The influence of electro-acupuncture at Neiguan (PC 6)and five shu points of twelve regular meridians at different time on systolic time interval (ST I) of normal young adultswere studied with 6×6 Latin square design in this work.In 12 groups according to 12 regular merldlans,the STIs were measured respectively just before and after electro-acupuncture on Neiguan andfive shu points at 00:00,04:00,08:00,12:00,16:00 and 20:00 of a day.It was found thatexcept the PEPI and PEPI/LVETI ratio among Shenmen(HT 7),Shaochong(HT 9)and Neiguan,Guanchong(TE)and Yemen(TE 2)points,the effects of electro-acupuncture at Neiguan pointand five shu points of twelve regular meridians on the indices of STI of normal young adults wereshowing no slgnificant differences.There also was no difference in different sex.But the indices ofSTI were changed when electroacupuncture on the different points of different meridians at differenttime. 展开更多

OBJECTIVE:To observe the effects of electroacupuncture at Neiguan(PC6)at different time points on reperfusion arrhythmia(RA)after myocardial ischemia and reperfusion in rats,and to investigate the correlation of this protective effect with nerve growth factor(NGF),tyrosine kinase A(Trk A),tyrosine hydroxylase(TH),and norepinephrine(NE).METHODS:A total of 72 Sprague-Dawley male rats were randomly divided into six groups(n=12 rats/group):normal group(Norm),sham operation group(Sham),ischemia reperfusion group(I/R),pre-ischemic electroacupuncture group(EAI),pre-reperfusion electroacupuncture group(EAII),post-reperfusion electroacupuncture group(EAIII).The myocardial ischemia-reperfusion injury(MIRI)model was induced by occlusion of left anterior descending coronary artery for 20 min followed by reperfusion for 40 min in rats.With no intervention in the Norm group and only threading without ligation in the Sham group.Electroacupuncture pretreatment at 20 min/d for 7 d before ligation in the EAⅠgroup,20 min of electroacupuncture before reperfusion in the EAII group and 20 min of electroacupuncture after reperfusion in the EAIII group.The electrocardiogram(ECG)of each group was recorded throughout the whole process,and the success of the MIRI model was determined based on the changs of J-point and T-wave in the ECG.The arrhythmia score was used to record premature ventricular contractions,ventricular tachycardia and ventricular fibrillation during the reperfusion period to assess the reperfusion induced arrhythmias.The expression levels of NGF,Trk A,TH protein were measured by Western blot.Moreover,the expression levels of plasma and myocardial NE levels were detected by enzyme linked immunosorbent assay.RESULTS:The differences between Norm group and Sham group were not statistically significant in all indexes.Arrhythmia score,myocardial NGF,Trk A,TH,and NE expression were significantly higher in the I/R group compared with the Sham group.Arrhythmia score,myocardial NGF,Trk A,TH,and NE expression were significantly lower in each EA group compared with the I/R group.CONCLUSION:Electroacupuncture at Neiguan(PC6)at different time points can reduce the incidence and severity of reperfusion arrhythmias in rats.This protective effect is related to electroacupuncture regulating NGF,Trk A,TH,NE expression and reducing sympathetic hyperactivation. 展开更多

OBJECTIVE:To investigate the effects of electronic stimulation at acupoints Neiguan(PC 6) and Lieque(LU 7) on the gene expression of the adenosine triphosphate(ATP)-Sensitive potassium channel(KATP:Kir6.1,Kir6.2,SUR2 A,and SUR2B) and protein kinases(PKA,PKG,and PKCβ2) in myocardial cells of rats with myocardial ischemia(Ml) induced by isoproterenol(ISO).METHODS:Rats were randomly divided into a control,model,Neiguan(PC 6),Lieque(LU 7),and non-acupoint groups.The Ml model was established by injecting rats with ISO.Electro-acupuncture treatment was given to the acupuncture groups,once a day for 7 days.Gene expression was analyzed with real-time PCR.RESULTS:The gene expression of KATP and protein kinases in the model group was higher than those in the control group(P < 0.05).After acupuncture treatment,the KATP and protein kinase expression levels were significantly lower in the Neiguan(PC 6)and Lieque(LU 7) groups compared with the model group[P < 0.05).The Neiguan(PC 6) group lowered these levels significantly more than that of the Lieque(LU 7) group(P < 0.05).No significant differences were observed between the model and non-acupoint groups(P > 0.05).CONCLUSION:Our findings suggest that electronic needling of Neiguan(PC 6) can both reduce the gene expression of KATP and protein kinases in rats with ISO-induced Ml. 展开更多

OBJECTIVE:To compare the effects of Neiguan(PC6)acupuncture at different depths and retention time on arrhythmia duration,myocardial tissue morphology,mRNA expression level of L-type calcium channelα1C subunit and Ca^(2+)-Mg^(2+)-At Pase activity in tachirrhythmia model of rabbits.METHODS:The tachyarrhythmia model was made by intravenous injection of barium chloride into the ears of rabbits.A total of 56 healthy adult male New Zealand big-eared white rabbits,apply the random number table method,divided into normal control group(group A),model group(group B),shallow needling Neiguan(PC6)10 min group(group C),shallow needling Neiguan(PC6)20 min group(group D),shallow needling Neiguan(PC6)30 min group(group E),deep needling Neiguan(PC6)10 min group(group F),deep needling Neiguan(PC6)20 min group(group G),deep needling Neiguan(PC6)30 min group(group H),7 animals in each group.Electrocardiograms were used to collect the duration of arrhythmia;hematoxylin-eosin staining method was performed on myocardial tissue,RT-PCR tested the expression ofα1C subunit mRNA,and the activity of Ca^(2+)-Mg^(2+)-ATPase were quantified by phosphorus determination method.RESULTS:The duration of arrhythmia in each acupuncture treatment group was shortened to varying degrees.Compare to the model group,the tissue damage from barium chloride inducing was improved in the acupuncture group.Compared to the model group,except for group E,most treatment groups had varying degrees of improvement with significantly down-regulated L-type calcium channelα1C subunit mRNA expressions level and increased Ca^(2+)-Mg^(2+)-ATPase activity.CONCLUSIONS:The effect of acupuncture at Neiguan(PC6)with different depths and retention time can reduce the duration of arrhythmia induced by barium chloride relatively,improve the induced pathological changes,down regulate L-type calcium channelα1C subunit mRNA expressions level and increase Ca^(2+)-Mg^(2+)-ATPase activity.Both the shallow and deep tissues of Neiguan(PC6)may be involved in transmitting acupuncture information.There is an optimal induction period for shallow needling at Neiguan(PC6)to reach the best therapeutic effect. 展开更多

Objective:To study and analyze the clinical effect of electroacupuncture at Baihui and Neiguan Points on cognitive function recovery of elderly patients after general anesthesia.Methods:The subjects were 80 elderly patients who received general anesthesia from January 2019 to February 2021.According to different anesthesia methods,they were divided into research group(electroacupuncture at Baihui and Neiguan combined with conventional drug anesthesia)and control group(simple drug anesthesia).The postoperative cognitive function recovery of the two groups was observed and compared.Results:Compared with the control group,the MMSE score and POCD incidence of research group were significantly better.There was significant difference between the two groups(P<0.05).Conclusion:Conclusion electroacupuncture at Baihui and Neiguan combined with conventional drug anesthesia can promote the recovery of cognitive function and prevent cognitive dysfunction in elderly patients under general anesthesia. 展开更多

OBJECTIVE: To investigate the effect of electroacupuncture(EA) of acupoints Neiguan(PC 6) and Tianquan(PC 2) on the skin temperature, blood perfusion, and alpha1-and beta2-adrenoreceptor(α1-AR and β2-AR) protein and m RNA level in rats with acute myocardial ischemia.METHODS: Thirty male adult Wistar rats [(230 ±20) g] were randomly assigned into five groups(n = 6 each): a control group, sham operation group, model group, model group treated with EA at low frequency(L-EA, 2 Hz, 1 m A) and model group treated with EA at high frequency(H-EA, 100 Hz, 1 m A). The rat model was prepared by ligating the left anterior descending coronary artery. Electroacupuncture was performed at the left Neiguan(PC 6) for 20 min daily for 3 d. After the 3 rd time of the treatment, measurements of skin blood perfusion and temperature in Neiguan(PC 6) and Tianquan(PC 2) in all groups were made by laser speckle contrast imaging and infrared thermal image instrument. Real-time PCR and ELISA were used to measure m RNA level and protein level of α1-AR andβ2-AR in the skin tissues of Neiguan(PC 6) and Tianquan(PC 2), respectively. Serum levels of c Tn T and electrocardiogram were used to identify the state of myocardium.RESULTS: In the group receiving electroacupunture at Neiguan(PC 6), compared with control group,the skin temperature, blood perfusion, and β2-AR m RNA and protein level of model group significantly decreased(P < 0.05), α1-AR m RNA and protein level of model group significantly increased(P < 0.05);compared with model group, the skin temperature,blood perfusion, and β2-AR m RNA and protein level of L-EA significantly increased(P < 0.05), α1-AR m RNA and protein level of L-EA and H-EA significantly decreased(P < 0.05). The same trend has taken place in the former four groups of the Tianquan(PC 2).CONCLUSION: Low-or high-frequency electroacupuncture can improve the skin temperature and blood perfusion which may be induced by decreasingthelevelofα1-ARandincreasingthelevelofβ2-AR of the Neiguan(PC 6) and Tianquan(PC 2) in the rat with acute myocaidial ischemia. 展开更多

OBJECTIVE: To investigate the effect of acupuncture at Neiguan(PC 6) on cardiac function using echocardiography in rat models of myocardial ischemia(MI) induced by isoproterenol(ISO).METHODS: Twenty-seven Sprague-Dawley rats were randomly assigned to normal, model, and acupuncture groups. The model and acupuncture groups were given injections of ISO(85 mg/kg) to establish the MI model. After model establishment,the acupuncture group was treated with acupuncture at Neiguan(PC 6) for 30 min. Echocardiography was used to monitor diastolic and systolic function for 30 min starting from the time after the acupuncture needles were removed. Changes in the length of left ventricular internal diameter at end-diastole(LVIDd), length of left ventricular internal diameter at end-systole(LVIDs), the ratio of mitral peak velocity at early diastole and atrial contraction(E/A), ejection fraction(EF), fractional shortening(FS), and stroke volume(SV) were measured.RESULTS: Compared with the model group at 0and 15 min after needles were removed, the means of LVIDd and LVIDs were significantly lower(P <0.01) and E/A, EF, FS, and SV significantly higher(P < 0.01) in the acupuncture group. In the acupuncture group, the means of LVIDd and LVIDs15 min after the needles were removed were significantly higher(P < 0.01) than those at 0 min. The means of E/A, EF, FS, and SV significantly decreased(P < 0.01) from 0 to 15 min in the acupuncture group.CONCLUSION: These findings indicate that acupuncture at Neiguan(PC 6) can affect cardiac function by increasing left ventricular diastolic and systolic function in MI rat models, but the effect only lasts for 15 min. 展开更多

OBJECTIVE: To observe the effect of transcutaneous electroacupuncture(TEA) at Neiguan(PC 6) on refractory vomiting in critically ill patients in intensive care(ICU) setting.METHODS: Ten patients admitted in ICU and presenting vomiting refractory to one or more antiemetic drugs were prospectively included in the study. TEA was applied at acupoint of Neiguan(PC6) during 30 min with a neuromuscular transmission monitor(single-twitch stimulation with 1 Hz at a constant current of 10 m A). Nausea and Vomiting were evaluated at the following intervals: immediately after 30 min of TEA at Neiguan(PC 6),30 min-6h and 6-24 h. The presence of nausea and/or vomiting throughout the observational period was defined as the primary end point.RESULTS: The presence of nausea or vomiting throughout the observational period was 10% at the end of TEA, 40% between 30 min and 6 h, and50% between 30 min and 24 h(P < 0.001, P = 0.01 and P = 0.03 vs pre-TEA, respectively). There were no complications or side effects related to TEA.CONCLUSION: TEA at Neiguan(PC 6) seems effective in reducing refractory vomiting in the patients in ICU setting, even if larger trials are needed to define optimal modalities. 展开更多

目的通过检测促红细胞生成素(EPO)、磷酸化磷脂酰肌醇3激酶(p-PI3K)、磷酸化AKT(p-AKT)及Bcl-2等蛋白的表达,探究电针内关穴对急性高原缺氧性脑损伤的防治效应和保护机制。方法SPF级雄性SD大鼠共48只,随机分为空白组(Control)、模型组(M... 目的通过检测促红细胞生成素(EPO)、磷酸化磷脂酰肌醇3激酶(p-PI3K)、磷酸化AKT(p-AKT)及Bcl-2等蛋白的表达,探究电针内关穴对急性高原缺氧性脑损伤的防治效应和保护机制。方法SPF级雄性SD大鼠共48只,随机分为空白组(Control)、模型组(Model)、西药组(WM)、内关组(Neiguan)。采用低压氧舱构建急性高原缺氧性脑损伤模型。西药组采用胞磷胆碱钠灌胃给药,内关组予以双侧内关穴电针刺激治疗。采用HE染色观察海马组织形态变化,酶联免疫吸附试验测定大鼠血清生化指标,原位末端转移酶标记法(TUNEL)观察大鼠海马组织神经元凋亡情况并计算阳性率,蛋白质印迹法检测大鼠海马组织中促红细胞生成素(EPO)、磷酸化磷脂酰肌醇3激酶(p-PI3K)、磷酸化AKT(p-AKT)及Bcl-2蛋白的表达水平。结果与空白组相比,模型组可见大量坏死的锥体细胞、细胞核固缩深染,血清神经胶质细胞原纤维酸性蛋白(GFAP)、S100B及泛素羧基末端水解酶L1(UCH-L1)水平显著升高(P<0.01),海马神经元凋亡细胞数目明显增多(P<0.01),海马组织中EPO、p-PI3K、p-AKT、Bcl-2蛋白水平升高(P<0.01)。与模型组相比,西药组和电针内关组脑组织坏死锥体细胞数量明显减少、有少量细胞核固缩并伴有深染现象,血清GFAP、S100B及UCH-L1水平均显著降低(P<0.01),海马神经元凋亡细胞数目减少(P<0.01),海马组织中EPO、p-PI3K、p-AKT、Bcl-2蛋白水平均升高(P<0.01)。西药组以上指标与内关组相比,差异无统计学意义(P>0.05)。结论电针内关穴能改善急性高原缺氧性脑损伤,可能与调节EPO蛋白的表达进而激活PI3K/AKT信号通路,抑制海马神经元细胞凋亡有关。 展开更多

目的:观察电针足三里、内关穴联合血必净注射液对脓毒症心肌损伤(SIMI)患者炎症因子及组织灌注的影响。方法:选取2023年6月—2024年6月浙江省立同德医院收治的120例SIMI血瘀证患者,采用随机数字表法分为对照组和观察组各60例。对照组给... 目的:观察电针足三里、内关穴联合血必净注射液对脓毒症心肌损伤(SIMI)患者炎症因子及组织灌注的影响。方法:选取2023年6月—2024年6月浙江省立同德医院收治的120例SIMI血瘀证患者,采用随机数字表法分为对照组和观察组各60例。对照组给予常规西药治疗,观察组加用电针足三里、内关穴联合血必净注射液治疗。比较2组病情改善情况、炎症因子、组织灌注指标、心肌损伤和心肌功能指标及预后情况。结果:治疗后,2组中医证候评分、脓毒症相关序贯器官衰竭(SOFA)评分、急性生理学和慢性健康评估(APACEⅡ)评分、急诊脓毒症死亡风险(MEDS)评分均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组血清C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平较治疗前降低(P<0.05),且观察组血清CRP、IL-6、TNF-α水平低于对照组(P<0.05)。治疗后,2组血乳酸(LAC)水平较治疗前降低,且观察组低于对照组(P<0.05);2组中心静脉血氧饱和度(ScvO_(2))、平均动脉压(MAP)较治疗前增加,且观察组高于对照组(P<0.05)。治疗后,2组肌钙蛋白I(cTnI)、B型钠尿肽(BNP)水平较治疗前降低,且观察组低于对照组(P<0.05);2组左心室射血分数(LVEF)、二尖瓣口舒张早期血流峰值流速(E峰)/二尖瓣口舒张晚期血流峰值流速(A峰)较治疗前升高,且观察组高于对照组(P<0.05)。与对照组比较,观察组机械通气时间和ICU住院时间更短,28 d病死率更低(P<0.05)。结论:电针足三里、内关穴联合血必净注射液治疗可以减轻SIMI患者的病情,减轻炎症反应,改善组织灌注和心肌功能,缩短机械通气时间和ICU住院时间,降低死亡风险。 展开更多

目的 探讨不同时机电针“内关”对肌肉注射西地兰大鼠药动学及药效学的影响及差异,明确针药结合对其影响的特点。方法 72只SD雄性大鼠随机分为药物组(C组),给药前电针组(EB+C组),给药同时电针组(ED+C组)和给药后电针组(EA+C组),每组18... 目的 探讨不同时机电针“内关”对肌肉注射西地兰大鼠药动学及药效学的影响及差异,明确针药结合对其影响的特点。方法 72只SD雄性大鼠随机分为药物组(C组),给药前电针组(EB+C组),给药同时电针组(ED+C组)和给药后电针组(EA+C组),每组18只。通过高效液相色谱质谱联用(HPLC-MS)检测药动学相关指标;另将24只SD雄性大鼠随机分为药物组(C组),电针组(E组),给药前电针组(EB+C组)和给药同时电针组(ED+C组),每组6只。通过左心室插管术,比较心功能指标及其干预前后变化率。结果 药动结果显示,针药结合组的血清和心脏药物浓度分别在不同时间点高于药物组,且与电针干预时机呈相关性。EB+C组和ED+C组的AUC、Cmax显著高于C组和EA+C组(P<0.05)。各组达峰时间无差异。药效结果显示,不同干预方式均能对大鼠HR、LVSP、±dp/dtmax产生显著影响。除心率变化率外,EB+C组和ED+C组的变化率显著高于C组(P<0.05),而E组与C组相比无明显差异,两个针药结合组之间也无明显差异。结论 针药结合对药动和药效的影响均优于单独给药;不同时机电针“内关”,对肌肉注射西地兰具有协同药物增强治疗效应,尤以给药前和给药同时电针为佳。 展开更多

目的:探讨电针“内关”对心肌缺血再灌注损伤(MIRI)大鼠不同时期(缺血期、再灌注期)线粒体自噬的调控作用,探讨电针“内关”的双向调控作用及可能机制。方法:将45只雄性SD大鼠按照随机数字表法分为6组,假手术组(9只)、模型A组(6只)、模... 目的:探讨电针“内关”对心肌缺血再灌注损伤(MIRI)大鼠不同时期(缺血期、再灌注期)线粒体自噬的调控作用,探讨电针“内关”的双向调控作用及可能机制。方法:将45只雄性SD大鼠按照随机数字表法分为6组,假手术组(9只)、模型A组(6只)、模型B组(9只)、电针A1组(6只)、电针B1组(6只)、电针B2组(9只)。除假手术组外,其余各组大鼠采用物理结扎加推管法复制MIRI模型,模型A组进行结扎后直接取材,模型B组进行结扎、再灌注后取材;电针A1组结扎的同时进行电针处理,之后直接取材;电针B1组结扎的同时进行电针处理,再灌注后取材;电针B2组结扎、开放冠状动脉左前降支的同时予电针处理,再灌注后取材。电针取双侧“内关”,采用疏密波,频率2 Hz/100 Hz,电流1 mA,时长为30 min。HE染色法观察大鼠心肌细胞形态,TUNEL法检测大鼠心肌细胞凋亡情况,转录组测序检测大鼠左心室差异表达基因,JC-1流式法检测大鼠心肌细胞线粒体膜电位(MMP),Western blot法检测大鼠左心室磷酸酶和张力蛋白同源物诱导激酶1(Pink1)、Parkin和p62蛋白表达,ELISA法检测大鼠血清肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTn-I)含量。结果:与假手术组比较,模型B组大鼠心肌细胞损伤严重,排列紊乱,边界不清,肌纤维断裂,细胞水肿、松散;电针B2组大鼠心肌细胞损伤轻微,细胞结构部分不清,细胞排列较规则,可见完整心肌细胞。与假手术组比较,模型B组大鼠心肌细胞凋亡加重(P<0.001);与模型B组比较,电针B2组心肌细胞凋亡减轻(P<0.001)。电针B2组、假手术组和模型B组之间的差异表达基因与细胞自噬和线粒体自噬密切相关。与假手术组比较,模型B组大鼠心肌细胞MMP降低(P<0.001),左心室Pink1、Parkin、p62蛋白表达和血清CK-MB、cTn-I含量升高(P<0.001);与模型A组比较,电针A1组心肌细胞MMP和血清CK-MB、cTn-I含量降低(P<0.001,P<0.05),左心室Pink1蛋白表达升高(P<0.01);与模型B组比较,电针B1组和电针B2组心肌细胞MMP升高(P<0.001),左心室Pink1、Parkin、p62蛋白表达及血清CK-MB、cTn-I含量降低(P<0.001);与电针A1组比较,电针B1组心肌细胞MMP升高(P<0.001),左心室Pink1、Parkin、p62蛋白表达及血清CK-MB、cTn-I含量降低(P<0.01)。结论:电针“内关”能改善大鼠MIRI,其机制可能通过Pink1/Parkin介导的线粒体自噬通路实现,在缺血期可以增强线粒体自噬来减轻心肌损伤,在再灌注期可以降低线粒体自噬来减轻再灌注损伤。 展开更多

目的 探讨按揉内关穴对不同脉息比值的抑郁症患者的临床疗效。方法 选取2024年4月-2025年3月在中山市第三人民医院中医科住院的60例中重度抑郁症患者作为此次研究对象。根据不同脉息比值将入选患者分为三组:脉息比值<4组、脉息比值4-... 目的 探讨按揉内关穴对不同脉息比值的抑郁症患者的临床疗效。方法 选取2024年4月-2025年3月在中山市第三人民医院中医科住院的60例中重度抑郁症患者作为此次研究对象。根据不同脉息比值将入选患者分为三组:脉息比值<4组、脉息比值4-5组、脉息比值>5组,每组20例。所有患者均接受常规的抗抑郁药物治疗和心理护理,同时在此基础上增加按揉内关穴的治疗。比较三组治疗前后抑郁症状、睡眠质量、中医证候积分,分析三组治疗前后脉息比值变化,统计三组抑郁症状及睡眠质量改善情况。结果 治疗后,脉息比值<4组的脉息比值低于脉息比值4-5组和脉息比值>5组(P<0.05),治疗后三组脉息比值均较治疗前降低(P<0.05),脉息比值<4组抑郁症状显著改善率高于脉息比值4-5组和脉息比值>5组(P<0.05),睡眠质量显著改善率高于脉息比值4-5组和脉息比值>5组(P<0.05)。结论 按揉内关穴对不同脉息比值的抑郁症患者具有显著的疗效,其中脉息比值较低者在接受按揉内关穴治疗后,抑郁症状、睡眠质量和中医证候改善程度优于脉息比值较高者。 展开更多

目的探讨内关-外关透刺针法对麻醉诱导期血流动力学及术后恶心呕吐(PONV)的影响。方法选择择期在全身麻醉(全麻)下行腹腔镜下三级手术的患者80例,随机分为观察组(40例)和对照组(40例)。观察组剔除3例,对照组剔除1例,实际纳入76例,其中... 目的探讨内关-外关透刺针法对麻醉诱导期血流动力学及术后恶心呕吐(PONV)的影响。方法选择择期在全身麻醉(全麻)下行腹腔镜下三级手术的患者80例,随机分为观察组(40例)和对照组(40例)。观察组剔除3例,对照组剔除1例,实际纳入76例,其中观察组37例,对照组39例。对照组给予常规麻醉诱导,观察组在常规麻醉诱导前行内关-外关透刺针法,右侧留针15 min,左侧留针30 min。两组患者均应用七氟烷复合瑞芬太尼维持麻醉。比较两组患者入室平卧5 min(T_(0))、麻醉诱导前(T_(1))、麻醉诱导后(T_(2))、气管插管前(T_(3))、气管插管后1 min(T_(4))的生命体征,入麻醉恢复室(PACU)(T_(5))、出PACU(T_(6))、术后6 h(T_(7))、术后24 h(T_(8))的视觉模拟评分法(VAS)评分、恶心呕吐发生情况及及恶心视觉模拟评分法(NVAS)评分,相关麻醉药物(枸橼酸舒芬太尼、七氟烷、丙泊酚、酮咯酸氨丁三醇)用量和满意率。结果观察组患者丙泊酚用量(71.8±28.4)mg明显少于对照组的(115.9±25.7)mg,满意率91.9%明显高于对照组的69.2%(P<0.05)。两组患者T_(0)、T_(1)、T_(2)时的收缩压(SBP)、心率(HR)比较差异无统计学意义(P>0.05);观察组T_(3)时SBP(88.5±9.8)mm Hg(1 mm Hg=0.133 kPa)、HR(61.3±10.7)次/min明显高于对照组的(82.0±9.0)mm Hg、(55.9±10.2)次/min,T_(4)时SBP(103.2±8.4)mm Hg、HR(66.9±10.3)次/min明显低于对照组的(124.5±13.8)mm Hg、(74.0±13.1)次/min(P<0.05)。观察组T_(0)时SBP、HR与本组T_(1)时比较差异无统计学意义(P>0.05);T_(2)、T_(3)、T_(4)时SBP、HR较本组T_(1)时明显降低(P<0.05)。对照组T_(0)、T_(4)时SBP、HR与本组T_(1)时比较差异无统计学意义(P>0.05);T_(2)、T_(3)时SBP、HR明显较本组T_(1)时明显降低(P<0.05)。与对照组比较,观察组患者T_(7)、T_(8)时恶心呕吐发生率、NVAS评分及T_(6)、T_(7)时VAS评分明显更低(P<0.05)。结论内关-外关透刺针法可以减轻麻醉诱导期血流动力学的剧烈波动,减少丙泊酚用量,降低术后疼痛程度、恶心呕吐发生率和严重程度,提高患者满意度。 展开更多

目的:探讨经皮穴位电刺激疗法(TEAS)对全麻腹腔镜胆囊切除术患者术后恶心呕吐(PONV)的防治效果及起效机制。方法:将50例行全麻腹腔镜下胆囊切除术伴PONV的女性患者随机分为治疗组和对照组,每组各25例。对照组予以盐酸氢吗啡酮治疗,治疗... 目的:探讨经皮穴位电刺激疗法(TEAS)对全麻腹腔镜胆囊切除术患者术后恶心呕吐(PONV)的防治效果及起效机制。方法:将50例行全麻腹腔镜下胆囊切除术伴PONV的女性患者随机分为治疗组和对照组,每组各25例。对照组予以盐酸氢吗啡酮治疗,治疗组在对照组基础上加用TEAS治疗。观察2组术后不同时间段[术后T1(≤6 h)、T2(>6 h,但≤12 h)、T3(>12 h,但≤24 h)、T4(>24 h,但≤48 h)]PONV发生例数及疼痛视觉模拟量表(VAS)评分、首次排气及首次排便时间、术前与术后24 h内血清胃肠激素[胃泌素(GAS)、胃动素(MTL)、血管活性肠肽(VIP)]及血清5-羟色胺(5-HT)的变化。结果:治疗组术后各时间段PONV发生例数均少于对照组,其中T2、T3、T4时间段发生例数分别为7、4、1例,对照组分别为15、11、6例,2组比较,差异均有统计学意义(P<0.05);治疗组4个时间段VAS评分均低于对照组(P<0.05);术后首次排气时间、首次排便时间均短于对照组(P<0.05);2组术后24 h内血清GAS、MTL水平均较术前下降,血清VIP、5-HT水平均较术前升高,且治疗组血清GAS、MTL水平下降幅度及血清VIP、5-HT水平升高幅度均小于对照组(P<0.05)。结论:TEAS刺激内关穴、合谷穴可有效减少行全麻腹腔镜下胆囊切除术患者PONV的发生,并减轻其其他并发症的严重程度,其机制可能与其可调节血清5-HT的水平有关。 展开更多
