Objective To explore the efficiency of endoscopic septoplasty with submucous resection for the treatment of 220 patients with deviated nasal septum and report our experience in this field.Methods From May 2006 to May ...Objective To explore the efficiency of endoscopic septoplasty with submucous resection for the treatment of 220 patients with deviated nasal septum and report our experience in this field.Methods From May 2006 to May 2010,220 patients with deviated nasal septum were treated by endoscopic septoplasty with submucous resection to decompress the stress points,resect the deviated parts and implant the reshaped bone of nasal septum,while retaining the normal structure of nasal septum with minimally invasive technique.Results Satisfactory effects were achieved in all the 220 patients,showing centered septal structure and disappeared symptoms such as nasal obstruction,headache and nasal hemorrhage,but no complications including perforation of nasal septum,depressed nasal bridge and septal chalasia and flaring.Conclusion Rhino-endoscopic septoplasty with submucous resection can substantially retain the original structure of nasal septum and thus is proven to be a desirable operation with multiple advantages including minimal invasiveness,quicker healing and fewer complications.展开更多

The aim of this study was to find out the common complications after Septoplasty and Septorhinoplasty in a series of 127 Saudi patients undergoing Septoplasty or Septorhinoplasty in Northern Saudi Arabia. Methodology: This study included a series of 127 patients presented with nasal blockage and subsequently undergone Septoplasty and /or Septorhinoplasty. Results: Complications were determined in 69/127 (54.3%) of the patients. The most common complication after three months of septoplasty or Septorhinoplasty was the change in the shape of the nose, which represented 57 (82.6%) out of 69 patients with complications. Bleeding was experienced in 8 patients of whom 5/8 (62.5%) were males and 3/8 (37.5%) were females. Conclusion: Nose deformities are the most common complication associated with Septoplasty or Septorhinoplasty in Northern Saudi Arabia. The risks for Septoplasty or Septorhinoplasty complications can be condensed with accumulative experience.展开更多
Objective:To compare and correlate the efficacy of the NOSE score&the VAS score in determining the symptomatic benefit in patients undergoing septoplasty.Materials and methods:Eighty patients with deviated nasal s...Objective:To compare and correlate the efficacy of the NOSE score&the VAS score in determining the symptomatic benefit in patients undergoing septoplasty.Materials and methods:Eighty patients with deviated nasal septum undergoing septoplasty were included in the study.NOSE score&VAS score(out of 100)was documented before and after surgery.Results were correlated and compared statistically.Results:In the NOSE score,the most bothersome symptom was trouble breathing through the nose(85.83);followed by Nasal obstruction or blockage(82.50).Wilcoxon test showed significant improvement with NOSE score and VAS score in all patients at 1 month and 3 months.Spearman’s coefficient showed a positive correlation between the two,though the score improvement and patient satisfaction rate was significantly high with NOSE score.Conclusions:NOSE score and the VAS score both provide effective framework for evaluating treatment responses after septoplasty.However,the NOSE score showed higher improvement and better patient satisfaction rate when used to measure of nasal obstruction as compared to the VAS score.展开更多
Objective:Nasal packing is routinely applied after septoplasty.Patients,however,report feeling very uncomfortable while the packing is in place.The aim of this study was to compare the effects of nasal septum suture c...Objective:Nasal packing is routinely applied after septoplasty.Patients,however,report feeling very uncomfortable while the packing is in place.The aim of this study was to compare the effects of nasal septum suture combined with inferior turbinate coblation to the effects of nasal packing after septoplasty.Methods:In this study,135 patients undergoing septoplasty were divided into 3 groups:group 1 patients had microdebrider with packing,group 2 received coblation with packing and group 3 had coblation with suture.Early postoperative quality of life and complications were compared between the 3 groups.Results:The patients in group 1 experienced the most postoperative nasal pain,headache,dysphagia,sleep disturbance and bleeding on the night of surgery;while the patients in group 3 experienced the fewest symptoms.No difference in epiphora was observed between the 3 groups.More pain and bleeding were experienced when comparing the pack removal (Group 1 and 2) with the clearance of the nasal cavity (Group 3).We noted one case of postoperative bleeding in group 1,one septal hematoma in group 1 and a second septal hematoma in group 2.No such postoperative complications were found in group 3.Conclusion:Nasal septum suture combined with inferior turbinate coblation was not only associated with less pain,increased patient satisfaction and an improved quality of life;but also reduced postoperative complications.Our results confirm that it is a more comfortable,reliable alternative to the more common nasal packing.展开更多

The present study makes an attempt to review its incidence and its associated pathology. Our study involves two groups of patients with 100 patients in each group. The first group consists of patients who are having nasal obstruction as a chief complaint and have deviated nasal septum. The second group consists of patients with either ear or throat complaints without any nasal symptoms, but were noted to have deviation of nasal septum on examination. An analysis was made to determine whether any particular type of deviation is more prone to develop pathology than other types. Symptoms are multisystem involvement such as headache, epistaxis, ophthalmological, otological, upper and lower respiratory tract infections;and many more. Septoplasty is commonly performed for nasal septal deviation as a primary indication of nasal airway obstruction. Physical examination, anterior rhinoscopy and nasal endoscopy are considered the gold standard for detecting septal deviation. Computed tomography (CT) can also be used to evaluate the nasal septum, anatomical variants and unveils the associated sinonasal diseases. This study aimed to find out the advantages of CT in endoscopic septoplasty for all out benefits to the patients. <strong>Patients and methods:</strong> 168 patients of nasal septal deviation were selected from 2015 to 2018 in the Bangladesh Medical and Popular Medical College Specialized Hospital. Age ranged between 10 years to 70 years of age. CT scanning in both coronal, axial and sagittal sections had been done in all patients after proper history taking, physical examination, anterior rhinoscopy and nasal endoscopy. Endoscopic septoplasty alone and/or other ancillary procedures were performed in all 168 patients under general anesthesia. <strong>Results:</strong> Amongst 168 patients, male was 116 (69%) and female was 52 (31%) with a ratio 2.2:1. Maximum 58 (35%) reported in 31 to 40 years and 48 (29%) was found in 21 to 31 years of age. Hypertrophied inferior turbinate (HIT) 54 (32%) and 48 (29%) concha bullosa (CB) were associated with 168 septal deviation. Associated pathologies like maxillary antral cyst 68 (40%) and maxillary sinusitis 62 (37%) were documented. <strong>Conclusion:</strong> CT scanning unveils the abnormal anatomic variants and associated sinonasal pathologies along with nasal septal deviation. Endoscopic septoplasty, correction of the anatomical variants and complete surgical clearance of the sinonasal pathologies can give the patient a grand success.展开更多
The excessively short nose is a challenging dilemma in rhinoplasty.The variability in patient goals and nasal anatomy prevents a one-size-fits-all approach,yet a collection of commonly employed techniques may be consi...The excessively short nose is a challenging dilemma in rhinoplasty.The variability in patient goals and nasal anatomy prevents a one-size-fits-all approach,yet a collection of commonly employed techniques may be considered when approaching an individual nose.This article discusses the common anatomical abnormalities associated with a short nose and reviews the existing literature on techniques to address these abnormalities and how they may be combined to create a unique surgical plan.Typically,correcting a short nose requires an increase in nasal length and projection,along with a decrease in rotation.Achieving this necessitates a complete release of the soft tissue and ligaments from all bony and cartilaginous attachments to allow for skin redraping,as the mucosa and/or soft tissue envelope often pose limitations.Commonly employed techniques include a septal extension graft,anterior septal reconstruction,lower lateral cartilage repositioning with grafting,tip grafts,and occasionally composite grafts.展开更多

Aim:Iliac crest(IC)is the most common source of bone for alveolar cleft repair,as it allows for harvesting a large amount of cancellous bone with a high rate of favorable outcomes.Its drawback is the donor-site morbid...Aim:Iliac crest(IC)is the most common source of bone for alveolar cleft repair,as it allows for harvesting a large amount of cancellous bone with a high rate of favorable outcomes.Its drawback is the donor-site morbidity.We propose a new technique for alveolar cleft grafting using vomerine bone(VB),which,through bone grafting,reconstructs the alveolus with the VB and simultaneously corrects the nasal septum deviation.Method:We performed 18 alveolar reconstructions with VB in patients with a small bony defect and septal deviation,which would benefit from septoplasty.A matched control group with IC bone grafting was selected.Panoramic X-rays were used for vertical assessment of ossification with the Bergland scale and CT scans for the evaluation of the thickness of the grafted area in the VB and IC groups and compared with an independent samples T-test.A paired T-test compared angular measurements of the septal deviation pre-and post-vomerine grafting.Results:All grafts healed uneventfully,with no complications at the donor site and respiratory function was improved.There was no statistically significant difference in ossification height between VB and IC,the alveolar thickness at the occlusal and middle third was higher with IC.Septal deviation was reduced significantly.Conclusion:Alveolar graft from VB seems to be a viable alternative to IC in patients who present with a mild bony alveolar defect in addition to septal deviation,allowing combined procedures while reducing the morbidity of the donor site.展开更多
