[Result] The pointed traits of cocoons displayed obviously in the wild silkworm descendants, its pointed parts were consistent with the rear part of silkworm chrysalis, and the female silkworm has slight difference, indicating the silkworm co- coon pointed trait was hereditary. [Conclusion] This study provided references for the study of silkworm cocoon traits of wild silkworm descendant and guides the selective breeding of silkworm variety.展开更多

The objective of our study was to describe the epidemiological, clinical and therapeutic aspects of breast cancer at the General Hospital of Loandjili in Pointe Noire. Patients and Methods: This was a descriptive and cross-sectional study that took place in the Cancer Department of the General Hospital of Loandjili in Pointe Noire during the period from January 1st, 2012 to December 31st, 2016 for duration of 5 years. 54 records of patients over 18 years of age with histological evidence were collected. The variables studied were: age, level of education, time to presentation, telltale sign, stage of extension, histological type, and histological grade. Chi-square test was used to compare variables and groups. Results: At the end of this study, we collected 54 files of breast cancer patients from 150 patients hospitalized in the Cancer Department. The frequency of breast cancer was 36%. The average age was 50 ± 13.6 years;the extremes were 27 years and 79 years. The most represented age group was from 37 to 40 years old with a percentage of 28%. The patient’s level of study was primary in the majority of cases (57%). The most represented histological type was invasive ductal carcinoma, (100%). The majority of patients arrived at advanced stages 44.4% and metastatic stages 26%. Patients were treated in most of cases by chemotherapy (82%), surgery (42%) and radiotherapy (10%). There was a correlation between time of presentation and stage of extension. Conclusion: Breast cancer is the first cancer of the woman in Pointe Noire. Patients arrive at advanced stages;the most common histological type is invasive ductal carcinoma. The presentation time is correlated with the extension stage. These results suggest the establishment of a cancer control program in developing countries.展开更多
BACKGROUND Sarcoidosis is a rare multisystem disease characterized histologically by noncaseating granuloma formation in the affected organ.While cardiac sarcoidosis is found on autopsy in up to 25%of sarcoidosis case...BACKGROUND Sarcoidosis is a rare multisystem disease characterized histologically by noncaseating granuloma formation in the affected organ.While cardiac sarcoidosis is found on autopsy in up to 25%of sarcoidosis cases,it is still underdiagnosed and is associated with a poor prognosis.Although the etiology of sarcoidosis remains unclear,an antigen triggered exaggerated immune response has been hypothesized.Early detection and prompt management of cardiac sarcoidosis remains pivotal.CASE SUMMARY A 60-year-old female,with pulmonary sarcoidosis in remission,presented to the cardiology outpatient clinic for evaluation of weeks-long dyspnea on moderate exertion(New York Heart Association class II)that was relieved by rest.Submaximal exercise stress test showed multifocal ventricular extrasystoles,followed by a self-limiting torsades de pointes.Cardiac magnetic resonance imaging showed nondilated and normotrophic left ventricle with basoseptal and mid-septal dyskinesis.The magnetic resonance imaging-derived left ventricular ejection fraction was 45%.Delayed enhancement showed patchy transmural fibrosis of the septum and hyperenhancement of the papillary muscles,all in favor of extensive cardiac involvement of sarcoidosis.A double-chamber implantable cardiac defibrillator was implanted,and methylprednisolone(12 mg/d)and methotrexate(12.5 mg/wk)treatment was initiated.Follow-up and implantable cardiac defibrillator interrogation showed episodes of asymptomatic nonsustained ventricular tachycardia and an asymptomatic episode of nonsustained ventricular tachycardia ending by the first antitachycardia pacing run.CONCLUSION Along an extensive review of the literature,this unusual case report highlights the importance of early detection of cardiac involvement of sarcoidosis,in order to avoid potential complications and increase survival.展开更多
BACKGROUND QT interval prolongation can induce torsades de pointes(TdP),a potentially fatal ventricular arrhythmia.Recently,an increasing number of non-cardiac drugs have been found to cause QT prolongation and/or TdP...BACKGROUND QT interval prolongation can induce torsades de pointes(TdP),a potentially fatal ventricular arrhythmia.Recently,an increasing number of non-cardiac drugs have been found to cause QT prolongation and/or TdP onset.Moreover,recent findings have demonstrated the key roles of systemic inflammatory activation and fever in promoting long-QT syndrome(LQTS)and TdP development.CASE SUMMARY A 30-year-old woman was admitted with a moderate to high-grade episodic fever for two weeks.The patient was administered with multiple antibiotics after hospitalization but still had repeating fever and markedly elevated C-reactive protein.Once after a high fever,the patient suddenly lost consciousness,and electrocardiogram(ECG)showed transient TdP onset after frequent premature ventricular contraction.The patient recovered sinus rhythm and consciousness spontaneously,and post-TdP ECG revealed a prolonged QTc interval of 560 ms.The patient’s clinical manifestations and unresponsiveness to the antibiotics led to the final diagnosis of adult-onset Still’s disease(AOSD).There was no evidence of cardiac involvement.After the AOSD diagnosis,discontinuation of antibiotics and immediate initiation of intravenous dexamethasone administration resulted in the normal temperature and QTc interval.The genetic analysis identified that the patient and her father had heterozygous mutations in KCNH2(c.1370C>T)and AKAP9(c.7725A>C).During the 2-year follow-up period,the patient had no recurrence of any arrhythmia and maintained normal QTc interval.CONCLUSION This case study highlights the risk of systemic inflammatory activation and antibiotic-induced TdP/LQTS onset.Genetic analysis should be considered to identify individuals at high risk of developing TdP.展开更多

About 5% to 10% of breast cancer patients present with metastases. While the 5-year survival of patients with local breast cancer varies around 98.8%, this survival rate drops to around 26.3% for metastatic patients. The objective of this study was to determine the survival of patients with metastatic breast cancer in resource-limited settings. Patients and Methods: This was a cross-sectional descriptive study that took place in the Cancer Department of the General Hospital of Loandjili in Pointe Noire during the period from January 1, 2013 to December 31, 2018, for duration of 6 years. 30 records of patients over 18 years of age and with histological evidence who received at least 3 courses of chemotherapy were collected. The variables studied were: age, level of education, socio-economic level, menopausal status, history, WHO status, menopausal status, tumor size, histological type, tumor location, the type of treatment and survival. Survival was calculated by Kaplan Meier method. Fisher’s exact test was used to search for correlation between variables. Results: The average age was 52.62 ± 10.96 years old. The extremes were 33 years and 75 years old. The most represented level of education was the primary level in 67% of cases. The majority of patients had low socioeconomic status in 50% of cases. The patients were menopausal in 57% of cases. The antecedents of cancer were present in 13% of cases. 50% of patients had a WHO status performance at 2. The tumor size was greater than 2 cm in 77% of cases. The most represented histological type was invasive ductal carcinoma in 93% of cases. The most represented histological grade was Scharff grade III Richardson bloom in 80% of cases. The most represented metastatic localization was pulmonary in 33% of cases. The metastatic localizations were unique in 47% of cases and multiple in 53% of cases. Anthracycline-based chemotherapy was more used in 53% of cases. Bivariate analysis revealed a correlation between tumor size and number of metastases, p 0.05. Patients treated with anthracyclines were greater than that of patients treated with taxanes in combination was 26.48 months, p > 0.05. Conclusion: Metastatic breast cancer remains an incurable disease, its survival remains low despite diagnostic and therapeutic advances that remain difficult to access for our resource-poor developing countries. Patients are treated with conventional chemotherapy (anthracyclines and taxanes). The most common metastases are respectively pulmonary, hepatic and cerebral in our context.展开更多

PATIENTS AND METHODS: This was a retrospective descriptive study that took place in the cancerology and internal medicine department during the period from January 1, 2012 to December 31, 2021, i.e. a period of 10 years. Women with histological diagnosis of gynecological and breast cancers were included in our study. The variables studied were: frequency, age, alcohol and tobacco consumption, histological type, stage of extension, location of cancer. Bivariate analysis was done between age and location of the tumour. The statistical test used was the KHI2 test. The results were statistically significant for a value of p RESULTS: We collected 400 files from women with cancer. Among the 400 files, 265 were represented by gynecological and breast cancers, that is a frequency of 65.43%. Among the 265 cases of gynecological and breast cancer identified during this study period, breast cancer represented the first rank with 52% frequency. Cervical cancer accounted for 41% followed by ovarian cancer (5%) and endometrial (2%). Cancer of vulva was represented by a single case. The average age of patients with gynecological and breast cancers during our study was 52 ± 12 years. The average age of patients with cancer of the breast, cervix, ovarian, endometrial was respectively 49 ± 11.97 years, 54.73 ± 12.91 years old, 50 ± 14 years, 67 ± 14.24 years, the age of the vulva cancer patient was 51 years old. The histological type of breast cancer was represented by infiltrating ductal adenocarcinoma in 91% of cases, invasive lobular carcinoma in 7% and sarcoma in 2%. Cervical cancers were represented by squamous cell carcinomas in 95% of cases and adenocarcinomas in 5% of cases. Vulva cancer was represented by squamous cell carcinoma, ovarian cancer was represented by epithelial tumors (adenocarcinoma) in 100% of cases. Endometrial cancers were represented by endometrial adenocarcinoma. All cancers were diagnosed at advanced stages (locoregional and metastatic stage). Patients over the age of 50 had more gynecological and breast cancers;but this result was not significant. CONCLUSION: Gynecological and breast cancers are frequent and constitute the first cancer of Congolese women in Pointe-Noire. Breast cancer is the first cancer followed by cancer of the cervix and ovaries. The histological types are those of the literature. Patients with gynecological and breast cancers consult at very advanced stages.展开更多

The effect of Cowl shape on the ramp surface pressure distribution is discussed and compared with existing V-Notch [90°] intake model at free exit condition. It was found that using pointed cowl [90°] intake model, a better pressure recovery was achieved compared to the V-Notch [90°] intake model at Mach 2.2. Both Pointed and V-Notch intake models showed good starting characteristics. For change in back pressure, the occurence of normal shock, flow separation zone and flow reversal were observed. All experiments are performed only for the Pointed cowl [90°] intake model. All the 3-D computations were performed by using software available at B.I.T, Mesra, Ranchi.展开更多

A 32 year-old female patient with a history of opioid abuse and no prior history of cardiovascular disease experienced Torsades de pointes. An electrocardiogram at the time of hospital presentation revealed a prolonged QT interval at 636 msec. Diagnostic evaluation was unremarkable for any underlying cardiovascular pathology. Further investigation revealed the patient had been taking 30-40 loperamide 2 mg tablets oral daily for 2 weeks prior to the onset of Torsades de pointes. At high doses, loperamide may prolong the QT interval through antagonism of the human ether-a-go-go potassium current, thus increasing the risk of ventricular arrhythmias. Clinicians should be cautious with administration of high doses of loperamide to all patients, especially patients at high risk for QT interval prolongation.展开更多

However, the causes of the sex differe...Background Torsade de pointes (TdP) is a form of polymorphic ventricular tachycardia featuring prolonged QT intervals. Female gender is associated with an increased risk of TdP. However, the causes of the sex difference in risk are poorly understood. Recently, transmural dispersion of repolarization (TDR) has been implicated in the genesis of TdP. Consequently, we compared TdP incidence and TDR between male and female rabbit hearts in order to investigate the mechanism of sex difference in TdP risk in rabbits in vitro. Methods By means of monophasic action potential recording techniques, the monophasic action potential of the epicardium, midmyocardium, and endocardium were simultaneously recorded using specially designed plunge-needle electrodes placed across the left ventricular free wall of both female (n=8) and male (n=8) rabbit hearts purfused by the Langendorff method. TdP was induced by bradycardia, d-sotalol, and low-K +, Mg 2+ Tyrode solution. Results TDR measurements in all three myocardial layers of male and female rabbit hearts were (18±2) ms and (21±2) ms, respectively (n=8, P>0.05). After perfusion with d-sotalol, the 90% monophasic action potential duration was prolonged in both male and femlae rabbits. TDR in male and female rabbit hearts increased to (29±2) ms and (61±2) ms, respectively, a difference that is significant. Eight female rabbit hearts had early afterdepolarization and 7 of them developed TdP. Seven male rabbit hearts had early after depolarization, but only one of these hearts developed TdP.Conclusion Greater TDR may play an important role in the higher incidence of TdP in female rabbit hearts.展开更多

The Hard X-ray Modulation Telescope (HXMT) is an X-ray astronomy satellite consisting of three slat-collimated instruments, the High Energy X-ray Instrument (HE), the Medium Energy X-ray Instrument (ME), and the...The Hard X-ray Modulation Telescope (HXMT) is an X-ray astronomy satellite consisting of three slat-collimated instruments, the High Energy X-ray Instrument (HE), the Medium Energy X-ray Instrument (ME), and the Low Energy X-ray Instrument (LE). HXMT will carry out an all sky survey and make pointed observations in the 1-250 keV energy band. In order to get the source and background fluxes simultaneously in the pointed observations, two methods, i.e., the combined field of view (FOV) method and the off-axis pointing method are proposed in this paper. Comprehensive analyses of the sensitivities of the three instruments by using these two methods are presented, respectively. It is found that the off-axis pointing method has a higher sensitivity for HE and ME but a lower sensitivity for LE. Since the axes of the three instruments are aligned along the same direction, the off-axis pointing method is recommended as the main method in the pointed observation for HXMT; the combined FOV method can be used when LE is the most relevant instrument in order to satisfy the scientific objective of the observation.展开更多

文摘BACKGROUND QT interval prolongation can induce torsades de pointes(TdP),a potentially fatal ventricular arrhythmia.Recently,an increasing number of non-cardiac drugs have been found to cause QT prolongation and/or TdP onset.Moreover,recent findings have demonstrated the key roles of systemic inflammatory activation and fever in promoting long-QT syndrome(LQTS)and TdP development.CASE SUMMARY A 30-year-old woman was admitted with a moderate to high-grade episodic fever for two weeks.The patient was administered with multiple antibiotics after hospitalization but still had repeating fever and markedly elevated C-reactive protein.Once after a high fever,the patient suddenly lost consciousness,and electrocardiogram(ECG)showed transient TdP onset after frequent premature ventricular contraction.The patient recovered sinus rhythm and consciousness spontaneously,and post-TdP ECG revealed a prolonged QTc interval of 560 ms.The patient’s clinical manifestations and unresponsiveness to the antibiotics led to the final diagnosis of adult-onset Still’s disease(AOSD).There was no evidence of cardiac involvement.After the AOSD diagnosis,discontinuation of antibiotics and immediate initiation of intravenous dexamethasone administration resulted in the normal temperature and QTc interval.The genetic analysis identified that the patient and her father had heterozygous mutations in KCNH2(c.1370C>T)and AKAP9(c.7725A>C).During the 2-year follow-up period,the patient had no recurrence of any arrhythmia and maintained normal QTc interval.CONCLUSION This case study highlights the risk of systemic inflammatory activation and antibiotic-induced TdP/LQTS onset.Genetic analysis should be considered to identify individuals at high risk of developing TdP.

文摘Introduction: Breast cancer is the leading cancer and the leading cause of cancer death in women worldwide. About 5% to 10% of breast cancer patients present with metastases. While the 5-year survival of patients with local breast cancer varies around 98.8%, this survival rate drops to around 26.3% for metastatic patients. The objective of this study was to determine the survival of patients with metastatic breast cancer in resource-limited settings. Patients and Methods: This was a cross-sectional descriptive study that took place in the Cancer Department of the General Hospital of Loandjili in Pointe Noire during the period from January 1, 2013 to December 31, 2018, for duration of 6 years. 30 records of patients over 18 years of age and with histological evidence who received at least 3 courses of chemotherapy were collected. The variables studied were: age, level of education, socio-economic level, menopausal status, history, WHO status, menopausal status, tumor size, histological type, tumor location, the type of treatment and survival. Survival was calculated by Kaplan Meier method. Fisher’s exact test was used to search for correlation between variables. Results: The average age was 52.62 ± 10.96 years old. The extremes were 33 years and 75 years old. The most represented level of education was the primary level in 67% of cases. The majority of patients had low socioeconomic status in 50% of cases. The patients were menopausal in 57% of cases. The antecedents of cancer were present in 13% of cases. 50% of patients had a WHO status performance at 2. The tumor size was greater than 2 cm in 77% of cases. The most represented histological type was invasive ductal carcinoma in 93% of cases. The most represented histological grade was Scharff grade III Richardson bloom in 80% of cases. The most represented metastatic localization was pulmonary in 33% of cases. The metastatic localizations were unique in 47% of cases and multiple in 53% of cases. Anthracycline-based chemotherapy was more used in 53% of cases. Bivariate analysis revealed a correlation between tumor size and number of metastases, p 0.05. Patients treated with anthracyclines were greater than that of patients treated with taxanes in combination was 26.48 months, p > 0.05. Conclusion: Metastatic breast cancer remains an incurable disease, its survival remains low despite diagnostic and therapeutic advances that remain difficult to access for our resource-poor developing countries. Patients are treated with conventional chemotherapy (anthracyclines and taxanes). The most common metastases are respectively pulmonary, hepatic and cerebral in our context.
文摘INTRODUCTION: The objective of our study was to determine the epidemiological clinical and histological aspects of gynecological and breast cancers in Pointe-Noire. PATIENTS AND METHODS: This was a retrospective descriptive study that took place in the cancerology and internal medicine department during the period from January 1, 2012 to December 31, 2021, i.e. a period of 10 years. Women with histological diagnosis of gynecological and breast cancers were included in our study. The variables studied were: frequency, age, alcohol and tobacco consumption, histological type, stage of extension, location of cancer. Bivariate analysis was done between age and location of the tumour. The statistical test used was the KHI2 test. The results were statistically significant for a value of p RESULTS: We collected 400 files from women with cancer. Among the 400 files, 265 were represented by gynecological and breast cancers, that is a frequency of 65.43%. Among the 265 cases of gynecological and breast cancer identified during this study period, breast cancer represented the first rank with 52% frequency. Cervical cancer accounted for 41% followed by ovarian cancer (5%) and endometrial (2%). Cancer of vulva was represented by a single case. The average age of patients with gynecological and breast cancers during our study was 52 ± 12 years. The average age of patients with cancer of the breast, cervix, ovarian, endometrial was respectively 49 ± 11.97 years, 54.73 ± 12.91 years old, 50 ± 14 years, 67 ± 14.24 years, the age of the vulva cancer patient was 51 years old. The histological type of breast cancer was represented by infiltrating ductal adenocarcinoma in 91% of cases, invasive lobular carcinoma in 7% and sarcoma in 2%. Cervical cancers were represented by squamous cell carcinomas in 95% of cases and adenocarcinomas in 5% of cases. Vulva cancer was represented by squamous cell carcinoma, ovarian cancer was represented by epithelial tumors (adenocarcinoma) in 100% of cases. Endometrial cancers were represented by endometrial adenocarcinoma. All cancers were diagnosed at advanced stages (locoregional and metastatic stage). Patients over the age of 50 had more gynecological and breast cancers;but this result was not significant. CONCLUSION: Gynecological and breast cancers are frequent and constitute the first cancer of Congolese women in Pointe-Noire. Breast cancer is the first cancer followed by cancer of the cervix and ovaries. The histological types are those of the literature. Patients with gynecological and breast cancers consult at very advanced stages.
文摘Computations and Experiments were performed to get an understanding of the flow field around a rectangular supersonic air intake with pointed cowl [90°] at different back pressures for Mach 2.2. The effect of Cowl shape on the ramp surface pressure distribution is discussed and compared with existing V-Notch [90°] intake model at free exit condition. It was found that using pointed cowl [90°] intake model, a better pressure recovery was achieved compared to the V-Notch [90°] intake model at Mach 2.2. Both Pointed and V-Notch intake models showed good starting characteristics. For change in back pressure, the occurence of normal shock, flow separation zone and flow reversal were observed. All experiments are performed only for the Pointed cowl [90°] intake model. All the 3-D computations were performed by using software available at B.I.T, Mesra, Ranchi.
文摘Abstract: Loperamide is a common, over-the-counter, antidiarrheal medication that has been implicated in cases of ventricular tachycardia. A 32 year-old female patient with a history of opioid abuse and no prior history of cardiovascular disease experienced Torsades de pointes. An electrocardiogram at the time of hospital presentation revealed a prolonged QT interval at 636 msec. Diagnostic evaluation was unremarkable for any underlying cardiovascular pathology. Further investigation revealed the patient had been taking 30-40 loperamide 2 mg tablets oral daily for 2 weeks prior to the onset of Torsades de pointes. At high doses, loperamide may prolong the QT interval through antagonism of the human ether-a-go-go potassium current, thus increasing the risk of ventricular arrhythmias. Clinicians should be cautious with administration of high doses of loperamide to all patients, especially patients at high risk for QT interval prolongation.

文摘Background Torsade de pointes (TdP) is a form of polymorphic ventricular tachycardia featuring prolonged QT intervals. Female gender is associated with an increased risk of TdP. However, the causes of the sex difference in risk are poorly understood. Recently, transmural dispersion of repolarization (TDR) has been implicated in the genesis of TdP. Consequently, we compared TdP incidence and TDR between male and female rabbit hearts in order to investigate the mechanism of sex difference in TdP risk in rabbits in vitro. Methods By means of monophasic action potential recording techniques, the monophasic action potential of the epicardium, midmyocardium, and endocardium were simultaneously recorded using specially designed plunge-needle electrodes placed across the left ventricular free wall of both female (n=8) and male (n=8) rabbit hearts purfused by the Langendorff method. TdP was induced by bradycardia, d-sotalol, and low-K +, Mg 2+ Tyrode solution. Results TDR measurements in all three myocardial layers of male and female rabbit hearts were (18±2) ms and (21±2) ms, respectively (n=8, P>0.05). After perfusion with d-sotalol, the 90% monophasic action potential duration was prolonged in both male and femlae rabbits. TDR in male and female rabbit hearts increased to (29±2) ms and (61±2) ms, respectively, a difference that is significant. Eight female rabbit hearts had early afterdepolarization and 7 of them developed TdP. Seven male rabbit hearts had early after depolarization, but only one of these hearts developed TdP.Conclusion Greater TDR may play an important role in the higher incidence of TdP in female rabbit hearts.

文摘The Hard X-ray Modulation Telescope (HXMT) is an X-ray astronomy satellite consisting of three slat-collimated instruments, the High Energy X-ray Instrument (HE), the Medium Energy X-ray Instrument (ME), and the Low Energy X-ray Instrument (LE). HXMT will carry out an all sky survey and make pointed observations in the 1-250 keV energy band. In order to get the source and background fluxes simultaneously in the pointed observations, two methods, i.e., the combined field of view (FOV) method and the off-axis pointing method are proposed in this paper. Comprehensive analyses of the sensitivities of the three instruments by using these two methods are presented, respectively. It is found that the off-axis pointing method has a higher sensitivity for HE and ME but a lower sensitivity for LE. Since the axes of the three instruments are aligned along the same direction, the off-axis pointing method is recommended as the main method in the pointed observation for HXMT; the combined FOV method can be used when LE is the most relevant instrument in order to satisfy the scientific objective of the observation.
文摘In this paper we study the pointed representations of the Virasoro algebra.We show that unitary irreducible pointed representations of the Virasoro algebra are Harish-Chandra repre- sentations,thus they either are of highest or lowest weights or have all weight spaces of dimension 1.Further,we prove that unitary irreducible weight representations of Virasoro superalgebras are either of highest weights or of lowest weights,hence they are also Harish-Chandra representations.

文摘Let T(G) be the Teichmüller space of a Fuchsian group G and T(G) be the pointed Teichmüller space of a corresponding pointed Fuchsian group G.We will discuss the existence of holomorphic sections of the projection from the space M(G) of Beltrami coefficients for G to T(G) and of that from T(G) to T(G) as well.We will also study the biholomorphic isomorphisms between two pointed Teichmüller spaces.
文摘We focus on the classification of pointed p^3-dimensional Hopf algebras H over any algebraically closed field of prime characteristic p 〉 0. In particular, we consider certain cases when the group of grouplike elements is of order p or p^2 that is, when H is pointed but is not connected nor a group algebra. The structures of the associated graded algebra gr H are completely described as bosonizations of graded braided Hopf algebras over group algebras, and most of the lifting structures of H are given. This work provides many new examples of (parametrized) non-commutative, non-cocommutative finite- dimensional Hopf algebras in positive characteristic.
