Radical prostatectomy is a commonly used surgical method in cases of localized prostate cancer.In recent years,with the advent of new medical technologies and surgical techniques,the evolution of radical prostatectomy...Radical prostatectomy is a commonly used surgical method in cases of localized prostate cancer.In recent years,with the advent of new medical technologies and surgical techniques,the evolution of radical prostatectomy has revolutionized,especially in robot-assisted radical prostatectomy(RARP).The evolution of surgical approaches for radical prostatectomy has occurred in three stages:open surgery,laparoscopic intervention,and robot-assisted surgery.Regarding the functional recovery of patients who underwent laparoscopic radical prostatectomy or RARP,with the improvement of disease conditions,oncological prognosis of patients was not compromised.Particularly,RARP boasts distinguished novel techniques and approaches for maintaining urinary continence and sexual function in the short-and long-term.In addition,studies in the last two decades have shown its correlation with decreasing postoperative morbidity.In this paper,the available literatures related to the surgical approaches ranging from open surgery to RARP were reviewed,the superiority of any novel procedure was analyzed,and the advantages and disadvantages among the three modalities were compared,hoping to provide guidance to urologists when considering surgical approaches in the treatment of localized prostate cancer.展开更多
Objective:Radical prostatectomy(RP)is considered the gold standard treatment for patients with localized prostate cancer(PCa).All patients undergoing RP,either open,laparoscopic,or robotic,are at risk of developing ur...Objective:Radical prostatectomy(RP)is considered the gold standard treatment for patients with localized prostate cancer(PCa).All patients undergoing RP,either open,laparoscopic,or robotic,are at risk of developing urinary incontinence after RP.Our study aimed to analyze possible predictors of early postoperative(within 3 months)urinary continence(UC)outcomes following extraperitoneal single-site robot-assisted RP(ss-RARP)for localized PCa.Methods:We retrospectively reviewed data from 113 patients with localized PCa who were operated on by a highly experienced surgeon between September 2022 and December 2023.Patient characteristics(age,body mass index,prostate volume,prostate dimensions,preoperative and postoperative membranous urethral length,and percentage of membranous urethra removed)were analyzed using logistic regression to determine the independent factors contributing to short-term UC recovery after extraperitoneal ss-RARP.From the time of urinary catheter removal after surgery,the recovery of UC was followed up every month,and UC was defined as using≤1 safety pad daily,known as social continence.Results:The proportions of continent patients immediately after urinary catheter removal,1 month,and 3 months after extraperitoneal ss-RARP were 22%,49%,and 82%,respectively.The multivariate logistic regression analysis showed that the percentage of membranous urethra removed(p=0.016)and prostate volume(p=0.049)were associated with social UC recovery immediately after urinary catheter removal,and craniocaudal dimension of the prostate(p=0.042)and age(p=0.014)were associated with social UC recovery 1 month and 3 months after extraperitoneal ss-RARP,respectively.Conclusion:The percentage of membranous urethra removed,prostate volume,craniocaudal dimension of the prostate,and age are independent risk factors for social UC early after extraperitoneal ss-RARP.展开更多

Harnessing bacteria for superoxide production in bioremediation holds immense promise,yet its practical application is hindered by slow production rates and the relatively weak redox potential of superoxide.This study...Harnessing bacteria for superoxide production in bioremediation holds immense promise,yet its practical application is hindered by slow production rates and the relatively weak redox potential of superoxide.This study delves into a cost-effective approach to amplify superoxide production using an Arthrobacter strain,a prevalent soil bacterial genus.Our research reveals that introducing a carbon source along with specific iron-binding ligands,including deferoxamine(DFO),diethylenetriamine pentaacetate(DTPA),citrate,and oxalate,robustly augments microbial superoxide generation.Moreover,our findings suggest that these iron-binding ligands play a pivotal role in converting superoxide into hydroxyl radicals by modulating the electron transfer rate between Fe(Ⅲ)/Fe(Ⅱ)and superoxide.Remarkably,among the tested ligands,only DTPA emerges as a potent promoter of this conversion process when complexed with Fe(Ⅲ).We identify an optimal Fe(Ⅲ)to DTPA ratio of approximately 1:1 for enhancing hydroxyl radical production within the Arthrobacter culture.This research underscores the efficacy of simultaneously introducing carbon sources and DTPA in facilitating superoxide production and its subsequent conversion to hydroxyl radicals,significantly elevating bioremediation performance.Furthermore,our study reveals that DTPA augments superoxide production in cultures of diverse soils,with various soil microorganisms beyond Arthrobacter identified as contributors to superoxide generation.This emphasizes the universal applicability of DTPA across multiple bacterial genera.In conclusion,our study introduces a promising methodology for enhancing microbial superoxide production and its conversion into hydroxyl radicals.These findings hold substantial implications for the deployment of microbial reactive oxygen species in bioremediation,offering innovative solutions for addressing environmental contamination challenges.展开更多

Objective:To evaluate the efficacy and user satisfaction of the MAIA^(TM)telemedicine platform for postoperative telemonitoring and telerehabilitation in patients undergoing robot-assisted radical prostatectomy(RARP)....Objective:To evaluate the efficacy and user satisfaction of the MAIA^(TM)telemedicine platform for postoperative telemonitoring and telerehabilitation in patients undergoing robot-assisted radical prostatectomy(RARP).Methods:Patients undergoing RARP(from April 2022 to January 2023)were divided into Group A,following the standard rehabilitation protocol,and Group B using the MAIA^(TM)platform for the first 30-day post-intervention period.Perioperative,pathological,and functional variables(continence and potency rates,assessed via validated questionnaires)were collected and telerehabilitation protocol’s adherence was monitored.The telerehabilitation system included an online platform for medical providers and a patient application,facilitating data acquisition,management,treatment planning,and monitoring.Patient and provider satisfactionwere evaluated using the visual analogue scale score and validated telemedicine satisfaction questionnaire.Results:Totally,227 patients were enrolled.No differences in perioperative or pathological variables were found.Group B had higher potency recovery rates than Group A(p=0.031);the potency recovery rates at 1 month and 3 months for Group B were 45%and 57%,respectively,and in Group A were 34%and 48%,respectively.At the multivariate analysis,MAIA^(TM)use and the nerve-sparing(NS)were independent predictors of erectile function at both 1 month(MAIA^(TM)use:odd ratio[OR]2.03,95%confidence interval[CI]1.13-3.63,p=0.018;NS:OR 2.08,95%CI 1.06-4.12,p=0.034)and 3 months(MAIA^(TM)use:OR 1.89,95%CI 1.07-3.34,p=0.028;NS:OR 1.90;95%CI 1.02-3.54,p=0.044).Adherencemonitoring revealed 4.6%of patients in Group B reported exercise issues due to pelvic pain onset and 10%did not take oral phosphodiesterase 5-inhibitors due to myalgia.All those patients restarted the rehabilitation program after televisit to address the causing problem,allowing restarting of a rehabilitation program.Patients reported high satisfaction with the MAIA^(TM)platform use(mean visual analogue scale score:88.7).Conclusion:The MAIA^(TM)telemedicine platform seems to have a role in optimizing early potency recovery after RARP.Patient and provider satisfaction levels were high,emphasizing the user-friendliness of the platform.展开更多
The performance of hydrogel radical polymerization under ambient conditions is a major challenge because oxygen is an effective radical quencher and the steps to remove or neutralize it are time consuming and laboriou...The performance of hydrogel radical polymerization under ambient conditions is a major challenge because oxygen is an effective radical quencher and the steps to remove or neutralize it are time consuming and laborious.A self-initiating system consisting of transition metals and acetylacetone has been successfully developed.The system is capable of initiating free radical polymerization of hydrogels at room temperature under aerobic conditions,which is attributed to carbon radicals generated by the oxidation of acetylacetone.Some of these carbon radicals reduce oxygen to generate hydroxyl radicals,which together induce self-coagulation of hydrogels.The polymerization system was effective for a variety of monomer and hydrogel swelling and shrinking schemes,and the reaction remained successful when exposed to saturated oxygen.In conclusion,the results demonstrate that the present strategy is an effective approach to addressing the challenge of deoxygenation in polymer synthesis,and provides a convenient method for synthesizing multifunctional hydrogels under ambient conditions.展开更多

BACKGROUND Patients with gastric cancer may have a risk of depression after radical surgery,which affects recovery and requires intensive intervention.AIM To explore the role of Bifidobacterium triple viable bacteria-...BACKGROUND Patients with gastric cancer may have a risk of depression after radical surgery,which affects recovery and requires intensive intervention.AIM To explore the role of Bifidobacterium triple viable bacteria-assisted mirtazapine in patients with depression after radical surgery for gastric cancer.METHODS This study included 80 patients with gastric cancer who experienced depression after undergoing radical surgery from January 2022 to December 2023 at Jiangnan University Hospital in Wuxi city in Jiangsu province.The patients were categorized into the control group(n=40)treated with mirtazapine and observation group(n=40)treated with combined Bifidobacterium triple viable bacteria-assisted mirtazapine.Neuroendocrine index,intestinal flora level,nutrition status,and patient quality of life were analyzed.RESULTS Before drug index level,no statistically significant difference was observed between the two groups (P>0.05).After drug administration,both groups experienced a decline in depression scores,cortisol,Escherichia coli,Enterococcus faecalis levels,and quality of life scores,whereas dopamine,serotonin,Lactobacillus,Bifidobacteria,hemoglobin,serum albumin and prealbumin levels increased.The observation group demonstrated significantly greater improvements across these indicators,with significant differences within groups and between groups(P<0.05).CONCLUSION Bifidobacterium has a relieving effect on depression in patients after radical surgery for gastric cancer,improves neuroendocrine status,regulates intestinal flora,and improves nutritional status and quality of life.展开更多
Lignin pyrolysis leads to the formation of diverse phenolic products that bear structural similarities to natural lignin,and the related mechanism has been widely explored based on the linkage cleavageinvolved reactio...Lignin pyrolysis leads to the formation of diverse phenolic products that bear structural similarities to natural lignin,and the related mechanism has been widely explored based on the linkage cleavageinvolved reactions.However,some unusual pyrolytic products exhibiting significant structure deviations from lignin,such as aldehydes,remain obscure in mechanism due to long-standing neglect of their formation pathways.The present work found the pivotal role of aryl migration,a special radical-mediated rearrangement process,in governing the formation of these atypical products for the first time.Herein,density functional theory calculations,electronic structure analyses,and pyrolysis experiments were combined to investigate rearrangement patterns and prerequisite structural characteristics of aryl migration by employing typical radicals derived from linkages and substituents of lignin as models.The results indicate that the radical with an unpaired electron located on the second atom of the aromatic side chain can undergo three-membered aryl 1,2-migration triggered by exo cyclization with the best superiority,determining the generation of aldehydes,alkenes,and other products through subsequent cleavage reactions.A clear correlation among the initial geometric and electronic structures of lignin,the patterns and types of aryl migration,the energy barriers,and the end products was established.This study contributes to systematically elucidating rearrangement mechanisms and constructing a more comprehensive lignin pyrolysis mechanism network.展开更多

BACKGROUND Gastric cancer remains a significant global health concern.Radical gastrectomy is the primary curative treatment.Diabetes mellitus is a common comorbidity in patients undergoing surgery for gastric cancer,i...BACKGROUND Gastric cancer remains a significant global health concern.Radical gastrectomy is the primary curative treatment.Diabetes mellitus is a common comorbidity in patients undergoing surgery for gastric cancer,including radical gastrectomy.Previous studies have suggested that diabetes can negatively affect postoperative outcomes,such as wound healing,infection rates,and overall recovery.However,the specific impact of diabetes on recovery after radical gastrectomy for gastric cancer remains poorly understood.evaluate the influence of diabetes on postope-rative recovery,including hospital stay duration,complications,and readmission rates,in patients undergoing gastrectomy for gastric cancer.Understanding these effects could help optimize perioperative management and improve patient out-comes.gastric cancer and associated postoperative outcomes.METHODS This retrospective cohort study was performed at the Endocrinology Department of Xuanwu Hospital,Capital Medical University,Beijing,China.We examined patients who underwent radical gastrectomy for cancer between January 2010 and December 2020.The patients were divided into the diabetes and non-diabetes groups.The main outcomes included length of hospital stay,postoperative com-plications,and 30-day readmission rate.Secondary outcomes included quality of life indicators.Propensity score matching was used to adjust for potential con-founding factors.RESULTS A total of 1210 patients were included in the study,with 302 diabetic patients and 908 non-diabetic patients.After propensity score matching,280 patients were included in each group.Diabetic patients demonstrated significantly longer hospital stays(mean difference 2.3 days,95%CI:1.7-2.9,P<0.001)and higher rates of postoperative complications(OR 1.68,95%CI:1.32-2.14,P<0.001).The 30-day readmission rate was also higher in the diabetic group as compared to the non-diabetic group(12.5%vs 7.8%,P=0.02).CONCLUSION Patients with diabetes mellitus undergoing radical gastrectomy for gastric cancer experience prolonged hospital stay,increased postoperative complications,and higher readmission rates,thus requiring optimized perioperative management strategies.展开更多
BACKGROUND Colorectal cancer(CRC)is a malignant tumor with high morbidity and mortality rates worldwide.With the development of medical imaging technology,imaging features are playing an increasingly important role in...BACKGROUND Colorectal cancer(CRC)is a malignant tumor with high morbidity and mortality rates worldwide.With the development of medical imaging technology,imaging features are playing an increasingly important role in the prognostic evaluation of CRC.Laparoscopic radical resection is a common surgical approach for treating CRC.However,research on the link between preoperative imaging and short-term prognosis in this context is limited.We hypothesized that specific preope-rative imaging features can predict the short-term prognosis in patients under-going laparoscopic CRC resection.AIM To investigate the imaging features of CRC and analyze their correlation with the short-term prognosis of laparoscopic radical resection.METHODS This retrospective study conducted at the Affiliated Cancer Hospital of Shandong First Medical University included 122 patients diagnosed with CRC who under-went laparoscopic radical resection between January 2021 and February 2024.All patients underwent magnetic resonance imaging(MRI)and were diagnosed with CRC through pathological examination.MRI data and prognostic indicators were collected 30 days post-surgery.Logistic regression analysis identified imaging fea-tures linked to short-term prognosis,and a receiver operating characteristic(ROC)curve was used to evaluate the predictive value.RESULTS Among 122 patients,22 had irregular,low-intensity tumors with adjacent high signals.In 55,tumors were surrounded by alternating signals in the muscle layer.In 32,tumors extended through the muscular layer and blurred boundaries with perienteric adipose tissue.Tumor signals appeared in the adjacent tissues in 13 patients with blurred gaps.Logistic regression revealed differences in longitudinal tumor length,axial tumor length,volume transfer constant,plasma volume fraction,and apparent diffusion coefficient among patients with varying prognostic results.ROC analysis indicated that the areas under the curve for these parameters were 0.648,0.927,0.821,0.809,and 0.831,respectively.Sensitivity values were 0.643,0.893,0.607,0.714,and 0.714,and specificity 0.702,0.904,0.883,0.968,and 0.894(P<0.05).CONCLUSION The imaging features of CRC correlate with the short-term prognosis following laparoscopic radical resection.These findings provide valuable insights for clinical decision-making.展开更多

文摘Radical prostatectomy is a commonly used surgical method in cases of localized prostate cancer.In recent years,with the advent of new medical technologies and surgical techniques,the evolution of radical prostatectomy has revolutionized,especially in robot-assisted radical prostatectomy(RARP).The evolution of surgical approaches for radical prostatectomy has occurred in three stages:open surgery,laparoscopic intervention,and robot-assisted surgery.Regarding the functional recovery of patients who underwent laparoscopic radical prostatectomy or RARP,with the improvement of disease conditions,oncological prognosis of patients was not compromised.Particularly,RARP boasts distinguished novel techniques and approaches for maintaining urinary continence and sexual function in the short-and long-term.In addition,studies in the last two decades have shown its correlation with decreasing postoperative morbidity.In this paper,the available literatures related to the surgical approaches ranging from open surgery to RARP were reviewed,the superiority of any novel procedure was analyzed,and the advantages and disadvantages among the three modalities were compared,hoping to provide guidance to urologists when considering surgical approaches in the treatment of localized prostate cancer.
文摘Objective:Radical prostatectomy(RP)is considered the gold standard treatment for patients with localized prostate cancer(PCa).All patients undergoing RP,either open,laparoscopic,or robotic,are at risk of developing urinary incontinence after RP.Our study aimed to analyze possible predictors of early postoperative(within 3 months)urinary continence(UC)outcomes following extraperitoneal single-site robot-assisted RP(ss-RARP)for localized PCa.Methods:We retrospectively reviewed data from 113 patients with localized PCa who were operated on by a highly experienced surgeon between September 2022 and December 2023.Patient characteristics(age,body mass index,prostate volume,prostate dimensions,preoperative and postoperative membranous urethral length,and percentage of membranous urethra removed)were analyzed using logistic regression to determine the independent factors contributing to short-term UC recovery after extraperitoneal ss-RARP.From the time of urinary catheter removal after surgery,the recovery of UC was followed up every month,and UC was defined as using≤1 safety pad daily,known as social continence.Results:The proportions of continent patients immediately after urinary catheter removal,1 month,and 3 months after extraperitoneal ss-RARP were 22%,49%,and 82%,respectively.The multivariate logistic regression analysis showed that the percentage of membranous urethra removed(p=0.016)and prostate volume(p=0.049)were associated with social UC recovery immediately after urinary catheter removal,and craniocaudal dimension of the prostate(p=0.042)and age(p=0.014)were associated with social UC recovery 1 month and 3 months after extraperitoneal ss-RARP,respectively.Conclusion:The percentage of membranous urethra removed,prostate volume,craniocaudal dimension of the prostate,and age are independent risk factors for social UC early after extraperitoneal ss-RARP.

文摘The significant role of square root mapping in probability,statistics,physics,architecture and engineering motivates us to emphasize on a new radical functional equation arising from a square root and reciprocal square root mappings.The interesting attribute of this equation is that it has both a square root mapping and a reciprocal square root mapping as solutions.We establish that the hyperstabilities of this equation exist using a fixed point alternative theorem.It is also demonstrated with an example that the stability may fail in special cases.

文摘The development of luminescent radicals withα-type transition properties is significant for advancing the understanding of luminescence mechanisms and photophysical properties in radical-based systems.Here,we present a straightforward strategy for acquiring stable luminescent radicals withα-type transition by directly decorating bis(2,4,6-trichlorophenyl)methyl radicals(BTM)-based luminescent radicals with strong electron acceptors.This approach effectively narrows the energy gap between the singly occupied molecular orbital(SOMOα)and lowest doubly unoccupied molecular orbital(LUMOα)(ΔE_(SOMOα→LOMOα)),enabling a transform of luminescent radicals from the conventionalβ-type transition to the rareα-type transition upon D_(0)→D_(1)excitation process.Theα-type transition was experimentally validated through the fabrication of organic light-emitting diodes(OLEDs)incorporating appropriate host materials.The result also expands the selection of available host materials for OLED devices exploiting radicals as emitters,as more host materials with higher highest occupied molecular orbital(HOMO)can now be considered.This work not only establishes a rational molecular design strategy for luminescent radicals withα-type transition but also provides valuable insights to guide future research in radical-based optoelectronic materials.

文摘Harnessing bacteria for superoxide production in bioremediation holds immense promise,yet its practical application is hindered by slow production rates and the relatively weak redox potential of superoxide.This study delves into a cost-effective approach to amplify superoxide production using an Arthrobacter strain,a prevalent soil bacterial genus.Our research reveals that introducing a carbon source along with specific iron-binding ligands,including deferoxamine(DFO),diethylenetriamine pentaacetate(DTPA),citrate,and oxalate,robustly augments microbial superoxide generation.Moreover,our findings suggest that these iron-binding ligands play a pivotal role in converting superoxide into hydroxyl radicals by modulating the electron transfer rate between Fe(Ⅲ)/Fe(Ⅱ)and superoxide.Remarkably,among the tested ligands,only DTPA emerges as a potent promoter of this conversion process when complexed with Fe(Ⅲ).We identify an optimal Fe(Ⅲ)to DTPA ratio of approximately 1:1 for enhancing hydroxyl radical production within the Arthrobacter culture.This research underscores the efficacy of simultaneously introducing carbon sources and DTPA in facilitating superoxide production and its subsequent conversion to hydroxyl radicals,significantly elevating bioremediation performance.Furthermore,our study reveals that DTPA augments superoxide production in cultures of diverse soils,with various soil microorganisms beyond Arthrobacter identified as contributors to superoxide generation.This emphasizes the universal applicability of DTPA across multiple bacterial genera.In conclusion,our study introduces a promising methodology for enhancing microbial superoxide production and its conversion into hydroxyl radicals.These findings hold substantial implications for the deployment of microbial reactive oxygen species in bioremediation,offering innovative solutions for addressing environmental contamination challenges.
文摘A photocatalyst-free photoinduced radical difluoromethylation approach for the convenient synthesis of 3-CF_(2)H-coumarins from alkynoates by difluoromethyl phenoxathiinium salt(PT-CF_(2)H^(+)BF_(4)^(-))was developed.This transformation in-volves radical difluoromethylation/5-exo spirocyclization/ester migration rearrangement cascade process and features mild reaction conditions,broad substrate scope and good functional groups tolerance.Using this method,a wide range of alkynoates was successfully converted to desired 3-CF_(2)H-coumarins in moderate to good yields.
文摘A new cobalt(Ⅱ)-radical complex:[Co(im4-py)_(2)(PNB)_(2)](im4-py=2-(4'-pyridyl)-4,4,5,5-tetramethylimidazole-1-oxyl,HPNB=p-nitrobenzoic acid)has been synthesized and characterized by X-ray diffraction analysis,elemental analysis,IR,and magnetic properties.X-ray diffraction analysis shows that the complex exists as mononuclear molecules and Co(Ⅱ)ion is four-coordinated with two radicals and two PNB-ligands.The magnetic susceptibility study indicates the complex exhibits weak ferromagnetic interactions between cobalt(Ⅱ)and im4-py radical.The magnetic property is explained by the magnetic and structure exchange mechanism.CCDC:976028.
文摘Objective:To evaluate the efficacy and user satisfaction of the MAIA^(TM)telemedicine platform for postoperative telemonitoring and telerehabilitation in patients undergoing robot-assisted radical prostatectomy(RARP).Methods:Patients undergoing RARP(from April 2022 to January 2023)were divided into Group A,following the standard rehabilitation protocol,and Group B using the MAIA^(TM)platform for the first 30-day post-intervention period.Perioperative,pathological,and functional variables(continence and potency rates,assessed via validated questionnaires)were collected and telerehabilitation protocol’s adherence was monitored.The telerehabilitation system included an online platform for medical providers and a patient application,facilitating data acquisition,management,treatment planning,and monitoring.Patient and provider satisfactionwere evaluated using the visual analogue scale score and validated telemedicine satisfaction questionnaire.Results:Totally,227 patients were enrolled.No differences in perioperative or pathological variables were found.Group B had higher potency recovery rates than Group A(p=0.031);the potency recovery rates at 1 month and 3 months for Group B were 45%and 57%,respectively,and in Group A were 34%and 48%,respectively.At the multivariate analysis,MAIA^(TM)use and the nerve-sparing(NS)were independent predictors of erectile function at both 1 month(MAIA^(TM)use:odd ratio[OR]2.03,95%confidence interval[CI]1.13-3.63,p=0.018;NS:OR 2.08,95%CI 1.06-4.12,p=0.034)and 3 months(MAIA^(TM)use:OR 1.89,95%CI 1.07-3.34,p=0.028;NS:OR 1.90;95%CI 1.02-3.54,p=0.044).Adherencemonitoring revealed 4.6%of patients in Group B reported exercise issues due to pelvic pain onset and 10%did not take oral phosphodiesterase 5-inhibitors due to myalgia.All those patients restarted the rehabilitation program after televisit to address the causing problem,allowing restarting of a rehabilitation program.Patients reported high satisfaction with the MAIA^(TM)platform use(mean visual analogue scale score:88.7).Conclusion:The MAIA^(TM)telemedicine platform seems to have a role in optimizing early potency recovery after RARP.Patient and provider satisfaction levels were high,emphasizing the user-friendliness of the platform.

文摘The performance of hydrogel radical polymerization under ambient conditions is a major challenge because oxygen is an effective radical quencher and the steps to remove or neutralize it are time consuming and laborious.A self-initiating system consisting of transition metals and acetylacetone has been successfully developed.The system is capable of initiating free radical polymerization of hydrogels at room temperature under aerobic conditions,which is attributed to carbon radicals generated by the oxidation of acetylacetone.Some of these carbon radicals reduce oxygen to generate hydroxyl radicals,which together induce self-coagulation of hydrogels.The polymerization system was effective for a variety of monomer and hydrogel swelling and shrinking schemes,and the reaction remained successful when exposed to saturated oxygen.In conclusion,the results demonstrate that the present strategy is an effective approach to addressing the challenge of deoxygenation in polymer synthesis,and provides a convenient method for synthesizing multifunctional hydrogels under ambient conditions.

文摘Chain-growth radical polymerization of vinyl monomers is essential for producing a wide range of materials with properties tailored to specific applications.However,the inherent resistance of the polymer's C―C backbone to degradation raises significant concerns regarding long-term environmental persistence,which also limits their potential in biomedical applications.To address these challenges,researchers have developed strategies to either degrade preexisting vinyl polymers or incorporate cleavable units into the backbone to modify them with enhanced degradability.This review explores the various approaches aimed at achieving backbone degradability in chain-growth radical polymerization of vinyl monomers,while also highlighting future research directions for the development of application-driven degradable vinyl polymers.
文摘BACKGROUND Patients with gastric cancer may have a risk of depression after radical surgery,which affects recovery and requires intensive intervention.AIM To explore the role of Bifidobacterium triple viable bacteria-assisted mirtazapine in patients with depression after radical surgery for gastric cancer.METHODS This study included 80 patients with gastric cancer who experienced depression after undergoing radical surgery from January 2022 to December 2023 at Jiangnan University Hospital in Wuxi city in Jiangsu province.The patients were categorized into the control group(n=40)treated with mirtazapine and observation group(n=40)treated with combined Bifidobacterium triple viable bacteria-assisted mirtazapine.Neuroendocrine index,intestinal flora level,nutrition status,and patient quality of life were analyzed.RESULTS Before drug index level,no statistically significant difference was observed between the two groups (P>0.05).After drug administration,both groups experienced a decline in depression scores,cortisol,Escherichia coli,Enterococcus faecalis levels,and quality of life scores,whereas dopamine,serotonin,Lactobacillus,Bifidobacteria,hemoglobin,serum albumin and prealbumin levels increased.The observation group demonstrated significantly greater improvements across these indicators,with significant differences within groups and between groups(P<0.05).CONCLUSION Bifidobacterium has a relieving effect on depression in patients after radical surgery for gastric cancer,improves neuroendocrine status,regulates intestinal flora,and improves nutritional status and quality of life.

文摘Lignin pyrolysis leads to the formation of diverse phenolic products that bear structural similarities to natural lignin,and the related mechanism has been widely explored based on the linkage cleavageinvolved reactions.However,some unusual pyrolytic products exhibiting significant structure deviations from lignin,such as aldehydes,remain obscure in mechanism due to long-standing neglect of their formation pathways.The present work found the pivotal role of aryl migration,a special radical-mediated rearrangement process,in governing the formation of these atypical products for the first time.Herein,density functional theory calculations,electronic structure analyses,and pyrolysis experiments were combined to investigate rearrangement patterns and prerequisite structural characteristics of aryl migration by employing typical radicals derived from linkages and substituents of lignin as models.The results indicate that the radical with an unpaired electron located on the second atom of the aromatic side chain can undergo three-membered aryl 1,2-migration triggered by exo cyclization with the best superiority,determining the generation of aldehydes,alkenes,and other products through subsequent cleavage reactions.A clear correlation among the initial geometric and electronic structures of lignin,the patterns and types of aryl migration,the energy barriers,and the end products was established.This study contributes to systematically elucidating rearrangement mechanisms and constructing a more comprehensive lignin pyrolysis mechanism network.

文摘Herein,we developed the first example of copper-catalyzed silicon radical-initiated 1,4-silylcyanation of unactivated 1,3-enynes,which provided an efficient method to access CN-bearing tri-and tetrasubstituted homoallenylsilane derivatives in high yields with excellent regioselectivities.This protocol featured good functional group compatibility and broad substrate scopes,enabling the formation of C-Si bond under cheap copper catalyst with a low loading.Furthermore,this means showed potential application value in the late-stage functionalization of natural products.
文摘This article explored the application of dexmedetomidine(Dex),a highly selective alpha-2 agonist,in managing postoperative cognitive dysfunction(POCD)in elderly patients undergoing radical colon cancer surgery.Aging is associated with a progressive decline in physiological functions and an increased risk of adverse surgical outcomes,including POCD,which encompasses many neurocognitive disorders that manifest during the perioperative period.The aging population is at a higher risk for POCD,which can lead to prolonged hospital stays,delayed recovery,and increased healthcare costs.Dex has neuroprotective,opioid-sparing,and sympatholytic properties,which reduces the incidence and severity of POCD.Dex was introduced for sedation in patients receiving mechanical ventilation but has since been adopted in anesthesia due to its multifaceted benefits.Its appli-cation extends to sedation,analgesia,maintenance of anesthesia,and controlling delirium.Its neuroprotective and anti-inflammatory effects have been explored in managing POCD.This article discussed the broad range of patient and procedure-related risk factors for POCD.Early identification and intervention are crucial to prevent the progression of POCD,which can have severe physical,psychological,and economic consequences.The article underscored the importance of a mul-tidisciplinary approach in managing POCD,involving the optimization of comor-bidities,depth of anesthesia monitoring,hemodynamic stability,and cerebral oxygenation monitoring.

文摘An N-heterocyclic carbene(NHC)catalyzed enantioselective cyclisation and trifluoromethylation of olefins with cinnamaldehydes via radical relay cross-coupling in the presence of Togni reagent is reported andδ-lactones tolerated with stereogenic centers atβ-andγ-positions are obtained in moderate to high yields and with high enantioselectivities.Further computational studies explain that the radical crosscoupling step is the key to determining the enantioselectivity.Energy analysis of key transition states and intermediates also provides a reasonable explanation for the difficulty of diastereoselective control.DFT calculations also reveal that the hydrogen-bonding interaction plays a vital role in the promotion of this chemistry.

文摘Rare-earth supramolecular compounds,such as lanthanide organic polyhedrons(LOPs),are of particular interest due to their many possible applications in various fields.Here we report the first syntheses of Ln_(4)(L^(·+))_(4)-type(Ln,lanthanides;L^(·+),radical ligand)radical-bridged lanthanide organic tetrahedrons by self-assembly of face-capping triphenylamine(TPA)-cored radical ligand with different lanthanide ions.Remarkable coordination enhanced radical stability has been observed,with half-life times(t_(1/2))for L_(1)^(·+),La_(4)(L_(1)^(·+))_(4),Eu_(4)(L_(1)^(·+))_(4),Gd_(4)(L_(1)^(·+))_(4),Tb_(4)(L_(1)^(·+))_(4)and Lu_(4)(L_(1)^(·+))_(4)estimated to be 53 min,482 min,624 min,1248 min,822 min and 347 min,respectively.The TPA radical in Ln_(4)(L_(1)^(·+))_(4)containing paramagnetic Ln ions(Ln=Eu^(Ⅲ),Gd^(Ⅲ)and Tb^(Ⅲ))is observed to be more stable than that in Ln_(4)(L_(1)^(·+))_(4)(Ln=La^(Ⅲ)and Lu^(Ⅲ))constructed by diamagnetic Ln ions.This difference in radical stability is possibly due to the magnetic interactions between paramagnetic Ln^(Ⅲ)ions and L_(1)^(·+)ligands,as confirmed by electron paramagnetic resonance(EPR)in La_(4)(L)_(4)(L=L_(1)and L_(1)^(·+))and Tb_(4)(L)_(4)(L=L_(1)and L_(1)^(·+)),and magnetic susceptibility measurements in Tb·_(4)(L)_(4)(L=L_(1)and L_(1)^(·+)).Our study reveals the coordination of radical ligands with lanthanide ions can improve the radical stability,which is crucial for their applications.

文摘The tert-butyl nitrite as a bifunctional reagent mediated radical alkene difunctionalization has emerged as a powerful strategy for synthesis of structurally diverse oxime-containing compounds.However,the phosphorus-centered radical initiated transformations remain largely elusive.Herein,a visible-lightinduced radical phosphinoyloximation of alkenes with secondary phosphine oxides and tert-butyl nitrite has been developed under photocatalyst-and metal-free conditions.This protocol features mild conditions,broad substrate scope,good functional tolerance,and operational simplicity,yielding a diverse array ofα-phosphinoyl oximes in moderate to good yields with high stereoselectivities.The photomediated homolytic cleavage of O–NO bond of tert-butyl nitrite generates the reactive tert-butoxyl radical and persistent NO radical to act as both HAT reagent and the source of oximes.
文摘BACKGROUND Gastric cancer remains a significant global health concern.Radical gastrectomy is the primary curative treatment.Diabetes mellitus is a common comorbidity in patients undergoing surgery for gastric cancer,including radical gastrectomy.Previous studies have suggested that diabetes can negatively affect postoperative outcomes,such as wound healing,infection rates,and overall recovery.However,the specific impact of diabetes on recovery after radical gastrectomy for gastric cancer remains poorly understood.evaluate the influence of diabetes on postope-rative recovery,including hospital stay duration,complications,and readmission rates,in patients undergoing gastrectomy for gastric cancer.Understanding these effects could help optimize perioperative management and improve patient out-comes.gastric cancer and associated postoperative outcomes.METHODS This retrospective cohort study was performed at the Endocrinology Department of Xuanwu Hospital,Capital Medical University,Beijing,China.We examined patients who underwent radical gastrectomy for cancer between January 2010 and December 2020.The patients were divided into the diabetes and non-diabetes groups.The main outcomes included length of hospital stay,postoperative com-plications,and 30-day readmission rate.Secondary outcomes included quality of life indicators.Propensity score matching was used to adjust for potential con-founding factors.RESULTS A total of 1210 patients were included in the study,with 302 diabetic patients and 908 non-diabetic patients.After propensity score matching,280 patients were included in each group.Diabetic patients demonstrated significantly longer hospital stays(mean difference 2.3 days,95%CI:1.7-2.9,P<0.001)and higher rates of postoperative complications(OR 1.68,95%CI:1.32-2.14,P<0.001).The 30-day readmission rate was also higher in the diabetic group as compared to the non-diabetic group(12.5%vs 7.8%,P=0.02).CONCLUSION Patients with diabetes mellitus undergoing radical gastrectomy for gastric cancer experience prolonged hospital stay,increased postoperative complications,and higher readmission rates,thus requiring optimized perioperative management strategies.
文摘BACKGROUND Colorectal cancer(CRC)is a malignant tumor with high morbidity and mortality rates worldwide.With the development of medical imaging technology,imaging features are playing an increasingly important role in the prognostic evaluation of CRC.Laparoscopic radical resection is a common surgical approach for treating CRC.However,research on the link between preoperative imaging and short-term prognosis in this context is limited.We hypothesized that specific preope-rative imaging features can predict the short-term prognosis in patients under-going laparoscopic CRC resection.AIM To investigate the imaging features of CRC and analyze their correlation with the short-term prognosis of laparoscopic radical resection.METHODS This retrospective study conducted at the Affiliated Cancer Hospital of Shandong First Medical University included 122 patients diagnosed with CRC who under-went laparoscopic radical resection between January 2021 and February 2024.All patients underwent magnetic resonance imaging(MRI)and were diagnosed with CRC through pathological examination.MRI data and prognostic indicators were collected 30 days post-surgery.Logistic regression analysis identified imaging fea-tures linked to short-term prognosis,and a receiver operating characteristic(ROC)curve was used to evaluate the predictive value.RESULTS Among 122 patients,22 had irregular,low-intensity tumors with adjacent high signals.In 55,tumors were surrounded by alternating signals in the muscle layer.In 32,tumors extended through the muscular layer and blurred boundaries with perienteric adipose tissue.Tumor signals appeared in the adjacent tissues in 13 patients with blurred gaps.Logistic regression revealed differences in longitudinal tumor length,axial tumor length,volume transfer constant,plasma volume fraction,and apparent diffusion coefficient among patients with varying prognostic results.ROC analysis indicated that the areas under the curve for these parameters were 0.648,0.927,0.821,0.809,and 0.831,respectively.Sensitivity values were 0.643,0.893,0.607,0.714,and 0.714,and specificity 0.702,0.904,0.883,0.968,and 0.894(P<0.05).CONCLUSION The imaging features of CRC correlate with the short-term prognosis following laparoscopic radical resection.These findings provide valuable insights for clinical decision-making.

文摘The photocatalytic reduction of CO_(2)is a crucial area of research aimed at addressing the dual challenges of mitigating rising CO_(2)emissions and producing sustainable chemical feedstocks.While multielectron reduction pathways for CO_(2)are well explored,the single electron reduction to produce the highly reactive carbon dioxide radical anion(CO_(2)^(·-))remains challenging yet promising for green organic transformations.This review contributes to the field by providing a comprehensive analysis of the mechanisms,materials,and reaction pathways involved in CO_(2)^(·-)generation,focusing on the use of visible-lightdriven photocatalytic materials to circumvent the need for high-energy ultraviolet irradiation.Through a systematic examination of CO_(2)^(·-)production,detection methods,and chemical utilization in photocatalytic carboxylation reactions,this review advances understanding of the chemistry of CO_(2)^(·-)and its applications in sustainable chemical synthesis.In addition,it highlights existing key challenges,such as redox potential limitations,and proposes strategies for scaling up photocatalytic systems to enable practical application.By illuminating the pathway to effectively photocatalyze CO_(2)^(·-)generation and its transformative potential in sustainable chemical synthesis,this review equips scientists with critical insights and strategic approaches for overcoming current limitations,driving innovation in photocatalytic materials for solar-to-chemical energy conversion.
