BACKGROUND Motoric cognitive risk(MCR)syndrome represents an“ultra-early”stage of dementia prevention, highlighting the need for effective screening tools.AIM To develop and validate a novel tool for MCR identification, comparing its effectiveness with existing methods.METHODS As part of a community study on healthy aging,a cross-sectional study recruited 1189 Chinese participants aged 50 years and older between May 1,2022,and March 15,2023.The cohort was randomly split into training(70%)and testing(30%)datasets.Relevant features were selected for logistic regression(LR)and decision tree(DT)models using the training dataset, and their performance was subsequently assessed using the testing dataset to validate reliability and generalizability.RESULTS The prevalence of MCR was 13.12%among 1189 participants.DT models had the area under the curves(AUCs)of 0.834 and 0.821 for training and testing datasets, respectively, while LR models indicated AUCs of 0.840 and 0.859.Non-inferiority tests confirmed the DT model’s comparable effectiveness to the LR models in predicting MCR.Both models demonstrated good calibration and clinical utility.Seven modifiable risk factors were identified:Age,education level, social engagement, physical activity, nutritional status, depressive symptoms, and purpose in life.Notably, social engagement emerged as a novel factor compared to those previously identified. Both models are integrated into an easy-to-use,interpretable web-based user interface.CONCLUSION The interactive,web-based user interface of both models effectively identifies MCR,with the DT model recommended for its simplicity and interpretability, supporting community nurses and clinicians in triaging MCR. 展开更多

The aging global population is driving an increase in dementia, making the early identification of at-risk individuals crucial.Studies have shown that elderly people often exhibit a slowing gait before dementia diagnosis, which is linked to cognitive decline and predicts dementia risk. With 30%of those over 65 years of age experiencing falls annually, managing fall risk is essential.Motoric cognitive risk syndrome(MCR),characterized by subjective memory impairment and slow gait,is a pre-dementia condition that can identify high-risk individuals without extensive evaluation. The prevalence of MCR varies globally and is associated with an increased risk of falls, disability, and death.Early screening and intervention for MCR can delay dementia and improve fall regulation, offering a new perspective on elderly health management. This review synthesizes the current understanding of MCR-related falls, evaluates risk assessment methods, and discusses health strategies to provide a theoretical basis for fall prevention in community-dwelling older adults. 展开更多

Benzodiazepine (BZD) is the most prescribed CNS depressant in America to treat hyper-excitatory disorders such as anxiety and insomnia. However, the chronic use of BZD often creates adverse effects including psychomotor deficit. In this study, we investigated a novel mechanism by which chronic BZD impedes motoric function in female mice. We used female mice because BZD use is much more prevalent in female than male populations. We tested the hypothesis that the accumulation of p38 (stress-activated protein) in cerebellar Purkinje neurons mediates motoric deficit induced by chronic BZD. To test this hypothesis, we generated transgenic mice that lack p38 incerebellar Purkinje neurons by crossing Pcp2 (Purkinje cell protein 2)-Cre mice with p38loxP/loxP mice. p38-knockdown mice and wild-type mice received BZD (lorazepam, 0.5 mg/kg) for 14 days. During this period, they were tested for motoric performance using Rotarod assay in which a quicker fall from rotating rod indicates poorer motoric performance. Cerebellum was then collected to detect p38 inPurkinje neurons and to measure mitochondrial respiration using immunohistochemistry and real-time XF respirometry, respectively. Compared to vehicletreated mice, BZD-treated mice showed poorer motoric performance, a higher number of Purkinje neurons containing p38, and lower mitochondrial respiration. These effects of BZD were much smaller in p38-knockdown mice. These results suggest that the excessive accumulation of p38 incerebellar Purkinje neurons contributes to motoric deficit associated with chronic BZD. They also suggest that Purkinje neuronal p38 mediates BZD-induced mitochondrial respiratory inhibition in cerebellum. Our findings may provide a new mechanistic insight into chronic BZD-induced motoric deficit. 展开更多

Objectives:This prospective cohort study examined the change trajectories of cognitive and physical functions of individuals with motoric cognitive risk(MCR)syndrome,as well as the longitudinal associations between MCR syndrome and changes in cognitive and physical functions,to provide a new perspective on preventing dementia. Methods:Participants were selected from the China Health and Retirement Longitudinal Study(CHARLS).Demographic characteristics, health status, and lifestyle variables were assessed in 2011.MCR syndrome was defined as the presence of subjective cognitive complaints and objective slow gait, with preserved activities of daily living and absence of dementia, and assessed in 2011.Cognitive function, including orientation, attention and calculation, episodic memory, and visuospatial ability, was measured from 2011 to 2018.Physical function, including grip strength, balance ability, and repeated chair stand tests, was measured from 2011 to 2015.Generalized estimating equation was employed to analyze the longitudinal associations between MCR syndrome in 2011 and changes in cognitive functions over 7 years and physical functions over 4 years. Results:Among 4,217 participants,475 had MCR syndrome in 2011.Both participants with MCR syndrome and those without exhibited a decline in both cognitive and physical function over 7 years and 4 years of follow-up,except for fluctuations in visuospatial ability.Non-MCR syndrome participants demonstrated significantly better overall cognitive function in 2018 compared to 2011(Group×Time:B=0.44,P=0.035)than those in the MCR syndrome group. However, participants with non-MCR syndrome demonstrated significantly worse visuospatial ability in 2013(Group×time:B=−0.44,P=0.002)and 2018(Group×time:B=−0.34,P=0.016)compared to those in the MCR syndrome group.Non-MCR syndrome participants demonstrated significantly better performance in repeated chair stand tests in 2013(Group×time:B=0.31,P<0.001)and 2015(Group×time:B=0.37,P<0.001)compared to those in the MCR syndrome group in 2011.Conclusions:Older adults with MCR syndrome experience worse overall cognitive and physical function performance, especially in repeated chair stand tests, than individuals without MCR syndrome over 7-year and 4-year follow-up periods. It is suggested that future interventional studies will target both physical and cognitive functions in MCR syndrome individuals, providing insights for the prevention of dementia. 展开更多

Spinal cord injury represents a severe form of central nervous system trauma for which effective treatments remain limited.Microglia is the resident immune cells of the central nervous system, play a critical role in spinal cord injury.Previous studies have shown that microglia can promote neuronal survival by phagocytosing dead cells and debris and by releasing neuroprotective and anti-inflammatory factors. However, excessive activation of microglia can lead to persistent inflammation and contribute to the formation of glial scars, which hinder axonal regeneration.Despite this, the precise role and mechanisms of microglia during the acute phase of spinal cord injury remain controversial and poorly understood. To elucidate the role of microglia in spinal cord injury,we employed the colony-stimulating factor 1 receptor inhibitor PLX5622 to deplete microglia. We observed that sustained depletion of microglia resulted in an expansion of the lesion area, downregulation of brain-derived neurotrophic factor, and impaired functional recovery after spinal cord injury.Next,we generated a transgenic mouse line with conditional overexpression of brain-derived neurotrophic factor specifically in microglia. We found that brain-derived neurotrophic factor overexpression in microglia increased angiogenesis and blood flow following spinal cord injury and facilitated the recovery of hindlimb motor function.Additionally,brain-derived neurotrophic factor overexpression in microglia reduced inflammation and neuronal apoptosis during the acute phase of spinal cord injury.Furthermore, through using specific transgenic mouse lines,TMEM119,and the colony-stimulating factor 1 receptor inhibitor PLX73086,we demonstrated that the neuroprotective effects were predominantly due to brain-derived neurotrophic factor overexpression in microglia rather than macrophages. In conclusion, our findings suggest the critical role of microglia in the formation of protective glial scars.Depleting microglia is detrimental to recovery of spinal cord injury, whereas targeting brain-derived neurotrophic factor overexpression in microglia represents a promising and novel therapeutic strategy to enhance motor function recovery in patients with spinal cord injury. 展开更多

Motor neuron diseases are sporadic or inherited fatal neurodegenerative conditions. They selectively affect the upper and/or lower motor neurons in the brain and spinal cord and feature a slow onset and a subacute course contingent upon the site of damage. The main types include amyotrophic lateral sclerosis, progressive muscular atrophy, primary lateral sclerosis, and progressive bulbar palsy, the pathological processes of which are largely identical, with the main disparity lying in the location of the lesions.Amyotrophic lateral sclerosis is the representative condition in this group of diseases, while other types are its variants.Hence, this article mainly focuses on the advancements and challenges in drug research for amyotrophic lateral sclerosis but also briefly addresses several other important degenerative motor neuron diseases.Although the precise pathogenesis remains elusive, recent advancements have shed light on various theories, including gene mutation, excitatory amino acid toxicity, autoimmunology, and neurotrophic factors. The US Food and Drug Administration has approved four drugs for use in delaying the progression of amyotrophic lateral sclerosis:riluzole,edaravone,AMX0035,and tofersen, with the latter being the most recent to receive approval. However, following several phaseⅢtrials that failed to yield favorable outcomes,AMX0035 has been voluntarily withdrawn from both the US and Canadian markets. This article presents a comprehensive summary of drug trials primarily completed between January 1,2023,and June 30,2024,based on data sourced from clinicaltrials.gov.Among these trials, five are currently in phaseⅠ,seventeen are in phaseⅡ,and eleven are undergoing phaseⅢevaluation.Notably,24 clinical trials are now investigating potential disease-modifying therapy drugs, accounting for the majority of the drugs included in this review. Some promising drugs being investigated in preclinical studies, such as ATH-1105,are included in our analysis, and another review in frontiers in gene therapy and immunotherapy has demonstrated their therapeutic potential for motor neuron diseases. This article was written to be an overview of research trends and treatment prospects related to motor neuron disease drugs, with the aim of highlighting the latest potentialities for clinical therapy. 展开更多

The increasing global prevalence of mild cognitive impairment(MCI)necessitates a paradigm shift in early detection strategies.Conventional neuropsychological assessment methods, predominantly paper-and-pencil tests such as the Mini-Mental State Examination and the Montreal Cognitive Assessment, exhibit inherent limitations with respect to accessibility, administration burden, and sensitivity to subtle cognitive decline, particularly among diverse populations. This commentary critically examines a recent study that champions a novel approach:The integration of gait and handwriting kinematic parameters analyzed via machine learning for MCI screening. The present study positions itself within the broader landscape of MCI detection, with a view to comparing its advantages against established neuropsychological batteries, advanced neuroimaging(e.g.,positron emission tomography, magnetic resonance imaging),and emerging fluid biomarkers(e.g.,cerebrospinal fluid,blood-based assays). While the study demonstrates promising accuracy(74.44%area under the curve 0.74 with gait and graphic handwriting)and addresses key unmet needs in accessibility and objectivity,we highlight its cross-sectional nature, limited sample diversity, and lack of dual-task assessment as areas for future refinement. This commentary posits that kinematic biomarkers offer a distinctive, scalable, and ecologically valid approach to widespread MCI screening, thereby complementing existing methods by providing real-world functional insights.Future research should prioritize longitudinal validation, expansion to diverse cohorts, integration with multimodal data including dual-tasking,and the development of highly portable, artificial intelligence-driven solutions to achieve the democratization of early MCI detection and enable timely interventions. 展开更多

Nonhuman primates are increasingly being used as animal models in neuroscience research. However, efficient neuronal tracing techniques for labeling motor neurons and primary sensory afferents in the monkey spinal cord are lacking. Here,by injecting the cholera toxin B subunit into the sciatic nerve of a rhesus monkey,we successfully labeled the motor neurons and primary sensory afferents in the lumbar and sacralspinal cord.Labeled alpha motor neurons were located in lamina IX of the L6–S1 segments, which innervate both flexors and extensors. The labeled primary sensory afferents were mainly myelinated Aβfibers that terminated mostly in laminae I and II of the L4–L7 segments.Together with the labeled proprioceptive afferents, the primary sensory afferents formed excitatory synapses with multiple types of spinal neurons. In summary, our methods successfully traced neuronal connections in the monkey spinal cord and can be used in spinal cord studies when nonhuman primates are used. 展开更多

Spinal cord injury necessitates effective rehabilitation strategies, with exercise therapies showing promise in promoting recovery. This study investigated the impact of rehabilitation exercise on functional recovery and morphological changes following thoracic contusive spinal cord injury. After a 7-day recovery period after spinal cord injury, mice were assigned to either a trained group(10 weeks of voluntary running wheel or forced treadmill exercise) or an untrained group. Bi-weekly assessments revealed that the exercise-trained group, particularly the voluntary wheel exercise subgroup, displayed significantly improved locomotor recovery, more plasticity of dopaminergic and serotonin modulation compared with the untrained group. Additionally, exercise interventions led to gait pattern restoration and enhanced transcranial magnetic motor-evoked potentials. Despite consistent injury areas across groups, exercise training promoted terminal innervation of descending axons. In summary, voluntary wheel exercise shows promise for enhancing outcomes after thoracic contusive spinal cord injury, emphasizing the role of exercise modality in promoting recovery and morphological changes in spinal cord injuries. Our findings will influence future strategies for rehabilitation exercises, restoring functional movement after spinal cord injury. 展开更多

Patients with complete spinal cord injury retain the potential for volitional muscle activity in muscles located below the spinal injury level. However, because of prolonged inactivity, initial attempts to activate these muscles may not effectively engage any of the remaining neurons in the descending pathway.A previous study unexpectedly found that a brief clinical round of passive activity significantly increased volitional muscle activation,as measured by surface electromyography. In this study,we further explored the effect of passive activity on surface electromyographic signals during volitional control tasks among individuals with complete spinal cord injury.Eleven patients with chronic complete thoracic spinal cord injury were recruited.Surface electromyography data from eight major leg muscles were acquired and compared before and after the passive activity protocol. The results indicated that the passive activity led to an increased number of activated volitional muscles and an increased frequency of activation.Although the cumulative root mean square of surface electromyography amplitude for volitional control of movement showed a slight increase after passive activity, the difference was not statistically significant. These findings suggest that brief passive activity may enhance the ability to initiate volitional muscle activity during surface electromyography tasks and underscore the potential of passive activity for improving residual motor control among patients with motor complete spinal cord injury. 展开更多

Permanent-magnet(PM)machines are the important driving components of various mechanical equipment and industrial applications, such as robot joints, aerospace equipment, electric vehicles, actuators, wind generators and electric traction systems. The PM machines are usually expected to have high torque/power density, low torque ripple, reduced rotor mass,a large constant power speed range or strong anti-magnetization capability to match different requirements of industrial applications. The structural topology of the electric machines, including stator/rotor arrangements and magnet patterns of rotor,is one major concern to improve their electromagnetic performance. However, systematic reviews of structural topology are seldom found in literature. Therefore, the objective of this paper is to summarize the stator/rotor arrangements and magnet patterns of the permanent-magnet brushless machines,in depth.Specifically, the stator/rotor arrangements of the PM machines including radial-flux,axialflux and emerging hybrid axial-radial flux configurations are presented, and pros and cons of these topologies are discussed regarding their electromagnetic performance. The magnet patterns including various surface-mounted and interior magnet patterns, such as parallel magnetization pole pattern,Halbach arrays,spoke-type designs and their variants are summarized, and the characteristics of those magnet patterns in terms of flux-focusing effect, magnetic self-shielding effect, torque ripple, reluctance torque, magnet utilization ratio, and anti-demagnetization capability are compared. This paper can provide guidance and suggestion for the structure selection and design of PM brushless machines for high-performance industrial applications. 展开更多
