Microneedles(MNs)have been extensively investigated for transdermal delivery of large-sized drugs,including proteins,nucleic acids,and even extracellular vesicles(EVs).However,for their sufficient skin penetration,conventional MNs employ long needles(≥600μm),leading to pain and skin irritation.Moreover,it is critical to stably apply MNs against complex skin surfaces for uniform nanoscale drug delivery.Herein,a dually amplified transdermal patch(MN@EV/SC)is developed as the stem cell-derived EV delivery platform by hierarchically integrating an octopusinspired suction cup(SC)with short MNs(≤300μm).While leveraging the suction effect to induce nanoscale deformation of the stratum corneum,MN@EV/SC minimizes skin damage and enhances the adhesion of MNs,allowing EV to penetrate deeper into the dermis.When MNs of various lengths are applied to mouse skin,the short MNs can elicit comparable corticosterone release to chemical adhesives,whereas long MNs induce a prompt stress response.MN@EV/SC can achieve a remarkable penetration depth(290μm)for EV,compared to that of MN alone(111μm).Consequently,MN@EV/SC facilitates the revitalization of fibroblasts and enhances collagen synthesis in middle-aged mice.Overall,MN@EV/SC exhibits the potential for skin regeneration by modulating the dermal microenvironment and ensuring patient comfort. 展开更多

Observation on the Efficacy and Mechanism of Blood-Letting and Cupping Therapy in Improving Upper Limb Lymphedema after Breast Cancer Surgery

Objective:To evaluate the clinical efficacy of blood-letting cupping combined with manual lymphatic drainage in treating breast cancer-related lymphedema(BCRL)and explore its mechanism of action from both traditional Chinese medicine and modern medical perspectives,providing a scientific basis and novel therapeutic approaches for clinical management of BCRL.Methods:Patients with BCRL admitted to the outpatient and inpatient departments of Hebei University Affiliated Hospital were enrolled.A prospective randomized controlled trial design was adopted,with eligible patients randomly assigned to a treatment group and a control group.The control group received manual lymphatic drainage alone,while the treatment group received manual lymphtic drainage combined with blood-letting cupping therapy.Posttreatment comparisons evaluated upper limb circumference reduction,edema severity grading,and upper limb functional scores.Vital signs and adverse reactions during treatment were recorded for both groups.Statistical software analyzed the data.Results:The treatment group demonstrated significantly greater reduction in upper limb circumference,improvement in edema severity,and higher upper limb function scores compared to the control group(P<0.05).Vital signs remained stable throughout treatment in both groups.No severe adverse reactions occurred in the treatment group;only isolated cases of mild skin itching were reported,which resolved after symptomatic management.Conclusion:The combination of bloodletting cupping and manual lymphatic drainage demonstrates reliable efficacy in treating BCRL,effectively alleviating edema symptoms and improving upper limb function with high safety.Its mechanism may relate to traditional Chinese medicine principles of“unblocking meridians,promoting blood circulation,and resolving stasis”and modern medical concepts of“enhancing local blood circulation,facilitating lymphatic drainage,and reducing inflammatory responses”. 展开更多

The rapid increase in traffic loads and frequencies has rendered conventional asphalt pavement inadequate to maintain its durability under tropical climates.This challenge has necessitated the exploration of new sources of modified asphalt with enhanced stiffness and superior performance at high temperatures.Natural rubber(NR)is a renewable biopolymer that has received growing interest as a modifier for asphalt binders.Cup lump rubber(CLR),a type of NR,is used to enhance asphalt properties and improve the performance of road pavements.This study evaluates the influence of wax-based surfactants(WS)on CLR-modified asphalt binder(CMB).The assessment focuses on changes in chemical characteristics,rheological behaviour,activation energy,and morphology.Four concentrations of WS(0.1%,0.15%,0.2%,and 0.25%)were incorporated into CMB.Analysis of CMB chemical changes showed that viscosity increased due to higher sulfoxide,carbonyl,and aromatic bond indices.These chemical modifications contributed to improved resistance of the binder to heat-induced deterioration.In both unaged and aged CMB samples,the incorporation of WS reduced the sulfoxide index of the binder.Rheological analysis indicated that CMB improved rutting resistance and anti-ageing performance,while WS further enhanced fatigue resistance.Activation energy analysis suggested that the combination of CMB with 0.15%WS produced the most favourable enhancement.Micrograph results showed that WS improved binder homogeneity and interconnectivity.In conclusion,the findings indicated that incorporating 0.15%WS into CMB enhanced the performance and durability of the asphalt pavement. 展开更多

Update evidence of effectiveness on pain relieving of cupping therapy:a systematic review and Meta-analysis of randomized controlled trials

OBJECTIVE:To update the current best evidence on the effectiveness and safety of cupping therapy in pain management.METHODS:The protocol of this systematic review was registered at PROSPERO(CRD42021261308).An updated literature searching in 7 databases was conducted from January 2014 to January 2023.Two authors extracted data and assessed the risk of bias independently.Statistical analysis was performed using RevMan 5.4.1 software(Cochrane Collaboration,London,UK).Meta-analysis with a random effect model was conducted when there was no serious statistical heterogeneity among trials(I^(2) ≤ 75%).Grading of Recommendations Assessment,Development,and Evaluation was also conducted to assess the quality of evidence.RESULTS:Seventy-two trials with 5720 participants were included.All included trials were assessed as having high risk of bias.The majority of the included trials reported the benefit of cupping plus other therapy or cupping alone on improving cure rate(average risk ratio more than 1.15) and reducing visual analogue scale [average mean difference(MD) reduction 0.16 to 7.0 cm],improving quality of life,quality of sleep or other symptoms related to pain condition.And there was low/very low quality evidence that the incidence of adverse events in the cupping groups were lower than that in the control groups.Although the heterogeneity between studies and the methodological quality of the study itself lead to the low evidence strength of the current conclusions,the results of this study are a valuable supplement to the founding of previous review.CONCLUSION:Cupping therapy alone or combined with other therapy was considered benefit in relieving pain,improving the quality of life,and increasing the cure rate of patients with pain conditions,though supported by the low quality of evidence.According to the limited evidence,cupping therapy seems to have less harm than drugs when treating pain conditions. 展开更多

BACKGROUND Clinical studies using Trabecular Titanium™acetabular cups have shown promising short and medium-term results.This material,due to its macro and micro surface roughness,provides a substrate for osseointegration and enhances implant stability.However,there is a lack of evidence in the literature on the use of this material in patients with femoral neck fracture.AIM To evaluate the short-term clinical-functional and radiographic outcomes in patients with femoral neck fractures undergoing total hip arthroplasty(THA)with Trabecular Titanium™acetabular cup implants.METHODS The study included 104 patients with medial femoral neck fractures who underwent THA between January 2020 and December 2020 with the Delta TT acetabular cup(Lima Corporate,Villanova di San Daniele del Friuli,Italy).The mean age of the patients was 69.57±10.16 years(range:36-85 years).The followup period ranged from a minimum of 3 to a maximum of 4 years.Three questionnaires(Harris Hip Score,Oxford Hip Score,and EQ5D)were administered along with radiographic evaluations.Statistical methods included the Student's t-test and one-way analysis of variance for comparisons(with significance set at 0.05),and the Kaplan-Meier curve for prosthetic implant survival.RESULTS The mean follow-up was 41.5 months.The Harris Hip Score(HHS)showed a mean increase of 2.74 points(mean HHS 88.52 at 6 months postoperatively and mean HHS 91.26 at the last follow-up)with statistical significance.Similarly,the Oxford Hip Score demonstrated a statistically significant difference between follow-up groups.However,the EQ5D did not show statistically significant differences among the three groups(preoperative,6-month follow-up,and last follow-up).Revision surgery was required in 6 patients.According to Moore's criteria,96%of the acetabular components were radiographically stable and well-integrated at the last follow-up.The Kaplan-Meier curve showed a 96%survival rate.CONCLUSION The clinical and radiographic results obtained in the short to medium term confirm the excellent performance of the Delta TT acetabular cup in terms of osseointegration,providing an optimal solution both for young patients with high functional recovery demands and for fragile patients requiring optimal stability of the acetabular component to reduce the risk of implant failure. 展开更多

Objective:Psoriasis,a common chronic inflammatory skin condition with genetic underpinnings,is traditionally managed with cupping therapy.Although used historically,the precise mechanical effects and therapeutic mechanisms of cupping in psoriasis remain largely unexamined.This study aimed to evaluate cupping therapy's efficacy for psoriasis and investigate its role in modulating inflammatory responses and cellular metabolism.Methods:Psoriasis was induced in mice using topical imiquimod(IMQ).The effects of cupping on psoriatic lesions were assessed using the Psoriasis Area and Severity Index score,histology,immunohistochemistry,and immunofluorescence staining.polymerase chain reaction sequencing(RNA-seq)and Western blotting were conducted to examine changes in mRNA expression and the AMP-activated protein kinase(AMPK)signaling pathway.Results:Cupping therapy significantly reduced inflammation,epidermal thickness,and inflammatory cell infiltration in mice with IMQ-induced psoriasis.Immunohistochemistry and immunofluorescence showed lower expression of inflammatory markers and a shift in T-cell populations.RNA-seq and Western blotting indicated that cupping upregulated Piezo1 and activated the AMPK pathway,improving energy metabolism in psoriatic skin.Conclusion:Cupping therapy reduces epidermal hyperproliferation and inflammation in psoriasis,rebalancing the local immune microenvironment.Mechanistically,cupping promotes calcium influx via Piezo1,activates AMPK signaling,and supports metabolic homeostasis,suggesting therapeutic potential for psoriasis. 展开更多

[Objectives]To observe the clinical efficacy of Mongolian medicine silver needle combined with cupping and bloodletting therapy in the treatment of cervical spondylotic radiculopathy(CSR).[Methods]A total of 120 patients diagnosed with CSR who received treatment at the International Mongolian Hospital of Inner Mongolia between October 2021 and October 2022 were selected and randomly divided into two groups:A test group and a control group,with 60 patients in each group.The patients in the test group received Mongolian medicine silver needle combined with cupping and bloodletting therapy,following the principles of syndrome differentiation in Mongolian medicine.In contrast,the patients in the control group received only the Mongolian medicine silver needle therapy.After treatment,the Mongolian medicine syndrome scores,visual analogue scale(VAS)scores,and total clinical therapeutic effects in the two groups of patients were compared both prior to and following the treatment.[Results]The improvement in VAS and Mongolian medicine syndrome scores in the test group was greater than that in the control group,with a statistically significant difference(P<0.05).The scores of the test group surpassed those of the control group,and this difference was also statistically significant(P<0.05).Furthermore,the overall therapeutic effect in the test group was 93.33%,which was markedly higher than 76.67%observed in the control group(P<0.05).[Conclusions]The therapeutic effect of Mongolian medicine silver needle combined with cupping and bloodletting therapy in the treatment of CSR is remarkable.This approach has demonstrated efficacy in alleviating patient pain and improving the functionality of the cervical vertebrae,thereby warranting clinical promotion and application. 展开更多

AIM:To explore the impact of insulin-like growth factor-1 receptorα(IGF-1Rα)on the differentiation fate of optic-cupderived retinal stem cells(OC-RSCs)into retinal ganglion cells(RGCs)in vitro.METHODS:OC-RSCs were isolated from optic cups of rats on embryonic day 12.5,and high-purity OC-RSCs were obtained by conditioned culture and passage.Differentiation of OC-RSCs into RGCs under different serum concentrations was examined using flow cytometry,and the serum concentration with high interference with differentiation ratio was selected.Furthermore,the effect of blocking IGF-1Rαon the differentiation of OC-RSCs into RGCs was analyzed through immunocytochemistry and Western blotting.RESULTS:Immunohistochemical analysis revealed IGF-1Rαwas highly expressed in rat embryos at day 12.5.OC-RSCs were isolated and purified,and high-purity OCRSCs were obtained.When 2.5%serum was administered,the ratio of differentiated RGCs(Thy-1.1 positive)decreased significantly,and the results of immunoblotting also confirmed the blockade of IGF-1Rαreduced Thy-1.1 protein expression.CONCLUSION:IGF-1Rαblocking can reduce the differentiation of OC-RSCs into RGCs. 展开更多

岐黄针疗法是在中医理论指导下,结合古“九针”与现代材料发展起来的新型针罐结合治疗技术,包含岐黄针(渝械注准20202200072)与硅胶岐黄罐(粤穗械备20250304)。该疗法以《灵枢·官针》“五刺法”为核心操作技术,具有操作简便、安全... 岐黄针疗法是在中医理论指导下,结合古“九针”与现代材料发展起来的新型针罐结合治疗技术,包含岐黄针(渝械注准20202200072)与硅胶岐黄罐(粤穗械备20250304)。该疗法以《灵枢·官针》“五刺法”为核心操作技术,具有操作简便、安全性高、临床适用范围广的特点。针对临床操作标准不统一、适应证界定模糊等问题,多学科专家团队遵循全国针灸标准化技术委员会(SAC/TC 475)规范及WHO指南制定原则,经系统检索数据库筛选循证证据、德尔菲法与多轮专家共识会议,制定本标准化规范。本共识明确其“决渎壅塞、通调经脉、调畅血气、平复阴阳”的核心功效,界定针罐定义与结构,规范操作准备、核心流程及“五刺法”应用细节,明确针罐配合方式;并基于循证分级,确定其适用于疼痛类、神经系统、消化系统、妇科疾病及亚健康调理。该共识为各级医疗机构提供了临床应用标准,也为疗法的学术传承、科研创新与推广奠定了基础。 展开更多

玄磁罐疗法对肺炎患儿的疗效及对炎症因子、免疫功能及睡眠质量的影响

目的探究玄磁罐疗法对肺炎患儿的临床疗效,观察其对炎症因子、免疫功能及睡眠质量的影响。方法将144例肺炎患儿随机分为对照组和观察组,各72例。对照组采用常规治疗,观察组在对照组基础上加用玄磁罐疗法。比较2组治疗总有效率,治疗前后... 目的探究玄磁罐疗法对肺炎患儿的临床疗效,观察其对炎症因子、免疫功能及睡眠质量的影响。方法将144例肺炎患儿随机分为对照组和观察组,各72例。对照组采用常规治疗,观察组在对照组基础上加用玄磁罐疗法。比较2组治疗总有效率,治疗前后炎症因子(TNF-α、IL-1β、IL-6、IL-8)、免疫因子(IgG、IgA、IgM、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平及睡眠质量评分。结果观察组治疗总有效率为97.22%,显著高于对照组的79.17%(P<0.001)。治疗后,观察组炎症因子水平均显著低于对照组(P<0.001);观察组免疫功能水平显著均高于对照组(P<0.001);观察组睡眠质量评分为(4.56±0.98)分,显著低于对照组的(6.22±1.24)分(P<0.001)。结论玄磁罐疗法联合常规治疗可显著提高肺炎患儿治疗有效率,降低炎症因子水平,改善免疫功能,提升睡眠质量。 展开更多

目的:探究中药竹罐治疗联合六字诀在慢性阻塞性肺疾病(COPD)稳定期病人中的应用效果。方法:选取2023年12月—2024年12月在我院呼吸内科就诊的150例COPD稳定期病人,采用随机数字表法将病人分为对照组和观察组,各75例。对照组实施常规护理... 目的:探究中药竹罐治疗联合六字诀在慢性阻塞性肺疾病(COPD)稳定期病人中的应用效果。方法:选取2023年12月—2024年12月在我院呼吸内科就诊的150例COPD稳定期病人,采用随机数字表法将病人分为对照组和观察组,各75例。对照组实施常规护理,观察组在常规护理基础上采用中药竹罐治疗联合六字诀。比较两组病人干预前后第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、第1秒用力呼气容积与用力肺活量比值(FEV1/FVC)、6 min步行距离以及圣乔治呼吸问卷(SGRQ)评分。结果:干预后,观察组FEV1、FVC、FEV1/FVC、6 min步行距离以及SGRQ评分改善效果均优于对照组,差异均有统计学意义(P<0.05)。结论:中药竹罐治疗联合六字诀能有效改善COPD稳定期病人的肺功能、运动耐力及生活质量。 展开更多

目的:探讨泻法平衡火罐联合鳖甲灸治疗下焦湿热型急性单纯性下尿路感染的临床疗效。方法:本研究采用回顾性设计,选取2023年5月至2024年5月在如皋市中医院接受常规治疗联合泻法平衡火罐、鳖甲灸法治疗的46例下焦湿热型急性单纯性下尿路... 目的:探讨泻法平衡火罐联合鳖甲灸治疗下焦湿热型急性单纯性下尿路感染的临床疗效。方法:本研究采用回顾性设计,选取2023年5月至2024年5月在如皋市中医院接受常规治疗联合泻法平衡火罐、鳖甲灸法治疗的46例下焦湿热型急性单纯性下尿路感染患者为研究对象(观察组),另按1:1的比例选取同期我院接受常规治疗的46例下焦湿热型急性单纯性下尿路感染患者(对照组)。对比两组疗效、中医证候积分、临床症状改善时间、白细胞计数、红细胞计数、尿培养转阴率、不良反应发生率及复发率。结果:观察组的临床总有效率显著优于对照组,而其复发率则显著较低(P<0.05)。与对照组治疗2周后相比,观察组主症、次症、总分、白细胞计数、红细胞计数更低,平均症状消失时间、平均退热时间、平均尿菌转阴时间更短,尿培养转阴率更高(P<0.05)。两组不良反应发生率对比无差异(P>0.05)。结论:泻法平衡火罐联合鳖甲灸治疗下焦湿热型急性单纯性下尿路感染,可改善临床症状,降低复发率。 展开更多
