Based on the comprehensive analysis of data from petrology and mineralogy,well logging,seismic surveys,paleontology,and geochemistry,a detailed research was conducted on paleoenvironmental and paleoclimatic conditions,and modeling of the source rocks in the second member of the Eocene Wenchang Formation(Wen 2 Member)in the Northern Shunde Subsag at the southwestern margin of the Pearl River Mouth Basin.The Wen 2 Member hosts excellent,thick lacustrine source rocks with strong longitudinal heterogeneity and an average total organic carbon(TOC)content of over 4.9%.The Wen 2 Member can be divided into three units(I,II,III)from bottom to top.Unit I features excellent source rocks with Type I organic matters(average TOC of 5.9%)primarily sourced from lake organisms;Unit II hosts source rocks dominated by Type II2 organic matters(average TOC of 2.2%),which are originated from mixed sources dominated by terrestrial input.Unit III contains good to excellent source rocks dominated by Type II1 organic matters(average TOC of 4.9%),which are mainly contributed by lake organisms and partially by terrestrial input.Under the background of rapid subsidence and limited source supply during intense rifting period in the Eocene,excellent source rocks were developed in Wen 2 Member in the Northern Shunde Subsag under the coordinated control of warm and humid climate,volcanic activity,and deep-water reducing conditions.During the deposition of Unit I,the warm and humid climate and volcanic activity promoted the proliferation of lake algaes,primarily Granodiscus,resulting in high initial productivity,and deep-water reducing conditions enabled satisfactory preservation of organic matters.These factors jointly controlled the development and occurrence of excellent source rocks.During the deposition of Unit II,a transition from warm to cool and semi-arid paleoclimatic conditions led to a decrease in lake algaes and initial productivity.Additionally,enhanced terrestrial input and shallow-water,weakly oxidizing water conditions caused a significant dilution and decomposition of organic matters,degrading the quality of source rocks.During the deposition of Unit III,when the paleoclimatic conditions are cool and humid,Pediastrum and Botryococcus began to thrive,leading to an increase in productivity.Meanwhile,the reducing environment of semi-deep water facilitated the preservation of excellent source rocks,albeit slightly inferior to those in Unit I.The study results clarify the differential origins and development models of various source rocks in the Shunde Sag,offering valuable guidance for evaluating source rocks and selecting petroleum exploration targets in similar marginal sags. 展开更多

Cost Assessment for Oncology Patients: Sodium Bicarbonate 2% Oral Solution Mouth Wash for Mucositis

Background: Chemotherapy induced mucositis is one of the deterring factors influencing adherence to cancer treatment. Sodium bicarbonate mouth wash was recently shown to increase patients’ compliance. However, the cost implication of this strategy was never explored. Aim: This study is designed to explore the compounding of sodium bicarbonate 2% mouth wash from sodium bicarbonate powder USP and commercially procured intravenous solution, and to determine the estimated cost implication for patients using this strategy. Materials and Methods: Sodium bicarbonate 2% were compounded using commercially procured sterile intravenous 8.4% solution and powder USP, diluted and dissolved in sterile water for irrigation respectively. The estimated cost savings between the 2 methods were compared to each other as well as to savings from when used in preventing or in adjuvant therapy for chemotherapy induced mucositis. Ethical approval not required by UVA Institutional Review Board. Study conducted according to the International Standards of Good Practice. Result: We came up with a new recipe, sodium bicarbonate 2% mouth wash using commercially procured sterile liquid formulation. Due to shortage, we compounded with sodium bicarbonate powder USP. Using USP 795 regulation, we assigned 14 days beyond use date with refrigeration to these formulations. These formulations resulted in estimated cost savings of $3597.52 and $3686.56 respectively if patients were to be treated for chemotherapy induced mucositis for 21 days. When compared to commercially procured sterile liquid formulation, the use of powder USP, will lead to additional estimated 60 to 66.67% savings for patients. Conclusion: By using sodium bicarbonate powder or solution to compound a 2% mouth wash, we came up with a cheap product that could be used by patients in the moment in the hospital. We were also able to suggest ways that an estimated cost savings for patients undergoing cancer treatment that use this product can be computed. 展开更多

Environmental Temperature and the Risk of Hand,Foot,and Mouth Disease Transmission in the Yangtze River Region of China

Objective To assess health equity in the Yangtze River region to improve understanding of the correlation between hand, foot, and mouth disease(HFMD) and socioeconomic factors.Methods From 2014–2016, data on HFMD incidence, population statistics, economic indicators, and meteorology from 26 cities along the Yangtze River were analyzed. A multi-city random-effects metaanalysis was performed to study the relationship between temperature and HFMD transmission, and health equity was assessed with respect to socio-economic impact.Results Over the study period, 919,458 HFMD cases were reported, with Shanghai(162,303) having the highest incidence and Tongling(5,513) having the lowest. Males were more commonly affected(male-to-female ratio, 1.49:1). The exposure-response relationship had an M-shaped curve, with two HFMD peaks occurring at 4 ℃ and 26 ℃. The relative risk had two peaks at 1.30 ℃(1.834, 95% CI:1.204–2.794) and 31.4 ℃(1.143, 95% CI: 0.901–1.451), forming an M shape, with the first peak higher than the second. The most significant impact of temperature on HFMD was observed between –2 ℃ and18.1 ℃. The concentration index(0.2463) indicated moderate concentration differences, whereas the Theil index(0.0418) showed low inequality in distribution.Conclusion The incidence of HFMD varied across cities, particularly with changes in temperature.Economically prosperous areas showed higher risks, indicating disparities. Targeted interventions in these areas are crucial for mitigating the risk of HFMD. 展开更多

Objective:To estimate the willingness to pay(WTP)of Ho Chi Minh City residents for the hand,foot,and mouth disease(HFMD)vaccine and its associated factors using the contingent valuation method.Methods:This cross-sectional study was conducted in 2024,involved face-to-face interviews with 423 participants in Ho Chi Minh City.The participants were divided into two groups:parents of children under six and individuals without young children.WTP for the HFMD vaccine was assessed using the Krinsky and Robb procedure and the double-bound dichotomous choice technique.A probit regression model was used to analyze factors influencing WTP.Results:The participants had a mean age of(34.0±10.3)years,and the majority of them were female,resided in urban areas,and held a university degree.54.6%had children under six years of age,and 63.8%had never heard of the EV71 vaccine.The mean WTP for two doses of the HFMD vaccine was VND 1240000(USD 51.66).Parents of children under six years old had a mean WTP of USD 49.16,while individuals without young children had a mean WTP of USD 56.25.Significant factors that were positively associated with WTP included vaccine price,younger age,higher income,and greater knowledge about vaccines.Conclusions:The study indicates a relatively high willingness to pay for HFMD vaccination in Ho Chi Minh City,suggesting strong potential for vaccine implementation.It is recommended that the government disseminate vaccine-related information to enhance public awareness and understanding. 展开更多

The Zhujiang(Pearl)River Mouth Basin(PRMB)is located in the northern part of the South China Sea,and it is one of China’s three major offshore hydrocarbon-rich basins,playing an indispensable role in meeting the country’s energy needs.Exploration for oil in the PRMB started early and has achieved remarkable results in some sags,but many sags have yet to yield significant discoveries,necessitating the search for new favorable exploration areas.The aim of this study is to analyze the deep structural characteristics of various sags in the PRMB and predict favorable exploration areas,providing corresponding support for the next strategic breakthrough in oil exploration.Some studies indicate a certain relationship between the Moho depth and crustal thickness and the occurrence of oil.In this paper,based on satellite altimetry gravity anomaly data,we utilize a Moho depth inversion method based on variable residual crustal density to obtain the Moho depth in the PRMB,from which the crustal thickness and crustal stretching factor of the basin are calculated.The results show that the Moho depth in the PRMB ranges from 10 km to 37 km,the crustal thickness ranges from 7 km to 35 km,and the crustal stretching factor ranges from 0.9 to 3.0.Finally,we propose a comprehensive evaluation scheme for oil resoureces based on the CRiteria Importance Through Intercriteria Correlation(CRITIC)method,which comprehensively evaluates multiple factors,such as the Cenozoic sedimentary filling scale,Cenozoic thickness,Moho depth,crustal thickness,and crustal stretching factor,and provides evaluation criteria for identifying hydrocarbon-rich sags.According to this evaluation scheme,the exploration potential is relatively high in the Liwan Sag,Jinghai Sag,Heshan Sag,and Jieyang Sag,which are favorable exploration areas. 展开更多

Based on a set of high-resolution 3D seismic data from the northern continental margin of the South China Sea,the lithospheric structure,thinning mechanisms and related syn-rift tectonic deformation response processes in the crustal necking zone in the deepwater area of the Pearl River Mouth Basin were systematically analyzed,and the petroleum geological significance was discussed.The necking zone investigated in the study is located in the Baiyun Sag and Kaiping Sag in the deepwater area of the Pearl River Mouth Basin.These areas show extreme crustal thinned geometries of central thinning and flank thickening,characterized by multi-level and multi-dipping detachment fault systems.The necking zone exhibits pronounced lateral heterogeneity in structural architectures,which can be classified into four types of thinned crustal architectures,i.e.the wedge-shaped extremely thinned crustal architecture in the Baiyun Main Sub-sag,dumbbell-shaped moderately thinned crustal architecture in the Baiyun West Sub-sag,box-shaped weakly thinned crustal architecture in eastern Baiyun Sag,and metamorphic core complex weakly thinned crustal architecture in the Kaiping Sag.This shows great variations in the degree and style of crustal thinning,types of detachment faults,distribution of syn-rift sedimentary sequences,and intensity of magmatism.The thinning of the necking zone is controlled by the heterogeneous rheological stratification of lithosphere,intensity of mantle-derived magmatism,and deformation modes of detachment faults.The syn-rift tectonic deformation of the necking zone evolved through three phases,i.e.uniform stretching during the early Wenchang Formation deposition period,necking during the late Wenchang Formation deposition period,and hyperextension during the Enping Formation deposition period.The crustal thinning extent and architectural differentiation in these phases were primarily controlled by three distinct mechanisms,i.e.the pure shear deformation activation of pre-existing thrust faults,the simple shear deformation of crust-mantle and inter-crust detachment faults,and differential coupling of lower crustal flow and ductile domes with main detachment faults.The hydrocarbon accumulation and enrichment in the necking zone exhibit marked spatial heterogeneity.Four distinct crustal thinned architecture-hydrocarbon accumulation models were identified in this study.The hydrocarbon accumulations in the shallow part exhibit significant correlations with their deep crustal thinned architectures.The unique lithospheric structure and deformation process predominantly control the favorable hydrocarbon accumulation zones with excellent source-fault-ridge-sand configurations,which is critical to reservoir-forming.The most promising exploration targets are mainly identified on the uplift zones and their seaward-dipping flanks associated with the middle and lower crustal domes.This research provides additional insights into lithospheric thinning-breakup process at intermediate continental margins of marine sedimentary basins,being significant for guiding the deepwater petroleum exploration in the Pearl River Mouth Basin. 展开更多

Driver distraction is a leading cause of traffic accidents,with fatigue being a significant contributor.This paper introduces a novel method for detecting driver distraction by analyzing facial features using machine deep learning and 68 face model.The proposed system assesses driver tiredness by measuring the distance between key facial landmarks,such as the distance between the eyes and the angle of the mouth,to evaluate signs of drowsiness or disengagement.Real-time video feed analysis allows for continuous monitoring of the driver’s face,enabling the system to detect behavioral cues associated with distraction,such as eye closures or changes in facial expressions.The effectiveness of this method is demonstrated through a series of experiments on a dataset of driver videos,which proves that the approach can accurately assess tiredness and distraction levels under various driving conditions.By focusing on facial landmarks,the system is computationally efficient and capable of operating in real-time,making it a practical solution for in-vehicle safety systems.This paper discusses the system’s performance,limitations,and potential for future enhancements,including integration with other in-vehicle technologies to provide comprehensive driver monitoring. 展开更多

This clinical study aimed to explore the therapeutic effect of recombinant human interferonα2b nebulization inhalation in the treatment of hand,foot,and mouth disease in children.Sixty-six patients were selected as the research subjects by the random sampling method and divided into a control group and an observation group.The patients in the control group were treated with ribavirin injection and oral lysine-inositol-vitamin B12 solution,while the patients in the observation group received the treatment of the control group combined with recombinant human interferonα2b nebulization inhalation.The clinical effects of the two groups were compared to determine the clinical efficacy of the two treatment plans.The total effective rates of treatment were 78.79%in the control group and 90.91%in the observation group.The research confirmed that the clinical effect of recombinant human interferonα2b nebulization inhalation in the treatment of hand,foot,and mouth disease in children is remarkable.It can significantly improve the symptoms of children with hand,foot,and mouth disease and is suitable for clinical application and promotion. 展开更多

The mouth bar in the Yangtze estuarine waterways has a significant influence on navigational transport within the estuary, flood discharge and construction of the Shanghai Port. In this paper the morphological evolution and mechanisms of mouth bar formation of the Yangtze estuarine waterways are studied by analyzing hundreds of years of historical data and the latest profile maps of some or the main mouth bar channels in the Yangtze Estuary. The results are shown as follows: The mouth bars in the North Branch have moved gradually from outside the mouth to the inside and formed a huge sand bar. In the North Channel, the head of the mouth bar has migrated about 30 kilometers downstream, and a channel bar has been developing since 2001. Two mouth bar tops, which always existed in the North Passage disappeared in 2010. The head of the mouth bar in the South Passage has migrated downstream about 14 km and the number of tops increased at first but is reduced to only one now. According to the results, we can conclude that the evolution of the mouth bars differs depending on their location. In the North Branch it is directly related to large-scale reclamation in Chongming Island, but in the North Passage it has a close relationship with regulation of the Yangtze Estuary Deepwater Channel. However, the evolution of mouth bars in the North Channel and South Passage is not only connected with the Yangtze Estuary Deepwater Channel Regulation Project, but also with the reclamation in the East Hengsha Shoal and the closure of the Qingcaosha Reservoir. 展开更多

Effect of Mouth Opening Training Stick Combined with Oral Massage on Mouth Opening Difficulty in Head and Neck Cancer Patients after Treatment 被引量:1

Objective: To study the application effect of mouth-opening training sticks combined with oral massage on patients with mouth-opening difficulty after treatment for head and neck cancer. Methods: Using convenient sampling, 60 patients with mouth-opening difficulty after treatment for head and neck cancer admitted to the Oncology Department from February 2022 to October 2023 were selected for a 2-week exercise and nursing program. The patients were divided into a control group (February 1, 2022 to November 30, 2022) and an observation group (December 1, 2022 to October 31, 2023), with 30 patients in each group. The control group underwent routine mouth-opening functional exercises combined with cork for oral support training, while the observation group underwent routine mouth-opening functional exercises combined with oral massage and mouth-opening training sticks for oral support training. The mouth-opening degree, mouth-opening difficulty level, comfort level, compliance, and quality of life were observed in both groups. Results: Before the intervention, there were no statistically significant differences in mouth-opening degree and mouth-opening difficulty level between the two groups (P > 0.05). After the intervention, the mouth-opening degree, mouth-opening difficulty level, oral comfort level, compliance, and QLICP-HN scores in the observation group were all better than those in the control group, with statistically significant differences (P < 0.05). Conclusion: The combination of mouth-opening training sticks and oral massage with mouth-opening functional exercises can effectively improve the degree of mouth-opening difficulty, enhance oral comfort, increase compliance with mouth-opening exercises, and improve quality of life. 展开更多

Objective Since HFMD was designated as a class C communicable disease in May 2008,18 months surveillance data have been accumulated to December 2009.This article was to describe the distribution of HFMD for age,sex,area,and time between 2008 and 2009,to reveal the characteristics of the epidemic.Methods We analyzed weekly reported cases of HFMD from May 2008 to December 2009,and presented data on the distribution of age,sex,area and time.A discrete Poisson model was used to detect spatial-temporal clusters of HFMD.Results More than 1 065 000 cases of HFMD were reported in China's Mainland from May 2008 to December 2009 (total incidence:12.47 per 10 000).Male incidence was higher than female for all ages and 91.9% of patients were 5 years old.The incidence was highest in Beijing,Shanghai,Zhejiang and Hainan.The highest peak of HFMD cases was in April and the number of cases remained high from April to August.The spatial-temporal distribution detected four clusters.Conclusion Children 5 years old were susceptible to HFMD and we should be aware of their vulnerability.The incidence was higher in urban than rural areas,and an annual pandemic usually starts in April. 展开更多

Clinical features in some of the severe and fatal cases were analyzed the etiology of the outbreaks was investigated. Results 89.1% of reported HFMD cases were found in children〈5 year-old with an age-specific incidence rate of 834.1/100 000 in the first year as the notifiable disease in China from May 2008 to April 2009. The incidence, mortality and percentage of severe cases were studied for three regions of China and found to be highest in the central region. The incidence of severe cases and mortality in rural population were significantly higher than those in urban population. Among the laboratory confirmed EV17 positive cases there were 52.6% mild, 83.5% severe, and 96.1% fatal cases. More myoclonic jerks were found in the severe case group than in group that died. Tachypnea, lip purpling, pink foaming and low limb temperature occurred more frequently in the fatal cases than in the severe cases. Conclusion The epidemic of HFMD in China was characterized predominantly by EV71 infections, had relatively high mortality rates especially in the central region, and was most prevalent in young, rural populations. 展开更多

Objective To identify patterns of hand, foot and mouth disease (HFMD) incidence in China during declining incidence periods of 2008, 2009, and 2010. Methods Reported HFMD cases over a period of 25 months were extracted from the National Disease Reporting System (NDRS) and analyzed. An interrupted time series (ITS) technique was used to detect changes in HFMD incidence rates in terms of level and slope between declining incidence periods of the three years. Results Over 3.58 million HFMD cases younger than 5 years were reported to the NDRS between May 1, 2008, and May 31, 2011. Males comprised 63.4% of the cases. ITS analyses demonstrated a significant increase in incidence rate level (P〈0.0001) when comparing the current period with the previous period. There were significant changes in declining slopes when comparing 2010 to 2009, and 2010 to 2008 (all P〈O.O05), but not 2009 to 2008. Conclusion Incremental changes in incidence rate level during the declining incidence periods of 2009 and 2010 can potentially be attributed to a few factors. The more steeply declining slope in 2010 compared with previous years could be ascribed to the implementation of more effective interventions and preventive strategies in 2010. Hand, foot and mouth disease(HFMD) is a major public health concern in China. The most predominant enteroviruses that cause HFMD have traditionally been attributed to enterovirus A71(EVA71) and coxsackievirus A16(CVA16). Since its first large outbreak in 2008, the dominant HFMD pathogens are constantly changing. In 2013 and 2015, CVA6 exceeded both EVA71 and CVA16 to become the leading cause of HFMD in some provinces. However, there still lacks a comprehensive overview on the molecular epidemiology and evolution of HFMD-related enteroviruses at the national level. In this study, we performed systematic epidemiological analyses of HFMD-related enteroviruses using the data of 64 published papers that met the inclusion criteria, and conducted phylogenetic analyses based on 12,080 partial VP1 sequences identified in China before 31 st June 2018. We found that EVA71 prevalence has decreased sharply but other enteroviruses have increased rapidly from 2008 to 2016 and that one subtype of each enterovirus is represented during the epidemic. In addition, four genotypes EVA71_C4, CVA16_B1, CVA6_D and CVA10_C are the most predominant enterovirus strains and collectively they cause over 90% of all HFMD cases in China according to the phylogenetic trees using representative partial VP1 sequences. These four major enterovirus genotypes have different geographical distributions, and they may cocirculate with other genotypes and serotypes. These results suggest that more molecular epidemiological studies should be performed on several enteroviruses simultaneously, and such information should have implications for virological surveillance, disease management, vaccine development and policy-making on the prevention and control of HFMD. 展开更多

The Yangjiang-Yitongdong Fault (YJF) is an important NW-trending regional fault, which divides the Zhujiang (Pearl) River Mouth Basin (ZRMB) into western and eastern segments. In Cenozoic, the northern continental margin of the South China Sea (SCS) underwent continental rifting, breakup, seafloor spreading and thermal subsidence processes, and the Cenozoic activities of YJF is one part of this series of complex processes. Two long NW-trending multichannel seismic profiles located on both sides of the YJF extending from the continental shelf to Continent-Ocean Boundary (COB) were used to study the tectonic and sedimentary characteristics of western ZRMB. Using the 2D-Move software and back-stripping method, we constructed the balance cross-section model and calculated the fault activity rate. Through the comprehensive consideration of tectonic position, tectonic evolution history, featured structure, and stress analysis, we deduced the activity history of the YJF in Cenozoic. The results showed that the YJF can be divided into two segments by the central uplift belt. From 65 Ma to 32 Ma, the YJF was in sinistral motion as a whole, inherited the preexisting sinistral motion of Mesozoic YJF, in which, the southern part of YJF was mainly in extension activity, controlling the formation and evolution of Yunkai Low Uplift, coupled with slight sinistral motion. From 32 Ma to 23.8 Ma, the sinistral motion in northern part of YJF continued, while the sinistral motion in southern part began to stop or shifted to a slightly dextral motion. After 23.8 Ma, the dextral motion in southern part of YJF continued, while the sinistral motion in northern part of YJF gradually stopped, or shifted to the slightly dextral motion. The shift of the YJF strike-slip direction may be related to the magmatic underplating in continent-ocean transition, southeastern ZRMB. According to the analysis of tectonic activity intensity and rift sedimentary structure, the activities of YJF in Cenozoic played a regulating role in the rift extension process of ZRMB. 展开更多

Based on porosity and permeability measurements, mercury porosimetry measurements, thin section analyses, SEM observations, X-ray diffraction (XRD) analysis and granulometric analyses, diagenetic features of reservoir sandstones taken from the Zhuhai formation in the Panyu low-uplift of the Pear River Mouth Basin were examined. This study shows that chlorite cements are one of the most important diagenetic features of reservoir sandstones. The precipitation of chlorite was controlled by multiple factors and its development occurred early in eo-diagenesis and continued till Stage A of middle diagenesis. The precipitation of chlorite at the early stage was mainly affected by the sedimentary environment and provenance. Abundant Fe- and Mg-rich materials were supplied during the deposition of distributary channel sediments in the deltaic front setting and mainly in alkaline conditions. With the burial depth increasing, smectite and kaolinite tended to be transformed into chlorite. Smectite cements were completely transformed into chlorite in sandstones of the studied area. Volcanic lithics rich in Fe and Mg materials were dissolved and released Fe2+ and Mg 2+ into the pore water. These cations precipitated as chlorite cements in middle diagenesis in an alkaline diagenetic environment. Chlorite coatings acted as porosity and permeability, thus helping preserve cements in the chlorite cemented sandstones. The reservoir quality of chlorite cemented sandstones is much better than sandstones without chlorite cements. Chlorite cements play an important role in the reservoir evolution that was mainly characterized by preserving intergranular porosity and forming better pore-throat structures of sandstones. 展开更多

Hand foot and mouth disease is a febrile sickness complex characterized by cutaneous eruption (exanthem) on the palms and soles with simultaneous occurrence of muco-cutanous vesiculo-ulcerative lesions (enanthem) affecting the mouth. The illness is caused by a number of enteroviruses with coxsackievirus A16 and enterovirus 71 as the main causative agents. Human enterovirus 71 (EV71) belongs to the species Human enterovirus A under the genus Enterovirus within the family Picornaviridae. EV71 has been associated with an array of clinical diseases including hand foot and mouth disease (HFMD), aseptic meningitis, encephalitis and poliomyelitis-like acute flaccid paralysis. A large outbreak of HFMD due to highly neurovirulent EV71 emerged in Malaysia in 1997, and caused 41 deaths amongst young children. In late 2000, a recurrence of an outbreak of HFMD occurred in Malaysia with 8 fatalities in peninsular Malaysia. Outbreak of HFMD due to EV71 recurred in 2003 with an unknown number of cases and mortalities. A similar outbreak of HFMD with 2 recorded deaths in young children occurred in peninsular Malaysia in late 2005 and this was followed by a larger outbreak in Sarawak (Malaysian Borneo) with 6 reported fatalities in the early part of 2006. The current on-going outbreak of HFMD started in peninsular Malaysia in epidemiological week 12 of 2010. As with other HFMD outbreaks in Malaysia, both EV71 and CA16 were the main aetiological viruses isolated. In similarity with the HFMD outbreak in 2005, the isolation of CA16 preceded the appearance of EV71. Based on the VP1 gene nucleotide sequences, 4 sub-genogroups of EV71 (C1, C2, B3 and B4) co-circulated and caused the outbreak of hand, foot and mouth disease in peninsular Malaysia in 1997. Two sub-genogroups (C1 and B4) were noted to cause the outbreak in 2000 in both peninsular Malaysia and Sarawak. EV71 of sub-genogroup B5 with smaller contribution from sub-genogroup C1 caused the outbreak in 2003. In the 2005 outbreak, besides the EV71 strains of sub-genogroup C1, EV71 strains belonging to sub-genogroup B5 were isolated but formed a cluster which was distinct from the EV71 strains from the sub-genogroup B5 isolated in 2003. The four EV71 strains isolated from clinical specimens of patients with hand, foot and mouth disease in the Sarawak outbreak in early 2006 also belonged to sub-genogroup B5. Phylogenetic analysis of the VP1 gene suggests that the EV71 strains causing the outbreak in Sarawak could have originated from peninsular Malaysia. Epidemiological and molecular data since 1997 show the recurrence of HFMD due to EV71 in Malaysia every 2 to 4 years. In each of the past outbreaks, more than one sub-genogroup of the virus co-circulate. 展开更多
