It is inevitable, however, that certain problems exist during the construction of ecological city, which include but not limited to non-harmonious development of urban complex ecosystem, and the difficulty in quantifying eco-city construction or incomplete quantification in assessing the con- structiori of present and future eco-city. Based on the analysis on social-economic conditions and regional conditions of the UAYD, this paper attempts to set up an index system of eco-cities combining with local characteristics, and to adopt the indices of eco-city, urban harmony, and eco-city colligate to evaluate the ecological level, urban harmonious development and eco-city construction of cities within the UAYD. Results indicate that among 15 cities in UAYD, Suzhou City ranks the highest in terms of eco-city construction, whereas Nantong ranks relatively lower; sustainable eco-city construction is possible only when cities are developed in every respect of harmony.展开更多

Methods: This study is a prospective controlled trial that selected patients undergoing two-port thoracoscopic valve surgery at a certain hospital from January 2019 to December 2024. Patients were randomly assigned to the control group and the negative pressure group using a random number table method. The control group consisted of 30 patients (20 males, 10 females, mean age 42.03 ± 12.89 years), and the negative pressure group consisted of 35 patients (26 males, 9 females, mean age 41.84 ± 11.83 years). The control group received traditional chest drainage, while the negative pressure group received negative pressure chest drainage. Postoperative pain scores, hospital stay, drainage time, number of tube blockages, and incidences of pneumothorax or subcutaneous emphysema were recorded and statistically analyzed. Results: The negative pressure group had a significantly shorter postoperative drainage time compared to the control group (49.09 ± 11.99 hours vs. 79.10 ± 7.32 hours, P < 0.001). The postoperative pain score was lower in the negative pressure group (4.49 ± 1.27 vs. 7.03 ± 0.85, P < 0.001), and the hospital stay was significantly shorter (9.83 ± 1.69 days vs. 14.73 ± 2.32 days, P < 0.001). The incidence of pneumothorax or subcutaneous emphysema was significantly lower in the negative pressure group than in the control group (14.29% vs. 56.67%, P = 0.0003). Conclusion: The application of negative pressure chest drainage in patients after two-port thoracoscopic valve surgery can effectively reduce postoperative pain, shorten hospital stay, and lower the incidence of tube blockage and pneumothorax, demonstrating good clinical outcomes.展开更多

文摘Pre-Authentication and Post-Connection(PAPC)plays a crucial role in realizing the Zero Trust security model by ensuring that access to network resources is granted only after successful authentication. While earlier approaches such as Port Knocking(PK)and Single Packet Authorization(SPA)introduced pre-authentication concepts, they suffer from limitations including plaintext communication, protocol dependency, reliance on dedicated clients, and inefficiency under modern network conditions. These constraints hinder their applicability in emerging distributed and resource-constrained environments such as AIoT and browser-based systems. To address these challenges, this study proposes a novel port-sequence-based PAPC scheme structured as a modular model comprising a client, server, and ephemeral Key Management System(KMS).The system employs the Advanced Encryption Standard(AES-128)to protect message confidentiality and uses a Hash-Based Message Authentication Code(HMAC-SHA256)to ensure integrity.Authentication messages are securely fragmented and mapped to destination port numbers using a signature-based avoidance algorithm, which prevents collisions with unsafe or reserved port ranges. The server observes incoming port sequences, retrieves the necessary keys from the KMS,reconstructs and verifies the encrypted data, and conditionally updates firewall policies.Unlike SPA,which requires decrypting all incoming payloads and imposes server-side overhead, the proposed system verifies only port-derived fragments, significantly reducing computational burden.Furthermore,it eliminates the need for raw socket access or custom clients, supporting browser-based operation and enabling protocol-independent deployment.Through a functional web-based prototype and emulated testing, the system achieved an F1-score exceeding 95%in detecting unauthorized access while maintaining low resource overhead.Although port sequence generation introduces some client-side cost,it remains lightweight and scalable.By tightly integrating lightweight cryptographic algorithms with a transport-layer communication model, this work presents a conceptually validated architecture that contributes a novel direction for interoperable and scalable Zero Trust enforcement in future network ecosystems.

文摘BACKGROUND Central venous access is essential for administering chemotherapy in patients with gastrointestinal cancer.Peripherally inserted central catheters(PICC)and totally implantable venous access ports(TIVAP)are widely used, but comparative data regarding their impact on catheter-related complications and quality of life(QoL)remain limited.AIM To evaluate the impact of TIVAPs compared with PICC on catheter-related complications and QoL in patients with gastrointestinal cancer undergoing chemotherapy.METHODS This retrospective study included adults with gastrointestinal cancer who underwent central venous access device insertion for chemotherapy at our institution between December 2021 and December 2024.Inclusion criteria encompassed indications for intermittent intravenous chemotherapy, anticipated treatment duration of≥12 weeks,an adequate preoperative hematologic profile, accessible upper body veins, and complete medical records.Patients were excluded if they had an anticipated survival of less than three months, active systemic infection, severe thrombosis or coagulopathy, communication barriers,or an urgent need for dialysis access.Patients were assigned to either the PICC or TIVAP group based on device type.Data collected included demographic variables, cancer characteristics, insertion procedure details, complications, and QoL,assessed via the EuroQol 5-Dimensions-3 levels, visual analogue scale, and the European Organization for Research and Treatment of Cancer QoL Questionnaire-Core 30.RESULTS A total of 346 patients were analyzed.Baseline demographic, clinical, and cancer characteristics were similar between groups. The TIVAP group demonstrated a significantly lower incidence of catheter-related complications than the PICC group, with no pneumothorax occurring in either group.QoL assessments at baseline were comparable. At one month, the TIVAP group exhibited significantly higher EuroQoL Five Dimensions health state scores and QLQ-C30 global health status scores.Multivariate analysis identified TIVAP use, catheter tip placement in the distal superior vena cava/right atrium, prophylactic antibiotic administration, and antimicrobial dressing application as independent protective factors associated with reduced complications and improved QoL.CONCLUSION In patients with gastrointestinal cancer undergoing chemotherapy,TIVAPs are associated with a lower incidence of catheter-related complications and improved QoL than PICCs.Optimal device selection, precise catheter tip positioning, and effective perioperative management are critical for minimizing complications and enhancing patient-reported outcomes during treatment.
文摘Objective: This study aims to investigate the drainage effect and clinical outcomes of negative pressure chest drainage in patients after two-port thoracoscopic valve surgery, comparing the differences in postoperative pain, hospital stay, and other factors between the negative pressure group and the control group. Conclusion: The application of negative pressure chest drainage in patients after two-port thoracoscopic valve surgery can effectively reduce postoperative pain, shorten hospital stay, and lower the incidence of tube blockage and pneumothorax, demonstrating good clinical outcomes.
文摘The port railway operation in Germany and Europe is not least due to its long history of technological and regulatory developments characterized by a high number of players and a lack of comprehensive planning and optimization of all relevant processes.Innovative technologies and business processes are therefore useful and needed to achieve key steps on the way to an overall optimization of rail transport within global supply chains.A substantial part of the European rail freight transport has its origin or its destination in an inland or sea port.Considering the railway system, ports play a more important role for urgently needed innovations than the pure interfaces between sea and land transport.Especially ports with their own railway system have an own responsibility in this matter.Insofar the goal of the Bremen Port Railway—which already now has a leading share of railway in modal split in Europe—is to exploit the opportunities offered by digitalization.Significant steps for this are the optimization and gradually automatization of rail operational processes on the last mile including modern and transparent IT systems and the designing of autonomous shunting processes.Together with research partners ISL(Institute of Shipping Economics and Logistics Bremen),BIBA(Bremen Institute for Production and Logistics)and IVE(Institute for Transport,Railway Construction and Operation in Brunswick)and in connection with associated business partners the project Rang-E has been applied for at the Ministry of Transport in the funding initiative IHATEC(Innovated HArbour TEChnologies)—and had won a grant to perform the proposed work.Basic thoughts are outlined in the following.
文摘AIM: To evaluate the feasibility of a second parallel transjugular intrahepatic portosystemic shunt (TIPS) to reduce portal venous pressure and control complications of portal hypertension.
