BACKGROUND The CAR-OLT score predicts major adverse cardiovascular events 1 year after liver transplant(LT).AIM To test the hypothesis that the CAR-OLT score may help avoid cardiac stress tests in LT candidates.METHOD...BACKGROUND The CAR-OLT score predicts major adverse cardiovascular events 1 year after liver transplant(LT).AIM To test the hypothesis that the CAR-OLT score may help avoid cardiac stress tests in LT candidates.METHODS This retrospective single-center cohort study included all adult patients undergoing elective evaluation for first cadaveric donor orthotopic LT for liver cirrhosis with or without hepatocellular carcinoma at Fondazione Policlinico Universitario Agostino Gemelli Istituto di Ricerca e Cura a Carattere Scientifico in Rome,Italy.Cardiac contraindications for LT listing were defined after a center-specific cardiac workup,which included cardiac stress tests for most patients.The diagnostic accuracy of the CAR-OLT score was evaluated using the area under the receiver operating characteristic(AUROC)method.RESULTS A total of 342 LT candidates were evaluated between 2015 and 2019,with a moderate cardiovascular risk profile(37%diabetes,34%hypertension,22%obesity).Of these,80(23%)candidates underwent coronary angiography.Twenty-one(6%)candidates were given cardiac contraindications to LT listing,48%of which were due to coronary artery disease.The CAR-OLT score predicted cardiac contraindications to LT listing with an AUROC of 0.81.The optimal cut-off for sensitivity was a CAR-OLT score≤23,which showed a 99%negative predictive value for cardiac contraindications to LT listing.A total of 84(25%)LT candidates with a CAR-OLT score≤23 underwent 87 non-invasive cardiac tests and 13 coronary angiographies pre-listing,with estimated costs of approximately 48000€.The estimated savings per patient was€574.70 for the Italian National Health System.CONCLUSION A CAR-OLT score≤23 can identify LT candidates who can be safely listed without the need for cardiac stress tests,providing time and cost savings.These findings require external validation.展开更多
Assessing the threat status of species in response to global change is critical for biodiversity monitoring and conservation efforts.However,current frameworks,even the IUCN Red List,often neglect critical factors suc...Assessing the threat status of species in response to global change is critical for biodiversity monitoring and conservation efforts.However,current frameworks,even the IUCN Red List,often neglect critical factors such as genetic diversity and the impacts of climate and land-use changes,hindering effective conservation planning.To address these limitations,we developed an enhanced extinction risk assessment framework using Diploderma lizards as a model.This framework incorporates long-term field surveys,environmental data,and land-use information to predict distributional changes for 10 recently described Diploderma species on the Qinghai-Xizang Plateau,which hold ecological significance but remain underassessed in conservation assessment.By integrating the distribution data and genetically inferred effective population sizes(Ne),we conducted scenario analyses and used a rank-sum approach to calculate Risk ranking scores(RRS)for each species.This approach revealed significant discrepancies with the IUCN Red List assessments.Notably,D.yangi and D.qilin were identified as facing the highest extinction risk.Furthermore,D.vela,D.batangense,D.flaviceps,D.dymondi,D.yulongense,and D.laeviventre,currently classified as“Least Concern”,were found to warrant reclassification as“Vulnerable”due to considerable threat from projected range contractions.Exploring the relationship between morphology and RRS revealed that traits such as snout-vent length and relative tail length could serve as potential predictors of extinction risk,offering preliminary metrics for assessing species vulnerability when comprehensive data are unavailable.This study enhances the precision of extinction risk assessment frameworks and demonstrates their capacity to refine and update risk assessments,especially for lesser-known taxa.展开更多

Between 2016 and 2024,the Chinese government incorporated several innovative drugs into the National Reimbursement Drug List(NRDL)through price negotiations.These negotiations led to significant price reductions,which...Between 2016 and 2024,the Chinese government incorporated several innovative drugs into the National Reimbursement Drug List(NRDL)through price negotiations.These negotiations led to significant price reductions,which in turn stimulated an increase in sales.This study aimed to assess the impact of this policy on the pricing,utilization,and overall expenditure of targeted lung cancer therapies included in the NRDL.Using an interrupted time series analysis of procurement data from 698 healthcare institutions,the study evaluated both immediate and long-term effects.In terms of immediate effects,price negotiations resulted in a significant decline in the defined daily dose cost(DDDc)for all targeted therapies(P<0.05).Regarding long-term trends,a significant shift was observed only in the pricing trajectory of Gefitinib,Icotinib,and Ensartinib(P<0.05).In terms of immediate effects on drug utilization,all targeted medicines experienced a substantial increase in volume(P<0.05),except for Gefitinib and Icotinib.Over the long term,the usage of all targeted therapies exhibited a significant upward trend(P<0.05).With respect to expenditure,the immediate impact of NRDL inclusion resulted in a significant increase in spending on Afatinib,Crizotinib,Osimertinib,Alectinib,and Ensartinib(P<0.05).Over time,total spending on targeted medicines showed a significant increase(P<0.05),except for Erlotinib.Overall,NRDL price negotiations successfully reduced the economic burden on lung cancer patients,improving both accessibility and affordability of targeted therapies in China.展开更多
Image-based similar trademark retrieval is a time-consuming and labor-intensive task in the trademark examination process.This paper aims to support trademark examiners by training Deep Convolutional Neural Network(DC...Image-based similar trademark retrieval is a time-consuming and labor-intensive task in the trademark examination process.This paper aims to support trademark examiners by training Deep Convolutional Neural Network(DCNN)models for effective Trademark Image Retrieval(TIR).To achieve this goal,we first develop a novel labeling method that automatically generates hundreds of thousands of labeled similar and dissimilar trademark image pairs using accompanying data fields such as citation lists,Vienna classification(VC)codes,and trademark ownership information.This approach eliminates the need for manual labeling and provides a large-scale dataset suitable for training deep learning models.We then train DCNN models based on Siamese and Triplet architectures,evaluating various feature extractors to determine the most effective configuration.Furthermore,we present an Adapted Contrastive Loss Function(ACLF)for the trademark retrieval task,specifically engineered to mitigate the influence of noisy labels found in automatically created datasets.Experimental results indicate that our proposed model(Efficient-Net_v21_Siamese)performs best at both True Negative Rate(TNR)threshold levels,TNR 0.9 and TNR 0.95,with==respective True Positive Rates(TPRs)of 77.7%and 70.8%and accuracies of 83.9%and 80.4%.Additionally,when testing on the public trademark dataset METU_v2,our model achieves a normalized average rank(NAR)of 0.0169,outperforming the current state-of-the-art(SOTA)model.Based on these findings,we estimate that considering only approximately 10%of the returned trademarks would be sufficient,significantly reducing the review time.Therefore,the paper highlights the potential of utilizing national trademark data to enhance the accuracy and efficiency of trademark retrieval systems,ultimately supporting trademark examiners in their evaluation tasks.展开更多

文摘Image-based similar trademark retrieval is a time-consuming and labor-intensive task in the trademark examination process.This paper aims to support trademark examiners by training Deep Convolutional Neural Network(DCNN)models for effective Trademark Image Retrieval(TIR).To achieve this goal,we first develop a novel labeling method that automatically generates hundreds of thousands of labeled similar and dissimilar trademark image pairs using accompanying data fields such as citation lists,Vienna classification(VC)codes,and trademark ownership information.This approach eliminates the need for manual labeling and provides a large-scale dataset suitable for training deep learning models.We then train DCNN models based on Siamese and Triplet architectures,evaluating various feature extractors to determine the most effective configuration.Furthermore,we present an Adapted Contrastive Loss Function(ACLF)for the trademark retrieval task,specifically engineered to mitigate the influence of noisy labels found in automatically created datasets.Experimental results indicate that our proposed model(Efficient-Net_v21_Siamese)performs best at both True Negative Rate(TNR)threshold levels,TNR 0.9 and TNR 0.95,with==respective True Positive Rates(TPRs)of 77.7%and 70.8%and accuracies of 83.9%and 80.4%.Additionally,when testing on the public trademark dataset METU_v2,our model achieves a normalized average rank(NAR)of 0.0169,outperforming the current state-of-the-art(SOTA)model.Based on these findings,we estimate that considering only approximately 10%of the returned trademarks would be sufficient,significantly reducing the review time.Therefore,the paper highlights the potential of utilizing national trademark data to enhance the accuracy and efficiency of trademark retrieval systems,ultimately supporting trademark examiners in their evaluation tasks.

文摘The Reference Listed drug(RLD)plays a critical role in the development and research of generic medicinal products,serving as the comparator product used in the marketing authorization application of new generic medicinal product and re-evaluation of generic medicinal product.In China,RLDs are sourced from various origins,but their quality and market availability can be constrained by multiple factors,including emergent issues such as nitrosamine impurities.Five classes of medicinal products have been reported to be at risk for containing nitrosamine impurities:sartan-based medicines,metformin-containing products,ranitidine medicines,rifampicin medicines,and Champix.This paper explores the control strategies implemented by drug regulatory agencies in the United States and the European Union to manage nitrosamine impurities and assesses their impact on the market availability and quality of RLDs in China.The aim is to offer valuable insights for generic drug manufacturers and regulatory bodies both domestically and internationally.
