At present, the HIV/AIDS testing laboratories includes three classes, the National AIDS Reference Laboratory,HIV/AIDS confirmatory laboratories and HIV/AIDS screening laboratories. All of them are accredited by the health authorities, and each class of laboratories take charge of their function strictly according to the "National Management of HIV/AIDS Detection (2006)". A complete quality assurance and quality control system for HIV/AIDS testing has been developed, which includes technical training, strict laboratory monitoring and approval, examination or proficiency testing on HIV/AIDS detection, and quality evaluation and supervision of HIV/AIDS diagnostic kits. Besides conduct the routine anti-HIV antibody test, more and more laboratories began to conduct other tests, such as CD4+ T lymphocyte cell counting,HIV viral load,HIV DNA PCR,genotyping, drug resistance, and HIV-1 recent infection test. The primary challenges faced by the HIV/AIDS testing laboratory network are in the areas of laboratory management and quality control. For example, the provincial PT program is inefficient, the internal quality control is conducted perfunctorily, personnel training can not met the needs of the workplace, which need to be improved.展开更多

Current laboratory information systems focus on the patient database, tests and results, with multiple modules available, connecting with the various analytical systems or work areas. However laboratory information systems functioned as “islands of information”, because their design was fundamentally inward-looking and disconnected from other healthcare computer applications. Actually, the Electronic Health Register (EHR) is considered by clinicians as a tool with great potential healthcare benefits. The EHR, in the sense of a unique and complete record of a patient’s healthcare and state of health, regardless of the healthcare level used, is a real attempt to eliminate these “islands of information” and need modules to act as “bridges” with the laboratory information systems. This type of module, which in generic terms may be referred to as a laboratory test request module, has become an essential feature of the EHR. These modules need to use a laboratory coding system as a common language for exchanging information, ensuring that tests and results are unequivocally identified. The development of the laboratory test request module requires the commitment of professionals and political authorities, being necessary time for their design and an adequate pilot phase. The laboratory professionals have to assume a leadership role in the whole process of design, development and implementation of these modules, integrating in the equipment of information technologies of healthcare providers. In our manuscript we review the elements that may prove electronic systems for requesting clinical laboratory test into digital clinical records and the key elements to move from theory to practice.展开更多

A variety of clinical and epidemiological investigations are conducted using culture data and the presence of an electronic system such as LIS enhances such investigations and improves the reliability of measures of antimicrobial resistance owing to improved data quality as well as completeness of data gathered as opposed to paper based system. Therefore to improve surveillance of antimicrobial resistance in South Africa, there is a need to reinforce the functionality of the LIS in both public and private microbiology laboratories as this will help to improve internal quality control methodologies.展开更多

文摘Background Small-scale research or individual seed cotton samples from breeders and geneticists are ginned using laboratory saws or roller gins. The objective of this study was to examine the effects of two different laboratory saw ginning(SG)rates and subsequent mechanical blending on fiber high volume instrument(HVI)and advanced fiber information system(AFIS)quality measurement.Seed cottons from diverse upland cotton cultivars, years, and locations were evaluated. Results As the SG rate was increased,HVI results for uniformity index(UI),short fiber index(SFI),and two non-lint parameters(area%,particle count),as well as AFIS results for some properties, were impacted, with significant effects on HVI UI and AFIS short fiber content(SFC).Apparent fiber blending impacts were observed for HVI UI,strength(STR),reflectance(Rd),SFI,and two trashes as well as for all AFIS parameters, with significant effects on HVI Rd and two trashes and all AFIS parameters except the length and maturity measurements.A combination of ginning and blending processes indicated statistically significant interactions for HVI STR,Rd,SFI,and two trashes, and also for AFIS neps,L(w)CV,UQL(w),L(n)CV,SFC(n),L5%(n),immature fiber content(IFC),fineness, total count, dust count, particle count, and visible foreign matter(VFM).Further analysis implied a few impacts of ginning and blending on correlations between HVI and relevant AFIS qualities. Conclusion Despite the observation, the laboratory saw gin and blending methods should continue to be a practical tool to study the differences when all samples are processed identically.

文摘This study aims to understand the effect of injection rate on injection-induced fracture activation in granite. We performed water injection-induced slip tests on samples containing either a smooth or a rough fracture at four different injection rates under undrained conditions and monitored the acoustic emission(AE)signals during the tests.Experimental results reveal that the critical activation fluid pressure is related to the injection rate, pressure diffusion rate, stress state, and fracture roughness. For the smooth fracture,as the injection rate increases, the critical activation fluid pressure increases significantly, while the injection rate has little effect on the critical activation fluid pressure of the rough fracture. The quasi-static slip distance of fractures decreases as the injection rate increases, with rough fractures exhibiting a greater overall slip distance compared to smooth fractures. The number of AE events per unit sliding distance increases with the injection rate, while the global b value decreases. These results indicate that higher injection rates produce more large-magnitude AE events and more severe slip instability and asperity damage. We established a linkage between fluid injection volume, injection rate, and AE events using the seismogenic index(Σ).The smooth fracture exhibits a steadily increasingΣwith the elapse of injection time, and the rate of increase is higher at higher injection rates;while the rough fracture is featured by a fluctuatingΣ,signifying the intermittent occurrence of large-magnitude AE events associated with the damage of larger fracture asperities. Our results highlight the importance of fracture surface heterogeneity on injection-induced fracture activation and slip.
文摘Background:Medical laboratory diagnostic services play a critical role in the diagnosis, treatment, and manage-ment of diseases, forming the cornerstone of effective healthcare systems.Despite the crucial role of laboratory services, the quality and accessibility of medical laboratory services in sub-Saharan Africa(SSA)face signifi-cant challenges.Global health development aid has been pivotal in supporting SSA laboratory services. This study aimed to examine global health development aid initiatives that have successfully enhanced the quality of medical laboratory services and the challenges and barriers to effectively improving medical laboratory services through global health development aid in SSA.Methods:We used a narrative review study design. We searched PubMed,Web of Science and Scopus for articles published in the last 15 years. These three databases are generally considered premier databases for peer-reviewed articles in global health, public health, health systems, and biomedical sciences. The inclusion criteria for this review included research studies, reports, and grey literature.Only articles published in English from 2010 on-ward were considered. The analysis followed a qualitative approach, emphasizing thematic synthesis and critical interpretation. Results:Forty articles were included in this study.Of these,18 were primary research studies,11 were reports,7 were commentaries, and 4 were reviews.Five sub-themes from the successful global health development aid initiative themes were capacity building and training programs, infrastructure development, partnership models, policy advocacy and regulatory support, quality control and standardization of laboratory services. The sub-themes from the challenges and barrier theme were insufficient funding and resource allocation, human resource constraints, inadequate infrastructure and equipment, and political and institutional barriers. This review re-vealed that several factors, including financial sustainability, human resource capacity, institutional support, resilience, and effective monitoring systems, shape the sustainability of improvements in medical laboratory services in SSA.Conclusion:Achieving long-term sustainability requires strategies that ensure financial self-sufficiency,foster a skilled and stable workforce, and integrate laboratory services into national health frameworks.
