Predictive Value of Diaphragmatic Thickening Fraction Combined with Cough Peak Flow Rate for Weaning from Mechanical Ventilation

Objective:To investigate the predictive value of diaphragm thickening fraction(DTF)combined with cough peak expiratory flow(CPEF)on the success rate of weaning from mechanical ventilation.Methods:The clinical data of patients undergoing invasive mechanical ventilation via oral endotracheal intubation in the ICU of our hospital from January 2022 to December 2023 were studied.All patients underwent a 30-minute spontaneous breathing trial(SBT)using low-level pressure support ventilation(PSV)after meeting the clinical weaning screening criteria.Among them,150 patients who met the clinical weaning criteria were weaned from the ventilator.They were divided into a successful weaning group(n=100)and a failed weaning group(n=50)based on the weaning outcome.Clinical data,including age,gender,APACHE II score,duration of mechanical ventilation,DTF,and CPEF,were collected from 150 patients.The differences in clinical data between the two groups were compared,and the correlation between DTF,CPEF,and the success rate of weaning was analyzed.Results:There were no significant differences between the two groups in gender ratio(χ^(2)=0.884,P=0.347>0.05),age(t=0.350,P=0.727>0.05),and APACHE II score(t=1.295,P=0.197>0.05),but there was a significant difference in the duration of mechanical ventilation(t=3.766,P<0.001).The DTF and CPEF values in the successful weaning group were significantly higher than those in the failed weaning group(P<0.05).ROC curves were drawn to predict the weaning results using DTF,CPEF,and the combination of DTF and CPEF.The results showed that the specificity of the combination of DTF and CPEF was comparable to that of either metric alone,but the sensitivity and AUC were significantly higher than those of either metric alone.Conclusion:The combination of DTF and CPEF can be used as an effective indicator to evaluate the weaning efficacy of mechanically ventilated patients,which has important clinical significance for guiding clinical weaning treatment,improving the success rate of weaning,reducing the incidence of ventilator-associated pneumonia,and shortening the length of hospital stay. 展开更多

Precipitation as rainfall directly contributes to the SSPF and influences the characteristics of the SSPF, while temperature indirectly impacts the SSPF by shaping snowmelt rate and determining the soil frozen state which partitions snowmelt water into surface runoff and soil infiltration water in spring. It is necessary to identify the important and significant paths of climatic factors influencing the SSPF and provide estimates of the magnitude and significance of hypothesized causal connections between climatic factors and the SSPF. This study used path analysis with a selection of five factors - the antecedent precipitation index (API), spring precipitation (SP), winter precipitation as snowfall (WS), 〈0℃ temperature accumulation in winter ([ATNI), and average 〉0℃temperature accumulation in spring (AT) - to analyze their influences on the SSPF in the Kaidu River in Xinjiang, China. The results show that {ATN}, AT and WS have a significant correlation with the SSPF, while API and SP do not show a significant correlation. AT and WS directly influence the SSPF, while as the influence of[ATN] on SSPF is indirect through WS and AT. The indirect influence of [ATN[ on SSPF through WS accounts for 69% of the total influence of [ATN] on SSPF. Compared to the multiple linear regression method, path analysis provides additional valuable information, including influencing paths from independent variables to the dependent variable as well as direct and indirect impacts of external variables on the internal variable. This information can help improve the description of snow melt and spring runoff in hydrologic models as well as the planning and management of water resources. 展开更多

The hot deformation behavior and the microstructure characteristics of alloy 617 and alloy C-HRA-2 were compared and analyzed.The removal of Al and Ti elements has a significant change in the hot deformation of the alloy,and there are two opposite effects on the flow stress before and after recrystallization.The results show that the removal of Al and Ti elements increases the flow stress of the alloy under high temperature or low strain rate deformation conditions.This is mainly due to the increase in the stacking fault energy of the alloy so that the alloy contains a higher twin boundary fraction after dynamic recrystallization(DRX).However,before DRX occurs,that is,at low temperature and high strain rate,the flow stress of this alloy is relatively reduced.This is due to the reduction in Peierls-Nabarro stress,making the alloy more prone to dislocation slip. 展开更多

Evaluating Functions of Reservoirs'Storage Capacities and Locations on Daily Peak Attenuation for Ganjiang River Basin Using Xinanjiang Model

Flooding is the most prevalent and costly natural disaster in the world and building reservoirs is one of the major structural measures for flood control and management. In this paper, a framework was proposed to evaluate functions of reservoirs′ locations and magnitudes on daily peak flow attenuation for a large basin of China, namely Ganjiang River Basin. In this study, the Xinanjiang model was adopted to simulate inflows of the reservoirs and flood hydrographs of all sub-catchments of the basin, and simple reservoir operation rules were established for calculating outflows of the reservoirs. Four reservoirs scenarios were established to analyze reservoirs′ locations on daily peak flow attenuation. The results showed that: 1) reservoirs attenuated the peak discharges for all simulated floods, when the flood storage capacities increase as new reservoirs were built, the peak discharge attenuation by reservoirs showed an increasing tendency both in absolute and relative measures; 2) reservoirs attenuated more peak discharge relatively for small floods than for large ones; 3) reservoirs reduced the peak discharge more efficiently for the floods with single peak or multi peaks with main peak occurred first; and 4) effect of upstream reservoirs on peak attenuation decreased from upper reaches to lower reaches; upstream and midstream reservoirs played important roles in decreasing peak discharge both at middle and lower reaches, and downstream reservoirs had less effect on large peak discharge attenuation at outlet of the basin. The proposed framework of evaluating functions of multiple reservoirs′ storage capacities and locations on peak attenuation is valuable for flood control planning and management at basin scale. 展开更多

Objective:This study aimed to determine the effect of combined diaphragmatic breathing and therapeutic walking exercise on peak expiratory flow in asthma patients.Methods:The research design used a quasi-experiment nonequivalent pretest and posttest design.The research sample contained 38 respondents divided into intervention and control groups,selected by simple random sampling.The control group received standard drug therapy,while the intervention group received standard drug therapy and a combination of diaphragmatic breathing and therapeutic walking exercise for 2 weeks.This exercise was carried out in the morning,6 times a week,with 5 diaphragmatic breathing and exhalations per exercise.After that,a 1-min break was followed by a therapeutic walking exercise of 5–15 min,with an increased duration.Both groups measured the peak expiratory flow before and after the intervention using a peak flow meter.The data analysis used central tendency and t-test.Results:The results showed that the mean peak expiratory flow in the intervention group was 306.84,while in the control group,it was 232.63,with the value of the t-test being-14.17(P<0.0001).Conclusions:Diaphragmatic breathing and therapeutic walking exercise significantly increased the peak expiratory flow in asthma patients. 展开更多

Background:Pulmonary hypertension(PH)is a life-threatening condition that can be triggered by pulmonary thromboembolism(PTE),which causes abrupt increases in pulmonary artery pressure and resistance.Although Doppler echocardiography is a useful screening tool,its ability to accurately reflect rapid hemodynamic changes during acute PTE remains limited.The Flowire catheter allows for real-time assessment of intravascular flow and may offer better insight into these changes.Aims:The aims were to investigate changes in pulmonary artery hemodynamics measured using a Flowire catheter and to validate the accuracy of Doppler echocardiography in assessing these changes in dogs with acute pulmonary thromboembolism(PTE).Methods:Hemodynamic and echocardiographic data were obtained from 10 anesthetized female beagles using a Flowire catheter and echocardiography at three preload conditions:baseline,bolus loading,and an acute pulmonary hypertension state induced by a 300-μm dextran microsphere injection.Results:With increases in pulmonary artery pressure and pulmonary vascular resistance,the proximal and distal pulmonary artery flow peak measured using the Flowire catheter significantly decreased during the acute pulmonary hypertension period.Echocardiography did not accurately capture these hemodynamic changes and tended to overestimate pulmonary artery flow peak in the distal pulmonary artery.Conclusion:Doppler echocardiography has limitations in accurately reflecting complex hemodynamic changes during acute PTE.In contrast,Flowire catheterization provides additional and precise local hemodynamic information. 展开更多

Intense human activities have greatly changed the flood generation conditions in most areas of the world, and have destroyed the consistency in the annual flood peak and volume series. For design flood estimation, coaxial correlation diagram and conceptual hydrological model are two frequently used tools to adjust and reconstruct the flood series under human disturbance. This study took a typical mountain catchment of the Haihe River Basin as an example to investigate the effects of human activities on flood regime and to compare and assess the two adjustment methods. The main purpose is to construct a conceptual hydrological model which can incorporate the effects of human activities. The results show that the coaxial correlation diagram is simple and widely-used, but can only adjust the time series of total flood volumes. Therefore, it is only applicable under certain conditions(e.g. There is a strong link between the flood peaks and volumes and the link is not significantly affected by human activities). The conceptual model is a powerful tool to adjust the time series of both flood peak flows and flood volumes over different durations provided that it is closely related to the catchment hydrological characteristics, specifically accounting for the effects of human activities, and incorporating expert knowledge when estimating or calibrating parameters. It is suggested that the two methods should be used together to cross check each other. 展开更多

The feasibility of using anaerobic baffled reactor (ABR) as onsite wastewater treatment system was discussed. The ABR consisted of one sedimentation chamber and three up-flow chambers in series was experimented under different peak flow factors (PFF of 1 to 6), superficial gas velocities (between 0.6 and 3.1 cm/hr) and hydraulic retention times (HRT) (24, 36 and 48 hr). Residence time distribution (RTD) analyses were carded out to investigate the hydraulic characteristics of the ABR. It was found that the PFF resulted in hydraulic dead space. The dead space did not exceed 13% at PFF of 1, 2 and 4 while there was 2-fold increase (26%) at PFF of 6. Superficial gas velocities did not result in more (biological) dead space. The mixing pattern of ABR tended to be a completely- mixed reactor when PFF increased. Superficial gas velocities did not affect mixing pattern. The effects of PFF on mixing pattern could be minimized by higher HRT (48 hr). The tank-in-series (TIS) model (N = 4) was suitable to describe the hydraulic behaviour of the studied system. The HRT of 48 hr was able to maintain the mixing pattern under different flow patterns, introducing satisfactory hydraulic efficiency. Chemical oxygen demand (COD) and total suspended solids (TSS) removals under all flow patterns were achieved more than 85% and 90%, respectively. The standard deviation of effluent COD and TSS concentration did not exceed 15 mg/L. 展开更多

The mechanical behavior and microstructures of an Al-Zn-Mg-Cu-Cr aluminum alloy during multi-stage hot deformation were investigated by thermal stimulation test, optical microscopy, and transmission electron microscopy. The true stress vs true strain curves and the microstructure evolution of two hot deformation procedures were gained. The flow stress of the alloy studied decreases with increasing the deforming passes and declining the temperature, and the larger the temperature decline between adjacent stages, the larger the peak stress fall is. The stress-strain behavior mainly result from the dynamic recovery during deformation, the static recovery and recrystallization in the delay time, and the second phases precipitated from the matrix at high temperature. 展开更多

Urbanization causes hydrological change and increases stormwater runoff volumes, leading to flooding, erosion, and the degradation of instream ecosystem health. Best management practices (BMPs), like detention ponds and infiltration trenches, have been widely used to control flood runoff events for the past decade. However, low impact development (LID) options have been proposed as an alternative approach to better mimic the natural flow regime by using decentralized designs to control stormwater runoff at the source, rather than at a centralized location in the watershed. For highly urbanized areas, LID stormwater management practices such as bioretention cells and porous pavements can be used to retrofit existing infrastructure and reduce runoff volumes and peak flows. This paper describes a modeling approach to incorporate these LID practices and the two BMPs of detention ponds and infiltration trenches in an existing hydrological model to estimate the impacts of BMPs and LID practices on the surface runoff. The modeling approach has been used in a parking lot located in Lenexa, Kansas, USA, to predict hydrological performance of BMPs and LID practices. A performance indicator system including the flow duration curve, peak flow frequency exceedance curve, and runoff coefficient have been developed in an attempt to represent impacts of BMPs and LID practices on the entire spectrum of the runoff regime. Results demonstrate that use of these BMPs and LID practices leads to significant stormwater control for small rainfall events and less control for flood events. 展开更多

Evaluation of the WEPP Model in a Belgian Agricultural Watershed

The physically based WEPP (Water Erosion Prediction Project) model was implemented in a small agricultural watershed located in central Belgium, called Ganspoel. The watershed, mainly agricultural and resulting in a smooth topography, covers about 115 ha in a landscape typical of large parts of central Europe. Seventeen runoff, peak flow and sediment yield events, collected during a 2-year monitoring period, were simulated by the model. Even though the runoff volume predictions were well correlated to the corresponding observations, WEPP prediction capability was generally unsatisfactory also when different set-up methods of the soil effective hydraulic conductivity were used. The poor performance achieved for runoff volume and peak flow simulations affected sediment yield predictions. The differences between observed and simulated values for runoff, peak flow and sediment yield events may depend on: i) the great number of small runoff and sediment yield events within the available database with which is associated large natural variation and which in many cases are not well reproduced by WEPP; ii) the lack of model calibration processes; iii) the scarceness of information about some important soil physical and hydrological parameters; iv) the land use heterogeneity and crop schedule complexity of the Ganspoel watershed. 展开更多

Evaluating the Performance of HEC-HMS and SWAT Hydrological Models in Simulating the Rainfall-Runoff Process for Data Scarce Region of Ethiopian Rift Valley Lake Basin

A number of physically-based and distributed watershed models have been developed to model the hydrology of the watershed. For a specific watershed, selecting the most suitable hydrological model is necessary to obtain good simulated results. In this study, two hydrologic models, Soil and Water Assessment Tool (SWAT) and Hydrological Engineering Centre<span style="font-family:;" "=""><span style="font-family:Verdana;">-The Hydrologic Modeling System (HEC-HMS), were applied to predict streamflow in Katar River basin, Ethiopia. The performances of these two models were compared in order to select the right model for the study basin. Both models were calibrated and validated with stream flow data of 11 years (1990-2000) and 7 years (2001-2007) respectively. Nash-Sutcliffe Error (NSE) and Coefficient of Determination (R</span><sup><span style="font-family:Verdana;">2</span></sup><span style="font-family:Verdana;">) were used to evaluate efficiency of the models. The results of calibration and validation indicated that, for river basin Katar, both models could simulate fairly well the streamflow. SWAT gave the model performance with the R</span><sup><span style="font-family:Verdana;">2</span></sup><span style="font-family:Verdana;"> > 0.78 and NSE > 0.67;and the HEC-HMS model provided the model performance with the R</span><sup><span style="font-family:Verdana;">2</span></sup><span style="font-family:Verdana;"> > 0.87 and NSE > 0.73. Piezocatalysis is an emerging approach for degrading organic dye,However,the limited availability of ultrasonic resources in nature restricts its practical application.Our proposed peak flow kinetic energy piezocatalytic strategy,based on a "waterfall flow" model,aims to simulate the piezocatalytic degradation of pollutants in nature.This innovative strategy can enhance degradation eficiency by adjusting the flow rate and drop height.When 140 mL of rhodamine B(RhB)dye solution flows at a rate of 1000mL/min from a height of 48 cm and impacts a 3 cm diameter BaTiO_(3) nanowires/PVDF piezoelectric composite film,a degradation rate of 90%can be achieved within 120 min.This rapid degradation is primarily attributed to the efficient conversion of kinetic energy into impact force as the water falls,which triggers the generation of piezopotential in the composite film.This,in turn,drives the separation and transmission of electron-hole pairs,leading to the promotion of reactive oxygen species(ROS)generation and facilitating fast organic dye degradation.The pulsating nature of the impact force ensures a continuous generation of ROS.This approach is poised to advance piezocatalysis for the degradation of organic dyes in natural environments and presents a novel method for wastewater treatment. 展开更多

The purpose of this study is to compare the efficiency of artificial neural network and empirical methods for estimating instantaneous peak flow in Kharestan Watershed located northwest of Fars Province, Iran. For this purpose, 25 years of daily peak and instantaneous peak flow of Jamal Beig Hydrometric Station was considered. Then the estimation was done based on empirical methods including Fuller, Sangal and Fill-Steiner and artificial neural network and were compared based on RMSE and R2 . Results showed that estimation of artificial neural network is more accurate than empirical methods with RMSE = 13.710 and R2=0.942 which indicated the lower errors of artificial neural network method compared with empirical methods. 展开更多

Objective:This study aimed to evaluate the effects of graded motor imagery(GMI)on fear of movement,pain,and rehabilitation in patients with kinesiophobia after video-assisted thoracoscopic surgery(VATS)for lung cancer.Methods:Fifty-eight participants with kinesiophobia after VATS were randomly assigned into two groups:one receiving usual care(the control group)and the other receiving usual care plus GMI(the GMI group).The GMI was delivered in three stages:left/right limb identification,motor imagery,and mirror therapy delivered by two researchers every afternoon starting on the first postoperative day,once a day for about 40 min,at least twice.Level of fear of movement,pain-related patient-reported outcomes(PROs),rehabilitation exercise participation,and peak expiratory flow(PEF)were compared between the two groups.Results:Twenty-seven eligible participants were included in the GMI group and 29 in the control group.Compared to the reports on the first postoperative day,the participants who received GMI reported at discharge significant reductions in kinesiophobia,intensity of worst pain and least pain,and interference of pain with activities and emotions,and increases in rehabilitation exercise participation and PEF than those in the control group(p<0.05).An unexpected finding was a reduced surgery-to-discharge interval in the patients who received GMI(almost a day earlier than those in the control group).Conclusion:GMI can reduce fear of movement,improve pain-related PROs,and increase rehabilitation exercise participation and PEF for lung cancer patients with kinesiophobia after VATS.The study was registered at the Chinese Clinical Trial Registry(ChiCTR2300072612). 展开更多

In individuals with ventilatory pump failure(VPF)prophylactic,mechanically assisted airway clearance(ACT)by means of Mechanical Insufflation Exsufflation(MIE),is an important mitigation strategy to prevent respiratory infection,decompensation,and ultimately acute respiratory failure(ARF).VPF secondary to respiratory muscle dysfunction is a significant risk factor in neuromuscular disorders,spinal cord injury,chest wall disease,and may occur in some instances of morbid obesity.In the intensive care unit(ICU),MIE has been shown to help avoid the need for tracheostomy tube placement and improve the success rate of extubation from mechanical ventilation,especially when MIE is used in combination with continuous noninvasive ventilatory support(CNVS).Globally,clinicians are recognizing that initial titration of the settings and ongoing management of MIE can be guided by the flow and pressure waveforms that are a feature of at least one widely available MIE device. 展开更多

Objective: To observe the effect of breathing and Daoyin exercises on the quality of life in patients with stable chronic obstructive pulmonary disease(COPD) due to deficiency of the lung and kidney(grade II-III)... Objective: To observe the effect of breathing and Daoyin exercises on the quality of life in patients with stable chronic obstructive pulmonary disease(COPD) due to deficiency of the lung and kidney(grade II-III).Methods: A total of 60 eligible cases were randomly allocated into a treatment group(n=30) and a control group(n=30) by random number table. Cases in the control group received routine Western medical treatment, whereas cases in the treatment group conducted breathing and Daoyin exercises in addition to routine Western medical treatment. Patients in both groups were treated for a total of 3 months. Then the observation was made on changes in pulmonary ventilation function, major clinical symptoms, modified Medical Research Council scale(m MRC), distance in 6-minute walk test(6-MWT), COPD assessment test(CAT) and efficacy satisfaction questionnaire for COPD(ESQ-COPD) before and after treatment. Results: After treatment, the total effective rate was 80.0% in the treatment group, versus 66.7% in the control group, showing a statistical difference(P〈0.05). Patients in the treatment group obtained more significant improvement in coughing, sputum production, dyspnea and shortness of breath than those in the control group(P〈0.05). Patients in the treatment group obtained more significant elevation in the forced expiratory volume in 1 second percentage of predicted value(FEV1%) and peak expiratory flow rate(PEF%) than those in the control group(P〈0.05). Patients in the treatment group obtained lower m MRC score than those in the control group(P〈0.05). Patients in the treatment group obtained longer 6-MWT distance than those in the control group(P〈0.05). Patients in the treatment group obtained lower CAT score(P〈0.01) and higher ESQ-COPD score(P〈0.05) than those in the control group. Conclusion: Breathing and Daoyin exercises combined with routine Western medical treatment are effective for stable COPD(grade II-III) due to deficiency of the lung and kidney and can improve the patients' quality of life. 展开更多

Objective:To observe the therapeutic efficacy of acupoint application at different groups of acupoints in treating bronchial asthma in remission stage.Methods:A total of 120 patients with bronchial asthma in remission stage were recruited and divided by the random number table method into acupoint application group 1,acupoint application group 2 and acupoint application group 3,with 40 cases in each group.In all the three groups,Tiantu(CV 22),Dazhui(GV 14)and Feishu(BL 13)were selected,with Dingchuan(EX-B 1)added in acupoint application group 1,Shenshu(BL 23)added in acupoint application group 2,and Gaohuang(BL 43)added in acupoint application group 3.Before intervention,one month and 3 months after intervention,clinical symptoms,peak expiratory flow(PEF)and forced expiratory volume in 1 second percentage of predicted value(FEV1%)of the three groups were observed,and their clinical efficacies were evaluated.Results:Comparing the therapeutic efficacy regarding traditional Chinese medicine symptoms and signs,after 1-month treatment,the total effective rate was 87.5%in acupoint application group 1,versus 62.5%in acupoint application group 2 and 55.0%in acupoint application group 3,and the between-group differences were statistically significant.After 3-month treatment,the total effective rate was 95.0%in acupoint application group 1,versus 70.0%in acupoint application group 2 and 65.0%in acupoint application group 3,and the between-group differences were statistically significant.After intervention,the three groups all showed significant improvements in pulmonary function with statistical significance;among the three groups,the improvement in acupoint application group 1 was more significant than that in the other two groups.Conclusion:Tiantu(CV 22),Dazhui(GV 14)and Feishu(BL 13)as basic prescription plus Dingchuan(EX-B 1)can improve symptoms of bronchial asthma in remission stage,and it works better in improving pulmonary function than the basic prescription plus Shenshu(BL 23)or Gaohuang(BL 43). 展开更多

This study is to investigate the influence of acupuncture at acupoints Neiguan (PC 6) and Ximen (PC 4) on the stroke volume and the flow rate of peak value of left ventricle ejection in the patients with coronary heart disease. Methods. 60 cases of coronary heart disease were divided into acupoint Neiguan (PC 6) group and acupoint Ximen (PC 4) group, and the stroke volume and the flow rate of peak value of left ventricle ejection were detected 3 min after manipulating needles and 20 rain after retaining needles respectively, then were compared with those before acupuncture. Results. 3 min after manipulating needles, the stroke volume and the flow rate of peak value of left ventricle ejection were enhanced in 46 cases, and 20 min after retaining needles, both the stroke volume andthe flow rate of peak value of left ventricle ejection were all enhanced in 34 cases. Conclusion. Puncturing acupoints Neiguan (PC 6) and Ximen (PC 4) could enhance the contractility of left ventricle wall, increase the stroke volume of the heart and improve the myocardial ischemia in the patients with coronary heart disease. 展开更多
