Granulomatosis with polyangiitis(GPA)poses significant therapeutic challenges due to its susceptibility to concurrent infections and frequent relapses.Professor SHI Zaixiang proposed the therapeutic theory of lifting depression and removing blood stasis, resolving phlegm and dispelling nodulation for GPA management. He identified the core pathogenesis as“Qi collapse with collateral obstruction and phlegm-stasis intermingling”,establishing the treatment principle of comprehensive intervention through ascending ancestral Qi,activating blood circulation, and resolving phlegm-stasis nodules. In clinical practice,Professor SHI emphasizes maintaining immune homeostasis with herbal medicine to enhance efficiency, while dynamically balancing Qi,blood,Yin,and Yang.Notably,he highlights the critical role of emotional factors in autoimmune disease progression.A representative case was provided to elucidate his clinical reasoning in GPA treatment. 展开更多

OBJECTIVE; To observe the influence of Qingrehuatan decoction (QRHT) on serum metabolic profile in young essential hypertension (YEH) patients with abundant phlegm-heat syndrome and provide a basis for treatment with the decoction. METHODS: Twelve male YEH patients were randomly selected and serum samples were collected for examination before and after 4 weeks of thetreatment with QRHT. Twelve healthy males were randomly selected and their serum samples were collected as a control. All serum samples were detected using metabolomic technology with 1H nuclear magnetic resonance. Differences in metabo- lites were studied by principal component analysis and partial least squares-discriminate analysis, which produced scores and Ioadings plots.RESULTS: After 4 weeks of treatment, serum substances could be distinguished between the YEH patients with abundant phlegm-heat syndrome and the control patients. The specific serum endogenous metabolites tended to improve after the treatment. QRHT can appropriately increase the levels of glucose, lactic acid, citric acid, high-density lipoprotein, phosphatidylcholine, glycerophosphate choline, hydroxybutyrate, alanine, and glutamate. QRHT could also decrease the levels of low-density lipoprotein/very low-density lipoprotein, lipids, N-acetyl glycoprotein, and O-acetyl glycoprotein.CONCLUSION: QRHT can effectively ameliorate metabolic disorders in YEH Patients with abundant phlegmheat syndrome. 1H NMR-based metabolo- mic technology can provide an objective basis for the treatment of YEH patients with abundant phlegm-heat syndrome using QRHT. 展开更多

OBJECTIVE: To explore the mechanism of reinforcing kidney-essence, removing phlegm, and promoting mental therapy in treating Alzheimer's disease (AD). METHODS: Sixty patients with AD in Wuhan No.1 Hospital of Traditional Chinese Medicine for Geriatrics from May 2009 to April 2011 were randomly divided into two groups, with 30 in each group. Patients in Bushenhuatanyizhi group (BHY group) took BHY instant granules (6 g, twice per day). Patients in the control group took piracetam (0.8 g, 3 times per day). There were twelve weeks in a course. Changes in the mini-mental state examination (MMSE) score and Activity of Daily Living Scale (ADL) score were analyzed before and after treatment. RESULTS: MMSE scores in the two groups in-creased and ADL scores decreased after treatment, compared with those before treatment (both P< 0.05).The total effective rate was 86.67% in the BHY group (10 very effective, 13 effective, 7 non-responsive). The control group was 57.69% (5 very effective, 16 effective, 9 non-responsive) (P<0.05). Superoxide dismutase levels were increased, lipid peroxide and triglyceride levels decreased after treatment in the BHY group as compared with the control group. CONCLUSION: Reinforcing kidney-essence, removing phlegm, and promoting mental therapy can improve cognitive function and daily life of AD patients. The mechanism of the therapy might be related to improving blood fat, scavenging free radicals, and inhibiting lipid peroxides. 展开更多

Erhuang capsule, a typical formula based on traditional Chinese medicine theory, is widely used to ameliorate multiple sclerosis, inflammation and side effects of glucocortJcoid treatment. Oligodendrocyte precursor cells are neural stem cells that are important for myelin repair and regeneration. In the present study, Erhuang capsule effectively improved clinical symptoms and neurological function scores, reduced mortality and promoted recovery of neurological functions of mice with experimental autoimmune encephalomyelitis. The mechanism of action involved significant increases in oligodendrocyte precursor cell proliferation in specific regions of the brain and spinal cord, increased oligodendrocyte lineage gene 2 expression and enhanced oligodendrocyte precursor cell differentiation. 展开更多

Objective To investigate clinical effects of acupuncture method for dredging collaterals and resolving phlegm on stroke in Congo. Methods The patients in Congo China-Congo Friendship Hospital were randomly divided into an acupuncture group and a medicine group, 30 cases in each group. For the acupuncture group, Moiluoning injection was administrated by intravenous drip and acupuncture was given at Sishencong (四神聪 EX-HN 1), Zhangwan (中脘 CV 12), Fenglong (丰隆 ST 40) and Neiguan (内关 PC 6), etc., once daily for 10 consecutive days as a course of treatment. For the medicine group, 20 mL Moiluoning or 500 mL low molecular Dextran injection was administrated by intravenous drip, once each day for 10 consecutive days as a course of treatment. After 3 therapeutic courses, clinical therapeutic effects were assessed. Results After treatment, the scores for nervous function were significantly decreased in the two groups (both P〈0.05), with a significant difference between the two groups (P〈0.05), the acupuncture group being superior to the medicine group; the effective rate (93.3%, 28/30) of the acupuncture group was significantly higher than that (80.0%, 24/30) of the medicine group (P〈0.05). Conclusion Acupuncture with the method for dredging collaterals and resolving phlegm has a significant clinical effect on stroke. 展开更多

Hyperlipidemia, type 2 diabetes mellitus, nonalcoholic fatty liver and many other metabolic disorder are frequently co-existing in patients. In addition, these diseases are closely related in pathophysiological settings. However, increasing of the disease incidence, lacking of comprehensive prevention and control measurements against the key pathology point concomitant occurrence with the pattem of the single disease, single target therapy, that is leading therapeutic strategy for these metabolic disorders in the setting of Western medicine (WM). On the basis of the combination of the advantages of integrated Chinese medicine (CM) and WM, with unified understanding of such diseases, the new concept of glucolipid metabolic disease (GLMD) is introduced. In this new concept, disorders in glucose and lipid metabolism are recognized as the key trigger and major driving force for the progress of GLMD. The key points of pathology included dysfunction of neuronal-endocrine-immune system, insulin resistance, oxidative stress, inflammation and intestinal flora imbalance. In the core pathogenic perspective of CM, it can be explained as "Gan (Liver) Shi Shu Xie" (dysfunction of Gan in metabolism and emotion regulation) that will lead to the occurence/production of endogenous dampness and phlegm, blood stasis and turbid. This leads to the new concept of "Liver-based regulatory system for metabolic homeostasis" to be introduced further. The comprehensive prevention and control strategy "Tiao Gan Qi Shu Hua Zhuo" (modulating Gan, trigging key metabolic system to resolve pathogenic factors such as phlegm retention and dampness). Its representative formula Fufang Zhenzhu Tiaozhi Capsule (复方贞术调脂胶囊) is innovated under such rationales. Comment for some commonly-used CM GLMD therapeutic drugs was presented. High-level evidence-based and epidemiological and mechanism studies should be carded out to further interpret and explain of the scientific connotation of GLMD. 展开更多
