血清Troponin I,CK-MBmass,Myoglobin在急性心肌梗塞诊断中的应用评价 被引量:23

为了解CTnI,CK-MBmass和Mb指标对急性心肌梗塞(AMI)早期诊断、后期监测及预后判断的临床意义,采用微粒子化学发光法,对20例AMI患者、24例不稳定心绞痛(UA)患者、17例骨骼肌损伤(SM)患者和19例健康者(对照组)进行血清CTnI,C... 为了解CTnI,CK-MBmass和Mb指标对急性心肌梗塞(AMI)早期诊断、后期监测及预后判断的临床意义,采用微粒子化学发光法,对20例AMI患者、24例不稳定心绞痛(UA)患者、17例骨骼肌损伤(SM)患者和19例健康者(对照组)进行血清CTnI,CK-MBmass和Mb的测定,并对AMI患者胸病发生3~5小时,8~14小时,20~24小时,72小时和7天共五个时段进行了测定。结果:AMI患者发病早期CTnI,CK-MBmass和Mb的阳性检出率分别为65%,70%,70%,CTnI在UA患者组和SM患者组中未检出。CTnI,CK-MBmass和Mb测定值含量在AMI组中明显高于UA组和SM组,AMI患者组CTnI分别是UA组和SM组的84倍和91.63倍,CK-Mbmass分别是该二组的8.82倍和4.67倍,Mb分别是该二组的11.57倍和2.25倍,AMI组与UA组和SM组比较,有显著性差异(P<0.001)。AMI患者组中该三项指标的动态测定显示,在疾病发生早期(3~5小时)时,CTnI,CK-MBmass和Mb都有显著性升高,随时间延长而逐渐升高,高峰值期均在8~14小时段,72小时时Mb已基本回到正常。7天时血清中仍有CTnI的持续存在,但CK-MBmass已接近正常。以上显示,CTnI,CK-MBmass和Mb在早期诊断AMI时,都具有较高的敏感性,CTnI在AMI患者中的阳性检出率具有心肌特异性。 展开更多

The glassy carbon electrode modified with Mb-TiO2/MWCNTs films exhibited a pair of weU defined, stable and nearly reversible cycle voltammetric peaks. The electron transfer between Mb and electrode surface, Ks of 3.08 s^-1, was greatly facilitated in the TiO2/ MWCNTs film. The electrocatalytic reductions of hydrogen peroxide were studied, the apparent Michaelis-Menten constant is calculated to be 83.10 μmol/L, which shows a large catalytic activity of Mb in the TiO2/MWCNTs film to H2O2. 展开更多

Background:Acute kidney injury(AKI)is the main life-threatening complication of crush syndrome(CS),and myoglobin is accepted as the main pathogenic factor.The pattern recognition receptor retinoicacid-inducible gene I(RIG-I)has been reported to exert anti-viral effects function in the innate immune response.However,it is not clear whether RIG-I plays a role in CS-AKI.The present research was carried out to explore the role of RIG-I in CS-AKI.Methods:Sprague-Dawley rats were randomly divided into two groups:the sham and CS groups(n=12).After administration of anesthesia,the double hind limbs of rats in the CS group were put under a pressure of 3 kg for 16 h to mimic crush conditions.The rats in both groups were denied access to food and water.Rats were sacrificed at 12 h or 36 h after pressure was relieved.The successful establishment of the CS-AKI model was confirmed by serum biochemical analysis and renal histological examination.In addition,RNA sequencing was performed on rat kidney tissue to identify molecular pathways involved in CS-AKI.Furthermore,NRK-52 E cells were treated with 200μmol/L ferrous myoglobin to mimic CS-AKI at the cellular level.The cells and cell supernatant samples were collected at 6 h or 24 h.Small interfering RNAs(siRNA)was used to knock down RIG-I expression.The relative expression levels of molecules involved in the RIG-I pathway in rat kidney or cells samples were measured by quantitative real-time PCR(qPCR),Western blotting analysis,and immunohistochemistry(IHC)staining.Tumor necrosis factor-α(TNF-α)was d etected by ELISA.Co-immunoprecipitation(Co-IP)assays were used to detect the interaction between RIG-I and myoglobin.Results:RNA sequencing of CS-AKI rat kidney tissue revealed that the different expression of RIG-I signaling pathway.qPCR,Western blotting,and IHC assays showed that RIG-I,nuclear factor kappa-B(NF-κB)P65,p-P65,and the a poptotic marker caspase-3 and cleaved caspase-3 were up-regulated in the CS group(P<0.05).However,the levels of interferon regulatory factor 3(IRF3),p-IRF3 and the antiviral factor interferon-beta(IFN-β)showed no significant c hanges between the sham and CS groups.Co-IP assays showed the interaction between RIG-I and myoglobin in the kidneys of the CS group.Depletion of RIG-I could alleviate the myoglobin induced expression of apoptosis-associated molecules via the NF-κB/caspase-3 axis.C onclusions:RIG-I is a novel damage-associated molecular patterns(DAMPs)sensor for myoglobin and participates in the NF-κB/caspase-3 signaling pathway in CS-AKI.In the development of CS-AKI,specific intervention in the RIG-I p athway might be a potential therapeutic strategy for CS-AKI. 展开更多

Direct electrochemistry and electrocatalysis of myoglobin(Mb) were studied with Mb immobilized on dodecyltrimethylammonium bromide(DTAB) film modified carbon ceramic(CC) electrode.Cyclic voltammetry showed a pair of well-defined and nearly reversible redox peaks of Mb(Fe~Ⅱ/Fe~Ⅲ) at about—0.3 V vs.SCE(pH = 6.98).The currents of the redox peak were linear to scan rate,and rate constant(Ks) was estimated to be 3.03 s^(-1).The formal potential(E°') of Mb in the DTAB/CC electrodes shifted linearly with pH with a slope of -36.44 mV/pH,implying that the electron transfer between DTAB and CC electrodes is accompanied by proton transportation.The immobilized Mb exhibited excellent electrocatalytic response to the reduction of hydrogen peroxide(H2O2). 展开更多

The characteristic of Myoglobin as mimetic enzyme was studied and applied in the determination of HeO2 with o-phenylenediamine as substrate. The absorbanee obtained at 40 ℃ was found to be proportional to the concentration of H2O2 in the range of 2.0×10^-5- 2.0×10^-7 mol/L, with a correlation coefficient of 0.999 1. The proposed meth od is simple and highly sensitive to a detection limit of 2.7 × 10^-8 mol/L. The relative standard deviations were within 5% for the determination of different concentrations of H2O2. Satisfied results were obtained in the determination of H2O2 in rainwater by this method, and the recoveries were at the range of 97.2%-105.4%. 展开更多

Background: The possibility exists that cocaine use in the absence of frank clinical presentation of acute cardiac events may be associated with subclinical injury including the release of myoglobin which is a nonspecific marker for cardiac and skeletal muscle damage. Objectives: This investigation examined urine specimens for potential differences between cocaine use and the expression of myoglobin (a marker associated with cardiovascular damage, inflammation and oxidative stress). Methods:40 urine specimens were assayed for cocaine metabolites, creatinine, total protein (BSA) and myoglobin using ELISA and colorimetry. Results: We observed significant differences between male control and male cocaine positive urines for myoglobin. Interestingly, there was no statistically significant difference in females between control and cocaine positive urines. Conclusion: Differences in the urinary expression of myoglobin may be important in evaluating the gender based effects of cocaine use and may have potential clinical applications which may be related to gender differences in signs and symptoms of cocaine toxicity. 展开更多

In order to elucidate the phylogenetic relationship of fish, DNA and deduced amino acid sequences of myoglobin (Mb) were used for the phylogenetic analyses based on different approaches, namely, maximum likelihood (ML), neighbor joining (NJ), unweighted pair group method with arithmetic means (UPGMA) and maximum parsimony (MP) methods in comparison with the conventional molecular markers, mitochondrial cytochrome b (cyt-b) and cytochrome c oxidase subunit I (COI). The phylogenetic trees drawn based on Mb sequences were similar to those by the traditional classification based on the other molecular markers. The primary and secondary structures, as well as the modeled tertiary structures of Mbs were similar to each other, but were clearly distinguishable among those species. Such differences in structure would be associated with adaptation of Mb molecule to the physiological conditions of each species. These results suggest that Mb can be a molecular marker for the phylogenetic relationship of fish. 展开更多

Myoglobin produced by fermentation using engineered Komagataella phaffii is an important color additive in meat analogue products,but its allergenicity is poorly understood.Here,we initially searched the Allergen Online database and did not find any allergic or cross-reactive proteins in porcine myoglobin(PM).In vitro simulated digestion demonstrated that PM did not exhibit notable acid-base resistance or anti-digestion capabilities.However,sensitization was observed in BALB/c mice,including a significant increase in specific antibodies and biomarkers for allergic reactions,as well as alterations in gut microbiome and serum metabolome.Interestingly,the intensity of sensitization exhibited a negative correlation with the purity of PM.60%and 88%purities showed weaker sensitization compared to the ovalbumin control group.These allergic reactions were likely due to the non-myoglobin protein portion,highlighting the importance of purification processes and the urgent need to assess the allergenicity of this portion. 展开更多

目的:探讨床旁快速检测(POCT)心肌生化标志物血清肌酸激酶同工酶(CK-MB)、肌红蛋白(MYO)、肌钙蛋白I(cTnI)水平对急性非ST段抬高型急性冠脉综合征(NSTE-ACS)病人诊断及短期预后的预测价值。方法:选取2020年7月—2022年3月在绵阳市第三... 目的:探讨床旁快速检测(POCT)心肌生化标志物血清肌酸激酶同工酶(CK-MB)、肌红蛋白(MYO)、肌钙蛋白I(cTnI)水平对急性非ST段抬高型急性冠脉综合征(NSTE-ACS)病人诊断及短期预后的预测价值。方法:选取2020年7月—2022年3月在绵阳市第三人民医院急诊科就诊时心电图为非ST段抬高型的急性心源性疾病病人110例,使用POCT检测血清CK-MB、MYO、cTnI水平,根据最终诊断分为观察组(NSTE-ACS)及对照组(非NSTE-ACS)。分析POCT检测的cTnI、CK-MB、MYO对NSTE-ACS病人的诊断价值;应用二元Logistic回归模型分析NSTE-ACS的影响因素;观察随访30 d内病人发生并发症及死亡情况。结果:两组cTnI、CK-MB比较,差异均有统计学意义(P<0.05),两组MYO比较差异无统计学意义(P>0.05)。cTnI、CK-MB、MYO对NSTE-ACS诊断有一定诊断效能,但诊断效能差,受试者工作特征(ROC)曲线下面积(AUC)均<0.7。性别、年龄、cTnI、CK-MB、MYO、GRACE评分对NSTE-ACS无明显影响(P>0.05)。Logistic回归分析显示,cTnI、CK-MB、MYO三者对NSTE-ACS病人短期预后有预测价值。两组随访30 d内并发症发生及死亡情况比较,差异均无统计学意义(P>0.05)。结论:POCT对急性NSTE-ACS病人的短期预后一定预测价值,但预测价值不高,需要探索其他标志物或联合检测来提高诊断的准确性及预测病人预后。 展开更多

CK、NHR及MYO在STEMI诊断及院内心力衰竭预测中的作用

目的 探究肌酸激酶(CK)、中性粒细胞与高密度脂蛋白胆固醇比值(NHR)及肌红蛋白(MYO)在急性ST段抬高型心肌梗死(STEMI)诊断及院内心力衰竭(以下简称心衰)预测中的作用。方法 选取2021年8月至2023年8月广西壮族自治区南溪山医院收治的STEM... 目的 探究肌酸激酶(CK)、中性粒细胞与高密度脂蛋白胆固醇比值(NHR)及肌红蛋白(MYO)在急性ST段抬高型心肌梗死(STEMI)诊断及院内心力衰竭(以下简称心衰)预测中的作用。方法 选取2021年8月至2023年8月广西壮族自治区南溪山医院收治的STEMI患者117例作为研究对象(观察组),另选取本院同期进行体检的健康人群90名为对照组。比较2组NHR、CK、MYO水平;计算NHR、CK、MYO单独及联合检测对STEMI的诊断效果;根据心衰标准统计患者入院后并发心衰情况并将观察组患者分为心衰组与非心衰组,分析2组NHR、CK、MYO水平;计算NHR、CK、MYO单独及联合检测对STEMI并发院内心衰的预测效果。结果 观察组NHR、CK、MYO水平均高于对照组,差异有统计学意义(P <0.05);NHR、CK、MYO单独及联合检测诊断STEMI的受试者工作特征(ROC)曲线下面积(AUC)为0.894,高于NHR、CK、MYO单独检测(P <0.05);经统计,117例患者中,住院后未出现心衰者75例(非心衰组),继发心衰者42例(心衰组);心衰组NHR、CK、MYO水平均高于对照组,差异有统计学意义(P <0.05);NHR、CK、MYO单独及联合检测对STEMI并发院内心衰的预测AUC为0.854,高于NHR、CK、MYO单独检测(P <0.05)。结论 NHR、CK及MYO在STEMI及并发院内心衰患者中表达异常,三指标联合检测在诊断STEMI及预测院内心衰中有一定的预测价值,可为临床诊治提供可靠依据。 展开更多

目的探讨老年高血压合并心力衰竭患者血清心肌损伤标志物肌钙蛋白I(cTNI)、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、N末端B型脑钠肽前体(NT-proBNP)水平变化及其临床价值。方法选取2022年2月至2024年2月该院的老年高血压合并心力衰竭患... 目的探讨老年高血压合并心力衰竭患者血清心肌损伤标志物肌钙蛋白I(cTNI)、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、N末端B型脑钠肽前体(NT-proBNP)水平变化及其临床价值。方法选取2022年2月至2024年2月该院的老年高血压合并心力衰竭患者110例为高血压合并心力衰竭组,同期选取单纯心力衰竭患者110例为心力衰竭组,单纯高血压患者110例为高血压组。比较各组血清cTNI、MYO、CK-MB、NT-proBNP水平,采用多因素Logistic回归分析高血压合并心力衰竭的影响因素,绘制受试者工作特征(ROC)曲线分析血清cTNI、MYO、CK-MB、NT-proBNP对高血压合并心力衰竭的诊断效能。结果与高血压组比较,高血压合并心力衰竭组、心力衰竭组血清cTNI、MYO、CK-MB、NT-proBNP水平升高(P<0.05);与心力衰竭组比较,高血压合并心力衰竭组血清MYO、NT-proBNP水平升高(P<0.05)。多因素Logistic回归分析结果显示,NT-proBNP是高血压合并心力衰竭的独立影响因素(P<0.05)。ROC曲线结果显示,血清cTNI、MYO、CK-MB、NT-proBNP单独及联合诊断高血压合并心力衰竭的曲线下面积分别为0.768、0.729、0.687、0.904、0.905,灵敏度分别为96.4%、50.0%、79.1%、90.0%、90.9%,特异度分别为54.1%、100.0%、60.0%、78.2%、75.0%。结论血清cTNI、MYO、CK-MB、NT-proBNP在老年高血压合并心力衰竭患者中呈高表达,四者联合对高血压合并心力衰竭具有较高的诊断价值。 展开更多

目的探讨ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患者经皮冠状动脉介入(percutaneous coronary intervention,PCI)治疗后N末端B型脑钠肽前体(N-terminal B-type brain natriuretic peptide precursor,NT... 目的探讨ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患者经皮冠状动脉介入(percutaneous coronary intervention,PCI)治疗后N末端B型脑钠肽前体(N-terminal B-type brain natriuretic peptide precursor,NT-proBNP)、肌红蛋白(myoglobin,Mb)、肌酸激酶同工酶(creatine kinase isoenzyme,CK-MB)、肌钙蛋白(cardiac troponin I,cTnI)浓度与主要不良心血管事件(major adverse cardiovascular events,MACE)的关系。方法回顾性分析2021年3月至2023年3月首都医科大学附属北京友谊医院收治的128例STEMI患者的临床资料,患者均接受PCI治疗,治疗后随访6个月,根据患者是否发生MACE分为MACE组(n=33)和非MACE组(n=95)。收集患者临床资料,PCI治疗前及治疗后即刻检测血浆NT-proBNP、Mb、CK-MB浓度及血清cTnI浓度。采用多因素Logistic回归分析STEMI患者PCI治疗后发生MACE的影响因素。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)分析血浆NT-proBNP、Mb、CK-MB浓度及血清cTnI浓度预测STEMI患者PCI治疗后发生MACE的价值。结果单因素分析结果显示,MACE组体质量指数(body mass index,BMI)≥24 kg/m^(2)、多支病变、左心室射血分数(left ventricular ejection fraction,LVEF)<50%、就诊至球囊扩张时间(door to balloon time,D2B)≥90 min的患者比例较高,并且MACE组血浆NT-proBNP、Mb、CK-MB浓度及血清cTnI浓度高于非MACE组,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,BMI、多支病变、LVEF、D2B、血浆NT-proBNP、Mb、CK-MB浓度较高是STEMI患者PCI治疗后发生MACE的影响因素(P<0.05);ROC分析结果显示,血浆NT-proBNP、Mb、CK-MB浓度及血清cTnI浓度预测STEMI患者PCI治疗后发生MACE的曲线下面积(area under the curve,AUC)分别为0.821(95%CI:0.758~0.884)、0.757(95%CI:0.681~0.833)、0.896(95%CI:0.854~0.938)、0.906(95%CI:0.865~0.947)。结论STEMI患者PCI治疗后6个月内发生MACE可能与BMI、病变支数、LVEF、D2B等因素有关。另外,通过检测血浆NT-proBNP、Mb、CK-MB浓度及血清cTnI浓度对MACE的发生具有一定预测价值。 展开更多

BACKGROUND Crush syndrome refers to the traumatic rhabdomyolysis leading to a spectrum of disorders culminating in acute kidney injury.The burden of crush syndrome is high,and mortality can be as high as 20%.The significant bulk of knowledge is from old articles.Over the last 10 years new research has occurred on diagnosis and treatment in animal models.AIM To overview of crush syndrome and discuss the newer advances related to the pathogenesis and management of a patient with crush syndrome.METHODS The search of databases such as MEDLINE,Google Scholar,Web of Science,and EMBASE revealed 8226 articles.A thorough screening culminated in 83 crush syndrome articles included in this study.RESULTS Acute kidney injury in crush syndrome is currently thought to be due to iron retention.The management of crush syndrome has also been updated with antioxidants,and several gases are being used to treat crush syndrome.In the end,treatment of crush syndrome also includes mental,social,and physical rehabilitation for better outcomes.CONCLUSION The outcomes of crush syndrome have significantly improved with the introduction of newer treatment modalities,including antioxidants,hyperbaric oxygen therapy,and comprehensive mental,social,and physical rehabilitation. 展开更多

冻藏期间鳝鱼肉色泽及食用品质的变化规律

为研究鳝鱼肉冻藏的品质变化,该研究将鳝鱼片(4 cm×5 cm)于-18℃下冻藏,监测其在6个月内结构和性质的变化规律。结果表明:随着冻藏时间增加,L*值、b*值和a*值总体呈下降趋势,氧合肌红蛋白含量下降19.91%,高铁肌红蛋白含量增加21.9... 为研究鳝鱼肉冻藏的品质变化,该研究将鳝鱼片(4 cm×5 cm)于-18℃下冻藏,监测其在6个月内结构和性质的变化规律。结果表明:随着冻藏时间增加,L*值、b*值和a*值总体呈下降趋势,氧合肌红蛋白含量下降19.91%,高铁肌红蛋白含量增加21.92%;肌纤维结构松散、排列混乱,肌束间间隙增大,肌肉组织完整性被破坏;持水率下降8.73%,剪切力下降了65.42%;pH值在第1个月显著下降了18.5%(P<0.05),后随冻藏时间的延长逐渐上升;在冻藏期间,鳝鱼肉脂质发生氧化蛋白质发生降解,TBARS值从0.73 nmol/mg prot增至1.62 nmol/mg prot,TVB-N值从7.69 mg/100 g增至20.24 mg/100 g,在第6个月超出限值。综上:鳝鱼肉在-18℃下冻藏6个月,其色泽和食用品质逐渐发生劣变。该研究可为鳝鱼的冷冻保鲜及其预制调理产品的品质调控和储藏运输提供一定的理论依据。 展开更多

非微生物条件下高氧气调包装牛排贮藏期间肉色和关键脂质氧化产物的关系

以育肥牛的背最长肌牛排为研究对象,在排除微生物生长影响因素的前提下,探讨其在高氧气调包装冷藏条件下的肉色及关键脂质氧化产物变化规律,并分析这些产物与肉色变化之间的关系。结果表明:高氧气调包装后的牛排其肉色稳定性随贮藏时间... 以育肥牛的背最长肌牛排为研究对象,在排除微生物生长影响因素的前提下,探讨其在高氧气调包装冷藏条件下的肉色及关键脂质氧化产物变化规律,并分析这些产物与肉色变化之间的关系。结果表明:高氧气调包装后的牛排其肉色稳定性随贮藏时间延长逐渐降低,高铁肌红蛋白还原能力和氧气消耗率显著下降;牛排样品的脂质氧化程度和产物种类及含量均随贮藏时间延长显著增加,共检测到27种脂质氧化产物,主要包括以己醛、壬醛、庚醛、(E)-2-辛烯醛、(E)-2-壬烯醛为代表的醛类物质和以3-羟基丁酮、2,3-辛二酮为代表的酮类物质等。贮藏过程中脂质氧化产物含量的增加与肉色稳定性的下降有极强的相关性(P<0.001),其中,2,3-辛二酮、1-己醇、苯甲醛、3-己烯-2-酮、(E,E)-2,4-壬二烯醛和(E,E)-2,4-癸二烯醛6种物质与a*值的相关系数绝对值高达0.95及以上。通过逐步回归分析进一步建立了a*值与脂质氧化产物的关系模型。本研究排除了微生物生长对肉色的影响,明晰了脂质氧化产物与肉色变化的关系,可为生鲜牛肉肉色控制提供理论参考。 展开更多

二硫化钼-金-正方形阵列结构复合SERS基底的制备技术

肌红蛋白的检测对于预防急性心肌梗死具有重要意义。基于聚焦离子束、磁控溅射、滴涂三种方法相结合制备二硫化钼-金-正方形阵列结构以作为复合表面增强拉曼散射(SERS)基底,以用于肌红蛋白的高灵敏检测。采用聚焦离子束方法制备不同边... 肌红蛋白的检测对于预防急性心肌梗死具有重要意义。基于聚焦离子束、磁控溅射、滴涂三种方法相结合制备二硫化钼-金-正方形阵列结构以作为复合表面增强拉曼散射(SERS)基底,以用于肌红蛋白的高灵敏检测。采用聚焦离子束方法制备不同边长和不同间距的正方形阵列结构。当正方形边长为300 nm时,相邻正方形间距(504、424、344、264 nm)的变化使其纳米孔的状态由分离到刚接触至相互叠加,且阵列结构的一致性高。采用该SERS基底检测10^(−5)mol/L的R6G溶液,试验结果表明,与相邻正方形纳米孔为分离或叠加状态相比,边长300 nm、间距424 nm的正方形阵列结构(刚接触状态)检测R6G溶液的增强强度最高。其次,与Au-MoS_(2)-L1S2和Au-L1S2相比,MoS_(2)-Au-L1S2复合SERS基底检测10^(−6)mol/L R6G分子的拉曼强度最高,其检测R6G溶液拉曼强度的标准偏差为4.66%,且检测R6G溶液的分辨率为10^(−8)mol/L。此外,在生物蛋白分子检测方面,采用该复合SERS基底能够检测0.02μg/mL的肌红蛋白分子,实现了对肌红蛋白分子的高灵敏度检测。 展开更多
