Multiple acupuncture and moxibustion therapies for allergic rhinitis in adults:a network Meta-analysis of randomized controlled trials

OBJECTIVE:To evaluate the effectiveness and safety of multiple acupuncture therapies in the treatment of allergic rhinitis(AR)using a network Meta-analysis.METHODS:This network Meta-analysis adhered to the PRISMA-NMA guideline.Eight databases were systematically searched from inception to December 31,2023,and retrieved references were managed using End Note 20.The risk of bias in individual studies was assessed with the Cochrane Ro B 2.0 tool(ROB 2.0).Data analysis was performed using R 4.2.1 and STATA 15.1.RESULTS:A total of 56 studies that had enrolled 4859 patients with AR were included.Hand acupuncture(HA)combined with acupoint catgut embedding(AE),hand acupuncture with heat sensitive moxibustion(HA+HSM),and acupoint application were the most effective in attaining the clinical effective rate.HA+AE,hand acupuncture with indirect moxibustion(HA+IM),and electroacupuncture(EA)with AE ranked the highest with respect to the Rhinoconjunctivitis Quality of Life Questionnaire Score.HA+IM,HA,and HA+EA yielded the best Total Nasal Symptom Score,whereas HA+AE,EA+AE,and HA had the best results for the Total NonNasal Symptom Score.However,subgroup analyses of clinical efficacy revealed that HA+HSM had the best therapeutic effects in the short term,whereas HA+AE was the most effective in the medium to long term.CONCLUSION:Combined acupuncture and moxibustion therapy may be a safe and effective intervention for patients with AR.HA+IM and HA+AE exhibited the most desirable therapeutic effects compared with other acupuncture and moxibustion therapies. 展开更多

Objective: To explore the application effect of moxibustion combined with Sihuang Anti-inflammatory Lotion in patients with herpes zoster after bone marrow transplantation (BMT). Methods: One patient with herpes zoster after BMT was selected and treated with moxibustion combined with Sihuang Anti-inflammatory Lotion in addition to conventional antiviral drugs. Results: The patient’s pain disappeared within 48 hours after the intervention (NRS score decreased from 6 to 2), large-scale crusting of herpes occurred within 72 hours, and complete shedding occurred within 10 days. No infection or other adverse reactions occurred, and the patient had high nursing satisfaction. Conclusion: Moxibustion combined with Sihuang Anti-inflammatory Lotion can safely and effectively improve the symptoms of herpes zoster after BMT, which is worthy of clinical promotion. 展开更多

Mongolian medicine posits that disruptions to the natural balance of the three roots and seven elements within the human body may lead to ocular disorders,vision impairment,and ultimately myopia.China’s children and adolescents not only exhibit high myopia rates but also face increasingly prominent issues of younger onset and severe progression,which critically impact the nation’s future and require urgent attention.Myopia prevention constitutes a systematic project.Traditional Mongolian moxibustion therapy works by applying heat stimulation to specific acupoints to warm meridians,harmonize Qi-blood circulation,regulate elemental balance,thereby enhancing immunity for disease prevention.This holistic approach features non-invasive application with minimal side effects.However,current interventions in myopia management through this method still face challenges including inconsistent operational protocols and insufficiently systematic collaborative research.This paper reviews recent advancements in early intervention using Mongolian moxibustion therapy for myopia,providing insights to optimize myopia prevention strategies. 展开更多

Objective:To observe the effects of moxibustion on the hematopoietic function in mice with aplastic anemia(AA)induced by bone marrow(BM)suppression,and to investigate the intervention effects of moxibustion on AA from the perspective of intestinal bacteria.Methods:A total of 24 C57BL/6 J male mice were randomly and evenly divided into control,model and moxibustion groups.The myelosuppression-induced AA model was established by cyclophosphamide(CTX)and cyclosporine(Cs)intraperitoneal injection.Mice in the moxibustion group were intervened in mild moxibustion at unilateral“Zusanli(ST36)”acupoint for 15 min per day,and the sides were switched the next day.The intervention of mild moxibustion lasted 60 days consecutively.The red blood cell(RBC),white blood cell(WBC),platelet(PLT)counts and haemoglobin(Hb)concentration levels of mice in each group were detected by peripheral blood cells count staining,and the BM hematopoietic cells and hematopoietic structures were observed by BM smear Wright-Giemsa staining and HE staining.16SrDNA sequencing was used to analyze the gut bacterial species abundance and diversity in mice from each group.Results:After all the intervention,compared to the control group,the model group had lower levels of RBC,WBC,PLT counts and Hb concentration in peripheral blood(P<0.05)and fewer hematopoietic cells and hematopoietic structures;compared to the model group,moxibustion group had higher levels of RBC,WBC,PLT counts and Hb concentration in peripheral blood(P<0.05),and more BM hematopoietic cells and hematopoietic structures.Gut flora showed that moxibustion increased the species richness and diversity of intestinal bacteria in mice;compared with the control group,the relative abundance of Faecalibaculum and Anaeroplasmataceae in the model group was higher(P<0.05);whereas,the relative abundance of Faecalibaculum and Anaeroplasmataceae in the moxibustion group was lower(P<0.05)when compared with the model group.In addition,Faecalibaculum was significantly correlated with RBC,WBC,PLT count and Hb concentration(P<0.05).Conclusion:Moxibustion can improved BM histology,restored hematopoietic cells function,and increased peripheral blood cells count and Hb concentration in AA mice.The mechanism may be related to the fact that moxibustion regulates the abundance of specific intestinal bacteria to maintain the stability of the flora structure. 展开更多

OBJECTIVE:To investigate the effect and mechanism of mild moxibustion on the non-neuronal cholinergic system(NNCS) in rats with ulcerative colitis(UC).METHODS:UC rat model was established by administering 4% dextran sulfate sodium.After 7 d,mild moxibustion,α7 nicotinic acetylcholine receptors(α7nAchRs) antagonist(α-bungarotoxin,α-BGT),vesicular acetylcholine transport inhibitor(vesamicol hydrochloride,VH) and organic cation transporters inhibitor(quinine,Qu) treatments were performed once daily for 7 d.Haematoxylin and eosin staining was used for morphological evaluation of colon tissues.Enzymelinked immunosorbent assay(ELISA) was used to measure the protein expressions of interleukin-1β(IL-1β) and choline acetyltransferase(ChAT) in colon tissue.Reverse transcription quantitative real-time polymerase chain reaction(RT-q PCR) was used to detect the mRNA expressions of IL-1β,carnosine acetyltransferase(CarAT),ChAT,and nuclear factor kappa-B p65 subunit(NF-κB p65) in colon tissue.Western blot was used to detect NF-κB p65 protein expression in colon tissue.Immunofluorescence was used to detect the expressions of neuronal acetylcholine(nAch) and non-neuronal acetylcholine(nnAch,released by NNCS) in colon tissue.RESULTS:Mild moxibustion inhibited colon inflammation and repaired mucosal damage to the colon in UC rats.Meanwhile,mild moxibustion could downregulate the expressions of IL-1β,NF-κB p65 protein and mRNA(P < 0.01),and upregulate the expressions of ChAT protein and CarAT mRNA(P < 0.05,P < 0.01).The α7nAChR antagonist α-BGT can reverse the protective effect of mild moxibustion on the UC and the inhibitory effect on the inflammatory factors.VH cannot affect the effect of mild moxibustion on the expressions of IL-1β and nnAch,while Qu can reverse the effect of mild moxibustion on the expression of IL-1β and nnAch.CONCLUSIONS:Mild moxibustion can inhibite colon inflammation in UC rats,which is closely related to the release of acetylcholine by NNCS and its mediated mechanism of cholinergic anti-inflammation pathway. 展开更多

Objective To investigate the effects of moxibustion on wound healing and evaluate whether the efficacy of moxibustion at different temperatures varies.Additionally,to explore the mechanism by which moxibustion regulates the phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/mammalian rapamycin target protein(mTOR)signaling pathway on wound healing and cell apoptosis.Methods A total of 90 SPF-grade SD rats were randomly divided into six groups(15 rats per group):control group,model group,recombinant bovine basic fibroblast growth factor gel(rb-bFGF)group,moxibustion group A[temperature(42±1)℃],moxibustion group B[temperature(45±1)℃],and moxibustion group C[temperature(48±1)℃].Except the control group,the interventions were conducted in the other groups with pressure ulcer induced by ischemia/reperfusion injury.In the control group,povidone-iodine was administered at the knee joint.In the model group,after povidone-iodine applied at the pressure ulcer,petroleum jelly gauze was bandaged.In the rb-bFGF group,after povidone-iodine applied at the pressure ulcer,rb-bFGF was smeared and petroleum jelly gauze was bandaged.In the moxibustion group A,the suspending moxibustion with moxa stick was delivered 3 cm to 5 cm above the ulcer.The paperless temperature recorder probe was placed on the wound surface where moxibustion was delivered,and the real-time temperature was monitored and adjusted,kept at(42±1)℃.The moxibustion temperature was kept at(45±1)℃ in the moxibustion group B,and(48±1)℃ in the moxibustion group C.The intervention lasted 14 days in each group.The wound healing rates were compared on the 3,5,7 and 14 days after the first intervention among the 6 groups.After 14-day intervention respectively,the histopathological changes were observed with HE staining,apoptosis was detected with TUNEL method and flow cytometry;VEGF-A protein expression determined with immunohistochemistry;the contents of Interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and transforming growth factor-β(TGF-β)in serum with ELISA,and PI3K/AKT/mTOR related protein and mRNA expression was with Western blotting and qRT-PCR in wound tissues of each group.Results Regarding the intervention effects,(1)compared with the control group,the model group showed significant inflammatory infiltration in wound tissues,higher levels of cell apoptosis,and elevated serum levels of IL-6,TNF-αand TGF-β(P<0.05);(2)compared with the model group,the rb-bFGF group and 3 moxibustion groups showed higher wound healing rates(P<0.05),reduced inflammatory infiltration in the wound tissues,lower levels of cell apoptosis,and decreased serum levels of IL-6,TNF-αand TGF-β(P<0.05).Among the moxibustion groups,group C showed significantly better results than group A(P<0.01).Regarding the mechanism,(1)compared with the control group,the model group exhibited significantly higher protein and mRNA expression levels of PI3K/AKT/mTOR signal pathway components(P<0.05);(2)compared with the model group,the rb-bFGF group and 3 moxibustion groups showed lower protein and mRNA expression levels of PI3K/AKT/mTOR signal pathway components(P<0.05).Among the moxibustion groups,group C showed significantly higher expression levels than group A(P<0.01).Conclusion Moxibustion significantly promotes wound healing,with high-temperature moxibustion being more effective than low-temperature moxibustion in reducing the expression of inflammatory factors and cell apoptosis,thereby improving wound healing rates.It is presented that the higher moxibustion temperature,the higher wound healing rate,which may be associated with the regulation of PI3K/AKT/mTOR signal pathway with moxibustion. 展开更多

Integrated plasma and synovial membrane lipidomic profiling revealing the therapeutic effects of moxibustion in collagen-induced arthritis rat models

Objective To reveal the therapeutic effects of moxibustion in collagen-induced arthritis(CIA)rat models using the combined analysis of plasma and synovial membrane lipidomic profil-ing and to enhance the understanding of how moxibustion affects lipid metabolism in rheumatoid arthritis(RA).Methods A total of 32 male Sprague-Dawley(SD)rats were randomly assigned to four groups:control,moxibustion control(MC),model,and moxibustion model(MM)groups,with 8 rats in each group.CIA was induced in SD rats by two immunizations.The paw volume was mea-sured before the induction of CIA.Following induction,after assessing paw volume and arthritis index(AI)scores,the MC and MM groups received treatment at bilateral Shenshu(BL23)and Zusanli(ST36)acupoints for 10 min per acupoint.The intervention included three treatment courses,each spanning 6 d and followed by a 1-d interval.Paw volume and AI scores were assessed after each treatment course.After the completion of the three treatment courses,serum,plasma,synovial tissue,and ankle joint samples were collected.Enzyme-linked immunosorbent assay(ELISA)was employed to quantify the levels of interleukin(IL)-6 and tumor necrosis factor(TNF)-αin serum.Hematoxylin and eosin(HE)staining was per-formed for histopathological examination of the ankle joint tissues.Meanwhile,ultra-high-performance liquid chromatography coupled with Q-Exactive Orbitrap mass spectrometry(UHPLC-Q-Exactive Orbitrap MS)was utilized to analyze the plasma and synovial tissue sam-ples.In addition,multivariate statistical analysis was performed to identify differential lipid metabolites,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis was applied to explore metabolic pathways modulated by moxibustion therapy.Results No significant difference in hind paw volume and AI scores was observed among the groups(P>0.05).After CIA induction,model group showed increased hind paw volume and AI scores compared with control group(P<0.05),which were significantly reduced after mox-ibustion treatment in MM group compared with model group(P<0.05).The levels of IL-6 and TNF-αwere significantly higher in model and MM groups compared with control group(P<0.05),but were lower in MM group than those in model group(P<0.05).Histopathologi-cal analysis showed improved cartilage and reduced inflammation in MM group.A total of 33 differential lipid metabolites in the plasma and 24 in the synovial membranes of CIA rat mod-els were identified when compared with control group.Among these lipid metabolites,31 in the plasma and all 24 in the synovial membranes were regulated by moxibustion treatment.Pathological analysis revealed upregulation of diacylglycerol(DG)and fatty acid(FA)levels,alongside downregulation of lysophosphatidylcholine(LPC),phosphatidylcholine(PC),and phosphatidylethanolamine(PE).Under physiological conditions,the treatment specifically reduced LPC and PC levels.Pathway enrichment analysis revealed that moxibustion predom-inantly affectedα-linolenic acid,glycerophospholipid,and sphingolipid metabolism under pathological conditions.Under physiological conditions,the regulation was centered aroundα-linolenic acid and glycerophospholipid metabolism.Conclusion The RA rat models exhibited significant lipid metabolic disturbances.Moxibus-tion alleviated paw swelling,reduced AI scores,modulated inflammatory cytokine levels,and partially corrected the altered levels of multiple lipid metabolites.The potential metabolic pathways implicated in the regulation of lipid metabolism under both physiological and pathological conditions includeα-linolenic acid,glycerophospholipid,and sphingolipid metabolism. 展开更多

Objective:The preventive and therapeutic effects of direct moxibustion on a gastric cancer rat model induced by the intragastric administration of N-methyl-N’-nitro-N-nitrosoguanidine(MNNG)were evaluated.Changes in the co-stimulatory molecules CD80 and CD86 on antigen-presenting cells in gastric tissues as well as related cytokines in serum were evaluated.The aim of the study was to explore the immunological mechanisms by which direct moxibustion may prevent gastric cancer lesions,thereby providing a basis for studies on the immunological mechanisms by which moxibustion prevents tumor development.Methods:Sixty healthy male Wistar rats were randomly divided into four groups:Control,control+moxibustion,model,and moxibustion groups.A gastric cancer rat model was induced by intragastric administration of 20 mg/mL MNNG,with a dose of 1 mL/100 g body weight,once daily for 16 weeks.The control+moxibustion and moxibustion groups received direct moxibustion simultaneously with modeling,continuing for 16 weeks.After the experiment,gastric tissue was collected,and morphological changes in the gastric mucosa in each group of rats were observed through H&E staining.Immunohistochemistry(IHC)and a western blotting were used to detect the expression levels of CD80 and CD86 in gastric tissues.Enzyme-linked immunosorbent assays(ELISA)were used to measure the levels of interleukin-12(IL-12),interferon-gamma(IFN-γ),and tumor necrosis factor-beta(TNF-β)in rat serum.Results:Upon macroscopic observation,the gastric mucosa of rats in the control and control+moxibustion groups appeared uniformly red,with a glossy mucosal surface,normal gastric wall elasticity,and clear,regular mucosal folds,without hyperplasia or bleeding points.In the model group,the gastric mucosa was reduced in volume,the gastric wall thinned,elasticity decreased,mucosal folds were disordered,and yellow-white cauliflower-like lesions and yellow-brown hyperkeratosis were observed.In the moxibustion group,the gastric mucosa showed decreased elasticity,with disordered mucosal folds and granular hyperplasia.After H&E staining,the gastric mucosal structure was clear and intact in the control and control+moxibustion groups displaying an organized and uniform arrangement of the mucosa,submucosa,and muscularis propria,without hyperplasia or keratinization.In the model group,the epithelial glands in the gastric mucosa were disordered,with varied cell morphologies,thickened submucosa,and disrupted squamous epithelium that invaded downward into the muscularis propria.In the moxibustion group,the squamous epithelium did not invade the muscularis propria.IHC results showed higher expression levels of CD80 and CD86 in the gastric mucosa of the control+moxibustion group than in the control group(P<0.05)and lower expression levels in the model group than in the control group(P<0.05).The moxibustion group showed higher CD80 and CD86 levels than those in the model group(P<0.05).Western blotting indicated that CD80 and CD86 levels were higher in the moxibustion group than in the model group(P<0.05).ELISA results showed higher IL-12 levels in the model group than in the control group(P<0.05)and higher TNF-βand IFN-γlevels in the moxibustion group than in the model group(P<0.01).Conclusion:Direct moxibustion alleviates the pathological progression of gastric cancer in an MNNGinduced rat model.Its mechanisms may involve effects on the state of antigen-presenting cells,thereby promoting T cell activation and enhancing immune function. 展开更多

Objective:In this study,we evaluated the methodological quality of systematic reviews on acupuncture and moxibustion for the prevention and management of cancer-related conditions,aiming to provide evidence-based support and establish levels of evidence for clinical application.Method:A comprehensive search was conducted to identify systematic reviews on acupuncture and mox-ibustion for the prevention and management of cancer-related conditions,covering databases including PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infrastructure,Chinese Biomedical Databases,China Science and Technology Journal Database,and Wanfang Data,from their in-ception to August 1,2024.The methodological quality of the included systematic reviews was assessed by using AMSTAR 2,and bubble plots were employed to visualize the evidence.Results:Overall,137 systematic reviews were included,with China being the most prolific country,with 117 reviews.These reviews addressed 21 cancer-related conditions,with the three most frequently stud-ied being cancer-related fatigue(25 reviews),nausea and vomiting(20 reviews),and cancer pain(19 re-views).The three most commonly used acupuncture modalities were manual acupuncture(81 reviews),electroacupuncture(55 reviews),and moxibustion(39 reviews).Quality assessment revealed that one systematic review was rated as“high quality,”eight as“low quality,”and 128 as“critically low quality,”primarily due to the absence of protocol registration and lists of excluded studies.Conclusion:Acupuncture and moxibustion have shown potential efficacy in alleviating cancer-related con-ditions and treatment-related adverse effects,and their clinical use has been widely reported.However,the overall methodological quality of the included systematic reviews was low,limiting the reliability and the strength of the evidence.Future high-quality studies are needed to improve the quality of evidence and support the standardized application and broader promotion of acupuncture and moxibustion clinical practice. 展开更多

OBJECTIVE:To comprehensively evaluate randomized controlled trials(RCTs)investigating the effects of moxibustion on people with human immunodeficiency virus(HIV)disease.METHODS:A systematic search was conducted across eight electronic databases up to August 20,2024.The primary outcome were all-cause mortality and acquired immunodeficiency syndrome(AIDS)-related mortality.Two authors independently screened titles and abstracts,and extracted data onto a pre-designed datasheet.Discrepancies were resolved through consensus.The Cochrane risk-of-bias tool 2.0 was used to assess methodological quality,Meta-analysis was performed when appropriate,and the quality of evidence was assessed through Grading of Recommendations,Assessment,Development and Evaluations approach.RESULTS:Eleven RCTs(n=834)on moxibustion for HIV/AIDS were included,focusing on individuals with HIV infection(5 RCTs,n=426),AIDS patients(3 RCTs,n=223),or both(3 RCTs,n=185).Complications identified included diarrhea(5 RCTs),pulmonary infection(1 RCT),anxiety and depression(1 RCT)and peripheral neuropathy(1 RCT).The risk of bias in the included RCTs was assessed as either high or uncertain.No trial reported mortality or the incidence of AIDS-related complications following treatment.Wheat-grain sized cone moxibustion was associated with increased CD4+counts in patients with lung infections[1 RCT,n=36,mean difference(MD)=78.83 cells/μL].Individual studies reported improvements of quality of life,as measured by the World Health Organization Quality of Life-Brief Version(WHOQOL-BREF)and WHOQOL HIV instrument,with various moxibustion types,but clinical heterogeneity prevented data pooling.Additionally,some studies reported symptom improvement,each using different criteria for symptom improvement.Moxa stick moxibustion plus Western Medicine compared with Western Medicine alone showed a non-significant trend towards improved symptom resolution[2 RCTs,n=125,risk ratio=1.19,95%confidence interval(0.99,1.43)].Moxibustion plus antiretroviral therapy(ART)may reduce gastrointestinal adverse events compared to ART alone[1 RCT,n=10014%vs 32%,P<0.05].The quality of evidence was low to very low.CONCLUSION:This systematic review suggests that moxibustion as an adjunct therapy may have potential benefits in improving immune function and quality of life for HIV/AIDS patients.Limited quality of evidence precludes definitive conclusions,and further high-quality research is needed. 展开更多

OBJECTIVE:To observe the effects of moxibustion at Zusanli(ST36)on rats with chronic fatigue syndrome(CFS)and to analyze the mechanisms of moxibustion through hippocampal Proteomics.METHODS:Male Sprague-Dawley(SD)rats were randomly divided into three groups:control group(CON),model group(MOD),and moxibustion group(MOX),with 12 rats in each group.The MOD and MOX groups underwent chronic multi-factor stress stimulation for 35 d to establish the CFS model.After modeling,the rats in the MOX group received mild moxibustion at Zusanli(ST36)(bilateral)for 10 minutes daily for 28 d.During the treatment period,rats in both the MOD and MOX groups continued modeling,while the CON group was kept under normal breeding conditions.The general condition of the rats was monitored,and behaviors were assessed using the Open Field Test(OFT),Exhaustion Treadmill Test,and Morris Water Maze(MWM).Hematoxylin and eosin(HE)staining and transmission electron microscopy(TEM)were employed to observe morphological changes in the hippocampus.Label-free Proteomics were utilized to identify differentially expressed proteins(DEPs)in the hippocampus,followed by bioinformatics analysis.The reliability of the Proteomics results was verified using Parallel Reaction Monitoring.RESULTS:A:Moxibustion at Zusanli(ST36)significantly reduced the general condition score of CFS rats,improved their behavioral performance in OFT,treadmill and MWM,and repaired the pathological and synaptic structural damage in the hippocampus.B:We identified DEPs by applying a fold change threshold of 1.2 and a significance level of P<0.05.In the comparison between the CON and the MOD,we identified a total of 72 DEPs(31 up-regulated and 41 down-regulated)associated with the development of CFS.In the comparison between the MOX and the MOD group,we identified a total of 103 DEPs(40 up-regulated and 63 down-regulated)related to the therapeutic effects of moxibustion.Gene Ontology(GO)enrichment analysis showed that CFS and moxibustion treatment were related to multiple biological processes,molecular functions,and cellular components.Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis revealed that CFS pathogenesis was linked to base excision repair,steroid biosynthesis,and systemic lupus erythematosus,Furthermore,the treatment of CFS with moxibustion was relevant to terpenoid skeleton biosynthesis.C:Compared with the two comparison groups,we identified 16 potential biomarkers,noting that moxibustion reversed the upregulation of 14 DEPs and the down-regulation of 2 DEPs in CFS.These proteins are mainly associated with synaptic plasticity,ribosomal function,neurotransmitter secretion,glycine metabolism,and mitochondrial function.CONCLUSION:Moxibustion at Zusanli(ST36)is effective in treating CFS,the potential biomarkers identified by Proteomics confirm that the mechanisms of moxibustion involve multiple targets and pathways,which may be key to regulating the structural and functional damage in the hippocampus associated with CFS,highlighting their significant value for future research. 展开更多

Background:There is a gap in understanding the effects of different acupoints and treatment methods(acupuncture and moxibustion)on microcirculatory changes in the lumbar region.Objective:This study aimed to assess the thermal effects of acupuncture at Weizhong(BL40),with acupuncture at Chize(LU5)and moxibustion at both acupoints as control interventions.Design,setting,participants and interventions:In this randomized controlled trial,140 healthy participants were equally divided into four groups:acupuncture at BL40(Acu-BL40),acupuncture at LU5(Acu-LU5),moxibustion at BL40(Mox-BL40)and moxibustion at LU5(Mox-LU5).Participants underwent a 30-minute session of their assigned treatment.Infrared thermal imaging was used to collect temperature data on the areas of interest for analysis.Main outcome measures:The primary measure was the change in average temperature of the observed area after the intervention.The secondary measures included periodic temperature changes every5 min and the temperature changes of the Governor Vessel and Bladder Meridian in the observed area after the intervention.Results:Significant interactions were observed between treatments and acupoints affecting temperature(P<0.001).The Acu-BL40 group showed a notably higher increase in mean temperature after 30 min compared to the Acu-LU5 and Mox-BL40 groups,with increases of 0.29(95%confidence interval[CI]=0.17 to 0.41)and 0.24(95%CI=0.08 to 0.41)℃,respectively.Conclusion:Acupuncture at BL40 acupoint can significantly increase the mean temperature in the observed area,highlighting the specific thermal effect of acupuncture compared to moxibustion in the lumbar area.This suggests a potential therapeutic benefit of acupuncture at BL40 for managing lumbar conditions. 展开更多

Objective To investigate the mechanism of in alleviating colonic mucosal inflammation in ten-eleven translocation(TET)protein 2 gene knockout(TET2^(-/-))mice with ulcerative colitis(UC)by regulating DNA methyltransferase(DNMT)and DNA hydroxymethylase.Methods Male specific pathogen-free(SPF)grade C57BL/6J wild-type(WT)mice(n=8)and TET2^(-/-)mice(n=20)were used to establish UC models by freely drinking 3%dextran sulfate sodium solution for 7 d.After UC model validation through histopathological examination in two mice from each type,the remaining mice were divided into four groups(n=6 in each group):WT model(WT+UC),TET2^(-/-)model(TET2^(-/-)+UC),TET2^(-/-)mild moxibustion(TET2^(-/-)+MM),and TET2^(-/-)electroacupuncture(TET2^(-/-)+EA)groups.TET2^(-/-)+MM group received mild moxibustion on Tianshu(ST25)and Qihai(CV6)for 10 min daily for 7 d.The TET2^(-/-)+EA group also applied electroacupuncture(1 mA,2/100 Hz)at the same acupoints for 10 min daily for 7 d.The disease activity index(DAI)scores of each group of mice were accessed daily.The colon lengths of mice in groups were measured following intervention.The pathological changes in the colon tissues were observed with hematoxylin and eosin(HE)staining.The concentrations of interleukin(IL)-6,C-C motif chemokine 17(CCL17),and C-X-C motif chemokine ligand 10(CXCL10)in serum were detected by enzyme-linked immunosorbent assay(ELISA).The expression of DNMT proteins(DNMT1,DNMT3A,and DNMT3B)in the colon tissues was detected by immunohistochemistry.The expression of 5-methylcytosine(5-mC),5-hydroxymethylcytosine(5-hmC),histone deacetylase 2(HDAC2),and DNA hydroxymethylase family proteins(TET 1 and TET3)was detected using immunofluorescence,which also determined the co-localization of TET1 and IL-6 protein.Results Compared with WT+UC group,TET2^(-/-)+UC group exhibited significantly higher DAI scores and shorter colon lengths(P<0.01).Both mild moxibustion and electroacupuncture significantly decreased DAI scores and ameliorated colon shortening in TET2^(-/-)mice(P<0.001).Histopathological scores of TET2^(-/-)+UC mice were significantly higher than those of WT+UC group(P<0.001)and were significantly reduced after both mild moxibustion and electroacupuncture interventions(P<0.001).Serum levels of IL-6,CCL17,and CXCL10 were significantly elevated in TET2^(-/-)+UC group compared with WT+UC group(P<0.001).Mild moxibustion significantly reduced IL-6,CCL17,and CXCL10 levels(P<0.001,P<0.001,and P<0.01,respectively),while electroacupuncture also significantly reduced IL-6,CCL17,and CXCL10 levels(P<0.05,P<0.01,and P<0.01,respectively).TET2^(-/-)+UC mice showed increased expression levels of DNMT1,DNMT3A,DNMT3B,and 5-mC(P<0.05,P<0.01 and P<0.001,respectively),with decreased expression levels of TET1,TET3,5-hmC,and HDAC2(P<0.001).Mild moxibustion significantly reduced DNMT1,DNMT3B,and 5-mC levels(P<0.05,P<0.01,and P<0.001,respectively),while increasing expression levels of TET1,TET3,5-hmC,and HDAC2(P<0.001,P<0.001,P<0.05,and P<0.001,respectively).Electroacupuncture significantly decreased 5-mC and DNMT3B levels(P<0.001 and P<0.01,respectively)and increased 5-hmC and HDAC2 levels(P<0.05 and P<0.001,respectively),but did not significantly affect TET1 and TET3 expression(P>0.05).Compared with TET2^(-/-)+MM group,TET2^(-/-)+EA group showed significantly higher 5-mC expression(P<0.001).TET2^(-/-)+UC group exhibited markedly increased IL-6 expression and higher co-localization of TET1 and IL-6 in mucosal epithelium,whereas minimal IL-6 expression was observed in the other groups.Conclusion Mild moxibustion and electroacupuncture significantly ameliorate colonic inflammation exacerbated by TET2 deficiency in UC mice via epigenetic modulation.Distinct mechanisms exist between the two interventions:mild moxibustion regulates both DNMT and hydroxymethylase,whereas electroacupuncture primarily affects DNMT. 展开更多

Clinical Application of Denghuo Moxibustion in Treating Post-Stroke Constipation

Objective:To study the clinical application of Denghuo moxibustion in treating post-stroke constipation.Methods:This study included 50 patients with post-stroke constipation admitted from October 2020 to December 2021.They were randomly divided into two groups using the envelope method:25 patients in the observation group and 25 patients in the control group.The control group received only lactulose,while the observation group received Denghuo moxibustion in addition to lactulose.The efficacy,quality of life(PAC-QOL scale),and laboratory indicators[serum substance P(SP)and vasoactive intestinal peptide(VIP)]were compared between the two groups.Results:The efficacy of the observation group was significantly better than that of the control group(P<0.05).After treatment,the PAC-QOL scores of both groups decreased significantly,and the improvement in the observation group was more significant(P<0.05).Changes in laboratory indicators showed that VIP decreased significantly and SP increased significantly in both groups,and the improvement of these two indicators in the observation group was significantly better than that in the control group(P<0.05).Conclusion:Denghuo moxibustion can enhance the efficacy of lactulose by regulating intestinal neuropeptides and is an effective treatment for post-stroke constipation. 展开更多

Objective To observe the effects of moxibustion at Huantiao(GB30)acupoint on nerve repair,regeneration,and function in rats with sciatic nerve injury(SNI),and explore the possible mechanism of SNI improvement via moxibustion.Methods A total of 70 specific pathogen-free(SPF)grade male Sprague-Dawley(SD)rats were randomly assigned to control group(n=10)and model group(n=60).Following replication of SNI to model group rats,60 SNI model rats were randomly allocated to SNI groups of 1 d,3 d,and 7 d and moxibustion groups of 1 d,3 d,and 7 d with 10 rats in each group.Moxibustion groups were given moxibustion at the Huantiao(GB30)acupoint on the affected side with a 5 cm distance from the skin under isoflurane respiratory anesthesia and treated once a day for 20 min for 1 d,3 d,and 7 d,respectively.Control and SNI groups were anesthetized with isoflurane daily for 20 min.Open field tests and thermal pain threshold tests were conducted,and the general condition of rats was observed in each group pre-modeling and on treatment day 1,3,and 7.At the end of the treatment,immunofluorescence was used to detect the axonal growth rate,axonal growth density,and Schwann cells(SCs)proliferation in the middle 1-mm cross-section of the crush injury segment in rats.The gastrocnemius muscles on both sides of the rats were taken and weighed to calculate the wet weight ratio of the gastrocnemius muscles on both sides to observe the muscle atrophy of the rats,and hematoxylin-eosin(HE)staining was used to observe the pathomorphological changes of the gastrocnemius muscles on the affected side.Quantitative real-time polymerase chain reaction(qPCR)was used to detect the expression levels of nerve growth factor(NGF),interferon(IFN),macrophage migration inhibitory factor(MIF),interleukin(IL)-4,and transforming growth factor(TGF)-βin the sciatic nerve tissue of the rats.Results After modeling,rats in both moxibustion and SNI groups showed typical signs of pain behaviors(bending and curling of the hind soles of the affected side,licking claws,and lameness)and decreased activity compared with control group.The main benefits of moxibustion were evident from day 3:compared with SNI group,rats in moxibustion group had marked relief of pain behavior,increased activity levels and movement,and a lower response to thermal pain.At the same time,moxibustion significantly promoted the repair of SNI,as evidenced by the significantly better axonal growth rate,growth density,and SCs proliferation density in the crush injury segment compared with SNI group(P<0.01).Moxibustion also regulated the local microenvironment of the injury,up-regulated the pro-nerve repair factors NGF,IL-4,and TGF-β(P<0.05),and down-regulated the pro-inflammatory factors IFN-γ(P<0.01)and MIF(P<0.05).By day 7,the histomorphology of the gastrocnemius muscle in moxibustion group was improved,as indicated by enlarged muscle fibers,elevated regular myocyte morphology and wet weight ratio of the affected and unaffected sides(P<0.05),as well as a sustained high expression levels of NGF,IL-4,and TGF-β(P<0.05,P<0.05,and P<0.01,respectively),and a maintenance of low level of IFN-γ(P<0.01).Concurrently,the MIF level was not significantly different from SNI group(P>0.05).Conclusion Moxibustion at the Huantiao(GB30)acupoint effectively improves motor function and promotes recovery of sensory function and nerve regeneration in SNI rats,which may be related to the regulation of local inflammatory response,the promotion of nerve growth factor expression,the improvement of regenerative microenvironment,and the acceleration of SCs proliferation and axonal growth rate in damaged nerves. 展开更多

BACKGROUND Cancer-induced incomplete bowel obstruction presents numerous clinical cha-llenges,notably pain management and inflammation control.Conventional thera-pies provide limited relief,prompting investigation into complementary approa-ches.This study compares the efficacy of bottle gourd moxibustion combined with umbilical therapy(BGM-UT)plus standard palliative care vs standard Wes-tern palliative care alone in symptom alleviation and inflammation reduction.AIM To compare the efficacy of BGM-UT plus standard palliative care vs standard pa-lliative care alone in reducing inflammatory cytokine levels,alleviating symp-toms,and improving gastrointestinal recovery in cancer-induced incomplete bowel obstruction.METHODS A retrospective analysis was conducted on 109 patients aged 18-75 years with cancer-induced incomplete bowel obstruction treated at the Foshan Hospital of Traditional Chinese Medicine between October 2023 and September 2024.The participants were categorized into two groups:(1)Regular group receiving stan-dard palliative care;and(2)BGM-UT group receiving additional moxibustion therapy.The inclusion criteria included ongoing opioid use and a Karnofsky per-formance status score below 60.The treatments were evaluated for 3 weeks in terms of opioid consumption,gastrointestinal function recovery,brief pain inven-tory scores,traditional Chinese medicine symptom scores,and inflammatory cytokine levels.RESULTS Initially,the groups were demographically and clinically comparable.Post-treatment,the BGM-UT group showed significant reductions in opioid intake(P=0.027),improved gastrointestinal recovery times,and enhanced pain management as reflected by their lower brief pain inventory scores(P<0.05).Similarly,the improvement in traditional Chinese medicine scores was greater in the BGM-UT group than in the regular group(P<0.05).Inflam-matory markers including interleukin-6,tumor necrosis factor-α,and C-reactive protein decreased significantly in the BGM-UT group,indicating the superior anti-inflammatory effects of this treatment(P<0.05).CONCLUSION The addition of BGM-UT to standard palliative care enhances pain relief,accelerates gastrointestinal recovery,and effectively reduces inflammatory cytokine levels in patients with cancer-induced incomplete bowel obstruction.This combination therapy offers a promising complementary approach to managing this challenging condition.Further prospective studies are warranted to validate these findings and explore underlying mechanisms. 展开更多

[Objectives]To discuss the curative effect of the external application of self-prepared Chinese prescription decoction combined with gabapentin and cotton moxibustion on postherpetic neuralgia(PHN)and analyze its mechanism.[Methods]96 patients with PHN were divided into the control group(group A)and observation group(group B)(n=48).In group A,the patients were given gabapentin orally and treated by cotton moxibustion.In group B,the patients were treated by the external application of self-prepared Chinese prescription decoction on the basis of the above basic treatment.The total effective rate,apparent time and recurrence rate of the two groups were compared.[Results]The cure rate(72.91%)and total effective rate(95.83%)of patients in group B were higher than those in group A(54.16%,83.33%).The average apparent time[(5.79±1.40)d]and recurrence rate(5.97%)of patients in group B were significantly lower than those in group A[(7.03±4.37)d,10.45%],and the difference between the two groups was statistically significant(P<0.05).[Conclusions]The external application of self-prepared Chinese prescription decoction combined with gabapentin and cotton moxibustion therapy had a significant curative effect and low recurrence rate. 展开更多
