Background:“Qi deficiency”(a pathological state where the body’s vital energy(Qi)is insufficient or weakened,impairing physiological functions and diminishing the body’s ability to perform daily activities,defend ...Background:“Qi deficiency”(a pathological state where the body’s vital energy(Qi)is insufficient or weakened,impairing physiological functions and diminishing the body’s ability to perform daily activities,defend against illness,and maintain homeostasis)syndrome is considered a critical syndrome in traditional Chinese medicine(TCM)and is associated with poor prognosis in heart failure(HF).This study investigates the clinical,metabolic,and transcriptomic differences between heart failure patients with and without Qi deficiency syndrome.Methods:56 heart failure patients were evaluated using a Qi deficiency syndrome scale and divided into Qi deficiency syndrome(QD)and non-Qi deficiency(non-QD)groups based on the median score.Clinical characteristics,including baseline N-terminal pro-B-type natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),total diuretic use during hospitalization,and 90-day rehospitalization rates,were compared between the groups.Differentially expressed genes(DEGs)and differential metabolites were identified,followed by enrichment analyses and validation using qPCR and Western blot in AC16 cardiomyocytes.Results:QD patients exhibited significantly higher NT-proBNP levels,lower LVEF,and increased 90-day rehospitalization rates.Metabolomic profiling revealed lipid metabolism disruptions,notably in linoleic acid and phospholipid pathways.Transcriptomic analysis highlighted 17 DEGs,including CISD2,a critical mitochondrial regulator,which was downregulated in QD patients.Correlation analysis identified significant associations between DEGs(e.g.,CISD2,BPGM)and lipid metabolites such as PC(16:0/P-16:0).Functional knockdown of CISD2 in AC16 cells led to upregulation of lipid oxidation enzymes ALOX15 and CYP1A2,linking CISD2 dysfunction to lipid metabolic dysregulation.Conclusion:Qi deficiency is associated with more severe heart failure symptoms,worse prognosis,and distinct metabolic and transcriptomic profiles,particularly in lipid metabolism.CISD2 emerges as a potential therapeutic target,offering new avenues for integrating molecular insights with TCM approaches to optimize HF management.展开更多

Background:Panacis Quinquefolii Radix(PQR)is known for its ability to nourish“Qi”(it serves as the driving force for the functional activities of the body’s organs and meridians,promoting and regulating various phy...Background:Panacis Quinquefolii Radix(PQR)is known for its ability to nourish“Qi”(it serves as the driving force for the functional activities of the body’s organs and meridians,promoting and regulating various physiological functions)and“Yin”(it represents the material foundation of the human body.It plays a role in nourishing,moistening,and cooling the body).Notoginseng Radix et Rhizoma(NRR)is recognized for its properties of resolving blood stasis(it refers to a pathological condition characterized by impaired or stagnant blood circulation within the body).Changes in the compatibility ratio of these herbs often lead to variations in their chemical composition and efficacy.However,the specific alterations in chemical composition and efficacy resulting from compatibility adjustments remain unclear.We aimed to compare the material basis and their effects of different compatibility ratios of PQR and NRR on“Qi”deficiency and blood stasis syndrome(QBS).Methods:This study employed UPLC-Q/TOF-MS to identify effective compounds in the compatibility of PQR and NRR and utilized UPLC-TQ-MS/MS to analyze the dissolution of 16 saponins in PQR and NRR at 9 different ratios.A rat model of QBS was established,and the efficacy of PQR and NRR in treating this syndrome was assessed using hemorheology and coagulation analyses.Results:The study results show that PQR and NRR exhibit significant efficacy,effectively reducing blood viscosity induced by platelet aggregation and lowering inflammatory markers such as IL-6,IL-10,TXB2 and ET associated with vascular injury.Moreover,this combination regulates ATP and ADP levels,enhances energy metabolism,and promotes overall health.A total of 104 compounds in the compatibility of PQR and NRR were identified.The ratios of 1:2 and 1:3 showed the highest total saponin content,but the ratio of 1:1 demonstrated a superior pharmacological effect for the treatment of QBS.Conclusion:In summary,the compatibility of PQR and NRR not only shows the complex interactions between traditional Chinese medicinal materials,but also provides a new idea and method for the treatment of QBS.展开更多
OBJECTIVE:To propose an automatic acupuncture robot system for performing acupuncture operations.METHODS:The acupuncture robot system consists of three components:automatic acupoint localization,acupuncture manipulati...OBJECTIVE:To propose an automatic acupuncture robot system for performing acupuncture operations.METHODS:The acupuncture robot system consists of three components:automatic acupoint localization,acupuncture manipulations,and De Qi sensation detection.The OptiTrack motion capture system is used to locate acupoints,which are then translated into coordinates in the robot control system.A flexible collaborative robot with an intelligent gripper is then used to perform acupuncture manipulations with high precision.In addition,a De Qi sensation detection system is proposed to evaluate the effect of acupuncture.To verify the stability of the designed acupuncture robot,acupoints'coordinates localized by the acupuncture robot are compared with the Gold Standard labeled by a professional acupuncturist using significant level tests.RESULTS:Through repeated experiments for eight acupoints,the acupuncture robot achieved a positioning error within 3.3 mm,which is within the allowable range of needle extraction and acupoint insertion.During needle insertion,the robot arm followed the prescribed trajectory with a mean deviation distance of 0.02 mm and a deviation angle of less than 0.15°.The results of the lifting thrusting operation in the Xingzhen process show that the mean acupuncture depth error of the designed acupuncture robot is approximately 2 mm,which is within the recommended depth range for the Xingzhen operation.In addition,the average detection accuracy of the De Qi keywords is 94.52%,which meets the requirements of acupuncture effect testing for different dialects.CONCLUSION:The proposed acupuncture robot system streamlines the acupuncture process,increases efficiency,and reduces practitioner fatigue,while also allowing for the quantification of acupuncture manipulations and evaluation of therapeutic effects.The development of an acupuncture robot system has the potential to revolutionize low back pain treatment and improve patient outcomes.展开更多
OBJECTIVE:To uncover the biological foundation of the prevailing TCM syndrome in individuals with Esophageal squamous cell carcinoma(ESCC),Zhengxu Xieshi(ZXXS),which is characterized by a deficiency in vital Qi and an...OBJECTIVE:To uncover the biological foundation of the prevailing TCM syndrome in individuals with Esophageal squamous cell carcinoma(ESCC),Zhengxu Xieshi(ZXXS),which is characterized by a deficiency in vital Qi and an excess in evil Qi.METHODS:We investigated shifts in vital Qi by quantifying systemic metabolic changes in the peripheral blood.Serum metabolomic profiling was conducted on the ESCC cohort 1 along with a matched healthy control cohort.Additionally,we assessed changes in evil Qi by examining metabolic perturbations in ESCC tissues.This analysis involved metabolomic and proteomic surveys of ESCC tissues and paired normal adjacent tissues as controls in an independent ESCC cohort 2.RESULTS:Serum metabolomic profiling highlighted the prevalent downregulation of differentially expressed metabolites in patient sera,in contrast to the upregulation observed in ESCC tissues,compared to their respective controls.Remarkably,the group of differential metabolites in the ESCC tissues was predominantly composed of amino acids.Thus,we focused on amino acid metabolism.Our integrative analysis showed the downregulation of a significant majority of disturbed amino acids in patient sera relative to the upregulation of an overwhelming proportion of perturbed amino acids within ESCC tissues.Enrichment analysis of these amino acids revealed seven metabolic pathways that contribute to the metabolism of antioxidants,energy intermediates,and biosynthetic precursors.Interestingly,these pathways displayed attenuation in patient sera but augmentation in ESCC tissues.Similarly,the proteomic data confirmed the activation of these pathways in ESCC tissues.CONCLUSION:This study presents a new perspective on the prevalence of ZXXS syndrome in patients with ESCC,contextualized within the realm of metabolic reprogramming.Specifically,diminished amino acid metabolism in the circulating blood corresponds to a deficiency in vital Qi.Conversely,hyperactive amino acid metabolism in ESCC tissues signifies an augmentation of local evil Qi.These findings hold potential to enrich the current medical framework and offer a deeper understanding of ESCC management by integrating the principles of ZXXS syndrome.展开更多
Objective To explore the clinical characteristics and methods for syndrome differentiation prediction,as well as to construct a predictive model for Qi deficiency and blood stasis syndrome in patients with acute ische...Objective To explore the clinical characteristics and methods for syndrome differentiation prediction,as well as to construct a predictive model for Qi deficiency and blood stasis syndrome in patients with acute ischemic stroke(AIS).Methods This study employed a retrospective case-control design to analyze patients with AIS who received inpatient treatment at the Neurology Department of The First Hospital of Hunan University of Chinese Medicine from January 1,2013 to December 31,2022.AIS patients meeting the diagnostic criteria for Qi deficiency and blood stasis syndrome were stratified into case group,while those without Qi deficiency and blood stasis syndrome were stratified into control group.The demographic characteristics(age and gender),clinical parameters[time from onset to admission,National Institutes of Health Stroke Scale(NIHSS)score,and blood pressure],past medical history,traditional Chinese medicine(TCM)diagnostic characteristics(tongue and pulse),neurological symptoms and signs,imaging findings[magnetic resonance imaging-diffusion weighted imaging(MRI-DWI)],and biochemical indicators of the two groups were collected and compared.The indicators with statistical difference(P<0.05)in univariate analysis were included in multivariate logistic regression analysis to evaluate their predictive value for the diagnosis of Qi deficiency and blood stasis syndrome,and the predictive model was constructed by receiver operating characteristic(ROC)curve analysis.Results The study included 1035 AIS patients,with 404 cases in case group and 631 cases in control group.Compared with control group,patients in case group were significantly older,had extended onset-to-admission time,lower diastolic blood pressure,and lower NIHSS scores(P<0.05).Case group showed lower incidence of hypertension history(P<0.05).Regarding tongue and pulse characteristics,pale and dark tongue colors,white tongue coating,fine pulse,astringent pulse,and sinking pulse were more common in case group.Imaging examinations demonstrated higher proportions of centrum semiovale infarction,cerebral atrophy,and vertebral artery stenosis in case group(P<0.05).Among biochemical indicators,case group showed higher proportions of elevated fasting blood glucose and glycated hemoglobin(HbA1c),while lower proportions of elevated white blood cell count,reduced hemoglobin,and reduced high-density lipoprotein cholesterol(HDL-C)(P<0.05).Multivariate logistic regression analysis identified significant predictors for Qi deficiency and blood stasis syndrome including:fine pulse[odds ratio(OR)=4.38],astringent pulse(OR=3.67),superficial sensory abnormalities(OR=1.86),centrum semiovale infarction(OR=1.57),cerebral atrophy(OR=1.55),vertebral artery stenosis(OR=1.62),and elevated HbA1c(OR=3.52).The ROC curve analysis of the comprehensive prediction model yielded an area under the curve(AUC)of 0.878[95%confidence interval(CI)=0.855-0.900].Conclusion This study finds out that Qi deficiency and blood stasis syndrome represents one of the primary types of AIS.Fine pulse,astringent pulse,superficial sensory abnormalities,centrum semiovale infarction,cerebral atrophy,vertebral artery stenosis,elevated blood glucose,elevated HbA1c,pale and dark tongue colors,and white tongue coating are key objective diagnostic indicators for the syndrome differentiation of AIS with Qi deficiency and blood stasis syndrome.Based on these indicators,a syndrome differentiation prediction model has been developed,offering a more objective basis for clinical diagnosis,and help to rapidly identify this syndrome in clinical practice and reduce misdiagnosis and missed diagnosis.展开更多
This paper first analyzes the complexity of pathological processes involved in acute hemorrhagic or ischemic stroke,including primary and secondary brain injury manifestations and mechanisms,potential transformations ...This paper first analyzes the complexity of pathological processes involved in acute hemorrhagic or ischemic stroke,including primary and secondary brain injury manifestations and mechanisms,potential transformations between hemorrhage and infarction,and the impact of postbrain injury inflammation on disease progression.Based on decades of extensive clinical and pharmacological research on the usage of Chinese herbal medicine(CHM)monomers or formulas that promote blood circulation and remove blood stasis(such as Angong Niuhuang Wan,Buyang Huanwu Tang,Dahuoluo Wan,and Dushen Tang)for stroke treatment,it proposes that tonifying qi,promoting blood circulation,removing blood stasis,and detoxifying are rational treatment principles of CHM.These principles correspond to the following Western medical implications:tonifying qi corresponds to neuroprotection effects,promoting blood circulation corresponds to anticoagulation and thrombolysis,removing blood stasis addresses hematoma absorption and cerebral edema reduction,and detoxifying corresponds to anti-inflammatory actions.This paper further proposes from a systems medicine perspective that acute stroke is a complex disease requiring individualized CHM treatment with timely modifications rather than a one-sizefits-all approach.CHM monomers or formulas for promoting blood circulation and removing blood stasis,which have various effects such as lowering blood pressure,hemostasis,anticoagulation,antiplatelet,anti-inflammatory,promoting fibrinolysis,and edema reduction,must align with disease progression and be applied within appropriate therapeutic time windows to ensure efficacy and safety.Finally,this paper suggests that a combined use of acupuncture and CHM can potentially synergistically leverage their respective therapeutic strengths.Additionally,acupuncture shows clear benefits in the acute phase of intracerebral hemorrhage(ICH),such as stimulating the vagus nerve to enhance cerebral blood flow,reducing inflammation,as well as triggering hemostatic effects.By applying these rational treatment principles in an integrated approach,better CHM treatment outcomes and higher efficacy of stroke management may be attained.展开更多
BACKGROUND Owing to the absence of specific symptoms in early-stage gastric cancer,most patients are diagnosed at intermediate or advanced stages.As a result,treatment often shifts from surgery to other therapies,with...BACKGROUND Owing to the absence of specific symptoms in early-stage gastric cancer,most patients are diagnosed at intermediate or advanced stages.As a result,treatment often shifts from surgery to other therapies,with chemotherapy and targeted therapies being the primary options for advanced gastric cancer treatment.A total of 116 patients with advanced gastric cancer,admitted from January 2021 to December 2023,were selected and divided into two groups of 58 each using the random number table method.The control group received FOLFOX4 chemothe-rapy(oxaliplatin+calcium+folinate+5-fluorouracil)combined with intravenous sindilizumab.The observation group received the same treatment as the control group,supplemented by oral administration of Senqi Shiyiwei granules.Both groups underwent treatment cycles of 3 weeks,with a minimum of two cycles.The therapeutic efficacy,immune mechanisms,and treatment-related toxicity and side effects were compared between the groups.The objective remission rate in the observation group(55.17%)was higher than that of the control group(36.21%)(P<0.05).After two treatment cycle,CD3+,CD4+,and CD4+/CD8+levels were higher in the observation group compared to the control group,while CD8+,regulatory T cells,and natural killer cells were lower(P<0.05).Additionally,the incidence of leukopenia,nausea,and vomiting was lower in observed group(P<0.05).No significant differences were observed in the incidence of other adverse reactions(P>0.05).CONCLUSION Adjuvant therapy with Shenqixian granules may enhance the efficacy of simudizumab combined with FOLFOX4 chemotherapy in advanced gastric cancer and the immune function by increasing immune cell counts,making it a valuable option in clinical treatment.展开更多

文摘Background:“Qi deficiency”(a pathological state where the body’s vital energy(Qi)is insufficient or weakened,impairing physiological functions and diminishing the body’s ability to perform daily activities,defend against illness,and maintain homeostasis)syndrome is considered a critical syndrome in traditional Chinese medicine(TCM)and is associated with poor prognosis in heart failure(HF).This study investigates the clinical,metabolic,and transcriptomic differences between heart failure patients with and without Qi deficiency syndrome.Methods:56 heart failure patients were evaluated using a Qi deficiency syndrome scale and divided into Qi deficiency syndrome(QD)and non-Qi deficiency(non-QD)groups based on the median score.Clinical characteristics,including baseline N-terminal pro-B-type natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),total diuretic use during hospitalization,and 90-day rehospitalization rates,were compared between the groups.Differentially expressed genes(DEGs)and differential metabolites were identified,followed by enrichment analyses and validation using qPCR and Western blot in AC16 cardiomyocytes.Results:QD patients exhibited significantly higher NT-proBNP levels,lower LVEF,and increased 90-day rehospitalization rates.Metabolomic profiling revealed lipid metabolism disruptions,notably in linoleic acid and phospholipid pathways.Transcriptomic analysis highlighted 17 DEGs,including CISD2,a critical mitochondrial regulator,which was downregulated in QD patients.Correlation analysis identified significant associations between DEGs(e.g.,CISD2,BPGM)and lipid metabolites such as PC(16:0/P-16:0).Functional knockdown of CISD2 in AC16 cells led to upregulation of lipid oxidation enzymes ALOX15 and CYP1A2,linking CISD2 dysfunction to lipid metabolic dysregulation.Conclusion:Qi deficiency is associated with more severe heart failure symptoms,worse prognosis,and distinct metabolic and transcriptomic profiles,particularly in lipid metabolism.CISD2 emerges as a potential therapeutic target,offering new avenues for integrating molecular insights with TCM approaches to optimize HF management.

文摘Background:Based on a profound awareness of crisis sense,Huai Nan-zi warns people of the importance of being prepared for danger in times of peace by using the principle that Heaven and Earth are ever-changing and never-ending.It also consciously constructs a cosmic life view of the organic,homologous,isomorphic,and harmonious unity of heaven,earth,and man based on human existence and destiny with a rational thinking attitude.The life philosophy of the Huai Nan-zi offers modern people a completely new holistic view of life and medicine.Not only in China,but also in Western countries,studying its medical philosophical ideas helps us better explore the theoretical roots of TCM in the era of globalized medicine.Methods:This paper mainly uses the analysis method of literature review and chinese philosophy intellectual concepts.It employs the I Ching’s image-number logic thinking method to compare images through analogy and the holistic thinking method of the three-talent view of heaven,earth,and human to understand life consciousness.Results:This article mainly interprets medical philosophy in Huai Nan-zi through three aspects:1)The body of Taoism:heaven,earth,and humanity constitute one unity within the body of the universe;2)The spirit of Taoism:keeping the spirit inward,preserving the essence and suppressing the superficial;and 3)The mind of Taoism:the principle of life governed by the dynamics of gain and loss,prosperity and decline.Conclusion:The philosophy of life presented in the Huai Nan-zi ultimately charts a course toward a state of profound theoretical integration.Its“conscious map”does not lead to a fixed destination,but to a continuous and dynamic mode of being–a life of flourishing known in Chinese as yang sheng,the nurturing of life.The destination,therefore,is the journey itself,undertaken with unwavering cosmic awareness and harmony.This ancient text reminds us that a truly healthy life is not a fragmented pursuit of physical fitness,mental peace,or spiritual insight in isolation.Instead,it is the symphony of all three(Taoist body,spirit,and mind),orchestrated by the fundamental principles of the cosmos.By aligning our inner nature with the outer Tao,we transform our existence from a series of reactive struggles into a graceful and spontaneous free flow.In a modern world characterized by fragmentation,overstimulation,and a relentless push against natural rhythms,the Huai Nan-zi’s life consciousness map is more relevant than ever.The philosophy of Huai Nan-zi not only plays a vital role in the construction of the theoretical system of TCM in ancient East life wisdom,but also is worthwhile for Western life sciences to conduct in-depth exploration and discovery in the age of AI.

文摘Traditional Chinese medicine has opened new pathways and provided fresh perspectives for the treatment of uremic pruritus(UP)through syndrome differentiation and treatment.This paper summarizes the clinical experience of Professor Li Qi in treating this condition.Professor Li Qi points out that the fundamental pathogenesis of UP lies in yang deficiency of the spleen and kidney.On this basis,either external cold entering the interior or yang deficiency leading to cold congelation may result in malnutrition of the skin,thereby triggering pruritus.In clinical practice,slight sweating therapy is applied to release the exterior and expel pathogenic factors or to warm and resolve cold fluid retention,achieving satisfactory therapeutic outcomes.

文摘This study makes an in-depth exploration of the core pathogenesis of chronic atrophic gastritis,Qi Deficiency with Stagnation,to systematically interpret it within the theoretical framework of Achieving Central Harmony,and to provide a treatment plan.Professor Li Tingquan believes that the occurrence and development of chronic atrophic gastritis(CAG)is fundamentally a process of harmony imbalance of the spleen and stomach.Its specific manifestation lies in the interaction between qi deficiency and stagnation,namely,spleen deficiency as the root cause,and qi stagnation,phlegm-dampness,and blood stasis obstructing the middle energizer as the secondary manifestations.These factors are mutually causal and interact with one another.Based on this,he proposed a four-step method to treat the disease:eliminating pathogenic factors,harmonizing the spleen and stomach,activating blood circulation to resolve stasis,and tonifying the kidney to generate blood(metaphorically described as"closing the mountains to cultivate forests and thicken the soil,breeding seeds and irrigating for stomach recovery").This approach aims to restore the harmonious state of the middle energizer,providing a complete clinical framework of theory,principle,formula,and herbs for the prevention and treatment of CAG.

文摘Background:Panacis Quinquefolii Radix(PQR)is known for its ability to nourish“Qi”(it serves as the driving force for the functional activities of the body’s organs and meridians,promoting and regulating various physiological functions)and“Yin”(it represents the material foundation of the human body.It plays a role in nourishing,moistening,and cooling the body).Notoginseng Radix et Rhizoma(NRR)is recognized for its properties of resolving blood stasis(it refers to a pathological condition characterized by impaired or stagnant blood circulation within the body).Changes in the compatibility ratio of these herbs often lead to variations in their chemical composition and efficacy.However,the specific alterations in chemical composition and efficacy resulting from compatibility adjustments remain unclear.We aimed to compare the material basis and their effects of different compatibility ratios of PQR and NRR on“Qi”deficiency and blood stasis syndrome(QBS).Methods:This study employed UPLC-Q/TOF-MS to identify effective compounds in the compatibility of PQR and NRR and utilized UPLC-TQ-MS/MS to analyze the dissolution of 16 saponins in PQR and NRR at 9 different ratios.A rat model of QBS was established,and the efficacy of PQR and NRR in treating this syndrome was assessed using hemorheology and coagulation analyses.Results:The study results show that PQR and NRR exhibit significant efficacy,effectively reducing blood viscosity induced by platelet aggregation and lowering inflammatory markers such as IL-6,IL-10,TXB2 and ET associated with vascular injury.Moreover,this combination regulates ATP and ADP levels,enhances energy metabolism,and promotes overall health.A total of 104 compounds in the compatibility of PQR and NRR were identified.The ratios of 1:2 and 1:3 showed the highest total saponin content,but the ratio of 1:1 demonstrated a superior pharmacological effect for the treatment of QBS.Conclusion:In summary,the compatibility of PQR and NRR not only shows the complex interactions between traditional Chinese medicinal materials,but also provides a new idea and method for the treatment of QBS.

文摘OBJECTIVE:To propose an automatic acupuncture robot system for performing acupuncture operations.METHODS:The acupuncture robot system consists of three components:automatic acupoint localization,acupuncture manipulations,and De Qi sensation detection.The OptiTrack motion capture system is used to locate acupoints,which are then translated into coordinates in the robot control system.A flexible collaborative robot with an intelligent gripper is then used to perform acupuncture manipulations with high precision.In addition,a De Qi sensation detection system is proposed to evaluate the effect of acupuncture.To verify the stability of the designed acupuncture robot,acupoints'coordinates localized by the acupuncture robot are compared with the Gold Standard labeled by a professional acupuncturist using significant level tests.RESULTS:Through repeated experiments for eight acupoints,the acupuncture robot achieved a positioning error within 3.3 mm,which is within the allowable range of needle extraction and acupoint insertion.During needle insertion,the robot arm followed the prescribed trajectory with a mean deviation distance of 0.02 mm and a deviation angle of less than 0.15°.The results of the lifting thrusting operation in the Xingzhen process show that the mean acupuncture depth error of the designed acupuncture robot is approximately 2 mm,which is within the recommended depth range for the Xingzhen operation.In addition,the average detection accuracy of the De Qi keywords is 94.52%,which meets the requirements of acupuncture effect testing for different dialects.CONCLUSION:The proposed acupuncture robot system streamlines the acupuncture process,increases efficiency,and reduces practitioner fatigue,while also allowing for the quantification of acupuncture manipulations and evaluation of therapeutic effects.The development of an acupuncture robot system has the potential to revolutionize low back pain treatment and improve patient outcomes.

文摘OBJECTIVE:To uncover the biological foundation of the prevailing TCM syndrome in individuals with Esophageal squamous cell carcinoma(ESCC),Zhengxu Xieshi(ZXXS),which is characterized by a deficiency in vital Qi and an excess in evil Qi.METHODS:We investigated shifts in vital Qi by quantifying systemic metabolic changes in the peripheral blood.Serum metabolomic profiling was conducted on the ESCC cohort 1 along with a matched healthy control cohort.Additionally,we assessed changes in evil Qi by examining metabolic perturbations in ESCC tissues.This analysis involved metabolomic and proteomic surveys of ESCC tissues and paired normal adjacent tissues as controls in an independent ESCC cohort 2.RESULTS:Serum metabolomic profiling highlighted the prevalent downregulation of differentially expressed metabolites in patient sera,in contrast to the upregulation observed in ESCC tissues,compared to their respective controls.Remarkably,the group of differential metabolites in the ESCC tissues was predominantly composed of amino acids.Thus,we focused on amino acid metabolism.Our integrative analysis showed the downregulation of a significant majority of disturbed amino acids in patient sera relative to the upregulation of an overwhelming proportion of perturbed amino acids within ESCC tissues.Enrichment analysis of these amino acids revealed seven metabolic pathways that contribute to the metabolism of antioxidants,energy intermediates,and biosynthetic precursors.Interestingly,these pathways displayed attenuation in patient sera but augmentation in ESCC tissues.Similarly,the proteomic data confirmed the activation of these pathways in ESCC tissues.CONCLUSION:This study presents a new perspective on the prevalence of ZXXS syndrome in patients with ESCC,contextualized within the realm of metabolic reprogramming.Specifically,diminished amino acid metabolism in the circulating blood corresponds to a deficiency in vital Qi.Conversely,hyperactive amino acid metabolism in ESCC tissues signifies an augmentation of local evil Qi.These findings hold potential to enrich the current medical framework and offer a deeper understanding of ESCC management by integrating the principles of ZXXS syndrome.

文摘Objective To explore the clinical characteristics and methods for syndrome differentiation prediction,as well as to construct a predictive model for Qi deficiency and blood stasis syndrome in patients with acute ischemic stroke(AIS).Methods This study employed a retrospective case-control design to analyze patients with AIS who received inpatient treatment at the Neurology Department of The First Hospital of Hunan University of Chinese Medicine from January 1,2013 to December 31,2022.AIS patients meeting the diagnostic criteria for Qi deficiency and blood stasis syndrome were stratified into case group,while those without Qi deficiency and blood stasis syndrome were stratified into control group.The demographic characteristics(age and gender),clinical parameters[time from onset to admission,National Institutes of Health Stroke Scale(NIHSS)score,and blood pressure],past medical history,traditional Chinese medicine(TCM)diagnostic characteristics(tongue and pulse),neurological symptoms and signs,imaging findings[magnetic resonance imaging-diffusion weighted imaging(MRI-DWI)],and biochemical indicators of the two groups were collected and compared.The indicators with statistical difference(P<0.05)in univariate analysis were included in multivariate logistic regression analysis to evaluate their predictive value for the diagnosis of Qi deficiency and blood stasis syndrome,and the predictive model was constructed by receiver operating characteristic(ROC)curve analysis.Results The study included 1035 AIS patients,with 404 cases in case group and 631 cases in control group.Compared with control group,patients in case group were significantly older,had extended onset-to-admission time,lower diastolic blood pressure,and lower NIHSS scores(P<0.05).Case group showed lower incidence of hypertension history(P<0.05).Regarding tongue and pulse characteristics,pale and dark tongue colors,white tongue coating,fine pulse,astringent pulse,and sinking pulse were more common in case group.Imaging examinations demonstrated higher proportions of centrum semiovale infarction,cerebral atrophy,and vertebral artery stenosis in case group(P<0.05).Among biochemical indicators,case group showed higher proportions of elevated fasting blood glucose and glycated hemoglobin(HbA1c),while lower proportions of elevated white blood cell count,reduced hemoglobin,and reduced high-density lipoprotein cholesterol(HDL-C)(P<0.05).Multivariate logistic regression analysis identified significant predictors for Qi deficiency and blood stasis syndrome including:fine pulse[odds ratio(OR)=4.38],astringent pulse(OR=3.67),superficial sensory abnormalities(OR=1.86),centrum semiovale infarction(OR=1.57),cerebral atrophy(OR=1.55),vertebral artery stenosis(OR=1.62),and elevated HbA1c(OR=3.52).The ROC curve analysis of the comprehensive prediction model yielded an area under the curve(AUC)of 0.878[95%confidence interval(CI)=0.855-0.900].Conclusion This study finds out that Qi deficiency and blood stasis syndrome represents one of the primary types of AIS.Fine pulse,astringent pulse,superficial sensory abnormalities,centrum semiovale infarction,cerebral atrophy,vertebral artery stenosis,elevated blood glucose,elevated HbA1c,pale and dark tongue colors,and white tongue coating are key objective diagnostic indicators for the syndrome differentiation of AIS with Qi deficiency and blood stasis syndrome.Based on these indicators,a syndrome differentiation prediction model has been developed,offering a more objective basis for clinical diagnosis,and help to rapidly identify this syndrome in clinical practice and reduce misdiagnosis and missed diagnosis.
文摘This paper first analyzes the complexity of pathological processes involved in acute hemorrhagic or ischemic stroke,including primary and secondary brain injury manifestations and mechanisms,potential transformations between hemorrhage and infarction,and the impact of postbrain injury inflammation on disease progression.Based on decades of extensive clinical and pharmacological research on the usage of Chinese herbal medicine(CHM)monomers or formulas that promote blood circulation and remove blood stasis(such as Angong Niuhuang Wan,Buyang Huanwu Tang,Dahuoluo Wan,and Dushen Tang)for stroke treatment,it proposes that tonifying qi,promoting blood circulation,removing blood stasis,and detoxifying are rational treatment principles of CHM.These principles correspond to the following Western medical implications:tonifying qi corresponds to neuroprotection effects,promoting blood circulation corresponds to anticoagulation and thrombolysis,removing blood stasis addresses hematoma absorption and cerebral edema reduction,and detoxifying corresponds to anti-inflammatory actions.This paper further proposes from a systems medicine perspective that acute stroke is a complex disease requiring individualized CHM treatment with timely modifications rather than a one-sizefits-all approach.CHM monomers or formulas for promoting blood circulation and removing blood stasis,which have various effects such as lowering blood pressure,hemostasis,anticoagulation,antiplatelet,anti-inflammatory,promoting fibrinolysis,and edema reduction,must align with disease progression and be applied within appropriate therapeutic time windows to ensure efficacy and safety.Finally,this paper suggests that a combined use of acupuncture and CHM can potentially synergistically leverage their respective therapeutic strengths.Additionally,acupuncture shows clear benefits in the acute phase of intracerebral hemorrhage(ICH),such as stimulating the vagus nerve to enhance cerebral blood flow,reducing inflammation,as well as triggering hemostatic effects.By applying these rational treatment principles in an integrated approach,better CHM treatment outcomes and higher efficacy of stroke management may be attained.
文摘BACKGROUND Owing to the absence of specific symptoms in early-stage gastric cancer,most patients are diagnosed at intermediate or advanced stages.As a result,treatment often shifts from surgery to other therapies,with chemotherapy and targeted therapies being the primary options for advanced gastric cancer treatment.A total of 116 patients with advanced gastric cancer,admitted from January 2021 to December 2023,were selected and divided into two groups of 58 each using the random number table method.The control group received FOLFOX4 chemothe-rapy(oxaliplatin+calcium+folinate+5-fluorouracil)combined with intravenous sindilizumab.The observation group received the same treatment as the control group,supplemented by oral administration of Senqi Shiyiwei granules.Both groups underwent treatment cycles of 3 weeks,with a minimum of two cycles.The therapeutic efficacy,immune mechanisms,and treatment-related toxicity and side effects were compared between the groups.The objective remission rate in the observation group(55.17%)was higher than that of the control group(36.21%)(P<0.05).After two treatment cycle,CD3+,CD4+,and CD4+/CD8+levels were higher in the observation group compared to the control group,while CD8+,regulatory T cells,and natural killer cells were lower(P<0.05).Additionally,the incidence of leukopenia,nausea,and vomiting was lower in observed group(P<0.05).No significant differences were observed in the incidence of other adverse reactions(P>0.05).CONCLUSION Adjuvant therapy with Shenqixian granules may enhance the efficacy of simudizumab combined with FOLFOX4 chemotherapy in advanced gastric cancer and the immune function by increasing immune cell counts,making it a valuable option in clinical treatment.
