Objectives:To explore the efficacy and safety of virtual reality(VR)in relieving negative emotions in patients with breast cancer with different personalities.Methods:A randomized controlled trial was conducted.Betwee...Objectives:To explore the efficacy and safety of virtual reality(VR)in relieving negative emotions in patients with breast cancer with different personalities.Methods:A randomized controlled trial was conducted.Between April 2023 and October 2023,we enrolled patients with breast cancer treated in the Department of Breast Cancer and Oncology at Sun Yat-Sen Memorial Hospital,Sun Yat-Sen University,Guangdong Province.The patients were randomly divided into an intervention group(n=118)and a control group(n=119)using block randomization.The intervention group received the VR intervention 3-5 times over 5±2 weeks using natural landscapes with music or relaxation guidance,and the duration of each VR intervention was 15±3 min.The control group received routine nursing care,including disease education and psychological counseling.Patients were assessed using the Type D Scale,Positive and Negative Affect Scale,and Distress Thermometer,and adverse events during the intervention were recorded.Results:Overall,85 patients completed the study(44 in the intervention group and 41 in the control group).Patients with Type D personalities showed more negative emotions[25.0(21.5,27.5)vs.19.0(16.0,24.0),P=0.001]and distressed attitudes[4.0(2.0,5.0)vs.3.0(1.0,4.0),P=0.020]with fewer positive emotions(27.2±5.6 vs.31.0±5.9,P=0.014)than those with non-Type D personalities.Total population analysis revealed no significant differences between the groups.However,in the subgroup analysis,patients with Type D personalities in the intervention group showed greater relief from negative emotions than those in the control group[median difference,-5.0(-9.0,-2.5)vs.-2.0(-4.0,2.0),P=0.046].No significant differences were found between groups of patients with non-Type D personality traits.The proportion of adverse events was not significantly different between groups(P=0.110).Conclusions:Breast cancer patients with Type D personalities suffer more severe negative emotions and distress,and more attention should be paid to them.VR intervention significantly and safely reduced negative emotions in patients with Type D personalities.展开更多

Consistent individual dif ferences in behavior have been found across taxa, and have been shown to influence a range of ecological processes. The role of personalities in sexual selection has been considered, and examples exist that show selection for personality traits with both assortative and disassortative mating patterns between personality types. One overlooked aspect of the personality and sexual se lection literature is the potential for personalitysignaling interactions, specifically with complex signaling. Complex signaling is a diverse topic in itself, and in short, consists of multiple signals within one or more modalities that interact to elicit a receiver response. Research into complex signaling has been thorough, although at times studies discover complex signaling systems that fail to fit into one of the existing hypotheses in the literature. Here, we argue that personalities may interact with complex signal ing, which should be considered by researchers of both personality and sexual selection and communication. We describe several ways in which personalitycomplex signaling interactions could affect both the signaler and receiver, and the way in which they may drive personalityspecific signals as well as receiver preferences. Finally, we discuss how considering personality in com plex signaling studies may inform theory as well as improve the ability of researchers to accurately describe its function.展开更多

These include the Great East Japan Earthquake, and the global financial crisis. Furthermore, Japan experienced the introduction of a specific education system. In this study, we traced and compared the psychological characters of university freshmen for 16 years (years 2000-2016) using Big-Five personalities inventory (Japanese version). Methods: A total of 1189 physically and mentally healthy university freshmen participated in this study. They were divided into four groups according to the year they entered university. They completed a self-filled questionnaire, Big-Five personalities inventory, which consists of Extraversion, Agreeableness, Neuroticism, Conscientiousness and Openness. Result and Discussion: Compared with the year 2008, Extraversion nominally increased in 2012, however, it statistically significantly decreased in 2016. Agreeableness notably decreased from 2000 to 2008, then showed an increase in 2012. Compared to 2000, Neuroticism was statistically significantly lower in 2008 and 2016. Such statistically significant differences were not observed in the other two personalities. The personalities of university freshmen have changed in the last 16 years, which may be been caused by drastic changes. However, further research (e.g., larger-scale studies or studies including other university students) is required to support the current findings.展开更多

We found the relationship between hippocampal responses to emotional stimuli and the mental stabilities of people in our preliminary study. In this study, we have also evaluated how the emotional stimuli would affect amygdala and thalamus in the brain, and how the personality stabilities could relate to the responses in the brain using functional magnetic resonance imaging (fMRI). We evaluated the subjects’ personality features with the Yatabe-Guilford Personality Test (Y-G test) and psychosomatic symptoms with the Cornell Medical Index (CMI). The subjects were categorized into the mentally stable group and the mentally unstable group according to the total scores of the Y-G test and the CMI. The brain functional responses under emotional stimuli were measured using fMRI. The region of interest (ROI) analysis was performed to abstract significant changes in order to compare responses among the different emotional stimuli. We conducted the regression analysis to abstract the relationship between the mean % signal change from fMRI and the personality stability. The fMRI results showed that the hippocampus, thalamus, and right amygdala activities under the human relationship stimuli increased with ascending value of mental instability. Our findings suggest that the memory process in the hippocampus and the threat alarm system in the thalamus under the human-related stimuli crucially influence the emotional reaction of mentally unstable people. These processes in the brain would affect the event that stresses on human relationships that often cause people to suffer from mental disorders.展开更多

Furth...The aim of this population study was to identify personality traits among older (>65 years) male and female owners of cats and dogs and to compare their general health status in relation to their personality. Further, the aim was to examine whether current cat and dog ownership could be predicted by the owners’ personality and health. Data were collected from the North-Trøndelag Health Study (HUNT) in Norway. Included were a total of 1897 cat or dog owners between the ages of 65 years and 101 years. The results showed that there were a higher proportion of introverted male cat owners than extraverted ones. Moreover, a majority of women with cats reported that their health was not good. Furthermore, female cat owners who displayed higher scores on neurotic traits experienced significantly poorer health compared to those female cat owners that experienced good health. The same was true for female cat owners who considered themselves to be introverted. Neither personality nor health could predict pet-ownership, but it was more likely for older individuals (80-101 years) to own a cat than a dog. This study has shown that human personality is associated with cat and dog ownership, but there are other factors connected with pet ownership as well.展开更多
Blood-brain barrier disruption and the neuroinflammatory response are significant pathological features that critically influence disease progression and treatment outcomes.This review systematically analyzes the curr...Blood-brain barrier disruption and the neuroinflammatory response are significant pathological features that critically influence disease progression and treatment outcomes.This review systematically analyzes the current understanding of the bidirectional relationship between blood-brain barrier disruption and neuroinflammation in traumatic brain injury,along with emerging combination therapeutic strategies.Literature review indicates that blood-brain barrier disruption and neuroinflammatory responses are key pathological features following traumatic brain injury.In the acute phase after traumatic brain injury,the pathological characteristics include primary blood-brain barrier disruption and the activation of inflammatory cascades.In the subacute phase,the pathological features are characterized by repair mechanisms and inflammatory modulation.In the chronic phase,the pathological features show persistent low-grade inflammation and incomplete recovery of the blood-brain barrier.Various physiological changes,such as structural alterations of the blood-brain barrier,inflammatory cascades,and extracellular matrix remodeling,interact with each other and are influenced by genetic,age,sex,and environmental factors.The dynamic balance between blood-brain barrier permeability and neuroinflammation is regulated by hormones,particularly sex hormones and stress-related hormones.Additionally,the role of gastrointestinal hormones is receiving increasing attention.Current treatment strategies for traumatic brain injury include various methods such as conventional drug combinations,multimodality neuromonitoring,hyperbaric oxygen therapy,and non-invasive brain stimulation.Artificial intelligence also shows potential in treatment decision-making and personalized therapy.Emerging sequential combination strategies and precision medicine approaches can help improve treatment outcomes;however,challenges remain,such as inadequate research on the mechanisms of the chronic phase traumatic brain injury and difficulties with technology integration.Future research on traumatic brain injury should focus on personalized treatment strategies,the standardization of techniques,costeffectiveness evaluations,and addressing the needs of patients with comorbidities.A multidisciplinary approach should be used to enhance treatment and improve patient outcomes.展开更多
BACKGROUND:Hemiplegia,a prevalent stroke-related condition,is often studied for motor dysfunction;however,spasticity remains under-researched.Abnormal muscle tone significantly hinders hemiplegic patients’walking rec...BACKGROUND:Hemiplegia,a prevalent stroke-related condition,is often studied for motor dysfunction;however,spasticity remains under-researched.Abnormal muscle tone significantly hinders hemiplegic patients’walking recovery.OBJECTIVE:To determine whether early suspension-protected training with a personal assistant machine for stroke patients enhances walking ability and prevents muscle spasms.METHODS:Thirty-two early-stage stroke patients from Shenzhen University General Hospital and the China Rehabilitation Research Center were randomly assigned to the experimental group(n=16)and the control group(n=16).Both groups underwent 4 weeks of gait training under the suspension protection system for 30 minutes daily,5 days a week.The experimental group used the personal assistant machine during training.Three-dimensional gait analysis(using the Cortex motion capture system),Brunnstrom staging,Fugl-Meyer Assessment for lower limb motor function,Fugl-Meyer balance function,and the modified Ashworth Scale were evaluated within 1 week before the intervention and after 4 weeks of intervention.RESULTS AND CONCLUSION:After the 4-week intervention,all outcome measures showed significant changes in each group.The experimental group had a small but significant increase in the modified Ashworth Scale score(P<0.05,d=|0.15|),while the control group had a large significant increase(P<0.05,d=|1.48|).The experimental group demonstrated greater improvements in walking speed(16.5 to 38.44 cm/s,P<0.05,d=|4.01|),step frequency(46.44 to 64.94 steps/min,P<0.05,d=|2.32|),stride length(15.50 to 29.81 cm,P<0.05,d=|3.44|),and peak hip and knee flexion(d=|1.82|to|2.17|).After treatment,the experimental group showed significantly greater improvements than the control group in walking speed(38.44 vs.26.63 cm/s,P<0.05,d=|2.75|),stride length,peak hip and knee flexion(d=|1.31|to|1.45|),step frequency(64.94 vs.59.38 steps/min,P<0.05,d=|0.85|),and a reduced support phase(bilateral:24.31%vs.28.38%,P<0.05,d=|0.88|;non-paretic:66.19%vs.70.13%,P<0.05,d=|0.94|).For early hemiplegia,personal assistant machine-assisted gait training under the suspension protection system helps establish a correct gait pattern,prevents muscle spasms,and improves motor function.展开更多
BACKGROUND Preoperative anxiety is a significant concern for patients,as it affects surgical outcomes,satisfaction,and pain perception.Although both anxiety and pain are common in surgical settings,their relationship ...BACKGROUND Preoperative anxiety is a significant concern for patients,as it affects surgical outcomes,satisfaction,and pain perception.Although both anxiety and pain are common in surgical settings,their relationship with personality traits has not been previously investigated in the Lebanese population.AIM To examine the prevalence of preoperative anxiety,pain perception,and personality traits among Lebanese surgical patients,and to assess the associations between these factors.METHODS A descriptive cross-sectional study was conducted between April 2024 and January 2025 across Lebanese hospitals.A total of 392 adult patients were recruited through convenience sampling.Data were collected using a questionnaire that included sociodemographic,clinical,and surgical variables,the Amsterdam Preoperative Anxiety and Information Scale for anxiety,the Visual Analog Scale and Numerical Pain Rating Scale for preoperative pain,and the Ten-Item Personality Inventory for personality traits.Ethical approval was obtained from the Institutional Review Boards of Makassed General Hospital and Hammoud University Medical Center.RESULTS Overall,25%of participants experienced preoperative anxiety,and 34.5%reported moderate pain.Personality assessment showed that the majority of participants had moderate extraversion(84.1%),moderate emotional stability(65.1%),high conscientiousness(61%),high agreeableness(54.1%),and moderate openness(49.2%).High conscientiousness was significantly associated with higher pain perception(P<0.05),while high emotional stability was associated with lower levels of anxiety(P<0.05).No significant association was found between preoperative anxiety and pain(P>0.05).CONCLUSION This study challenges the assumption that preoperative anxiety and pain are directly correlated and highlights the role of personality traits in shaping patient experience.These findings support the potential value of integrating psychological profiling into preoperative care and lay the groundwork for developing personalized interventions to improve patient-centered surgical outcomes.展开更多
Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer fro...Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer from poor bioavailability and stability.Nanoformulations address these limitations by improving solubility,targeted delivery,and controlled release.This approach opens new possibilities for treating chronic diseases like cancer,diabetes,and neurodegenerative disorders.This review aims to examine recent advancements in nanotechnology-based formulation strategies designed to enhance the delivery,stability,and therapeutic efficacy of phytochemicals and also discusses regulatory issues,safety concerns,scalability,and cost-effectiveness.Emphasis was placed on nanoformulation techniques employed for key phytoconstituents such as curcumin,resveratrol,epigallocatechin gallate,and quercetin.The most commonly employed nanocarriers included polymeric nanoparticles,solid lipid nanoparticles,and liposomes.These formulations significantly improved the solubility,stability,and controlled release profiles of phytochemicals.In vitro and in vivo studies demonstrated enhanced anti-inflammatory,anticancer,and antioxidant activities.Moreover,surface-modified and targeted nanoparticles showed promise in increasing site-specific targeting and enhancing bioavailability of the encapsulated compounds.Nanoformulations present a promising strategy for overcoming the pharmacokinetic limitations of phytochemicals.Despite encouraging preclinical results,further studies are needed to address issues related to long-term safety,clinical efficacy,and regulatory approval for successful clinical translation.展开更多

文摘Objectives:To explore the efficacy and safety of virtual reality(VR)in relieving negative emotions in patients with breast cancer with different personalities.Methods:A randomized controlled trial was conducted.Between April 2023 and October 2023,we enrolled patients with breast cancer treated in the Department of Breast Cancer and Oncology at Sun Yat-Sen Memorial Hospital,Sun Yat-Sen University,Guangdong Province.The patients were randomly divided into an intervention group(n=118)and a control group(n=119)using block randomization.The intervention group received the VR intervention 3-5 times over 5±2 weeks using natural landscapes with music or relaxation guidance,and the duration of each VR intervention was 15±3 min.The control group received routine nursing care,including disease education and psychological counseling.Patients were assessed using the Type D Scale,Positive and Negative Affect Scale,and Distress Thermometer,and adverse events during the intervention were recorded.Results:Overall,85 patients completed the study(44 in the intervention group and 41 in the control group).Patients with Type D personalities showed more negative emotions[25.0(21.5,27.5)vs.19.0(16.0,24.0),P=0.001]and distressed attitudes[4.0(2.0,5.0)vs.3.0(1.0,4.0),P=0.020]with fewer positive emotions(27.2±5.6 vs.31.0±5.9,P=0.014)than those with non-Type D personalities.Total population analysis revealed no significant differences between the groups.However,in the subgroup analysis,patients with Type D personalities in the intervention group showed greater relief from negative emotions than those in the control group[median difference,-5.0(-9.0,-2.5)vs.-2.0(-4.0,2.0),P=0.046].No significant differences were found between groups of patients with non-Type D personality traits.The proportion of adverse events was not significantly different between groups(P=0.110).Conclusions:Breast cancer patients with Type D personalities suffer more severe negative emotions and distress,and more attention should be paid to them.VR intervention significantly and safely reduced negative emotions in patients with Type D personalities.
文摘Animal personalities have been a major focus of behavioral ecology over the past decade. Consistent individual dif ferences in behavior have been found across taxa, and have been shown to influence a range of ecological processes. The role of personalities in sexual selection has been considered, and examples exist that show selection for personality traits with both assortative and disassortative mating patterns between personality types. One overlooked aspect of the personality and sexual se lection literature is the potential for personalitysignaling interactions, specifically with complex signaling. Complex signaling is a diverse topic in itself, and in short, consists of multiple signals within one or more modalities that interact to elicit a receiver response. Research into complex signaling has been thorough, although at times studies discover complex signaling systems that fail to fit into one of the existing hypotheses in the literature. Here, we argue that personalities may interact with complex signal ing, which should be considered by researchers of both personality and sexual selection and communication. We describe several ways in which personalitycomplex signaling interactions could affect both the signaler and receiver, and the way in which they may drive personalityspecific signals as well as receiver preferences. Finally, we discuss how considering personality in com plex signaling studies may inform theory as well as improve the ability of researchers to accurately describe its function.
文摘Introduction: With the arrival of the 21st century, drastic worldwide changes to natural, economic and social events have occurred. These include the Great East Japan Earthquake, and the global financial crisis. Furthermore, Japan experienced the introduction of a specific education system. In this study, we traced and compared the psychological characters of university freshmen for 16 years (years 2000-2016) using Big-Five personalities inventory (Japanese version). Methods: A total of 1189 physically and mentally healthy university freshmen participated in this study. They were divided into four groups according to the year they entered university. They completed a self-filled questionnaire, Big-Five personalities inventory, which consists of Extraversion, Agreeableness, Neuroticism, Conscientiousness and Openness. Result and Discussion: Compared with the year 2008, Extraversion nominally increased in 2012, however, it statistically significantly decreased in 2016. Agreeableness notably decreased from 2000 to 2008, then showed an increase in 2012. Compared to 2000, Neuroticism was statistically significantly lower in 2008 and 2016. Such statistically significant differences were not observed in the other two personalities. The personalities of university freshmen have changed in the last 16 years, which may be been caused by drastic changes. However, further research (e.g., larger-scale studies or studies including other university students) is required to support the current findings.

文摘In the past decade there has been a profusion of studies highlighting covariation between individual differences in stress physiology and behavioural profiles, here called personalities. Such individual differences in ways of coping with stress are relevant both in biomedicine, since different personalities may experience a different stress and disease vulnerability, and in behavioural ecology, since their adaptive value and evolutionary maintenance are the subject of debate. However, the precise way in which individual stress differences and personalities are linked is unclear. Here we provide an updated overview of this covariation across different species and taxa, consider its functional significance and present working hypotheses for how behavioural and physiological responses to stress might be causally linked, affecting life-history traits such as dispersal and life-span [Current Zoology 56 (6): 728-740, 2010].

文摘Hippocampus is crucial for the formation of emotional memory. We found the relationship between hippocampal responses to emotional stimuli and the mental stabilities of people in our preliminary study. In this study, we have also evaluated how the emotional stimuli would affect amygdala and thalamus in the brain, and how the personality stabilities could relate to the responses in the brain using functional magnetic resonance imaging (fMRI). We evaluated the subjects’ personality features with the Yatabe-Guilford Personality Test (Y-G test) and psychosomatic symptoms with the Cornell Medical Index (CMI). The subjects were categorized into the mentally stable group and the mentally unstable group according to the total scores of the Y-G test and the CMI. The brain functional responses under emotional stimuli were measured using fMRI. The region of interest (ROI) analysis was performed to abstract significant changes in order to compare responses among the different emotional stimuli. We conducted the regression analysis to abstract the relationship between the mean % signal change from fMRI and the personality stability. The fMRI results showed that the hippocampus, thalamus, and right amygdala activities under the human relationship stimuli increased with ascending value of mental instability. Our findings suggest that the memory process in the hippocampus and the threat alarm system in the thalamus under the human-related stimuli crucially influence the emotional reaction of mentally unstable people. These processes in the brain would affect the event that stresses on human relationships that often cause people to suffer from mental disorders.

文摘The aim of this population study was to identify personality traits among older (>65 years) male and female owners of cats and dogs and to compare their general health status in relation to their personality. Further, the aim was to examine whether current cat and dog ownership could be predicted by the owners’ personality and health. Data were collected from the North-Trøndelag Health Study (HUNT) in Norway. Included were a total of 1897 cat or dog owners between the ages of 65 years and 101 years. The results showed that there were a higher proportion of introverted male cat owners than extraverted ones. Moreover, a majority of women with cats reported that their health was not good. Furthermore, female cat owners who displayed higher scores on neurotic traits experienced significantly poorer health compared to those female cat owners that experienced good health. The same was true for female cat owners who considered themselves to be introverted. Neither personality nor health could predict pet-ownership, but it was more likely for older individuals (80-101 years) to own a cat than a dog. This study has shown that human personality is associated with cat and dog ownership, but there are other factors connected with pet ownership as well.

文摘Blood-brain barrier disruption and the neuroinflammatory response are significant pathological features that critically influence disease progression and treatment outcomes.This review systematically analyzes the current understanding of the bidirectional relationship between blood-brain barrier disruption and neuroinflammation in traumatic brain injury,along with emerging combination therapeutic strategies.Literature review indicates that blood-brain barrier disruption and neuroinflammatory responses are key pathological features following traumatic brain injury.In the acute phase after traumatic brain injury,the pathological characteristics include primary blood-brain barrier disruption and the activation of inflammatory cascades.In the subacute phase,the pathological features are characterized by repair mechanisms and inflammatory modulation.In the chronic phase,the pathological features show persistent low-grade inflammation and incomplete recovery of the blood-brain barrier.Various physiological changes,such as structural alterations of the blood-brain barrier,inflammatory cascades,and extracellular matrix remodeling,interact with each other and are influenced by genetic,age,sex,and environmental factors.The dynamic balance between blood-brain barrier permeability and neuroinflammation is regulated by hormones,particularly sex hormones and stress-related hormones.Additionally,the role of gastrointestinal hormones is receiving increasing attention.Current treatment strategies for traumatic brain injury include various methods such as conventional drug combinations,multimodality neuromonitoring,hyperbaric oxygen therapy,and non-invasive brain stimulation.Artificial intelligence also shows potential in treatment decision-making and personalized therapy.Emerging sequential combination strategies and precision medicine approaches can help improve treatment outcomes;however,challenges remain,such as inadequate research on the mechanisms of the chronic phase traumatic brain injury and difficulties with technology integration.Future research on traumatic brain injury should focus on personalized treatment strategies,the standardization of techniques,costeffectiveness evaluations,and addressing the needs of patients with comorbidities.A multidisciplinary approach should be used to enhance treatment and improve patient outcomes.
文摘BACKGROUND:Hemiplegia,a prevalent stroke-related condition,is often studied for motor dysfunction;however,spasticity remains under-researched.Abnormal muscle tone significantly hinders hemiplegic patients’walking recovery.OBJECTIVE:To determine whether early suspension-protected training with a personal assistant machine for stroke patients enhances walking ability and prevents muscle spasms.METHODS:Thirty-two early-stage stroke patients from Shenzhen University General Hospital and the China Rehabilitation Research Center were randomly assigned to the experimental group(n=16)and the control group(n=16).Both groups underwent 4 weeks of gait training under the suspension protection system for 30 minutes daily,5 days a week.The experimental group used the personal assistant machine during training.Three-dimensional gait analysis(using the Cortex motion capture system),Brunnstrom staging,Fugl-Meyer Assessment for lower limb motor function,Fugl-Meyer balance function,and the modified Ashworth Scale were evaluated within 1 week before the intervention and after 4 weeks of intervention.RESULTS AND CONCLUSION:After the 4-week intervention,all outcome measures showed significant changes in each group.The experimental group had a small but significant increase in the modified Ashworth Scale score(P<0.05,d=|0.15|),while the control group had a large significant increase(P<0.05,d=|1.48|).The experimental group demonstrated greater improvements in walking speed(16.5 to 38.44 cm/s,P<0.05,d=|4.01|),step frequency(46.44 to 64.94 steps/min,P<0.05,d=|2.32|),stride length(15.50 to 29.81 cm,P<0.05,d=|3.44|),and peak hip and knee flexion(d=|1.82|to|2.17|).After treatment,the experimental group showed significantly greater improvements than the control group in walking speed(38.44 vs.26.63 cm/s,P<0.05,d=|2.75|),stride length,peak hip and knee flexion(d=|1.31|to|1.45|),step frequency(64.94 vs.59.38 steps/min,P<0.05,d=|0.85|),and a reduced support phase(bilateral:24.31%vs.28.38%,P<0.05,d=|0.88|;non-paretic:66.19%vs.70.13%,P<0.05,d=|0.94|).For early hemiplegia,personal assistant machine-assisted gait training under the suspension protection system helps establish a correct gait pattern,prevents muscle spasms,and improves motor function.
文摘BACKGROUND Preoperative anxiety is a significant concern for patients,as it affects surgical outcomes,satisfaction,and pain perception.Although both anxiety and pain are common in surgical settings,their relationship with personality traits has not been previously investigated in the Lebanese population.AIM To examine the prevalence of preoperative anxiety,pain perception,and personality traits among Lebanese surgical patients,and to assess the associations between these factors.METHODS A descriptive cross-sectional study was conducted between April 2024 and January 2025 across Lebanese hospitals.A total of 392 adult patients were recruited through convenience sampling.Data were collected using a questionnaire that included sociodemographic,clinical,and surgical variables,the Amsterdam Preoperative Anxiety and Information Scale for anxiety,the Visual Analog Scale and Numerical Pain Rating Scale for preoperative pain,and the Ten-Item Personality Inventory for personality traits.Ethical approval was obtained from the Institutional Review Boards of Makassed General Hospital and Hammoud University Medical Center.RESULTS Overall,25%of participants experienced preoperative anxiety,and 34.5%reported moderate pain.Personality assessment showed that the majority of participants had moderate extraversion(84.1%),moderate emotional stability(65.1%),high conscientiousness(61%),high agreeableness(54.1%),and moderate openness(49.2%).High conscientiousness was significantly associated with higher pain perception(P<0.05),while high emotional stability was associated with lower levels of anxiety(P<0.05).No significant association was found between preoperative anxiety and pain(P>0.05).CONCLUSION This study challenges the assumption that preoperative anxiety and pain are directly correlated and highlights the role of personality traits in shaping patient experience.These findings support the potential value of integrating psychological profiling into preoperative care and lay the groundwork for developing personalized interventions to improve patient-centered surgical outcomes.
文摘Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer from poor bioavailability and stability.Nanoformulations address these limitations by improving solubility,targeted delivery,and controlled release.This approach opens new possibilities for treating chronic diseases like cancer,diabetes,and neurodegenerative disorders.This review aims to examine recent advancements in nanotechnology-based formulation strategies designed to enhance the delivery,stability,and therapeutic efficacy of phytochemicals and also discusses regulatory issues,safety concerns,scalability,and cost-effectiveness.Emphasis was placed on nanoformulation techniques employed for key phytoconstituents such as curcumin,resveratrol,epigallocatechin gallate,and quercetin.The most commonly employed nanocarriers included polymeric nanoparticles,solid lipid nanoparticles,and liposomes.These formulations significantly improved the solubility,stability,and controlled release profiles of phytochemicals.In vitro and in vivo studies demonstrated enhanced anti-inflammatory,anticancer,and antioxidant activities.Moreover,surface-modified and targeted nanoparticles showed promise in increasing site-specific targeting and enhancing bioavailability of the encapsulated compounds.Nanoformulations present a promising strategy for overcoming the pharmacokinetic limitations of phytochemicals.Despite encouraging preclinical results,further studies are needed to address issues related to long-term safety,clinical efficacy,and regulatory approval for successful clinical translation.

文摘Male breast cancer(MBC)is rare,representing 0.5%–1%of all breast cancers,but its incidence is increasing due to improved diagnostics and awareness.MBC typically presents in older men,is human epidermal growth factor receptor 2(HER2)-negative and estrogen receptor(ER)-positive,and lacks routine screening,leading to delayed diagnosis and advanced disease.Major risk factors include hormonal imbalance,radiation exposure,obesity,alcohol use,and Breast Cancer Gene 1 and 2(BRCA1/2)mutations.Clinically,it may resemble gynecomastia but usually appears as a unilateral,painless mass or nipple discharge.Advances in imaging and liquid biopsy have enhanced early detection.Molecular mechanisms involve hormonal signaling,HER2/epidermal growth factor receptor(EGFR)pathways,tumor suppressor gene alterations,and epigenetic changes.While standard treatments mirror those for female breast cancer,emerging options such as cyclin-dependent kinase 4 and 6(CDK4/6),and poly(ADP-ribose)polymerase(PARP)inhibitors,immunotherapy,and precision medicine are reshaping management.Incorporating artificial intelligence,molecular profiling,and male-specific clinical trials is essential to improve outcomes and bridge current diagnostic and therapeutic gaps.
文摘Therapy discontinuation in inflammatory bowel disease,particularly involving immunomodulators,biologics,and small molecules,remains a controversial and evolving topic.This letter reflects on developments following the publication by Meštrovićet al,emphasizing the complex balance between risks of relapse,antidrug antibody formation,and potential complications of long-term immunosuppression.Recent evidence underscores high relapse rates following withdrawal-especially of anti-tumor necrosis factor agents-and highlights the lack of robust data for newer biologics.Updated guidelines from European Crohn’s and Colitis Organization,British Society of Gastroenterology,and American College of Gastroenterology all support cautious and individualized approaches,with strict criteria and close follow-up,particularly in Crohn’s disease.For ulcerative colitis,therapeutic cycling remains insufficiently addressed.We proposed a flowchart to support clinical decision-making and stress the importance of shared decisionmaking in the era of personalized medicine since,despite new drug classes and evolving strategies,the therapeutic ceiling in inflammatory bowel disease has yet to be fully overcome.
