BACKGROUND Colorectal cancer is one of the malignant tumors with a high incidence and mortality rate globally,and the occurrence of liver metastasis significantly affects patient survival prognosis.In recent years,the...BACKGROUND Colorectal cancer is one of the malignant tumors with a high incidence and mortality rate globally,and the occurrence of liver metastasis significantly affects patient survival prognosis.In recent years,the application of immune checkpoint inhibitors(ICIs)in cancer treatment has made important progress,especially showing good therapeutic effects in patients with high microsatellite instability or mismatch repair deficiency.However,for the majority of patients with microsatellite stable(MSS)or proficient mismatch repair(pMMR)colorectal cancer,the efficacy of ICIs is limited,prompting researchers to explore combination therapy strategies to improve efficacy.Targeted drugs such as tyrosine kinase inhibitors(TKIs)and radiotherapy are believed to work synergistically with ICIs by modifying the tumor microenvironment and enhancing antigen presentation.AIM To investigate the efficacy and safety of the combination therapy of radiotherapy,ICIs,and TKIs in patients with MSS or pMMR colorectal cancer liver metastasis(CCLM),in order to provide new clinical treatment references.METHODS A retrospective analysis was conducted on the clinical data of 43 MSS or pMMR CCLM patients treated at our hospital from September 2021 to July 2024.Based on the treatment interventions received,the patients were divided into a control group(n=21,receiving ICIs and TKIs combination therapy)and an observation group(n=22,receiving radiotherapy,ICIs,and TKIs triple therapy).The therapeutic effects,serum tumor markers(carcinoembryonic antigen and RESULTS The disease control rate in the observation group(63.64%)was significantly higher than that of the control group(23.81%)(P<0.05)).Both groups showed a decrease in carcinoembryonic antigen and carbohydrate antigen 199 levels post-treatment,with the observation group demonstrating a more significant change(P<0.05).The median progression-free survival and median overall survival in the control group were 5.1 months and 7.6 months,respectively,while the observation group had a median progression-free survival and overall survival of 4.3 months and 6.9 months,respectively.The control group had longer survival times than the observation group,but the differences were not statistically significant(P>0.05).The incidence of adverse reactions,including nausea and vomiting,gastrointestinal reactions,skin reactions,bone marrow suppression,liver and kidney function impairment,neurotoxicity,leukopenia,neutropenia,and thrombocytopenia,showed no significant differences between the two groups(P>0.05).CONCLUSION Compared to the ICIs and TKIs combination therapy,the radiotherapy,ICIs,and TKIs triple therapy can further improve the disease control rate and serum tumor marker levels in MSS or pMMR CCLM patients without increasing the risk of related adverse reactions,making it a treatment regimen worthy of further validation.展开更多

Apart from that, the research of teacher autonomy at different phases of teachers' professional development may provide insight into how to strengthen their autonomy capability and to better understand the differences in teachers' development at different stages. The current study aims to explore the different level of how university novice and proficient English teachers see themselves as autonomous practitioners on the five dimensions of teacher autonomy: teaching autonomy, assessment autonomy, school management autonomy, professional development autonomy, and curriculum development autonomy. BACKGROUND Endometrial cancer(EC)is one of the most common cancers of the female reproductive tract,and the incidence is increasing rapidly.Immunotherapy using programmed cell death-1(PD-1)inhibitors is an emerging re...BACKGROUND Endometrial cancer(EC)is one of the most common cancers of the female reproductive tract,and the incidence is increasing rapidly.Immunotherapy using programmed cell death-1(PD-1)inhibitors is an emerging research topic and treatment strategy for refractory gynecological malignancies.However,clinical management of EC with checkpoint inhibitors requires improvement.Herein,we discuss a case of refractory proficient mismatch repair(pMMR)/miscrosatellitestable(MSS)EC treated with a combination of PD-1 and angiogenesis inhibitors and offer a review of the pathophysiology and clinical outcomes based on previous studies.CASE SUMMARY A 62-year-old woman diagnosed with invasive or metastatic EC in 2015 was treated with six courses of chemotherapy and refused further radiotherapy.Four years later,she developed chest pain,and lung biopsy indicated thyroid transcription factor-1(-),Napsin A(-),estrogen receptor(+),progesterone receptor(+),anaplastic lymphoma kinase(D5F3)(-),and receptor tyrosine kinase(D4D6)(-)metastatic EC.Genetic testing results showed low tumor mutation burden,pMMR,PD ligand 1(-),MSS,and HLA-class 1 heterogeneous disease.The patient was started on toripalimab combined with nab-paclitaxel for seven cycles(every 3 wk),but this regimen was terminated because of an intolerable chemotherapy adverse event.The disease progressed in 2020,and the patient’s treatment was switched from nab-paclitaxel to anlotinib,while immunotherapy using toripalimab was continued.The patient achieved a major partial response with well-tolerated toxicities,and treatment is ongoing.CONCLUSION Molecular testing is advised for clinical classifications of EC owing to its high heterogeneity.In this case,the patient had pMMR/MSS EC and achieved a positive outcome with combination PD-1 inhibitor treatment.These results warrant further clinical exploration.展开更多

A class cannot be good or well performed without planning or preparation. Referring to the five stages model of Dreyfus(1986) and considering the teaching context of middle schools, the researcher uses qualitative and quantitative methods to get and analyze the data from 96 English teachers in three Chinese middle school. The findings show that in teaching content planning, novice and some proficient teachers cares more about the knowledge and the teaching itself, but the expert teachers pay more attention to the students' abilities and development as well as the knowledge; moreover, the novice teachers are more likely to focus on the teaching aims and choosing of teaching aid. This study also has significance in contributing to the teachers' expertise in planning.展开更多
BACKGROUND Immunotherapy is an approved treatment for metastatic rectal cancer in patients with defective mismatch repair(MMR).AIM To examine the clinical efficacy of neoadjuvant immunotherapy combined with radiothera...BACKGROUND Immunotherapy is an approved treatment for metastatic rectal cancer in patients with defective mismatch repair(MMR).AIM To examine the clinical efficacy of neoadjuvant immunotherapy combined with radiotherapy and chemotherapy for the treatment of locally advanced rectal cancer(LARC),with a focus on patients with proficient MMR(pMMR)and mic-rosatellite stability.METHODS Two researchers searched multiple databases for publications up to September 2024.All included publications examined neoadjuvant immunotherapy for LARC,and reported major pathological response(MPR),pathological complete response(pCR),clinical complete response(CCR),and rates of R0 resection and anus-pre-serving surgery.Meta-analysis,subgroup analysis,sensitivity analysis,and ana-lysis of publication bias were performed.RESULTS We included 15 publications(796 patients).The MPR,pCR,and CCR were sig-nificantly better in the group that received immunotherapy(all P<0.05),espe-cially for patients with pMMR.In addition,the rate of R0 resection and anus-preserving surgery were also significantly greater in the group that received neoadjuvant immunotherapy(both P<0.05).Hematological toxicity and abnormal liver function were the most common clinical adverse events above grade 3.Most patients successfully completed the immunotherapy treatment.The incidence of immune-related adverse reactions was 0%-13.5%,and the severities of these events were generally considered acceptable.CONCLUSION The addition of neoadjuvant immunotherapy improved the clinical remission rate of patients who had LARC with pMMR,and the treatment-related adverse reactions were generally acceptable.Neoadjuvant immunotherapy combined with radiotherapy and chemotherapy should be considered for patients with LARC.展开更多

Identification process of LEP (Limited English Proficient) students via nomination, testing and testing criteria to be identified as LEP, hence ESL if placed in the program is defined in detail Standardized testing process of these students based on the federal AYP (Adequate Yearly Progress) measures through regular state standardized tests depending on three types of instructional program that LEP student is placed in the school that are Bilingual Education Program, ESL Program, and Two-Way Immersion Dual Language Programs are described. Standardized testing process of these students based on the federal AYP measures through other means if exempted from regular state standardized tests such as LAT (Linguistically Accommodated Testing) and the conditions to be exempted are discussed in detail. Holistically rated English Language Proficiency Assessments that are implemented to fulfill federal AMAOs (Annual Measurable Achievement Objectives) measures are defined briefly. Physical features of LAT test as well as direct and indirect accommodations for LAT reading, mathematics and science tests are defined via the accommodation needed and the type of assistance provided. 文摘With the continuous advancement of information technology,corpora and knowledge graphs(KGs)have become indispensable tools in modern language learning.This study explores how the integration of corpora and KGs in integrated English teaching can enhance students’abilities in vocabulary acquisition,grammar understanding,and discourse analysis.Through a comprehensive literature review,it elaborates on the theoretical foundations and practical values of these two technological tools in English instruction.The study designs a teaching model based on corpora and KGs and analyzes its specific applications in vocabulary,grammar,and discourse teaching within the Integrated English course.Additionally,the article discusses the challenges that may arise during implementation and proposes corresponding solutions.Finally,it envisions future research directions and application prospects.

文摘The monitoring data is undoubtedly important to the water quality monitor- ing department. The proficiency testing is an important way to improve the monitor- ing capacity and enhance the quality management of laboratories. It plays an impor- tant role in ensuring the accuracy, integrity and comparability of monitoring data. In this paper, the positive role of proficiency testing in the water quality monitoring was analyzed. In addition, how to improve the water quality monitoring capacity and the quality management level of laboratories through the proficiency testing was also discussed.
文摘This paper presents a survey of OPI of ICAO English test in greater depth.The study of OPI provides a clear orientation to OPI ICAO English test requirements in speaking competences,the CAAC OPI characteristics,ICAO Language Proficiency Rating scales and the assessing principles.The paper also offers some suggestions in the interview which will be no doubt of great help to the candidates.
文摘The English level of the staff is generally intermediate since they have learned general English and medical English during their college and universities,the courses can be intensively taught in two months with three courses every week.The proficiency level of the exchanged clinical doctors,their linguistic needs as well as the communicative needs will be imaginatively illustrated according to the literatures which are about the medical staff's needs for English as a surviving tool of working in foreign hospitals.
文摘The existing summative evaluation method cannot reveal all the relevant facts about students’learning,potentially leading students to only pay attention to memorizing fragmented language knowledge instead of improving their communicative competence.Therefore,the formative evaluation method should be used to supplement the current summative evaluation method in College English teaching in vocational institutes.When using formative evaluation,plans and objectives need to be designed,tools for evaluation selected,information collected regularly,and plans adjusted accordingly.
文摘Vocabulary is the core of language learning.The efficiency of vocabulary learning may influence language learning outcome.Vocabulary learning strategies can help the learners to learn vocabulary more efficiently,and they are very useful and beneficial for language learners.This paper synthesizes the research on vocabulary learning strategies in ESL and EFL learning,including the studies on the definitions of vocabulary learning strategies,classifications of vocabulary learning strategies and previous main research areas.The previous research areas mainly involve in the following aspects:patterns and taxonomies of vocabulary learning strategies,gender difference and vocabulary learning strategies,language proficiency and vocabulary learning strategies,major or disciplinary difference and vocabulary learning strategies,vocabulary learning strategies used frequently and effectively,vocabulary learning strategies and learning outcomes,and vocabulary learning strategy training.
文摘Based on the interactive theory of reading, this study mainly explored the etlects ot topic familiarity and second language proficiency on IVA (Incidental Vocabulary Acquisition) of second language through reading. By using two different measures (translation production and selection), this study found: (1) Vocabulary can be acquired incidentally in reading passages; (2) There are no significant interactive effects of topic familiarity and second language proficiency on vocabulary acquisition, but the two independent variables of topic familiarity and second language proficiency exerted their positively significant effect on incidental vocabulary acquisition, and (3) As for the two vocabulary measures, learners can acquire more words in translation selection than in translation production.
文摘In the light of need to use learning styles to enhance verbal proficiency,this paper will explore the interconnectedness of learning styles and proficiency in verbal communication.It will first introduce the background of the study.This will be followed by an overview of the research on related literature and studies in the field.Lastly the paper concludes that foreign language teachers should give attention to the styles for languages learning and verbal proficiency since they are closely correlated.
文摘A comprehensive overview of standardized testing system for ESL (English as a Second Language) students in U.S kindergarten thru grade 12 public school system is reviewed within the context of basis on federal and state laws, as well as instructional basis. Identification process of LEP (Limited English Proficient) students via nomination, testing and testing criteria to be identified as LEP, hence ESL if placed in the program is defined in detail Standardized testing process of these students based on the federal AYP (Adequate Yearly Progress) measures through regular state standardized tests depending on three types of instructional program that LEP student is placed in the school that are Bilingual Education Program, ESL Program, and Two-Way Immersion Dual Language Programs are described. Standardized testing process of these students based on the federal AYP measures through other means if exempted from regular state standardized tests such as LAT (Linguistically Accommodated Testing) and the conditions to be exempted are discussed in detail. Holistically rated English Language Proficiency Assessments that are implemented to fulfill federal AMAOs (Annual Measurable Achievement Objectives) measures are defined briefly. Physical features of LAT test as well as direct and indirect accommodations for LAT reading, mathematics and science tests are defined via the accommodation needed and the type of assistance provided. Lastly, testing criteria to exit LEP status and other issues concerning exiting LEP status are reviewed
