Cancer pain is one of the most prevalent and debilitating symptoms in patients with advanced malignancies,arising from multifactorial mechanisms involving peripheral,central,and systemic pathways.Conventional analgesics,including opioids and nonsteroidal anti-inflammatory drugs,are often limited by their insufficient efficacy,tolerance,and risk of dependence.Traditional Chinese Medicine(TCM),characterized by its multi-component,multi-target,and systemic regulatory properties,has shown promising potential in cancer pain management.This review provides a comprehensive overview of the clinical classification and underlying mechanisms of cancer pain(including nerve infiltration,dysregulation of inflammatory mediators and ion channels,central sensitization,neuro-immune crosstalk,metabolic reprogramming,and gut-brain axis impairment),as well as the analgesic effects of representative TCM agents in cancer pain management.For example,bioactive components such as tetrahydroberberine,levo-tetrahydropalmatine,and piperine exert analgesic effects,thereby improving the quality of life of patients by inhibiting inflammatory cascades,regulating neurotransmitter systems,and preserving neural integrity.Commonly used preclinical models,including bone cancer pain,pancreatic cancer pain,and chemotherapy-induced peripheral neuropathy models,are summarized for their utility in mechanistic studies and efficacy evaluations.This review also discusses the current limitations of clinical evidence,such as small sample sizes,short follow-up periods,and limited translation from animal models,alongside major challenges in standardization,mechanistic elucidation,and clinical trial design.Future directions should focus on precise pain phenotyping,integrated multi-target interventions,rigorous efficacy safety validation,and innovations in drug delivery to facilitate the standardization and global adoption of TCM in cancer pain management. 展开更多

Objective To develop a depression recognition model by integrating the spirit-expression diagnostic framework of traditional Chinese medicine(TCM)with machine learning algorithms.The proposed model seeks to establish a TCM-informed tool for early depression screening,thereby bridging traditional diagnostic principles with modern computational approaches.Methods The study included patients with depression who visited the Shanghai Pudong New Area Mental Health Center from October 1,2022 to October 1,2023,as well as students and teachers from Shanghai University of Traditional Chinese Medicine during the same period as the healthy control group.Videos of 3–10 s were captured using a Xiaomi Pad 5,and the TCM spirit and expressions were determined by TCM experts(at least 3 out of 5 experts agreed to determine the category of TCM spirit and expressions).Basic information,facial images,and interview information were collected through a portable TCM intelligent analysis and diagnosis device,and facial diagnosis features were extracted using the Open CV computer vision library technology.Statistical analysis methods such as parametric and non-parametric tests were used to analyze the baseline data,TCM spirit and expression features,and facial diagnosis feature parameters of the two groups,to compare the differences in TCM spirit and expression and facial features.Five machine learning algorithms,including extreme gradient boosting(XGBoost),decision tree(DT),Bernoulli naive Bayes(BernoulliNB),support vector machine(SVM),and k-nearest neighbor(KNN)classification,were used to construct a depression recognition model based on the fusion of TCM spirit and expression features.The performance of the model was evaluated using metrics such as accuracy,precision,and the area under the receiver operating characteristic(ROC)curve(AUC).The model results were explained using the Shapley Additive exPlanations(SHAP).Results A total of 93 depression patients and 87 healthy individuals were ultimately included in this study.There was no statistically significant difference in the baseline characteristics between the two groups(P>0.05).The differences in the characteristics of the spirit and expressions in TCM and facial features between the two groups were shown as follows.(i)Quantispirit facial analysis revealed that depression patients exhibited significantly reduced facial spirit and luminance compared with healthy controls(P<0.05),with characteristic features such as sad expressions,facial erythema,and changes in the lip color ranging from erythematous to cyanotic.(ii)Depressed patients exhibited significantly lower values in facial complexion L,lip L,and a values,and gloss index,but higher values in facial complexion a and b,lip b,low gloss index,and matte index(all P<0.05).(iii)The results of multiple models show that the XGBoost-based depression recognition model,integrating the TCM“spirit-expression”diagnostic framework,achieved an accuracy of 98.61%and significantly outperformed four benchmark algorithms—DT,BernoulliNB,SVM,and KNN(P<0.01).(iv)The SHAP visualization results show that in the recognition model constructed by the XGBoost algorithm,the complexion b value,categories of facial spirit,high gloss index,low gloss index,categories of facial expression and texture features have significant contribution to the model.Conclusion This study demonstrates that integrating TCM spirit-expression diagnostic features with machine learning enables the construction of a high-precision depression detection model,offering a novel paradigm for objective depression diagnosis. 展开更多

Targeting tumor angiogenesis with traditional Chinese medicine:mechanisms,challenges,and future directions

Tumor growth depends on neoangiogenesis,and current anti-angiogenic therapies face limitations owing to side effects and drug resistance.Traditional Chinese medicines(TCM)have shown potential to inhibit tumor angiogenesis owing to their multi-component and multi-target properties.This review focuses on the mechanisms of action of TCM monomers(e.g.,curcumin,tanshinone IIA,and ginsenoside)through the modulation of key pathways,such as HIF-1α,VEGF/VEGFRs,and Notch signaling,and explores their modulatory effects on the tumor microenvironment(e.g.,tumor-associated macrophage polarization).Despite the low toxicity and multi-targeting advantages of TCM,their clinical translation is still limited by low bioavailability and standardization difficulties.In this review,we further analyze the role of modern approaches,such as nanocarriers(liposomes,polymer nanoparticles,metal-coordinated nanomaterials,and plant exosome-like vesicles),in overcoming these bottlenecks and propose a strategy of combining TCM with chemotherapy,radiotherapy,and immunotherapy to overcome drug resistance and enhance therapeutic efficacy.By integrating traditional wisdom with cutting-edge technologies,anti-angiogenesis research in TCM is expected to contribute to the development of safer and more effective individualised cancer treatment. 展开更多

Background:Cyperi Rhizoma,derived from Cyperus rotundus L.,is a widely used medicinal herb in traditional Chinese medicine(TCM),with Shandong Province recognized as its geo-authentic habitat.However,the quality of Cyperi Rhizoma varies significantly across different regions,potentially influencing its therapeutic efficacy.This study investigates the influence of geographic origin on the chemical composition and overall quality of Cyperi Rhizoma.Methods:A comprehensive approach,including traditional quality assessment,GC-MS(g as c hromatography-m ass s pectrometry),RP-HPLC(r everse p hase h igh-p erformance l iquid c hromatography),and FT-IR(f ourier t ransform i nfrared s pectroscopy)techniques,was employed to analyze Cyperi Rhizoma samples from Shandong Province.These methods examined the physical appearance,chemical profile,and content variations,particularly focusing onα-cyperone.Results:Traditional quality assessments revealed noticeable differences in the external characteristics of the samples.GC-MS analysis identified a variety of unique chemical constituents,while RP-HPLC and FT-IR showed significant variations inα-cyperone content,with higher levels found in Shandong samples.Conclusion:These results demonstrate that geographic origin is a critical determinant of Cyperi Rhizoma quality,with Shandong specimens exhibiting superiorα-cyperone levels and characteristic phytochemical profiles.This validates the geo-authenticity concept in TCM and provides actionable data for developing evidence-based quality standards,suggesting that provenance should be prioritized in medicinal material selection and pharmacopeial specifications. 展开更多

Potentilla discolor Bunge, a traditional Chinese medicinal herb, has been extensively utilized for treating diverse ailments including diabetes, bacterial infections, viral diseases, and inflammatory disorders. Modern phytochemical investigations have identified over 200 bioactive compounds, predominantly flavonoids, tannins, triterpenoids, and phenolic acids. Pharmacological studies demonstrate that P. discolor exerts multi-target therapeutic effects through antioxidant, anti-inflammatory, antimicrobial, antiviral, antidiabetic, and anticancer activities. Its mechanisms involve modulation of key signaling pathways including PI3K/Akt, MAPK, NF-κB, and Nrf2/HO-1, alongside regulation of glucose-lipid metabolism and gut microbiota. This review systematically summarizes the phytochemistry, traditional applications, and contemporary pharmacological advances of P. discolor, emphasizing its multi-target characteristics and molecular mechanisms. Furthermore, current research limitations and future perspectives are discussed to provide scientific evidence for its clinical development and therapeutic applications. 展开更多

Objective: To investigate the current status of inheritance and development of folk traditional Chinese medicine in Linfen City, analyze practitioner characteristics, inheritance modes, and social acceptance, and to provide references for standardized management and rational development of folk traditional Chinese medicine. Methods: A questionnaire-based survey was conducted from January to December 2025 among folk traditional Chinese medicine practitioners, medical staff, and urban and rural residents in Linfen City. The survey content included basic characteristics of practitioners, inheritance modes, practice status, and levels of recognition of folk traditional Chinese medicine among different population groups. An Excel database was established, and descriptive statistical analysis was performed using SPSS version 26.0. Results: A total of 349 participants were surveyed, including 99 folk traditional Chinese medicine practitioners, 100 medical staff members, and 150 urban and rural residents. Most practitioners were middle-aged or elderly, had more than 10 years of practice experience, primarily inherited their skills through apprenticeship or family transmission, and had a relatively low proportion of systematic theoretical training in traditional Chinese medicine. Residents showed a high level of acceptance of folk traditional Chinese medicine, whereas medical staff demonstrated a relatively cautious attitude. Conclusion: Folk traditional Chinese medicine in Linfen City has a certain social foundation and practical value;however, further improvements are needed in standardized inheritance, management mechanisms, and sustainable development. 展开更多

This paper divides the centuries-long communication of traditional Chinese medicine(TCM)in France into three progressive stages:the first stage(17th to 19th centuries),characterized by academic recognition and knowledge transmission;the second stage(mid-19th century to late 20th century),focusing on clinical verification and school-based innovation;and the third stage(late 20th century to the present),marked by cultural integration and pluralistic coexistence within systems.The introduction of TCM in France is a process of cross-cultural adaptation and coexistence driven by specific health needs in French society,such as chronic disease management and complementary therapies.The French medical community and sociocultural context have not only verified and interpreted TCM through an empirical scientific perspective,but also realized creative localization by developing indigenous schools such as electroacupuncture and auricular acupuncture.Eventually,through institutional measures including national certification,terminology standardization,and networked healthcare services,TCM has been deeply integrated into France’s medical system and social context,forming a stable ecosystem of cross-cultural adaptation and pluralistic coexistence.The French experience,which takes aligning with local health needs as the starting point,stimulating localized cultural innovation as the core,and promoting systematic integration into local systems as the support,presents a picture of traditional medicine serving modern society,the improvement of global health,as well as mutual learning and symbiosis among different civilizations. 展开更多

Objective To develop a dual-branch deep learning framework for accurate multi-label classification of fundus diseases,addressing the key limitations of insufficient complementary feature extraction and inadequate cross-modal feature fusion in existing automated diagnostic methods.Methods The fundus multi-label classification dataset with 12 disease categories(FMLC-12)dataset was constructed by integrating complementary samples from Ocular Disease Intelligent Recognition(ODIR)and Retinal Fundus Multi-Disease Image Dataset(RFMiD),yielding 6936 fundus images across 12 retinal pathology categories,and the framework was validated on both FMLC-12 and ODIR.Inspired by the holistic multi-regional assessment principle of the Five Wheels theory in traditional Chinese medicine(TCM)ophthalmology,the dualbranch multi-label network(DBMNet)was developed as a novel framework integrating complementary visual feature extraction with pathological correlation modeling.The architecture employed a TransNeXt backbone within a dual-branch design:one branch processed redgreen-blue(RGB)images to capture color-dependent features,such as vascular patterns and lesion morphology,while the other processed grayscale-converted images to enhance subtle textural details and contrast variations.A feature interaction module(FIM)effectively integrated the multi-scale features from both branches.Comprehensive ablation studies were conducted to evaluate the contributions of the dual-branch architecture and the FIM.The performance of DBMNet was compared against four state-of-the-art methods,including EfficientNet Ensemble,transfer learning-based convolutional neural network(CNN),BFENet,and EyeDeep-Net,using mean average precision(mAP),F1-score,and Cohen's kappa coefficient.Results The dual-branch architecture improved mAP by 15.44 percentage points over the single-branch TransNeXt baseline,increasing from 34.41%to 44.24%,and the addition of FIM further boosted mAP to 49.85%.On FMLC-12,DBMNet achieved an mAP of 49.85%,a Cohen’s kappa coefficient of 62.14%,and an F1-score of 70.21%.Compared with BFENet(mAP:45.42%,kappa:46.64%,F1-score:71.34%),DBMNet outperformed it by 4.43 percentage points in mAP and 15.50 percentage points in kappa,while BFENet achieved a marginally higher F1-score.On ODIR,DBMNet achieved an F1-score of 85.50%,comparable to state-of-the-art methods.Conclusion DBMNet effectively integrates RGB and grayscale visual modalities through a dual-branch architecture,significantly improving multi-label fundus disease classification.The framework not only addresses the issue of insufficient feature fusion in existing methods but also demonstrates outstanding performance in balancing detection across both common and rare diseases,providing a promising and clinically applicable pathway for standardized,intelligent fundus disease classification. 展开更多

Serbisütherapy(ST)is a distinctive external treatment modality within traditional Mongolian medicine(TMM),historically developed within a nomadic cultural framework.This study presents a comprehensive philological and historical analysis of ST,tracing its evolution from early battlefield applications to contemporary clinical use.By critically examining classical Mongolian medical texts alongside modern case studies,we aim to systematize ST’s therapeutic methods,indications,and limitations,while exploring its mechanisms of action through both traditional theory and modern biomedical perspectives.ST has undergone significant transformation,shifting from whole-body cavity immersion in the 13th century to targeted,organ-specific applications in modern practice.Its four primary methods–Covering,Mounted,Organ Placement,and Suction–demonstrate efficacy in treating cold-natured diseases,musculoskeletal disorders,gynecological conditions,and certain emergencies.ST embodies the core principles of TMM,particularly the balance of the“Three Roots”and the correction of cold-induced pathologies through heat.Despite challenges related to standardization,cultural translation,and regulatory acceptance,ST holds translational potential for integrative medicine.Future research should prioritize mechanistic validation,clinical standardization,and the development of biocompatible thermal technologies to bridge traditional practice with modern healthcare systems. 展开更多

The global prevalence of metabolic-associated fatty liver disease(MAFLD)is on the rise,seriously threatening human health.Currently,no specific approved drugs are available for its treatment.This paper reviews the pathogenesis of MAFLD,covering aspects like lipid accumulation and insulin resistance,oxidative stress,endoplasmic reticulum stress(ERS),lipotoxicity-induced hepatocyte damage,and fibrosis.It also elaborates on multiple treatment approaches for MAFLD,including metabolic regulation,improvement of the gut-liver axis interaction,modulation of immune and inflammatory pathways,enhancement of the adipose tissue-liver interaction,alleviation of fibrosis,prevention of hepatocyte injury,and traditional Chinese medicine(TCM)external therapies.Additionally,natural product research advancements,individual Chinese medicine components,and mixed herbal formulas for MAFLD treatment is provided.Many natural products and traditional Chinese medicines exhibit favorable effects in regulating lipid metabolism,anti-inflammation,and anti-oxidation,offering new directions and potential drug options for MAFLD treatment.This is expected to provide a reference for future clinical treatment and drug development. 展开更多

The disruption of gut microbiota and excessive immune responses are commonly observed in various respiratory conditions,including Corona virus disease 2019(COVID-19).As supported by the theories of“treating the lung diseases from the large intestine(从肠治肺)”and“simultaneous treatment of lung and large intestine(肺肠同治)”,TCM(traditional Chinese medicine)has made significant contributions to the management of pneumonia,acute respiratory distress syndrome(ARDS),chronic obstructive pulmonary disease(COPD),asthma,and other respiratory conditions,demonstrating remarkable clinical effectiveness.Thus,our review sought to investigate the underlying material basis and scientific implications of this theory,specifically focusing on the regulation of gut microbiota,thus providing new ideas for clinical treatment.We reviewed the therapeutic efficacy of TCM in treating lung diseases using the“treating the lung diseases from the large intestine”and“simultaneous treatment of lung and large intestine”methods.The mechanisms of action(MOAs)of TCM were also summarized.TCM can protect the abundance and inhibit the translocation of intestinal microbiota caused by lung diseases,exhibiting significant anti-inflammatory roles,etc.Additionally,TCM can regulate metabolic process,e.g.,promoting the production of anti-inflammatory short-chain fatty acids(SCFAs).Meanwhile,Gut microbiota might be an important regulatory target for the therapeutic role of TCM to treat lung diseases. 展开更多

Chemotherapy-induced diarrhea(CID)is a major concern for cancer patients and is associated with significant morbidity and mortality.Currently,the clinical management of CID is limited.The utilization of antidiarrheal medications,such as loperamide and octreotide,is relatively limited because of their unsatisfactory efficacy and adverse effects.In recent years,traditional Chinese medicine(TCM)has attracted great interest because of its beneficial effect in treating CID,which has multitarget and low-toxicity therapeutic characteristics.TCM exhibits remarkable therapeutic potential in the prevention and treatment of CID.It can alleviate and treat CID by regulating chemical drug metabolism,improving the integrity of the intestinal barrier,stimulating proliferation while suppressing the apoptosis of intestinal epithelial cells,ameliorating oxidative stress and inflammation and regulating bile acids and aquaporins.However,large-scale,randomized,double-blind clinical trials of TCM for the treatment of CID are lacking,and most preclinical experiments have not been translated to clinical trials.Accordingly,this review highlights the clinical efficacy and molecular mechanisms of TCM against CID via PubMed,Web of Science and China National Knowledge Infrastructure and proposes that future research on TCM against CID should focus on strengthening the connection from bench to bed,which may help to comprehensively evaluate the therapeutic potential of TCM against CID. 展开更多
