Incidence of gastro-esophageal reflux disease in mechanically ventilated full-term Egyptian neonates by detection of pepsin in endotracheal aspirate

BACKGROUND Gastroesophageal reflux disease(GERD)is common among neonates,particularly those requiring mechanical ventilation.Pepsin,a reliable marker of gastric aspi-ration,may help detect GER episodes in ventilated neonates and assess associated clinical outcomes.AIM To determine the incidence of GERD,associated risk factors,and morbidities among full-term mechanically ventilated neonates by detecting pepsin in endo-tracheal aspirates(ETA).METHODS This study included 97 full-term neonates admitted to the neonatal intensive care unit at Cairo University Hospitals from April 2023 to March 2024.ETA samples were collected at three intervals:Immediately post-intubation(Sample A),48 hours after intubation(Sample B),and just before extubation(Sample C).Pepsin concentration was measured using enzyme-linked immunosorbent assay.Clinical data,including hospital stay duration and feeding parameters,were correlated with pepsin levels.RESULTS Pepsin was detected in 76(78.4%)of Sample A,78(81.3%)of Sample B,and 47(68.1%)of Sample C.A significant positive correlation was found between pepsin levels and FiO_(2) in Sample B(r=0.203,P=0.047).Prolonged hospital stay was also associated with pepsin detection in Samples B and C(P<0.05).A negative correlation was observed between feeding amount and pepsin levels across all samples(P<0.05).CONCLUSION The incidence of GERD in full-term mechanically ventilated neonates is high,correlating with pepsin levels,FiO_(2),feeding intolerance,and hospital stay,highlighting the importance of early detection. 展开更多

Phlorizin(PHL)is a natural compound with strong antioxidant properties mainly found in apples.In this paper,the interaction mechanism of PHL with pepsin and trypsin was comparatively evaluated by computer simulation,fluorescence spectra,circular dichroism(CD),and Fourier transform infrared(FT-IR)spectra at a molecular level.Fluorescence spectra showed that PHL quenches the pepsin/trypsin by static quenching.Thermodynamic parameters indicated that PHL binds to pepsin mainly through hydrogen bonds and van der Waals forces,and that of trypsin was electrostatic forces.The ground state complexes PHL and protease have a moderate affinity of 105 L/mol PHL binds more strongly to trypsin than to pepsin.CD and FT-IR spectra results showed that pepsin/trypsin decreased theβ-sheet content and slightly changed its secondary structure upon PHL.These experimental results are mutually verified with the predicted computer-aid simulation results.Upon PHL and trypsin binding,the antioxidant capacity of PHL was elevated.Nevertheless,the antioxidant capacity of PHL was decreased after binding to pepsin.This work elucidates the binding of PHL binding mechanisms to pepsin/trypsin and provides useful information for the digestion of PHL to improve the application of PHL in food processing. 展开更多

Hispidin is a pyranone compound found in edible and medicinal mushrooms of the Phellinus and Inonotus genera.This investigation used fluorescence spectroscopy,UV absorption spectroscopy,and molecular docking to examine the interaction of hispidin with pepsin.The Stern-Volmer method was used to perform the fluorescence quenching measurements at different temperatures(298 K,303 K,and 310 K).According to the findings,hispidin induced a static quenching mechanism in pepsin that resulted in the creation of a hispidin-pepsin complex with binding constants(Ka)ranging from 9.56×10^(4) to 3.45×10^(5) L mol^(-1).The positive values ofΔH(84.6 kJ mol-1)andΔS(337.9 J mol^(-1) K^(-1))demonstrated that hydrophobic forces contributed to forming the hispidin-pepsin complex.The findings of UV-vis absorption,synchronous fluorescence,and 3D fluorescence spectraspectra demonstrated that hispidin altered the conformation and microenvironment of pepsin.According to the analysis of molecular docking,hispidin got into the pepsin's active cavity.The research clarifies the molecular mechanisms by which hispidin binds to pepsin and helps understand its possible biological activity in vivo. 展开更多

By using the wastes fish skin of sturgeon processed as a raw material, a macromolecule biomaterial of collagen was extracted. Acid-soluble collagen(ASC) and pepsin-soluble collagen(PSC) were successfully isolated from the skin of hybrid sturgeon with two extraction methods. The yields of ASC and PSC based on the wet weight of skin were 5.73 ± 0.11% and 10.26 ± 0.39%, respectively. The denaturation and melting points of ASC(26.83 ℃ and 110.49 ℃) and PSC(26.54 ℃ and 102.99 ℃) were assessed by Circular dichroism(CD) and Differential scanning calorimetry(DSC). ASC and PSC appeared to be dense sheet-like film linked by random-coiled filaments under scanning electron microscopy(SEM). Sodium dodecyl sulfate-polyacrylamide gel electrophoresis(SDS-PAGE) and Fourier transform infrared spectroscopy(FTIR) confirmed that both the ASC and PSC were Type I collagen and maintained a complete triple helix structure. These results indicated that both ASC and PSC possessed good biological activity and could be widely used in medical biomaterials and other fields. 展开更多

The results were as follows: the highest level of trypsin activity is in the foregut in all measured tissues; from foregut to hindgut, trypsin activities decrease; the pH optimum of trypsin activity is pH9.0; the strongest pepsin activity is in the stomach; the proper density of haemoglobin for detecting pepsin activity is 1.0%. These data are useful in solving applied nutritional problems, such as the adequacy of artificial food to the digestive abilities of the fish. 展开更多

To make more effective use of underutilized resources, pepsin-solubilized collagen (PSC) was successfully extracted from the skin of black carp (Mylopharyngdon piceus) with a yield of 45.7% based on dry weight. The PSC comprising two identical α1-chains and one α2-chain with no disulfide bond was characterized as type I, and it contained 195 imino acid residues vs. 1000 amino acid residues. The collagen showed an absorption edge around 218 nm, which was lower than the maximum absorption wavelength of other PSC. The denaturation temperature of PSC was 25.6°C, which was lower than that of porcine collagen by approximately 11°C. The isoelectric point (pI) was estimated to be 8.23, and the collagen was soluble at an acidic pH as well as below 40 g/L NaCl. It is thought that the high yield and stability of PSC from the skin of black carp warrant its application as a new source of collagen for industrial purposes. 展开更多

Characteristics and antioxidant activities of pepsin-soluble collagen (PSC) from yellow goosefish (Lophius litulon) skins were investigated. PSC was characterized as a type I collagen, and its imino acid content was 193 residues/1 000 residues. PSC's denaturation temperature was -17.56℃ and Fourier transform infrared spectra confirmed the presence of triple helices. Solubility analysis showed good solubility at acidic pH (1-6) or low NaCl concentrations (≤2%). PSC showed scavenging activity against hydroxyl radicals and superoxide anions in a concentration-dependent manner. Furthermore, PSC could protect D-galactose-induced skin aging by significantly controlling malondialdehyde formation and improving the activity of superoxide dismutase, glutathione peroxidase, catalase, glutathione, and hydroxyproline. PSC may be a promising antioxidant in appropriate applications. 展开更多

The aim of this study was to compare chemical and biochemical properties of crude jellyfish protein extracted from salted sand jellyfish (Rhopilema hispidum) and white jellyfish (Lobonema smithii). Sodium dodecyl sulfate polyacylamide gel electrophoresis (SDS-PAGE) showed that the principal proteins of collagen standard types I and II were found among the protein constituents of desalted jellyfish collagen prepared in this study. The amino acid compositions of extracted collagen were typical collagen. The denaturation temperature (Td) of extracted collagen samples was 28-29 ℃. The solubility of all extracted collagens was found up to 4% NaCl. The results indicated that the commercial salting process could partially denature native jellyfish protein functionalities. However, the collagen could be extracted from desalted jellyfish for future use as a food ingredient. 展开更多

The adsorption state and catalytic properties of pepsin and acidic protease from microorganisms Asp. awamori and Asp. oryzae were studied in solid phase system (in presence of sorsilen, DEAE- and CM-cellulose). According to the results, adsorption capacity and catalytic activity of enzymes depend on the physical nature of surface groups of the solid phase. Changing the stability of enzymes in the system with solid phase is observed even the adsorption bond is less stable (in the case of DEAE- and CM-cellulose in acidic media). Injection to the medium ethanol, surfactants, sodium chloride and changing the temperature of the incubation medium could prevent the negative effects of the solid phases. When sorsilen is used as solid phase, pepsin and acidic protease from Asp. awamori suffer from high surface inactivation. Various surfactants influence adsorption state of enzymes differently. Non-ionic surfactants (Triton X-100) prevent adsorption and restore catalytic properties of enzymes. 展开更多

Acid-soluble collagen(ASC) and pepsin-soluble collagen(PSC) from the spine(ASC-SP and PSC-SP) and skull(ASC-SK and PSC-SK) of the skipjack tuna, Katsuwonus pelamis, were successfully isolated and characterized. The yields of ASC-SP, PSC-SP, ASC-SK and PSC-SK were(2.47 ± 0.39)%,(5.62 ± 0.82)%,(3.57 ± 0.40)%, and(6.71 ± 0.81)%, respectively, on the basis of dry weight. The four collagens contained Gly(330.2-339.1 residues/1 000 residues) as the major amino acid, and their imino acid contents were between 168.8 and 178.2 residues/1 000 residues. Amino acid composition, SDS-PAGE, and FTIR investigations confirmed that ASC-SP and ASC-SK were mainly composed of type I collagen, and had higher contents of high-molecular weight cross-links than those of PSC-SK and PSC-SP. The FTIR investigation also certified all the collagens had triple helical structure. The denaturation temperatures of ASC-SK, PSC-SK, ASC-SP, and PSC-SP were 17.8, 16.6, 17.6, and 16.5 °C, respectively. All isolated collagens were soluble at acidic pH(1-5) and lost their solubilities when the NaCl concentration was above 2%(W/V). The isolated collagens from the spines and skulls of skipjack tuna could serve as an alternative source of collagens for further application in food, cosmetic, biomedical, and pharmaceutical industries. 展开更多

目的探究益生菌联合含铋剂四联疗法对小儿幽门螺杆菌(helicobacter pylori,Hp)感染相关性胃炎肠道微生态、免疫和胃蛋白酶的影响。方法选取自贡市第一人民医院2021年5月至2023年5月收治的104例小儿Hp感染相关性胃炎患者,采用Doll随机分... 目的探究益生菌联合含铋剂四联疗法对小儿幽门螺杆菌(helicobacter pylori,Hp)感染相关性胃炎肠道微生态、免疫和胃蛋白酶的影响。方法选取自贡市第一人民医院2021年5月至2023年5月收治的104例小儿Hp感染相关性胃炎患者,采用Doll随机分组法将其分为对照组(n=52)和试验组(n=52),对照组采用奥美拉唑+阿莫西林+胶体果胶铋胶囊+甲硝唑治疗,试验组在此基础上联合益生菌治疗。观察治疗后两组症状缓解程度,Hp根除率及治疗后6个月Hp复发率,比较治疗前后胃炎肠道菌落对数、免疫功能水平、胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ以及血清胃泌素释放肽前体水平,并记录治疗期间不良反应。结果试验组症状缓解程度大于对照组[腹痛腹胀(0.91±1.23)分vs(1.32±0.56)分、反酸灼热(0.63±0.79)分vs(1.13±0.89)分、食欲减退(0.76±0.51)分vs(1.03±0.56)分],差异均有统计学意义(t=2.187,3.029,2.570,均P<0.05);试验组Hp根除率大于对照组[94.23%(49/52)vs 78.85%(41/52),χ^(2)=5.283,P=0.022];复发率低于对照组[6.12%(3/49)vs26.82%(11/41),χ^(2)=7.286,P=0.007]。治疗后,试验组双歧杆菌菌落数对数值[(9.92±1.25)lgCFU·g^(-1)vs(6.61±1.22)lgCFU·g^(-1)]、乳杆菌菌落数对数值[(9.25±1.21)lgCFU·g^(-1)vs(7.21±1.23)lgCFU·g^(-1)]、双歧杆菌菌落对数值/肠杆菌菌落对数值[(1.20±1.81)lgCFU·g^(-1)vs(0.80±1.69)lgCFU·g^(-1)]均高于对照组,差异均有统计学意义(t=13.665,8.525,1.164,均P<0.05);CD3^(+)[(69.70±3.68)%vs(62.31±3.60)%]、CD4^(+)[(58.75±2.56)%vs(48.22±2.51)%]、CD8^(+)[(40.52±1.56)%vs(38.24±1.58)%]、CD4^(+)/CD8^(+)[(1.45±0.51)vs(1.24±0.53)]均高于对照组,差异均有统计学意义(t=10.351,20.769,7.404,2.058,均P<0.05);试验组胃蛋白酶原Ⅰ[(136.22±7.24)μg·L^(-1)vs(145.10±7.26)μg·L^(-1)]、胃蛋白酶原Ⅱ[(8.20±1.06)μg·L^(-1)vs(11.76±1.09)μg·L^(-1)]、血清胃泌素释放肽前体[(131.79±12.15)ng·L^(-1)vs(138.52±10.26)ng·L^(-1)]均低于对照组(t=6.245,45.815,3.051,均P<0.05);两组不良反应比较差异无统计学意义(Z=0.013,1.385,0.612,0.707,P>0.05)。结论益生菌联合奥美拉唑治疗小儿Hp感染相关性胃炎疗效突出且安全性良好,预后复发风险低,对改善肠道微生态,增强肠道免疫力,缓解胃黏膜炎症反应有积极意义。 展开更多

目的探讨酸性及非酸性条件下胃蛋白酶对咽喉部黏膜上皮的损伤作用,为难治性咽喉反流性疾病治疗提供新思路。方法30只健康豚鼠随机分为3组,每组10只:A组灌注胃蛋白酶+盐酸溶液(pH=2),B组灌注胃蛋白酶+蒸馏水(pH=7),C组灌注蒸馏水(pH=7)... 目的探讨酸性及非酸性条件下胃蛋白酶对咽喉部黏膜上皮的损伤作用,为难治性咽喉反流性疾病治疗提供新思路。方法30只健康豚鼠随机分为3组,每组10只:A组灌注胃蛋白酶+盐酸溶液(pH=2),B组灌注胃蛋白酶+蒸馏水(pH=7),C组灌注蒸馏水(pH=7)。灌注14 d后取环后区黏膜采用HE染色、免疫组化、透射电镜检测炎症细胞浸润程度、occludin和claudin-3蛋白表达、上皮细胞间隙增宽程度,电镜下分析细胞亚显微结构。结果①A组黏膜下白细胞计数28.61±7.10个,B组29.89±7.04个,C组7.81±1.40个,A、B两组高于C组(P<0.05);②与C组相比较,A组occludin和claudin-3蛋白表达均下调(P<0.05),B组occludin和claudin-3蛋白表达差异无统计学意义(P>0.05);③电镜下A组黏膜上皮细胞间隙增宽,线粒体肿胀、嵴退化;B组黏膜上皮细胞膜完好,细胞间隙无增宽,但胞质内线粒体肿胀、嵴退化;C组黏膜上皮细胞间隙无增宽,线粒体形态标志正常。结论与酸性条件下相比,非酸性条件下胃蛋白酶虽不引起细胞间隙增宽,但也可造成咽喉部黏膜上皮炎症性改变和线粒体损伤。 展开更多

目的探讨雷贝拉唑联合不同用药时长荜铃胃痛颗粒治疗胃食管反流病(GERD)的临床效果。方法回顾性队列研究分析,收集2022年6月至2024年6月就诊的150例GERD患者病例资料,根据治疗方案分为A(n=56)、B(n=50)、C(n=44)3组。A组单用雷贝拉唑治... 目的探讨雷贝拉唑联合不同用药时长荜铃胃痛颗粒治疗胃食管反流病(GERD)的临床效果。方法回顾性队列研究分析,收集2022年6月至2024年6月就诊的150例GERD患者病例资料,根据治疗方案分为A(n=56)、B(n=50)、C(n=44)3组。A组单用雷贝拉唑治疗,共治疗8周;在此基础上B组联合应用4周的荜铃胃痛颗粒治疗,C组联合应用8周的荜铃胃痛颗粒治疗。观察3组临床疗效、不良反应以及短期(1个月)复发率,比较治疗前后中医证候积分、胃食管反流病问卷(GERDQ)评分、食管黏膜损伤程度以及血清胃蛋白酶Ⅰ/Ⅱ水平。结果3组治疗后中医证候积分和GERDQ评分均较治疗前降低,其中C组上述指标低于A组和B组,B组低于A组(P<0.05)。B组和C组治疗后食管黏膜损伤程度较A组轻(P<0.05),B组与C组比较无统计学差异(P>0.05)。3组治疗后血清胃蛋白酶Ⅰ/Ⅱ水平均较治疗前降低,其中C组上述指标低于A组和B组,B组低于A组(P<0.05)。C组和B组临床总有效率高于A组,且C组高于B组(P<0.05)。3组不良反应总发生率无统计学差异(P>0.05)。C组和B组1个月短期复发率均低于A组(P<0.05)。结论雷贝拉唑联合荜铃胃痛颗粒能更有效地缓解患者的症状、降低GERDQ评分、减少复发率,且不会增加不良反应发生率,长期联合使用效果更佳。 展开更多
