RATIONALE Epinova is a non-invasive skin treatment that combines the correct concentrations and combinations of topicals and modalities to elicit facial rejuvenation with no down-time or side effects. Purpose: This paper focuses on facial rejuvenation improvements combining the RATIONALE Essential Six skincare system (RATIONALE, Victoria, Australia) to protect and repair the skin with the RATIONALE Epinova facial treatment every 4-6 weeks—which uses non-invasive technologies and professional strength active ingredients to deliver visible changes to skin tone and texture. Methods: Subjects underwent a RATIONALE consultation, including taking a skin history and skin imaging, followed by a data analysis and diagnosis of skin condition and prescription of a customized RATIONALE treatement (Epinova), including appropriate pharmacologic agents and treatment with personalized photo/sono therapeutic devices. Results: Subjects reported increased skin hydration, tactile improvements, skin firmness and visible radiance following the RATIONALE Epinova treatment. Further investigations will be initiated to explore the potential for longer term improvements, including connenctive tissue deposition, reduction of erythema etc. Treatments should be performed every 4-6 weeks for patients under 40 and every 3-4 weeks for patients over 40, to support cell differentiation, migration and desquamation to achieve non-invasive facial rejuvenation. Conclusion: This study demonstrated that the synergy of pharmacologic, LED light therapy and ultrasonic technologies when prescribed and administered by a trained skin therapist, can lead to a visible improvement in the signs of facial ageing and photodamage, restoring the appearance of healthy, radiant skin. .展开更多

To evaluate the role of 18F-FDG-PET after neoadjuvant chemoradiation as a valid, non-invasive predictor for early therapy response and its effect on survival as compared to histopathologic tumor response, data of 32 of 210 randomized patients with NSCLC stage IIIA/IIIB, who were treated in a?prospective randomized controlled multicenter trial (LUCAS- MD), were re-evaluated. Material and Methods: For 32 patients with NSCLC stage IIIA (44%) IIIB (56%) neoadjuvant treatment consisted of two to three cycles of chemotherapy (225 mg/m2 paclitaxel and carboplatin AUC 6 d1q22) and concomitant chemoradiation (50 mg/m2 paclitaxel and carboplatin AUC 2 d1, d8, d15;1.5 Gy b.i.d. up to 45 Gy). Documentation of involved lymph node stations as detected by 18F-FDG-PET/CT and lymph node sampling during surgery according to the IASLC lymph node mapping (2009). Evaluation of histological regression grade (RG) according to Junker et al. (2001) and correlation with?18F-FDG-PET/CT for primary tumor and each lymph node station. Calculation of disease free survival using Kaplan-Meier estimates and log rank tests. Results: Actuarial tumor specific survival for the 32 patients with concomitant chemoradiation plus?chemotherapy: complete vs. incomplete metabolic remission prior to surgery after 60 months:?40% vs. 24% (p?= 0.018). RG III/IIb (no/less than 10% of vital tumor cells) vs. RG IIa/I (more than 10% vital tumor cells) after 60 months: 46% vs. 15% (p?= 0.006). 18/32 (56%) patients had RG III/IIb, 8/32 (25%) patients had regression grade III. 1/8 pts. with RG III were in the 18F-FDG- PET/CT false positive, 10 pts. with RG IIb (i.e. all pts. with RG IIb) were in the 18F-FDG-PET/CT false negative. One patient with RG IIa was in the 18F-FDG-PET/CT false negative. Hence, the cut-off level in detecting vital tumor cells by 18F-FDG-PET/CT after neoadjuvant chemoradiation for NSCLC is about 10%. Conclusion: Histological regression grading correlates well with metabolic remission as detected by 18F-FDG-PET. Thus, 18F-FDG-PET precedes CT in measuring the tumor response and may predict long-term therapeutic outcome in patients with stage III NSCLC. Invasive staging procedures may be avoided and patients who will not profit from resection due to insufficient downstaging after neoadjuvant treatment will be easily detected by using 18F-FDG-PET as standard imaging in workup and evaluation of treatment response.展开更多

Atopic eczema is a chronic inflammatory disease where the skin barrier has become leaky, and inflammation occurs. It affects up to 20% of children and 3% of adults worldwide, manifesting as red itchy patches of skin with varying severity. This review aims to investigate the leaky skin barrier and immune mechanisms from the perspective of potential novel treatments. The complexity of atopic eczema as a disease is what makes it difficult to treat. Genome-wide association studies have highlighted possible genetic variations associated with atopic eczema, however in some cases, individuals develop the disease without these genetic risk factors. Loss of function mutations in the filaggrin gene are one of these associations and this is plausible due to its key role in barrier function. The Th2 immune response is the link with regards to the immune mechanisms as atopic inflammation often occurs through increased levels of interleukin(IL)-4 and IL-13. Eczematous inflammation also creates susceptibility to colonisation and damage by bacteria such as Staphylococcus aureus. Potential novel treatments are becoming ever more specific, offering the hope of fewer side effects and better disease control. The best new treatments highlighted in this review target the immune response with human beta defensin 2, phosphodiesterase-4 inhibitors and monoclonal antibodies all showing promise.展开更多

The aim of this paper is to report our experience in the management of skin toxicity evaluating radiotherapy planning and using preventive local aids. Materials and Methods: We have observed 300 patients undergoing radiotherapy. All received the prescription of a prophylactic moisturizing cream. Skin toxicity was valuated according to Acute Radiation Morbidity Scoring Criteria of the RTOG. Moreover, in a subgroup of 100 patients, we correlated the study of breast volume and features of treatment plans, with the addition of topic prophylactic treatment, using an oral therapy based on Resveratrol, Lycopene, Vitamin C and Anthocyanins (Ixor?). In another subgroup, 100 patients were subjected to corneometry assessing numerically skin hydration before, during and after radiotherapy. Results: In all patients, we related skin toxicity and the type of cream used. All patients completed the radiotherapy treatment, and G4 cutaneous toxicity was not observed in any of them. In patients treated with topic treatment and therapy based on Resveratrol, Lycopene, Vitamin C and Anthocyanins (Ixor?), the protective effect of (Ixor?) is more detected in patients with PTV (Planning Target Volume) <500 ml, when Dmax reaches values lower or equal to 107%, but not exceeding 110% of the prescribed dose, and in patients undergoing adjuvant chemotherapy with anthracyclines and taxanes. The values of corneometry allow us to evaluate the moisturizing effect for products used, and identify cases of skin toxicity in a first phase. Conclusions: Our study confirms the value of moisturizers in the prevention and resolution of radiotherapy-induced skin damage. An instrumental assessment of skin hydration with corneometry can help the radiation oncologist to use strategies that prevent the onset of toxicity of high degree.展开更多

Thixotropy is desired for providing fluidity when agitated and a suitably degree of stiffening thereafter. This requires low shear resistance in the coating phase and microstructural reorganization when leaving the paste to rest. Following the principle of using only mineral components for skin treatment,?use?of expandable hydrophilic clay minerals should be considered. They sorb?cations and positively charged organic molecules and are impermeable to fluids and gas under low pressure, hence providing oxygen-free micro-environment. They can balance pH and are excellent agents for cleaning skin.展开更多

To analyze which curative effect is more effective, the currently adopted method is to select the relevant patients whose time of investigation is nearly two years after August 2019, and use the method of random classification to divide the patients into different groups, compare the operation combined treatment, and analyze certain difference effects. After treatment, it was found that the recurrence rate of surgery has a great impact on our patients, which is of significance in comparison with science. The recurrence rate of the first group is within 10% and that of the second group is higher than 20%. In terms of statistical significance, it conforms to the relevant data of statistics. At the same time, the operation treated by photodynamic therapy has obvious effect on the patients and can obviously reduce the recurrence rate of the operation, which is good for our operation. The experiment will implement photodynamic therapy combined with surgical treatment for patients with skin tumors. Compared with the clinical application effect, the experiment can reduce the recurrence rate, and reduce the depth and area of the surgical resection site, which is helpful to control the tumor development and has promotional value.展开更多

These conditions have overlapping symptoms which are, in part, reflective of overlapping neuropathology. These conditions become problematic because their co-occurrence can exacerbate symptoms. Therefore, treatments must be developed that are inclusive to all three conditions. Repetitive transcranial magnetic stimulation (rTMS) is non-invasive and may be an ideal treatment for co-occurring AUD + mTBI + PTSD. There is accumulating evidence on rTMS as a treatment for people with AUD, mTBI, and PTSD each alone. However, there are no published studies to date on rTMS as a treatment for co-occurring AUD + mTBI + PTSD. This review article advances the knowledge base for rTMS as a treatment for AUD + mTBI + PTSD. This review provides background information about these co-occurring conditions as well as rTMS. The existing literature on rTMS as a treatment for people with AUD, TBI, and PTSD each alone is reviewed. Finally, neurobiological findings in support of a theoretical model are discussed to inform TMS as a treatment for co-occurring AUD + mTBI + PTSD. The peer-reviewed literature was identified by targeted literature searches using PubMed and supplemented by cross-referencing the bibliographies of relevant review articles. The existing evidence on rTMS as a treatment for these conditions in isolation, coupled with the overlapping neuropathology and symptomology of these conditions, suggests that rTMS may be well suited for the treatment of these conditions together.展开更多

Histological study demonstrated deep ablation and collagen restructuring in the papillary and reticular dermis. The Fractora device combines the more “cone shaped” ablation seen with CO2 and Erbium lasers with a deep non-ablative heating pattern, seen with other bipolar RF fractional needle resurfacing devices. Ablation, coagulation zones and healing dynamics are analyzed for different energy settings. Two different treatment protocols are suggested: one for light skin and then one for darker skin with a higher risk of post-inflammatory hypperpigmentation. Treatment results show improvement in skin texture, pores, wrinkles and skin dyschromia.展开更多

Enzymatic preparations from moulds are a mixed of different enzymes with strong and unspe-cific activities. Some Saccharomyces cerevisiae pro-duce pectinases which can be used instead of com-mercial preparations. The objectives of this work were to study the enzyme secretion by one Saccharo-myces cerevisiae (CECT 11783) for growing on grape skin (industry oenological by-product) as carbon source. Preliminary experiments showed that the strain produced pectinases for growing on grape skin without any other carbon source. Statistical treat-ment (factorial design 25) was applied to evaluate the influences of related factors (agitation, temperature, presence of peptone and detergent in the medium and time of growth) Variables with the most significant interactions for pectinase production were agitation and nitrogen source concentration. Response surface methodology showed that a first order model was not adequate for results. Nevertheless, the built of a sec-ond order model offered a polynomial equation which surface predicted a maximum of activity (52.68 enzymatic units) for specific values of the studied variables (147.8 rpm of agitation and 15.9 g of pep-tone/ L culture medium).展开更多

The optimal focal length (L), operating frequency (f), element size (a) and central opening radius for lodging an imaging probe (r) of a device that would safely treat tissue within the prostate were obtained. Images from the Visible Human Project were used to determine simulated organ sizes and treatment locations. Elliptical tumors were placed throughout the simulated prostate and their lateral and axial limits were selected as test locations. Using graphics processors, the acoustic field and Bio-Heat Transfer Equation were solved to calculate the heating produced during a simulated treatment. L, f, a and r were varied from 45 to 75 mm, 2.25 to 3 MHz, 1.5 to 8 times the wavelength and 9 to 12.5 mm, respectively. The resulting optimal device was a 761-element concentric-ring transducer with L = 68 mm, f = 2.75 MHz, a = 2.05λ and r = 9 mm. Simulated thermal lesions showed that it was possible to treat target tumors consistent with reported locations and sizes for prostate cancer.展开更多

An instrumental assessment of skin hydration with corneometry can help the radiation oncologist to use strategies that prevent the onset of toxicity of high degree.
文摘It is important that pastes and creames for skin treatment have suitable rheological properties and ability to establish a?good contact with the tissues while retaining their tightness. They can balance pH and are excellent agents for cleaning skin.
文摘Objective: to compare photodynamic therapy with clinical experiments in the surgical treatment of skin tumors. Compared with the clinical application effect, the experiment can reduce the recurrence rate, and reduce the depth and area of the surgical resection site, which is helpful to control the tumor development and has promotional value.
文摘Objective: to investigate the effect of non-peeling dot array laser combined with photon skin rejuvenation in the treatment of acne scar. Methods: 22 patients with acne scar in our hospital from January 2019 to December 2019 were randomly divided into two groups by number table method. The control group was treated with non-peeling dot array laser, and the experimental group was treated with non-peeling dot array laser combined with photon skin freshening. Acne scar symptom score and treatment effect were compared between the two groups before and after treatment. Results: after treatment, the scores of acne scar symptom in experimental group were lower than those in control group (P < 0.05). The therapeutic effect of the experimental group was higher than that of the control group (P < 0.05). Conclusion: the treatment of acne scar patients with non-peeling lattice laser combined with photon skin freshening has definite effect, can improve the symptoms, improve the treatment effect, so it is worth popularizing and applying.
文摘Metabolic-associated steatotic liver disease(MASLD),formerly referred to as non-alcoholic fatty liver disease, represents an escalating worldwide medical burden defined by hepatic steatosis, inflammation, fibrosis, and potential progression to cirrhosis or hepatocellular carcinoma.Artificial intelligence(AI)has emerged as a transformative tool in MASLD management, enhancing diagnostic accuracy, risk stratification, and treatment optimization. This review explores the integration of AI in MASLD diagnosis, including AI-based histopathological assessment,non-invasive screening models, imaging diagnostics, and gut microbiota-based app-roaches.Additionally,AI-driven treatment strategies facilitate personalized management, assess therapeutic response, and contribute to drug discovery.Despite its advantages, challenges such as data integration, model interpretability, and cost-effectiveness remain obstacles to widespread adoption.Future advan-cements in explainable AI,multi-modal data fusion, and cost-efficient implement-ations will be crucial for maximizing AI’s impact on MASLD care.AI-driven innovations hold great promise for improving early detection, guiding person-alized treatment, and ultimately enhancing patient outcomes in MASLD.

文摘Alcohol use disorder (AUD), mild traumatic brain injury (mTBI), and posttraumatic stress disorder (PTSD) commonly co-occur (AUD + mTBI + PTSD). The existing evidence on rTMS as a treatment for these conditions in isolation, coupled with the overlapping neuropathology and symptomology of these conditions, suggests that rTMS may be well suited for the treatment of these conditions together.
文摘Psoriatic arthritis(PsA) is a rheumatoid factor(RF)-seronegative systemic inflammatory disorder associated with psoriasis.Current treatment for PsA in China is still focused on disease modifying anti-rheumatic drugs(DMARDs).In this paper,we report two Chinese patients with active longstanding PsA treated with infliximab,a human-mouse chimeric monoclonal antibody against tumor necrosis factor alpha(TNF-α).The results show that infliximab acted quickly and effectively in relieving peripheral and axial symptoms and refractory skin lesions, even in recombinant human TNF-α receptor(rhTNFR)-resistant case. The take-home message from our cases is that infliximab is a useful therapeutic option for refractory PsA,especially when a patient has a combination of psoriasis and psoriatic arthritis.Further local evidence and experience must be accumulated in order to make anti-TNF-α therapy more accessible to PsA patients in China.

文摘This paper reports the clinical experience of a multi-center, multiple physician trial with a novel fractional radiofrequency ablative skin resurfacing and rejuvenation device (Fractora, Invasix, Israel) deployed on both Caucasian skin types I - III and Asian skin type IV. Treatment results show improvement in skin texture, pores, wrinkles and skin dyschromia.
文摘Pectinases are used in Enology for some different utilities. Nevertheless, the built of a sec-ond order model offered a polynomial equation which surface predicted a maximum of activity (52.68 enzymatic units) for specific values of the studied variables (147.8 rpm of agitation and 15.9 g of pep-tone/ L culture medium).
文摘A parametric study was performed to design a device capable of treating small targeted regions within the prostate using high intensity focused ultrasound, while sparing the surrounding organs and minimizing the number of elements. Simulated thermal lesions showed that it was possible to treat target tumors consistent with reported locations and sizes for prostate cancer.
文摘BACKGROUND Aggressive natural killer cell leukemia(ANKL)is a rare natural killer cell neoplasm characterized by systemic infiltration of Epstein–Barr virus and rapidly progressive clinical course.ANKL can be accompanied with hemophagocytic lymphohistiocytosis(HLH).Here,we report a case of ANKL with rare skin lesions as an earlier manifestation, accompanied with HLH,and review the literature in terms of etiology, clinical manifestation, diagnosis and treatment.CASE SUMMARY A 30-year-old woman from Northwest China presented with the clinical characteristics of jaundice, fever, erythema, splenomegaly, progressive hemocytopenia, liver failure, quantities of abnormal cells in bone marrow, and associated HLH.The immunophenotypes of abnormal cells were positive for CD2,cCD3,CD7,CD56,CD38 and negative for sCD3,CD8 and CD117.The diagnosis of ANKL complicated with HLH was confirmed.Following the initial diagnosis and supplementary treatment, the patient received chemotherapy with VDLP regimen(vincristine,daunorubicin,L-asparaginase and prednisone). However, the patient had severe adverse reactions and complication such as severe hematochezia, neutropenia, and multiple organ dysfunction syndrome, and died a few days later.CONCLUSION This is the first reported case of ANKL with rare skin lesions as an earlier manifestation and associated with HLH.
