Evaluation of lower trapezius function after transfer of axillary nerve to suprascapular nerve in patients with ERB’s palsy

BACKGROUND It is expected that transfer of spinal accessory nerve to suprascapular nerve,which is widely used in the restoration of the shoulder function in brachial plexus birth injury(BPBI),impairs the trapezius function.AIM To hypothesize that the lower trapezius muscle remains functional after this neve transfer.METHODS In a retrospective cross-sectional study,patients with BPBI who underwent nerve transfer from accessory nerve to supraclavicular were followed for at least six months following the operation and demographic data were extracted from the database.To assess the lower trapezius function,shoulder abduction and external rotation were examined,and electromyography and nerve conduction velocity(EMG-NCV)was performed.RESULTS A total of 19 patients with a mean age of 2.69±1.40 years and a mean follow-up of 10.5 months were included in the study.Shoulder abduction was disabled completely only in one patient(5.26%);10(52.63%)had good,3(15.78%)moderate,and 5(26.31%)had poor shoulder abduction.Regarding external rotation,one(5.26%)was unable to externally rotate the shoulder;among 18(94.73%)patients who had satisfactory results,8(42.10%)were evaluated to be good,5(26.31%)moderate,and 5(26.31%)poor.EMG-NCV showed functional lower trapezius in all patients;its function was evaluated to be good in 11(57.89%),moderate in 6(31.57%),and poor in 2(10.52%)cases.CONCLUSION This study supports the hypothesis that the lower trapezius muscle has a dual motor innervation which provides the possibility of further trapezius tendon transfer to restore a better shoulder function. 展开更多

AIM:To measure abducens nerve palsy patients’visual angle using real time video-based gaze tracking system.METHODS:This research was a cross-sectional study.The subjects were taken by convenience sampling technique in the Neuro-ophthalmology department at Sardjito General Hospital Yogyakarta,Indonesia,and had met the inclusion and exclusion criteria.The visual angle measurements were performed using an eye tracker with a computer webcam(GENICULA system/Gaze Tracking Artificial Intelligence for Ocular Motor Palsy).The analytical method used was the Mann-Whitney test to compare the visual angle between the abducens nerve palsy group and the healthy eye group.The Wilcoxon test was used to see the significance of differences in visual angle improvement in abducens nerve palsy patients.RESULTS:A total of 39 subjects participated,with a mean age of 46.54±15.67y;17 were male and 22 were female.The visual angle was 20.88±3.76 in the abducens nerve palsy group(n=39 eyes)and 23.10±2.91 in the normal group(n=39 eyes,P=0.011).The visual angle improvement of abducens nerve palsy before and after cured was statistically significant(P=0.039).CONCLUSION:The real time video-based gaze tracking system is easy to use,efficient,and accurate.A slight decrease in visual angle measurement can be detected using this GENICULA system and therefore it is important for diagnosis slight deviation in abducens nerve palsy. 展开更多

Background Cerebral palsy(CP)is the most prevalent motor disability among children,for which effective treatment options remain limited.This research sought to evaluate the safety and initial effectiveness of multiple intrathecal administrations of autologous bone marrow-derived mesenchymal stem cells(MSCs)in children with spastic CP.Methods A phase I,prospective,single-arm,open-label clinical trial was conducted on 16 children aged 2–12 years with spastic CP.Patients received four intrathecal injections of MSCs at monthly intervals.Safety was evaluated based on the occurrence of adverse events,while efficacy was assessed using various clinical scales,including the Gross Motor Function Classification System(GMFCS),Gross Motor Function Measure(GMFM),Berg Balance Scale(BBS),and the Modified Ashworth Scale(MAS).Assessments were conducted at baseline before treatment,and at a 12-month post-treatment follow-up.Results Safety assessment revealed no severe adverse events.Mild adverse events included nausea(18.75%),fatigue(12.5%),and headache(12.5%).Statistically significant improvements were observed in motor function,balance,and spasticity.GMFCS levels improved,with the proportion of patients in Level V decreasing from 62.5%to 43.75%(p=0.0047).Total GMFM scores increased significantly from 47.01±21.45 to 54.69±22.62(p<0.001),with domain-specific improvements in lying(p=0.002),sitting(p=0.001),crawling(p=0.002),standing(p=0.004),and walking(p=0.043).BBS scores improved from 6.75±10.88 to 9.88±13.60(p=0.001),while MAS reduced from 2.25±1.18 to 1.62±0.96(p=0.004).Functional independence improved significantly,with FIM motor scores increasing from 38.63±24.35 to 47.44±25.27(p=0.001)and cognitive scores from 57.06±29.15 to 70.88±26.96(p=0.001).Conclusion Repeated intrathecal injections of autologous MSCs are safe and significantly improve motor function,balance,and spasticity in children with spastic CP.These findings suggest that MSC therapy enhances functional outcomes and quality of life,though larger randomized controlled trials are needed. 展开更多

Prevalence of idiopathic thoracic scoliosis in children and adolescents with superior oblique palsy:a cross-sectional study

Objective:Strabismus is associated with a higher prevalence of idiopathic thoracic scoliosis(ITS)in children.This study aims to investigate whether superior oblique palsy(SOP)poses a higher risk for developing ITS among children and adolescents.Methods:A cross-sectional study.The study group comprised 347 consecutive candidates for SOP surgery aged 4 to 18 years;Patients within the same age range with ocular trauma were enrolled as the control group.Preoperative chest plain radiographs were used to measure the Cobb angle.Demographic information and clinical data,including diopter,best corrected visual acuity,deviation degree,and binocular function,were analyzed.Results:A significantly higher prevalence of ITS was found in study group compared with control group(12.68%vs 4.18%,P<0.001).Additionally,the mean Cobb angle was lagger in SOP group than that in control group(5.02°±3.87°vs 3.84°±3.09°,P<0.001).Males in SOP group showed a higher prevalence of ITS(12.9%vs 2.87%,P=0.007),but there was no significant difference in females between two groups(12.3%vs 7.69%,P=0.295).Good near stereopsis acuity was significantly associated with high prevalence of thoracic scoliosis(P<0.001).Multivariate logistic regression analysis revealed that a large distant magnitude of deviation(>20 PD)and near stereoacuity were significantly associated with ITS.Conclusions:Patients with SOP have a significantly higher risk of developing idiopathic thoracic scoliosis,especially those with good near stereoscopic and large distant magnitude of deviation. 展开更多

Ce rebral palsy is a diagnostic term utilized to describe a group of permanent disorders affecting movement and posture.Patients with cerebral palsy are often only capable of limited activity,resulting from non-progressive disturbances in the fetal or neonatal brain.These disturbances severely impact the child’s daily life and impose a substantial economic burden on the family.Although cerebral palsy encompasses various brain injuries leading to similar clinical outcomes,the unde rstanding of its etiological pathways remains incomplete owing to its complexity and heterogeneity.This review aims to summarize the current knowledge on the genetic factors influencing cerebral palsy development.It is now widely acknowledged that genetic mutations and alterations play a pivotal role in cerebral palsy development,which can be further influenced by environmental fa ctors.Des pite continuous research endeavors,the underlying fa ctors contributing to cerebral palsy remain are still elusive.However,significant progress has been made in genetic research that has markedly enhanced our comprehension of the genetic factors underlying cerebral palsy development.Moreove r,these genetic factors have been categorized based on the identified gene mutations in patients through clinical genotyping,including thrombosis,angiogenesis,mitochondrial and oxidative phosphorylation function,neuronal migration,and cellular autophagy.Furthermore,exploring targeted genotypes holds potential for precision treatment.In conclusion,advancements in genetic research have substantially improved our understanding of the genetic causes underlying cerebral palsy.These breakthroughs have the potential to pave the way for new treatments and therapies,consequently shaping the future of cerebral palsy research and its clinical management.The investigation of cerebral palsy genetics holds the potential to significantly advance treatments and management strategies.By elucidating the underlying cellular mechanisms,we can develop to rgeted interventions to optimize outcomes.A continued collaboration between researchers and clinicians is imperative to comprehensively unravel the intricate genetic etiology of cerebral palsy. 展开更多

BACKGROUND Cerebral palsy(CP)describes a group of disorders affecting movement,balance,and posture.Disturbances in motor functions constitute the main body of CP symptoms.These symptoms surface in early childhood and patients are affected for the rest of their lives.Currently,treatment involves various pharmacotherapies for different types of CP,including antiepileptics for epilepsy and Botox A for focal spasticity.However,none of these methods can provide full symptom relief.This has prompted researchers to look for new treatment modalities,one of which is mesenchymal stem cell therapy(MSCT).Despite being a promising tool and offering a wide array of possibilities,mesenchymal stem cells(MSCs)still need to be investigated for their efficacy and safety.AIM To analyze the efficacy and safety of MSCT in CP patients.METHODS Our sample consists of four CP patients who cannot stand or walk without external support.All of these cases received allogeneic MSCT six times as 1×106/kg intrathecally,intravenously,and intramuscularly using umbilical cord-derived MSCs(UC-MSC).We monitored and assessed the patients pre-and post-treatment using the Wee Functional Independence Measure(WeeFIM),Gross Motor Function Classification System(GMFCS),and Manual Ability Classification Scale(MACS)instruments.We utilized the Modified Ashworth Scale(MAS)to measure spasticity.RESULTS We found significant improvements in MAS scores after the intervention on both sides.Two months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;four months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;12 months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046.However,there was no significant difference in motor functions based on WeeFIM results(P>0.05).GMFCS and MACS scores differed significantly at 12 months after the intervention(P=0.046,P=0.046).Finally,there was no significant change in cognitive functions(P>0.05).CONCLUSION In light of our findings,we believe that UC-MSC therapy has a positive effect on spasticity,and it partially improves motor functions. 展开更多

Congenital unilateral lower lip palsy(CULLP),or congenital hypoplasia of the depressor anguli oris muscle,also known as asymmetric crying facies,is a rare condition that results in asymmetry of the lower lip during smiling,laughing,and crying.Although the etiology is unknown,weakness of the depressor labii inferioris(DLI)muscle is implicated as a contributing factor.Currently,no well-established treatment options are available.This case report describes an 18-year-old male patient diagnosed with CULLP.Physical examination revealed a symmetric face at rest,but asymmetry when smiling and opening the mouth.Following the administration of lidocaine into the affected DLI muscle,the patient’s smile and lower lip symmetry were immediately restored without any adverse effects.Subsequently,administration of botulinum toxin for neuromodulation of the DLI muscle led to a significant improvement in symmetry and oral function within 2 weeks,which was sustained at 1 month and 3 months post-treatment.No adverse effects were reported,and both patients and families expressed high satisfaction with the outcomes.This case highlights the potential use of neuromodulation as a minimally invasive and effective treatment for CULLP. 展开更多

Femoral nerve palsy following Girdlestone resection arthroplasty:An observational cadaveric study

BACKGROUND When patients with a failed hip arthroplasty are unsuitable for reimplantation,Girdlestone resection arthroplasty(GRA)is a viable treatment option.We report on a patient who was treated with a GRA due to a periprosthetic infection.We discovered partial paralysis of the quadriceps muscle in this patient.We investigated the femoral nerve anatomy,particularly the nerve entry points,to better understand this phenomenon.AIM To reveal the femoral nerve anatomy with respect to severe proximal migration after GRA.METHODS Eight cadaveric hemipelves were investigated.The branches of the femoral nerve were dissected and traced distally.The GRA was performed by the direct anterior approach.Axial stress to the lower extremity was applied,and the relative movement of the femur was recorded.The femoral nerve and its entry points were assessed.RESULTS GRA led to a 3.8 cm shift of the femur in vertical direction,a 1.8 cm shift in the dorsal direction,and a 2.3 cm shift in the lateral direction.A 36.5°external shift was observed.This caused stress to the lateral division of the femoral nerve.We observed migration of the femoral nerve entry point at the following locations:(1)Vastus medialis(5.3 mm);(2)The medial part of the vastus intermedius(5.4 mm);(3)The lateral part of the vastus intermedius(16.3 mm);(4)Rectus femoris(23.1 mm);(5)Tensor vastus intermedius(30.8 mm);and(6)Vastus lateralis(28.8 mm).CONCLUSION Migration of the femur after GRA altered the anatomy of the femoral nerve.Stress occurred at the lateral nerve division leading to poor functional results. 展开更多

Methods: The 86 children with cerebral palsy were all from the First People’s Hospital of Jingzhou City from January 2023-December 2023, and were divided into the control group and the study group with 43 cases according to the principle of voluntariness. Results: In terms of total effective rate of treatment, the gross motor function scale-88 (GMFM-88) was used to evaluate the effective rate before and after treatment, and the effective rate of the study group was higher than that of the control group, and the difference was statistically significant, and the scores of gross motor items of GMFM-88 were better than those of the control group after treatment, and the difference was statistically significant (P 0.05). Conclusion: The application of VOJTA therapy in the treatment of children with cerebral palsy can not only promote the rehabilitation of gross motor function, but also help to improve the treatment effect, and the earlier the treatment, the better. 展开更多

Objective:To examine what challenges and experiences a mother faces in caring for children with cerebral palsy(CP).As the main caregiver of children with CP,mothers are under multiple pressures.Even though mothers’challenges and experiences of children with CP will affect their quality of life and the rehabilitation of the children,little is known about their challenges and experiences during the whole care.Methods:A qualitative meta-synthesis was conducted to extract and analyze qualitative research from the PubMed,CINAHL,PsycINFO,Web of Science,Ovid and MEDLINE databases.Thirteen articles were selected and analyzed using thematic syntheses.Results:From 2522 screened abstracts,84 texts were reviewed,and 13 studies met the inclusion criteria.Mothers’challenges and experiences of children with CP include 4 themes,both positive and negative.Conclusions:The mothers of children with CP suffer from physical burden,as well as psychological pressure,in the care experience;social and healthcare support are their main challenges.Interventions to help them face the difficulties and adapt to the role of caregivers as soon as possible should target multiple aspects in order to maximize effectiveness,to promote the healthy growth of children with CP. 展开更多

Objectives To assess the feasibility of methods and estimate the potential effect of interrupting sedentary behaviour,with intermittent or continuous physical activity breaks,on cognitive performance in young people with Cerebral Palsy.Methods A randomised three-arm exposure response cross-over design with process evaluation.Participants were recruited throughout the Thames Valley,UK between 01/11/2018 to 31/03/2020.The three 2 h activity exposure visits included:(i)sitting only(controls),(ii)sitting plus 20 min of moderate-to-vigorous activity burst,or(iii)4×5 min of moderate-to-vigorous activity bursts,during a 2.5 h sedentary session.Measures of feasibility were sought.Cognitive performance outcomes(using the Eriksen Flanker task and Forward and Backward Digit Span)were delivered before and after the 2 h testing period.Results 36 participants were randomised(age 13.2±2.7,Gross-Motor Functional Classification System 1–3).Study retention was 83%across all three interventions and overall missing data for measures was 4%.A small intervention effect was found in reaction time in the 4×5 min physical activity exposure session compared to the sedentary control condition(0.42;95%CI 0.40 to 0.79).There were two research-related minor adverse effects,an allergic reaction to the FreeStyle Libre and feeling faint and vomiting after consumption of glucose solution.Both events were resolved and participants continued with the study.Conclusions The study design and intervention implementing short bursts of physical activity was feasible and indicated a potential effect on reaction time as a measure of cognitive performance in young people with cerebral palsy. 展开更多

Cerebral palsy is a common chronic disabling disease in children with special needs,and rehabilitation for cerebral palsy is characterized by a long period,multiple methods and limited efficacy.Due to the limited motor function of cerebral palsy patients,the quality of life of patients is reduced,and their ability to participate in society is reduced,so the improvement of patients’motor function has become an important goal of cerebral palsy rehabilitation.The motor function of cerebral palsy has become an important goal of cerebral palsy rehabilitation.Limb spasticity is one of the main clinical signs of cerebral palsy patients,and the more severe the limb spasticity is,the greater the impact on the patient's motor function,so improving limb spasticity in patients with cerebral palsy is particularly important.Acupuncture,as a Chinese medicine treatment method,has been widely used in the clinical treatment of cerebral palsy in China,and has shown certain efficacy.To promote the clinical application of acupuncture technique in cerebral palsy limb spasticity,this paper analyses and summarizes the relevant literature on acupuncture treatment of cerebral palsy limb spasticity summarizes the current status of the clinical application of acupuncture in cerebral palsy limb spasticity,and provides clinical references for the reasonable application of acupuncture in cerebral palsy limb spasticity. 展开更多

A Case Report on Treating Progressive Supranuclear Palsy with a Combination of Chinese and Western Medicine

A case of progressive supranuclear palsy in a 66-year-old woman is presented.The patient complained of“progressive stiffness of both lower limbs for five years,accompanied by choking and coughing for more than 1 year,”and was diagnosed with paralysis in traditional Chinese medicine(TCM),which was characterized by a deficiency of liver and kidney,and phlegm and blood stasis blocking the collaterals.Western medicine diagnosed it as progressive supranuclear palsy.The patient was diagnosed with“Parkinson’s disease”several times in the past and was given“polybasic hydrazide”and“amantadine hydrochloride tablets,”etc.The patient did not have significant symptomatic relief,so she sought further treatment with a combination of traditional Chinese and Western medicine.Based on the concept of deficiency and excess,this disease is categorized as a paralytic disease in Chinese medicine,and the onset of the disease is closely related to the deficiency of the body after a long period of illness,with wind,phlegm,blood stasis,and deficiency as the main pathogenetic mechanism.The treatment is based on the combination of disease and evidence,with traditional Chinese medicine and acupuncture as the main treatments,supplemented by intermediate-frequency pulsed electrical stimulation,transcranial repetitive magnetic stimulation,medicated canisters,wax therapy,traditional Chinese medicine guasha,and acupoint injections.Western medicine is based on symptomatic treatment,and the patient’s symptoms were relieved significantly after the combination of Chinese and Western medicine. 展开更多

Objective:To explore the main causes of oculomotor nerve palsy,providing a reference for clinical diagnosis and treatment.Methods:A total of 248 patients with oculomotor nerve palsy treated in the hospital from January 2016 to January 2018 were selected,and their causes and treatments were summarized.Results:This study found that the main causes of oculomotor nerve palsy were intracranial aneurysms,brain stem lesions,and brain inflammation.A few cases were due to viral infections and congenital oculomotor nerve palsy,while some patients had unknown causes.After treatment in our hospital,most patients experienced relief,although 12 patients died.Conclusion:The etiology of oculomotor nerve palsy is complex,and choosing appropriate drugs during treatment is challenging.Timely identification of the cause is an effective method for treatment. 展开更多

Objective:To observe the efficacy of oral-facial acupressure combined with oral placement therapy(OPT)in improving articulation clarity in 120 children with spastic cerebral palsy,and to explore effective therapeutic solutions for speech disorders associated with spastic cerebral palsy.Methods:A total of 120 children with spastic cerebral palsy and speech disorders,meeting the inclusion criteria,were randomly assigned into two groups:60 cases in the treatment group and 60 cases in the control group.The treatment group received orofacial acupressure combined with OPT,while the control group received only OPT.The Oral Motor Function Assessment Scale(OMFAS),developed by the China Rehabilitation Research Centre(CRRC),was used to evaluate the treatment outcomes before and after the intervention.Results:After the treatment,both the treatment and control groups showed improved mobility of the mandible,lips,and tongue.However,the treatment group exhibited significantly better improvement than the control group,with the difference between the two groups being statistically significant(P<0.05).Conclusion:Oral-facial acupressure combined with OPT can effectively improve articulation clarity in children with spastic cerebral palsy.This combined therapy is recommended for clinical promotion and application. 展开更多

Studies evaluate that 50% of people with diabetes will develop neuropathy. Diabetic nerve injury is impairment of a single solitary nerve (also named mononeuropathy). Com- monly 2 cranial nerves Viz., cranial nerve VII and cranial nerve VI are involved in facial nerve palsies. The former one is called Bell’s palsy and is acute Lower Motor Neuron facial nerve paralysis (>80%) causing an inability to control facial muscles on the affected side. The later one is sixth nerve palsy, (the abducens nerve), which is responsible for triggering contraction of the lateral rectus muscle to abduct (i.e., turn out) the eye resulting in double vision on the affected side. In recent times, we have found to our amazement, rapid restoration of normalcy within a few hours to a few days, of Cranial Nerve palsies in diabetic subjects at our centre. We are presenting a group of cases having Type2DM over different time periods from 6 to 20 years and those who suddenly suffered facial nerve palsy and they came to VHSDRC for treatment. They were started on the new modality called the Dynamic Acupuncture Mediated Meta-physical Energy Therapy (DAMM Therapy), to recover from the facial palsy. The DAMM therapy is a unique novel way of infusing and transferring healing energy from the therapist to the patient. Within 2 - 7 sittings of DAMM therapy patients showed 75% - 100% improvement in their clinical condition. 展开更多

Objective To explore the effect of heat-reinforcing acupuncture combined with Bobath rehabilita on training for the gross motor func on measurement(GMFM) in children with cerebral palsy. Methods Sixty cases of cerebral palsy were randomized into a heat-reinforcing acupuncture combined with rehabilitation training group(group A) and a rehabilitation group(group B),30 cases in each group. Rehabilitation training was applied in group B and heat-reinforcing acupuncture was added in group A. Jiāji(夹脊 EX-B 2),Jiānyú(肩髃 LI 15),Qūchí(曲池 LI 11),Hégi(合谷 LI 4),Yánglíngquán(阳陵泉 GB 34),Yīnlíngquán(阴陵泉 SP 9),Xuánzhōng(悬钟 GB 39),Zúsānli(足三里 ST 36),Sānyīnjiāo(三阴交 SP 6),Chéngshān(承山 BL 57),Tài chōng(太冲 LR 3),Tàixī(太溪 KI 3) and Shénmén(神门 HT 4) were chosen in group A and the heat-reinforcing acupuncture was applied once a day. For scalp acupuncture,Biihuì(百会 GV 20),Sìshéncōng(四神聪 EX-HN 1),Zhìsānzhēn(智三针),Nǎosānzhēn(脑三针),Nièsānzhēn(颞三针) and motor area(运动区) were punctured without any manipulation,once every other day,3 months as a treatment course and 2 courses were needed. GMFM were selected to assess the children with cerebral palsy before treatment,a er 3 months treatment and a er 6 months treatment. Results The total eff ec ve rate in group A was 70.00%(21/30),which is superior to that of 60.00%(24/30) in group B(P0.05). In group A,the GMFM scores of decubitus position and turn-over of body,creeping and kneeling,sitting,standing,walking,running and jumping after6 months treatment were significantly improved compared with that before treatment and after 3 months treatment(all P0.05). In group B,above indices a er 6 months treatment were significantly improved compared with those before treatment,the standing score after 6 months treatment was significantly improved compared with that a er 3 months treatment(P0.05); the GMFM scores of creeping,kneeling,sitting,standing,walking,running and jumping after 6 months treatment in group A were more significantly improved than that in group B(all P0.05). Conclusion The heat-reinforcing acupuncture combined with Bobath rehabilita on training has a be er eff ect than rehabilitation training on the GMFM scores of creeping and kneeling,si ng,standing,walking,running and jumping of children with cerebral palsy. It can be shown that the combination of heat-reinforcing acupuncture and Bobath rehabilita on training can improve the gross motor func on of children with cerebral palsy. 展开更多
