A controversial taxon,Hipparion plocodus,is reviewed in the present study.Hi.plocodus has been confirmed to be a valid species with definite diagnostic characteristics, represented by cranial specimens from Baode,Shanxi Province. The phylogenetic analysis performed in the present study, with a new matrix, shows that Hi.plocodus forms a monophyletic group with a European species,Hippotherium malpassii.Actually,no close relationship between so-called Hm.malpassii and the genus Hippotherium has been identified, and the record of stratigraphic range of this genus in late stage of Late Miocene is currently absent.Herein previously Hi.plocodus and Hm.malpassii have both attributed into“Hipparion”before the discovery of better material.Evolutionary stages and correlative absolute age showed that these two species should derive independently from some primitive clade.During the late stage of the Late Miocene, the development of the Asian summer monsoon enhanced the humidity of China, with forest and wood habitats expanding considerably under this setting. As the result, one Eurasian closed-habitat lineage thus extended its range into China, which had become very suited for it,give rise to“Hi.”plocodus. 展开更多

BACKGROUND Two-stage revision is the most common treatment for chronic periprosthetic joint infection of the hip, involving a resection arthroplasty with or without placement of an antibiotic-loaded spacer, followed by antibiotic therapy before reimplantation.AIM To compare the outcomes and complications of two consecutive treatment protocols for two-stage revision arthroplasty of the infected hip:One using Girdlestone with an antibiotic holiday, the other using custom-made articulating spacers(CUMARS)without an antibiotic holiday.METHODS In this retrospective study, two consecutive cohorts were compared.Group A(2017-2020)underwent two-stage revision with a Girdlestone and an antibiotic holiday before reimplantation, while Group B(2020-2023)received CUMARS whenever possible, and no antibiotic holiday,or a Girdlestone if indicated. The primary outcome was successful infection eradication after one year.Secondary outcomes included surgical duration, length of hospital stay,weight-bearing allowance, discharge destination, and complications.RESULTS A total of 98 patients were included:39 patients in Group A and 59 patients in Group B.Successful infection eradication after one year was achieved in 69%of Group A and 83%of Group B(P=0.164).Patients in Group B were more frequently allowed to bear weight(64%vs 18%,P<0.001),had a shorter in-hospital stay(9 vs 16 days,P<0.001),and were more often discharged home after the first surgery(48%vs 24%,P=0.048).No significant differences were found in(mechanical)complications.CONCLUSION A protocol including CUMARS is a safe and effective treatment, offering faster recovery, shorter length of hospital stay, and enabling more patients to return home during the interval. This reduces strain on patients and the healthcare system, potentially saving costs, without compromising infection control or increasing(mechanical)complications. 展开更多

BACKGROUND Total knee arthroplasty(TKA)is a widely used treatment for advanced knee osteoarthritis;however, the incidence of failures requiring revision surgery is increasing.Identifying the causes of TKA failure and assessing the outcomes of revision procedures are essential for improving patient care.We hypothesized that infection and aseptic loosening are the primary causes of TKA failure and that revision TKA(rTKA)significantly enhances functional outcomes.AIM To examine the primary causes of TKA failure and evaluate the functional outcomes following rTKA.METHODS This descriptive study was conducted at the Department of Orthopedic Surgery,Civil Hospital,Bahawalpur, from April to September 2024.A total of 118 patients undergoing rTKA for failed primary TKA were included.Data on demographics, causes of failure, and surgical details were collected.Functional outcomes were evaluated using the Knee Society Score and Visual Analog Scale before and six months after surgery.Statistical analysis was performed using SPSS version 25.0,with statistical significance set at P<0.05.RESULTS The leading causes of TKA failure were infection(45.8%),aseptic loosening(44.1%),and periprosthetic fractures(10.2%).The rTKA significantly improved knee function, with the mean Knee Society Score increasing from 39.43±6.18 to 78.91±6.17(P<0.001).Pain levels decreased substantially, with the mean Visual Analog Scale scores reducing from 7.99±1.37 to 1.42±1.17(P<0.001).No significant differences in outcomes were observed between single-stage and two-stage revision procedures.CONCLUSION Infection and aseptic loosening are the predominant causes of TKA failure. The rTKA effectively enhances knee function and alleviates pain, offering significant benefits to patients. 展开更多

BACKGROUND Leg length discrepancy(LLD)following total hip arthroplasty(THA)is a common complaint, leading to decreased patient satisfaction. However, the effect of LLD before THA on outcomes and complications is not well defined.AIM To assess the effect of prior LLD on rates of falls,implant-related complications, stay length, readmissions, and implant survival following THA.METHODS A retrospective review of a nationwide insurance database was conducted from 2010 to 2021.All cases of THA and those with a prior diagnosis of LLD were identified.THA patients with LLD were matched to control patients 1:5 based on demographic and comorbidity profiles.Two-year fall rates and implant complications, lengths of stay,90-day readmissions, and time to revision were compared between cohorts.RESULTS A total of 2038 patients with LLD were matched to 10165 control patients. The LLD group showed significantly greater rates of falls[odds ratio(OR)=1.58;95%confidence interval(95%CI):1.24-2.01],dislocation(OR=2.61;95%CI:2.10-3.24),mechanical loosening(OR=4.58;95%CI:3.28-6.29),and periprosthetic fracture(OR=2.70;95%CI:1.96-3.72)compared to the control group(all P<0.001).Mean length of stay(LOS)was also significantly higher in the LLD group(3.1 days vs 2.8 days,P=0.034).No significant difference in 90-day readmission rates(7.75%vs 7.02%,P=0.244)was observed between the groups(P=0.244).Time to revision was significantly less in the LLD group(225 days vs 544 days,P<0.001).CONCLUSION LLD in patients having THA is related with significantly higher fall risk, rates of implant-related complications,LOS,and quicker time to revision.Identifying patients with LLD before their THA may help in identifying risks, better patient counselling, and more effective preoperative planning. However, the study have important limitations:Its design lacks information on the degree and cause of LLD,the time between diagnosis and surgery, and which leg with the discrepancy underwent the operation.Future well-designed studies should confirm the findings of this study. 展开更多

Two racemic pairs of new stilbenoid dimers,(±)-heterosmilaxones A(1)and B(2),with unique 6/6/6and 6/5/7 tricyclic core systems, respectively, were isolated from the rhizomes of Heterosmilax yunnanensis.Their structures were elucidated through comprehensive spectroscopic analyses, quantum chemical calculations and X-ray diffraction crystallography.Compound(+)-1,initially reported as syagrusin A with a 1,4,4a,9a-tetrahydrofluoren-9-one skeleton,is now revised to a new structure characteristic with a benzo bicyclo[3.3.1]nonene scaffold. And compound 2 bears an unprecedented carbon skeleton with four continuous chiral centers in the central benzo bicyclo[4.2.1]nonene motif.Biogenetically, both 1 and2 were proposed to derive from 3,3',4,5,5'-pentahydroxy stilbene and could be generated through key inverse-electron-demand[4+2]and[5+2]cycloadditions,respectively.Interestingly,both(±)-1 and(±)-2 showed significant inhibition againstα-glucosidase.(±)-1 and its pure enantiomers could modulate protein tyrosine phosphatase-1B(PTP1B)enzyme activities and increased glucose consumption in HepG2 cells in a dose-dependent manner. 展开更多

The AGM postulates ([1]) are for the belief revision (revision by a single belief), and the DP postulates ([2]) are for the iterated revision (revision by a finite sequence of beliefs). Li [3] gave an R-calculus for R-configurations △|Γ, where Δ is a set of literals, and Γ is a finite set of formulas. We shall give two R-calculi such that for any consistent set Γ and finite consistent set △ of formulas in the propositional logic, in one calculus, there is a pseudo-revision Θ of Γ by Δ such that is provable and and in another calculus, there is a pre-revision Ξ of Γ by Δ such that is provable, and for some pseudo-revision Θ;and prove that the deduction systems for both the R-calculi are sound and complete with the pseudo-revision and the pre-revision, respectively. 展开更多

Urban planning construction land standard is the technical specification for scientifically allocating various types of urban construction land, and it is the basis for drawing up and revising the overall urban planning scheme.Considering China’s current urban planning construction land standard, many problems exist, such as the gap in the land use control threshold, the lack of regional differences in the climate revision, and failing to consider the topographic factors. To resolve these problems, this study proposed a step-by-step process framework and quantitative calculation method for the establishment and revision of standards in accordance with the principle of Total-Structure control.By setting the conditions,a universal basic standard for construction land was established.Quantitative analysis was then conducted on the relationship between the basic standard and the selected key indicators, such as urban population size, sunshine spacing coefficient, the width of river valleys or inter-montane basins, and terrain slope, among others. Finally, revised standards were formed for climate conditions, topography, and geomorphologic conditions, which were matched with the basic standards. The key results are three-fold:(1)The per capita construction land standard of 95 m~2/person can be used as the total indicator of China’s urban planning basic standard, and the corresponding per capita single construction land comprises 32.50%of residential land,7.42%of public management and public service land,22.50%of industrial land,17.50%of transportation facilities,12.50%of green space, and 7.58%of other land-use types. The results of the revision of the urban population size indicate that the difference in population size has little effect on the total amount of per capita construction land.(2)The climate revision results of per capita residential land and per capita construction land in major cities reveal that the revised climate value varies greatly between north and south China. The revised climate values of the per capita area of construction land vary by latitude as follows:the value at 20°N is 93 m^(2)/person,the value at 30°N is 97 m^(2)/person,the value at 40°N is 103 m^(2)/person,and the value at 50°N is 115 m^(2)/person.The basic standard land value of 95 m^(2)/person is generally distributed across the Xiamen-Guilin-Kunming line.(3)The cities located in mountainous areas, hilly valleys,or inter-montane basins can reduce the allocation of community parks and comprehensive parks when the average width of an existing river valley or inter-montane basin is less than 2 km.When the average width of the valley or inter-montane basin is between 2 km to 4 km,the allocation of the comprehensive parks can be reduced. The revised results of per capita sloping construction land reveal that the terrain slope greatly affects the revised value of per capita construction land.Specifically, the revised value at 3°is 3.68%higher than the basic standard value, and the increase rates at 8°,15°,and 25°are 11.25%,26.49%,and 68.47%,respectively. 展开更多

Multifactorial aetiology contributes to its refractoriness. Functional endoscopic sinus surgery (FESS) is the most common surgical technique preferred in CRS not responding to medical management, showing high success rates. However, some failure has been noted, requiring revision FESS. CT (Computed Tomography) is a diagnostic tool for CRS, and aids in improving surgical outcomes. Combined with the Lund-Mackay scoring system, CT can help stage the CRS, predicting the need for revision surgery. <strong>Methods:</strong> In our paper, we aimed to study the relationship between severity of CRS using radiological staging and revision of functional endoscopic sinus surgery, assessing outcome after at least 1 year of follow-up. There was a single centre retrospective study involving 150 patients selected by random sampling, to study the correlation between the severity of chronic rhinosinusitis and revision of functional endoscopic sinus surgery (FESS). The study was conducted in the ENT department of King Hamad University Hospital, Bahrain for a time period of 6 months. Records were scanned and scored using Lund-Mackay system. Lund Mackay score of 10 and above was considered severe. The relation between increased severity with Lund Mackay score on the CT scan and the need for revision sinus surgeries was then determined. <strong>Results and Conclusion:</strong> This study concludes that LMS score in CT scans can be used to predict the outcomes of primary FESS procedure and the need for revision surgery in the management of CRS. 展开更多

BACKGROUND Metallosis is the result of metallic wear debris in the soft tissues and is associated to both local and systemic inflammatory response.Metallosis has been reported after total hip and total knee arthroplasty(TKA),but rarely after a unicompartimental knee arthroplasty(UKA).In the context of UKA metallosis, surgeons often opt for revision using a TKA.However,in this paper, the authors successfully treated UKA revising the metal back only.CASE SUMMARY Prior to treat our patient we conducted a literature research through which we identified eleven cases of metallosis after UKA,ten(90.9%)were treated revising using though a TKA.Only one case was managed through a uni-on-uni revision, reporting high knee function. Our patient complained worsening pain and function after a snap occurred at 16 mo after UKA implantation. At 18 mo following surgical debridment and uni-on-uni revision surgery, our patient exhibited a relevant improvement in Oxford Knee Score and a reduction of metal ion levels in the blood.CONCLUSION Our study highlights that in case of metallosis after UKA,the treatment may be based on surgical debridement and just revising the mobilized components. 展开更多

AIM To compare infection rates in primary and revision total knee arthroplasty(TKA) procedures using antibiotic impregnated bone cement(AIBC) to those rates in procedures not using AIBC.METHODS A systematic review and meta-analysis was conducted in search for randomized controlled trials/studies(RCTs) pertaining to the field of antibiotic AIBC vs non-AIBC groups in both primary and revision TKA procedures. The primary literature search performed was to identify all RCTs that assessed AIBC in primary and revision TKA procedures. This search was done strictly through the Pub Med database using the article "filters" setting that identified and separated all RCTs from the overall search. The original search was "Primary/revision total knee arthroplasty using AIBC". Other key terms and phrases were included in the search as well. Eligible articles that were used in the "results" of this review met the following criteria:(1) Involved primary or revision TKA procedures(for any reason);(2) included TKA outcome infection rate information;(3) analyzed an AIBC group vs a non-AIBC control group;(4) were found through the RCT filter or hand search in Pub Med; and(5) published 1985-2017. Exclusion criteria was as follows:(1) Patients that were not undergoing primary or revision TKA procedures;(2) articles that did not separate total hip arthroplasity(THA) vs TKA results if both hip and knee revisions were evaluated;(3) papers that did not follow up on clinical outcomes of the procedure;(4) extrapolation of data was not possible given published results;(5) knee revisions not done on human patients;(6) studies that were strictly done on THAs;(7) articles that were not found through the RCT filter or through hand search in Pub Med;(8) articles that did not evaluate AIBC used in a prosthesis or a spacer during revision;(9) articles that did not compare an AIBC group vs a non-AIBC control group; and(10) articles that were published before 1985.RESULTS In total, 11 articles were deemed eligible for this analysis. Nine of the 11 studies dealt with primary TKA procedures comparing AIBC to non-AIBC treatment. The other two studies dealt with revision TKA procedures that compared such groups. From these papers, 4092 TKA procedures were found. 3903 of these were primary TKAs, while 189 were revision TKAs. Of the 3903 primary TKAs, 1979 of these used some form of AIBC while 1924 were part of a non-AIBC control group. Of the 189 revision TKAs, 96 of these used some form of AIBC while 93 were part of a non-AIBC control group. Average followup times of 47.2 mo and 62.5 mo were found in primary and revision groups respectively. A two-tailed Fisher's exact test was done to check if infection rates differed significantly between the groups. In the primary TKA group, a statistically significant difference between AIBC and non-AIBC groups was not found(AIBC infection rate = 23/1979, non-AIBC infection rate = 35/1924, P = 0.1132). In the revision TKA group, a statistically significant difference between the groups was found(AIBC infection rate = 0/96, non-AIBC infection rate = 7/93, P = 0.0062). No statistically significant differences existed in Knee Society Scores, Hospital for Special Surgery Scores, or Loosening Rates.CONCLUSION AIBC did not have a significant effect on primary TKA infection rates. AIBC did have a significant effect on revision TKA infection rates. 展开更多

Peer revision is a collaborative activity during which student writers can exchange drafts and make suggestions for improvement. It has been widely used at present in process-oriented writing instruction. In this paper, the researcher carried out an empirical study about the effects and efficacy of peer revision on a group of Chinese Juniors majoring in English. It was found that student writers focused specially on grammar and vocabulary in PE session and organization was the least attended to. PE was quite effective in improving the overall writing quality, as the participants did incorporate most of the revisions suggested by their peers. However, the influences of students'writing proficiencies and of the rhetorical modes of the writing tasks on the effects of PE were proved to be not very significant. 展开更多

Healthcare security and privacy breaches are occurring in the United States (US), and increased substantially during the pandemic. This paper reviews the National Institute of Standards and Technology (NIST) publication base as an effective solution. The NIST Special Publication 800-66 Revision 1 was an essential standard in US healthcare, which was withdrawn in February 2024 and superseded by SP 800-66 Revision 2. This review investigates the academic papers concerning the application of the NIST SP 800-66 Revision 1 standard in the US healthcare literature. A systematic review method was used in this study to determine current knowledge gaps of the SP 800-66 Revision 1. Some limitations were employed in the search to enforce validity. A total of eleven articles were found eligible for the study. Consequently, this study suggests the necessity for additional academic papers pertaining to SP 800-66 Revision 2 in the US healthcare literature. In turn, it will enhance awareness of safeguarding electronic protected health information (ePHI), help to mitigate potential future risks, and eventually reduce breaches. 展开更多

The single prefrontal configuration has historically been used as an important diagnostic character for many natricine taxa. For example, the genus Trimerodytes Cope, 1895 was long been regarded as a junior synonym of Opisthotropis Günther, 1872 for their similar prefrontal configurations and the type species, T. balteatus Cope, 1895, has been assigned to the genus Opisthotropis. However, as the number and arrangement of prefrontal vary frequently both at species and generic level, it is questionable whether the synonymization of Trimerodytes reflects their evolutionary relationships. On the basis of recently collected specimens of O. balteata, the generic status of the species was assessed using both molecular and morphological data. Opisthotropis was recovered as polyphyletic with reference to O. balteata, because O. balteata is nested within the genus Sinonatrix Rossman and Eberle, 1977 and is the sister species of the type species of Sinonatrix. Consequently, we herein resurrect the long-overlooked synonym Trimerodytes from Opisthotropis and synonymize the junior generic nomen Sinonatrix with Trimerodytes. In addition, based on morphological similarities between the monotypic genus Paratapinophis Angel, 1929 and Trimerodytes, we doubt about the validity of Paratapinophis. Following taxonomic changes in this work, the taxonomic account of the genus Trimerodytes, updated descriptions of its type species, and diagnostic key to Trimerodytes species are provided. 展开更多

The formation process of the Dianqiangui basin, a special basin, occurred after the Caledonian orogeny, in the south of Guizhou, the west of Guangxi and the southeast of Yunnan, experienced three periods: it began in the Devonian, persisted in the Carboniferous, and became fiercer in the Permian. Controlled by syndepositional fault-zones, varieties of isolated carbonate platforms, large and small, were developed in the background of a deep-water basin, namely, an inter-platform ditch. And a special paleogeographical Late Paleozoic pattern marked by “platform-basin-hill-trough” was produced in both the Dianqiangui basin and its adjacent areas. Affected by regional tectonic activities and the global changes in the sea level, the platform carbonates and coal measures superimposed each other cyclically on the attached platform. The reef-building on the isolated platform and the margin of the attached platform corresponds to the development of the shale succession in the deep-water basin. All of these elementary characteristics reflect a regular and sophisticated filling succession of the Dianqiangui basin, a result of the dual controls of the regionally tectonic activities and the eustacy. Based on the two elementary features of the third-order sequences, i.e. the regularity of sedimentary-facies succession in space and the simultaneity of environmental changes in time, 25 third-order sequences could be discerned in the Upper Paleozoic strata in the Dianqiangui basin and its adjacent areas. On the basis of the two kinds of facies-changing surfaces and the two kinds of diachronisms in stratigraphic records, the regional Late Paleozoic sequence-stratigraphic framework in the Dianqiangui basin and its adjacent areas can be established. There are two types of facies-changing surfaces and two types of diachronisms in stratigraphic records: the static type, a result of the change in sedimentary facies in space, and the dynamic type, a result of the change in time. These two types of facies-changing surfaces led to the generation of the two types of diachronisms: the diachronism of facies-changing surfaces that was formed by the static facies-changing surfaces, and the diachronism of punctuated surfaces that was formed by the dynamic facies-changing surfaces. The two types of facies-changing surfaces and the two types of diachronisms in stratigraphic records are the key to the establishment of the sequence-stratigraphic framework. The sequence boundaries could be divided geologically into four types: tectonic unconformity, sedimentary unconformity, drowned unconformity and their correlative surfaces. All of these four types can be further grouped into exposed punctuated surfaces and deepened punctuated surfaces. The tectonic unconformity is similar to Type Ⅰ sequence boundary, and the sedimentary unconformity is similar to Type Ⅱ sequence boundary defined by Vail et al.. In terms of sequence stratigraphy, the tectonic unconformities of the Ziyun movement, the Qiangui epeirogeny and the Dongwu revolution as well as the drowned unconformity in the transitional period from the Permian to the Triassic can be systematically defined and their geological characteristics are briefly presented. 展开更多
