The prevalence of Class Ⅲ malocclusion varies among different countries and regions. The populations from Southeast Asian countries (Chinese and Malaysian) showed the highest prevalence rate of 15.8%, which can seriously affect oral function, facial appearance, and mental health. As anterior crossbite tends to worsen with growth, early orthodontic treatment can harness growth potential to normalize maxillofacial development or reduce skeletal malformation severity, thereby reducing the difficulty and shortening the treatment cycle of later-stage treatment. This is beneficial for the physical and mental growth of children. Therefore,early orthodontic treatment for Class Ⅲ malocclusion is particularly important. Determining the optimal timing for early orthodontic treatment requires a comprehensive assessment of clinical manifestations, dental age, and skeletal age, and can lead to better results with less effort. Currently, standardized treatment guidelines for early orthodontic treatment of Class Ⅲ malocclusion are lacking. This review provides a comprehensive summary of the etiology, clinical manifestations, classification, and early orthodontic techniques for Class Ⅲ malocclusion, along with systematic discussions on selecting early treatment plans. The purpose of this expert consensus is to standardize clinical practices and improve the treatment outcomes of Class Ⅲ malocclusion through early orthodontic treatment. 展开更多

Corticotomy is a clinical procedure to accelerate orthodontic tooth movement characterized by the regional acceleratory phenomenon(RAP).Despite its therapeutic effects,the surgical risk and unclear mechanism hamper the clinical application.Numerous evidences support macrophages as the key immune cells during bone remodeling.Our study discovered that the monocyte-derived macrophages primarily exhibited a pro-inflammatory phenotype that dominated bone remodeling in corticotomy by CX3CR1CreERT2;R26GFP lineage tracing system.Fluorescence staining,flow cytometry analysis,and western blot determined the significantly enhanced expression of binding immunoglobulin protein(BiP)and emphasized the activation of sensor activating transcription factor 6(ATF6)in macrophages.Then,we verified that macrophage specific ATF6 deletion(ATF6f/f;CX3CR1CreERT2 mice)decreased the proportion of pro-inflammatory macrophages and therefore blocked the acceleration effect of corticotomy.In contrast,macrophage ATF6 overexpression exaggerated the acceleration of orthodontic tooth movement.In vitro experiments also proved that higher proportion of pro-inflammatory macrophages was positively correlated with higher expression of ATF6.At the mechanism level,RNA-seq and CUT&Tag analysis demonstrated that ATF6 modulated the macrophage-orchestrated inflammation through interacting with Tnfαpromotor and augmenting its transcription.Additionally,molecular docking simulation and dual-luciferase reporter system indicated the possible binding sites outside of the traditional endoplasmic reticulum-stress response element(ERSE).Taken together,ATF6 may aggravate orthodontic bone remodeling by promoting Tnfαtranscription in macrophages,suggesting that ATF6 may represent a promising therapeutic target for non-invasive accelerated orthodontics. 展开更多

Orthodontic appliances are essential for dentofacial deformity corrections.However,orthodontic appliances inadvertently increase the risk of bacterial colonization and dental calculus formation,which may lead to dental caries and gingivitis.Herein,this study developed a pH-responsive antifouling coating by integrating a zwitterionic hydrogel(ZH)with pH-responsive microcapsules(PRMs)encapsulating bactericide,displaying excellent synergies of anti-bacteria and anti-calculus for orthodontic appliances.The excellent antifouling properties can be attributed to two following points:ZH provides anti-adhesion properties via electrostatically induced hydration layers,while the PRMs can kill bacteria by on-demand bactericide release under acidic conditions.Results demonstrated that ZH+PRMs coating significantly reduced bacterial adhesion and inhibited calculus formation while maintaining excellent biocompatibility.By optimizing PRMs concentrations(0–15 wt%),compared with ZH,the antibacterial efficiency of ZH+PRMs(optimal concentration 10 wt%)increased from 49.8%±7.3%to 95.2%±1.1%for E.coli and from 85.7%±3.5%to 91.3%±1.4%for S.mutans.Compared with pristine steel(SS),ZH+PRMs coating showed ca.97.0%reduction for calcium carbonate and ca.87.3%reduction for calcium phosphate.In an in vitro model,compared with SS,our coating extended the crystal biofilm inhibition effect from one day to five days.Therefore,this study can provide promising strategies for reducing the risk of dental caries and gingivitis during orthodontic treatment. 展开更多

Objective: To compare the stress distribution in the periodontal ligament under different orthodontic forces during canine distalization using long-arm brackets, and to determine the optimal force value for this device in orthodontic treatment. Methods: A finite element model was constructed after extracting the mandibular first premolar, and a long-arm bracket with a traction height of 6 mm was placed on the labial side of the mandibular canine. Three working conditions of 50 g, 100 g, and 150 g were simulated, and the magnitude and distribution of von Mises stress in the periodontal ligament were compared for each condition. Results: The maximum von Mises stress in the periodontal ligament was 0.013281 MPa in the 50 g condition, 0.02536 MPa in the 100 g condition, and 0.035549 MPa in the 150 g condition. As the orthodontic force increased, the stress distribution area in the periodontal ligament also expanded. Conclusion: A 100 g orthodontic force is the most suitable when using long-arm brackets, providing a relatively uniform stress distribution in the periodontal ligament and keeping the stress within a reasonable range. 展开更多

BACKGROUND The generation of intrabony defects due to the iatrogenic use of elastic bands is an undesirable situation that can result in persistent gingival inflammation with subsequent bone degradation,thus ultimately leading to tooth loss.CASE SUMMARY This clinical case involved a 27-year-old male patient who complained of persistent inflammation and bleeding in the upper anterior region of the gums,despite having undergone dental cleaning for at least 4 years.The dental and medical history indicated the use of removable orthodontic appliances for 8 years,after which braces were placed for 2 years.The intraoral evaluation revealed inflammation and localized suppuration in teeth 11 and 12.Measurements of 2-7 mm for probing depth and 1-5 mm for clinical attachment loss were detected,and combined bone loss was observed via radiographs.Based on the clinical and radiographic findings,localized stage III,grade C periodontitis was diagnosed.During subgingival debridement,two elastic bands emerged around the involved teeth.The bone defects persisted;therefore,they were surgically addressed using a papilla preservation flap and guided tissue regeneration(GTR).CONCLUSION The use of elastic bands of various sizes and elasticities is often essential in multiple orthodontic treatments.However,it is crucial to perform a thorough check-up for each patient during treatment and at the end of treatment to remove any remaining residue of resin,metal bands,or orthodontic bands.Additionally,it is imperative to inform the patients of the importance of attending their follow-up appointments.The use of elastic bands in orthodontics requires special care;moreover,GTR is a management option for intrabony defects associated with the iatrogenic use of bands. 展开更多

Effect of 5.25% Sodium Hypochlorite on Shear Bond Strength of Orthodontic Brackets Bonded with OBA-MCP

Introduction: Bracket debonding is a frequent issue that clinicians encounter, leading to increased chair time, lost revenue, and material usage. In addition to patient compliance with their diet recommendations, the preparation and conditioning of teeth for bonding significantly influence bond strength and consequently impact orthodontic treatment success and efficiency. Because of OBA-MCP’s (orthodontic bonding adhesive with modified calcium phosphate) decreased shear bond strength (SBS), the purpose of this study was to evaluate the effects of conditioning with 5.25% sodium hypochlorite (NaOCl) before etching in the bonding protocol. Materials and Methods: 90 extracted teeth were divided into 3 groups to be bonded with orthodontic brackets with different bonding protocols: 1) Transbond XT with regular bonding protocol (etch + prime + adhesive);2) OBA-MCP with regular bonding protocol;and 3) OBA-MCP with NaOCl prior to acid etching in the regular bonding protocol. SBS (in Newtons) were measured using an MTS universal testing machine with a custom jig to apply a vertical force onto the bracket and ARI (adhesive remnant index) scores were recorded for each sample after de-bond to rate the amount of adhesive remaining. Results: The addition of NaOCl to the bonding protocol statistically significantly increased the SBS of OBA-MCP to comparable levels to Transbond XT. The ARI scores showed that when NaOCl was added, more adhesive remained. Conclusion: The addition of NaOCl to the bonding protocol can increase the SBS of adhesives with historically weaker bond strengths. However, the increased amount of adhesive remaining and the increased time spent during bonding must be considered. Further testing can be done in vivo to demonstrate the practicality of this new procedure. 展开更多

Over the past decade,dramatic progress has been made in dental research areas involving laser therapy.The photobiomodulatory effect of laser light regulates the behavior of periodontal tissues and promotes damaged tissues to heal faster.Additionally,photobiomodulation therapy(PBMT),a non-invasive treatment,when applied in orthodontics,contributes to alleviating pain and reducing inflammation induced by orthodontic forces,along with improving tissue healing processes.Moreover,PBMT is attracting more attention as a possible approach to prevent the incidence of orthodontically induced inflammatory root resorption(OIIRR)during orthodontic treatment(OT)due to its capacity to modulate inflammatory,apoptotic,and anti-antioxidant responses.However,a systematic review revealed that PBMT has only a moderate grade of evidence-based effectiveness during orthodontic tooth movement(OTM)in relation to OIIRR,casting doubt on its beneficial effects.In PBMT-assisted orthodontics,delivering sufficient energy to the tooth root to achieve optimal stimulation is challenging due to the exponential attenuation of light penetration in periodontal tissues.The penetration of light to the root surface is another crucial unknown factor.Both the penetration depth and distribution of light in periodontal tissues are unknown.Thus,advanced approaches specific to orthodontic application of PBMT need to be established to overcome these limitations.This review explores possibilities for improving the application and effectiveness of PBMT during OTM.The aim was to investigate the current evidence related to the underlying mechanisms of action of PBMT on various periodontal tissues and cells,with a special focus on immunomodulatory effects during OTM. 展开更多

Orthodontic treatment offers great advantages in improving facial and smile aesthetics, self-confidence and the function of the stomatognathic apparatus. The pursuit of these advantages makes use of orthodontic appliances that could be fixed or removable. However, it’s worth stating that these appliances interfere with tooth brushing, making it more difficult to brush teeth effectively. Orthodontics appliances therefore promote the accumulation of dental plaque, which results in both quantitative and qualitative changes in the oral microbiota, hence, exposing patients to several adverse effects such as White spot lesions, dental caries, periodontal pathologies and halitosis. For this reason, oral assessment of patients before, during and after treatment is necessary as well as oral hygiene instructions and motivation. Orthodontists therefore, should educate patients on oral and periodontal hygiene in order to control dental and periodontal complications. Prescriptions of plaque control materials adapted to each patient are done in order to optimize the final result and minimize unwanted complications. 展开更多

BACKGROUND In the clinical scenario,adult patients with periodontal diseases and dental malformation,characterized by dental crowding in lower anterior teeth with the thin biotype,often require orthodontic treatment.This case report aimed to evaluate the clinical and radiographic outcomes of periodontally accelerated osteogenic orthodontics(PAOO)combined with autologous platelet-rich fibrin(PRF)in an adult patient with class I malocclusion along with dental crowding,a thin periodontal biotype,and buccal plate deficiency.CASE SUMMARY A 32-year-old female complaining of dental crowding and gingival bleeding was referred to the orthodontic clinic.The patient underwent periodontal risk assessment prior to orthodontic treatment.She was diagnosed with a high risk of gingival recession due to dental crowding,root prominence,loss of buccal plates,and a thin gingival tissue biotype.The treatment regimen included PAOO combined with autologous PRF for alveolar augmentation and interproximal enamel reduction for moderate dental crowding.Clinically,PAOO-assisted orthodontic tooth movement in this case showed enhanced periodontium remodeling.Radiographic outcomes also showed statistically significant improvements(P<0.01)in the mandibular buccal alveolar bone.CONCLUSION This case report suggests the combination of autologous PRF with PAOO to enhance bone augmentation and long-term tissue support in adult orthodontic patients with periodontal disease. 展开更多

A simple overview of daily orthodontic practice involves use of brackets, wires and elastomeric modules. However, investigating the underlying effect of orthodontic forces shows various molecular and cellular changes. Also, orthodontics is in close relation with dentofacial orthopedics which involves bone regeneration. In this review current and future applications of stem cells(SCs) in orthodontics and dentofacial orthopedics have been discussed. For craniofacial anomalies, SCs have been applied to regenerate hard tissue(such as treatment of alveolar cleft) and soft tissue(such as treatment of hemifacial macrosomia). Several attempts have been done to reconstruct impaired temporomandibular joint. Also, SCs with or without bone scaffolds and growth factors have been used to regenerate bone following distraction osteogenesis of mandibular bone or maxillary expansion. Current evidence shows that SCs also have potential to be used to regenerate infrabony alveolar defects and move the teeth into regenerated areas. Future application of SCs in orthodontics could involve accelerating tooth movement, regenerating resorbed roots and expanding tooth movement limitations. However, evidence supporting these roles is weak and further studies are required to evaluate the possibility of these ideas. 展开更多

The purpose of this article is to evaluate the accuracy of 3D printed retainers in comparison to vacuum formed retainers. Methods: Alginate impressions were taken for ten patients who have a CBCT scan. A 3D printed retainer and vacuum formed retainer were fabricated. Linear measure-ments were measured by two assessors using digital caliper. Every measure-ment on the 3D printed retainer was compared to the corresponding measure-ment on the thermoformed retainer. The linear measurements were Inter-canine width, Inter-premolar width (first and second premolars), Inter-molar width, Canine-midline length (both sides) and Canine-molar length (both sides). Intra-observer, and inter-observer reliability measurements were done. Results: Results showed excellent intra-observer reliability for the thermoformed retainer and the 3D printed retainer. Inter-observer measurements showed strong agreement between the measurements of the two assessors, for both retainers. The comparison of the thermoformed retainer to the 3D printed retainer showed high statistical agreement, except for the canine-molar on the right side, but with no clinical significance, p value of 0.038 and mean difference 0.19. Conclusions: The new method for fabricating a 3D printed retainer is accurate and reliable in comparison to the vacuum formed retainer (conventional method). CBCT proved to be efficient for fabrication of a custom made appliances. 展开更多

Excessive forces may cause root resorption and insufficient forces would introduce no effect in orthodontics. The objective of this study was to investigate the optimal orthodontic forces on a maxillary canine, using hydrostatic stress and logarithmic strain of the periodontal ligament(PDL) as indicators. Finite element models of a maxillary canine and surrounding tissues were developed. Distal translation/tipping forces, labial translation/tipping forces, and extrusion forces ranging from 0 to 300 g(100 g=0.98 N) were applied to the canine, as well as the force moment around the canine long axis ranging from 0 to 300 g·mm. The stress/strain of the PDL was quantified by nonlinear finite element analysis, and an absolute stress range between 0.47 k Pa(capillary pressure) and 12.8 k Pa(80% of human systolic blood pressure) was considered to be optimal, whereas an absolute strain exceeding 0.24%(80% of peak strain during canine maximal moving velocity) was considered optimal strain. The stress/strain distributions within the PDL were acquired for various canine movements, and the optimal orthodontic forces were calculated. As a result the optimal tipping forces(40–44 g for distal-direction and 28–32 g for labial-direction) were smaller than the translation forces(130–137 g for distal-direction and 110–124 g for labial-direction). In addition, the optimal forces for labialdirection motion(110–124 g for translation and 28–32 g for tipping) were smaller than those for distal-direction motion(130–137 g for translation and 40–44 g for tipping). Compared with previous results, the force interval was smaller than before and was therefore more conducive to the guidance of clinical treatment. The finite element analysis results provide new insights into orthodontic biomechanics and could help to optimize orthodontic treatment plans. 展开更多

Low-level laser therapy(LLLT) may have an effect on the pain associated with orthodontic treatment. The aim of this study was to evaluate the effect of LLLT on pain and somatosensory sensitization induced by orthodontic treatment. Forty individuals(12–33 years old; mean ± standard deviations: 20.8 ± 5.9 years) scheduled to receive orthodontic treatment were randomly divided into a laser group(LG) or a placebo group(PG)(1:1). The LG received LLLT(810-nm gallium-aluminium-arsenic diode laser in continuous mode with the power set at 400 mW, 2 J·cm–2) at 0 h, 2 h, 24 h, 4 d, and 7 d after treatment, and the PG received inactive treatment at the same time points. In both groups, the non-treated side served as a control. A numerical rating scale(NRS) of pain, pressure pain thresholds(PPTs), cold detection thresholds(CDTs), warmth detection thresholds(WDTs), cold pain thresholds(CPTs), and heat pain thresholds(HPTs) were tested on both sides at the gingiva and canine tooth and on the hand. The data were analysed by a repeated measures analysis of variance(ANOVA). The NRS pain scores were significantly lower in the LG group(P = 0.01). The CDTs,CPTs, WDTs, HPTs, and PPTs at the gingiva and the PPTs at the canine tooth were significantly less sensitive on the treatment side of the LG compared with that of the PG(P < 0.033). The parameters tested also showed significantly less sensitivity on the nontreatment side of the LG compared to that of the PG(P < 0.043). There were no differences between the groups for any quantitative sensory testing(QST) measures of the hand. The application of LLLT appears to reduce the pain and sensitivity of the tooth and gingiva associated with orthodontic treatment and may have contralateral effects within the trigeminal system but no generalized QST effects. Thus, the present study indicated a significant analgesia effect of LLLT application during orthodontic treatment.Further clinical applications are suggested. 展开更多
