Vailati's monograph, which is the most important study of the Leibniz-Clarke ...The intellectual background of the concept of force in the dispute between Leibniz and Clarke has not received enough scholarly attention. Vailati's monograph, which is the most important study of the Leibniz-Clarke correspondence, focuses on a non-theological dimension in terms of the concept of force in this debate. Based on this perspective, Vailati's conclusion is that Clarke's understanding of force was totally different from that of Newton. However, the historical context shows that this is not the case. Clarke's concept of motive force bore a strong resemblance to that of Newton, according to which force was an active principle that had been endowed upon matter at the beginning of God's creation. Furthermore, the close link between force, matter and God's providence had a long tradition of debate between Cartesian and Gassendian philosophers since early modem times. The different concepts of force dividing Cartesian and Gassendian philosophers were actually related to, and conditioned by, their underlying fundamental theological differences. The concept of force in the Leibniz-Clarke controversy, accordingly, could be regarded as along the lines of the earlier disputes between Cartesian and Gassendian philosophers.展开更多
This study explores the housing distribution effect of the Housing Provident Fund(HPF)system on households.Utilizing data from the China Household Finance Survey conducted in 2013,2015,2017,and 2019,this study empiric...This study explores the housing distribution effect of the Housing Provident Fund(HPF)system on households.Utilizing data from the China Household Finance Survey conducted in 2013,2015,2017,and 2019,this study empirically investigates this effect from two dimensions:the impact of HPF payments on household income and housing loan behavior,and the impact on the Gini coefficient of housing assets within the group.The results indicate that the HPF payment does not significantly impact household income levels.Instead,it increases their likelihood of obtaining housing loans,particularly for middle-income households.Additionally,for any group of families,the HPF payment decreased the housing Gini coefficient among households by approximately 0.11 units.This study may be the first to provide the most direct empirical evidence on the extent of HPF’s impact on housing inequality within a group.It also questions the inference in the literature that“HPF will aggravate housing inequality within the group.”Based on this study’s findings,we expect that housing inequality will continue to be alleviated with continued HPF implementation.In particular,this effect will be more significant if the credit support for lowincome families to purchase houses through the HPF can be further enhanced.展开更多

BACKGROUND:Identifying and managing medical emergencies presents challenges in healthcare,where familiarity with established algorithms is essential for high-quality care.This study assessed healthcare professionals’...BACKGROUND:Identifying and managing medical emergencies presents challenges in healthcare,where familiarity with established algorithms is essential for high-quality care.This study assessed healthcare professionals’ understanding of the latest resuscitation guidelines and explored their views on lifelong training models.METHODS:This cross-sectional study used two multiple-choice questionnaires with 50 questions developed by academic emergency and critical care consultants based on the 2021 Consensus on Science with Treatment Recommendations (CoSTRs) by the International Liaison Committee on Resuscitation(ILCOR).Healthcare staff involved in emergency coverage completed assessments on emergency management,self-evaluated their knowledge,and shared perspectives on continuous workplace education.RESULTS:Of the 1,427 distributed questionnaires,1,034 (72.5%) were completed.Knowledge gaps were more pronounced for pediatric algorithms from the European Resuscitation Council (ERC) and American Heart Association (AHA) compared to adult protocols (P<0.001).In multivariate logistic regression,being a physician,holding a Master of Science (MSc) degree,and younger age were independently associated with passing scores≥70%(all P<0.001).Most participants (97.3%) favored brief,employer-funded teamwork refresher sessions every4–6 months over the current four-year training model (0.6%)(P<0.001).CONCLUSION:This study highlights healthcare life support providers’ insufficient expertise in current resuscitation guidelines.The importance of short-format retraining,upskilling,and reskilling programs with post-training assessments is evident,as most respondents expressed a strong learning motivation to participate if employer-funded.展开更多
BACKGROUND Glaucoma is the leading cause of irreversible blindness worldwide,with 60.5 million affected individuals,of whom 11 million are from India.Due to its asymptomatic nature,the disease largely remains underdia...BACKGROUND Glaucoma is the leading cause of irreversible blindness worldwide,with 60.5 million affected individuals,of whom 11 million are from India.Due to its asymptomatic nature,the disease largely remains underdiagnosed or diagnosed in advanced stages,where little can be done to salvage functional vision.The literature suggests that a lack of knowledge is one of the reasons for its grave consequences.Assessment of awareness is the first step in planning management.Several studies have been conducted in the Indian community,but data from healthcare providers,who play a significant role in educating the masses directly or indirectly,are limited.AIM To identify awareness,knowledge,and attitudes about glaucoma among healthcare workers in a tertiary center in India.METHODS This cross-sectional study was conducted at a tertiary care institute in Eastern India.Data were collected from 423 participants by systematic stratified sampling after Institutional Ethics Committee approval via a pretested,self-designed,semistructured,validated questionnaire.Statistical analyses were performed using the Statistical Package for Social Sciences Software v22.0.Continuous variables are expressed as the means±SD for parametric values and medians with interquartile ranges for nonparametric values.The associations between the variables were studied via multivariate linear and logistic regression.P<0.05 was considered statistically significant.RESULTS Most respondents were 20–30 years old(n=345,81.6%).The knowledge regarding glaucoma was good,and almost 56.3%of the participants gained knowledge from their medical training.The majority were aware that it has a familial predisposition and is secondary to high intraocular pressure,leading to irreversible peripheral vision loss.Only 42%knew about the life-long requirements of treatment.The resident group scored highest on knowledgeand attitude-based questions,whereas the faculty group scored highest on practice-based questions.Although 62%of the nursing staff had good attitude scores,their knowledge and practice scores were lower.The occupation group response difference was statistically significant(P<0.05)for all the knowledge-based questions.CONCLUSION Although the majority of healthcare providers are aware of glaucoma,there is a dearth of knowledge about treatment modalities.Education via seminars and media can improve their knowledge,attitudes,and practices.展开更多

Introduction Family physicians(FPs)provide essential support during life’s most challenging moments,including experiences of grief and loss.After a patient’s death,FPs are expected to provide information and emotion...Introduction Family physicians(FPs)provide essential support during life’s most challenging moments,including experiences of grief and loss.After a patient’s death,FPs are expected to provide information and emotional support to bereaved family members.Prior research suggests that bereaved relatives expect follow-up contact from their FP,acknowledging it as part of the physician’s role.This study aims to explore the practices and attitudes of FPs towards bereaved family members.Methods A cross-sectional,descriptive study was conducted through an online questionnaire,exploring participants’perceived role in bereavement care,the type of support offered to families,available resources in primary care,as well as personal knowledge and approach on grief.This questionnaire was distributed via digital platforms to general and family medicine doctors.Results A total of 210 physicians participated(84.3%female;53.8%in residency),with a median age of 32 years old.On average,each FP experienced five patient deaths,most often reported by family members.More than half(53.3%)contacted bereaved families in fewer than 25%of cases,typically within 7 days of the death,most commonly by phone(76.5%).Conclusion This study highlights significant gaps in the bereavement follow-up practices of FPs in Portugal.Findings emphasise the need for integrated bereavement care guidelines in primary care,institutional policies and investment in targeted training programmes to enhance grief’s approach.The reported lack of grief training reinforces the need to integrate grief education into medical curriculum and continuous professional development.Structural improvements,such as the implementation of automatic death notifications within the healthcare system,are also critical for enabling timely and effective support.展开更多

Background:Community-based first responders face high levels of workplace stressors that can pro-foundly impact their physical and mental health.Mindfulness-based interventions have shown promise in decreasing stress ...Background:Community-based first responders face high levels of workplace stressors that can pro-foundly impact their physical and mental health.Mindfulness-based interventions have shown promise in decreasing stress and increasing psychological resilience;however,implementation is difficult due to unpredictability of the job,department culture,and generational preferences.The objective of this qualitative study was to identify and enhance understanding of the specific needs and potential barriers and facilitators for the implementation of mindfulness-based programming for community-based first responders.Methods:A phenomenological qualitative study design was used to gain insights into the lived experiences of first responders and elicit perceptions regarding barriers and facilitators for engaging in mindfulness activities.During virtual semi-structured interviews,eleven career firefighters and emergency medical service(EMS)providers offered feedback relative to worksite mindfulness practices,and the use of a stress reduction smartphone application.The Consolidated Framework for Implementation Research was used to guide the analysis relative to barriers and facilitators of implementation success and sustainability.Results:First responders expressed openness to mindfulness-based interventions and a smartphone app that would provide stress reduction content,however,they were adamant that programming needed to be accessible and easily integrated into their workday.They also reinforced that organizational culture and leadership support for their health and well-being were overarching factors essential for implementation success.Conclusions:First responders were supportive of evidence-based mindfulness practices,optimally developed and/or facilitated by someone with intimate knowledge of their unique work environments and challenges.Organizational culture and leadership support were essential for implementation success and sustainability.展开更多
Background:Limited mental health literacy(MHL)among university service providers is a significant obstacle to effective psychological support.Developing and systematically assessing evidence-based interventions is an ...Background:Limited mental health literacy(MHL)among university service providers is a significant obstacle to effective psychological support.Developing and systematically assessing evidence-based interventions is an urgent priority,particularly in low-and middle-income countries(LMICs).This study aimed to evaluate the effectiveness of the Acceptance&Commitment to Empowerment:Linking Youths AND‘Xin’(Hearts)(ACE-LYNX)intervention in reducing stigma,improving psychological well-being,and enhancing the MHL and empowerment practices of university mental health providers in China.Methods:A total of 124 trained providers participated in this longitudinal study.Quantitative data were collected at baseline,immediately post-intervention,and three-month follow-up using the validated scale(CAMI,DASS-21)and weekly activity logs recording empowerment practices.Generalized estimating equations(GEEs)and qualitative content analysis were used for data analysis.Results:Quantitative analysis showed a significant reduction in stigma immediately postintervention,particularly in the Social Restriction subscale(β=1.35,p<0.001),though this effect diminished by the 3-month follow-up(β=1.80,p=0.001).Notably,a lasting reduction in the providers’stress levels was maintained.Activity logs showed the highest level of engagement at the individual level(51.4%),followed by group level(32.0%),organizational level(10.5%),and community level(6.1%).Qualitative analysis revealed three themes:Skill-based empowerment enhances professional efficacy,embedded interventions expand service boundaries,and organizational empowerment fosters sustainability.Conclusions:This dual-focus ACE-LYNX intervention effectively improved MHL and both attitudinal and functional competencies among providers.It provides a scalable framework for fostering sustainable and inclusive campus mental health ecosystems,with significant implication for enhance psychological services in resource-constrained educational settings.展开更多

These programs were implemented by a combination of local providers and payors. They included the development of outpatient services such as ambulatory surgery, as well as preventive care, case management, telemedicine, and mental health. The impact of these programs was a combination of these services, rather than individual efforts. The impact of these efforts was the product of a range of individual services, especially care management. Additional efforts should make it possible to extend these efforts among providers and payors in the Syracuse area. This approach should make it possible to extend the impact of health care efficiency further.展开更多

Background/objective Patients often expect antibiotics for self-limiting diseases,pressuring providers to prescribe antibiotics unnecessarily.These expectations also contribute to the unsafe practice of taking antibio...Background/objective Patients often expect antibiotics for self-limiting diseases,pressuring providers to prescribe antibiotics unnecessarily.These expectations also contribute to the unsafe practice of taking antibiotics without a prescription(non-prescription use),such as pills retained from prior prescriptions or antibiotics from non-medical sources.Previous work shows that non-prescription use is due to strong,widely held misconceptions regarding the curative power of antibiotics.To reduce unnecessary use of antibiotics,we developed and pilot-tested a patient-focused,bilingual(English and Spanish)educational tool with patient and provider stakeholder input.The tool,a trifold brochure,included information on safe antibiotic use,potential antibiotic harms and symptom management with over-the-counter medications.Design Using a qualitative design,we conducted a two-phase study to(1)develop a provider–patient communication tool and(2)pilot-test the tool in primary care clinics.Development of the tool involved patient advisory board meetings and healthcare professional(HCP)focus groups.Pilot-testing of the tool was done through semistructured interviews of randomly recruited patients from primary clinic waiting rooms and their providers.Setting Publicly funded safety net primary care clinics in Texas.Participants Patients(n=18)and HCPs(nurses,medical assistants,pharmacists,nurse practitioners and physicians)(n=14)from participating clinics.Result Themes were extracted from the qualitative data.Main themes from the development phase highlighted the need to create a simple tool to make it clear that antibiotics are not used to treat viral infections,pain or allergies and that using antibiotics without consulting a medical professional is not safe.During pilot-testing,providers noted the tool helped adjust patients’antibiotic expectations.Providers felt that the tool gave them credibility in scenarios where antibiotics were not indicated.Patients felt that the tool provided alternatives to antibiotics for symptom relief.Patients and providers found the tool useful in supporting patient–provider communication around antibiotic use.Conclusions A stakeholder-driven,patient-focused educational tool addressing inappropriate antibiotic use facilitated patient–provider communication around antibiotic usage and helped manage patients’antibiotic expectations.Embedding this tool into a community-facing intervention may reduce use of antibiotics without a prescription.展开更多

文摘In recent years,the prevalence of diabetes in China has increased significantly,and approximately 11.9%of Chinese adults had diabetes in 2020.Moreover,there are several rigorous challenges in diabetes prevention and glycaemic control,especially at the primary medical care level.In order to guide primary healthcare providers in providing comprehensive and continuous care to affected patients,the Office for Primary Diabetes Care of the National Basic Public Health Service Program and the Chinese Diabetes Society issued national guidelines for the prevention and control of diabetes at the primary care level in 2025.The management objects were adults with type 2 diabetes aged≥18 years.The main contents include basic requirements for management,health management process,diagnosis,screening,evaluation,treatment,recognition and management of acute complications,traditional Chinese medicine,referral and health management and education.

文摘Introduction Family physicians(FPs)provide essential support during life’s most challenging moments,including experiences of grief and loss.After a patient’s death,FPs are expected to provide information and emotional support to bereaved family members.Prior research suggests that bereaved relatives expect follow-up contact from their FP,acknowledging it as part of the physician’s role.This study aims to explore the practices and attitudes of FPs towards bereaved family members.Methods A cross-sectional,descriptive study was conducted through an online questionnaire,exploring participants’perceived role in bereavement care,the type of support offered to families,available resources in primary care,as well as personal knowledge and approach on grief.This questionnaire was distributed via digital platforms to general and family medicine doctors.Results A total of 210 physicians participated(84.3%female;53.8%in residency),with a median age of 32 years old.On average,each FP experienced five patient deaths,most often reported by family members.More than half(53.3%)contacted bereaved families in fewer than 25%of cases,typically within 7 days of the death,most commonly by phone(76.5%).Conclusion This study highlights significant gaps in the bereavement follow-up practices of FPs in Portugal.Findings emphasise the need for integrated bereavement care guidelines in primary care,institutional policies and investment in targeted training programmes to enhance grief’s approach.The reported lack of grief training reinforces the need to integrate grief education into medical curriculum and continuous professional development.Structural improvements,such as the implementation of automatic death notifications within the healthcare system,are also critical for enabling timely and effective support.

文摘The“notice-necessary measures”rule began with the“notice-and-takedown”rule,which originated from the United States safe harbors rule,established to limit the liability of Internet Service Providers(ISPs)for helping infringement in network infringement.China’s Tort Liability Law and Civil Code amended the“notice-and-takedown”rule so that when the right holder sends an effective notice of infringement to the ISP,the ISP shall take necessary measures more than removal to be exempted from liability.This article will discuss the transformation of the rule and types of necessary measures,and reconstruct the relationship between effectiveness of notice and types of necessary measures.

文摘Background:Community-based first responders face high levels of workplace stressors that can pro-foundly impact their physical and mental health.Mindfulness-based interventions have shown promise in decreasing stress and increasing psychological resilience;however,implementation is difficult due to unpredictability of the job,department culture,and generational preferences.The objective of this qualitative study was to identify and enhance understanding of the specific needs and potential barriers and facilitators for the implementation of mindfulness-based programming for community-based first responders.Methods:A phenomenological qualitative study design was used to gain insights into the lived experiences of first responders and elicit perceptions regarding barriers and facilitators for engaging in mindfulness activities.During virtual semi-structured interviews,eleven career firefighters and emergency medical service(EMS)providers offered feedback relative to worksite mindfulness practices,and the use of a stress reduction smartphone application.The Consolidated Framework for Implementation Research was used to guide the analysis relative to barriers and facilitators of implementation success and sustainability.Results:First responders expressed openness to mindfulness-based interventions and a smartphone app that would provide stress reduction content,however,they were adamant that programming needed to be accessible and easily integrated into their workday.They also reinforced that organizational culture and leadership support for their health and well-being were overarching factors essential for implementation success.Conclusions:First responders were supportive of evidence-based mindfulness practices,optimally developed and/or facilitated by someone with intimate knowledge of their unique work environments and challenges.Organizational culture and leadership support were essential for implementation success and sustainability.

文摘Background:Limited mental health literacy(MHL)among university service providers is a significant obstacle to effective psychological support.Developing and systematically assessing evidence-based interventions is an urgent priority,particularly in low-and middle-income countries(LMICs).This study aimed to evaluate the effectiveness of the Acceptance&Commitment to Empowerment:Linking Youths AND‘Xin’(Hearts)(ACE-LYNX)intervention in reducing stigma,improving psychological well-being,and enhancing the MHL and empowerment practices of university mental health providers in China.Methods:A total of 124 trained providers participated in this longitudinal study.Quantitative data were collected at baseline,immediately post-intervention,and three-month follow-up using the validated scale(CAMI,DASS-21)and weekly activity logs recording empowerment practices.Generalized estimating equations(GEEs)and qualitative content analysis were used for data analysis.Results:Quantitative analysis showed a significant reduction in stigma immediately postintervention,particularly in the Social Restriction subscale(β=1.35,p<0.001),though this effect diminished by the 3-month follow-up(β=1.80,p=0.001).Notably,a lasting reduction in the providers’stress levels was maintained.Activity logs showed the highest level of engagement at the individual level(51.4%),followed by group level(32.0%),organizational level(10.5%),and community level(6.1%).Qualitative analysis revealed three themes:Skill-based empowerment enhances professional efficacy,embedded interventions expand service boundaries,and organizational empowerment fosters sustainability.Conclusions:This dual-focus ACE-LYNX intervention effectively improved MHL and both attitudinal and functional competencies among providers.It provides a scalable framework for fostering sustainable and inclusive campus mental health ecosystems,with significant implication for enhance psychological services in resource-constrained educational settings.
文摘This study reviewed a combination of health care programs in the metropolitan area of Syracuse, New York. They were designed to improve care, however a major purpose was to support efficiency. The study described a number of individual programs that were developed in order to improve the quality and the efficiency of care. These programs were implemented by a combination of local providers and payors. They included the development of outpatient services such as ambulatory surgery, as well as preventive care, case management, telemedicine, and mental health. The impact of these programs was a combination of these services, rather than individual efforts. The impact of these efforts was the product of a range of individual services, especially care management. Additional efforts should make it possible to extend these efforts among providers and payors in the Syracuse area. This approach should make it possible to extend the impact of health care efficiency further.

文摘Background/objective Patients often expect antibiotics for self-limiting diseases,pressuring providers to prescribe antibiotics unnecessarily.These expectations also contribute to the unsafe practice of taking antibiotics without a prescription(non-prescription use),such as pills retained from prior prescriptions or antibiotics from non-medical sources.Previous work shows that non-prescription use is due to strong,widely held misconceptions regarding the curative power of antibiotics.To reduce unnecessary use of antibiotics,we developed and pilot-tested a patient-focused,bilingual(English and Spanish)educational tool with patient and provider stakeholder input.The tool,a trifold brochure,included information on safe antibiotic use,potential antibiotic harms and symptom management with over-the-counter medications.Design Using a qualitative design,we conducted a two-phase study to(1)develop a provider–patient communication tool and(2)pilot-test the tool in primary care clinics.Development of the tool involved patient advisory board meetings and healthcare professional(HCP)focus groups.Pilot-testing of the tool was done through semistructured interviews of randomly recruited patients from primary clinic waiting rooms and their providers.Setting Publicly funded safety net primary care clinics in Texas.Participants Patients(n=18)and HCPs(nurses,medical assistants,pharmacists,nurse practitioners and physicians)(n=14)from participating clinics.Result Themes were extracted from the qualitative data.Main themes from the development phase highlighted the need to create a simple tool to make it clear that antibiotics are not used to treat viral infections,pain or allergies and that using antibiotics without consulting a medical professional is not safe.During pilot-testing,providers noted the tool helped adjust patients’antibiotic expectations.Providers felt that the tool gave them credibility in scenarios where antibiotics were not indicated.Patients felt that the tool provided alternatives to antibiotics for symptom relief.Patients and providers found the tool useful in supporting patient–provider communication around antibiotic use.Conclusions A stakeholder-driven,patient-focused educational tool addressing inappropriate antibiotic use facilitated patient–provider communication around antibiotic usage and helped manage patients’antibiotic expectations.Embedding this tool into a community-facing intervention may reduce use of antibiotics without a prescription.
