BACKGROUND Adults with type 2 diabetes mellitus(T2DM)in Malaysia continue to have substantial comorbidities and struggle to achieve glycemic targets.AIM To comprehensively evaluate diabetes self-care and glycemic control using multiple self-reporting questionnaires.METHODS Adults diagnosed with T2DM attending the Seremban Health Clinic were recruited in this cross-sectional study.Eligible participants were recruited based on a consecutive sampling technique,first-come-first-served-basis if they fulfilled the inclusion and exclusion criteria. In addition to the usual sociodemographic, clinical, and laboratory data, the participants answered seven specific self-reporting questionnaires. This report was focused on six key variables:Glycemic control;self-care;self-efficacy;diabetes knowledge;health literacy;and medication adherence.RESULTS A total of 100 adults with T2DM participated. The proportions of participants achieving specific thresholds in the key variables were:Acceptable glycemic control 39.4%;adequate diabetes knowledge 59.6%;sufficient or higher health literacy 80.2%;and medication adherence 51.0%.The mean self-efficacy score was 110.6(73.3%of maximum),and the mean self-care score was 30.7(43.9%of maximum).A statistically significant linear correlation was observed for eight pairs of key variables with Pearson’s correlation values varying between 0.21 to 0.55.Selfefficacy had a relatively higher correlation while glycated hemoglobin was not correlated with other key variables.Path analysis was conducted to examine the relationships among diabetes self-efficacy(Diabetes Management Self Efficacy scale score),self-care behavior(Summary of Diabetes Self-Care Activities score),and glycemic control, but the model demonstrated a poor fit(χ^(2)=28.1,P<0.001).CONCLUSION We found substantial suboptimal glycemic control and low self-care practices but acceptable levels of diabetes knowledge,self-efficacy,health literacy and medication adherence among the patients with T2DM.In spite of the correlations between self-care,self-efficacy,and medication adherence,it was surprising that self-care did not correlate with glycemic control.Prospective cohort studies are needed to explore whether these factors influence diabetes outcomes. 展开更多

BACKGROUND Diabetes is becoming increasingly common and has become an important global health issue. In addition to physical damage, diabetes often leads to psychological complications, such as depressive symptoms.Self-care is considered to be the cornerstone of type 2 diabetes mellitus(T2DM)management.This research evaluated depression and explored the associations between self-care activities,self-perceived burden, and depression among T2DM patients in China.AIM To investigate the self-care activities and the association between depression and self-perceived burden among Chinese inpatients with T2DM.METHODS A cross-sectional study was conducted in participants with T2DM.The data collected encompassed basic characteristics, diabetes self-care activities,depre-ssion levels, and self-perceived burdens.Bootstrapping was utilized to assess the mediating role of diabetes self-care activities.RESULTS There were 599 T2DM patients in the survey, and 71.8%had been diagnosed with the disease for 1–10 years. There were significant correlations between self-care activities, depression, and self-perceived burden. The significant coefficients for paths a(B=-0.281,P<0.001)and b(B=-0.041,P<0.05)suggested negative associations between self-perceived burden and self-care behavior and between self-care activities and depression. The indirect effect(path a×b)of self-perceived burden on depression through self-care behaviors was significant(B=0.020,P<0.05),with a 95%bias-corrected bootstrap confidence interval of 0.007–0.036.CONCLUSION The mediating model presented here highlights the role of self-care activities in exerting both direct and indirect effects on depression in participants with T2DM. 展开更多

Stroke survivors often face significant challenges when performing daily self-care activities due to upper limb motor impairments.Traditional surface electromyography(sEMG)analysis typically focuses on isolated hand postures, overlooking the complexity of object-interactive behaviors that are crucial for promoting patient independence. This study introduces a novel framework that combines high-density sEMG(HD-sEMG)signals with an improved Whale Optimization Algorithm(IWOA)-optimized Long Short-Term Memory(LSTM)network to address this limitation. The key contributions of this work include:(1)the creation of a specialized HD-sEMG dataset that captures nine continuous self-care behaviors, along with time and posture markers,to better reflect real-world patient interactions;(2)the development of a multi-channel feature fusion module based on Pascal’s theorem, which enables efficient signal segmentation and spatial–temporal feature extraction;and(3)the enhancement of the IWOA algorithm, which integrates optimal point set initialization,a diversity-driven pooling mechanism, and cosine-based differential evolution to optimize LSTM hyperparameters, thereby improving convergence and global search capabilities.Experimental results demonstrate superior performance, achieving 99.58%accuracy in self-care behavior recognition and 86.19%accuracy for 17 continuous gestures on the Ninapro db2 benchmark. The framework operates with low latency, meeting the real-time requirements for assistive devices.By enabling precise,context-aware recognition of daily activities, this work advances personalized rehabilitation technologies, empowering stroke patients to regain autonomy in self-care tasks. The proposed methodology offers a robust, scalable solution for clinical applications, bridging the gap between laboratory-based gesture recognition and practical,patient-centered care. 展开更多

Diabetes is a common chronic disease in clinical practice. The current global average prevalence is 2–6%,and more than 90%of patients have type 2 diabetes.Although there is a correlation between independent self-care and self-management,improving patients’self-care and self-management behaviors can effectively improve patients’quality of life. This study aimed to determine if a significant relationship exists between independent self-care and self-management.This study used a quantitative research design. The inclusion criteria included 198 participants using purposeful sampling. This study adopted a correlational research design to determine the association between self-care readiness(independent variable)and self-management(dependent variable). This study surveyed respondents using a three-part questionnaire, and their scores were recorded using the Demographic Profile Questionnaire,Transition Readiness Assessment Questionnaire(TRAQ),and Diabetes Self-Management Questionnaire(DSQM).The data were analyzed using statistical tools such as frequency, percentage, mean, standard deviation,Pearson R and hypothesis testing. The results showed a correlation between independent self-care and self-management in diabetic patients. These findings indicate that the respondents generally reported moderate to high levels of readiness and self-management,with more variability noted in self-management scores.Overall, the findings suggest that preparedness for independent self-care plays a statistically significant role in influencing self-management,although the strength of this relationship is relatively weak. 展开更多

BACKGROUND The treatment of heart failure not only needs to relieve the clinical symptoms and improve the quality of life for patients but also needs to select scientific and reasonable ways to prevent or delay the progression of the disease, thus reducing the mortality and hospitalization rate.Although the previous regimen can effectively relieve symptoms in the early stage of treatment,long-term use may cause adverse events, such as arrhythmia, and even increase mortality. Therefore, conventional treatment cannot meet the actual health needs of patients, and scientific nursing intervention is very necessary.AIM To investigate the application of self-care based on full-course individualized health education (FCIHE) and its influencing factors in patients with chronic heart failure (CHF).METHODS We enrolled CHF patients who were admitted to our center between September 2015 and June 2016 and divided them into an intervention group (n = 50) and control group (n = 50) using a random number table.Routine nursing care was applied to the control group, and FCIHE was offered to the intervention group. The self-care behavior,6-min walking distance (6MWD),and 36-item short form health survey (SF-36) scores were compared between the two groups. The influencing factors of the self-care were also analyzed.RESULTS The 6MWD was not significantly different between the two groups at admission (P > 0.05);however,at 3 and 6 mo after discharge,6MWD was significantly increased, and it was significantly longer in the intervention group (P < 0.05).The scores for self-care behavior showed no significant difference at admission between the two groups (P > 0.05);however,at 3 and 6 mo after discharge, the total scores for self-care maintenance, management, confidence, and behavior of the intervention group were significantly higher than those of the control group (P < 0.05).There were no significant differences in the SF-36 scores at admission (P > 0.05);however,at 3 mo and 6 mo after discharge, the scores for all eight subscales, including physical functioning, role limitations due to physical problems, bodily pain, general health perceptions, vitality, social functioning,role-limitations due to emotional problems, and mental health, were significantly higher in the intervention group (P < 0.05).As shown by logistic regression analysis, the influencing factors of self-care mainly included age, cardiac function class, and education background (odds ratio > 1;all P < 0.05).CONCLUSION FCIHE improved self-care behavior and cardiac function in CHF patients.Age,cardiac function, and education level affected the implementation of self-care among CHF patients. 展开更多

India has approximately 73 million people living with diabetes and another 37 million with prediabetes while nearly 47% of the diabetes cases are undiagnosed. The high burden of poor glycemic control and early onset of complications with associated economic costs indicates a high prevalence of poor self-management practices. It is well-established that achieving patient-centered primary care consistent with a chronic care model ensures optimum diabetes self-management support and improves long-term clinical and health outcomes in diabetes patients. The public sector primary care system in India provides services free of cost to beneficiaries but lacks patient-centered care that undermines diabetes selfmanagement education and support. Furthermore, factors like poor patient knowledge of diabetes, suboptimal medication adherence, persistent clinical inertia, lack of data for monitoring and evaluation through clinical audit worsens the standards of diabetes care in primary care settings of India. There is a need for government initiatives to be directed towards the provision of comprehensive outpatient care that is inclusive of uninterrupted supply of drugs, provision of essential laboratory investigators, training and availability of qualified diabetes educators and availability of specialist support when required. Furthermore, the integration of depression screening and smoking cessation services at the primary care level is warranted. 展开更多

Purpose:The aim of this study was to examine the self-care practices and health-seeking behaviours of older adults in urban Indonesia. Methods:The cross-sectional study was performed from January to March 2014 in the Tammua sub-district of Indonesia. At the time of the study,273 older adults resided in Tammua, and half of them(51.2%)participated in this study.Data collection was carried out including self-care practices, health literacy,self-efficacy and basic conditioning factors. Results:It was found that most respondents(124;88.6%)always ate various protein sources daily. However, many participants never limited consumption of sugar(55;39.3%)or salt(40;28.6%),and more than half of respondents(96;68.6%)did not regularly visit MHCs.Health status(p<0.05),health maintenance(p<0.01)and salt limitation(p<0.05)were all significantly associated with salt limitation. It was found that respondents with higher selfefficacy, those who did not want to get information, and those of younger ages are less likely to visit MHCs regularly. Conclusion:An understanding of self-care practices and self-efficacy is needed to improve health care in developing countries.High self-efficacy should be promoted along with adequate health literacy.Older persons should learn the importance of regular health examinations to promote health, prevent diseases, and slow the progress of chronic diseases. The number of respondents who never limit their sugar and salt intake was especially surprising. An intervention program should be developed to limit salt and sugar intake of Indonesian elderly and to motivate older persons to use primary health services. 展开更多

Purpose: The review aimed to find the best evidence on the relationship between health literacy, self-care behavior and perceived quality of life (QoL) among patients with acute coronary syndrome (ACS). Methods: Searching engines included Cumulative Index to Nursing Allied Health Literature (CINAHL), Pro Quest, MEDLINE, Google Scholar, SAGE Journals, Wiley on Line Library, and Science Direct electronic databases from 2010 to 2018. Fifteen research articles assessed the main variables and their associations with demographic and clinical variables. Results: Different methodologies and samples were analyzed: analytic descriptive, systematic review, a randomized control trial, retrospective cohort, a prospective Cohort, cross-sectional, and correlational designs. The fifteen studies showed that the patients with ACS have inadequate health literacy. Patients’ ability to understand basic and complex information or to accurately assess health risks was impaired and may hinder patients’ engagement in self-care. Low HL in patients having ACS is consistently associated with poor QoL. Physical domain of QoL remained the significant outcome of both self-care behavior and health literacy after adjusting for demographics and clinical variables in patients with ACS. Conclusion: The literature pointed to the importance of considering health literacy and self-care behaviors as predictors of quality of life among patients with ACS. However, there has been no previous evidence on the best process of the association between these three variables. 展开更多

Introduction: Controlling hypertension across world continues to be challenging. Managing hypertension is not only concerned with lowering blood pressure by using antihypertensive medications;it also aims to minimize its consequences through adopting self-care practices. Compliance with self-care practices among patients with hypertension is considered a multidimensional phenomenon. The phenomenon of hypertension has been studied quantitatively, however;little qualitative studies were conducted to understand the compliance with self-care among patients with hypertension. Aim: To understand the process that patients with hypertension go through to comply with self-care practices. Methods: This study used a qualitative design that followed constructivist grounded theory approach;purposive sampling was used to recruit participants from cardiac clinics;semi structured, in-depth and face-to-face interview was used as a major method for data collection. Findings: Four participants with hypertension participated in this study;the phenomena of self-care was identified as the central phenomena;the start of the disease was identified as a casual condition;beliefs toward hypertension disease, beliefs toward self-care practices, knowledge and awareness regarding hypertension disease and self-care practices were identified as strategies;experiencing self-care practices was identified as consequence and being patients with hypertension in a social context. Conclusions: The process of compliance with self-care has a path of actions and interactions. The process started from the moment of diagnosis where the patients start to think about self-care. The absence of health care context leads to varying level of compliance with self-care among patients with hypertension. This indicated the need for more effective patient and health care provider relationship, education and awareness campaign. 展开更多
