Paraquat(PQ),a highly effective herbicide,is widely used worldwide.PQ poisoning can cause multiple organ failure,in which the lung is the primary target organ.After PQ poisoning,the patient mortality rate is as high as 90%,and there is currently no specific antidote.The main clinical treatment is the use of glucocorticoids and cyclophosphamide for pulse therapy,but its effectiveness and safety are still uncertain.We investigated the effectiveness and safety of immunosuppressive pulse therapy with glucocorticoids and cyclophosphamide to evaluate the treatment value in patients with acute PQ poisoning.This meta-analysis,combined with seven trials that enrolled a total of 426 patients,showed that immunosuppressive pulse therapy with glucocorticoids and cyclophosphamide for PQ poisoning significantly reduced mortality of the study group(59.3%,134/226) compared with the control group(81.0%,162/200).There was no significant difference of hepatitis or renal failure between the control and study groups,indicating that immunosuppressive pulse therapy was relatively safe.Several patients were reported to have leukopenia and returned to normal after 1-2 weeks without any abnormalities.Two cases of non-fatal sepsis were reported and considered to be a side effect of the immunosuppressive pulse therapy.Thus,immunosuppressive pulse therapy can efficiently reduce the mortality of PQ poisoning and it is relatively safe. 展开更多

BACKGROUND High-dose steroid administration is a common initial therapeutic approach for Vogt–Koyanagi–Harada disease(VKH).Nonetheless,administering substantial doses of steroids to pregnant women necessitates meticulous consideration due to the potential impacts on the mother and fetus.We present a case wherein steroid pulse therapy was administered to a patient who developed VKH during the late stages of pregnancy.CASE SUMMARY The patient was a 26-year-old nulliparous woman.At 33 weeks and 1 day of her pregnancy,she experienced a decline in visual acuity and noticed metamorphopsia in her left eye.Examination revealed bilateral serous retinal detachment,leading to VKH diagnosis.A collaborative effort involving the departments of ophthalmology,internal medicine,and neonatology was initiated.Steroid pulse therapy was administered at 34 weeks and 1 day of pregnancy under hospital supervision.Complications,such as threatened preterm labor and gestational diabetes,emerged,necessitating the initiation of oral ritodrine hydrochloride and insulin therapy.Then,serous retinal detachment was resolved,and visual acuity was restored.Labor pains initiated 32 days post-initiation of steroid pulse therapy(at 38 weeks and 4 days of gestation),culminating in a normal delivery.Mother and newborn experienced an uneventful puerperal course and were discharged from the hospital on the 5th day following delivery.CONCLUSION VKH management in pregnancy requires multidisciplinary coordination,emphasizing collaboration with ophthalmologists and specialists in internal medicine and neonatology. 展开更多

Combining orbital intensity modulated radiation therapy with periorbital triamcinolone acetonide injection for Graves’ orbitopathy

AIM:To evaluate the efficacy of combined orbital radiation and periorbital triamcinolone acetonide injection for patients with Graves’orbitopathy(GO)who experienced treatment failure with glucocorticoid pulse therapy(GPT).METHODS:A total of 57 eligible patients(35.09%males,mean age of 51.19±11.90y)were included in this case-series study.The medical information collected during each visit was evaluated and analyzed.RESULTS:Significant improvement was observed in patients six months after radiation therapy.Both the clinical activity score and the efficacy score showed substantial improvement(P<0.001).Furthermore,there was significant resolution of extraocular muscle inflammation on magnetic resonance imaging at three and six months after radiation therapy.The initial high signal intensity ratio(SIR)max was found to be associated with greater improvement in SIR sum(P<0.001,B=2.002,95%CI:1.377 to 2.628),while the presence of sight-threatening stage or moderate to severe diplopia negatively influenced the improvement of SIR sum(P=0.045,0.008,0.006;B=-1.966,-1.478,-0.997;95%CI:-3.886 to-0.045,-2.552 to-0.403,-1.694 to-0.300;respectively).CONCLUSION:The combination therapy demonstrates significant effectiveness in treating patients with GO who experienced severe ocular inflammation and have previous GPT failure.Noticeable improvement is observed as early as one month after initiating radiation therapy,and patients with more severe inflammatory states showes greater benefit from the treatment. 展开更多

Objective:Based on the theories of“defensive Qi entering Yin”and“Qi and blood in meridians”in traditional Chinese medicine(TCM),this study explores the synergistic intervention effect of the Brain Health Sleep-Aid Pillow on brain diseases(such as insomnia,vertigo)and cervical spondylosis.The pillow achieves“marrow sea replenishment and unobstructed cerebral collaterals”by regulating cerebral blood perfusion and cervical mechanical balance through linear pulsed magnetic guidance of the Du Meridian and Bladder Meridian.Methods:A total of 120 patients with“neck impediment”and“insomnia”who met the Criteria for Diagnosis and Therapeutic Effect of TCM Syndromes were enrolled.They were randomly divided into an experimental group(60 cases,using Yuxuangong magnetic therapy pillow)and a control group(60 cases,using ordinary buckwheat pillow)by block randomization.The intervention lasted for 8 weeks.The efficacy was comprehensively evaluated using indicators including TCM syndrome scores,tongue and pulse analysis,cervical range of motion assessment,sleep quality(PSQI scale),and transcranial Doppler(TCD)[1].Results:The total TCM syndrome score in the experimental group decreased significantly more than that in the control group(-4.5±1.2 vs.-2.3±0.8,P<0.001);The cervical flexion range of motion increased by 15.3°±3.2°(P<0.01),and the total PSQI score decreased by 6.5 points(P<0.001);In the experimental group,the vertebral artery blood flow velocity increased by 12.3%(P<0.05),and the serum IL-6 level decreased by 0.08 pg/mL(P<0.05)[2].Conclusion:The Yuxuangong Brain Health Sleep-Aid Pillow provides a modern scientific interpretation of the TCM theory of“defensive Qi entering Yin”through a three-dimensional mechanism of“magnetic field regulating Qi and dredging collaterals,and physical mechanical reset”,which has significant clinical transformation value[3]. 展开更多

Pulse microwave excite thermoacoustic(TA)shockwave to destroy tumor cells in situ.This has promising applications for precise tumor therapy in deep tissue.Nanoparticle(NP)with high microwave-acoustic conversion is the key to enhance the efficiency of therapy.In this study,we firstly developed defect-rich titanium nitride nanoparticles(TiN NPs)for pulse microwave excited thermoacoustic(MTA)therapy.Due to a large number of local structural defects and charge carriers,TiN NPs exhibit excellent electromagnetic absorption through the dual mechanisms of dielectric loss and resistive loss.With pulsed microwave irradiation,it efficiently converts the microwave energy into shockwave via thermocavitation effect,achieving localized mechanical damage of mitochondria in the tumor cell and yielding a precise antitumor effect.In addition to the therapeutic function,the NP-mediated TA process also generates images that provide valuable information,including tumor size,shape,and location for treatment planning and monitoring.The experimental results showed that the TiN NPs could be efficiently accumulated in the tumor via intravenous infusion.With the deep tissue penetration characteristics of microwave,the proposed TiN-mediated MTA therapy effectively and precisely cures tumors in deep tissue without any detectable side effects.The results indicated that defect-rich TiN NPs are promising candidates for tumor therapy. 展开更多
