Impact of Linggui Zhugan decoction on microwave ablation outcomes and recurrence in liver cancer

BACKGROUND Liver cancer is one of the most common malignant tumors of the digestive system,and early detection and effective treatment are crucial for improving the prognosis.Microwave ablation(MWA)has shown promising results as a local therapeutic method for liver cancer;however,further improvement of its efficacy remains a key focus of current research.AIM To evaluate the clinical efficacy of Linggui Zhugan decoction combined with MWA for the treatment of primary liver cancer.METHODS Data were collected from 164 patients with primary liver cancer who underwent MWA at our hospital between March 2019 and April 2021.Among them,79 patients(control group)received routine treatments and 85 patients(research group)received Linggui Zhugan decoction in addition to routine treatment.The clinical efficacy,incidence of adverse reactions,and levels of serum alphafetoprotein(AFP),des-γ-carboxy prothrombin(DCP),AFP-L3,total bilirubin(TBil),alanine aminotransferase(ALT),CD4 cell count,CD8 cell count,and CD4/CD8 ratio were compared between the two groups,before and after treatment.The three-year recurrence rates between the two groups were compared,and independent prognostic factors for recurrence were identified.RESULTS The study results revealed that the objective response rate(ORR)in the research group was significantly higher than that in the control group(P=0.005).After treatment,the CD4 cell count and CD4/CD8 ratio significantly increased,whereas the CD8 cell count and TBil,ALT,AFP,DCP,and AFP-L3 Levels were significantly lower in the research group than in the control group(P<0.001).The Cox regression analysis revealed that the treatment regimen(P=0.003),presence of cirrhosis(P=0.019),tumor diameter(P=0.037),Child-Pugh score(P=0.003),pretreatment AFP level(P=0.006),and AFP-L3 Level(P=0.002)were independent prognostic factors for disease-free survival.CONCLUSION The combination of Linggui Zhugan decoction with MWA significantly improved the clinical efficacy and longterm prognosis of patients with primary liver cancer. 展开更多

Objective To observe the clinical effect of fire needle combined with Linggui Bafa for post-herpetic neuralgia (PHN). Methods One hundred and twenty patients were randomly divided into an observation group and a control group with 60 cases each. For observation group, fire needle was applied to prick Zusanli (足三里 ST 36), Yanglingquan (阳陵泉 GB 34), Taichong (太冲 LR 3), Sanyinjiao (三阴交 SP 6) and partial Ashi points around the lesion, and Linggui Bafa was combined to perform acupuncture therapy, once every other day, 10 times as a course of treatment; for the control group, only fire needle was applied. After 2 courses of treatment, the comparison of the value of visual analogue scale (VAS) and the total effective rate for the two groups were carried out. Results The VAS value of the observation group after treatment was obviously lower than that before treatment (2.28±2.08 vs 6.12±1.96) and that of the control group (2.28±2.08 vs 3.62±2.90), there were significant differences (all P〈0.01). The total effective rate of the observation group was obviously higher than that of the control group [93.3% (56/60) vs 76.7% (46/60)], the difference between the two groups was statistically significant (P〈0.05). Conclusion The effect of fire needle combined with Linggui Ba.fa for PHN is obviously superior to that of single fire needle therapy. 展开更多

Objective To evaluate the therapeutic effect and safety of time-oriented points opening of Linggui Bafa for Bell's palsy and provide a new idea for the clinical treatment of this problem. Methods Fifty-four cases included in the study were randomized into two groups, and the treatment group was treated with the routine acupuncture combined with time-oriented points opening of Linggui Bafa. The control group was treated with the routine acupuncture only, Hegu (合谷 El 4), Kunlun (昆仑 BL 60), Jiache (颊车 ST 6), Dicang (地仓 ST 4), Xiaguan (下关 ST 7), Cuanzhu (攒竹 BL 2), Yangbai (附白 GB 14), and Quanliao (颧髎 SI 18) were selected as the main points. The manipulation and time of retaining needles were as same as the treatment group. Both groups were treated once a day, 5 days made one session and 4 sessions were required in all. Modified Portmann's scale, Horse-Brackmann grading scale (H-B), FDIP and FDIS were used to assess the therapeutic effect after the treatment. Results The curative and improvement rate in the treatment group was 80.8% (22/26), and that in the control group was 66.7% (16/24), with the statistically significant difference (P〈O.05), the comparison of scores of Portmann's scale, H-B, FDIP and FDIS in the same group and between the two groups before and after the treatment also had statistically significant differences (all P〈0.05). Conclusion The method of time-oriented points opening of Linggui Bafa for Bell's palsy is better than the routine acupuncture for Bell's palsy. 展开更多

Objective:To evaluate the effects of the modified Linggui Zhugan Decoction(加味苓桂术甘汤) combined with short-term very low calorie diets(VLCDs) on glycemic control in newly diagnosed type 2 diabetes mellitus(T2DM) patients.Methods:A total of 20 subjects with newly diagnosed T2DM were treated with the modified Linggui Zhugan Decoction(one-month administration) combined with short-term VLCDs(5 days),and 3-months follow-up.A standard 75-g oral-glucose-tolerance test(OGTT) indexes fasting plasma glucose(FPG),post-prandial 0.5 h and 2 h plasma glucose(P0.5hPG,P2hPG),glycated hemoglobin A1C(GHbA1C),body weight,body mass index(BMI),insulin function,insulin resistance index,incidence of hypoglycemia,and the liver and renal functions were evaluated before and after treatment.Correlations of BMI with insulin function and insulin resistance were also assessed.Results:After the treatment,the patients' plasma glucose decreased steadily,FPG decreased from 5.8±0.9 mmol/L at pre-treatment to 5.0±0.6 mmol/L at 3-months follow-up(P<0.05),and P2hPG decreased from 11.7±3.8 mmol/L at pre-treatment to 6.9±0.9 mmol/L at 3-months follow-up(P<0.01).The level of GHbA1C declined from(6.47±1.24)% at pre-treatment to(6.14±0.99)% at 3-months follow-up(P<0.01).Body weight and BMI also declined significantly.Insulin resistance index was improved obviously and no event of hypoglycemia occurred.Part of the patients companied with fatty liver had a transient increase in hepatic transaminase during the treatment,but it turned to normal after the treatment.Conclusions:The modified Linggui Zhugan Decoction combined with short-term VLCDs can be safely implemented for steady glycemic control in newly diagnosed T2DM patients. 展开更多

OBJECTIVE:To observe the protective effects of modified Linggui Zhugan Decoction combined with short-term very low calorie diets(VLCDs) on cardiovascular risk factors in obese patients with impaired glucose tolerance(IGT).METHODS:Eighty-five obese patients with IGT of spleen hypofunction and dampness accumulation type were randomly divided into treated group(n=45)and control group(n=40).The treated group was given Linggui Zhugan Decoction combined short-term VLCDs.The control group was given basic weight-reduction treatment.The total course was 6 months.Changes of fasting plasma glucose(FPG),2-hour postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c),fasting insulin(FINS),lipid metabolism,blood pressure,body mass index(BMI) and waist circumference(WC) were observed,and the outcomes were reviewed after the treatment.RESULTS:Compared with the control group,the levels of FPG,2hPG,HbA1c,FINS,HOMA-IR decreased significantly(P<0.05),and the indexes of total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL),systolic blood pressure(SBP),diastolic blood pressure(DBP),BMI and WC were improved significantly(P<0.05) in the treated group.The transformation rate of normal glucose tolerance(NGT) was higher in treatment group than control group(P<0.01).No severe adverse reaction was found in the therapeutic course.CONCLUSION:The modified Linggui Zhugan Decoction combined with short-term VLCDs has good protective effects on cardiovascular risk factors as a treatment intervention for IGT with obesity,as it could improve glycometabolism,significantly decrease the levels of blood pressure,blood lipids,BMI,WC and effectively cut-off and reverse the development of diabetes mellitus. 展开更多

OBJECTIVE:To identify Cald1 as a novel regulator of Linggui Zhugan decoction(苓桂术甘汤)for improving insulin resistance in vivo and in vitro.METHODS:Sprague-Dawley rats were randomly assigned to 3 groups that were received a normal rat chow diet,high-fat diet(HFD),and an HFD plus LGZGD,respectively.The homeostatic model assessment(HOMA)-insulin resistance(IR)index was used to determine IR.Gene microarray methodology was used to identify differentially expressed genes(DEGs)in the three groups of rats.The DEGs associated with IR were confirmed by quantitative real-time polymerase chain reaction.Additionally,Mouse 3 T3-L1 pre-adipocytes were differentiated into mature 3 T3-L1 adipocytes,which were then treated with tumor necrosis factor(TNF)-αto induce cellular IR.Lipid accumulations were identified by Oil Red O staining.Glucose uptake was assessed using the3 H-2-DG test.RESULTS:In this study,we found Cald1 was further screened to validate its biological function in3 T3-L1 adipocytes induced to develop IR.In vitro experiments showed that insulin-stimulated 3 H-2-DG uptake by IR 3 T3-L1 adipocytes was increased after LGZGD intervention,which was associated with a down-regulation of Cald1 expression.CONCLUSION:LGZGD ameliorates HFD-induced IR in rats and TNF-αinduced IR in adipocytes by down-regulating Cald1 expression. 展开更多

Objective To explore the influence of Linggui Zhugan Decoction(LGZGD) on high glucose induced podocyte autophagy.Methods LGZGD containing serum was prepared by intragastric administation of 4.2 g/kg(low dose), 8.4 g/kg(medium dose), and 12.6 g/kg(high dose) LGZGD into SD rats respectively. MPC5 and AB8/13 podocyte cells were treated with 60 mmol/L glucose to establish diabetic nephropathy podocyte model in vitro. Both podocytes were divided into control group, high glucose group, low dose LGZGD group, medium dose LGZGD group, and high dose LGZGD group, respectively. For the three LGZGD groups, before LGZGD intervention, podocytes were treated with 60 mmol/L glucose for 3 days. After treated with LGZGD containing serum, cells were collected to analyze cell migration using Transwell assay, proliferation using CCK8, apoptosis and cell cycle using flow cytometry, autophagosome formation using transmission electron microscopy, and expression levels of Beclin-1, Atg5, LC3II/I, and P62 proteins using Western blot.Results Compared with the control group, the proliferation and migration of MPC5 and AB8/13 cells in the high glucose group slightly decreased, whereas these parameters restored after intervention with low and medium concentrations of LGZGD, with the medium dose LGZGD having the better effect(P < 0.05). Flow cytometry showed that the medium dose LGZGD group had a significantly lower apoptosis rate(P < 0.05) and higher survival rate(P > 0.05) compared to the high dose LGZGD group. High glucose arrested podocytes in G1 phase, whereas LGZGD shifted podocytes from being predominant in G1 phase to G2 phase. High dose LGZGD significanly reduced high glucose-increased autophagosome formation in both podocytes(P < 0.05). Western blot analysis showed that Beclin-1, Atg5, LC3II/I, and P62 expressions were increased in MPC5 cells treated with high glucose and reversed after adminstration of low and medium doses of LGZGD(P < 0.05).Conclusion LGZGD reduced apoptosis and enhanced autophagy in high glucose treated podocytes via regulating Beclin-1/LC3II/I/Atg5 expression. 展开更多

Objective: To explore the therapeutic effect of Linggui Zhugan Decoction on nonalcoholic fatty liver disease through in vivo animal experiments, and to explore the mechanism of action of Linggui Zhugan Decoction in the intervention of nonalcoholic fatty liver disease through network pharmacology and molecular docking technology. Methods: A rat model of non- alcoholic fatty liver disease was established after 20 weeks of high-fat feeding, and the rats were divided into six groups, normal group, model group, simvastatin group (4 mg/kg), low- dose group of Linggui Zhugan Decoction (2.1 g/kg), medium-dose group of Linggui Zhugan Decoction (4.2 g/kg), and high-dose group of Linggui Zhugan Decoction (8.4 g/kg), and the liver morphological changes of HE staining and oil red O staining after the intervention of Linggui Zhugan Decoction were observed, and the microplate reader detected aspartate aminotransferase, alanine aminotransferase, Four levels of blood lipids. Network pharmacology combined with molecular docking was used to screen the potential mechanism and target of Linggui Zhugan Decoction for the intervention of non-alcoholic fatty liver disease. Results: Linggui Zhugan Decoction significantly improved the levels of alanine aminotransferase and alanine aminotransferase in model rats (P<0.05). HE staining and oil red O staining showed that Linggui Zhugan Decoction significantly reduced liver lipid deposition in rats in the model group. Linggui Zhugan Decoction could significantly reduce the quadruplical levels of blood lipids in model rats (P<0.05). Network pharmacology screened out 10 key targets and 5 key signaling pathways;The molecular docking results showed that the first 20 active ingredients all had good binding ability to the top 10 targets. Conclusion: The results show that Linggui Zhugan Decoction has the effect of regulating blood lipids and improving liver tissue lesions in non-alcoholic fatty liver disease, and preliminarily verifies the regulatory effect of Linggui Zhugan Decoction on lipid metabolism-related genes. 展开更多

Objective:To investigate the inhibitory effect of Linggui Zhugan Decoction(LZD,苓桂术甘汤)on the ventricular remodeling(VR)after acute myocardial infarction(AMI)and related mRNA and proteins expression in transforming growth factor-beta 1(TGF-β1)/Smad signaling pathway,and explain its putative mechanism.Methods:A VR model was generated by ligation of coronary artery in mice.Two weeks after surgery,60 mice were randomly divided into the model group,the sham-operation group(distilled water),the positive control group(2.4 mg/kg simvastatin),and the low-,medium-and high-dose LZD groups(2.1,4.2,8.4 g crude drug/kg,respectively)by a random number table,10 mice in each group.Mice in each group was treated for 4 weeks.Changes of hemodynamics indices and cardiac weight index were detected by the PowerLab data acquisition and analysis recording instrument.Morphology changes of myocardial tissue were observed by hematoxylin-eosin and Masson staining.The expressions of TGF-β1,Smad2,Smad3,p-Smad2 and p-Smad3 in myocardial tissue were detected by Western blotting.The mRNA expressions of TGF-β1,Smad2 and Smad3 were detected by reverse transcription-quantitative polymerase chain reaction(RT-qPCR).The expressions of matrix metalloprotein 2(MMP2),MMP9,collagenⅠand collagen Ⅲ were observed by immunohistochemical methods.Results:VR mice showed significant dysfunction in hemodynamic indices and cardiac structure and function.Compared with the shamoperation group,myocardial tissue damage,interstitial fibrosis occurred in the model mice,left ventricular systolic pressure(LVSP),left ventricular pressure maximum contraction rate(+dp/dtmax)and left ventricular pressure maximum relaxation rate(-dp/dtmax)decreased significantly(all P<0.01),while left ventricular end-diastolic pressure(LVEDP),cardiac weight index and left ventricular weight index elevated significantly,meanwhile TGF-β1,p-Smad2,p-Smad3,Smad2,Smad3,MMP2,MMP9,collagen Ⅰ,collagen Ⅲ protein expressions in myocardial tissue and TGF-β1 Smad2 and Smad3 mRNA expressions increased significantly(all P<0.01).Compared with the model group,LZD could significantly improve the pathological changes of myocardial tissue,increase LVSP,+dp/dtmax and-dp/dtmaxf lower LVEDP,reduce the whole heart weight index and left ventricular weight index and inhibit the over-expressions of TGF-β1f p-Smad2,p-Smad3,Smad2,Smad3,MMP2,MMP9,collagenⅠand collagenⅢproteins in myocardial tissue and mRNA expressions of TGF-β1 Smad2 and Smad3(P<0.05 or P<0.01).Conclusion:LZD can significantly suppress VR induced by AMI,and its underlying mechanism may be associated with its inhibitory effect on the TGF-β1/Smad signaling pathway. 展开更多

目的研究苓桂术甘颗粒通过调节脂质代谢、调控氧化应激和炎症反应改善代谢功能障碍相关脂肪性肝炎(MASH)的作用机制。方法采用西方饮食(高脂高胆固醇高果糖饮食)诱导建立MASH小鼠模型,检测其氧化应激、脂质代谢和炎症相关指标。使用0.75... 目的研究苓桂术甘颗粒通过调节脂质代谢、调控氧化应激和炎症反应改善代谢功能障碍相关脂肪性肝炎(MASH)的作用机制。方法采用西方饮食(高脂高胆固醇高果糖饮食)诱导建立MASH小鼠模型,检测其氧化应激、脂质代谢和炎症相关指标。使用0.75 mol·L-1游离脂肪酸(FFA)[油酸钠(OA)∶棕榈酸钠(PA)=2∶1]诱导人源肝癌细胞HepG2,建立肝细胞脂肪变性模型,检测脂质变性细胞氧化应激、脂质代谢和炎症相关指标。结果与正常组相比,模型组小鼠体质量和肝脏质量显著增加(P<0.001),血清中GLU、LDL-C、HDL-C、TC、ALT、AST含量均显著升高(P<0.05,P<0.001),肝组织病理损伤严重,肝脏内TC、TG、MDA含量显著升高(P<0.001),而GSH含量和SOD活性显著降低(P<0.001)。在分子水平上,模型组小鼠肝组织中PPARα的mRNA表达显著下调(P<0.001),SREBP-1c、FAS、Toll样受体(TLR-2/4/9)、IL-1β、TNF-α、TGF-β1、COL1A1及α-SMA的mRNA表达均显著上调(P<0.001);此外,NLRP3炎症小体及其下游效应蛋白Cleaved caspase-1的蛋白表达量也显著升高(P<0.05,P<0.001);在HepG2细胞模型中,模型组细胞内脂滴相对面积,TC、TG和MDA含量均显著增加(P<0.001),GSH含量和SOD活性显著降低(P<0.001),细胞内CPT1A、ACOX1、PPARα、ACADM、SIRT1、Nrf2、HO-1蛋白表达量均显著降低(P<0.05,P<0.01,P<0.001),NLRP3、Caspase-1、Cleaved caspase-1蛋白表达量显著升高(P<0.05,P<0.01);与模型组相比,各给药组小鼠的血清中ALT和AST含量显著降低(P<0.001),高剂量给药组血清中LDL-C、HDL-C和GLU含量显著降低(P<0.05,P<0.001),肝脏病理损伤明显改善,肝脏内TC、TG、MDA含量显著降低(P<0.01,P<0.001),高剂量给药组肝脏中GSH含量和SOD活性显著升高(P<0.05,P<0.01)。同时,高剂量给药组肝组织PPARα的mRNA表达显著上调(P<0.001),各剂量给药组SREBP-1c、FAS、Toll样受体(TLR-2/4/9)、TNF-α、TGF-β1、COL1A1、α-SMA的mRNA表达显著下调(P<0.001),高剂量给药组中NLRP3、Caspase-1、Cleaved caspase-1蛋白表达量显著下降(P<0.05,P<0.01,P<0.001)。细胞实验中,200、400µg⋅mL-1给药处理显著降低了HepG2脂质变性细胞内脂滴相对面积、TG和MDA含量(P<0.001),提高了GSH含量和SOD活性(P<0.01,P<0.001),并显著上调了细胞内CPT1A、ACOX1、PPARα、ACADM、SIRT1、Nrf2蛋白表达(P<0.05,P<0.01,P<0.001),下调了NLRP3、Caspase-1蛋白表达(P<0.05,P<0.001)。结论苓桂术甘颗粒能够有效改善MASH小鼠肝脏组织病理损伤及HepG2细胞的脂质变性状态,其机制可能与调控SREBP与PPARα信号通路、促进脂质代谢调节有关;同时通过调控SIRT1/Nrf2/HO-1通路改善氧化应激,抑制NLRP3炎症小体通路减轻炎症反应。 展开更多

目的:研究苓桂术甘汤(LGZG)对非酒精性脂肪性肝炎(NASH)小鼠的治疗效果及其对铁调素(Hepcidin)-铁转运蛋白(Ferroportin)信号通路介导的铁死亡的影响。方法:将65只小鼠随机分为正常组(10只)和造模组(55只)。正常组小鼠常规饲料喂养,造... 目的:研究苓桂术甘汤(LGZG)对非酒精性脂肪性肝炎(NASH)小鼠的治疗效果及其对铁调素(Hepcidin)-铁转运蛋白(Ferroportin)信号通路介导的铁死亡的影响。方法:将65只小鼠随机分为正常组(10只)和造模组(55只)。正常组小鼠常规饲料喂养,造模组小鼠通过蛋氨酸和胆碱缺乏饮食诱导建立NASH小鼠模型。将50只造模成功小鼠随机分为模型组、水飞蓟宾(SFJB)组、LGZG低剂量组、LGZG中剂量组、LGZG高剂量组,每组10只。各组灌胃不同剂量的相应药物。6周后称量各组小鼠体质量、肝脏质量,并计算肝指数;检测血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)水平及肝组织中总胆固醇(TC)、甘油三酯(TG)水平;肝组织HE染色评估肝脏病变情况,并评定非酒精性脂肪性肝病(NAFLD)活动度评分(NAS);冰冻组织油红O染色观察脂肪在肝组织中的分布情况,并计算平均光密度(AOD)评估小鼠肝组织脂质堆积水平;检测肝组织总铁(Fe)、活性氧(ROS)、4-羟基壬烯醛(4-HNE)、丙二醛(MDA)水平;采用qPCR检测小鼠肝组织酰基辅酶A合成酶长链家族成员4(ACSL4)mRNA、转铁蛋白(Trf)mRNA、铁反应元件结合蛋白2(IREB2)mRNA、铁蛋白重链(Fth)mRNA、铁调素(Hamp)mRNA、铁转运蛋白(Slc40a1)mRNA表达水平;采用Western blotting法检测小鼠肝组织ACSL4、铁传递蛋白(Transferin)、IREB2、Fth、Hepcidin、Ferroportin蛋白表达水平。结果:模型组小鼠体质量低于正常组(P<0.01),肝指数高于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠体质量高于模型组(P<0.01),肝指数低于模型组(P<0.01)。模型组小鼠血清ALT、AST水平及肝脏TC、TG水平高于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠血清ALT、AST水平及肝脏TC、TG水平均低于模型组(P<0.05或P<0.01)。HE和油红O染色结果显示,正常组小鼠肝脏结构清晰,肝板排列有序;模型组小鼠肝脏出现明显的肝细胞索排列异常,存在显著的脂肪变性特征;与模型组相比,LGZG低剂量组小鼠肝组织损伤有所减轻,脂滴数量减少,肝细胞结构较为完整;SFJB组、LGZG中剂量组和LGZG高剂量组小鼠肝组织结构明显改善,肝细胞排列趋于正常,脂肪变性程度显著减轻。模型组小鼠NAS、AOD高于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠NAS、AOD均低于模型组(P<0.05或P<0.01)。模型组小鼠肝组织Fe、ROS、4-HNE、MDA水平高于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠肝组织Fe、ROS、4-HNE、MDA水平低于模型组(P<0.01)。模型组小鼠肝脏ACSL4、Transferrin、IREB2、Hepcidin蛋白相对表达量高于正常组(P<0.01),Fth、Ferroportin蛋白相对表达量低于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠肝组织ACSL4、Transferrin、IREB2蛋白相对表达量均低于模型组,Fth蛋白相对表达量均高于模型组,差异均有统计学意义(P<0.05或P<0.01);SFJB组、LGZG低剂量组、LGZG中剂量组及LGZG高剂量组小鼠肝组织Hepcidin蛋白相对表达量均低于模型组,Ferroportin蛋白相对表达量均高于模型组,差异均有统计学意义(P<0.05或P<0.01)。模型组小鼠肝脏ACSL4 mRNA、Trf mRNA、IREB2 mRNA、Hamp mRNA相对表达量高于正常组(P<0.01),Fth mRNA、Slc40a1 mRNA相对表达量低于正常组(P<0.01);SFJB组、LGZG中剂量组、LGZG高剂量组小鼠肝组织ACSL4 mRNA、Trf mRNA、IREB2 mRNA、Hamp mRNA相对表达量均低于模型组,Fth mRNA、Slc40a1 mRNA相对表达量均高于模型组,差异均有统计学意义(P<0.05或P<0.01)。结论:苓桂术甘汤对NASH小鼠具有治疗作用,可改善NASH小鼠肝脏脂质过氧化,其作用机制可能与调控Hepcidin-Ferroportin信号通路来改善铁死亡有关。 展开更多

[目的]研究苓桂术甘汤(LGZG)对非酒精性脂肪肝炎(NASH)模型小鼠的治疗效果及对PTEN诱导推定激酶1(PINK1)/泛素蛋白连接酶(Parkin)信号通路的影响。[方法]通过蛋氨酸和胆碱缺乏(MCD)饮食诱导建立NASH小鼠模型,并灌胃不同剂量的LGZG。通过检测各组小鼠体质量、肝指数、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、三酰甘油(TG)、总胆固醇(TC)、肝组织苏木精-伊红(HE)染色及冰冻组织油红O染色评估LGZG对NASH模型小鼠的治疗作用;通过对超氧化物歧化酶(SOD)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSHPx)、活性氧(ROS)的检测来评估LGZG对NASH模型肝脏抗氧化的能力;通过检测线粒体标志物TOM20、细胞色素c氧化酶Ⅳ亚型(COXⅣ)、电压依赖性阴离子通道蛋白1(VDAC1)的蛋白水平进而评估PINK1/Parkin通路的关键蛋白PINK1、Parkin、微管相关蛋白1轻链3-Ⅱ(LC3-Ⅱ)、P62、Beclin1的水平来探究LGZG对线粒体自噬的影响。[结果]LGZG可明显提升NASH模型小鼠体质量;降低肝指数;降低血清ALT、AST活性;降低TC、TG水平;同时改善NASH模型小鼠肝组织病理学变化,提示LGZG对NASH模型小鼠具有治疗作用;提高了SOD和GSH-Px水平的同时降低了MDA和ROS水平,提示LGZG提高了肝脏抗氧化能力。提高了PINK1、Parkin、LC3-Ⅱ、beclin1蛋白水平同时降低了TOM20、COXⅣ、VDAC1、P62蛋白水平,说明LGZG改善了NASH小鼠由PINK1/Parkin介导的线粒体自噬。[结论]LGZG对NASH模型小鼠具有治疗作用,并且提高肝脏抗氧化能力,其作用机制可能与通过调控PINK1/Parkin信号通路来改善线粒体自噬有关。 展开更多

心力衰竭是各种病因引起的心脏结构或功能的改变,受限于目前医疗技术而难以完全治愈,临床再住院率高且预后不良,为改善患者心功能指标及维持患者现有生存质量,应用中医药联合治疗逐渐成为临床医生与患者常选的方案,中医药改善心衰症状... 心力衰竭是各种病因引起的心脏结构或功能的改变,受限于目前医疗技术而难以完全治愈,临床再住院率高且预后不良,为改善患者心功能指标及维持患者现有生存质量,应用中医药联合治疗逐渐成为临床医生与患者常选的方案,中医药改善心衰症状的相关作用机制也成为近年来学者研究的重要方向,在心衰疾病的治疗中,其症状改善的原因与抑制心肌纤维化、防止炎症反应过度激活、减轻氧化应激反应等作用机制相关。苓桂术甘汤作为张仲景所创经典方剂,在治疗心系疾病中应用广泛,在苓桂术甘汤治疗心衰疾病的相关文献中,关于其治疗有效的相关作用机制可概括为以下几个方面:调节血脂、调控细胞自噬、调控细胞凋亡及焦亡、调控转化生长因子、调控炎症细胞因子等,以上相关作用机制涵盖了多种不同信号通路,通过这些信号通路,苓桂术甘汤中的主要药物有效成分可达到保护心肌细胞进而改善心衰患者心功能指标及临床症状的作用,且苓桂术甘汤治疗心衰的相关研究表明其有效率较高而不良反应发生率较低。 展开更多

报道1例以灵龟八法按时开穴配合针刺治疗动眼神经麻痹的情况。本案患者表现为右上眼睑下垂,自主睁眼不能,视物模糊、重影,辨证属脾虚肝郁证,给予针刺治疗,以灵龟八法按时开穴(开双侧外关、足临泣、公孙、内关、照海、列缺、申脉、后溪)... 报道1例以灵龟八法按时开穴配合针刺治疗动眼神经麻痹的情况。本案患者表现为右上眼睑下垂,自主睁眼不能,视物模糊、重影,辨证属脾虚肝郁证,给予针刺治疗,以灵龟八法按时开穴(开双侧外关、足临泣、公孙、内关、照海、列缺、申脉、后溪),结合局部(患侧风池、阳白、攒竹等)及远端(双侧合谷、足三里、光明等)配穴。每日针刺1次,第1次治疗后患者上睑下垂症状明显改善、视物重影减轻,治疗5次即获痊愈。随访半年,未见复发。 展开更多

基于生物信息学和动物实验探讨苓桂术甘汤改善心肌梗死后心肌纤维化的潜在机制

利用生物信息学和体内实验研究苓桂术甘汤(Linggui Zhugan Decoction,LZD)对急性心肌梗死(acute myocardial infarction,AMI)后心肌纤维化(myocardial fibrosis,MF)的潜在作用机制。在TCMSP数据库获取LZD活性成分及对应靶点,在GeneCards... 利用生物信息学和体内实验研究苓桂术甘汤(Linggui Zhugan Decoction,LZD)对急性心肌梗死(acute myocardial infarction,AMI)后心肌纤维化(myocardial fibrosis,MF)的潜在作用机制。在TCMSP数据库获取LZD活性成分及对应靶点,在GeneCards、OMIM、TTD和GEO数据库收集疾病靶点;运用Cytoscape软件绘制“药物-成分-靶点”网络;STRING网站构建蛋白互作网络;R语言进行富集分析。结扎小鼠左前降支冠状动脉构建AMI后MF模型进行实验验证,4周后使用多普勒超声成像仪检测小鼠心功能;HE、Masson染色观察心肌组织形态学和胶原纤维沉积变化情况;Western blot法检测心肌纤维化和核心靶点蛋白表达水平;免疫组化法检测心肌组织中α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)水平。生物信息学结果显示,LZD有效成分对应靶点、疾病靶点和GSE97358差异基因三者的交集靶点共33个;核心活性成分为槲皮素、山柰酚、β-谷甾醇等。实验结果表明,LZD可以显著改善AMI后MF小鼠心功能和病理形态学变化,且间质胶原纤维沉积显著减少;小鼠心肌组织中纤维化相关蛋白α-SMA、胶原蛋白Ⅰ(collagenⅠ,ColⅠ)、ColⅢ、基质金属蛋白酶2(matrix metalloproteinase-2,MMP-2)以及核心靶点白细胞介素-1β(interleukin-1β,IL-1β)、前列腺素内过氧化物合酶2(prostaglandin-endoperoxide synthase 2,PTGS2)和CXC趋化因子配体8(CXC chemokine ligand 8,CXCL8)的蛋白表达水平显著下降,过氧化物酶体增殖物激活受体γ(peroxisome proliferators activated receptor gamma,PPARγ)和B淋巴细胞瘤-2(B-cell lymphoma-2,Bcl-2)蛋白表达水平显著上升。综上所述,LZD可以调节多个信号通路及靶点,且显著改善小鼠AMI后MF,延缓AMI转化为慢性心衰的病理进程,其作用机制可能与生物信息学筛选得到的靶点密切相关。 展开更多

目的 基于网络药理学和分子对接技术探讨苓桂术甘汤治疗银屑病的作用机制。方法 运用中药系统药理学数据库与分析平台(TCMSP)获取苓桂术甘汤的有效成分及靶点基因。在GeneCards数据库、人类孟德尔遗传综合数据库(OMIM)、DrugBank数据库... 目的 基于网络药理学和分子对接技术探讨苓桂术甘汤治疗银屑病的作用机制。方法 运用中药系统药理学数据库与分析平台(TCMSP)获取苓桂术甘汤的有效成分及靶点基因。在GeneCards数据库、人类孟德尔遗传综合数据库(OMIM)、DrugBank数据库、治疗靶点数据库(TTD)和PharmGKB数据库中以“psoriasis”为关键词进行文献检索银屑病相关靶点。通过化学成分靶点预测和注释,利用Cytoscape3.8.2软件进行“药物-成分-靶点”网络图的构建,利用STRING在线分析平台构建蛋白质-蛋白质相互作用(PPI)网络图,并进行相关拓扑分析,预测重要的成分及靶点。利用DAVID数据库对潜在靶点进行基于基因本体(GO)的富集分析和基于京都基因和基因组数据库(KEGG)的信号通路富集分析。利用Autodock软件进行核心靶点与其成分的分子对接,随后使用PyMOL进行可视化。最后构建咪喹莫特银屑病鼠模型验证苓桂术甘汤对银屑病的治疗作用及其可能靶点。结果 共筛选出苓桂术甘汤的90个有效成分(白术7个、茯苓15个、甘草92个和桂枝7个),对应靶点207个,其中与银屑病相关靶点142个,富集分析并根据P值筛选出15条潜在信号通路和30条GO条目。分子对接得出苓桂术甘汤活性化合物与核心靶点蛋白的结合性较好,动物实验进一步证实苓桂术甘汤对银屑病小鼠的治疗作用。结论 苓桂术甘汤治疗银屑病与多基因、多靶点和多通路相关,为进一步探讨其作用的物质基础及作用机制提供了参考。 展开更多

目的探讨苓桂术甘汤联合奥美沙坦治疗慢性心力衰竭(CHF)的疗效及对患者心肌酶、心功能、血管内皮功能的影响。方法选取2022年2月至2025年2月该院收治的90例CHF患者作为研究对象,采用随机数字表法将患者分为对照组、试验组,每组45例,对... 目的探讨苓桂术甘汤联合奥美沙坦治疗慢性心力衰竭(CHF)的疗效及对患者心肌酶、心功能、血管内皮功能的影响。方法选取2022年2月至2025年2月该院收治的90例CHF患者作为研究对象,采用随机数字表法将患者分为对照组、试验组,每组45例,对照组采用奥美沙坦治疗,试验组在对照组基础上给予苓桂术甘汤治疗。比较2组临床疗效、不良反应情况;比较2组治疗前后血清心肌酶[肌酸激酶同工酶(CK-MB)、α-羟丁酸脱氢酶(α-HBDH)、乳酸脱氢酶(LDH)]水平、心功能指标[血清B型利钠肽(BNP)及超声心动图参数,包括左室射血分数(LVEF)、左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)]、血管内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)]。结果试验组总有效率为95.56%,高于对照组的82.22%(P<0.05)。治疗后,试验组血清CK-MB、α-HBDH、LDH、BNP、ET-1水平均低于对照组(P<0.05),LVESD、LVEDD均小于对照组(P<0.05),NO水平及LVEF均高于对照组(P<0.05)。对照组不良反应总发生率为15.56%,试验组为11.11%,2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论苓桂术甘汤联合奥美沙坦治疗CHF的疗效突出,对患者心肌酶水平及血管内皮功能有良好的调节作用,还能提高心功能,且安全性较好。 展开更多
