Aims and objectives: The frequent and unprescribed use of antibiotics has led to the development of resistance by microorganisms responsible for urinary tract infection (UTI). In order to facilitate the follow-up of this microbial resistance, the aim of this study was to determine the bacteria resistant phenotypes. Method: To achieve the following objectives, this study was conducted from June to August 2023. The isolation and identification were performed by standard methods why susceptibility testing was done by Kirby-Bauer disk diffusion technique according to CLSI guidelines. Double-disk synergy test was applied to determine the presence of extended-spectrum β-lactamase (ESBL) produced by bacteria. The Imipenem EDTA Combined Disc Test (CDT) for Metallo beta lactamase (MBL) screening, the D-zone test to detect macrolides, lincosamides and streptogramins type B (MLSB) and Meticillin resistant Staphylococcus aureus (MRS A) which was assessed using a Cefoxitin (30 µg) disc. Results: A total of 40 bacteria were identified with a prevalence of 37.03%. Overall, E. coli was the predominant isolate 14 (35%), followed by Staphylococcus aureus 10 (25%) and Klesbsiella pneumonia 4 (10%). Pseudomonas aeruginosa, Salmonella arinosa and Enterobacter were the most sensible (100%) bacteria against ciprofloxin, ceftriaxone and cefixime. Almost all bacteria were resistant to Amoxicillin/clavulanic acid (>50%). The isolates were in the majority resistant to imipenem. ESBL-producing Enterobacteriaceae represented 25.92%, with a higher rate among E. coli. No MBL production was found among the isolates while 38.46% represented cMLSB, 15.38% represented iMLSB, 23.07% represented MSB and 23.07% represented MRSA. Conclusion: Clinical strains of UTI from Protestant Hospital of Ngaoundere exhibiting ESBL, cMLSB, iMLSB, MSB and MRSA. The regular updating of antibiotic resistance statistics of isolated strains allows for a better adaptation of probabilistic antibiotic therapy. 展开更多

Urinary tract infections are among the most prevalent extra-intestinal infections, with high prevalence globally. This cross-sectional study established prevalence of bacterial aetiology causing urinary tract infection (UTI) and their antimicrobial susceptibility profiles. A questionnaire was used to capture socio-demographic data and possible UTI risk factors among the 206 consented adults seeking medicare at Kiambu Level 5 Hospital. The collected midstream urine samples were subjected to dipstick analysis, microscopy and culture for UTI diagnosis. Results: The overall prevalence rate of UTIs was 27.6%, with women’s prevalence rate being significantly higher at 80.7% compared to men 19.2%. Pregnant women had UTI prevalence at 34% which was higher than other sets of participants. Women who did not frequently change their underpants daily had a higher UTI cases at 34.8%. Escherichia coli, Staphylococcus aureus and Klebsiella pneumoniae were the most prevalent bacterial pathogens at 38.5%, 21% and 19.3%, respectively. Antimicrobial sensitivity analysis revealed high resistances towards Sulfamethoxazole and Ampicillin at range between 50% - 85%, suggesting that these drugs are no longer effective for UTI empirical treatment. The resistance patterns towards Cefotaxime, Cefepime and Ciprofloxacin were below 40%. However, more resistance patterns at a range between 14% - 40% revealed towards Amoxicillin-clavulanic and Nitrofurantoin imply that these are drugs remain potent but there is the need to revise the current UTI management guidelines. In addition, to elude treatment failure, innovation of prophylactic measures is key to halt UTI contraction and offer support to pharmaceutical industries that have fewer new antibiotics in the pipeline. 展开更多

Introduction:Aeromonas is widely distributed in aquatic environments. This study describes the pathogenic characteristics of Aeromonas isolated from adult diarrhea patients and aquatic environments in Beijing,China. Methods:Aeromonas isolates from patients with diarrhea and river water samples were assessed using whole-genome sequencing(WGS)and antibiotic resistance profiling. Results:In total,38 Aeromonas isolates were collected.Among these,13 isolates were from patients with common clinical symptoms, including diarrhea, abdominal pain, and nausea.Four of 13 Aeromonaspositive patients were co-infected with other intestinal pathogens.Patient-derived Aeromonas isolates showed high resistance to ampicillin,ampicillin-sulbactam,nalidixic acid, and tetracycline, whereas water-derived isolates showed high resistance to ampicillin,ampicillin-sulbactam,and nalidixic acid.Phylogenetic analysis revealed seven independent branches, without significant clustering among patient-and water-derived isolates. Conclusions:This study provides valuable insights into the prevalence and characteristics of Aeromonas. The intertwined distribution of patient-and waterderived isolates in the phylogenetic tree deserves attention. 展开更多
