C反应蛋白/D-二聚体比值和纤维蛋白原/白蛋白比值对冠心病患者PCI术后MACE发生的预测价值及模型构建

目的全面评估C反应蛋白(CRP)/D-二聚体(D-D)联合白蛋白/纤维蛋白原(FAR)在预测冠心病(CHD)患者经皮冠状动脉介入治疗(PCI)术后主要不良心血管事件(MACE)中的预测价值,并据此构建用于预测CHD患者术后MACE的列线图(Nomogram)模型。方法回... 目的全面评估C反应蛋白(CRP)/D-二聚体(D-D)联合白蛋白/纤维蛋白原(FAR)在预测冠心病(CHD)患者经皮冠状动脉介入治疗(PCI)术后主要不良心血管事件(MACE)中的预测价值,并据此构建用于预测CHD患者术后MACE的列线图(Nomogram)模型。方法回顾性选取2022年6月至2025年3月期间在昆明医科大学第一附属医院接受PCI治疗的201例CHD患者作为研究对象(训练集)。根据是否发生MACE分为MACE组(n=77)和非MACE组(n=124)。同时收集了来自另一医疗中心的84例CHD患者作为验证集。比较两组患者CRP/D-D及FAR表达水平;通过单因素和多因素Logistic回归分析筛选CHD患者术后MACE的独立预测因素;采用ROC曲线评估CRP/D-D及FAR对CHD患者术后MACE发生的预测价值;整合CRP/D-D、FAR等指标建立Nomogram模型,采用ROC曲线、校准曲线和DCA曲线对Nomogram模型进行内部验证和外部验证。结果与非MACE组CHD患者相比,MACE组CRP/D-D及FAR水平升高(P<0.05)。多因素Logistic分析显示,年龄、NTproBNP、WBC、CRP/DD、FAR均为CHD患者术后MACE的独立风险因素(P<0.05)。ROC曲线分析显示,CRP/D-D联合FAR预测的AUC高于CRP/D-D(Z=3.473,P<0.001)、FAR(Z=2.812,P<0.05)单独使用时的AUC(P<0.05)。基于上述影响因素构建Nomogram模型并进行内外部验证,结果显示,该Nomogram模型具有良好的校准度、优异的判别能力以及可靠的临床实用价值,能够准确预测术后MACE的发生风险。结论CRP/D-D比值与FAR作为综合反映炎症和凝血功能的复合生物标志物,在预测CHD患者术后MACE风险方面展现出较高的判别能力,为临床风险分层提供了新的可靠工具。 展开更多

Fibroblast derived C3 promotes the progression of experimental periodontitis through macrophage M1 polarization and osteoclast differentiation

Complement C3 plays a critical role in periodontitis.However,its source,role and underlying mechanisms remain unclear.In our study,by analyzing single-cell sequencing data from mouse model of periodontitis,we identified that C3 is primarily derived from periodontal fibroblasts.Subsequently,we demonstrated that C3a has a detrimental effect in ligature-induced periodontitis.C3ar−/−mice exhibited significantly less destruction of periodontal support tissues compared to wild-type mice,characterized by mild gingival tissue damage and reduced alveolar bone loss.This reduction was associated with decreased production of proinflammatory mediators and reduced osteoclast infiltration in the periodontal tissues.Mechanistic studies suggested that C3a could promote macrophage polarization and osteoclast differentiation.Finally,by analyzing single-cell sequencing data from the periodontal tissues of patients with periodontitis,we found that the results observed in mice were consistent with human data.Therefore,our findings clearly demonstrate the destructive role of fibroblast-derived C3 in ligature-induced periodontitis,driven by macrophage M1 polarization and osteoclast differentiation.These data strongly support the feasibility of C3a-targeted interventions for the treatment of human periodontitis. 展开更多

A finite element model'of 2.5D composites is established, by considering the fiber undulation and the porosity in 2.5D C/SiC composites. The fiber direction of warp is defined by cosine function to simulate the undulation of warp, and based on uniform strain assumption, analytical model of the elastic modulus and coefficient of thermal expansion (CTE) for 2.5D C/SiC composites were established by using dual- scale model. The result is found to correlate reasonably well with the predicted results and experimental results. The parametric study also demonstrates the effects of the fiber volume fraction, distance of warp yarn, and porosity in micro-scale on the mechanical properties and the coefficients of thermal expansion. 展开更多

Three-dimensional atom probe (3DAP) technique has been used to study the nucleation and growth of M23C6 carbide in a supersaturated multi-component Fe-based alloy aged at 800℃. 3D images indicate that the radius of M23C6 carbide after ageing for 10 min is about 9 nm. Concentration profiles of alloy elements in the carbide are also obtained. Combined with PANDAT and Thermo-Calc software, attempts to model the early stages of precipitation are present. The calculated particle size and composition of M23C6 carbide is in good agreement with 3DAP data. 展开更多

Thermal conductivity is one of the most significant criterion of three-dimensional carbon fiber-reinforced SiC matrix composites(3D C/SiC).Represent volume element(RVE)models of microscale,void/matrix and mesoscale proposed in this work are used to simulate the thermal conductivity behaviors of the 3D C/SiC composites.An entirely new process is introduced to weave the preform with three-dimensional orthogonal architecture.The 3D steady-state analysis step is created for assessing the thermal conductivity behaviors of the composites by applying periodic temperature boundary conditions.Three RVE models of cuboid,hexagonal and fiber random distribution are respectively developed to comparatively study the influence of fiber package pattern on the thermal conductivities at the microscale.Besides,the effect of void morphology on the thermal conductivity of the matrix is analyzed by the void/matrix models.The prediction results at the mesoscale correspond closely to the experimental values.The effect of the porosities and fiber volume fractions on the thermal conductivities is also taken into consideration.The multi-scale models mentioned in this paper can be used to predict the thermal conductivity behaviors of other composites with complex structures. 展开更多

Experiments on rock joint behaviors have shown that joint surface roughness is mobilized under shearing,inducing dilation and resulting in nonlinear joint shear strength and shear stress vs.shear displacement behaviors.The Barton-Bandis(B-B) joint model provides the most realistic prediction for the nonlinear shear behavior of rock joints.The B-B model accounts for asperity roughness and strength through the joint roughness coefficient(JRC) and joint wall compressive strength(JCS) parameters.Nevertheless,many computer codes for rock engineering analysis still use the constant shear strength parameters from the linear Mohr-Coulomb(M-C) model,which is only appropriate for smooth and non-dilatant joints.This limitation prevents fractured rock models from capturing the nonlinearity of joint shear behavior.To bridge the B-B and the M C models,this paper aims to provide a linearized implementation of the B-B model using a tangential technique to obtain the equivalent M-C parameters that can satisfy the nonlinear shear behavior of rock joints.These equivalent parameters,namely the equivalent peak cohesion,friction angle,and dilation angle,are then converted into their mobilized forms to account for the mobilization and degradation of JRC under shearing.The conversion is done by expressing JRC in the equivalent peak parameters as functions of joint shear displacement using proposed hyperbolic and logarithmic functions at the pre-and post-peak regions of shear displacement,respectively.Likewise,the pre-and post-peak joint shear stiffnesses are derived so that a complete shear stress-shear displacement relationship can be established.Verifications of the linearized implementation of the B-B model show that the shear stress-shear displacement curves,the dilation behavior,and the shear strength envelopes of rock joints are consistent with available experimental and numerical results. 展开更多

This model involves micro-scale and unit-cell scale. In micro-scale, the tensile properties of fiber tows which involves matrix cracking, interfacial debonding, and fiber failure are studied. The unit-cell scale model can reflect the braided structure and simulate the tensile properties of 3D-4d CMCs by introducing the tensile properties of fiber tows into it. Quasi-static tensile tests of 3D-4d braided CMCs were performed on a PWS-100 test system. The predicted tensile stressstrain curve by the double scale model is in good agreement with that of the experimental results. 展开更多

The Lobo watershed is an agricultural area where the use of fertilizers by farmers is intensive, causing eutrophication problems that deteriorate the quality of drinking water distributed to the population. Since the phenomenon of eutrophication is directly linked to runoff, it is essential to model the flow in order to better control the transfer of nutrients responsible for eutrophication. It is within this framework that this study was conducted. The objective of this study is to assess the ability of the semi-distributed SWAT (Soil and Water Assessment Tool) model to simulate runoff in the Lobo watershed. The methodology adopted was based on the use of the QSWAT graphical interface to manipulate and execute the main functions of the SWAT model from QGIS tools. The hydrological modeling was carried out with the QSWAT interface for SWAT 2012. The results showed good performance for the flow calibration (1982-1984) with the evaluation criteria R<sup>2</sup>, NSE and PBIAS respectively of 0.64, 0.64 and 3.1. In the validation period (1984-1987), the model also showed good performance in the streamflow simulation for R<sup>2</sup> and NSE of 0.84 and 0.76 respectively as values. However, for the PBAIS criterion, the result was less good but still remains satisfactory with a value of 19.6. It emerges from this study that the SWAT model is suitable for simulating water transfer and can therefore be used to study the transfer of pollutants in the fight against eutrophication in the Lobo watershed. 展开更多

In order to satisfy the needs of compressive resistance under the conditions of high water pressure, a stress analysis is required for the dam. During the stress analysis process however, due to the complexity of the three-dimensional modeling, it is very hard to form a model. Therefore, the stress analysis process is a barrier for the arch dam. In this article, based on the research of the new line-type arch dam, a mathematical model in different degree of convexity conditions of the dam is established; using the C + + language program, a computer three-dimensional model simulation is realized on AutoCAD. The accurate three-dimensional model is providing a finite element optimization design of the involute hyperbolic arch dam for the next step. 展开更多

We prove that the underlying operator has uncountable eigenvalues on the left real line and these results describe the point spectra of the operator. Then, we show that the essential growth bound of the C_0-semigroup generated by the operator is 0 and therefore it is not quasi compact, the essential spectral bound of the C_0-semigroup is equal to 1. Moreover, our results imply it is impossible that the time-dependent solution of the model exponentially converges to its steady-state solution. 展开更多

Double-scale model for three-dimension-4 directional(3D-4d) braided C/SiC composites has been proposed to investigate its elastic properties. The double-scale model involves micro-scale that takes fiber/ matrix/porosity in fibers tows into consideration with unit cell which considers the 3D-4d braiding structure. Micro-optical photographs of composites have been taken to study the braided structure. Then a parameterized finite element model that reflects the structure of 3D-4d braided composites is proposed. Double-scale elastic modulus prediction model is developed to predict the elastic properties of 3D-4d braided C/SiC composites. Stiffness and eompliance-averaging method and energy method are adopted to predict the elastic properties of composites. Static-tension experiments have been conducted to investigate the elastic modulus of 3D-4d braided C/SiC composites. Finally, the effect of micro-porosity in fibers tows on the elastic modulus of 3D-4d braided C/SiC composites has been studied. According to the conclusion of this thesis, elastic modulus predicted by energy method and stiffness-averaging method both find good agreement with the experimental values, when taking the micro-porosity in fibers tows into consideration. Differences between the theoretical and experimental values become smaller. 展开更多

An experimental advanced superconducting tokamak (EAST) operation window with the lower hybrid current drive (LHCD) in H-mode is estimated by using a eore-SOL-divertor (C-S-D) model validated by the present EAST divertor experiments. The operation window consists of four limits including two usual limits, one of which is the maximum allowable heat load onto the divertor plate, and two additional limits associated with the LHCD. The predictive EAST operation window is not qualified to fulfill its mission for high input power. To extend the operation window, gas puffing and impurity seeding are presented as two effective methods. In addition, the effect of the LHCD current on the operation window is also discussed. Our numerical analysis results provide a reference for the safe operation of EAST experiments with LHCD in future. 展开更多

In order to assess the impact of these hydraulic structures on the <span>river hydrodynamic functioning, a 1D-2D model was realized. The</span> implementation of the 1D-2D model consisted </span><span style="font-family:;" "="">of </span><span style="font-family:;" "="">first </span><span style="font-family:;" "="">running</span><span style="font-family:;" "=""> </span><span style="font-family:;" "="">the 1D model, then the 2D model, and finally in coupling them. The 1D-2D model was designed with <span>the 1988 flood hydrograph, a Manning’s coefficient of 0.052 m<sup>1/3</sup>/s for the </span>minor bed and 0.06 m<sup>1/3</sup>/s for the major bed. The results of the hydraulic model show that the velocities are almost identical to those of the Cavally in natural operation. The values of the velocities are included between 0.4 m/s and 1.3 m/s at the level of the minor bed of the river and between 0.06 m/s and 0.71 m/s at the level of the floodplains. The average water level for flood propagation is 262.37 ± 0.44 m before construction of the structures and 262.23 ± <span>0.85 m after construction of the structures. The 0.41 m reduction in water</span> level due to the diversion canal and bridge is negligible compared to the total fluctuations of the Cavally River, which vary from 6 to 7 m over the year.</span> 展开更多
