Effects of 7.5% hypertonic saline on fluid balance after radical surgery for gastrointestinal carcinoma 被引量:11

AIM: To investigate the effects of 7.5% hypertonic saline on positive fluid balance and negative fluid balance, after radical surgery for gastrointestinal carcinoma. METHODS: Fifty-two patients with gastrointestinal carcinoma undergoing radical surgery were studied. The patients were assigned to receive either Ringer lactate solution following 4 mL/kg of 7.5% hypertonic saline (the experimental group, n = 26) or Ringer lactate solution (the control group, n = 26) during the early postoperative period in SICU. Fluid infusion volumes, urine outputs, fluid balance, body weight change, PaO2/FiO2 ratio, anal exhaust time as well as the incidence of complication and mortality were compared between the two groups. RESULTS: Urine outputs on the operative day and the first postoperative day in experimental group were significantly more than in control group (P<0.000001, P=0.000114). Fluid infusion volumes on the operative day and the first postoperative day were significantly less in experimental group than in control group (P= 0.000042, P= 0.000415). The volumes of the positive fluid balance on the operative day and during the first 48 h after surgery, in experimental group, were significantly less than in control group (P<0.000001). Body weight gain post-surgery was significantly lower in experimental group than in control group (P<0.000001). The body weight fall in experimental group occurred earlier than in control group (P<0.000001). PaO2/FiO2 ratio after surgery was higher in experimental group than in control group (P= 0.000111). The postoperative anal exhaust time in experimental group was earlier than in control group (P= 0.000006). The overall incidence of complications and the incidence of pulmonary infection were lower in experimental group than in control group (P= 0.0175, P= 0.0374). CONCLUSION: 7.5% hypertonic saline has an intense diuretic effect and causes mobilization of the retained fluid, which could reduce fluid infusion volumes and positive fluid balance after radical surgery for gastrointestinal carcinoma, as well as, accelerate the early appearance of negative fluid balance after the surgery, improve the oxygen diffusing capacity of the patients' alveoli, and lower the overall incidence of complications and pulmonary infection after the surgery. 展开更多

Background:Athletes have been shown to exhibit better balance compared to non-athletes(NON).However,few studies have investigated how the surface on which athletes train affects the strategies adopted to maintain balance.Two distinct athlete groups who experience different types of sport-specific balance training are stable surface athletes(SSA) such as basketball players and those who train on unstable surfaces(USA) such as surfers.The purpose of this study was to investigate the effects of training surface on dynamic balance in athletes compared to NON.Methods:Eight NON,eight SSA,and eight USA performed five 20-s trials in each of five experimental conditions including a static condition and four dynamic conditions in which the support surface translated in the anteroposterior(AP) or mediolateral(ML) planes using positive or negative feedback paradigms.Approximate entropy(Ap En) and root mean square distance(RMS) of the center of pressure(Co P) were calculated for the AP and ML directions.Four 3 × 5(group × condition) repeated measures ANOVAs were used to determine significant effects of group and condition on variables of interest.Results:USA exhibited smaller Ap En values than SSA in the AP signals while no significant differences were observed in the ML Co P signals.Generally,the negative feedback conditions were associated with significantly greater RMS values than the positive feedback conditions.Conclusion:USA exhibit unique postural strategies compared to SSA.These unique strategies seemingly exhibit a direction-specific attribute and may be associated with divergent motor control strategies. 展开更多

The objective of this study was to assess the occurrence of metabolic disorders in periparturient low milk production (〈 15 kg/d) dairy cows. Data were collected between April 2009 and April 2010 from randomly selected Holstein crossed breed dairy cow and heifer (30 cows ',and 10 heifers) on faculty farm of faculty of Natural Resources Rajamangala University of Technology Isan, Skon-nakon Campus. Blood samples were collected weekly from each cow, starting 2 weeks before expected calving until 4 weeks postpartum. Serum was analyzed for NEFA and BHBA. The critical threshold for [NEFA] was defined as: 〉 0.4 mmol/L when 〈 -7 to 0 DIM, and 〉 0.7 mmol/L when 3 DIM. Postpartum sub-clinical ketosis (SK) was defined using [BHBA] 〉 14.4 mg/dl. The prevalence of elevated NEFA was 82.5%, 27.5%, 27.5% 7.5% and 7.5% in the dry period, 1st, 2nd 3rd and 5th weeks of lactation, respectively. The prevalence of SK was 12.5% at 2nd weeks of lactation. High prevalence of elevated NEFA during dry period through first 2 weeks of postpartum indicated that periparturient dairy cows suffered some degree of negative energy balance (NEB). These resulted indicated that though, milk yield of these cows were not that high, NEB and ketosis are still major nutrient-related problems. 展开更多
