AIM:To evaluate the effect of different monochromatic lights on the refractive status and the secretion levels of neurotransmitters in the progressive myopic model of guinea pigs.METHODS:Guinea pigs(n=90)underwent different monochromatic lights irradiation for two weeks were randomly divided into 6 groups:white light(control),ultraviolet(UV),blue, green, red, and simulative sunlight(simSUN).The refractive status and axial length(AL)were measured.Transmission electron microscopy,Masson’s trichrome staining and hematoxylin-eosin staining were performed to observe the structural changes of retina and sclera.High-performance liquid chromatography(HPLC),western blotting and immunofluorescence were used to measure neurotransmitters and their receptors.RESULTS:Myopia models were established successfully. When compared the degrees of change in myopic eyes of control group, the UV group showed a minor decrease in AL and refraction, along with a significant increase in scleral thickness. In contrast, the red and green groups revealed a net increase in AL and refraction, coupled with a net decrease in scleral thickness(all,P<0.01).The dopamine concentration increased in the UV group, while concentrations of serotonin and melatonin significantly decreased(all,P<0.01).The groups that were exposed to UV,blue and simSUN,the expression of dopamine receptor D2(DRD2)increased,and the expression of hydroxytryptamine receptor 2A(HTR2A)and melatonin receptor type 2(MT2)decreased significantly when compared to the control group(all P<0.01).CONCLUSION:Exposure to short-wavelength light could slow the development of myopia by promoting the production of dopamine and suppressing the serotonin and melatonin concentration. The neurotransmitter receptors MT2,DRD2,and HTR2A in the sclera appear to play different roles by different color lights in myopic guinea pigs. 展开更多

AIM:To compare the changes in preschool refractive status, ocular biological parameters, and higher-order aberrations in children with retinopathy of prematurity(ROP)after retinal laser photocoagulation or anti-vascular endothelial growth factor(anti-VEGF)treatment and explore their underlying factors.METHODS:This observational study involved 118 eyes of 59 children, aged 3 to 6y,with ROP followed up between March 2023 and October 2024.They were divided into the laser,anti-VEGF,and anti-VEGF+laser groups. The laser group received a single session of laser photocoagulation. The anti-VEGF group received a single anti-VEGF treatment. The anti-VEGF+laser group received a single anti-VEGF treatment after birth followed by supplementary laser treatment within 2wk to 6mo.Ocular biological parameters were measured using IOL Master 700 and Pentacam HR.Right-eye higher-order aberrations were measured using the OPD-Scan III.Best-corrected visual acuities(BCVA),refractive statuses, ocular biological parameters, and higherorder aberrations were assessed and compared.Multiple linear regression analysis was conducted to evaluate the relationships among ocular biological parameters, higherorder aberrations, spherical equivalent(SE),and treatment methods.Pearson’s correlation coefficients were used to assess the relationships between the SE and higher-order aberrations.RESULTS:The laser group had a higher incidence of myopia and a lower SE than the anti-VEGF group. The incidence of astigmatism and cylindrical power were significantly lower for the anti-VEGF than for the laser and anti-VEGF+laser groups(P<0.05).The anterior corneal surface astigmatism was higher for the laser and anti-VEGF+laser groups than for the anti-VEGF group. The anterior corneal surface K2 and lens thickness were higher for the laser and anti-VEGF+laser groups than for the anti-VEGF group. The whole-eye higher-order aberration root mean square(RMS)values for the right eye were significantly higher for the laser and anti-VEGF+laser groups than for the anti-VEGF group. The whole-eye trefoil RMS values for the right eye were also significantly higher for the laser and anti-VEGF+laser groups than for the anti-VEGF group(P<0.05).Laser treatment was significantly associated with SE,anterior corneal surface curvature and astigmatism, lens thickness,whole-eye highorder aberrations, and whole-eye trefoil(all P<0.05).CONCLUSION:Children with ROP who received laser treatment have higher myopia and astigmatism than those who received anti-VEGF treatment.Children treated with laser or supplementary laser treatment have higher anterior corneal astigmatism, anterior corneal curvature, thicker lenses,whole-eye higher-order aberrations, and whole-eye trefoil. The cause of myopia in children with ROP after laser treatment is increased anterior corneal surface curvature and lens thickness. 展开更多

AIM: To study the evolution of the refractive status and examine the affected factors in infants during the first year of life in a large sample size in China. METHODS: A total of 1258 babies (2516 eyes) aged 32wk gestational age to ty participated in the study, including 766 premature and 492 full-term infants. First, each baby received an orthoptic examination, slit-lamp checking and fundus imaging. Patients with diseases which might affect refractive status were excluded from the cohort. The cycloplegia retinoscopy was performed. Their neonatal histories were reviewed. Each measurement contained the refractive status and calculation of the spherical equivalent (SE). RESULTS: Refractive state showed an average hyperopia of +0.94_+1.63 D at early ages, followed by a trend toward more hyperopia. The refractive state reached the top (+2.431.46 D) at the age of one to two months. Then gliding till one year old when the refractive state reached +0.59:1.41 D. The prevalence of astigmatism was 42.17% in the study, being 2.82% myopic astigmatism and 39.35% hyperopic astigmatism. The 94.1% of hyperopic astigmatism was with-the-rule astigmatism and 71.83% of myopic astigmatism was with-the-rule astigmatism. Refractive state between boys and girls was different. The mean SE of boys was +1.97+1.57 D, while that of girls was +1.79+1.46 D, and the difference was significant. CONCLUSION: Before one year old, the change of refractive status is associated with checking age and sex. At the age of one to two months, the degree of hyperopia reaches the top. Boys have more hyperopic degree than girls, and with- the-rule astigmatism is predominant. Excluding premature infants with advanced retinopathy of prematurity, premature and full-term children have same refraction status. 展开更多

Introduction:Myopia prevalence among Chinese children and adolescents remains persistently high, with an alarming trend toward earlier onset.Understanding the correlation between ocular biometric parameters and myopia development provides essential insights into underlying mechanisms. This study evaluated the associations between axial length(AL),axial length-to-corneal radius ratio(AL/CR),and spherical equivalent refraction(SER)across different age groups to inform early detection strategies and myopia management protocols. Methods:Students aged 5–18 years were recruited from 10 provincial-level administrative divisions across China between November 2020 and July 2024 using hierarchical clustering and probability proportionate to size(PPS)sampling methods. The final analytical sample comprised 60,270 participants.Trained professionals conducted cycloplegic optometry and measured ocular biometric parameters.Data underwent weighted analysis using nonparametric tests and receiver operating characteristic(ROC)curve analysis to establish predictive thresholds for myopia detection. Results:Overall myopia prevalence reached 29.24%,demonstrating a progressive increase across school grades.Median SER values exhibited increasingly negative trends with advancing educational levels.Correlation analysis revealed that AL/CR demonstrated stronger associations with SER(R=−0.750,P<0.001)compared to AL alone(R=−0.657,P<0.001).ROC analysis confirmed superior predictive accuracy for AL/CR over AL across all age groups, with area under the curve values approaching or exceeding 0.88 among upper-grade students. Conclusions:Both AL/CR and AL serve as effective indicators for identifying children at elevated myopia risk during early childhood.Routine monitoring of AL and AL/CR within school health programs can facilitate early intervention strategies and myopia control measures, particularly in regions with limited access to cycloplegic optometry services. 展开更多

AIM:To propose a novel classification system for late postoperative capsular block syndrome(CBS)based on the turbidity of intracapsular fluid, and to investigate the imaging and refractive changes before and after Nd:YAG laser posterior capsulotomy for each subtype.METHODS:A retrospective analysis was performed on 5 eyes from 5 patients with late postoperative CBS.Patients were categorized into turbid(3 eyes)or clear(2 eyes)types based on the turbidity of intracapsular fluid.Uncorrected visual acuity(UCVA),best-corrected visual acuity(BCVA),refractive status,slit-lamp images,Pentacam Scheimpflug data, and ultrasound biomicroscopy(UBM)images were evaluated before and 1mo after successful Nd:YAG laser posterior capsulotomy.RESULTS:Nd:YAG laser posterior capsulotomy significantly improved UCVA and BCVA in all 5 late CBS eyes.Pentacam imaging:turbid intracapsular fluid showed hyperreflection between posterior capsule and intraocular lens(IOL);clear fluid showed hyporeflection.UBM:posterior capsule was clear in turbid type but poorly defined in clear type.Capsulotomy increased aqueous depth only in clear type.Refractive changes:turbid fluid induced myopic shift.Clear type myopic shift was due to anterior IOL displacement and clear fluid-induced concave lens effect.CONCLUSION:Nd:YAG laser posterior capsulotomy is effective for late CBS.Turbid and clear late CBS types differ in imaging(Pentacam/UBM)and refractive mechanisms, supporting the proposed classification’s clinical value. 展开更多
