Mood disorders include a set of psychiatric manifestations of increasing prevalence in our society, being mainly represented by major depressive disorder(MDD)and bipolar disorder(BD).The etiopathogenesis of mood disorders is extremely complex, with a wide spectrum of biological, psychological, and sociocultural factors being responsible for their appearance and development. In this sense, immune system dysfunction represents a key mechanism in the onset and pathophysiology of mood disorders, worsening mainly the central nervous system(neuroinflammation)and the periphery of the body(systemic inflammation). However, these alterations cannot be understood separately, but as part of a complex picture in which different factors and systems interact with each other.Psychoneuroimmunoendocrinology(PNIE)is the area responsible for studying the relationship between these elements and the impact of mind–body integration, placing the immune system as part of a whole. Thus, the dysfunction of the immune system is capable of influencing and activating different mechanisms that promote disruption of the psyche, damage to the nervous system, alterations to the endocrine and metabolic systems, and disruption of the microbiota and intestinal ecosystem,as well as of other organs and,in turn, all these mechanisms are responsible for inducing and enhancing the immune dysfunction.Similarly, the clinical approach to these patients is usually multidisciplinary, and the therapeutic arsenal includes different pharmacological(for example, antidepressants, antipsychotics, and lithium)and non-pharmacological(i.e.,psychotherapy,lifestyle,and electroconvulsive therapy)treatments. These interventions also modulate the immune system and other elements of the PNIE in these patients, which may be interesting to understand the therapeutic success or failure of these approaches. In this sense, this review aims to delve into the relationship between immune dysfunction and mood disorders and their integration in the complex context of PNIE.Likewise,an attempt will be made to explore the effects on the immune system of different strategies available in the clinical approach to these patients,in order to identify the mechanisms described and their possible uses as biomarkers. 展开更多

BACKGROUND The central link between septic shock and acute skin failure(ASF)is the inflammatory response, which occurs throughout disease progression and can lead to systemic inflammatory response syndrome.Patients often experience bad moods, sleep disorders, and other health issues.Despite recognizing these factors,no studies have examined the correlation between inflammatory factors, lactic acid levels,ASF,mood disturbances, and sleep quality in critically ill patients. We hypothesize that higher levels of inflammatory factors and lactic acid are associated with more severe ASF and poorer mood and sleep quality, which may inform clinical treatment for septic shock and ASF.AIM To explore the relationship between inflammatory factors, lactic acid levels, the severity of ASF,bad mood, and sleep quality.METHODS The retrospective study included 150 patients with septic shock from the Second Hospital of Dalian Medical University, categorized into ASF(n=35)or non-ASF groups(n=115).We compared the peripheral blood inflammatory factors, including tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-6(IL-6),lactic acid levels, skin mottling score(SMS),modified early warning score(MEWS),self-rating depression scale(SDS),self-rating anxiety scale(SAS),and Pittsburgh sleep quality index(PSQI)scores.Pearson correlation analysis assessed relationships among these variables.RESULTS The ASF group had significantly higher levels of CRP(19.60±4.10 vs 15.30±2.96 mg/mL),IL-6(298.65±48.65 vs 268.66±33.66 pg/L),procalcitonin,lactic acid(8.42±2.32 vs 5.70±1.27 mmol/L),SMS[0(0,1)vs 3(2,3)],MEWS(9.34±1.92 vs 6.48±1.96),SAS(61.63±12.03 vs 53.71±12.48),SDS(60.17±12.64 vs 52.27±12.64),and PSQI scores(14.23±3.94 vs 8.69±2.46)compared with the non-ASF group(all P<0.001).Pearson correlation analysis revealed that IL-6,CRP,TNF-α,and lactic acid were positively correlated with SMS,MEWS,SAS,SDS,and PSQI scores(P<0.05).CONCLUSION Peripheral blood levels of IL-6,CRP,TNF-α,and lactic acid correlate positively with SMS,MEWS,SAS,SDS,and PSQI in critically ill patients with ASF. 展开更多

The aim of this study was to investigate changes in autonomic nervous system (ANS) activity and mood caused by Mindfulness Art Therapy Short version (MATS). The participants were 20 Japanese college students who were separated into high and low risk groups based on the median score of the General Health Questionnaire (GHQ). MATS consisted of mindfulness exercise and making of art in one session. ANS activity (TP: total energy, LF/HF: sympathetic nervous, HF: parasympathetic nervous system, LF: both sympathetic and parasympathetic) and mood (TA: tension arousal, EA: energy arousal) were measured psychologically before and after MATS. In the high risk group, TP significantly decreased and LF, HF, and LF/HF did not change significantly;while TA significantly decreased and EA significantly increased. In the low risk group, TP and LF significantly increased and HF and LF/HF did not change significantly;while TA significantly decreased and EA showed a non-significant increase. These results suggest that MATS affects the ANS differently for participants with different states of mental health, and particularly promotes activity in low-risk participants. Psychologically, MATS decreased tension or anxiety and increased energy. These findings justify further use of this therapy. 展开更多

Marked alterations in the normal female hormonal milieu in the perimenopausal period significantly affect women’s health, leading to decreased well-being,psychological distress, and impaired quality of life.Common menopausal symp-toms include hot flashes, sleep and mood changes, fatigue, weight gain, and urogenital disturbances.Clinicians often neglect mood swings and disrupted sleep, although those can significantly limit the productivity of women and impair their cognitive function and mental health.Evidence-based management should include a personalized, holistic approach to alleviate symptoms and careful consideration of the risks vs benefits of hormone replacement therapy(HRT),with due consideration of personal preferences.A research paper in the recent issue of the World Journal of Psychiatry by Liu et al investigated the role of HRT in altering mood changes and impaired sleep quality in menopausal women, which helps us to understand the benefits of this treatment approach. 展开更多

Noise is one of the environmental factors with mental and physical effects. The workload is also the multiple mental and physical demands of the task. Therefore, his study investigated the relationship between noise exposure and mood states at different levels of workload. The study recruited 50 workers from the manufacturing sector(blue-collar workers)as the exposed group and 50 workers from the office sector(white-collar workers)as the control group.Their occupational noise exposure was measured by dosimetry. The Stress-Arousal Checklist(SACL)and the NASA Task Load Index(NASA-TLX)were used to measure mood and workload, respectively. The equivalent noise exposure level of the exposed group at high and very high workload levels was 85 and 87 dBA,respectively. The mean mood score of the exposed group was 76 at very high workload. The correlation coefficient between noise exposure level and mood state based on workload levels ranged from 0.3 at medium workload to 0.57 at very high workload.Noise exposure at high workload levels can increase its adverse effects,so controlling and optimizing the multiple demands of the task in the workplace can be used as a privative measure to reduce the adverse effects of noise. 展开更多

The primary and secondary study outcomes were patient mood, evaluated using a two-dimensional mood scale, and postoperative recovery, evaluated using Quality of Recovery-15, which were assessed on postoperative day 3. There were no statistical intergroup differences in the scores of the Two-Dimensional Mood Scale (11.2 [intervention group] vs. 10.4 [control group], P = 0.75) and Quality of Recovery-15 (126.8 [intervention group] vs. 129.3 [control group], P = 0.73).
