Parkinson’s disease(PD)is a common neurodegenerative disorder with increasing incidence and disability rates globally,placing a heavy burden on patients and their families.In the prodromal phase of PD,nonmotor symptoms,particularly depression and sleep disorders,are frequent,with profound effects on disease progression and patient quality of life.Emerging research highlights the critical role of inflammatory markers-including interleukins and tumor necrosis factor-in the pathogenesis of prodromal PD.These inflammatory mediators participate in neurodegenerative processes and may induce or exacerbate depressive symptoms and sleep disorders by disrupting the function of the hypothala-micpituitary-adrenal axis and affecting neurotransmitter,including serotonin,metabolism.Understanding their correlations with nonmotor symptoms in prodromal PD remains incomplete,limiting our ability to develop targeted interventions.This comprehensive review aims to investigate the specific correlations between inflammatory markers and nonmotor symptoms-particularly depression and sleep disorders-in prodromal PD.The findings could have important practical applications,potentially leading to the development of new diagnostic tools and therapeutic strategies for managing PD.By identifying and understanding these correlations,healthcare providers may better predict disease progression and implement more effective treatments for nonmotor symptoms in PD. 展开更多

BACKGROUND Mild cognitive impairment(MCI)has a high risk of progression to Alzheimer’s disease.The disease is often accompanied by sleep disorders,and whether sleep disorders have an effect on brain function in patients with MCI is unclear.AIM To explore the near-infrared brain function characteristics of MCI with sleep disorders.METHODS A total of 120 patients with MCI(MCI group)and 50 healthy subjects(control group)were selected.All subjects underwent the functional near-infrared spec-troscopy test.Collect baseline data,Mini-Mental State Examination,Montreal Cognitive Assessment scale,fatigue severity scale(FSS)score,sleep parameter,and oxyhemoglobin(Oxy-Hb)concentration and peak time of functional near-infrared spectroscopy test during the task period.The relationship between Oxy-RESULTS Compared with the control group,the FSS score of the MCI group was higher(t=11.310),and the scores of Pittsburgh sleep quality index,sleep time,sleep efficiency,nocturnal sleep disturbance,and daytime dysfunction were higher(Z=-10.518,-10.368,-9.035,-10.661,-10.088).Subjective sleep quality and total sleep time scores were lower(Z=-11.592,-9.924).The sleep efficiency of the MCI group was lower,and the awakening frequency,rem sleep latency period,total sleep time,and oxygen desaturation index were higher(t=5.969,5.829,2.887,3.003,5.937).The Oxy-Hb concentration at T0,T1,and T2 in the MCI group was lower(t=14.940,11.280,5.721),and the peak time was higher(t=18.800,13.350,9.827).In MCI patients,the concentration of Oxy-Hb during T0 was negatively correlated with the scores of Pittsburgh sleep quality index,sleep time,total sleep time,and sleep efficiency(r=-0.611,-0.388,-0.563,-0.356).It was positively correlated with sleep efficiency and total sleep time(r=0.754,0.650),and negatively correlated with oxygen desaturation index(r=-0.561)and FSS score(r=-0.526).All comparisons were P<0.05.CONCLUSION Patients with MCI and sleep disorders have lower near-infrared brain function than normal people,which is related to sleep quality.Clinically,a comprehensive assessment of the near-infrared brain function of patients should be carried out to guide targeted treatment and improve curative effect. 展开更多

Severe anxiety, depression, and sleep disorders worsen prognosis in patients with septic shock

Recent studies have revealed that patients with septic shock and acute skin failure(ASF)exhibit significantly higher levels of inflammatory cytokines,lactate,and C-reactive protein,along with elevated skin mottling score;modified early warning score(MEWS);and anxiety,depression,and Pittsburgh sleep quality index scores compared to those without ASF.Notably,these indicators are significantly correlated.Patients with septic shock accompanied by ASF tend to have a mean MEWS greater than 9,indicating severe disease progression.Therefore,it is ess-ential to not only manage septic shock but also mitigate anxiety,depression,sleep disorders,and disturbances of consciousness.This article explores the impact of severe anxiety,depression,and sleep disorders on the prognosis of septic shock and discusses intervention strategies. 展开更多

BACKGROUND Doctoral students often encounter mental health challenges,including stress,anxiety,depression,and sleep disorders.It is important to explore effective intervention methods to enhance their overall physical and mental well-being.It is anticipated that targeted exercise will lead to a significant reduction in stress,anxiety,and depression levels as well as an improvement in sleep quality.AIM To assess the feasibility and potential benefits of both intervention models in enhancing the sleep quality of doctoral students while alleviating stress,anxiety,and depression.METHODS A retrospective analysis of health data from 64 doctoral students across three universities in Shenyang during the 2024-2025 academic year was conducted.The participants were divided into a targeted exercise group and a Tai Chi group.The study employed the Epworth Sleepiness Scale,Insomnia Severity Index,Pittsburgh Sleep Quality Index,Perceived Stress Scale-10,Generalized Anxiety Disorder Scale-7,and Patient Health Questionnaire-9 to evaluate the impact of the two interventions on reducing stress,anxiety,depression,and sleep disorders.RESULTS The primary results of the study indicated that targeted exercise interventions are significantly effective in alleviating symptoms of anxiety,stress,and depression,as well as in improving sleep quality.Compared to Tai Chi interventions,this approach demonstrates greater durability of effects.Although the efficacy of targeted interventions may gradually diminish over time,the overall research findings suggest that targeted exercise remains a more effective therapeutic approach than Tai Chi interventions.CONCLUSION The impact of targeted exercise on stress,anxiety,depression,and sleep disorders was greater than that of Tai Chi,confirming the potential benefits for psychological health intervention for doctoral students. 展开更多

Intervention and mechanism of Xiaoyin Anshen Yin(消银安神饮)in treatment of psoriasis combined with sleep disorders

OBJECTIVE:To explore the therapeutic mechanisms of Xiaoyin Anshen Yin( 消银安神饮, XYAS) in treating psoriasis associated with sleep focusing on melatonin and the regulation of the nuclear factor kappa-B(NF-κB) pathway. METHODS:Forty Sprague-Dawley rats were randomly divided into four groups, and administered distilled water, XYAS and its two different disassembly prescriptions by gavage respectively. Four types of drug-containing serums corresponding to the four groups were then prepared. Tumor necrosis factor(TNF)-α stimulated Ha Ca T was used to establish a psoriasis cell model, and the serums and the retinoid related orphan receptor alpha(RORα) inverse agonist were used respectively to intervene in the model. Enzyme-linked immunosorbent assay was used to detect the levels of interleukin(IL)-6 and melatonin in each group;flow cytometry was used to detect the levels of reactive oxygen species(ROS), mitochondrial membrane potential, and apoptosis;Western blot was used to evaluate the levels of superoxide dismutase 2(SOD2), cytochrome-c(Cyt-c), inhibitor of kappa-B alpha(IκBα), p65 and phosphorylated p65. RESULTS:XYAS and its disassembly prescriptions inhibited the secretion of inflammatory factors such as IL-6, reduced the ROS content and Cyt-c expression, increased the mitochondrial membrane potential and SOD2 content, promoted the apoptosis in Ha Ca T cells and inhibited the activation of the NF-κB pathway. XYAS was also found increase the melatonin content. The above effects are beneficial in the treatment of psoriasis combined with sleep disorders. Meanwhile, XYAS no longer had a significant ameliorative effect after applying the RORα inverse agonist, suggesting that the therapeutic effect of XYAS is related to RORα. CONCLUSIONS:The results of this study confirm that XYAS can be utilized for the treatment of psoriasis combined with sleep disorders via inhibiting the NF-κB pathway, anti-inflammatory, antioxidant and proapoptotic, which is in part related to the regulatory role of melatonin and its receptor RORα. 展开更多

As one of the most prevalent malignant tumors,hepatocellular carcinoma(HCC)represents a major global public health burden.Traditionally,HCC pathogenesis has been attributed to chronic liver diseases(viral hepatitis,cirrhosis)and aflatoxin exposure.However,with evolving lifestyles and environmental changes,sleep disorders have become increasingly prevalent.Emerging evidence suggest that sleep disorders may contribute to hepatocarcinogenesis through multiple mechanisms,including immunity environment disorder,oxidative stress,metabolic dysregulation,disruption of gut microbiota,and circadian rhythm disruption,thereby influencing disease progression and patient prognosis.This review summarizes epidemiological evidence on the relationship between sleep disorders and HCC incidence,explores the underlying mechanisms through which sleep disorders contribute to HCC,and discusses clinical challenges and potential intervention strategies.Our objective is to provide novel insights into HCC prevention and therapeutic approaches. 展开更多

Background:Social media addiction,one of the behavioural addictions,is a significant predictor of job performance.It has also been posited that individuals whose fundamental requirements(e.g.,sleep)are not sufficiently met andwho lack adequate support(e.g.,perceived social support)are incapable of effectivelyharnessing theirpotential.The primary objective of this study is to examine themediating effects of sleep disorder and perceived social support on the relationship between social media addiction and job performance.Furthermore,it seeks to explore the moderating effects of perceived social support on sleep disorders and job performance.Methods:The data were collected through the questionnairemethod,and data analysis was performed using SPSS 26.0.Moreover,statistical analysis encompasses correlation analysis,mediation,and moderation analysis.The data were gathered from 488 healthcare professionals(57.2%female),whose ages ranged from 24 to 56 years(Meanage±SD=37.86±6.71),using a convenience sample approach.Results:The results revealed significant relationships between social media addiction,job performance,perceived social support,and sleep disorder.The findings indicate that social media addiction negatively predicts job performance(β=−0.11,p<0.05).Sleep disorder(effect size=−0.02,95%CI=[−0.04,−0.00])and perceived social support(effect size=−0.01,95%CI=[−0.02,−0.00])mediate this relationship.Furthermore,perceived social supportmoderates the pathway between sleep disorder and job performance(index ofmoderatedmediation:−0.0040,95%CI=[−0.0070,−0.0010]).Conclusions:This study suggests that social media addiction negatively affects job performance through sleep disorders and perceived social support among healthcare professionals.The study’s findings are significant,as they suggest that treatments aimed at alleviating sleep disorders and enhancing perceived social support among medical workers may improve their job performance. 展开更多

BACKGROUND Non-operative spinal infections(NOSI)are caused by tuberculosis,brucella,and other specific bacteria.The etiology of the disease is insidious,the onset is slow and the diagnosis and treatment are difficult.Identifying the factors associated with spinal infection and early intervention can reduce the occurrence of the disease.At present,the research mainly focuses on the accurate diagnosis and treatment of spinal infection,and there are few studies on the prevention of spinal infection.The concept of"preventive treatment of diseases"in traditional Chinese medicine may help identify the causes and reduce the occurrence of NOSI.AIM To determine the association of age,bowel movements,and sleep patterns with NOSI.METHODS Data of 69 NOSI patients and 84 healthy controls in a tertiary hospital from January 2019 to June 2024 were collected.Patients with NOSI had imaging evidence(magnetic resonance imaging)of spinal infections(including infections caused by tuberculosis,brucopathy,and other pathogens)and had no history of spinal surgery in the last 1 year were included in the analysis.Patients with spinalinfection due to spinal surgery are excluded in the study.Data including age,sex,place of residence,sleeping status,and bowel movements were collected.SPSS22.0 was used for correlation analysis of all data.RESULTS The mean age of the NOSI group and the control group was 63.55±14.635 years and 59.18±17.111 years,respectively,without statistical difference(P=0.096).There was also no statistically significant difference in gender between the two groups.In the NOSI group,45(65.22%)were over 60 years old,and 44(63.77%)were rural residents.Compared with the control group,the NOSI group had more patients with sleep disorder and defecation disorder,accounting for 69.57%and 68.12%,respectively,with significant statistical difference(both P<0.001).Regression analysis showed that defecation and sleep disorders were closely related to NOSI(both P<0.001).CONCLUSION Most patients with NOSI are older and have sleep disorders and abnormal defecation. 展开更多

Objective:To explore the core acupuncture acupoints and pattern-adapted acupoint combination rules for autism spectrum disorder(ASD)complicated with sleep disorder using clinical data mining technology.Methods:A retrospective analysis was conducted on the diagnosis and treatment data of 104 children with ASD complicated with sleep disorder admitted to Xi’an Traditional Chinese Medicine(TCM)Encephalopathy Hospital from January 2022 to December 2024.Cross-pattern main acupoints were screened via frequency statistics,chi-square test,and factor analysis;pattern-specific auxiliary acupoints were extracted by combining multiple correspondence analysis,cluster analysis,and association rule mining.Results:Ten cross-pattern main acupoints(Baihui,Sishenzhen,Language Area 1,Language Area 2,Neiguan,Shenmen,Yongquan,Xuanzhong)were identified,and acupoint combination schemes for four major TCM patterns(Hyperactivity of Liver and Heart Fire,Deficiency of Kidney Essence,Deficiency of Both Heart and Spleen,Hyperactivity of Liver with Spleen Deficiency)were established.Conclusion:Acupuncture treatment should follow the principle of“regulating spirit and calming the brain as the root,and dredging collaterals based on pattern differentiation as the branch”.The synergy between main and auxiliary acupoints can accurately regulate the disease,providing a basis for precise clinical treatment. 展开更多

Methods Seventy-six patients suffering from sleep disorder in depression were randomly divided into an observation group (39 cases) and a control group (37 cases). In observation group, acupuncture at Baihui (百会 GV 20), Shangxing (上星 GV 23), Shenting (神庭 GV 24), Fengchi (风池 GB 20) and Shenmen (神门 HT 7), etc., which was combined with ear point embedding, and in control group, Fluoxetine Hydrochloride Dispersible tablet (Prozac) was orally given. Four weeks of treatments were given as a therapeutic course in both groups. Comparison of changes in scores of HAMD and PSQI was made between the two groups. Results After treatment, HAMD scores were obviously lowered in both groups (both P〈0.01), and the reduction in HAMD score was more obvious in observation group than that in control group (P〈0.05). After treatment, the reduction in PSQI score was more obvious in observation group (P〈0.01), it was more significantly decreased than that in control group. Conclusion Acupuncture combined with ear point embedding provids remarkable therapeutic effects in treating sleep disorder in depression, which is superior to those provided by Fluoxetine Hydrochloride Dispersible Fablet orally given. 展开更多

Studies have shown that discrepancies can occur between subjective data regarding sleep disturbances and the impact of insomnia and objective assays, and surrogate markers of sleep and sleep disturbances. Both objective and subjective measures should be incorporated into clinic studies. It seems likely that sleep quality is represented by a combination of more than one subjective sleep parameter. Objective and subjective assessments of sleep quality may relate to different parameters. Future studies incorporated both subjective and objective measures could help to address the sleep disorders. 展开更多

Sleep disorders are common in patients with Alzheimer’s disease,and can even occur in patients with amnestic mild cognitive impairment,which appears before Alzheimer’s disease.Sleep disorders further impair cognitive function and accelerate the accumulation of amyloid-βand tau in patients with Alzheimer’s disease.At present,sleep disorders are considered as a risk factor for,and may be a predictor of,Alzheimer’s disease development.Given that sleep disorders are encountered in other types of dementia and psychiatric conditions,sleep-related biomarkers to predict Alzheimer’s disease need to have high specificity and sensitivity.Here,we summarize the major Alzheimer’s disease-specific sleep changes,including abnormal non-rapid eye movement sleep,sleep fragmentation,and sleep-disordered breathing,and describe their ability to predict the onset of Alzheimer’s disease at its earliest stages.Understanding the mechanisms underlying these sleep changes is also crucial if we are to clarify the role of sleep in Alzheimer’s disease.This paper therefore explores some potential mechanisms that may contribute to sleep disorders,including dysregulation of the orexinergic,glutamatergic,andγ-aminobutyric acid systems and the circadian rhythm,together with amyloid-βaccumulation.This review could provide a theoretical basis for the development of drugs to treat Alzheimer’s disease based on sleep disorders in future work. 展开更多

Methods: A total of 2,411 grass-root military personnel were randomly selected by cluster sampling, and administered the Chinese Military Personnel Sleep Disorder Scale, Military Acute Stress Scale and Social Support Rating Scale.Results: The total score of acute stress scale was positively correlated with the total score and factor scores of sleep disorder scale(r=0.209~0.465, P【0.01); The total score of social support scale was positively correlated with the total score of acute stress scale and the total score and factor scores of sleep disorder scale(r=0.356~0.537, P【0.01). The analysis of mediating effects showed that lack of social support partially mediated between acute stress and the factors of sleep disorder. The analysis of structural equation model showed that acute stress not only had a direct effect on sleep disorder(the path coefficient was 0.29, P=0.000), but also on lack of social support(the path coefficient was 0.39, P=0.000); lack of social support had a direct effect on sleep disorder(the path coefficient was 0.48, P=0.000).Conclusions: Acute stress and lack of social support are two significant factors of sleep disorder in grass-root military personnel. Well-established social support could alleviate sleep disorder induced by acute stress. Lack of social support was a partial mediator between acute stress and sleep disorder. 展开更多

Exercise-with-melatonin therapy has complementary and synergistic effects on spinal cord injury and Alzheimer's disease,but its effect on stroke is still poorly understood.In this study,we established a rat model of ischemic stroke by occluding the middle cerebral artery for 60 minutes.We treated the rats with exercise and melatonin therapy for 7 consecutive days.Results showed that exercise-with-melatonin therapy significantly prolonged sleep duration in the model rats,increased delta power values,and regularized delta power rhythm.Additionally,exercise-with-melatonin therapy improved coordination,endurance,and grip strength,as well as learning and memory abilities.At the same time,it led to higher hippocampal CA1 neuron activity and postsynaptic density thickness and lower expression of glutamate receptor 2 than did exercise or melatonin therapy alone.These findings suggest that exercise-withmelatonin therapy can alleviate sleep disorder and motor dysfunction by increasing glutamate receptor 2 protein expression and regulating hippocampal CA1 synaptic plasticity. 展开更多

Research studies have shown the association between sleep disorders and day time cognitive impairment, behavioral problems, poor school performance and inattention in children. OBJECTIVE: To analyze part of the mechanism of electroacupuncture on Sishencong(EX-HN1) for strokerelated sleep disorders(SSD) and post-stroke cognitive impairment(PSCI). METHODS: Using a randomized controlled trial(RCT) design, 72 patients were assigned to the electroacupuncture(EA) group or the sham acupuncture(SA) group. A healthy control(HC) group was also included. Both groups were given routine rehabilitation treatment. Then, patients in the EA group were given additional electroacupuncture at Sishencong(EX_HN1). Meanwhile, patients in the SA group were given a flat-head needle sham/placebo treatment placed at the bilateral Jianyu (LI15) and Binao(LI14) line midpoints and the Jianyu(LI15) and Jianzhen(SI9) line midpoints. Before and after treatment, scales were collected and analyzed. In the second phase of the study, some subjects from the EA group were selected for functional magnetic resonance imaging(f MRI) data acquisition and comparative analysis with the HC group using a non-RCT design. RESULTS: The EA group performed better than the SA group on the Pittsburgh sleep quality index(PSQI), Montreal cognitive assessment basic(Mo CA_B), selfrating anxiety scale(SAS), and self-rating depression scale(SDS). Analysis of the f MRI showed that lowfrequency(2 Hz) electroacupuncture stimulation at Sishencong(EX_HN1) can restrain frontal sup medial right(SFGmed.R), precuneus right(PCUN.R), and posterior cingulate cortex right(PCC.R) and enhance angular left(ANG.L), parietal inf left(IPL.L) and occipital mid left(MOG.L). The functional connectivity(FC) of SFGmed.R was positively correlated with PSQI. Electroacupuncture stimulation at Sishencong(EX_HN1) can reduce the side efficiency of the whole brain connection with the Thalamus.L, Hippocampus.L, and Occipital.Mid.L. CONCLUSIONS: Low frequency(2 Hz) electroacupuncture stimulation at Sishencong(EX_HN1) can simultaneously improve sleep quality, negative emotions, and cognitive functions, the first two of which may be related to SFGmed.R restraint. Electroacupuncture can make some brain areas approach the physiological bias state, which is characterized by dominant hemispheric enhancement and non-dominant hemispheric weakening. The reduced whole brain connection side efficiency with some key nodes of the brain net may relate to sleep quality improvements in SSD patients. 展开更多

Sleep disorders have a profound and well-documented impact on overall health and quality of life in the general population. In patients with chronic disease, sleep disorders are more prevalent, with an additional morbidity and mortality burden. The complex and dynamic relationship between sleep disorders and chronic kidney disease(CKD) remain relatively little investigated. This article presents an overview of sleep disorders in patients with CKD, with emphasis on relevant pathophysiologic underpinnings and clinical presentations. Evidence-based interventions will be discussed, in the context of individual sleep disorders, namely sleep apnea, insomnia, restless leg syndrome and excessive daytime sleepiness. Limitations of the current knowledge as well as future research directions will be highlighted, with a final discussion of different conceptual frameworks of the relationship between sleep disorders and CKD. 展开更多

OBJECTIVE: To investigate the safety and efficacy of Huatan Jieyu granules in treatment of Parkinson’s disease(PD) patients with sleep disorder identified as symptom pattern of phlegma-heat-stirring wind in terms of the theory of Traditional Chinese Medicine.METHODS: In total, 107 Parkinson’s disease patients with sleep disorders identified as symptom pattern of phlegma-heat-stirring wind were selected and randomly divided into the experimental group(55 cases) and the control group(52 cases).Both groups were given basic treatment with prednisone. The experimental group of patients was treated with Huatan Jieyu granules and the control group of patents was treated with only the basic treatment. Treatment lasted for 4 weeks. Sleep polygraph were recorded before the study as well as 3 months and 6 months after treatment.RESULTS: After treated with Huatan Jieyu granules,the total sleep time, and the percentage of non rapid eye movement 2(NREM 2), non rapid eye movement 3(NREM 3) and rapid eye movement(REM)sleep period increased significantly, while the percentage of NREM1 sleep period decreased significantly compared with before treatment(P < 0.05).CONCLUSION: The treatment of PD patients with sleep disorder by Huatan Jieyu granules can improve their sleep structure and their sleep quality. 展开更多

Studies have shown that non-alcoholic fatty liver disease(NAFLD)may be associated with sleep disorders.In order to explore the explicit relationship between the two,we systematically reviewed the effects of sleep disorders,especially obstructive sleep apnea(OSA),on the incidence of NAFLD,and analyzed the possible mechanisms after adjusting for confounding factors.NAFLD is independently associated with sleep disorders.Different sleep disorders may be the cause of the onset and aggravation of NAFLD.An excessive or insufficient sleep duration,poor sleep quality,insomnia,sleep-wake disorders,and OSA may increase the incidence of NAFLD.Despite that some research suggests a unidirectional causal link between the two,specifically,the onset of NAFLD is identified as a result of changes in sleep characteristics,and the reverse relationship does not hold true.Nevertheless,there is still a lack of specific research elucidating the reasons behind the higher risk of developing sleep disorders in individuals with NAFLD.Further research is needed to establish a clear relationship between NAFLD and sleep disorders.This will lay the groundwork for earlier identification of potential patients,which is crucial for earlier monitoring,diagnosis,effective prevention,and treatment of NAFLD. 展开更多
