,and 0.51%,respectively.Correlation analysis revealed significant associations between improved psychological parameters and enhanced glycemic control.CONCLUSION Tuina’s abdominal vibration technique,especially at high frequency reduces depression,anxiety,and stress in prediabetes patients,correlating with enhanced glycemic control and insulin sensitivity,suggesting a bidirectional relationship between psychological and metabolic health.
文摘Objective:To clarify the preferences and core demands of professional Master’s students in Tuina at Yunnan University of Chinese Medicine regarding“concentrated”versus“dispersed”rotation schedules in the Tuina department,as well as their preferred year for primary rotation,providing evidence for optimizing rotation programs and improving the quality of standardized training.Methods:Paper questionnaires,including two multiple-choice questions and one open-ended question,were distributed to 30 professional Tuina Master’s students,completed on site,and collected immediately.Descriptive statistics were used to analyze preference distributions.Results:A total of 30 questionnaires were distributed and 30 valid responses were collected,achieving a 100%response rate.Among them,23 students(76.7%)supported“concentrated training,”while seven students(23.3%)preferred“dispersed training.”Regarding rotation year preference,18 students(60.0%)selected the second year for concentrated Tuina rotations,eight students(26.7%)selected the third year,and four students(13.3%)selected the first year.The core demands supporting concentrated training were“skill continuity,deep involvement in diagnosis and treatment,and smooth mentorship transition,”whereas those supporting dispersed training were“integration of multi-department knowledge,coordination of research time,and avoidance of fatigue from a single department.”Conclusion:Most professional Tuina Master’s students prefer concentrated Tuina rotations in the second year.It is recommended that institutions adopt a“second-year core department concentrated+auxiliary department flexible dispersed”model and establish flexible coordination and feedback mechanisms to balance clinical skill development with individualized growth needs.

文摘Objective:To investigate the effects of“Three Methods and Three Acupoints”(TMTP)Tuina therapy on spinal microcirculation in sciatic nerve injury(SNI).Methods:Thirty-six SpragueeDawley rats were randomly assigned to four groups:normal,sham operation,model,and TMTP Tuina.Successful model induction was confirmed by observable hind limb lameness.After 20 sessions,hind limb grip strength and motor nerve conduction velocity(MNCV)were measured at baseline and following the 10th and 20th intervention.CD31 and a-SMA in the ventral horn of SNI model rats were detected using immunofluorescence.Motor neurons in the ventral horn were detected by Nissl staining.PTEN levels in the ventral horn were measured by ELISA,and PI3K,Akt,BDNF,VEGF,and HIF-1a expression was determined by RT-PCR.Spinal cord microcirculation was evaluated by western blotting analysis of the levels of Akt,p-Akt,BDNF,and VEGF.Results:Hind limb grip strength and MNCV significantly improved in the TMTP Tuina group compared to the model group(both P<.001).Morphology of ventral horn motor neurons in the TMTP Tuina group improved compared to the model group,with increased expressions of a-SMA(P=.002)and CD31(P=.006).Western blot analysis indicated increased expression of VEGF(P=.005),p-Akt(P<.001),and BDNF(P=.008)in the ventral horn following Tuina treatment.RT-PCR analysis revealed increased expression of PI3K,Akt,BDNF,VEGF and HIF-1a(all P<.05).In contrast,expression of PTEN decreased compared to the model group(P<.001).Conclusion:TMTP Tuina therapy may restore motor function in rats,enhance ventral horn motor neuron morphology,and promote angiogenesis and vascular smooth muscle proliferation.The mechanism may involve the activation of the PI3K/Akt signaling pathway.

文摘Objective To explore the clinical efficacy of tuina combined with moxibustion for treatment of lung-wei insecurity-type sweating syndrome in children.Methods Infantile tuina combined with moxibustion was conducted for 28 cases of lung-wei insecurity-type sweating syndrome in children.Tuina was carried out for once every other day,and moxibustion was conducted for once every other three days.Clinical efficacy was observed after treatment for 4 weeks.Results The total effective rate of tuina combined with moxibustion for treatment of lung-wei insecurity-type sweating syndrome in children was 92.85%.Conclusion The efficacy of tuina combined with moxibustion for treatment of lung-wei insecurity-type sweating syndrome in children is definite,which is worthy of being popularized clinically.
文摘Objective To discuss the clinical efficacy of treating pediatric allergic rhinitis by combiningTuina with Chinese herbal medicines. Methods Twentyeight cases of pediatric allergic rhinitis were treated by combining pediatric Tuine and oral administration of Yupingfeng powder (玉屏风散) combined with Guizhi decoction (桂枝汤). The clinical efficacy was observed after 6 treatments. Results The treatment of pediatric allergic rhinitis by combining Tuina and Chinese herbal medicines had an overall effectivness rate of 85.71%. Conclusion Combined treatment using pediatric Tuina and an herbal mixture of Yupingfeng powder (玉屏风散) and Guizhi decoction (桂枝汤) has positive therapeutic efficacy for treatment of pediatric allergic rhinitis, and should be considered as a treatment for this condition.
文摘Objective To explore the efficacy of Tuina for postpartum lactation and work out an optimal protocol involved. Methods With a randomized, controlled and clinical method, Eighty-four full-term primiparas were divided into a Tuina group (58 cases) and a control group (26 cases). While the patients in the control group received rooming in conventional nursing, those in the Tuina group were additionally treated with Tuina, including local manipulations at the breasts combined with acupoint manipulations. The colostrum time, lactation volume and serum prolactin were observed to make the comparisons between the two groups. Results The scores of lactation quantity after the 1st, 2nd, 3rd of the treatment were 1.660±0.785, 2.530±1.030, 2.880±1.171 in the Tuina group and 1.270±0.533, 1.460±0.811, 1.500±0.583 in the control group respectively, where there were significant differences in each time stage between the two groups (all P0.001). The time of colostrum was (21.6±10.5) h in the Tuina group and (22.5±9.7) h in the control group, in which the difference was not statistically significant (P0.05). The levels of prolactin were (314.35±110.37) ng/mL and (321.56±109.61) ng/mL in the Tuina group, (385.78±85.19) ng/mL and (340.12±103.10) ng/mL in the control group before and after treatment respectively, suggested that there were no significant difference (both P0.05). Conclusion Postpartum Tuina at the breasts could increase the quantity of lactation and delay the decreasing of the levels of prolactin, which contributes primiparas to lactate more and sooner.
文摘Objective To observe the therapeutic effect of quick-impact Tuina method on treatment of pediatric muscular torticollis.Methods Pushing,kneading,and grasping manipulations combined with traction and flicking method were applied at sternocleidomastoid on the affected side for 38 cases of pediatric muscular torticollis.The treatment was given 1 time a day,10 times as a course.The therapeutic effect was evaluated after 3 courses of treatment.Results Thirty-four cases was evaluated as full recovery,3 cases as remarkable effectiveness,1 case as effectiveness,0 case as ineffectiveness,the total effective rate was 100.0%.Conclusion The combined method treating pediatric muscular torticollis regulated tendons and relieved blood stasis,improved muscular spasm,and dispersed mass,so as to enhance and restore the regular neck function with significant therapeutic effect.

文摘OBJECTIVE: To demonstrate the analgesic effect of Tuina mainly from mechanically sensitive ion channels in peripheral myelinated nerve fibers.METHODS: A total of 40 healthy and pathogen-free adult male Sprague-Dawley rats were used in the study [weight:(220.0 ± 1.4) g, Shanghai Slac Laboratory Animal Co., Ltd., Shanghai, China; license No. Shanghai ICP 05033115]. The rats were housed in cages with free access to water and food in a temperature-controlled room [(22 ± 1) ℃] and12-h/12-h light-dark cycle. Thirty-two rats were randomly divided into five groups: naive, sham, chronic compression of dorsal root ganglion(CCD), Tuina(7 d) and Tuina(21 d). CCD rat model was established via unilateral DRG compression by"L"liked steel bar. Chinese Tuina treatment was accepted once per day. Behavior monitoring of paw withdrawal threshold(PWT) and paw withdrawal latency(PWL) were tested. The expression of Piezo1 and Piezo2 in myelinated nerve fiber were analyzed by immunohistochemistry and Western-blotting.RESULTS: There was a high expression of Piezo2 and a low expression of Piezo1 in the naive and CCD groups. In contrast, the expression of Piezo2 was down regulated and Piezo1 was increased after a period of Tuina. There was significant difference(P ≤ 0.05) between the groups.CONCLUSION: Our findings suggest that Tuina therapy can increase the expression of Piezo2 and decrease the expression of Piezo1 in the test rats.The different changes in the expressions of Piezo1 and Piezo2 may play an important role in alleviating CCD-induced allodynia and hyperalgesia.

文摘OBJECTIVE:To determine the clinical effect,treatment times,and rheoencephalogram changes in vertebral artery type cervical spondylosis patients treated with innovative Tuina manipulations.METHODS:One hundred and twenty six cervical spondylosis patients(vertebral artery type) were randomly divided into test and control groups.Patients in the test group were treated with innovative Tuina manipulations,while those in the control group were treated with the routine Tuina manipulations according to the textbook of Chinese Massage for Acupuncture and Moxibustion majors.The clinical effects,treatment times,clinical symptoms,and cerebral blood flow were measured.RESULTS:The response to the treatment was 100% in the test group and 88.71% in the control group.Patients in the test group required(7 ± 4) treatments before recovery,while those in the control group required(15 ± 7) treatments before recovery(P<0.05).The clinical symptoms exhibited greater improvement in the test group compared to the control group(P<0.05).There were no differences in cerebral blood flow between the two groups.CONCLUSION:Both innovative Tuina manipulations and routine Tuina manipulations produced satisfactory therapeutic results in vertebral artery type cervical spondylosis patients.However,the innovative manipulation was more effective in improving the functional symptoms,although there were no changes in the cerebral blood flow.

文摘OBJECTIVE:To investigate whether the Chinese massage system,Tuina,exerts analgesic effects in a rat model of chronic constriction injury(CCI)by remodeling the synaptic structure in the spinal cord dorsal horn(SCDH).METHODS:Sixty-nine male Sprague–Dawley rats were randomly and evenly divided into the normal group,sham group,CCI group,CCI+Tuina group,CCI+MK-801[an N-methyl D-aspartate receptor subtype 2B(NR2B)antagonist]group,and CCI+MK-801+Tuina group.The neuropathic pain model was established using CCI with right sciatic nerve ligation.Tuina was administered 4 d after CCI surgery,using pressing manipulation for 10 min,once daily.Motor function was observed with the inclined plate test,and pain behaviors were observed by the Von Frey test and acetone spray test.At 19 d after surgery,the L3-L5 spinal cord segments were removed.Glutamate,interleukin 1β(IL-1β),and tumor necrosis factor-α(TNF-α)levels were detected by enzyme-linked immunosorbent assay.The protein expression levels of NR2B and postsynaptic density protein-95(PSD-95)were detected by Western blot,and the synaptic structure was observed by transmission electron microscopy(TEM).RESULTS:CCI reduced motor function and caused mechanical and cold allodynia in rats,increased glutamate concentration and TNF-αand IL-1βlevels,and increased expression of synapse-related proteins NR2B and PSD-95 in the SCDH.TEM revealed that the synaptic structure of SCDH neurons was altered.Most of these disease-induced changes were reversed by Tuina and intrathecal injection of MK-801(P<0.05 or<0.01).For the majority of experiments,no significant differences were found between the CCI+MK-801 and CCI+MK-801+Tuina groups.CONCLUSIONS:Chinese Tuina can alleviate pain by remodeling the synaptic structure,and NR2B and PSD-95 receptors in the SCDH may be among its targets.

文摘BACKGROUND The most effective treatment for knee joint pain is total knee arthroplasty(TKA),but the risk of pain and swelling in patients after surgery is high.Ice application,ankle pump exercise and non-steroidal anti-inflammatory painkillers are the primary clinical treatments after surgery.However,long-term use of nonsteroidal anti-inflammatory pain relievers can easily cause gastrointestinal damage.Traditional Chinese medicine(TCM)ointments and tuina therapy integrate TCM and manipulation,which effectively promotes the penetration of TCM into the skin lesions,improves local blood circulation and inflammatory reaction and has good long-term effects on patients.AIM To evaluate the efficacy of TCM ointment combined with tuina therapy in the treatment of pain and swelling after TKA.METHODS The randomized controlled clinical trial enrolled 80 patients who underwent TKA via the same procedure.The patients were randomly divided among the treatment group(n=40)and the control group(n=40).The control group was given an analgesia pump in addition to oral painkillers as the postoperative intervention.The treatment group received TCM ointment with tuina therapy in addition to the analgesia pump and oral painkillers in the postoperative period.The following variables were recorded 3 d before surgery and 3 d,7 d and 14 d after surgery:Visual analogue scale(VAS)score;skin temperature;circumferences at 15 cm above and below the patella;maximum active knee flexion angle;and the knee injury and Osteoarthritis Outcome score(KOOS).RESULTS After treatment,VAS was significantly lower in the treatment group than the control group at 7 d(t=7.536,P<0.001)and 14 d(t=8.563,P<0.001).The skin temperature of participants in the treatment group was significantly lower than that in the control group at 7 d(t=2.968,P=0.004)and 14 d(t=4.423,P<0.001).The circumference values of the two positions in the treatment group were lower than those in the control group at 7 d[t=2.315,P=0.023(above);t=2.121,P=0.037(below)]and 14 d[t=2.374,P=0.020(above);t=2.095,P=0.039(below)].After 14 d of treatment,the maximum active knee flexion angle and KOOS of the two groups were significantly improved but were significantly higher in the treatment group(P<0.05 for both).CONCLUSION TCM ointment and tuina therapy have significant advantages over standard care in the treatment of pain and swelling after TKA.This additional treatment may improve knee function but additional studies are needed to confirm our observations.

文摘OBJECTIVE:To observe the analgesic effects of Tuina on neuropathic pain(NPP)and the underlying mechanisms.METHODS:Forty-eight Sprague-Dawley(SD)rats were assigned by random into three treatment groups:sham,chronic constriction injury(CCI),and Tuina.Each group contained sixteen rats.CCI model was generated by ligating the right sciatic nerve.Behavioral changes of CCI were assessed by the paw withdrawal threshold(PWT)and paw withdrawal latency(PWL).In addition,biochemical techniques such as immunofluorescence staining,enzyme-linked immunosorbent assay(ELISA)and Western blotting were used to profile levels of microglia activation and inflammatory factors in the spinal dorsal horn(SDH)of rats.Tuina(clockwise pressing and rubbing)was performed at Chengshan(BL57)to observe the analgesic effects on CCI rats and the underlying mechanisms.RESULTS:Rats with CCI experienced significant reduction in the PWT and PWL of the right hind paw relative to CCI group at day 3.Tuina treatment rescued this situation significantly on days 10 and 14.Besides,Iba-1,microglia M1 receptor CD68,tumor necrosis factor-α(TNF-α),and interleukin-1β(IL-1β)were higher in the right SDH for CCI group compared to the sham group on day 14.As expected,Tuina partially downregulated the CCI-induced overexpressed Iba-1,CD68,TNF-α,and IL-1βin the SDH of CCI model.CONCLUSION:Tuina induces a time-dependent cumulative analgesic effect in CCI rats by inhibiting the activation of microglia and the secretion of IL-1βand TNF-αin SDH.
