Contrast-induced acute kidney injury (CI-AKI) is a complex syndrome induced by exposure to in- travascular contrast media (CM). Currently, the relationships between the CM, pre-existing kidney insufficiency, CI-AKI and myonecrosis after elective PCI are unclear. To investigate the relationship between CI-AKI and post-procedural myonecrosis (PMN) after PCI, we analyzed 327 non-ST-segment elevation acute coronary syndrome subjects un- dertaking elective PCI. The levels of cardiac troponins (cTns), cTnl and cTnT, at baseline and on at least one occasion 18-24 h after PCI were measured. We also recorded serum levels of creatinine (SCr) and the urine albumin:creatinine ratio (ACR) before coronary angiography, and 24-48 h and 48-72 h after contrast administration. A post-procedure increase in cTns was detected in 16.21% (53/327) of subjects with cTns levels 〉99th to 5x99th percentile upper ref- erence limit (URL). Twenty-seven patients (8.26%) developed CI-AKI. CI-AKI occurred more often in subjects with PMN than in those without PMN (20.8% versus 5.8%, respectively, /〉=-0.001). Multiple logistic regression analysis revealed that pre-existing microalbuminuria (MA) was an important independent predictor of PMN (OR: 3.31; 95% CI: 1.26-8.65, P=0.01). However, there was no correlation between the incidence of CI-AKI and PMN (OR: 2.38; 95% CI: 0.88-6.46, P=-0.09). We conclude that pre-existing MA was not only an important independent predictor of CI-AKI but also of PMN.展开更多
Spontaneous metastatic clostridial myonecrosis is a rare condition caused by Clostridium septicum.The underlying lesion is usually either a colonic neoplasm or leukemia.The authors reported a 67-year-old female who pr...Spontaneous metastatic clostridial myonecrosis is a rare condition caused by Clostridium septicum.The underlying lesion is usually either a colonic neoplasm or leukemia.The authors reported a 67-year-old female who presented with acute abdomen secondary to a perforated sigmoid cancer and who developed gas gangrene in her right leg.Unfortunately,despite all resuscitative measures,she died.The authors reviewed the literature;the diagnosis of metastatic myonecrosis was based on a high index of suspicion,development of bullae containing gram-positive rods,and subcutaneous crepitus(although this was a late sign).Treatment involves aggressive fluid replacement,high doses of intravenous penicillin,high concentration of oxygen,and surgical debridement,and/or amputation.The mortality remains very high,despite all the above measures.展开更多
This study aimed to investigate the protective effect of the nature product osthole(OST)against Clostridium perfrin-gens type A infection-caused myonecrosis in a mouse model.Male mice were divided into(1)control,(2)in...This study aimed to investigate the protective effect of the nature product osthole(OST)against Clostridium perfrin-gens type A infection-caused myonecrosis in a mouse model.Male mice were divided into(1)control,(2)infected,(3)OST50 and(4)OST100 treatment groups.In the infected groups,mice were intramuscularly injected with 1×10^(8) CFU of C.perfringens per day for 6 days.Mice in the OST50 and OST100 groups were administrated intraperitoneally with OST at the doses of 50 or 100 mg/kg per day post C.perfringens infection.Our results showed that C.perfringens infection caused marked necrosis and inflammatory cell infiltration in the muscle tissues of mice.Mice in the OST50 and OST100 treatment groups displayed significantly attenuated C.perfringens infection-induced lipid peroxida-tion,oxidative stress,and apoptosis in their muscle tissue.Furthermore,OST treatment significantly downregulated the expressions of NF-κB,IL-1β,and TNF-αmRNA and protein levels,while concomitantly upregulating the expressions of Nrf2 and HO-1 mRNA and protein.OST treatments also inhibited the expression of phosphorylation(p)-p38,p-mTOR,and p-Erk1/2 proteins,and upregulated LC3II and Beclin1 proteins.In summary,our results reveal that OST therapy confers a protective effect against C.perfringens infection-induced oxidative stress and inflammation in muscle tissue,via activation of Nrf2/HO-1 and autophagy pathways and inhibition of p38,Erk1/2 and NF-κB pathways.展开更多
