NAFLD includes a wide spectrum of liver conditions ranging from simple steatosis to nonalcoholic steatohepatitis and advanced hepatic fibrosis. NAFLD has been recognized as a hepatic manifestation of metabolic syndrome linked with insulin resistance. NAFLD should be considered not only a liver specific disease but also an early mediator of systemic diseases. Therefore, NAFLD is usually associated with cardiovascular disease, chronic kidney disease, type 2 diabetes, obesity, and dyslipidemia. NAFLD is highly prevalent in the general population and is associated with increased cardiovascular morbidity and mortality. The underlying mechanisms and pathogenesis of NAFLD with regard to other medical disorders are not yet fully understood. This review focuses on pathogenesis of NAFLD and its relation with other systemic diseases.展开更多

However, the post-infectious immune dysregulation may result in the Multisystem Inflammatory Syndrome in Children (MIS-C). The most common presentations of MIS are fever, gastrointestinal (diarrhea, vomiting, abdominal pain), cardiovascular, mucocutaneous (rash, mucus membrane changes, conjunctival injection), respiratory (including sore throat), headache, limb and periorbital edema, and elevated inflammation markers. Some clinical and laboratory features of MIS-C are similar to other systemic diseases of childhood as Kawasaki disease and Toxic Shock Syndrome. Here are reported three cases in children with MIS-C, Kawasaki disease and Toxic Shock Syndrome to highlight the similarities and differences of these diseases.展开更多

A fleet of independently operating systems and a single maintenance unit axe considered. It is the objective in this paper to increase overall performance or workload respectively by avoiding delay due to busy maintenance units. This is achieved by concerted and coordinated derating of individual system performance, which increases reliability. Quantification is carried out by way of a convolution-based approach. The approach is tailored to fleets of ships, aeroplanes, spacecraft, and vehicles (trains, trams, buses, cars, trucks, etc.) - Finally, the effectiveness of derating is validated using different criteria. The RAS concept makes sense if average system output loss due to lowered performance level (yielding longer time to failure) is smaller than average loss due to waiting for maintenance in a non-adaptive case.展开更多

Scorpion envenoming causes ...Death due to scorpion envenoming syndrome is a common event in many of the tropical and non-tropical counties. Initial transient hypertension is commonly observed in scorpion sting victims. Scorpion envenoming causes autonomic storm resulting in initial transient hypertension followed by hypotension, cold clammy skin, hypothermia, cardiovascular disturbances, acute myocarditis, sarcolemmal defects, pulmonary oedema, acute pancreatitis, disseminated intravascular coagulation (DIC), Adult respiratory distress syndrome (ARDS), and many other clinical manifestations. All these manifestations could be due to sudden increase in catecholamines, angiotensin II, glucagon, Cortisol and either due to suppressed insulin secretion or insulin resistance and death. The sudden increase of metabolic A in counter-regulatory hormones along with either suppressed insulin secretion/or insulin resistance results in glycogenolysis in liver, cardiac and skeletal muscles causing hyperglycemia and a sudden increase in free fatty acid levels. Free Fatty Acids increase the susceptibility of the ventricles to the disorganized electrical behavior, inhibit cardiac sarcolemmal Na+-K+ ATPase activity, increase the tendency to intravascular thrombus, increase myocardial oxygen consumption, interfere with tropomyosin-troponin activation of Actin-Myosin coupling, show detergent effects on cell membranes and they could alter the stabilization of lysosomal membranes and probably become toxic to the myocardium. Based on our animal experiments in which insulin administration reversed the metabolic and ECG changes induced by scorpion envenoming and treating the poisonous scorpion sting victims with insulin, we consider that insulin has a primary metabolic role in preventing, counter-acting and reversing all the deleterious effects of FFA by inhibiting the catecholamine induced by lipolysis, and increasing intra-cellular K+, facilitating glucose transport to the myocardium and glucose metabolism through different pathways. Administration of insulin-glucose infusion to scorpion sting victims appears to be the physiological basis for the control of the metabolic response when that has become a determinant to survival. Treatment using continuous infusion of regular crystalline insulin should be given at the rate of 0.3 U/g glucose and glucose at the rate of 0.1 g/kg body weight/hour, for 48 - 72 hours, with supplementation of potassium as needed and maintenance of fluid, electrolytes and acid-base balance.展开更多

文摘Death due to scorpion envenoming syndrome is a common event in many of the tropical and non-tropical counties. 文摘In order to simulate the actual operation process of gas turbine more accurately, this paper uses multi system integration simulation technology to study the dynamic characteristics of gas turbine.Firstly, considering the factors such as variable stator vanes, air extraction, cooling mixing and multiple inertias,a refined model of gas turbine is established. Then,AMESim is used to establish the model of the VSV actuator of the gas turbine, and Flowmaster is used to establish the model of the secondary air system of the gas turbine. Finally, the multi-system integration simulation model of gas turbine is established by using interface technology to study the dynamic characteristics of gas turbine under load mutation. The simulation results show that the established multisystem simulation model of gas turbine can effectively simulate the dynamic operation process of gas turbine under the condition of load mutation, which is of great significance to the simulation research of gas turbine.
文摘Two years after the coronavirus disease 2019(COVID-19)pandemic,acute hepatitis of unknown etiology in children(AHUCD)began to be reported worldwide. The novel coronavirus and adenovirus were found in pathogen and antibody tests in AHUCD cases reported by the World Health Organization.Children are not exposed to the viruses that children are generally exposed to owing to COVID-19 infection preventive measures such as isolation and wearing masks;therefore, some researchers have speculated that this disease is related to reduced exposure to pathogens. Some scientists have also speculated that the disease is related to liver injury and adenoviral hepatitis, which are the sequelae of COVID-19.Some evidence also suggests a weak association between the disease and COVID-19 vaccination. Therefore, further research and investigation of the pathogenesis, preventive measures, and early treatment of hepatitis of unknown etiology are required. This study aimed to synthesize available evidence to further elucidate this disease in order to treat and prevent it effectively.

文摘Hand, foot, and mouth disease(HFMD)is a prevalent infectious affliction primarily affecting children, with a small portion of cases progressing to neurological complications.Notably,in a subset of severe HFMD cases, neurological manifestations may result in significant sequelae and pose a risk of mortality. We systematically conducted literature retrieval from the databases PubMed(1957–2023),Embase(1957–2023),and Web of Science(1957–2023),in addition to consulting authoritative guidelines.Subsequently,we rigorously selected the most relevant articles within the scope of this review for comprehensive analysis. It is widely recognized that the severity of HFMD is attributed to a multifaceted array of pathophysiological mechanisms. The implication of multi-system dysfunction appears to be perturbances of the human defense system;therefore,it contributes to the severity of HFMD.In this review,we provide an overview and analysis of recent insights into the molecular mechanisms contributing to the severity of HFMD,with a particular focus on cytokine release syndrome, the involvement of the renin-angiotensin system, regional immunity, endothelial dysfunction, catecholamine storm, viral invasion, and the molecular mechanisms of neurological damage. We speculate that the domino effect of diverse physiological systems, initiated by damage to the central nervous system, serve as the primary mechanisms governing the severity of HFMD.Simultaneously,we emphasize the knowledge gaps and research urgently required to delineate a quick roadmap for ongoing and essential studies on HFMD.

文摘In order to reduce the environmental impact of conventional sludge treatment methods and to utilize the energy in sludge more effectively,a coupled system based on sewage sludge gasifier(SSG),solid oxide fuel cells(SOFC),supercritical CO_(2)cycle(S-CO_(2)),and organic Rankine cycle(ORC)is proposed. The clean syngas obtained from sludge gasification is mixed with CH4 and then first utilized by the fuel cell. The exhaust gas from the fuel cell is fully combusted in the afterburning chamber and then enters the bottom cycle system consisting of S-CO_(2)&ORC to generate electricity. To understand the performance of the system, thermodynamic and economic analyses were conducted to examine the project's performance. The thermodynamics as well as the economics of the coupled system were analyzed to arrive at the following conclusions, the power production of the system is 37.34 MW;the exergy efficiency is 55.62%,and the net electrical efficiency is 61.48%.The main exergy destruction is the gasifier and SOFC,accounting for 62.45%of the total exergy destruction. It takes only6.13 years to repay the construction investment in the novel system, and the project obtains a NPV of 17723820USD during 20 years lifetime. The above findings indicate that the new coupled system has a better performance in terms of energy utilization and economy.

文摘Global navigation satellite system(GNSS)carrier phase observations are two orders of higher accuracy than pseudo-range observations, and they are less affected by multipath besides. As a result, the time transfer accuracy can reach 0.1 ns,and the frequency transfer stability can reach 1×10^-15 with carrier phase(CP)method, therefore CP method is considered the most accurate and promising time transfer technology. The focus of this paper is to present a comprehensive summary of CP method, with specific attention directed toward day-boundary clock jump, ambiguity resolution(AR),multi-system time transfer and real-time time transfer.Day-boundary clock jump is essentially caused by pseudo-range noise.Several approaches were proposed to solve the problem, such as continuously processing strategy, sliding batch and bidirectional filtering methods which were compared in this study.Additionally, researches on AR in CP method were introduced. Many scholars attempted to fix the single-difference ambiguities to improve the time transfer result, however, owing to the uncalibrated phase delay(UPD)was not considered, the current studies on AR in CP method were still immature. Moreover, because four GNSS systems could be used for time-transfer currently, which was helpful to increase the accuracy and reliability, the researches on multi-system time transfer were reviewed. What’s more,real-time time transfer attracted more attention nowadays, the preliminary research results were presented.
