BACKGROUND Pharmacological treatments are commonly used in individuals experiencing perinatal depression(PPD);however,a debate regarding the reproductive safety of antidepressants is ongoing.Many pregnant women opt to...BACKGROUND Pharmacological treatments are commonly used in individuals experiencing perinatal depression(PPD);however,a debate regarding the reproductive safety of antidepressants is ongoing.Many pregnant women opt to discontinue antidepressant out of concern about potential negative effects on the developing fetus,while slow and ineffective antidepressant medications hinder improved outcomes in women with PPD.In recent years,bright light therapy(BLT)has gained traction as a treatment option for PPD;however,clinical trials findings examining the efficacy of BLT in this population have been inconclusive.AIM To validate the feasibility and safety of BLT for the treatment of PPD.METHODS We performed a meta-analysis of randomized controlled trials of patients with PPD treated with BLT vs placebo following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis.We searched PubMed,Embase,the Cochrane Library,and Web of Science for randomized controlled studies published up to December 2023.The results were evaluated using the standardized mean difference of improvement for depression scores and odds ratios(ORs)for remission rate,response rate,incidence of adverse events,and dropout rate.RESULTS The BLT group had higher PPD response rate[50.68%vs 33.08%;OR=2.05;95% confidence interval(CI):1.25-3.35;P=0.004;I^(2)=35%]and remission rate(54.10%vs 18.52%;OR=5.00;95%CI:2.09-11.99;P=0.0003;I^(2)=0%)than the placebo group.Improvements in depression scores were higher in the BLT group than the placebo group for the overall efficacy(standardized mean difference=-0.47;95%CI:-0.80 to-0.13;P=0.007).No significant differences between the two groups in drop-outs(21.84%vs 29.63%;OR=0.63;95%CI:0.31-1.29;P=0.21;I^(2)=0%)or adverse events(17.89%vs 9.68%;OR=2.01;95%CI:0.95-4.25;P=0.07;I^(2)=0%)were observed.CONCLUSION BLT can potentially treat PPD,showing better results than the control group in this study.BLT is effective and safe and could increase the available therapeutic options for PPD.展开更多

In this review,we discuss the therapeutic effect of light on major depression and its relationship to direct retinal projections in the brain. Results: after treatment, the therapeutic effect of the study group was higher than the control group, daily defecation times were more than the control group, jaundice duration and neonatal treatment time were shorter than the control group, the differences were statistically significant (P < 0.05). Objective To investigate the efficacy of lightroom therapy on depressive mood and sleep problemssin patients with depression,and the potential effects on physiological indices related to circadian rhythms.Methods From...Objective To investigate the efficacy of lightroom therapy on depressive mood and sleep problemssin patients with depression,and the potential effects on physiological indices related to circadian rhythms.Methods From October 2021 to July 2023,54 patients with acute-phase depression hospitalized in the Mental Health Center of the First Hospital of Hebei Medical Universityywererecruited.Theparticipants were randomly assigned to either medication combined with the bright light therapy group(bright light group,n=36)or medication combined with the dim light therapy group(dim light group,n=18).Both groups received light therapy for 2 weeks,at 10000 lx in the bright light group and 300 Ix in the dim light group.Both groups received 30 minutes of light therapy from 7:30-8:00 a.m daily over two weeks,followed up for 1 week posttreatment.TThe Hamilton Depression Rating Scale(HAMDi7)was used to assess patients'depressive symptoms,and the Pittsburgh Sleep Quality Index(PSQI)was used to assess patients’sleep quality at baseline,at the end of every week.The 32-Item Hypomania Checklist(HCL-32)was used at the end of week 2 to assess the risk of manic switching after treatment.Daily measurements of body temperature,heart rate,and blood pressure were taken before and after light therapy,along with recording adverse events related to the therapy.Paired t-tests were used to compare changes in physiological indicators before and after treatment,and repeated measures ANOVA was applied to compare clinical symptom changes between the two groups.Results Thirty-one and fifteen patients completed this study in the bright light and dim light groups,respectively,with no statistically significant difference in dropout rates(P>0.05).There were significant interaction effects between the time and group for HAMD_(17) and PSQI score(F=5.51,4.11,both P<0.05).Both groups showed significant reductions in HAMD_(17) and PSQI scores at baseline,week 1,week 2,and week 3(P<0.001).In the bright light group,body temperature increased significantly post-treatment on days 1-4,day 7,and day 12(all P<0.05).Heart rate elevated on day 5(P<0.05).Systolic blood pressure decreased on days 4,5,11,and 12 compared to the pre-treatment baseline(all P<0.05).In the dim light group,systolic blood pressure increased on day 11(P<0.05).Diastolic blood pressure in the bright light group decreased on days 1,5,and 6(P<0.05).No serious adverse events,vision loss,ocular structural changes occurred in either group.No hypomania or mania episodes were observed.The incidence of adverse events did not differ significantly(P>0.05).Conclusion Medication combined with indoor bright light is more effective than the combination of dim light for depressive symptomssandsleepproblems inpatients with depression.Patients receiving bright light alsomay exhibit a higher body temperature,accelerated heart rate,and reduced blood pressure.展开更多
Given the increasing incidence of neurodegenerative disease(ND),recent research efforts have intensified the search for curative treatments.Despite significant research,however,existing therapeutic options for ND can ...Given the increasing incidence of neurodegenerative disease(ND),recent research efforts have intensified the search for curative treatments.Despite significant research,however,existing therapeutic options for ND can only slow down the progression of the disease,but not provide a cure.Light therapy(LT)has been used to treat some mental and sleep disorders.This review illustrates recent studies of the use of LT in patients with ND and highlights its potential for clinical applications.The literature was collected from PubMed through June 2020.Selected studies were primarily English articles or articles that could be obtained with English abstracts and Chinese main text.Articles were not limited by type.Additional potential publications were also identified from the bibliographies of identified articles and the authors’reference libraries.The identified literature suggests that LT is a safe and convenient physical method of treatment.It may alleviate sleep disorders,depression,cognitive function,and other clinical symptoms.However,some studies have reported limited or no effects.Therefore,LT represents an attractive therapeutic approach for further investigation in ND.LT is an effective physical form of therapy and a new direction for research into treatments for ND.However,it requires further animal experiments to elucidate mechanisms of action and large,double-blind,randomized,and controlled trials to explore true efficacy in patients with ND.展开更多
Objectives: Peripheral neuropathy (PN) is a significant contributor to disability in the elderly. It is also one of the most prevalent complications of type 2 diabetes, prediabetes and metabolic syndrome. PN is common...Objectives: Peripheral neuropathy (PN) is a significant contributor to disability in the elderly. It is also one of the most prevalent complications of type 2 diabetes, prediabetes and metabolic syndrome. PN is commonly associated with pain, numbness, tingling, burning, and cramping in the feet and legs. Current treatment options are limited to controlling pain, seizures and use of antidepressant medications. These treatments have undesirable side effects and don’t stop PN progression. Here we utilized a combination of individual-specific modalities to improve local circulation and relieve PN symptoms. Methods: We conducted an open-label, multicenter pilot trial with 34 subjects (19 males and 15 females ranging from 40 - 85 years of age). All of the participants were diagnosed with peripheral neuropathy and had bilateral symptoms in their feet, and many reported the same symptoms (pain, numbness, tingling, burning, and cramping) in their lower legs. The duration of symptoms ranged from four months to over six years. On Day 0, subjects were given a 90-day supply of the oral supplement with dosing instructions and a LED light therapy device. They also received three platelet-rich plasma (PRP) injections in their lower extremities. Subjects also received an extracorporeal shockwave therapy (ESWT) treatment for each foot and subsequently twice per week for the first six weeks, then once weekly for the duration of the study. Subjects filled out the Brief Pain Index (BPI) at weekly intervals. On Day 90, subjects completed the Patient Global Impression of Change (PGIC) survey. Results: There were significant responses to pain, as evidenced by BPI scores at weeks 8, 9, 10 and 11 (p = 0.02, 0.01, 0.02, and 0.003, respectively). Analysis of the final day PGIC survey showed a favorable outcome for 73% of participants (p = 0.003), with the majority reporting Very Much Improved. Conclusions: By utilizing a multi-modality treatment protocol that includes PRP, LED light therapy, ESWT and an oral dietary supplement, we observed significant reductions in BPI scores. Quality of life and their overall impression of change (PGIC) were significantly improved, and there were no significant side effects.展开更多

Methods: 148 cases of jaundice children born in our hospital from June 2020 to June 2021 were randomly divided into study group and control group, with 74 cases in each group. Results: the actual prevalence rate of the study group (97.30%) was higher than that of the control group (86.49%). After treatment, serum bilirubin was lower than that of the control group. The incidence of adverse reactions in the study group (2.70%) was lower than that in the control group (12.16%), P<0.05. Conclusion: blue light intermittent irradiation is effective in the treatment of infant jaundice. Can obviously improve the clinical symptoms of children and reduce the serum bilirubin index. It has high safety, can promote health recovery, and is worthy of application.展开更多
Photobiomodulation has been known to have potential medicinal effects for ages.It involves the use of specific wavelengths to target specific regions in the cell.Different health conditions have been reported to be tr...Photobiomodulation has been known to have potential medicinal effects for ages.It involves the use of specific wavelengths to target specific regions in the cell.Different health conditions have been reported to be treated with exposure to light such as cardiovascular conditions,inflammatory diseases,infectious diseases,and most importantly ocular diseases.This review specifically targets the treatment of retinal diseases including age-related macular degeneration,diabetic macular edema,myopia and acute retinal light injury with photobiomodulation.Red light is used in this therapy since this wavelength has lower frequency and hence minimal chance of causing any damage.Red light has the potential to penetrate cellular structures such as mitochondria and facilitate cellular processes.For ocular diseases,the target wavelength ranges between 630 to 800 nm.In most of the cases the primary target for red light is the cytochrome C oxidase enzyme in mitochondria,which alters the gene expression and promotes cellular energy production.Clinical evidence shows improvement of visual activity and reduction in thickness of retina post treatment.Future prospects of photobiomodulation involve target-specific treatment,combinational therapy to treat complex retinal diseases including gene therapy,and longitudinal studies to predict long-term efficacy and the chance of any recurrence in the patients.Hence the future of photobiomodulation holds significant potential in medicine especially in ocular diseases characterized by progress in research,technology,and clinical trials.展开更多

METHODS:A total of 120 patie...OBJECTIVE:To evaluate the effects of external application of warm meridian medicated wine and polarized light therapy combined with acupuncture on pain management following vertebroplasty. METHODS:A total of 120 patients with osteoporotic vertebral compression fractures treated by vertebroplasty at our hospital were divided into four groups. The control group received non-steroidal anti-inflammatory drugs, the Treatment Group Ⅰ received acupuncture alone, Treatment Group Ⅱ was treated with medicated wine for warming meridians alongside polarized light physiotherapy, and Treatment Group Ⅲ received a combination of medicated wine for warming meridians, polarized light therapy, and acupuncture. The clinical efficacy, pain thresholds at various time points, temperature pain threshold, electric pain threshold, quality of life, sleep quality index, lumbar dysfunction index, visual analog scale(VAS) scores, and incidence of adverse reactions were compared and analyzed across the four groups. RESULTS:The total clinical effective rate in Treatment Group Ⅲ was significantly higher than that in the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ(P < 0.05). At 24 and 72 h post-treatment, the VAS scores, temperature pain thresholds, and electric pain thresholds in Treatment Group Ⅲ were significantly lower than those in the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ(P < 0.05). Additionally, quality-of-life scores in Treatment Group Ⅲ were markedly higher compared to the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ, while the Pittsburgh Sleep Quality Index scores, Oswestry Disability Index scores, and incidence of adverse reactions in Treatment Group Ⅲ were significantly lower than in the other groups(P < 0.05). CONCLUSION:The external application of warm meridian medicated wine and polarized light therapy combined with acupuncture significantly reduces postoperative pain following vertebroplasty, enhances lumbar function, and improves both sleep quality and overall quality of life for patients. Chronic diabetic wounds,a common and severe complication of diabetes,are characterized by their inability to heal due to impaired blood and oxygen supply.In addition to glycemic control,various clinical treatments suc...Chronic diabetic wounds,a common and severe complication of diabetes,are characterized by their inability to heal due to impaired blood and oxygen supply.In addition to glycemic control,various clinical treatments such as wound dressings,hyperbaric oxygen therapy,and phototherapy have been employed to manage these wounds.Low-level light therapy has emerged as an effective,noninvasive,and painless therapeutic approach for wound management.However,the bulkiness of traditional light sources and the need for frequent clinic visits have limited its widespread adoption.We have developed a wearable,flexible light-emitting bandage with cyanobacterial impregnation(LEB@Cyan).The bioactive bandage is designed to provide sustained oxygen generation and robust photobiomodulation,promoting keratinocyte migration,fibroblast proliferation,and angiogenesis.This addresses the hypoxic conditions and enhances bioenergetic supply to accelerate the healing process of diabetic wounds.In detail,the wound area of diabetic rats treated with LEB@Cyan showed significant reductions of 74.76%and 96.32%compared with that of LEB-treated diabetic rats and untreated diabetic rats,respectively.Such self-oxygenated wearable light-emitting fabric holds great promise for future clinical and commercial applications,potentially revolutionizing the management of chronic diabetic wounds.展开更多

Methods: a total of 200 children with NNJ from January 2019 to December 2019 in our hospital were divided into two groups by odd-even method. The levels of bilirubin (BILirubin) before and after treatment, the outcome, the time of disease improvement and the incidence of phototherapy damage events in the two groups were compared. Results: there were significant differences in BILI level, outcome, improvement time and incidence of phototherapy damage events between the intermittent blue light treatment group and the continuous blue light treatment group (P < 0.05). Conclusion: in the aspect of blue light therapy for CHILDREN with NNJ, intermittent intervention can more significantly promote the decline of BILI level, accelerate the regression of yellow skin stain, accelerate the rehabilitation of children, and improve bilirubin level, which is worth promoting.展开更多

Near-infrared(NIR)light has been shown to produce a range of physiological effects in hunans,however,there is still no agreement on whether and how a single parameter,like the flicker frequency of NIR light,affects th...Near-infrared(NIR)light has been shown to produce a range of physiological effects in hunans,however,there is still no agreement on whether and how a single parameter,like the flicker frequency of NIR light,affects the brain.An 810 nm NIR LED was used as the stimulator.Fifty subjects participated in this experiment.Forty subjects were randomly divided into four groups.Each group underwent a 30-minute NIR LED radiation with four different frequencies(i.e.,0 Hz,5 Hz,10 Hz and 20 Hz,respectively)on the forehead.The remaining 10 subjects formed the control group,in which they underwent a 30-minute rest period without light radiation.EEG signals of all subjects during each test were recorded.Gravity frequency(GF),relative energy change,and sample entropy were analyzed.The experimental groups had larger GF values compared to the control group.Higher stimulation frequency would cause larger growth of GF(F=14.75,P<0.001).The amplitude of alpha waves relative energy increased,while theta waves decreased remarkably in the experimental groups(p<0.02),and the extent of increase/decrease was larger at higher stimulation frequency,compared to that of the control.Sample entropy of electrodes in the frontal areas were much larger than those in other brain areas in the experimental groups(p<0.001).Larger frequency of the NIR LED light would cause more distinct brain activities in the stimulated areas.It indicates that NIR LED light may have a positive effect on modulating brain activity.These results may help improve the design of photobiomodulation treatments in the future.展开更多

Despite the growing evidence of the clinical efficacy of these therapies, the optimal treatment stimulation parameters remain unknown and there are no standard treatment protocols. We introduce a closed-loop control design as an experimental system to study the dose-response of wound healing therapy treatment within a prescribed multidimensional and multimodal stimulation parameter space. Systems engineering approaches are applied to the control problem for estimation of a transfer function and model equations derived for use in optimal model-based control. The experimental control system design consisted of simultaneous application of biophysical energies inputted into a wound system. A study design set up including the use of negative pressure wound therapy, electrical stimulation therapy, and photobiomodulation device systems was described. Treatment stimulation parameters were selected from experimental ranges used in the scientific literature. Classical control methods and model-based control were suggested for model selection and evaluation and design of the overall control system. An experimental design for multimodal biophysical wound healing therapy control system is introduced to establish the dose-response interactions for development of therapeutic applications and device design.展开更多

文摘BACKGROUND Pharmacological treatments are commonly used in individuals experiencing perinatal depression(PPD);however,a debate regarding the reproductive safety of antidepressants is ongoing.Many pregnant women opt to discontinue antidepressant out of concern about potential negative effects on the developing fetus,while slow and ineffective antidepressant medications hinder improved outcomes in women with PPD.In recent years,bright light therapy(BLT)has gained traction as a treatment option for PPD;however,clinical trials findings examining the efficacy of BLT in this population have been inconclusive.AIM To validate the feasibility and safety of BLT for the treatment of PPD.METHODS We performed a meta-analysis of randomized controlled trials of patients with PPD treated with BLT vs placebo following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis.We searched PubMed,Embase,the Cochrane Library,and Web of Science for randomized controlled studies published up to December 2023.The results were evaluated using the standardized mean difference of improvement for depression scores and odds ratios(ORs)for remission rate,response rate,incidence of adverse events,and dropout rate.RESULTS The BLT group had higher PPD response rate[50.68%vs 33.08%;OR=2.05;95% confidence interval(CI):1.25-3.35;P=0.004;I^(2)=35%]and remission rate(54.10%vs 18.52%;OR=5.00;95%CI:2.09-11.99;P=0.0003;I^(2)=0%)than the placebo group.Improvements in depression scores were higher in the BLT group than the placebo group for the overall efficacy(standardized mean difference=-0.47;95%CI:-0.80 to-0.13;P=0.007).No significant differences between the two groups in drop-outs(21.84%vs 29.63%;OR=0.63;95%CI:0.31-1.29;P=0.21;I^(2)=0%)or adverse events(17.89%vs 9.68%;OR=2.01;95%CI:0.95-4.25;P=0.07;I^(2)=0%)were observed.CONCLUSION BLT can potentially treat PPD,showing better results than the control group in this study.BLT is effective and safe and could increase the available therapeutic options for PPD.
文摘Implantable light therapy devices,utilizing red and infrared light,have garnered significant interest for their potential to enhance tissue repair and regeneration,particularly in the context of spinal cord injuries(SCI).These light wavelengths,known for their ability to penetrate the skin,initiate cellular processes that promote healing,reduce inflammation,and support regenerative mechanisms.Emerging research suggests that red and infrared light therapies can facilitate SCI recovery by stimulating neural regeneration,improving blood circulation,and mitigating oxidative stress.This review explores the underlying mechanisms by which implantable light therapy devices influence spinal cord recovery,emphasizing their noninvasive nature and potential advantages over conventional treatment strategies.The therapeutic effects of these light therapies offer a promising novel approach for addressing spinal cord damage and improving patient outcomes.Although further studies are necessary to establish optimal treatment protocols by advancing systems with artificial intelligence(AI)and evaluating longterm effects,the evidence suggests that light-based therapies could play a critical complementary role in SCI treatment and rehabilitation.The findings presented highlight the growing potential of these therapies in advancing SCI recovery and enhancing therapeutic efficacy.

文摘Observations from clinical trials have frequently demonstrated that light therapy can be an effective therapy for seasonal and non-seasonal major depression. Despite the fact that light therapy is known to have several advantages over antidepressant drugs like a low cost,minimal side-effects, and fast onset of therapeutic effect,the mechanism underlying light therapy remains unclear.So far, it is known that light therapy modulates mood states and cognitive functions, involving circadian and noncircadian pathways from retinas into brain. In this review,we discuss the therapeutic effect of light on major depression and its relationship to direct retinal projections in the brain. We finally emphasize the function of the retino-raphe projection in modulating serotonin activity,which probably underlies the antidepressant effect of light therapy for depression.

文摘Light is a powerful environmental factor influencing diverse brain functions.Clinical evidence supports the beneficial effect of light therapy on several diseases,including depression,cognitive dysfunction,chronic pain,and sleep disorders.However,the precise mechanisms underlying the effects of light therapy are still not well understood.In this review,we critically evaluate current clinical evidence showing the beneficial effects of light therapy on diseases.In addition,we introduce the research progress regarding the neural circuit mechanisms underlying the modulatory effects of light on brain functions,including mood,memory,pain perception,sleep,circadian rhythm,brain development,and metabolism.

文摘Objective: to analyze the nursing experience of probiotic-assisted blue light therapy in neonates with hyperbilirubinemia. Methods: in our college in July 2020 to October 2021 newborns treated 40 cases of children with high blood bilirubin, random number table method is divided into research group and control group, 20 cases, and blue light therapy and routine nursing care, the team with auxiliary intervention at the same time to carry out the probiotics, the treatment effect in two groups of children with treatment basic statistical comparison. Results: after treatment, the therapeutic effect of the study group was higher than the control group, daily defecation times were more than the control group, jaundice duration and neonatal treatment time were shorter than the control group, the differences were statistically significant (P < 0.05). Conclusion: probiotic-assisted blue light therapy combined with neonatal nursing can improve the curative effect and promote rehabilitation in neonates with hyperbilirubinemia.
文摘Objective To investigate the efficacy of lightroom therapy on depressive mood and sleep problemssin patients with depression,and the potential effects on physiological indices related to circadian rhythms.Methods From October 2021 to July 2023,54 patients with acute-phase depression hospitalized in the Mental Health Center of the First Hospital of Hebei Medical Universityywererecruited.Theparticipants were randomly assigned to either medication combined with the bright light therapy group(bright light group,n=36)or medication combined with the dim light therapy group(dim light group,n=18).Both groups received light therapy for 2 weeks,at 10000 lx in the bright light group and 300 Ix in the dim light group.Both groups received 30 minutes of light therapy from 7:30-8:00 a.m daily over two weeks,followed up for 1 week posttreatment.TThe Hamilton Depression Rating Scale(HAMDi7)was used to assess patients'depressive symptoms,and the Pittsburgh Sleep Quality Index(PSQI)was used to assess patients’sleep quality at baseline,at the end of every week.The 32-Item Hypomania Checklist(HCL-32)was used at the end of week 2 to assess the risk of manic switching after treatment.Daily measurements of body temperature,heart rate,and blood pressure were taken before and after light therapy,along with recording adverse events related to the therapy.Paired t-tests were used to compare changes in physiological indicators before and after treatment,and repeated measures ANOVA was applied to compare clinical symptom changes between the two groups.Results Thirty-one and fifteen patients completed this study in the bright light and dim light groups,respectively,with no statistically significant difference in dropout rates(P>0.05).There were significant interaction effects between the time and group for HAMD_(17) and PSQI score(F=5.51,4.11,both P<0.05).Both groups showed significant reductions in HAMD_(17) and PSQI scores at baseline,week 1,week 2,and week 3(P<0.001).In the bright light group,body temperature increased significantly post-treatment on days 1-4,day 7,and day 12(all P<0.05).Heart rate elevated on day 5(P<0.05).Systolic blood pressure decreased on days 4,5,11,and 12 compared to the pre-treatment baseline(all P<0.05).In the dim light group,systolic blood pressure increased on day 11(P<0.05).Diastolic blood pressure in the bright light group decreased on days 1,5,and 6(P<0.05).No serious adverse events,vision loss,ocular structural changes occurred in either group.No hypomania or mania episodes were observed.The incidence of adverse events did not differ significantly(P>0.05).Conclusion Medication combined with indoor bright light is more effective than the combination of dim light for depressive symptomssandsleepproblems inpatients with depression.Patients receiving bright light alsomay exhibit a higher body temperature,accelerated heart rate,and reduced blood pressure.

文摘Given the increasing incidence of neurodegenerative disease(ND),recent research efforts have intensified the search for curative treatments.Despite significant research,however,existing therapeutic options for ND can only slow down the progression of the disease,but not provide a cure.Light therapy(LT)has been used to treat some mental and sleep disorders.This review illustrates recent studies of the use of LT in patients with ND and highlights its potential for clinical applications.The literature was collected from PubMed through June 2020.Selected studies were primarily English articles or articles that could be obtained with English abstracts and Chinese main text.Articles were not limited by type.Additional potential publications were also identified from the bibliographies of identified articles and the authors’reference libraries.The identified literature suggests that LT is a safe and convenient physical method of treatment.It may alleviate sleep disorders,depression,cognitive function,and other clinical symptoms.However,some studies have reported limited or no effects.Therefore,LT represents an attractive therapeutic approach for further investigation in ND.LT is an effective physical form of therapy and a new direction for research into treatments for ND.However,it requires further animal experiments to elucidate mechanisms of action and large,double-blind,randomized,and controlled trials to explore true efficacy in patients with ND.
文摘Objectives: Peripheral neuropathy (PN) is a significant contributor to disability in the elderly. It is also one of the most prevalent complications of type 2 diabetes, prediabetes and metabolic syndrome. PN is commonly associated with pain, numbness, tingling, burning, and cramping in the feet and legs. Current treatment options are limited to controlling pain, seizures and use of antidepressant medications. These treatments have undesirable side effects and don’t stop PN progression. Here we utilized a combination of individual-specific modalities to improve local circulation and relieve PN symptoms. Methods: We conducted an open-label, multicenter pilot trial with 34 subjects (19 males and 15 females ranging from 40 - 85 years of age). All of the participants were diagnosed with peripheral neuropathy and had bilateral symptoms in their feet, and many reported the same symptoms (pain, numbness, tingling, burning, and cramping) in their lower legs. The duration of symptoms ranged from four months to over six years. On Day 0, subjects were given a 90-day supply of the oral supplement with dosing instructions and a LED light therapy device. They also received three platelet-rich plasma (PRP) injections in their lower extremities. Subjects also received an extracorporeal shockwave therapy (ESWT) treatment for each foot and subsequently twice per week for the first six weeks, then once weekly for the duration of the study. Subjects filled out the Brief Pain Index (BPI) at weekly intervals. On Day 90, subjects completed the Patient Global Impression of Change (PGIC) survey. Results: There were significant responses to pain, as evidenced by BPI scores at weeks 8, 9, 10 and 11 (p = 0.02, 0.01, 0.02, and 0.003, respectively). Analysis of the final day PGIC survey showed a favorable outcome for 73% of participants (p = 0.003), with the majority reporting Very Much Improved. Conclusions: By utilizing a multi-modality treatment protocol that includes PRP, LED light therapy, ESWT and an oral dietary supplement, we observed significant reductions in BPI scores. Quality of life and their overall impression of change (PGIC) were significantly improved, and there were no significant side effects.
文摘Objective: to explore the clinical effect of different blue light irradiation on neonatal jaundice. Methods: 148 cases of jaundice children born in our hospital from June 2020 to June 2021 were randomly divided into study group and control group, with 74 cases in each group. Results: the actual prevalence rate of the study group (97.30%) was higher than that of the control group (86.49%). After treatment, serum bilirubin was lower than that of the control group. The incidence of adverse reactions in the study group (2.70%) was lower than that in the control group (12.16%), P<0.05. Conclusion: blue light intermittent irradiation is effective in the treatment of infant jaundice. Can obviously improve the clinical symptoms of children and reduce the serum bilirubin index. It has high safety, can promote health recovery, and is worthy of application.
文摘Photobiomodulation has been known to have potential medicinal effects for ages.It involves the use of specific wavelengths to target specific regions in the cell.Different health conditions have been reported to be treated with exposure to light such as cardiovascular conditions,inflammatory diseases,infectious diseases,and most importantly ocular diseases.This review specifically targets the treatment of retinal diseases including age-related macular degeneration,diabetic macular edema,myopia and acute retinal light injury with photobiomodulation.Red light is used in this therapy since this wavelength has lower frequency and hence minimal chance of causing any damage.Red light has the potential to penetrate cellular structures such as mitochondria and facilitate cellular processes.For ocular diseases,the target wavelength ranges between 630 to 800 nm.In most of the cases the primary target for red light is the cytochrome C oxidase enzyme in mitochondria,which alters the gene expression and promotes cellular energy production.Clinical evidence shows improvement of visual activity and reduction in thickness of retina post treatment.Future prospects of photobiomodulation involve target-specific treatment,combinational therapy to treat complex retinal diseases including gene therapy,and longitudinal studies to predict long-term efficacy and the chance of any recurrence in the patients.Hence the future of photobiomodulation holds significant potential in medicine especially in ocular diseases characterized by progress in research,technology,and clinical trials.
文摘OBJECTIVE:To evaluate the effects of external application of warm meridian medicated wine and polarized light therapy combined with acupuncture on pain management following vertebroplasty. METHODS:A total of 120 patients with osteoporotic vertebral compression fractures treated by vertebroplasty at our hospital were divided into four groups. The control group received non-steroidal anti-inflammatory drugs, the Treatment Group Ⅰ received acupuncture alone, Treatment Group Ⅱ was treated with medicated wine for warming meridians alongside polarized light physiotherapy, and Treatment Group Ⅲ received a combination of medicated wine for warming meridians, polarized light therapy, and acupuncture. The clinical efficacy, pain thresholds at various time points, temperature pain threshold, electric pain threshold, quality of life, sleep quality index, lumbar dysfunction index, visual analog scale(VAS) scores, and incidence of adverse reactions were compared and analyzed across the four groups. RESULTS:The total clinical effective rate in Treatment Group Ⅲ was significantly higher than that in the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ(P < 0.05). At 24 and 72 h post-treatment, the VAS scores, temperature pain thresholds, and electric pain thresholds in Treatment Group Ⅲ were significantly lower than those in the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ(P < 0.05). Additionally, quality-of-life scores in Treatment Group Ⅲ were markedly higher compared to the control group, Treatment Group Ⅰ, and Treatment Group Ⅱ, while the Pittsburgh Sleep Quality Index scores, Oswestry Disability Index scores, and incidence of adverse reactions in Treatment Group Ⅲ were significantly lower than in the other groups(P < 0.05). CONCLUSION:The external application of warm meridian medicated wine and polarized light therapy combined with acupuncture significantly reduces postoperative pain following vertebroplasty, enhances lumbar function, and improves both sleep quality and overall quality of life for patients. This approach is recommended for clinical application.

文摘Chronic diabetic wounds,a common and severe complication of diabetes,are characterized by their inability to heal due to impaired blood and oxygen supply.In addition to glycemic control,various clinical treatments such as wound dressings,hyperbaric oxygen therapy,and phototherapy have been employed to manage these wounds.Low-level light therapy has emerged as an effective,noninvasive,and painless therapeutic approach for wound management.However,the bulkiness of traditional light sources and the need for frequent clinic visits have limited its widespread adoption.We have developed a wearable,flexible light-emitting bandage with cyanobacterial impregnation(LEB@Cyan).The bioactive bandage is designed to provide sustained oxygen generation and robust photobiomodulation,promoting keratinocyte migration,fibroblast proliferation,and angiogenesis.This addresses the hypoxic conditions and enhances bioenergetic supply to accelerate the healing process of diabetic wounds.In detail,the wound area of diabetic rats treated with LEB@Cyan showed significant reductions of 74.76%and 96.32%compared with that of LEB-treated diabetic rats and untreated diabetic rats,respectively.Such self-oxygenated wearable light-emitting fabric holds great promise for future clinical and commercial applications,potentially revolutionizing the management of chronic diabetic wounds.
文摘Objective: to investigate the effect difference of intermittent and continuous treatment with Yinzhi Huang oral liquid in the treatment of neonatal jaundice (NNJ). Methods: a total of 200 children with NNJ from January 2019 to December 2019 in our hospital were divided into two groups by odd-even method. The levels of bilirubin (BILirubin) before and after treatment, the outcome, the time of disease improvement and the incidence of phototherapy damage events in the two groups were compared. Results: there were significant differences in BILI level, outcome, improvement time and incidence of phototherapy damage events between the intermittent blue light treatment group and the continuous blue light treatment group (P < 0.05). Conclusion: in the aspect of blue light therapy for CHILDREN with NNJ, intermittent intervention can more significantly promote the decline of BILI level, accelerate the regression of yellow skin stain, accelerate the rehabilitation of children, and improve bilirubin level, which is worth promoting.

文摘Light has been clinically utilized as a stimulation in medical treatment,such as Low-level laser therapy and photodynamic thenapy,which has been more and more widely accepted in public.The penetration depth of the treatment light is important for precision treatment and safety control.The isue of light penetration has been highlighted in biomedical optics field for decades.However,quantitative research is sparse and even there are conflicts of view on the capability of near-infrared light penet ration into brain tissue.This study attempts to quantitatively revisit this issue by innovative high-realistic 3D Monte Caro modeling of stimulated light penetnation within high precision Visible Chinese human head.The properties of light,such as its wave-length,ilumination profle and size are concern in this study.We made stra ightforward and quantitative comparisons among the ffects by the light properties(i.e.,wavelengths:660,810 and 980 nn;beam types:Gaussi an and flat beamn;bear dianeters:0,2,4 and 6 cm)which are in the range of light treatment.The findings include about 3%of light dosage within brain tisue;the combination of Gaussian beam and 810nm light make the max imum light penetration(>5cm),which allows light to cross through gray matter into white mater.This study offered us,the first time as we know,quantitative guide for light stimulation parameter optimization in medical treatment.

文摘Near-infrared(NIR)light has been shown to produce a range of physiological effects in hunans,however,there is still no agreement on whether and how a single parameter,like the flicker frequency of NIR light,affects the brain.An 810 nm NIR LED was used as the stimulator.Fifty subjects participated in this experiment.Forty subjects were randomly divided into four groups.Each group underwent a 30-minute NIR LED radiation with four different frequencies(i.e.,0 Hz,5 Hz,10 Hz and 20 Hz,respectively)on the forehead.The remaining 10 subjects formed the control group,in which they underwent a 30-minute rest period without light radiation.EEG signals of all subjects during each test were recorded.Gravity frequency(GF),relative energy change,and sample entropy were analyzed.The experimental groups had larger GF values compared to the control group.Higher stimulation frequency would cause larger growth of GF(F=14.75,P<0.001).The amplitude of alpha waves relative energy increased,while theta waves decreased remarkably in the experimental groups(p<0.02),and the extent of increase/decrease was larger at higher stimulation frequency,compared to that of the control.Sample entropy of electrodes in the frontal areas were much larger than those in other brain areas in the experimental groups(p<0.001).Larger frequency of the NIR LED light would cause more distinct brain activities in the stimulated areas.It indicates that NIR LED light may have a positive effect on modulating brain activity.These results may help improve the design of photobiomodulation treatments in the future.
文摘Advanced biophysical wound healing therapies can apply mechanical, electrical, or light energy to re-stimulate healing processes in chronic wounds. Despite the growing evidence of the clinical efficacy of these therapies, the optimal treatment stimulation parameters remain unknown and there are no standard treatment protocols. We introduce a closed-loop control design as an experimental system to study the dose-response of wound healing therapy treatment within a prescribed multidimensional and multimodal stimulation parameter space. Systems engineering approaches are applied to the control problem for estimation of a transfer function and model equations derived for use in optimal model-based control. The experimental control system design consisted of simultaneous application of biophysical energies inputted into a wound system. A study design set up including the use of negative pressure wound therapy, electrical stimulation therapy, and photobiomodulation device systems was described. Treatment stimulation parameters were selected from experimental ranges used in the scientific literature. Classical control methods and model-based control were suggested for model selection and evaluation and design of the overall control system. An experimental design for multimodal biophysical wound healing therapy control system is introduced to establish the dose-response interactions for development of therapeutic applications and device design.
