Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in ...Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in preterm infants.The aspects and extent to which the home environment affects the early(18 months corrected age)neuropsychological development of preterm infants are still unclear.Aims This study aimed to analyse the effect of the home environment on the neuropsychiatric development of preterm infants at 18 months corrected age after discharge from the neonatal intensive care unit(NICU).It also sought to provide a basis for promoting neuropsychiatric development among preterm infants by improving the home environment.Methods In this retrospective cross-sectional study,275 preterm infants born between January 2019 and January 2022 were followed up for systematic management after discharge from the NICU at Shanghai Children's Hospital.The Home Nurture Environment Questionnaire was used to assess the home environment of the infants and analyse its impact on the developmental quotient(evaluated by the Gesell Developmental Scale)and the rate of developmental delays at 18 months corrected age.Results A total of 41.454%of the infants were extremely preterm.The developmental quotient scores at 18 months corrected age were in the middle of the scale.The language domain had the highest rate of developmental delay(46.182%),followed by the adaptive domain(37.091%).Multiple logistic regression analyses showed that compared with infants in supportive home environments,infants with moderate/unsupportive home environments had significantly elevated risks of development delay:2.162-fold for global(odds ratio(OR)2.162,95% confidence interval(Cl)1.274 to 3.665,p=0.004),2.193-fold for fine motor(OR 2.193,95%CI 1.161 to 4.140,p=0.016),2.249-fold for language(0R 2.249,95%CI 1.336 to 3.786,p=0.002)and 2.042-fold for personal-social(OR 2.042,95%CI 1.149 to 3.628,p=0.015).Conclusions A supportive home environment is a crucial protective factor for the neuropsychological development of preterm infants.It is associated with higher developmental quotient scores and protects against neuropsychiatric delays.Incorporating evaluation and continuous improvement of the home environment into the management framework for preterm infants to promote optimal neurodevelopment is essential.展开更多

This paper focuses mainly on visual comfort affected by characteristics of disparity for multiple objects. To find the relationship between disparity distribution and visual comfort perception, several subject evaluation experiments are done. The study contains two spatial distribution types of disparity: 1) only one of the foreground objects has zero disparity; 2) one of the foreground objects has positive disparity, while the other one has negative disparity. The experimental results and relative regression analysis provide appropriate relationship between disparity distribution and visual comfort for both conditions, which is significant to meet the applicant field in S3 D content acquisition, display adjustment and quality evaluation.展开更多

Objective To evaluate the factors affecting the incidence of intrahepatic venovenous shunt(IVVS)in patients with cirrhosis and its impact on hepatic venous pressure gradient(HVPG).Methods A retrospective analysis was ...Objective To evaluate the factors affecting the incidence of intrahepatic venovenous shunt(IVVS)in patients with cirrhosis and its impact on hepatic venous pressure gradient(HVPG).Methods A retrospective analysis was performed on the data of patients with liver cirrhosis who received HVPG measurement in Nanjing Drum Tower Hospital from April 2013 to March 2022.Univariate and multivariate regression analyses were used to investigate the incidence rate and risk factors of IVVS and its impact on HVPG.The t-test and rank-sum test were used for the measurement data,and the x?test was used for the count data.Results A total of 242 cases with cirrhosis were included in the statistical analysis,including 54(22.3%)with IVVS and 188(77.7%)without IVVS.There was a statistically significant difference(P<0.05)in prothrombin time(PT),HVPG,and splenectomy history between the two groups of patients'baseline data(all P<0.05).The multiple logistic regression analysis results showed that PT was an independent risk factor for the occurrence of IVVS(P<0.05),and patients combined with IVVS had lower HVPG values[(17.58±5.57)mmHg vs.(11.92±5.38)mmHg,1 mmHg=0.133 kPa;t=6.623,P<0.001].Conclusion Patients with liver cirrhosis have a high incidence rate of IVVS,which is closely associated with a low prothrombin time.Additionally,patients combined with IVVS have low HVPG values,which affect its accuracy.展开更多

The rates of the return of spontaneous circulation (ROSC...Background: Animal models of asphyxiation cardiac arrest (ACA) are frequently used in basic research to mirror the clinical course of cardiac arrest (CA). The rates of the return of spontaneous circulation (ROSC) inACA animal models are lower than those from studies that have utilized ventricular fibrillation (VF) animal models. The purpose of this study was to characterize the factors associated with the ROSC in the ACA porcine model. Methods: Forty-eight healthy miniature pigs underwent endotracheal tube clamping to induce CA. Once induced, CA was maintained untreated for a period of 8 min. Two minutes following the initiation of cardiopulmonary resuscitation (CPR), defibrillation was attempted until ROSC was achieved or the animal died. To assess the factors associated with ROSC in this CA model, logistic regression analyses were performed to analyze gender, the time of preparation, the amplitude spectrum area (AMSA) from the beginning of CPR and the pH at the beginning of CPR. A receiver-operating characteristic (ROC) curve was used to evaluate the predictive value ofAMSA for ROSC. Results: ROSC was only 52.1% successful in this ACA porcine model. The multivariate logistic regression analyses revealed that ROSC significantly depended on the time of preparation, AMSA at the beginning of CPR and pH at the beginning of CPR. The area under the ROC curve in for AMSA at the beginning of CPR was 0.878 successful in predicting ROSC (95% confidence intervals: 0.773-0.983), and the optimum cut-offvalue was 15.62 (specificity 95.7% and sensitivity 80.0%). Conclusions: The time of preparation, AMSA and the pH at the beginning of CPR were associated with ROSC in this ACA porcine model. AMSA also predicted the likelihood of ROSC in this ACA animal model.展开更多
