BACKGROUND On January 22,2020,Macao reported its first case of coronavirus disease 2019(COVID-19)infection.By August 2021,the situation had escalated into a crisis of community transmission. In response, the government launched a recruitment campaign seeking assistance and services of healthcare workers(HCWs)from the private sector throughout Macao. These participants faced concerns about their own health and that of their families,as well as the responsibility of maintaining public health and wellness. This study aims to determine whether the ongoing epidemic has caused them physical and psychological distress.AIM To examine the influence of COVID-19 on the sleep quality and psychological status of HCWs in private institutions in Macao during the pandemic.METHODS Data were collected from December 2020 to January 2022.Two consecutive surveys were conducted. The Pittsburgh Sleep Quality Index(PSQI)scale,Self-Rating Anxiety Scale(SAS),and Self-Rating Depression Scale(SDS)were employed as investigation tools.RESULTS In the first-stage survey,32%of HCWs experienced a sleep disorder, compared to 28.45%in the second-stage survey.A total of 31.25%of HCWs in the first-stage survey and 28.03%in the second had varying degrees of anxiety.A total of 50.00%of HCWs in the first-stage survey and 50.63%in the second experienced varying degrees of depression. No difference in PSQI scores,SAS scores,or SDS scores were observed between the two surveys, indicating that the COVID-19 epidemic influenced the sleep quality and psychological status of HCWs.The negative influence persisted over both periods but did not increase remarkably for more than a year. However,a positive correlation was observed between the PSQI,SAS,and SDS scores(r=0.428-0.775,P<0.01),indicating that when one of these states deteriorated, the other two tended to deteriorate as well.CONCLUSION The sleep quality, anxiety, and depression of HCWs in private institution in Macao were affected by the COVID-19 epidemic. While these factors did not deteriorate significantly, the negative effects persisted for a year and remained noteworthy. 展开更多

AIM: To assess the effects of poor nutritional and psychological status on tolerance of cancer treatment and the recovery of physical performance status in patients with gastrointestinal cancer. METHODS: An epidemiological survey with respect to nutritional and psychological status in patients with gastrointestinal cancer was conducted among 182 operated patients in four provincial-level hospitals from December 2005 to June 2006. The food frequency survey method, state-trait anxiety inventory (STAI) and depression status inventory (DSI) were used to obtain information about the diet and psychological status in the patients. Nutritional status in the participants was reflected by serum albumin (Alb), hemoglobin (HB) and body mass index (BMI). RESULTS: Alb, protein intake and anxiety were associated with the severity of side effects of treatment. The adjusted relative risk (RR) for Alb, protein intake and anxiety was 3.30 (95% CI: 1.08, 10.10, P = 0.03), 3.25 (95% CI: 1.06, 9.90, P = 0.04) and 1.48 (95% CI: 1.29, 1.70, P < 0.0001), respectively. Moreover, calorie intake, HB and depression were associated with the recovery of physical performance status in the patients. Adjusted relative risk was 2.12 (95% CI: 1.09, 4.03, P = 0.028), 2.05 (95% CI: 1.08, 3.88, P =0.026) and 1.07 (95% CI: 1.02, 1.12, P = 0.007), respectively. CONCLUSION: Both poor nutrition status and psychological status are independent risk factors for severe side effects of cancer treatment, and have impact on the recovery of physical performance status in patients after treatment. 展开更多

Epidemiological studies showed that night workers are at higher risk of developing chronic metabolic diseases. However,no study has investigated the changes in circadian rhythms caused by a combined effect of sleep and diet in a real-life setting on cardiometabolic health, gut microbiota, and psychological status in healthy people.A 4-week step-wise misaligned-realigned controlled-feeding trial with a 2×2 factorial design(sleep and diet)was conducted on healthy young adults. At first, subjects experienced a one-week circadian rhythm misalignment with a high-fat fast-food diet, extended eating window, and delayed sleep schedules, then gradually transited to a complete circadian rhythm realignment with a high-fiber balanced diet,8-h timerestricted eating, and normal sleep schedules.Circadian rhythm misalignment led to significantly higher levels of fasting glucose and homeostatic model assessment for insulin resistance(HOMA-IR)of subjects compared to baseline and failed to recover to the baseline level in circadian rhythm realignments.Notably, the incremental area under the curve(iAUC)of postprandial glucose decreased with circadian rhythm adjustments as compared to that in circadian rhythm misalignment, suggesting circadian rhythm realignment by sleep or/and diet could partly restore glucose metabolism impaired by a short-term circadian rhythm misalignment. However, circadian rhythm changes did not result in overall perturbations of gut microbiota diversities. 展开更多

BACKGROUND The intensive care unit(ICU)is a core hospital unit for critically ill patients.A high-intensity treatment environment, frequent invasive procedures, and isolation from family members often lead to severe psychological stress reactions in patients, including anxiety, depression, and posttraumatic stress disorder.Studies have reported that up to 50%of patients in the ICU experience varying degrees of psychological disorders, leading to reduced treatment compliance and exacerbated metabolic disorders through neuroendocrine pathways, thereby negatively affecting prognosis.AIM To investigate the influence of psychological nursing interventions on psychological status and all-cause mortality among patients admitted to the ICU.METHODS Data were obtained from 100 patients with nutritional risk in the ICU of the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture, admitted from June 2021 to June 2023.They were randomly divided into two groups(n=50 each)based on nursing intervention:Control and study groups. At follow-up,the Zung Self-Rating Anxiety Scale(SAS)and the Self-Rating Depression Scale(SDS)were used.All-cause mortality and time to death were compared between the two groups. The relationship between psychological status and all-cause mortality was assessed using the Cox proportional hazards regression model.RESULTS There were no statistically significant differences in SAS and SDS scores between the two groups before the nursing intervention(P>0.05).However,after the psychological intervention, the SAS and SDS scores of the study group were significantly lower than those of the control group(P<0.05).The all-cause mortality rates in the study and control groups were 20%and 40%,respectively(P<0.05),indicating that psychological nursing interventions can significantly reduce all-cause mortality and improve clinical outcomes.Cox proportional hazards regression analysis revealed that a good psychological state reduced all-cause mortality(P<0.05).Nursing satisfaction in the study group was significantly higher than that in the control group(P<0.05),indicating a high level of recognition of the psychological intervention.CONCLUSION Psychological nursing interventions can effectively reduce the incidence of anxiety and depression in ICU patients with nutritional risk, improve their psychological state, reduce all-cause mortality, and improve their prognoses. 展开更多

Methods: the study period was from February 2019 to June 2021. A total of 116 patients with liver cancer underwent laparoscopic hepatectomy. They were divided into accelerated group and control group by drawing lots. The former was given accelerated rehabilitation surgical nursing while the latter was given routine nursing. The postoperative rehabilitation, complications, psychological status and pain degree before and after nursing were compared. Results: the time of exhaust, getting out of bed and hospitalization in the acceleration group was shorter than that in the control group, with significant difference (P < 0.05). The total incidence of postoperative complications in the accelerated group was lower than that in the control group, with significant difference (p < 0.05). The scores of HAMA and HAMD in the accelerated group were significantly lower than those in the control group (P < 0.05). The pain scores of the accelerated group at 6h and 48h were significantly lower than those of the control group (p < 0.05). Conclusion: the implementation of the concept of accelerated rehabilitation surgery in laparoscopic hepatectomy can improve the operation safety and rehabilitation efficiency of patients, help patients to improve their psychological state and reduce perioperative discomfort and postoperative pain. It is a more suitable nursing plan for such patients. 展开更多

Introduction:Physical inactivity among older adults represents a growing global concern.Understanding the characteristics and determinants of physical activity patterns(PAPs)is crucial for developing targeted physical activity(PA)promotion strategies for diverse subgroups. Methods:This study analyzed data from 39,957 adults aged 60–79 years recruited from 31 Provinciallevel administrative divisions(PLADs)through the 2020 Chinese National Survey on Adults’Fitness.Participants completed comprehensive questionnaires and underwent physical fitness assessments. The selfadministered questionnaire captured demographic characteristics, living environment conditions, life satisfaction, and stress levels.PA was evaluated under normal conditions using a domain-specific questionnaire that measured weekly activity across leisure, occupational, transport, and home domains, along with sedentary behavior.Physical fitness measurements encompassed anthropometric parameters(height, weight, body fat percentage),cardiorespiratory fitness(lung capacity,2-minute step test),muscular strength(grip strength),flexibility(sit and reach test),functional capacity(30-second chair stand test),balance(one-leg standing with eyes-closed test),and neuromuscular function(reaction time test).K-means clustering analysis is used for identifying PAPs.Results:K-means clustering analysis identified three distinct PAPs among Chinese older adults:low activity(LA)cluster(n=21,291,53.3%),active leisure(AL)cluster(n=12,543,31.4%),and active home(AH)cluster(n=6,123,15.3%).The AL cluster demonstrated superior psychological status, physical fitness, and built environment conditions compared to both AH and LA clusters, while the AH cluster exhibited better physical fitness and built environment characteristics than the LA cluster. Conclusions:These findings reveal distinct PA profiles across various life domains among older adults. The observed inter-cluster differences suggest that targeted intervention strategies and policies may benefit different elderly subgroups, particularly through enhanced sociopsychological and built environment support, coupled with precise guidance for promoting physical health. 展开更多

BACKGROUND Owing to age-related physiological declines, elderly patients undergoing radical gastrectomy(RG)often experience physical and psychological challenges.Hence, appropriate nursing strategies need to be identified for optimizing patient outcomes.AIM To evaluate the effects of the Neuman Systems Model-guided stepped care intervention(NGSC)model on postoperative physical and psychological recovery in elderly patients with gastric cancer who underwent RG.METHODS In total,80 elderly patients(≥65 years old)who underwent RG at the Shanxi Fenyang Hospital between January 2023 and June 2024 were included and randomly divided as follows:Control group(n=40,those receiving conventional stepped care intervention);observation group(n=40,those receiving NGSC).Postoperative recovery indices,anxiety/depression scores(Self-Rating Anxiety Scale/Self-Rating Depression Scale),quality of life(36-item short form health survey),and adverse event rates were compared between the groups.RESULTS Compared with the control group, the observation group presented significantly shorter recovery times for first bowel sounds, ambulation, flatus, oral intake, catheter removal, and hospitalization(all P<0.05).Notably,in the observation group,Self-Rating Anxiety Scale/Self-Rating Depression Scale scores and the adverse event rate were significantly lower(all P<0.05),whereas 36-item short form health survey scores were higher(P<0.05),compared with those in the control group.CONCLUSION Overall, the findings of this study show that the proposed NGSC model accelerates postoperative recovery, alleviates psychological distress, improves quality of life, and reduces complications in elderly patients with gastric cancer who underwent RG,presenting potential for clinical application. 展开更多
