BACKGROUND: No retrievable and repositionable second generation transcatheter aortic valve is available in China. Here, we report the first-in-man implantation of the retrievable and repositionable VenusA-Plus valve. METHODS: A 76-year-old patient with symptomatic severe aortic stenosis and high surgical risk(STS 13.8%) was recommended for transcatheter aortic valve replacement(TAVR) by heart valve team. Type 0 bicuspid aortic valve with asymmetric calcification was identified by dual source computed tomography, and the unfavorable anatomies increased the possibility of malposition and paravalvular leakage during TAVR. Therefore, we used the retrievable and repositionable Venus APlus valve for the patient.RESULTS: Transfemoral TAVR was performed under local anesthesia with sedation, and a 26-mm VenusA-Plus valve was successfully implanted. No transvalvular pressure gradient and trace paravalvular leakage were found. CONCLUSION: The successful first-in-man implantation indicates the retrievable and repositionable Venus A-Plus valve is feasible in complicated TAVR cases such as bicuspid aortic valve. 展开更多

OBJECTIVE: To ascertain if it is effective to use extradural injections and manual repositioning of the spine to treat megalgia caused by cervical spondy- Iotic radiculopathy (CSR). METHODS: Patients with megalgia caused by CSR were divided into a treatment group (n=46) treated by extradural injection and manual repositioning of the spine and a control group (n=46) treated by a conventional method. RESULTS: The prevalence of cure was 58.69% and the total prevalence of effective cure in the treatment group was 97.83% and was 23.91% and 78.26%, respectively, in the control group: this difference between the two groups was significant (P〈 0.05). CONCLUSION: Treatment of megalgia caused by CSR by extradural injections and manual repositioning of the spine has a good curative effect with rapid analgesia and short therapeutic course. 展开更多

A transient postanaphase repositioning of the centriole is found to control the completion of cytokinesis. Using agreen fluorescent protein-calmodulin fusion protein as a living cell probe, we have previously found that calmodulin isassociated with the initiation and progression of cytokinesis. In this study, we further studied the effect of calmodulinon the repositioning of the centriole and subsequent cell cycle progression. When activity of calmodulin is inhibited, theregression of the centriole from the intercellular bridge to the cell center is blocked, and thus the completion of celldivision is repressed and two daughter cells are linked by longer cell bridge in perturbed cells. W7 treatment duringcytokinesis also results in unfinished cytokinesis and stopped G1 phase. These results suggest that calmodulin activity isrequired for centriole repositioning and can affect the completion of cytokinesis and cell cycle progression. 展开更多

Step-by-step external fixation of unstable pelvis with separate anterior and posterior modules

Objective: To evaluate the treatment outcomes of patients with pelvic ring injury by applying step-by-step external pelvic fixation and circular external fixation device.Methods: A total of 28 patients suffering from disintegrated pelvic ring injury are involved in the study. Fourteen patients(the treatment group) underwent step-by-step external pelvic fixation by applying anterior(anti-shock) and posterior modules. For the rest 14 patients(the experimental group), the osteosynthesis were conducted by means of a circular external fixation device. The long-term outcomes were evaluated in a year after the injury.Results: The residual deformity of 5(4–7) mm was observed in 10 patients(71.4%) from the experimental group. In the treatment group, the residual deformity was evident only in 4(28.6%) cases being 2.5(2–3) mm(P = 0.000 319) on the average. The functional result(according to the Majeed scale) was statistically better in the treatment group(P = 0.000 319). Nine(64.3%) and five(35.7%) patients in treatment group showed excellent and positive results, respectively. The excellent result was demonstrated by 3patients(21.4%) of the experimental group, the positive outcomes were observed in 6 cases(42.9%) and the unsatisfactory one was displayed by 1 patient(7.1%) of the same group.Conclusions: The modular approach applied is the advantage of the transosseous osteosynthesis allowing for a separate anterior(anti-shock) fixation and final posterior reposition of the pelvic ring preceded by the stabilization of vital functions. The above mentioned method gives an opportunity to increase the amount of techniques applied for the pelvic external fixation in polytrauma cases. 展开更多

Objective: To observe the therapeutic effect of acupuncture plus manual reposition for treatment of acute lumbar vertebral articular dyskinesia for choosing a better remedy. Methods: 66 cases of acute lumbar vertebral articular dyskinesia were randomly divided into acupuncture plus manual reposition group (treatment group, n=33) and routine manual reposition group (control group, n=33). Yaotong point was punctured, when, the patient was asked to move his or her waist simultaneously. Results: After one session of treatment, of the two 33 cases in treatment and control groups, 28 (84.85%) and 20 (60.61%) were cured, 4 (12.12%) and 9 (27.27%) were improved, and 1 (3.03%) and 4 (12.12%) failed in the treatment. The therapeutic effect of treatment group was significantly superior to that of control group (P<0.05). Conclusion: Acupuncture combined with manual reposition is apparently superior to simple routine manual reposition in relieving acute lumbar vertebral articular dyskinesia. 展开更多

This study was designed to observe and compare the circadian fluctuations in tactile sense, joint reposition sense and two-point discrimination in healthy subjects. Twenty-one healthy adult subjects received perceptual ability tests through these three different sensory modules at approximately 9:00, 13:00 and 18:00 in a day. The distribution of ranking for perceptual ability was significantly different among the three different time points in each individual, with highest perceptual ability in the evening compared with noon and morning, in terms of tactile sense and two-point discrimination. These findings suggest that the perceptual ability of healthy subjects fluctuates according to the time points in a day. 展开更多

A case is presented of a patient with an unexpected poor visual result and subsequent correction following cataract removal surgery via phacoemulsification and intraocular lens implantation using a toric intraocular lens implant (IOL). The initial operation resulted in an uncorrected vision of 20/100 (0.70 logMAR). Retrospective analysis of the patient’s corneal topography revealed irregular astigmatism secondary to remote trauma to the cornea. The cylinder axis on manifest refraction (MR) was significantly different from measured keratometry, so a second procedure was performed to align the cylinder axis of the IOL with the steep axis on MR. This repositioning procedure improved visual outcome to a final uncorrected vision of 20/25 (0.10 logMAR) and best corrected acuity of 20/20 (0.0 logMAR). 展开更多

Freeway logistics plays a pivotal role in economic development.Although the rapid development in big data and artificial inteligence motivates long+haul freeway logistics towards informatization and intellectualization,the transportation of bulk commodities still faces serious challenges arisen from dispersed freight demands and the lack of co-ordination among different operators.The present study thereby proposed inteligent algorithms for truck dispatching for freeway logistics.Specifically,our contributions include the establishment of mathematical models for full-truckload(FTL)and less-than-truckload(LTL)transportation modes,respectively,and the introduction of reinforcement learning with deep Q-networks tailored for each transportation mode to improve the decision-making in order acceptance and truck repositioning.Simulation experiments based on the realworld freeway logistics data collected in Guiyang,China show that our algorithms improved operational profitability substantially with a 76%and 30%revenue increase for FTL and LTL modes,respectively,compared with single-stage optimization.These results demonstrate the potential of reinforcement learning in revolutionizing freeway logistics and should lay a foundation for future research in intelligent logistics systems. 展开更多

In this review, we briefly introduce classical graph theory based protein-protein interaction networks. We also describe the commonly used experimental methods to construct these networks, and the insights that can be gained from these networks. We then discuss the recent transition from graph theory based networks to structure based protein-protein interaction networks and the advantages of the latter over the former, using two networks as examples. We further discuss the usefulness of structure based protein-protein interaction networks for drug discovery, with a special emphasis on drug repositioning. 展开更多

Technologies associated with cardiac resynchronization therapy (CRT) devices and lead systems have progressed.However, dislocation after coronary sinus (CS) lead placement continues to be a problem. We reported on the patient treated with CRT, in whom dislocation of CS lead occurred. In the case, we tried to reposition the CS lead without the left heart delivery system only using pre-shaped stylet and guidewire, and the dislocated CS lead could be successfully repositioned by the method. The method of only using a pre-shaped stylet and guidewire is easier than the conventional way, and it can shorten procedure duration and fluoroscopy time, as well as reduce the cost of treatment, but it is not always feasible. 展开更多
